Abstract:Introduction: Similar to the US, Japan has seen a decline in sulfonylureas (SUs) use in the elderly patients (pts) with diabetes, but the detailed usage status of SU is unknown. Methods: To investigate real-world SU prescribing patterns, we conducted a retrospective cross-sectional survey using pharmacy data in Japan, targeting pts prescribed SUs, aged 20-100 years. From November 2022 to October 2023, we extracted data for 91,230 pts from the electronic prescription system Musubi (KAKEHASHI Inc., adopted in over 7,000 pharmacies, accounting for 10% market share, as of 2023 in Japan). The most commonly prescribed SUs were glimepiride (73,061 pts, 80.1%), gliclazide (14,841 pts, 16.3%), and glibenclamide (3,327 pts, 3.6%). Our analysis focused on adherence to Japanese guidelines in prescribing these SUs in elderly pts (adhering to limits of 1 mg/day for glimepiride and 40 mg/day for gliclazide, and avoiding glibenclamide). Results: Of 91,230 pts showed 55,947 were male (61.3%) and a mean age of 68.8 years; 66,319 (72.7%) were aged ≥65 years. Elderly pts on average used 2.7 antidiabetic medications concurrently. The most common additional medications with SUs were DPP-4 inhibitor (49,963 pts, 75.3%), biguanide (34,266 pts, 61.2%), and SGLT-2 inhibitor (25,878 pts, 46.3%). Insulin was used concomitantly in 3,682 elderly pts (6.6%). Among elderly pts, 25.0% and 7.8% exceeded the recommended dosages for glimepiride (>1 mg/day) and gliclazide (>40 mg/day), respectively. Glibenclamide was prescribed to 2,578 elderly pts (77.5%). However, elderly pts used fewer diabetes medications on average: 3.3 for those <65 years, 3.0 for 65-<75 years, and 2.7 for ≥75 years. Conclusion: We identified high-dose prescription of glimepiride and gliclazide, and glibenclamide use in elderly pts, indicating potential non-adherence to guidelines. Clinical features and sulfonylurea usage among outpatients with diabetes aged ≥90 years in an urban diabetes clinic in Tokyo

Abstract:Aims/introduction: Aging of society is accelerating in many countries. The purpose of this study was to describe the clinical features and sulfonylurea usage among diabetes outpatients aged ≥90 years (nonagenarians). Materials and methods: This study was a retrospective observational study. The study population consisted of 69 nonagenarian diabetes outpatients and 857 diabetes outpatients aged <90 years. Patients were classified into four groups: group 1, <65 years; group 2, 65-74 years; group 3, 75-89 years; and group 4, ≥90 years. The presence of hypoglycemic episodes was defined as having self-reported symptoms, or self-monitored or clinically measured blood glucose level <70 mg/dL. Results: The median glycated hemoglobin (HbA1c) in group 1 and group 4 was 7.0% and 7.2%, respectively (P = 0.506). The proportion of sulfonylurea treatment in group 4 was 45.5%, which is significantly higher compared with the other three groups (20.0-27.8%, P < 0.001). In group 4, there was no difference between patients with or without sulfonylurea in age, sex, body mass index, HbA1c and number of antihyperglycemic agents. Five out of 25 nonagenarian sulfonylurea-treated patients had hypoglycemic episodes within the last 2 years, their HbA1c were all 7.0 ≤ HbA1c < 8.0, and sulfonylurea or insulin was tapered in all cases after confirming hypoglycemia. Tapering dosage was attempted in all 25 sulfonylurea-treated nonagenarian patients, but 15 needed to continue sulfonylurea for glycemic control, and 10 continued sulfonylurea with unknown reasons from their medical records. Conclusions: Although tapering the dosage of sulfonylurea was attempted in nonagenarian patients, sulfonylurea was widely continued for glycemic control. Reverse clinical inertia may exist in some sulfonylurea-treated nonagenarian patients.

Abstract:<span class="a-plus-plus abstract-section id-a-sec1"><h3 class="a-plus-plus">Aim</h3><p class="a-plus-plus">We aimed to investigate the certainty of using sulfonylureas in Japanese patients with type 2 diabetes mellitus (T2DM) by analyzing data from the 2018 Nationwide Survey on Actual Intervention for T2DM by Japanese Practitioners (NSAID Study).</p></span><span class="a-plus-plus abstract-section id-a-sec2"><h3 class="a-plus-plus">Methods</h3><p class="a-plus-plus">Of the 6525 and 1545 participants in the NSAID Study under the care of general practitioners (GP) and diabetes specialists (SP), respectively, we included 5423 (83.1%) and 1058 (68.5%) patients who were treated with only oral antidiabetic drugs (OADs) by GPs and SPs, respectively, in the analysis.</p></span><span class="a-plus-plus abstract-section id-a-sec3"><h3 class="a-plus-plus">Results</h3><p class="a-plus-plus">Among the seven OAD classes in monotherapy, sulfonylureas were the third and fifth most prescribed OADs by GPs (7.1%) and SPs (6.4%), respectively. Sulfonylurea usage increased with combination therapy. Glimepiride was the most commonly prescribed sulfonylurea. Patients who used sulfonylureas had higher hemoglobin A1c (HbA1c) levels and lower body mass indices (BMIs) than patients who did not use sulfonylureas.</p></span><span class="a-plus-plus abstract-section id-a-sec4"><h3 class="a-plus-plus">Conclusion</h3><p class="a-plus-plus">The results of this study clearly demonstrated that, among the OADs, sulfonylureas were not frequently used in monotherapy, although the opportunity of using sulfonylurea increased with the number of OADs prescribed in combination therapies by both GPs and SPs in Japan. Moreover, low-dose glimepiride was the most-prescribed sulfonylurea for Japanese T2DM patients, especially for those who were lean and had higher HbA1c levels.</p></span>
Long-Term Patterns of Antidiabetic Medication Use in Patients with Type 2 Diabetes.

Abstract:BACKGROUND This study aimed to investigate the patterns of use of antidiabetic medication among patients with newly diagnosed type 2 diabetes mellitus (T2DM), focusing on the comparison in glycemic control between sulfonylureas and metformin. MATERIAL AND METHODS Data from patients newly diagnosed and treated for T2DM between 2011 and 2014, who were ³18 years of age were obtained from the Yinzhou Regional Health Care Database, and patterns of medication and glycemic control were analyzed. The Poisson probability distribution was used to determine the rate ratio (incidence density ratio) of uncontrolled hyperglycemia between sulfonylureas and metformin. Cox regression analysis was used to determine the association between initial treatment with sulfonylureas and metformin and the requirement for additional medications. RESULTS Of the 4,017 patients included in the study, 33.58% began treatment with sulfonylureas and 20.41% began treatment with metformin, and during follow-up, 21.13% and 22.68%, respectively were treated with a second drug. After adjustment for body mass index (BMI) and fasting blood glucose (FBG), the rate ratio of uncontrolled blood glucose for sulfonylurea monotherapy compared with metformin monotherapy was 1.30 (95% CI, 1.17-1.45). Patients who began treatment with sulfonylureas were 18% less likely to progress to dual medication compared with metformin (HR=0.82; 95% CI, 0.68-0.99), CONCLUSIONS Sulfonylurea monotherapy was the most common initial treatment for patients with newly diagnosed T2DM and was associated with an increased risk of uncontrolled hyperglycemia, but patients were less likely to receive additional drugs when compared with patients initially treated with metformin monotherapy.
Dose-response relationship between sulfonylureas and major adverse cardiovascular events in elderly patients with type 2 diabetes

Abstract:Purpose: The objective of this study was to determine if there is a dose-response relationship between sulfonylureas and major adverse cardiovascular events (MACE). Methods: We conducted a retrospective cohort study among elderly patients with no history of acute coronary syndrome or stroke who initiated gliclazide or glyburide therapy between 1998 and 2010. Gliclazide and glyburide users were evaluated separately, and a high-dimensional propensity score (HDPS) was used to match patients initiating therapy with a low or high dose. A time-dependent variable was used to further characterize exposure, which can change during follow-up. Cox proportional hazard regression models were used to compare the risk of MACE between low (reference) and high doses. Results: We identified 14,213 new users of gliclazide or glyburide (mean age, 74.7 (standard deviation 6.4) years; males, 52.8%; and mean follow-up duration, 2.7 (standard deviation 2.9) years). Among gliclazide users, there was a higher risk of MACE with high compared with low dose (crude rates: 32.8 and 28.2 per 1000 person-years, respectively), but this did not reach statistical significance (HDPS-matched hazard ratio) 1.15; 95% confidence interval (0.96-1.38). For glyburide users, however, MACE occurred more frequently in the high compared with low dose (crude rates: 38.9 and 31.5 per 1000 person-years, respectively; HDPS-matched hazard ratio 1.24; 95% confidence interval 1.02-1.50). Conclusions: Among new users of sulfonylureas, there appears to be a dose-response relationship between glyburide and MACE, but not for gliclazide. Copyright © 2016 John Wiley & Sons, Ltd.

Abstract:Abstract Although revision total knee arthroplasty (TKA) procedures are successful in relieving pain and restoring function in failed knees, long-term results are inferior to primary procedures. Mobile bearing (MB) revision knees can potentially improve functional performance. Clinical results of 44 MB rotating platform (RP) revision TKAs demonstrated mean knee injury and osteoarthritis outcome score (KOOS) activity of daily living score of 77 2 years postoperatively. Clinical results were put into context through wear testing which demonstrated improved wear performance compared with fixed bearing (FB) revision knees. The RP construct is a good choice for revision TKA because it permits the surgeon to align the tray for fixation, not compromise rotation of the tibial insert, and reduce transmission of shear stresses to the bone, cement and implant interface, which is a known cause of failure.

Abstract:Previous studies have hypothesised that mitochondrial DNA (mtDNA) polymorphisms may influence aerobic performance. The matrilineal inheritance and accumulation of polymorphisms in mtDNA means that mtDNA haplogroups, characterised by key polymorphisms, are often represented at different frequencies in different populations. The present study aimed to compare the mtDNA haplogroup distribution of elite Ethiopian athletes relative to the general Ethiopian population. The haplogroup distribution of 76 endurance athletes (E), members of the Ethiopian national athletics team, was compared to 108 members of the general Ethiopian population (C). DNA was extracted from buccal swabs and haplogroups assigned by sequencing part of the hypervariable sequence (HVS-I), followed by analysis of key coding-region polymorphisms. A high proportion of African 'L' haplogroups was found in athletes and controls (C=53%; E=55%). Haplogroup distribution of endurance runners did not differ from that of C (P=0.63). Elite Ethiopian athletes are not a mitochondrially distinct group relative to the Ethiopian population. It appears that environment and, perhaps, polymorphisms in the nuclear genome are more important determinants of Ethiopian running success than mtDNA polymorphisms.
Efficacy and safety of oral semaglutide in older patients with type 2 diabetes: a retrospective observational study (the OTARU-SEMA study)

Abstract:Background: Oral semaglutide in older subjects with type 2 diabetes was as effective as in younger subjects, according to phase 3 clinical trials. However, its efficacy can be limited in very aged population, due to the presence of impaired cognitive function and the complex instructions for its use. Here, we investigated its efficacy and safety by further age bracket in older subjects in real-world. Methods: We retrospectively studied subjects > 65 years of age with type 2 diabetes who started oral semaglutide treatment. The primary outcome was the change in glycated hemoglobin (HbA1c) over 6 months. Adverse events and cognitive function were evaluated using the Common Terminology Criteria for Adverse Events (CTCAE) and the Hasegawa Dementia Rating Scale-revised (HDS-R). The achievement rate of glycemic targets was evaluated based on the age, health status of subjects and their use of anti-diabetic agents which can cause hypoglycemia, with additional analysis between two subgroups; early (65-74) versus late (≥ 75) older. Furthermore, we evaluated the relationships between their improvements in HbA1c and the baseline characteristics of the subjects, including their cognitive function and insulin secretory capacity. Results: We studied the efficacy of the drug in 24 subjects. Their HbA1c and body weight significantly decreased (- 13.1 ± 7.5 mmol/mol and - 3.0 ± 2.4 kg, respectively; P < 0.01). Although cognitive function was lower in the late older group (r = -0.57, P < 0.01), changes in HbA1c showed no difference between the two subgroups (P = 0.66) and it correlated with the insulin secretory capacity rather than cognitive function (r = -0.49, P < 0.05). Glycemic targets were more likely to be achieved (P < 0.01), but HbA1c excessively decreased in late older subjects who were also using insulin or an insulin secretagogue. The frequency of adverse events was similar to that in the clinical trial, whereas discontinuation of medication were more frequent among the late older subjects (Early; n = 2, Late; n = 4). Conclusions: Oral semaglutide improves the glycemic control of older subjects, but it might be a risk for potential hypoglycemia and discontinuation because of adverse events in subjects of ≥ 75 years. Attention should be paid to insulin secretory capacity and concomitant medications rather than concern about adherence.

Abstract:A BSTRACT Background and Objective: The treatment of elderly people who have developed Type II diabetes is a major challenge. Therefore, this study was conducted to ascertain the drug prescription pattern and the adverse effects of drugs in a tertiary care hospital. Methodology: This was a cross-sectional, observational study. This study was carried out on 164 elderly patients with diabetes mellitus attending inpatient and outpatient of the institute. This study assesses the prescribing pattern of antidiabetic drugs and adverse drugs reaction to the drugs administered. Naranjo-Algorithm Probability of American depositary receipt (ADR) scale and WHO prescribing indicators were used for assessment. This study also identifies the drug utilization in diabetic patients associated with other co-morbidities. Results: Mean age of the patient was 72.5 in both inpatient and outpatient and male:female ratio among inpatient and outpatient 1.5:1 and 1:1.3, respectively. Metformin (biguanide) was the most commonly prescribed antidiabetic drug and was followed by glimepiride. Insulin was used in 65% of inpatient. Potential drug-drug interactions were noted in 71% of inpatient and 50.8% of outpatient. The incidence of ADR observed in this study was3.6%. Conclusion: Metformin and glimepiride were the most common drugs used in both inpatient and outpatient groups. Human regular insulin analog was commonly used in both patients group. The poor reporting in pharmacovigilance although mandated by the Indian Council of Medical Research is falling short; in preempting ADR. Effective pharmacovigilance will improve the quality of life in geriatric patients and would help the treating physicians to steer towards safe and effective healthcare.
Benefits and Risks With Glyburide and Glipizide in Elderly NIDDM Patients

Abstract:Objective –To compare the efficacy, benefits, and risks of glyburide and glipizide in elderly patients with non-insulin-dependent diabetes mellitus (NIDDM). Research Design and Methods –Twenty-one elderly outpatients (mean age 70 yr) were treated for 8 wk, after being dose-titrated to achieve a fasting plasma glucose (FPG) concentration of < 7.8 mM with glyburide or glipizide in a randomized crossover trial. FPG and postprandial plasma glucose, serum C-peptide, and HbA1c levels were measured. In 13 patients, self-monitoring of blood glucose (SMBG) with a memory meter was performed seven times per week. Results –Glipizide (11.9 mg) and glyburide (8.4 mg) produced similar fasting and postprandial plasma glucose and HbA1c concentrations. No significant differences in basal or stimulated C-peptide levels were detected. Despite a few patient reports of hypoglycemia, a high incidence of SMBG readings < 4.5 mM was attributed to the use of both drugs. Conclusions –Both treatments proved effective for glycemic control; however, both second-generation sulfonylureas are associated with a significant risk of hypoglycemia in elderly NIDDM patients. The proper use of sulfonylureas in this population should include close surveillance of ambulatory glucose monitoring and intensive and repeated patient education about the risks of hypoglycemia.

Evaluation of glucose-lowering medications in older people: a comprehensive systematic review and network meta-analysis of randomized controlled trials

Abstract:Background: Type 2 diabetes mellitus (T2DM) is increasingly being diagnosed in older adults. Our objective is to assess the advantages and potential drawbacks of different glucose-lowering medications in this specific population. Methods: A network meta-analysis was conducted to identify randomized controlled trials that examined patient-centered outcomes in adults aged ≥65 years with T2DM. We searched PubMed, Cochrane CENTRAL, and Embase up to September 23, 2023. Quality of eligible studies were assessed using the Cochrane RoB 2.0 tool. Results: A total of 22 trials that involved 41 654 participants were included, incorporating sodium-glucose cotransporter-2 (SGLT2) inhibitors, glucagon-like peptide-1 receptor agonists (GLP-1RAs), dipeptidyl peptidase-4 (DPP-4) inhibitors, metformin, sulfonylureas (SU) and acarbose. Our findings reveal that GLP-1RAs reduce the risk of major adverse cardiovascular events (risk ratio [RR], 0.83; 95% confidence interval [CI], 0.71 to 0.97) and body weight (mean difference [MD], -3.87 kg; 95% CI, -5.54 to -2.21). SGLT2 inhibitors prevent hospitalization for heart failure (RR, 0.66; 95% CI, 0.57 to 0.77), renal composite outcome (RR, 0.69; 95% CI, 0.53 to 0.89), and reduce body weights (MD, -1.85 kg; 95% CI, -2.42 to -1.27). SU treatment increases the risk of any hypoglycaemia (RR, 4.19; 95% CI, 3.52 to 4.99) and severe hypoglycaemia (RR, 7.06; 95% CI, 3.03 to 16.43). GLP-1RAs, SGLT2 inhibitors, metformin, SU and DPP-4 inhibitors are effective in reducing glycaemic parameters. Notably, the number of treatments needed decreases in most cases as age increases. Conclusions: Novel glucose-lowering medications with benefits that outweigh risks should be prioritized for older patients with diabetes.
Long-term safety and efficacy of SGLT2 inhibitor use in older east Asians with type 2 diabetes

Abstract:Sodium-glucose cotransporter 2 inhibitors (SGLT2is) have been shown in cardiovascular outcome trials to reduce the risk of heart failure and major adverse cardiovascular as well as renal events in individuals with type 2 diabetes. Moreover, clinical evidence indicates that SGLT2i use reduces heart failure and chronic kidney disease (CKD) in east Asian patients with type 2 diabetes. Thus, SGLT2is might seem to be the preferred treatment for older patients with type 2 diabetes even in the presence of multiple comorbidities. However, older patients with type 2 diabetes may well have impaired physiological function, making the risk of certain adverse events higher than that in the general population. While a randomized clinical trial has been conducted to evaluate changes in skeletal muscle mass and function as well as those in cognitive function with SGLT2i use in older Japanese individuals with type 2 diabetes who are otherwise healthy, the safety of SGLT2is remains to be established among older individuals with type 2 diabetes also having impaired activity of daily living and/or cognitive impairment. Even so, international and domestic consensus reports recommend SGLT2is for patients with type 2 diabetes and heart failure, CKD, and/or cardiovascular diseases, and SGLT2is are being widely prescribed by general practitioners to older individuals with type 2 diabetes with little regard to the patient's comorbidities. We maintain that SGLT2i use in older patients with type 2 diabetes should be prescribed cautiously in consideration of the pathophysiology of the disease and the presence of complications and comorbidities as well as the individual's lifestyle.

Abstract:Renal oncocytomas are classified as benign renal neoplasms in the 2004 World Health Organization classification of renal tumours .1 Cases of metastatic tumours have been documented,2–4 but subsequent reports have identified subtypes of renal tumours having similar histological morphology but having malignant potential. These tumours have been classified as either eosinophilic variants of renal cell carcinoma (RCC) or chromophobe carcinomas. Differentiating these tumours from oncocytomas has proved to be difficult and relies on a combination of histological morphology, immunohistochemistry and electron microscopy. There is still a presumption that if metastasis occurs then the tumour should not be classified as a renal oncocytoma and should be called an eosinophilic RCC. We present a patient who presented with liver metastasis 9 years after the removal of a renal oncocytoma. A woman in her 70s presented with left-sided abdominal pain. She was noted to have a mobile mass in the left upper quadrant. Baseline blood examinations were normal and she had no other significant medical history. An abdominal ultrasound was suggestive of a left RCC. A CT scan showed a 12 cm renal mass extending just across the midline, arising from the upper anterior part of the left kidney (fig 1). The left renal vein, liver and lymph nodes appeared unaffected. The conclusion was that this was most probably an RCC, but the pattern of enhancement was suggestive of a renal oncocytoma. Treatment patterns, persistence and adherence rates in patients with type 2 diabetes mellitus in Japan: a claims-based cohort study

Abstract:Objective To determine real-world trends in antidiabetic drug use, and persistence and adherence, in Japanese patients with type 2 diabetes mellitus (T2DM). Design Retrospective evaluation of administrative claims data (2011–2015) using the Japan Medical Data Center (JMDC) and Medical Data Vision (MDV) databases. Setting Analysis of two administrative claims databases for Japanese patients with T2DM. Participants Adults (aged ≥18 years) with an International Classification of Diseases, 10th Revision code of T2DM and at least one antidiabetic drug prescription. Main outcome measures Treatment patterns in untreated (UT) or previously treated (PT) patients receiving antidiabetic therapy; persistence with treatment at 12 months; adherence to treatment at 12 months. Results 40 908 and 90 421 patients were included from the JMDC and MDV databases, respectively. The most frequently prescribed therapy at the index (first prescription) date was dipeptidyl peptidase-4 inhibitor (DPP-4i) in UT patients (JMDC: 44.0%, MDV: 54.8%) and combination therapy in PT patients (74.6%, 81.1%). Most common combinations were DPP-4i plus: biguanide (BG; 11.4%, 10.9%), sulfonylurea (SU; 8.4%, 11.0%) or BG+SU (7.8%, 9.1%). In UT or PT patients from either database whose index prescription was for any antidiabetic drug class(es) other than DPP-4i, the most frequent add-on or switch was to DPP-4i. 12-month persistence with index monotherapy was highest with DPP-4i and BG. Adherence was high (≥80%) for all monotherapy schedules, except insulin and glucagon-like peptide-1 agonist, and for the five most frequent two-drug and three-drug combinations. Persistence was greater in elderly UT patients and in those receiving ≤5 medications, but comparatively worse in UT patients with ≥3 index antidiabetic drug classes. Conclusions The findings indicate that DPP-4i is the most commonly used antidiabetic drug class in Japanese patients with T2DM, and persistence and adherence to this antidiabetic drug class are high.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Aims/Introduction</h3><p>Type 2 diabetes mellitus (T2DM) is caused by a relative imbalance between insulin secretion and sensitivity related to the body mass index (BMI). Seven categories of oral antidiabetic drugs (OADs) are available in Japan. It is important to assess the OAD utilization patterns based on patients' BMI levels.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Materials and methods</h3><p>OAD prescribing patterns from 2002 to 2019 were analyzed using the data collected in the computerized diabetes care (CoDiC) database provided by the Japan Diabetes Clinical Data Management Study Group (JDDM); OAD utilization patterns in 25,751 OAD-treated T2DM patients registered in 2019 were analyzed after classifying them into 5 categories of BMI.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Comparing OAD usage between 2002 and 2019, sulfonylureas (SUs) decreased from 44.5% to 23.2%, and biguanides (BGs) increased from 19.3% to 50.3%. Dipeptidyl peptidase-4 inhibitors (DPP4is) increased to 56.9% in 2019. Sodium-glucose co-transporter-2 inhibitors (SGLT2is) increased to 23.6% in 2019. About 90% of type 2 diabetes mellitus patients had BMI <30 kg/m<sup>2</sup>. DPP4is were the most used OADs in 2019. When BMI exceeded 30 kg/m<sup>2</sup>, use of BGs and SGLT2is increased, and use of SUs and DPP4is decreased. Although DPP4is were the most used OADs in patients with BMI <30 kg/m<sup>2</sup>, they were the third most prescribed OADs in patients with BMI >35 kg/m<sup>2</sup> after BGs and SGLT2s. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>DPP4i usage was as high as that of BG in the analysis of Japanese T2DM patients with relatively low BMI. This was considered to be a treatment option appropriate for the pathophysiology in Japanese patients.</p></section>

Biphasic interaction of gamma-globulin with copper cations.

Abstract:Interactions of human serum gamma-globulin with copper cations in solution were studied by differential ultraviolet spectrophotometry. Copper in supraphysiological concentrations increases optical density of protein solution, reflecting the effect of gamma-globulin saturation with metal. In physiological and lower concentrations of copper cations we observed hypochromia in the protein absorption spectrum. Conformational changes in g-globulin molecule during interactions with copper by the surface and intramolecular binding sites and possible role of bivalent metal cations in the maintenance of certain conformations of immunoactive serum proteins are discussed.

Abstract:Purpose The risk of heart failure associated with sulphonylureas is unclear. We evaluated the association between sulphonylureas and hospitalization of heart failure (HHF) in patients with type 2 diabetes mellitus (T2DM) in China. Methods A retrospective cohort study was implemented using the Yinzhou Regional Health Care Database (YRHCD). We identified 15 752 adult patients with T2DM who were newly exposed to sulphonylurea monotherapy (N = 12 487) or acarbose monotherapy (N = 3265) from January 2010 to September 2016. Cox proportional hazards models weighted by inverse probability of treatment weights were used to compare the risk of HHF between initiators of sulphonylurea and acarbose. Results During a median follow-up of 0.55 (0.49, 1.11) and 0.49 (0.35, 0.70) years for sulphonylureas and acarbose initiators separately, 320 patients developed HHF, with 279 events in sulphonylureas group, and 41 events in acarbose group. The incidence rates of HHF among sulphonylureas initiators and acarbose initiators were 22.2 (95% CI 19.6-24.9) and 18.3 (95% CI 13.2-24.9) per 1000 person-years, respectively. The adjusted hazard ratio (aHR) of HHF for sulphonylureas vs acarbose was 1.61 (95% CI 1.14-2.27). When stratified by history of heart failure, aHR was 1.55 (95% CI 0.79-3.06) in patients with a history of heart failure, and 1.64 (95% CI 1.10-2.45) in patients with no history of heart failure. Conclusions Our study suggested that use of sulphonylureas monotherapy compared with acarbose monotherapy for initial treatment of T2DM for approximately 0.5 years are significantly associated with a higher risk of HHF.

Abstract:Recombinant human erythropoietin (rHuEPO) has proved to be a useful drug that can aid in protecting patients from possible hazards of homologous blood. Use of rHuEPO for patients scheduled for coronary artery bypass graft (CABG) surgery can enhance the benefit of a preoperative autologous blood donation program while simultaneously minimizing its risks. The main objective are to decrease homologous transfusion requirements by increasing the amount of collected autologous blood and to maximize safety by ameliorating a pre- or postoperative anemia. The goal of this review is to assess the possible benefits of perioperative use of rHuEPO during CABG surgery. Furthermore, we evaluate its ability to augment the quality of collected blood and to lower the risk of autologous blood donation. Side effects, dosages, manner of administration, concomitant iron therapy and suggestions for special patient groups will be discussed.

Abstract:A propensity-score matched cohort study was conducted in two US commercial insurance databases, including 365,816 adults ≥60 years with type 2 diabetes who initiated dipeptidyl peptidase-4 inhibitors (DPP4i) or sulfonylureas. We assessed the risk of a composite endpoint of cardiovascular events, fractures, and severe hypoglycemia comparing DPP4i and sulfonylureas in persons with mild (CFI <0.15), moderate (0.15–0.24) and severe frailty (≥0.25). Compared with sulfonylureas, DPP4i were associated with fewer outcomes: the hazard ratios (HRs) (95% confidence interval) for mild, moderate and severe frailty were 0.62 (0.54–0.70), 0.67 (0.57–0.78) and 0.63 (0.55–0.73), respectively. With increasing levels of frailty, DPP4i were associated with decreasing cardiovascular protection (HRs for mild, moderate, and severe frailty: 0.77, 0.86, 0.99), and with a non-significant reduction in fractures (HRs: 1.17, 0.84, 0.80). Severe hypoglycemia was consistently lower with DPP4i across frailty levels (HRs: 0.39, 0.52, 0.46). This information is useful for diabetes management in older adults.
