Abstract:Simultaneous dual-contrast imaging of iodine and bismuth has shown promise in prior phantom and animal studies utilizing spectral CT. However, it is noted that in previous studies, Pepto-Bismol has frequently been employed as the source of bismuth, exceeding the recommended levels for human subjects. This investigation sought to assess the feasibility of visually differentiating and precisely quantifying low-concentration bismuth using clinical dual-source photon-counting CT (PCCT) in a scenario involving both iodinated and bismuth-based contrast materials. Four bismuth samples (0.6, 1.3, 2.5, and 5.1 mg/mL) were prepared using Pepto-Bismol, alongside three iodine rods (1, 2, and 5 mg/mL), inserted into multi-energy CT phantoms with three different sizes, and scanned on a PCCT system at three tube potentials (120, 140, and Sn140 kV). A generic image-based three-material decomposition method generated iodine and bismuth maps, with mean mass concentrations and noise levels measured. The root-mean-square errors for iodine and bismuth determined the optimal tube potential. The tube potential of 140 kV demonstrated optimal quantification performance when both iodine and bismuth were considered. Distinct differentiation of iodine rods with all three concentrations and bismuth samples with mass concentrations ≥ 1.3 mg/mL was observed across all phantom sizes at the optimal kV setting.

Abstract:Abstract Objective To evaluate the dual-energy (DE) performance and spectral separation with respect to iodine imaging in a photon-counting CT (PCCT) and compare it to dual-source CT (DSCT) DE imaging. Methods A semi-anthropomorphic phantom extendable with fat rings equipped with iodine vials is measured in an experimental PCCT. The system comprises a PC detector with two energy bins (20 keV, T ) and ( T , e U ) with threshold T and tube voltage U . Measurements using the PCCT are performed at all available tube voltages (80 to 140 kV) and threshold settings (50–90 keV). Further measurements are performed using a conventional energy-integrating DSCT. Spectral separation is quantified as the relative contrast media ratio R between the energy bins and low/high images. Image noise and dose-normalized contrast-to-noise ratio (CNRD) are evaluated in resulting iodine images. All results are validated in a post-mortem angiography study. Results R of the PC detector varies between 1.2 and 2.6 and increases with higher thresholds and higher tube voltage. Reference R of the EI DSCT is found as 2.20 on average overall phantoms. Maximum CNRD in iodine images is found for T = 60/65/70/70 keV for 80/100/120/140 kV. The highest CNRD of the PCCT is obtained using 140 kV and is decreasing with decreasing tube voltage. All results could be confirmed in the post-mortem angiography study. Conclusion Intrinsically acquired DE data are able to provide iodine images similar to conventional DSCT. However, PCCT thresholds should be chosen with respect to tube voltage to maximize image quality in retrospectively derived image sets. Key Points • Photon-counting CT allows for the computation of iodine images with similar quality compared to conventional dual-source dual-energy CT. • Thresholds should be chosen as a function of the tube voltage to maximize iodine contrast-to-noise ratio in derived image sets. • Image quality of retrospectively computed image sets can be maximized using optimized threshold settings.

Abstract:Objective Photon counting CT (PCCT) has been a research focus in the last two decades. Recent studies and advancements have demonstrated that systems using semiconductor-based photon counting detectors (PCDs) have the potential to provide better contrast, noise and spatial resolution performance compared to conventional scintillator-based systems. With multi-energy threshold detection, PCD can simultaneously provide the photon energy measurement and enable material decomposition for spectral imaging. In this work, we report a performance evaluation of our first CdZnTe-based prototype full-size photon counting CT system through various phantom imaging studies. Approach This prototype system supports a 500 mm scan field-of-view (FOV) and 10 mm z-coverage at isocenter. Phantom scans were acquired using 120 kVp from 50 to 400 mAs to assess the imaging performance on: CT number accuracy, uniformity, noise, spatial resolution, material differentiation and quantification. Main Results Both qualitative and quantitative evaluations show that PCCT, under the tested conditions, has superior imaging performance with lower noise and improved spatial resolution compared to conventional energy integrating detector (EID)-CT. Using projection domain material decomposition approach with multiple energy bin measurements, PCCT virtual monoenergetic images (VMIs) have lower noise, and good accuracy in quantifying iodine and calcium concentrations. These results lead to increased contrast-to-noise ratio (CNR) for both high and low contrast study objects compared to EID-CT at matched dose and spatial resolution. PCCT can also generate super-high resolution (SHR) images using much smaller detector pixel size than EID-CT and greatly improve image spatial resolution. Significance Improved spatial resolution and quantification accuracy with reduced image noise of the PCCT images can potentially lead to better diagnosis at reduced radiation dose compared to conventional EID-CT. Increased CNR achieved by PCCT suggests potential reduction in iodine contrast media load, resulting in better patient safety and reduced cost.

Abstract:Computed tomography (CT) imaging with high energy resolution detectors shows great promise in material decomposition and multi-contrast imaging. Multi-contrast imaging was studied by imaging a phantom with iodine (I), gadolinium (Gd), and gold (Au) solutions, and mixtures of the three using a cadmium telluride (CdTe) spectrometer with an energy resolution of 1% as well as with a cadmium zinc telluride (CZT) detector with an energy resolution of 13%. The phantom was imaged at 120 kVp and 1.1 mA with 7 mm of aluminum filtration. For the CdTe data collection, the phantom was imaged using a 0.2 mm diameter x-ray beam with 96 ten-second data acquisitions across the phantom at 45 rotation angles. For the CZT detector, we had 720 projections using a cone beam, and the six detector energy thresholds were set to 23, 33, 50, 64, 81, and 120 keV so that three thresholds corresponded to the K-edges of the contrast agents. Contrast agent isolation methods were then examined. K-edge subtraction and novel spectrometric algebraic image reconstruction (SAIR) were used for the CdTe data. K-edge subtraction alone was used for the CZT data. Linearity plots produced similar R 2 values and slopes for all three reconstruction methods. Comparing CdTe methods, SAIR offered less noise than CdTe K-edge subtraction and better geometric accuracy at low contrast concentrations. CdTe contrast agent images of I, Gd, and Au offered less noise and greater contrast than the CZT images, highlighting the benefits of high energy resolution CdTe detectors for possible use in pre-clinical or clinical CT imaging.

Abstract:Spectral CT offers enhanced material discrimination over single-energy systems and enables quantitative estimation of basis material density images. Water/iodine decomposition in contrast-enhanced CT is one of the most widespread applications of this technology in the clinic. However, low concentrations of iodine can be difficult to estimate accurately, limiting potential clinical applications and/or raising injected contrast agent requirements. We seek high-sensitivity spectral CT system designs which minimize noise in water/iodine density estimates. In this work, we present a model-driven framework for spectral CT system design optimization to maximize material separability. We apply this tool to optimize the sensitivity spectra on a spectral CT test bench using a hybrid design which combines source kVp control and k-edge filtration. Following design optimization, we scanned a water/iodine phantom with the hybrid spectral CT system and performed dose-normalized comparisons to two single-technique designs which use only kVp control or only kedge filtration. The material decomposition results show that the hybrid system reduces both standard deviation and crossmaterial noise correlations compared to the designs where the constituent technologies are used individually.

Abstract:Iodine contrast agent is widely used in liver cancer radiotherapy at CT simulation stage to enhance detectability of tumor. However, its application in cone beam CT (CBCT) for image guidance before treatment delivery is still limited because of poor image quality and excessive dose of contrast agent during multiple treatment fractions. We previously developed a multienergy element-resolved (MEER) CBCT framework that included x-ray projection data acquisition on a conventional CBCT platform in a kVp-switching model and a dictionary-based image reconstruction algorithm that simultaneously reconstructed x-ray attenuation images at each kilovoltage peak (kVp), an electron density image, and elemental composition images. In this study, we investigated feasibility using MEER-CBCT for low-concentration iodine contrast agent visualization. We performed simulation and experimental studies using a phantom with inserts containing water and different concentrations of iodine solution and the MEER-CBCT scan with 600 projections in a full gantry rotation, in which the kVp level sequentially changed among 80, 100, and 120 kVps. We included iodine material in the dictionary of the reconstruction algorithm. We analyzed iodine detectability as quantified by contrast-to-noise ratio (CNR) and compared results with those of CBCT images reconstructed by the standard filter back projection (FBP) method with 600 projections. MEER-CBCT achieved similar contrast enhancement as FBP method but significantly higher CNR. At 2.5% iodine solution concentration, FBP method achieved 170 HU enhancement and CNR of 2.0, considered the standard CNR for successful tumor visualization. MEER-CBCT achieved the same CNR but at ~6.3 times lower iodine concentration of 0.4%.

Abstract:Rapid development of the X-ray phonon-counting detection technology brings tremendous research and application opportunities. In addition to improvements in conventional X-ray imaging performance such as radiation dose utilization and beam hardening correction, photon-counting detectors allows significantly more efficient X-ray fluorescence (XRF) and K-edge imaging, and promises a great potential of X-ray functional, cellular and molecular imaging. XRF is the characteristic emission of secondary X-ray photons from a material excited by initial X-rays. The phenomenon is widely used for chemical and elemental analysis. K-edge imaging identifies a material based on its chemically-specific absorption discontinuity over X-ray photon energy. In this paper, we try to combine XRF and K-edge signals from the contrast agents (e.g., iodine, gadolinium, gold nanoparticles) to simultaneously realize XFCT and K-edge CT imaging for superior image performance. As a prerequisite for this dual-modality imaging, the accurate energy calibration of multi-energy-bin photon-counting detectors is critically important. With the measured XRF data of different materials, we characterize the energy response function of a CZT detector for energy calibration and spectrum reconstruction, which can effectively improve the energy resolution and decrease the inconsistence of the photon counting detectors. Then, a simultaneous K-edge and X-ray fluorescence CT imaging (SKYFI) experimental setup is designed which includes a cone-beam X-ray tube, two separate photon counting detector arrays, a pin-hole collimator and a rotation stage. With a phantom containing gold nanoparticles the two types of XFCT and K-edge CT datasets are collected simultaneously. Then, XFCT and K-edge CT images are synergistically reconstructed in a same framework. Simulation results are presented and quantitative analyzed and compared with the separate XFCT and K-edge CT results.

Abstract:Contrast-enhanced computed tomography (CT) is an indispensable technique in modern diagnostic medicine because it not only allows non-destructive, three-dimensional observation of the inside of the body but also provides images that highlight lesions. Recent developments in photon-counting (PC) CT systems have enabled contrast-enhanced CT scanning and quantitative evaluations, such as material discrimination and concentration estimation of contrast agents, using X-ray energy information. We established a PC-CT system that combines fast scintillators and multipixel photon counters (MPPCs) and demonstrated its performance. In this paper, we report the imaging performance of a PC-CT system using a mixture of contrast agents containing gadolinium and iodine. We performed CT imaging of a static phantom containing a mixture of the two contrast agents. The concentration of each contrast agent from the CT images in multiple energy bands were estimated. We succeeded in obtaining the concentration distributions for each contrast agent and verified the possibility of discriminating between two substances with different atomic numbers. In addition, the same procedures and analytical methods used for the static phantom were applied to the in vivo imaging of a mouse, in which gadolinium- and iodine-containing contrast agents were injected at different time intervals. We also succeeded in discriminating between the two contrast agents in vivo, making it possible to provide new imaging diagnostics such as the determination of necrotic tissues and normal blood vessels. Furthermore, we succeeded in creating three-dimensional concentration maps of gadolinium and iodine in a mouse, demonstrating the great potential of MPPC-based PC-CT for future clinical applications.

Abstract:Compared to traditional multi-scan single-energy CT (SECT), one potential advantage of single-scan multi-energy CT (MECT) proposed for simultaneous imaging of multiple contrast agents is the radiation dose reduction. This phantom study aims to rigorously evaluate whether the radiation dose can truly be reduced in a single-scan MECT protocol (MECT_1s) in biphasic liver imaging with iodine and gadolinium, and small bowel imaging with iodine and bismuth, compared to traditional two-scan SECT protocols (SECT_2s). For MECT_1s, mixed iodine/gadolinium samples were prepared corresponding to late arterial/portal-venous phase for biphasic liver imaging. Mixed iodine/bismuth samples were prepared representing the arterial/enteric enhancement for small bowel imaging. For SECT_2s, separate contrast samples were prepared to mimic separate scans in arterial/venous phase and arterial/enteric enhancement. Samples were placed in a 35-cm wide water phantom and scanned by a research whole-body photon-counting-detector-CT (PCD-CT) system ("chess" mode). MECT images were acquired with optimized kV/threshold settings for each imaging task, and SECT images were acquired at 120 kV. Total CTDIvol was matched for the two protocols. Image-based three-material decomposition was employed in MECT_1s to determine the basis material concentration values, which were converted to CT numbers at 120 kV (i.e., virtual SECT images) to compare with the SECT images directly acquired with SECT_2s. The noise difference between the SECT and the virtual SECT images was compared to evaluate the dose efficiency of MECT_1s. Compared to SECT_2s, MECT_1s was not dose efficient for both imaging tasks. The amount of noise increase is highly task dependent, with noise increased by 203%/278% and 110%/82% in virtual SECT images for iodine/gadolinium and iodine/bismuth quantifications, respectively, corresponding to dose increase by 819%/1328% and 340%/230% in MECT_1s to achieve the same image noise level. MECT with the current PCD-CT technique requires higher radiation dose than SECT to achieve the same image quality.

Abstract:Purpose A dual‐source CT system can be operated in a high‐pitch helical mode to provide a temporal resolution of 66 ms, which reduces motion artifacts in CT pulmonary angiography (CTPA). It can also be operated in a multi‐energy (ME) mode to provide iodine maps, beneficial in the evaluation of pulmonary embolism (PE). No energy‐integrating detector (EID) CT can perform simultaneous ME and high‐pitch acquisition. This phantom study aimed to evaluate the ability of a photon‐counting‐detector (PCD) CT to perform simultaneous high‐pitch and ME imaging for CTPA. Methods A motion phantom was used to mimic the respiratory motion. Two tubes filled with iodine with intravascular thrombus mimicked by injecting glue within the tubes were placed along with 5, 10, and 15 mg/mL iodine samples, on a motion phantom at 20 and 30 revolutions per minute. Separate high‐pitch and ME EID‐CT scans and a single high‐pitch ME PCD scan were acquired and virtual monoenergetic images and iodine maps reconstructed. Percent thrombus occlusion was measured and compared between static and moving images. Results When there was motion, EID‐CT ME suffered from significant motion artifacts. The measured iodine concentrations with PCD‐CT in high‐pitch ME were more stable when there was a motion, with a lower standard deviation than EID‐CT in ME mode. The estimated percent thrombus occlusion dropped significantly with applied motion on EID‐CT, while PCD‐CT high‐pitch ME mode showed good agreement between measurements on static or moving images. Conclusion PCD‐CT with combined ME and high‐pitch mode facilitates simultaneous accurate iodine quantification and assessment of intravascular occlusion.

Abstract:Objective: Computed tomography (CT) has advanced since its inception, with breakthroughs such as dual-energy CT (DECT), which extracts additional information by acquiring two sets of data at different energies. As high-flux photon-counting detectors (PCDs) become available, PCD-CT is also becoming a reality. PCD-CT can acquire multi-energy data sets in a single scan by spectrally binning the incident x-ray beam. With this, K-edge imaging becomes possible, allowing high atomic number (high-Z) contrast materials to be distinguished and quantified. In this study, we demonstrated that DECT methods can be converted to PCD-CT systems by extending the method of Bourque et al (2014). We optimized the energy bins of the PCD for this purpose and expanded the capabilities by employing K-edge subtraction imaging to separate a high-atomic number contrast material. Approach: The method decomposes materials into their effective atomic number (Z eff ) and electron density relative to water (ρ e ). The model was calibrated and evaluated using tissue-equivalent materials from the RMI Gammex electron density phantom with known ρ e values and elemental compositions. Theoretical Z eff values were found for the appropriate energy ranges using the elemental composition of the materials. Z eff varied slightly with energy but was considered a systematic error. An ex-vivo bovine tissue sample was decomposed to evaluate the model further and was injected with gold chloride to demonstrate the separation of a K-edge contrast agent. Main results: The mean root mean squared percent errors on the extracted Z eff and ρ e for PCD-CT were 0.76% and 0.72%, respectively and 1.77% and 1.98% for DECT. The tissue types in the ex-vivo bovine tissue sample were also correctly identified after decomposition. Additionally, gold chloride was separated from the ex-vivo tissue sample with K-edge imaging. Significance: PCD-CT offers the ability to employ DECT material decomposition methods, along with providing additional capabilities such as K-edge imaging.

Abstract:Background: The first commercially available photon-counting-detector CT (PCD-CT) has been introduced for clinical use. However, its spectral performance on single- and dual-contrast imaging tasks has not been comprehensively assessed. Purpose: To evaluate the spectral imaging performance of a clinical PCD-CT system for single-contrast material [iodine (I) or gadolinium (Gd)] and dual-contrast materials (I and Gd) in comparison with a dual-source dual-energy CT (DS-DECT). Methods: Iodine (5, 10, and 15 mg/mL) and gadolinium (3.3, 6.6, and 9.9 mg/mL) samples, and their mixtures (I/Gd: 5/3.3 and 10/6.6 mg/mL) were prepared and placed in two torso-shaped water phantoms (lateral dimensions: 30 and 40 cm). These phantoms were scanned on a PCD-CT (NAEOTOM Alpha, Siemens) at 90, 120, and 140 kV. The same phantoms were scanned on a DS-DECT (SOMATOM Force, Siemens) with 70/Sn150, 80/Sn150, 90/Sn150, and 100/Sn150 kV. The radiation dose levels were matched [volume CT dose index (CTDIvol): 10 mGy for the 30 cm phantom and 20 mGy for the 40 cm phantom] across all tube voltage settings and between scanners. Two-material decomposition (I/water or Gd/water) was performed on iodine or gadolinium samples, and three-material decomposition (I/Gd/water) on both individual samples and mixtures. On each decomposed image, mean mass concentration (± standard deviation) was measured in circular region-of-interests placed on the contrast samples. Root-mean-square-error (RMSE) values of iodine and gadolinium concentrations were reported based on the measurements across all contrast samples and repeated on 10 consecutive slices. Results: For all material decomposition tasks on the DS-DECT, the kV pairs with greater spectral separation (70/Sn150 kV and 80/Sn150 kV) yielded lower RMSE values than other DS-DECT and PCD-CT alternatives. Specifically, for the optimal 70/Sn150 kV, RMSE values were 1.2 ± 0.1 mg/mL (I) for I/water material decomposition, 1.0 ± 0.1 mg/mL (Gd) for Gd/water material decomposition, and 4.5 ± 0.2 mg/mL (I) and 3.7 ± 0.2 mg/mL (Gd), respectively, for I/Gd/water material decomposition. On the PCD-CT, the optimal tube voltages were 120 or 140 kV for I/water decomposition with RMSE values of 2.0 ± 0.1 mg/mL (I). For Gd/water decomposition on PCD-CT, the optimal tube voltage was 140 kV with gadolinium RMSE values of 1.5 ± 0.1 mg/mL (Gd), with the 90 kV setting on PCD-CT generating higher RMSE values for gadolinium concentration compared to all DS-DECT and PCD-CT alternatives. For three material decomposition, both imaging modalities demonstrated substantially higher RMSE values for iodine and gadolinium, with 90 kV being the optimal tube potential for Gd/I quantitation on PCD-CT [5.4 ± 0.3 mg/mL (I) and 3.9 ± 0.2 mg/mL (Gd)], and DS-DECT at 100/Sn150 kV having larger RMSE values for both materials compared to the alternatives for either modality. Conclusion: Optimal tube voltage for material decomposition on the clinical PCD-CT is task-dependent but inferior to DS-DECT using 70/Sn150 kV or 80/Sn150 kV in two-material decomposition for single-contrast imaging (iodine/water or gadolinium/water). Three material decomposition (iodine/gadolinium/water) in dual-contrast imaging yields substantially higher RMSE for both imaging platforms.
Combining Spectral CT Acquisition Methods for High-Sensitivity Material Decomposition

Abstract:Quantitative estimation of contrast agent concentration is made possible by spectral CT and material decomposition. There are several approaches to modulate the sensitivity of the imaging system to obtain the different spectral channels required for decomposition. Spectral CT technologies that enable this varied sensitivity include source kV-switching, dual-layer detectors, and source-side filtering (e.g., tiled spatial-spectral filters). In this work, we use an advanced physical model to simulate these three spectral CT strategies as well as hybrid acquisitions using all combinations of two or three strategies. We apply model-based material decomposition to a water-iodine phantom with iodine concentrations from 0.1 to 5.0 mg/mL. We present bias-noise plots for the different combinations of spectral techniques and show that combined approaches permit diversity in spectral sensitivity and improve low concentration imaging performance relative to the those strategies applied individually. Better ability to estimate low concentrations of contrast agent has the potential to reduce risks associated with contrast administration (by lowering dosage) or to extend imaging applications into targets with much lower uptake.

The potential of photon-counting CT for quantitative contrast-enhanced imaging in radiotherapy

Abstract:The aim of this study is to use a simulation environment to evaluate the potential of using photon-counting CT (PCCT) against dual-energy CT (DECT) in the context of quantitative contrast-enhanced CT for radiotherapy. An adaptation of Bayesian eigentissue decomposition by Lalonde (2017 Med. Phys. 44 5293-302) that incorporates the estimation of contrast agent fractions and virtual non-contrast (VNC) parameters is proposed, and its performance is validated against conventional maximum likelihood material decomposition methods for single and multiple contrast agents. PCCT and DECT are compared using two simulation frameworks : one including ideal CT numbers with image-based Gaussian noise and another defined as a virtual patient with projection-based Poisson noise and beam hardening artifacts, with both scenarios considering spectral distortion for PCCT. The modalities are compared for their accuracy in estimating four key physical parameters : 1) the contrast agent fraction, as well as VNC parameters relevant to radiotherapy such as the 2) electron density, 3) proton stopping power and 4) photon linear attenuation coefficient. Considering both simulation frameworks, a reduction of root mean square errors with PCCT is noted for all physical parameters evaluated, with the exception of the error on the contrast agent fraction being about constant through modalities in the virtual patient. Notably, root mean square errors on VNC electron density and stopping power are respectively reduced from 2.0 to 1.4% and 2.7 to 1.4% when going from DECT to PCCT with 4 energy bins. The increase in accuracy is comparable to the differences between contrast-enhanced and non-contrast DECT. This study suggests that in a realistic simulation environment, the overall accuracy of radiotherapy-related parameters can be increased when using PCCT with 4 energy bins instead of DECT. This confirms the potential of PCCT to provide robust and quantitative tissue parameters for contrast-enhanced CT required in radiotherapy applications.

Abstract:Multi-energy CT imaging of large patients with conventional dual-energy (DE)-CT using an energy-integrating-detector (EID) is challenging due to photon starvation-induced image artifacts, especially in lower tube potential (80–100 kV) images. Here, we performed phantom experiments to investigate the performance of DECT for morbidly obese patients, using an iodine and water material decomposition task as an example, on an emulated dual-source (DS)-photon-counting-detector (PCD)-CT, and compared its performance with a clinical DS-EID-CT. An abdominal CT phantom with iodine inserts of different concentrations was wrapped with tissue-equivalent gel layers to emulate a large patient (50 cm lateral size). The phantom was scanned on a research whole-body single-source (SS)-PCD-CT (140 kV tube potential), a DS-PCD-CT (100/Sn140 kV; Sn140 indicates 140 kV with Sn filter), and a clinical DS-EID-CT (100/Sn140 kV) with the same radiation dose. Phantom scans were repeated five times on each system. The DS-PCD-CT acquisition was emulated by scanning twice on the SS-PCD-CT using different tube potentials. The multi-energy CT images acquired on each system were then reconstructed, and iodine- and water-specific images were generated using material decomposition. The root-mean-square-error (RMSE) between true and measured iodine concentrations were calculated for each system and compared. The images acquired on the DS-EID-CT showed severe artifacts, including ringing, reduced uniformity, and photon starvation artifacts, especially for low-energy images. These were largely reduced in DS-PCD-CT images. The CT number difference that was measured using regions-of-interest across field-of-view were reduced from 20.3 ± 0.9 (DS-EID-CT) to 2.5 ± 0.4 HU on DS-PCD-CT, showing improved image uniformity using DS-PCD-CT. Iodine RMSE was reduced from 3.42 ± 0.03 mg ml−1 (SS-PCD-CT) and 2.90 ± 0.03 mg ml−1 (DS-EID-CT) to 2.39 ± 0.05 mg ml−1 using DS-PCD-CT. DS-PCD-CT out-performed a clinical DS-EID-CT for iodine and water-based material decomposition on phantom emulating obese patients by reducing image artifacts and improving iodine quantification (RMSE reduced by 20%). With DS-PCD-CT, multi-energy CT can be performed on large patients that cannot be accommodated with current DECT.

Abstract:The image quality entitlement is evaluated for multi-energy bin photon counting (PC) spectral CT relative to that of energy integration an dual kVp (dkVp) imaging. Physics simulations of X-ray projection channel data and CT images are used to map contrast-to-noise metrics for simple numerical phantoms objects with soft tissue, calcium and iodine materials. The benefits are quantified under ideal detector conditions. Spectral optimization yields on the order of 2X benefit for iodine visualization measured by CNR<<^>>2/dose in two different imaging modes: optimal energy weighting, and optimal mono-energy imaging. In another case studied, strict dose equivalence is maintained by use of a composite spectrum for PC simulation that combines simultaneously the two kVp excitations used sequentially for dkVp. In this case, mono-energetic imaging of iodine contrast agent is shown to achieve 40% higher dose efficiency for photon counting compared to dual kVp although non-ideal characteristics of the photon counting response can eliminate much of this benefit.
Supply chain modifications to improve additive manufacturing cost-benefit balance

Abstract:Additive manufacturing (AM) offers unique production characteristics which among those, toollessness and production of complex geometries are potentially significant to operations efficiency. Previous research has illustrated the potential sufficiency of this technology to affect the supply chains‟ arrangements and enabling decentralized production configurations. While, one of the important advantages of AM enabled distributed production is the increased flexibility, which is a necessity in today‟s competitive and ever changing global supply chains, number of obstacles have kept this method from wide implementation. In this paper, we study the possible supply chain modifications to decrease the cost of an AM-enabled decentralized production system. In other words, we perform a cost-benefit analysis on various AM supply chain strategies in a spare parts context to realize the independent operational factors affecting the implementation cost of additive manufacturing. Moreover, we analyze the ways to adapt the supply chain management to enable full potential of AM considering the present technology.

Abstract:Spectral CT systems with photon counting detectors have more advantages compared to conventional CT systems. However, clinical applications have been hampered for a long time due to the high demands of clinical systems and limitations of spectroscopic x-ray detectors. Photon counting detector technology has gained considerable improvements in the past decade, and spectral CT has become a hot topic. Several experimental spectral CT systems are under investigation. The purpose of this work was to perform the first direct, side-by-side comparison of existing spectral CT technology with a mature clinical CT system based on a conventional energy integrating detector. We have built an experimental spectral CT system whose main parameters are similar to the parameters of a clinical CT system. The system uses a spectroscopic cadmium zinc telluride (CZT) detector. The detector includes two rows of CZT pixels with 256 pixels in each row. The pixel size is 1 × 1 mm(2), and the maximum count rate is 2 Mcounts/pixel/s. The spectral CT system has a magnification factor of 1.62 and the source to detector and source to image distances of 85 and 53 cm, respectively. The above parameters are similar to those of the clinical CT system, Siemens Sensation 16, used for comparison. The two systems were compared by imaging spatial resolution and contrast resolution phantoms made from acrylic cylinders with 14 cm diameters. The resolution phantom included Al wires with 0.3, 0.6, and 1 mm diameters, and 0.25 g cc(-1) CaCO(3) contrast. The contrast phantom included contrast elements with 1.7, 5, and 15 mg cc(-1) iodine, and 1.1, 3.3, and 10 mg cc(-1) gadolinium. The phantoms were imaged with the two systems using 120 kVp tube voltage and 470 mR total skin exposure. The spectral CT showed CT numbers, image noise, and spatial and contrast resolutions to be similar within 10% compared to the Siemens 16 system, and provided an average of 10% higher CNR. However, the spectral CT system had a major advantage in that the iodine, gadolinium, and CaCO(3) contrasts were decomposed by dual-energy and K-edge subtraction methods using energy selective CT data acquired in a single CT scan and fixed tube voltage. It is concluded that photon counting spectral CT technology is close to feasibility for routine clinical applications. Furthermore, it is ready for some clinical applications such as dedicated breast CT which has relatively lower demands on photon counting detectors.
