Abstract:Introduction T2* weighted gradient echo MRI has been increasingly recognized as a sensitive tool in detecting intracerebral hemorrhage. However, its blooming artifacts is highly dependent on imaging parameters including TE, field strength and voxel size, making it difficult to reliably estimate the hematoma volume, a key predictor of morbidity and mortality of hemorrhage. Recently, a novel quantitative susceptibility mapping (QSM) technology has been developed for processing gradient echo MRI data to map tissue susceptibility property without blooming artifacts and dependence on imaging parameters. Hypothesis We assessed the hypothesis that hematoma volume measurement on QSM is independent of imaging parameters, eliminating its TE dependence on gradient echo MRI. Method A retrospective image analysis of MRI was approved by our IRB with HIPPA compliance. We randomly selected 16 patients who underwent intracerebral hemorrhage MRI including a 3D multiecho T2*w sequence: 8-11 echoes with first echo TE/ echo spacing/ TR= 5/5/50 msec. Postprocessed images of gradient echo MRI included susceptibility weighted imaging (SWI), R2* (quantitative 1/T2* mapping), and QSM at various TEs. Hematoma volumes were measured from all these images. Results Linear regression of hematoma volume vs TE over all subjects showed substantial slopes for gradient echo magnitude (0.45±0.31 L/s), SWI (0.52±0.46) and R2* (0.39±0.30) but nearly zero slope for QSM (0.01±0.05). At TE=20 msec, hematoma volume on QSM was 0.80x that on gradient echo magnitude image (R2=0.99), and hematoma volume on CT is also 0.8x that on gradient echo magnitude image according to literature (Stroke 2008;39:2017-2020). Conclusion In conclusion, quantitative susceptibility mapping can provide reliable measurement of hematoma volume, independent echo time and similar to CT.

Abstract:Background: Magnetic resonance (MR) quantitative T-1 rho imaging has been increasingly used to detect the early stages of osteoarthritis. The small volume and curved surface of articular cartilage necessitate imaging with high in-plane resolution and thin slices for accurate T-1 rho measurement. Compared with 2D T-1 rho mapping, 3D T-1 rho mapping is free from artifacts caused by slice cross-talk and has a thinner slice thickness and full volume coverage. However, this technique needs to acquire multiple T-1 rho-weighted images with different spin-lock times, which results in a very long scan duration. It is highly expected that the scan time can be reduced in 3D T-1 rho mapping without compromising the T-1 rho quantification accuracy and precision. Methods: To accelerate the acquisition of 3D T-1 rho mapping without compromising the T-1 rho quantification accuracy and precision, a signal-compensated robust tensor principal component analysis method was proposed in this paper. The 3D T-1 rho-weighted images compensated at different spin-lock times were decomposed as a low-rank high-order tensor plus a sparse component. Poisson-disk random undersampling patterns were applied to k-space data in the phase-and partition-encoding directions in both retrospective and prospective experiments. Five volunteers were involved in this study. The fully sampled k-space data acquired from 3 volunteers were retrospectively undersampled at R=5.2, 7.7, and 9.7, respectively. Reference values were obtained from the fully sampled data. Prospectively undersampled data for R=5 and R=7 were acquired from 2 volunteers. Bland-Altman analyses were used to assess the agreement between the accelerated and reference T-1 rho measurements. The reconstruction performance was evaluated using the normalized root mean square error and the median of the normalized absolute deviation (MNAD) of the reconstructed T-1 rho-weighted images and the corresponding T-1 rho maps. Results: T1 rho parameter maps were successfully estimated from T-1 rho-weighted images reconstructed using the proposed method for all accelerations. The accelerated T-1 rho measurements and reference values were in good agreement for R=5.2 (T-1 rho: 40.4 +/- 1.4 ms), R=7.7 (T-1 rho: 40.4 +/- 2.1 ms), and R=9.7 (T-1 rho: 40.9 +/- 2.2 ms) in the Bland-Altman analyses. The T-1 rho parameter maps reconstructed from the prospectively undersampled data also showed promising image quality using the proposed method. Conclusions: The proposed method achieves the 3D T-1 rho mapping of in vivo knee cartilage in eight minutes using a signal-compensated robust tensor principal component analysis method in image reconstruction.
Cerebral Microbleeds: Burden Assessment by Using Quantitative Susceptibility Mapping

Abstract:Purpose: To assess quantitative susceptibility mapping (QSM) for reducing the inconsistency of standard magnetic resonance (MR) imaging sequences in measurements of cerebral microbleed burden.Materials and Methods: This retrospective study was HIPAA compliant and institutional review board approved. Ten patients (5.6%) were selected from among 178 consecutive patients suspected of having experienced a stroke who were imaged with a multiecho gradient-echo sequence at 3.0 T and who had cerebral microbleeds on T2*-weighted images. QSM was performed for various ranges of echo time by using both the magnitude and phase components in the morphology-enabled dipole inversion method. Cerebral microbleed size was measured by two neuroradiologists on QSM images, T2*-weighted images, susceptibility-weighted (SW) images, and R2* maps calculated by using different echo times. The sum of susceptibility over a region containing a cerebral microbleed was also estimated on QSM images as its total susceptibility. Measurement differences were assessed by using the Student t test and the F test; P < .05 was considered to indicate a statistically significant difference.Results: When echo time was increased from approximately 20 to 40 msec, the measured cerebral microbleed volume increased by mean factors of 1.49 +/- 0.86 (standard deviation), 1.64 +/- 0.84, 2.30 +/- 1.20, and 2.30 +/- 1.19 for QSM, R2*, T2*-weighted, and SW images, respectively (P < .01). However, the measured total susceptibility with QSM did not show significant change over echo time (P = .31), and the variation was significantly smaller than any of the volume increases (P < .01 for each).Conclusion: The total susceptibility of a cerebral microbleed measured by using QSM is a physical property that is independent of echo time. Abstract:Background It is of great clinical importance to assess the microstructure of the articular cartilage and cortical bone of the human knee joint. While quantitative susceptibility mapping (QSM) is a promising tool for investigating the knee joint, however, previous QSM studies using conventional gradient recalled echo sequences or ultrashort echo time (UTE) sequences only focused on mapping the magnetic susceptibility of the articular cartilage or cortical bone, respectively. Simultaneously mapping the underlying susceptibilities of the articular cartilage and cortical bone of human in vivo has not been explored and reported. Method Three-dimensional multi-echo radial UTE sequences with the shortest TE of 0.07 msec and computed tomography (CT) were performed on the bilateral knee joints of five healthy volunteers for this prospective study. UTE-QSM was reconstructed from the local field map after water-fat separation and background field removal. Spearman’s correlation analysis was used to explore the relationship between the magnetic susceptibility and CT values in 158 representative regions of interest of cortical bone. Result The susceptibility properties of the articular cartilage and cortical bone were successfully quantified by UTE-QSM. The laminar structure of articular cartilage was characterized by the difference of susceptibility value in each layer. Susceptibility was mostly diamagnetic in cortical bone. A significant negative correlation (r=−0.43, p<0.001) between the susceptibility value and CT value in cortical bone was observed. Conclusion UTE-QSM enables simultaneous susceptibility mapping of the articular cartilage and cortical bone of human in vivo . Good association between susceptibility and CT values in cortical bone suggests the potential of UTE-QSM for bone mapping for further clinical application.

Abstract:BackgroundQuantitative susceptibility mapping (QSM) has been used to study the magnetic susceptibility properties of collagen fibers in articular cartilage; however, it is unclear whether QSM is sensitive to changes due to degradation caused by long‐distance running. It is clinically important to understand the link between long‐distance running and microstructural changes in knee cartilage.PurposeTo investigate the ability of QSM to assess microstructural changes within cartilage after repetitive loading.Study TypeProspective.PopulationThirteen recreational, male long‐distance runners.Field Strength/SequenceThree‐dimensional gradient recalled echo acquired at 3 T.AssessmentMagnetic resonance imaging (MRI) and 3D kinematics (translations and rotations during treadmill walking and running) of the knee joint were collected before and after marathon running. The compartments for analysis included the patella, trochlea, and subregions of femoral and tibial cartilage. Changes in regional susceptibility and cartilage thickness were calculated after marathon running. A susceptibility profile was obtained by fitting susceptibility as a function of the normalized depth of cartilage from the superficial to deep layers.Statistical TestsPaired t‐test or Wilcoxon signed‐rank test, 95% confidence interval (CI) of the depth‐wise susceptibility profile, Pearson correlation or Spearman correlation.ResultsThere was a statistically significant increase in susceptibility value in the weight‐bearing region of central medial femoral cartilage (cMF‐c) after marathon running (pre‐marathon: −0.0219 ± 0.0151 ppm, post‐marathon: −0.0070 ± 0.0213 ppm, P < 0.05), while the cartilage thickness did not show significant changes in any regions (P‐value range: 0.068–0.963). Significant susceptibility elevations occurred in the middle and deep layers of cMF‐c (95% CIs did not overlap). A trend toward a positive correlation was found between the changes in susceptibility value in cMF‐c and proximal–distal translation of the knee joint during walking (r = 0.55, P = 0.101) and running (r = 0.57, P = 0.089).Data ConclusionLocalized magnetic susceptibility alterations were observed within knee cartilage in the weight‐bearing area after repetitive loading without any morphologic changes.Level of Evidence2Technical EfficacyStage 2

Abstract:Background: Automatic segmentation of knee cartilage and quantification of cartilage parameters are crucial for the early detection and treatment of knee osteoarthritis (OA). The aim of this study was to develop an automatic cartilage segmentation method for three-dimensional water-selective (3D_WATS) cartilage magnetic resonance imaging (MRI) and conduct cartilage morphometry and magnetic susceptibility measurements such as cartilage thickness, volume, and susceptibility values for knee OA assessment. Methods: Sixty-five consecutively sampled subjects, who had undergone health checks at our hospital, were enrolled in this cross-sectional study and were divided into three groups: 20 normal, 20 mild OA, 25 severe OA. Sagittal 3D_WATS sequence was used to image cartilage at 3T. The raw magnitude images were used for cartilage segmentation and the phase images were used for quantitative susceptibility mapping (QSM)based assessment. Manual cartilage segmentation was performed by two experienced radiologists, and the automatic segmentation model was constructed using nnU-Net. Quantitative cartilage parameters were extracted from the magnitude and phase images based on the cartilage segmentation. Pearson correlation coefficient and intra-class correlation coefficient (ICC) were then used to assess the consistency of obtained cartilage parameters between automatic and manual segmentation. Cartilage thickness, volume, and susceptibility values among different groups were compared using one-way analysis of variance (ANOVA). Support vector machine (SVM) was used to further verify the classification validity of automatically extracted cartilage parameters. Results: The constructed cartilage segmentation model based on nnU-Net achieved an average Dice score of 0.93. The consistency of cartilage thickness, volume, and susceptibility values calculated using automatic and manual segmentations ranged from 0.98 to 0.99 (95% CI: 0.89-1.00) for the Pearson correlation coefficient, and from 0.91-0.99 (95% CI: 0.86-0.99) for ICC, respectively. Significant differences were found in OA patients; including decreases in cartilage thickness, volume, and mean susceptibility values (P<0.05), and increases in standard deviation ( SD) of susceptibility values (P< 0.01). Moreover, the automatically extracted cartilage parameters can achieve an AUC value of 0.94 (95% CI: 0.89-0.96) for OA classification using the SVM classifier. Conclusions: The 3D_WATS cartilage MR imaging allows simultaneously automated assessment of cartilage morphometry and magnetic susceptibility for evaluating the severity of OA using the proposed cartilage segmentation method.
Repeatability of Quantitative Knee Cartilage T1, T2, and T1ρ Mapping With 3D-MRI Fingerprinting

Abstract:Background: Three-dimensional MR fingerprinting (3D-MRF) techniques have been recently described for simultaneous multiparametric mapping of knee cartilage. However, investigation of repeatability remains limited. Purpose: To assess the intra-day and inter-day repeatabilities of knee cartilage T1, T2, and T1ρ maps using a 3D-MRF sequence for simultaneous measurement. Study type: Prospective. Subjects: Fourteen healthy subjects (35.4 ± 9.3 years, eight males), scanned on Day 1 and Day 7. Field strength/sequence: 3 T/3D-MRF, T1, T2, and T1ρ maps. Assessment: The acquisition of 3D-MRF cartilage (simultaneous acquisition of T1, T2, and T1ρ maps) were acquired using a dictionary pattern-matching approach. Conventional cartilage T1, T2, and T1ρ maps were acquired using variable flip angles and a modified 3D-Turbo-Flash sequence with different echo and spin-lock times, respectively, and were fitted using mono-exponential models. Each sequence was acquired on Day 1 and Day 7 with two scans on each day. Statistical tests: The mean and SD for cartilage T1, T2, and T1ρ were calculated in five manually segmented regions of interest (ROIs), including lateral femur, lateral tibia, medial femur, medial tibia, and patella cartilages. Intra-subject and inter-subject repeatabilities were assessed using coefficient of variation (CV) and intra-class correlation coefficient (ICC), respectively, on the same day and among different days. Regression and Bland-Altman analysis were performed to compare maps between the conventional and 3D-MRF sequences. Results: The CV in all ROIs was lower than 7.4%, 8.4%, and 7.5% and the ICC was higher than 0.56, 0.51, and 0.52 for cartilage T1, T2, and T1ρ, respectively. The MRF results had a good agreement with the conventional methods with a linear regression slope >0.61 and R2 > 0.59. Conclusion: The 3D-MRF sequence had high intra-subject and inter-subject repeatabilities for simultaneously measuring knee cartilage T1, T2, and T1ρ with good agreement with conventional sequences. Abstract:Background Cartilage T2 can detect joints at risk of developing osteoarthritis. The quantitative double‐echo steady state (qDESS) sequence is attractive for knee cartilage T2 mapping because of its acquisition time of under 5 minutes. Understanding the reproducibility errors associated with qDESS T2 is essential to profiling the technical performance of this biomarker. Purpose To examine the combined acquisition and segmentation reproducibility of knee cartilage qDESS T2 using two different regional analysis schemes: 1) manual segmentation of subregions loaded during common activities and 2) automatic subregional segmentation. Study Type Prospective. Subjects 11 uninjured participants (age: 28 ± 3 years; 8 (73%) female). Field Strength/Sequence 3‐T, qDESS. Assessment Test–retest T2 maps were acquired twice on the same day and with a 1‐week interval between scans. For each acquisition, average cartilage T2 was calculated in four manually segmented regions encompassing tibiofemoral contact areas during common activities and 12 automatically segmented regions from the deep‐learning open‐source framework for musculoskeletal MRI analysis (DOSMA) encompassing medial and lateral anterior, central, and posterior tibiofemoral regions. Test–retest T2 values from matching regions were used to evaluate reproducibility. Statistical Tests Coefficients of variation (%CV), root‐mean‐square‐average‐CV (%RMSA‐CV), and intraclass correlation coefficients (ICCs) assessed test–retest T2 reproducibility. The median of test–retest standard deviations was used for T2 precision. Bland–Altman (BA) analyses examined test–retest biases. The smallest detectable difference (SDD) was defined as the BA limit of agreement of largest magnitude. Significance was accepted for P 0.75 were observed in the majority of regions but were more variable in interday tibial comparisons. Test–retest T2 precision was 5% (3.8 msec) could represent changes to cartilage composition. Level of Evidence 2. Technical Efficacy Stage 2.

A Systematic Post-Processing Approach for Quantitative $T_{1ρ}$ Imaging of Knee Articular Cartilage

Abstract:Objective: To establish an automated pipeline for post-processing of quantitative spin-lattice relaxation time constant in the rotating frame ($T_{1\rho}$) imaging of knee articular cartilage. Design: The proposed post-processing pipeline commences with an image standardisation procedure, followed by deep learning-based segmentation to generate cartilage masks. The articular cartilage is then automatically parcellated into 20 subregions, where $T_{1\rho}$ quantification is performed. The proposed pipeline was retrospectively validated on a dataset comprising knee $T_{1\rho}$ images of 10 healthy volunteers and 30 patients with knee osteoarthritis. Three experiments were conducted, namely an assessment of segmentation model performance (using Dice similarity coefficients, DSCs); an evaluation of the impact of standardisation; and a test of $T_{1\rho}$ quantification accuracy (using paired t-tests; root-mean-square deviations, RMSDs; and coefficients of variance of RMSDs, $CV_{RMSD}$). Statistical significance was set as p<0.05. Results: There was a substantial agreement between the subregional $T_{1\rho}$ quantification from the model-predicted masks and those from the manual segmentation labels. In patients, 17 of 20 subregions, and in healthy volunteers, 18 out of 20 subregions, demonstrated no significant difference between predicted and reference $T_{1\rho}$ quantifications. Average RMSDs were 0.79 ms for patients and 0.56 ms for healthy volunteers, while average $CV_{RMSD}$ were 1.97% and 1.38% for patients and healthy volunteers. Bland-Altman plots showed negligible bias across all subregions for patients and healthy volunteers. Conclusion: The proposed pipeline can perform automatic and reliable post-processing of quantitative $T_{1\rho}$ images of knee articular cartilage.

Abstract:Low‐field scanners are cheaper alternatives that can improve accessibility to MRI while bringing challenges such as lower signal‐to‐noise ratio efficiency (SNRe). Using MRI fingerprinting (MRF), phantom and volunteers were scanned in a 0.55T scanner to assess the SNRe and accuracy. Measurements showed excellent repeatability and correlation with reference measurements. Low‐field strength scanners present an opportunity for more inclusive imaging exams and bring several challenges including lower signal‐to‐noise ratio (SNR) and longer scan times. Magnetic resonance fingerprinting (MRF) is a rapid quantitative multiparametric method that can enable multiple quantitative maps simultaneously. To demonstrate the feasibility of an MRF sequence for knee cartilage evaluation in a 0.55T system we performed repeatability and accuracy experiments with agar‐gel phantoms. Additionally, five healthy volunteers (age 32 ± 4 years old, 2 females) were scanned at 3T and 0.55T. The MRI acquisition protocols include a stack‐of‐stars T1ρ‐enabled MRF sequence, a VIBE sequence with variable flip angles (VFA) for T1 mapping, and fat‐suppressed turbo flash (TFL) sequences for T2 and T1ρ mappings. Double‐Echo steady‐state (DESS) sequence was also used for cartilage segmentation. Acquisitions were performed at two different field strengths, 0.55T and 3T, with the same sequences but protocols were slightly different to accommodate differences in signal‐to‐noise ratio and relaxation times. Cartilage segmentation was done using five compartments. T1, T2, and T1ρ values were measured in the knee cartilage using both MRF and conventional relaxometry sequences. The MRF sequence demonstrated excellent repeatability in a test–retest experiment with model agar‐gel phantoms, as demonstrated with correlation and Bland–Altman plots. Underestimation of T1 values was observed on both field strengths, with the average global difference between reference values and the MRF being 151 ms at 0.55T and 337 ms at 3T. At 0.55T, MRF measurements presented significant biases but strong correlations with the reference measurements. Although a larger error was present in T1 measurements, MRF measurements trended similarly to the conventional measurements for human subjects and model agar‐gel phantoms.

Abstract:OBJECTIVES: Quantitative MRI techniques such as T2 and T1$\rho$ mapping are beneficial in evaluating cartilage and meniscus. We aimed to evaluate the MIXTURE (Multi-Interleaved X-prepared Turbo-Spin Echo with IntUitive RElaxometry) sequences that provide morphologic images with clinical turbo spin-echo (TSE) contrasts and additional parameter maps versus reference TSE sequences in an in-situ model of human cartilage defects. MATERIALS AND METHODS: Prospectively, standardized cartilage defects of 8mm, 5mm, and 3mm diameter were created in the lateral femora of 10 human cadaveric knee specimens (81$\pm$10 years, nine male/one female). Using a clinical 3T MRI scanner and knee coil, MIXTURE sequences combining (i) proton-density weighted fat-saturated (PD-w FS) images and T2 maps and (ii) T1-weighted images and T1$\rho$ maps were acquired before and after defect creation, alongside the corresponding 2D TSE and 3D TSE reference sequences. Defect delineability, bone texture, and cartilage relaxation times were quantified. Inter-sequence comparisons were made using appropriate parametric and non-parametric tests. RESULTS: Overall, defect delineability and texture features were not significantly different between the MIXTURE and reference sequences. After defect creation, relaxation times increased significantly in the central femur (for T2) and all regions combined (for T1$\rho$). CONCLUSION: MIXTURE sequences permit time-efficient simultaneous morphologic and quantitative joint assessment based on clinical image contrasts. While providing T2 or T1$\rho$ maps in clinically feasible scan time, morphologic image features, i.e., cartilage defect delineability and bone texture, were comparable between MIXTURE and corresponding reference sequences.

Abstract:ObjectivesTo evaluate articular cartilage degeneration using quantitative three-dimensional ultrashort-echo-time cones adiabatic-T1ρ (3D UTE-Cones-AdiabT1ρ) imaging.MethodsSixty-six human subjects were recruited for this study. Kellgren-Lawrence (KL) grade and Whole-Organ Magnetic-Resonance-Imaging Score (WORMS) were evaluated by two musculoskeletal radiologists. The human subjects were categorized into three groups, namely normal controls (KL0), doubtful-minimal osteoarthritis (OA) (KL1–2), and moderate–severe OA (KL3–4). WORMS were regrouped to encompass the extent of lesions and the depth of lesions. The UTE-Cones-AdiabT1ρ values were obtained using 3D UTE-Cones data acquisitions preceded by seven paired adiabatic full passage pulses that corresponded to seven spin-locking times (TSLs) of 0, 12, 24, 36, 48, 72, and 96 ms. The performance of the UTE-Cones-AdiabT1ρ technique in evaluating the degeneration of knee cartilage was assessed via the ANOVA comparisons with subregional analysis and Spearman’s correlation coefficient as well as the receiver-operating-characteristic (ROC) curve.ResultsUTE-Cones-AdiabT1ρ showed significant positive correlations with KL grade (r = 0.15, p < 0.05) and WORMS (r = 0.57, p < 0.05). Higher UTE-Cones-AdiabT1ρ values were observed in both larger and deeper lesions in the cartilage. The differences in UTE-Cones-AdiabT1ρ values among different extent and depth groups of cartilage lesions were all statistically significant (p < 0.05). Subregional analyses showed that the correlations between UTE-Cones-AdiabT1ρ and WORMS varied with the location of cartilage. The AUC value of UTE-Cones-AdiabT1ρ for mild cartilage degeneration (WORMS=1) was 0.8. The diagnostic threshold value of UTE-Cones-AdiabT1ρ for mild cartilage degeneration was 39.4 ms with 80.8% sensitivity.ConclusionsThe 3D UTE-Cones-AdiabT1ρ sequence can be useful in quantitative evaluation of articular cartilage degeneration.Key Points• The 3D UTE-Cones-AdiabT1ρsequence can distinguish mild cartilage degeneration from normal cartilage with a diagnostic threshold value of 39.4 ms for mild cartilage degeneration with 80.8% sensitivity.• Higher UTE-Cones-AdiabT1ρvalues were observed in both larger and deeper lesions in the articular cartilage.• UTE-Cones-AdiabT1ρis a promising biomarker for quantitative evaluation of early cartilage degeneration.

Abstract:Conventional magnetic resonance imaging (MRI) is not capable of detecting signal from the deep cartilage due to its short transverse relaxation time (T2). Moreover, several quantitative MRI techniques are significantly influenced by the magic angle effect. The combinations of ultrashort echo time (UTE) MRI with magnetization transfer (UTE-MT) and Adiabatic T1ρ (UTE-AdiabT1ρ) imaging allow magic angle-insensitive assessments of all regions of articular cartilage. The purpose of this study was to investigate the correlations between quantitative three-dimensional UTE MRI biomarkers and mechanical properties of human tibiofemoral cartilage specimens. In total, 40 human tibiofemoral cartilage specimens were harvested from three male and four female donors (64 ± 18 years old). Cartilage samples were scanned using a series of quantitative 3D UTE Cones T2* (UTE-T2*), T1 (UTE-T1), UTE-AdiabT1ρ, and UTE-MT sequences in a standard knee coil on a clinical 3T scanner. UTE-MT data were acquired with a series of MT powers and frequency offsets to calculate magnetization transfer ratio (MTR), as well as macromolecular fraction (MMF) and macromolecular T2 (T2mm) through modeling. Cartilage stiffness and Hayes elastic modulus were measured using indentation tests. Correlations of 3D UTE Cones MRI measurements in the superficial layer, deep layer, and global regions of interest (ROIs) with mechanical properties were investigated. Cartilage mechanical properties demonstrated highest correlations with UTE measures of the superficial layer of cartilage. AdiabT1ρ, MTR, and MMF in superficial layer ROIs showed significant correlations with Hayes elastic modulus (p < 0.05, R = -0.54, 0.49, and 0.66, respectively). These UTE measures in global ROIs showed significant, though slightly lower, correlations with Hayes elastic modulus (p < 0.05, R = -0.37, 0.52, and 0.60, respectively). Correlations between other UTE MRI measurements (T2*, T1, and T2mm) and mechanical properties were non-significant. The 3D UTE-AdiabT1ρ and UTE-MT sequences were highlighted as promising surrogates for non-invasive assessment of cartilage mechanical properties. MMF from UTE-MT modeling showed the highest correlations with cartilage mechanics.

Abstract:Knee osteoarthritis (OA) is one of major reasons to causes pain and limit mobility for aging people. Early diagnosis of articular cartilage degeneration is of utmost importance for the prevention of OA. Thus, it is necessary to develop a novel magnetic resonance imaging technique to detect the early articular cartilage degeneration. Methods. T1ρ and T2-weighted images of the knees were obtained for 50 asymptomatic young subjects (23 men and 27 women; average age, 23.9 ± 1.8 years) and 17 Chinese National Men's hockey players (average age, 20.7 ± 1.1 years), using a 3.0-T MRI scanner. Quantitative T1ρ relaxation maps of the bilateral patella cartilage were generated by post-processing, and each map was divided into 15 regions of interest (ROIs). T1ρ values of each ROI were measured and compared between asymptomatic subjects and athletes. Results. T1ρ values of left patellar cartilage in athletes were significantly higher than those in healthy subjects (P < 0.05). T1ρ values of right patellar cartilage in athletes were also higher than healthy controls (P < 0.05). T1ρ values of patellar cartilage in both athletes and healthy subjects uncovered no apparent difference between the left knees and right knees (P > 0.05). Conclusion. T1ρ MRI could potentially represent a useful tool for the measurement of early articular cartilage degeneration in clinical practice.

Abstract:PurposeTo evaluate the magnetic susceptibility properties of different anatomical structures within the knee joint using quantitative susceptibility mapping (QSM).MethodsA collagen tissue model was simulated and ex vivo animal cartilage experiments were conducted at 9.4 Tesla (T) to evaluate the B0 orientation-dependent magnetic susceptibility contrast observed in cartilage. Furthermore, nine volunteers (six healthy subjects without knee pain history and three patients with known knee injury, between 29 and 58 years old) were scanned using gradient-echo acquisitions on a high-field 7T MR scanner. Susceptibility values of different tissues were quantified and diseased cartilage and meniscus were compared against that of healthy volunteers.ResultsSimulation and ex vivo animal cartilage experiments demonstrated that collagen fibrils exhibit an anisotropic susceptibility. A gradual change of magnetic susceptibility was observed in the articular cartilage from the superficial zone to the deep zone, forming a multilayer ultrastructure consistent with anisotropy of collagen fibrils. Meniscal tears caused a clear reduction of susceptibility contrast between the injured meniscus and surrounding cartilage illustrated by a loss of the sharp boundaries between the two. Moreover, QSM showed more dramatic contrast in the focal degenerated articular cartilage than R2* mapping.ConclusionThe arrangement of the collagen fibrils is significant, and likely the most dominant source of magnetic susceptibility anisotropy. Quantitative susceptibility mapping offers a means to characterize magnetic susceptibility properties of tissues in the knee joint. It is sensitive to collagen damage or degeneration and may be useful for evaluating the status of knee diseases, such as meniscal tears and cartilage disease. Abstract:Purpose To identify the topographical variation of T2 relaxation time of normal patella cartilage identified at arthroscopy.Materials and Methods Fortythree patients(1.5T MR) and 31 patients(3.0T MR) without patella cartilage lesions confirmed by arthroscopy underwent axial T2-mapping MR of patella cartilages.The patellar cartilages were divided into superior,middle and inferior parts.Each part was further divided into lateral and medial regions.T2 values of each region was compared both on 1.5T and 3.0T MR.Results There was no statistical difference in T2 values among the medial,lateral and total slice of patella cartilage(P 0.05),or among superior,middle and inferior parts(P 0.05) both on 1.5T and 3.0T MR.There were significant statistical differences of T2 values among deep,middle and superficial slices of the thickest slice of patella cartilage both on 1.5T and 3.0T MR(P 0.001) and T2 values of superficial slices were significantly higher than those of deep slices.T2 value of normal patella cartilage was decreased with the rising of magnetic field(P 0.001).Conclusion The normal variation of T2 value of patella cartilages is not significant among the different locations of compartment both on 1.5T and 3.0T MR.T2 values decrease significantly from the cartilage surface to the bone/cartilage interface.The T2 values of patella cartilage decrease with the rising of magnetic field.
Synthetic Knee MRI T1p Maps as an Avenue for Clinical Translation of Quantitative Osteoarthritis Biomarkers

Abstract:A 2D U-Net was trained to generate synthetic T1p maps from T2 maps for knee MRI to explore the feasibility of domain adaptation for enriching existing datasets and enabling rapid, reliable image reconstruction. The network was developed using 509 healthy contralateral and injured ipsilateral knee images from patients with ACL injuries and reconstruction surgeries acquired across three institutions. Network generalizability was evaluated on 343 knees acquired in a clinical setting and 46 knees from simultaneous bilateral acquisition in a research setting. The deep neural network synthesized high-fidelity reconstructions of T1p maps, preserving textures and local T1p elevation patterns in cartilage with a normalized mean square error of 2.4% and Pearson's correlation coefficient of 0.93. Analysis of reconstructed T1p maps within cartilage compartments revealed minimal bias (-0.10 ms), tight limits of agreement, and quantification error (5.7%) below the threshold for clinically significant change (6.42%) associated with osteoarthritis. In an out-of-distribution external test set, synthetic maps preserved T1p textures, but exhibited increased bias and wider limits of agreement. This study demonstrates the capability of image synthesis to reduce acquisition time, derive meaningful information from existing datasets, and suggest a pathway for standardizing T1p as a quantitative biomarker for osteoarthritis.
Measurement of T1ρ dispersion with compressed sensing and magnetization prepared radial balanced steady-state free precession in spontaneous human osteoarthritis

Abstract:Purpose: To assess the potential for accelerating continuous-wave (CW) T1ρ dispersion measurement with compressed sensing approach via studying the effect that the data reduction has on the ability to detect differences between intact and degenerated articular cartilage with different spin-lock amplitudes and to assess quantitative bias due to acceleration. Methods: Osteochondral plugs (n = 27, 4 mm diameter) from femur (n = 14) and tibia (n = 13) regions from human cadaver knee joints were obtained from commercial biobank (Science Care, USA) under Ethical permission 134/2015. MRI of specimens was performed at 9.4T with magnetization prepared radial balanced SSFP (bSSFP) readout sequence, and the CWT1ρ relaxation time maps were computed from the measured data. The relaxation time maps were evaluated in the cartilage zones for different acceleration factors. For reference, Osteoarthritis Research Society International (OARSI) grading and biomechanical measurements were performed and correlated with the MRI findings. Results: Four-fold acceleration of CWT1ρ dispersion measurement by compressed sensing approach was feasible without meaningful loss in the sensitivity to osteoarthritic (OA) changes within the articular cartilage. Differences were significant between intact and OA groups in the superficial and transitional zones, and CWT1ρ correlated moderately with the reference measurements (0.3 < r < 0.7). Conclusion: CWT1ρ was able to differentiate between intact and OA cartilage even with four-fold acceleration. This indicates that acceleration of CWT1ρ dispersion measurement by compressed sensing approach is feasible with negligible loss in the sensitivity to osteoarthritic changes in articular cartilage.

Abstract:Background T 2 mapping of short-T 2 tissues in the knee (meniscus, tendon, and ligament) is needed to aid the clinical MRI knee diagnosis, which is hard to realize using traditional clinical methods. Purpose To accelerate the acquisition of T 2 values for short-T 2 tissues in the knee by analyzing the signal equation of balanced steady-state free precession (bSSFP) sequence in MRI. Methods Effect of half-radial acquisition on pixel bandwidth was analyzed mathematically. A modified 3D radial dual-echo bSSFP sequence was proposed for 0.53 mm isotropic resolution knee imaging with 2 different TEs at 3 T, which alleviated the problem of off-resonance artifacts caused by traditional half-radial acquisition scheme. A novel pixel-based optimization method was proposed for efficient T 2 mapping of short-T 2 tissues in the knee given off-resonance values. Simulation was conducted to evaluate the sensitivity of the proposed method to other parameters. Phantom results were compared with 2D spin-echo (SE), and in vivo results were compared with SE and previously studies. Results Simulation showed that the proposed method is insensitive to T 1 and B 1 variations (estimation error < 1% for T 1 /B 1 error of ±90%), avoiding the need for separated T 1 and B 1 scans. High isotropic resolution knee imaging was achieved using the modified dual-echo bSSFP. The total scan time was within 3.5 min, including a separate off-resonance scan for T 2 measurement. Measured mean T 2 values for phantoms correlated well with SE (R 2 = 0.99), and no significant difference was observed ( P = 0.45). In vivo meniscus T 2 measurements and ligament T 2 measurements agreed with the literature, while tendon T 2 measurements were much lower (31.7% lower for patellar tendon, and 13.5% lower for quadriceps tendon), which might result in its bi-component property. Conclusions The proposed method provides an efficient way for fast, robust, high-resolution imaging and T 2 mapping of short-T 2 tissues in the knee.

Abstract:Background The biomechanical properties of deep and superficial cartilage may be different, yet in vivo MRI validation is required. Purpose To compare the effect of mechanical loading on deep and superficial cartilage in young healthy adults using ultrashort echo time (UTE)‐T2* mapping. Study Type Prospective, intervention. Subjects Thirty‐one healthy adults (54.8% females, median age = 23 years). Field Strength/Sequence 3‐T, PD‐FS, and UTE sequences with four echo times (TEs = 0.1, 0.5, 2.8, and 4.0 msec; 0.6 mm isotropic spatial resolution) of the left knee, acquired before and after loading exercise. Assessment Quantitative UTE‐T2* maps of the entire knee were generated using UTE images of four TEs. In deep and superficial cartilage of patella, medial and lateral femur, medial and lateral tibia cartilage (PC, MFC, LFC, MTC, and LTC), which were segmented manually, cartilage thickness and T2* values before and after loading were measured, extracted, taken averages of, and compared. Scan–rescan repeatability was evaluated. Body weight and body mass index (BMI) data were collected. Physical activity levels were evaluated using International Physical Activity Questionnaire.
