In one of the first reports, Mellins et al. observed that patients with advanced cystic fibrosis spontaneously adopted the prone position to improve ventilation (2). From these early experiences, the beneficial effects of the prone position began to be documented. In the supine position, the dorsal areas of the lung are compressed by the weight of the mediastinal structures, the pleural pressure gradient, and the rest of the lung itself. In contrast, in the prone position, these areas are relieved of this pressure, with an increase in functional residual capacity, resulting in improved ventilation/perfusion ratio, cardiac output, and diaphragm function (3). Prone positioning in intubated and mechanically ventilated patients has been shown to improve pulmonary gas exchange and lung mechanics in patients with severe hypoxemic respiratory failure. In fact, it is part of most intensive care unit protocols. There are current guidelines (4) that support this intervention in intubated patients with acute respiratory distress syndrome (ARDS), specifying that patients must be in the prone position for at least 16 hours per day to be effective. During the coronavirus disease (COVID) pandemic, different strategies have been tried in the management of patients with COVID and moderate hypoxaemia to prevent worsening of the disease. Perhaps the most studied has been the early administration of therapy with high flow nasal cannula (HFNC). Crimi et al. found no significant difference in disease progression in a cohort of patients with an arterial partial pressure of oxygen (PaO 2 )/fraction of inspired oxygen (FiO 2 ) ratio between 200 and 300 after randomisation to high flow or conventional oxygen therapy (5). In contrast, differences in intubation rates have been demonstrated in patients with PaO 2 /FiO 2 below 200 (6,7). Awake proning position (APP) has been also used as a strategy, suggesting that it could improve the prognosis of non-intubated patients with moderate or severe respiratory failure. The aim of APP would be to prevent the progression of ARF, thereby reducing the need for orotracheal intubation and ultimately improving survival. Some early studies demonstrated a lower rate of intubation in patients under APP (8). However, the results of the studies are conflicting and merit careful consideration (9). Although considered a non-invasive treatment, tolerance to APP is highly variable, with most patients not tolerating it for long periods of time. For example, in the meta-trial published by Ehrmann et al. , which included 6 RCTs, the number of hours of pronation per day ranged from 1.6 to 8 hours (10). Some other studies achieved a mean duration of 12 hours in the first day of admission to intensive care unit (ICU) (11). Although no ideal number of hours of compliance has been defined for awake patients, in the aforementioned meta-trial, the rate of APP failure and progression of lung disease was significantly lower in patients with longer hours of compliance. Table 1 shows the most relevant studies on APP. But the question did not stop at critical care. In a randomised controlled trial, Nay et al. recruited 265 patients on inpatient wards in 15 hospitals. Patients were randomised to awake proning plus usual care in the intervention arm and usual care in the control arm. The target time interval for patients to be in the prone position was 270 min/day. Optimal timing for awake prone positioning in Covid-19 patients: Insights from an observational study from two centers

Abstract:BACKGROUND: The widespread application and interest in awake prone positioning stems from its ease and availability and its ability to enhance patients' oxygenation. Nevertheless, due to the absence of consensus over the regimen of awake prone positioning, the efficacy of awake prone positioning remains uncertain.OBJECTIVE: To explore the optimal regimen for awake prone positioning, including the timing of initiation, ideal daily duration, and strategies for improving patient comfort and encouraging adherence.DESIGN: Retrospective observational study.SETTING(S): Two university-affiliated hospitals in Shanghai.PARTICIPANTS: Between December 2022 and February 2023, a total of 475 patients with COVID-19-related pneumonia who received awake prone positioning were observed.METHODS: The data were collected from the hospital's electronic medical record system. The differentiation efficiency of peripheral blood oxygen saturation [SpO2]:fractional oxygen concentration in inspired air [FiO2] ratio at first awake prone positioning for different outcomes was tested by the area under the receiver operating characteristic curve. The Cox proportional hazard regression model was used to analyze the relationship between time to occurrence of 28-day outcomes and collected variables. Kaplan-Meier curves were plotted with the percentage of 28-day outcomes according to the SpO2:FiO2 ratio at first awake prone positioning after controlling covariates through Cox regression.RESULTS: The best efficiency in predicting patient outcomes was achieved when the cutoff SpO2:FiO2 ratio at first awake prone positioning was 200. Patients with a reduced SpO2:FiO2 ratio (≤200) experienced more adverse respiratory outcomes (RR = 5.42, 95%CI [3.35, 8.76], p < 0·001) and higher mortality (RR = 16.64, 95%CI [5.53, 50.13], p < 0.001). Patients with a SpO2:FiO2 ratio of ≥200 at first awake prone positioning, longer duration between first awake prone positioning and admission, more awake prone positioning days, and better awake prone positioning completion were significantly protected from 28-day adverse respiratory outcomes and mortality.CONCLUSIONS: Initiating awake prone positioning with a SpO2:FiO2 ratio exceeding 200, increasing the number of awake prone positioning days, prolonging the time between first awake prone positioning and admission, and achieving better completion of awake prone positioning were found to be significantly associated with reduced adverse respiratory outcomes and mortality.REGISTRATION: ClinicalTrials.gov; No.: NCT05795751; URL: www.CLINICALTRIALS: gov.

Prone Position Improves Survival rate of Covid-19 Invasive Mechanical Ventilation Patients by Improving Oxygenation Index

Abstract:Background: Invasive mechanical ventilation is a crucial intervention for the management of critical COVID-19 patients. However, the impact of prone position (PP) on patients undergoing invasive mechanical ventilation remains uncertain. This study aims to investigate the potential benefits of PP in terms of improving the oxygenation index and prognosis. Methods: A total of 289 critically ill COVID-19 patients were retrospectively gathered from ICU of three general hospitals located in Taizhou, Zhejiang Province from December 1, 2022 to February 1, 2023, all patients were invasive mechanical ventilated. 78 cases of PP group and 78 cases of non-PP group were matched with propensity score matching. The study compared clinical data, laboratory results, and hospitalization survival rate between two groups of patients. Furthermore, we compared the laboratory results, and hospitalization survival across varying numbers of PPs. Results: The mean oxygenation index exhibited a greater increase in PP group compared to non-PP group (48 vs 32mmHg). Hospital survivors of PP group (63 patients) demonstrated more substantial decrease in their Sequential Organ Failure Assessment scores and C-reactive protein levels compared to non-PP group (51 patients). The initial PP cycle resulted in a significant elevation of the oxygenation index by 30.8 (-16.4,46.9) mmHg; the second PP cycle demonstrated a maximum increase of 56.3 (13.0,92.8) mmHg. A higher frequency of PP yielded a more pronounced improvement in oxygenation and had the potential to enhance the survival rate. Additionally, the eight patients who was improvements in their oxygenation index during the initial three PP cycles and successfully survived had higher lymphocyte counts (0.2-2.4) vs (0.1-0.5)×10 /L and a longer duration of PP (53.0-113.5) vs (36.0-98.5)h. Conclusion: PP has the potential to enhance the oxygenation index and survival rates among critically ill COVID-19 patients invasive mechanical ventilated. Notably, a positive correlation was observed between the frequency of PP and the improvement of oxygenation. Our investigation further revealed that the efficacy of the PP may be influenced by lymphocyte count and duration of PP.
Effects of awake prone position vs. usual care on acute hypoxemic respiratory failure in patients with COVID-19: A systematic review and meta-analysis of randomized controlled trials

Abstract:Background Previous studies have shown that an awake prone position may be beneficial for the treatment of acute respiratory distress syndrome (ARDS) or acute hypoxic respiratory failure (AHRF) in patients with COVID-19, but the results are not consistent, especially in terms of oxygenation outcomes and intubation rate. This systematic review and meta-analysis assessed the effects of the awake prone position on AHRF in patients with COVID-19 with all randomized controlled trials (RCTs). Methods An extensive search of online databases, including MEDLINE, Embase, Web of Science, and Cochrane Central Register of Controlled Trials from 1 December 2019 to 30 October 2022, with no language restrictions was performed. This systematic review and meta-analysis are based on the PRISMA statement. We only included RCTs and used the Cochrane risk assessment tool for quality assessment. Results Fourteen RCTs fulfilled the selection criteria, and 3,290 patients were included. A meta-analysis found that patients in the awake prone position group had more significant improvement in the SpO 2 /FiO 2 ratio [mean difference (MD): 29.76; 95% confidence interval (CI): 1.39–48.13; P = 0.001] compared with the usual care. The prone position also reduced the need for intubation [odd ratio (OR): 0.72; 95% CI: 0.61 to 0.84; P < 0.0001; I 2 = 0%]. There was no significant difference in mortality, hospital length of stay, incidence of intensive care unit (ICU) admission, and adverse events between the two groups. Conclusion The awake prone position was a promising intervention method, which is beneficial to improve the oxygenation of patients with ARDS or AHRF caused by COVID-19 and reduce the need for intubation. However, the awake prone position showed no obvious advantage in mortality, hospital length of stay, incidence of ICU admission, and adverse events. [Application and significance of prone position in the treatment of patients with severe pneumonia in intensive care unit]

Abstract:Objective: To investigate the effect of prone position on the prognosis of patients with severe pneumonia in intensive care unit (ICU). Methods: A retrospective cohort study was conducted. The patients with severe pneumonia admitted to the ICU of Qingdao Municipal Hospital from May 2022 to August 2023 were enrolled. The general information, etiology, underlying diseases, vital signs and laboratory indicators at ICU admission, clinical treatment and prognosis during ICU hospitalization were collected. The above clinical data of patients with different prognosis were compared. Multifactorial Logistic regression analysis was used to screen the related factors affecting survival during ICU in patients with severe pneumonia. The change in oxygenation index (PaO2/FiO2) of patients with severe pneumonia were observed at 1 hour before the first prone position, 1 hour after the first prone position, and 1 hour after the end of the first prone position. The effect of prone position on oxygenation in patients with severe pneumonia was analyzed. Spearman correlation analysis was used to investigate the correlation between the duration to first prone position and the change in the PaO2/FiO2 before and after prone position in patients with severe pneumonia. Results: Finally, a total of 144 patients with severe pneumonia were enrolled, 45 survived and 99 died during ICU hospitalization, with a mortality of 68.8%. Compared with the survival group, the patients in the death group were older [years old: 81.00 (70.75, 86.00) vs. 71.00 (60.50, 81.50), P < 0.01], the proportion of pre-existing lung disease, heart rate (HR), respiratory rate (RR), blood lactic acid (Lac) and the ratio of continuous renal replacement therapy (CRRT) were higher [ratio of pre-existing lung disease: 23.2% (23/99) vs. 8.9% (4/45), HR (bpm): 99.61±22.47 vs. 91.49±18.76, RR (times/min): 22.50 (19.75, 29.25) vs. 20.00 (17.50, 24.50), Lac (mmol/L): 2.00 (1.55 , 3.25) vs. 1.60 (1.20, 1.95), CRRT ratio: 25.3% (25/99) vs. 6.7% (3/45), all P < 0.05], and the proportion of prone position was lower [41.4% (41/99) vs. 68.9% (31/45), P < 0.01]. Multifactorial Logistic regression analysis showed that age [odds ratio (OR) = 0.946, 95% confidence interval (95%CI) was 0.912-0.980, P = 0.002] and Lac (OR = 0.563, 95%CI was 0.340-0.930, P = 0.025) were negatively correlated with survival during ICU hospitalization in severe pneumonia patients, while prone position was positively correlated with survival (OR = 2.551, 95%CI was 1.067-6.095, P = 0.035), indicating that prone position was beneficial for improving ICU prognosis in severe pneumonia patients. The results of PaO2/FiO2 at different time points in prone position showed that PaO2/FiO2 at 1 hour of the first prone position in the patients with severe pneumonia was significantly higher than that at 1 hour before the first prone position [mmHg (1 mmHg ≈ 0.133 kPa): 146.69 (113.92, 257.25) vs. 111.75 (70.15, 212.20), P < 0.01], indicating that the prone position had a relevant effect on the improvement of oxygenation in patients. Spearman correlation analysis showed that the duration of the first prone position in patients with severe pneumonia was significantly and positively correlated with the improvement of oxygenation at 1 hour of the first prone position (r = 0.565, P < 0.001). Conclusions: The prone position is a therapeutic measure that can independently influence the prognosis of patients with severe pneumonia during ICU hospitalization. The prone position effectively improves oxygenation in patients with severe pneumonia and the first change in oxygenation in patients is related to the duration of the prone position.

Published online Jan 11, 2024. doi: 10.21037/jtd-23-1389 The advent of the coronavirus disease 2019 (COVID-19) pandemic presented an unprecedented challenge to the medical field, necessitating innovative approaches to address the severe respiratory symptoms linked to the virus. Influenced by its success in treating acute respiratory distress syndrome (ARDS), prone positioning as a rescue maneuver, after the lack of improvement under optimal ventilatory settings, was used for COVID-19 patients. However, it's noteworthy that COVID-19 pneumonia diverges from traditional ARDS in several key aspects. Autopsies of COVID-19 pneumonia fatalities have revealed distinctive features like hyaline membrane changes and micro-vessel thrombosis, uncommon in classic ARDS cases (1-4). Furthermore, the anatomical presentation of COVID-19 pneumonia differs, with predominant bilateral, peripheral, and multilobar ground-glass opacities on computed tomography (CT) scans, deviating from the characteristic involvement of dependent lung regions observed in ARDS (5). Consequently, the efficacy of prone positioning in ameliorating ventilation-perfusion mismatch might not be as pronounced due to the atypical location of lung abnormalities. In the context of ventilated patients with COVID-19 pneumonia, research suggests that prone positioning may not significantly alter the respiratory system and lung mechanics. This indicates that lung aeration tends to remain preserved in COVID-19 pneumonia patients and might even trend towards deterioration due to ventilation in the prone position (6). Paradoxically, this finding suggests that despite severe lung damage, lung mechanics might present as normal during COVID-19 pneumonia, and trans-pulmonary pressures could remain below detrimental thresholds (6). The effectiveness of prone positioning as a therapeutic approach for COVID-19 patients remains a topic of debate due to conflicting findings from various studies. In a randomized trial involving awake patients across fifteen hospitals in North America, patients were assigned to either prone positioning or standard treatment, both groups showing similar clinical outcomes, including mortality rates, need for mechanical ventilation, and respiratory failure worsening (7). Another meta-trial spanning six countries indicated that awake-prone positioning reduced treatment failure and intubation needs in hypoxemic respiratory failure cases without significant mortality improvement (8). Conversely, a meta-analysis encompassing mechanically ventilated patients showed no reduction in mortality, intensive care unit (ICU) stay, or mechanical ventilation duration in prone positioned patients (9,10). Some of the studies included in this meta-analysis had limitations that were addressed (11-13). The study conducted by Nay et al. (14) aimed to assess the effectiveness of prone positioning as a treatment for less severe hospitalized COVID-19 patients. The study was conducted as a randomized trial with patients from France and Monaco, from August 2020 to January 2022 and involved 268 awake patients who were admitted for COVID-19 in 15 medical wards at 12 hospitals. Patients were randomly assigned to either the prone positioning group or the usual care group. In the prone positioning group, patients were placed in a prone position for a minimum of two sessions with a cumulative time of at least 150 minutes during the daytime. Prone positioning was allowed at night if it was the patient's natural sleeping position. Patients in the usual care group were kept in the semi-sitting position in bed or a chair during the daytime. The results showed that the patients in the prone positioning group received at least one session of prone positioning, with 74.8% of patients proned on the day of enrollment. The median time spent in the prone position per day during the daytime was 138 minutes. Some patients experienced intolerance or refusal, resulting in 12.1% of patients having at least one day without prone positioning. Thirty-one-point-one percent of patients in the prone positioning group laid prone for more than 2 hours each day during the daytime. [Application effect analysis of lateral prone position ventilation in patients with acute respiratory distress syndrome]

Abstract:Objective: To investigate the effect of lateral prone position ventilation in patients with acute respiratory distress syndrome (ARDS). Methods: A prospective control study was conducted. A total of 75 patients with moderate to severe ARDS admitted to the department of critical care medicine of Jingxian Hospital in Anhui province from January 2020 to December 2022 were selected as the research objects. According to the envelope method, the patients were divided into the lateral prone position ventilation group (38 cases) and the traditional prone position ventilation (PPV) group (37 cases), using lateral prone position ventilation and traditional PPV, respectively. The mechanical ventilation parameters were set according to the ARDS treatment guidelines and lung protective ventilation requirements in both groups, and the time of prone position for the first 3 times was not less than 16 hours per day. General data of patients were recorded, including heart rate (HR), mean arterial pressure (MAP), airway resistance and lung static compliance (Cst) before prone position (T0), 1 hour (T1), 4 hours (T2), 8 hours (T3), and before the end of prone position (T4), oxygenation index (PaO2/FiO2) before the first prone position (t0) and 12 hours (t1), 24 hours (t2), 48 hours (t3), and 72 hours (t4) after the intensive care unit (ICU) admission, as well as the incidence of pressure injury (PI) and vomiting, tracheal intubation time, and mechanical ventilation time. Repeated measures analysis of variance was used to compare the effects of different prone positions on patients before and after the prone position. Results: There were no significant differences in age, gender, body mass index (BMI), acute physiology and chronic health evaluation II (APACHE II), underlying diseases, HR, MAP, pH value, PaO2/FiO2, blood lactic acid (Lac), arterial blood pressure of carbon dioxide (PaCO2) and other general information between the two groups. The HR (intergroup effect: F = 0.845, P = 0.361; time effect: F = 1.373, P = 0.247; interaction: F = 0.245, P = 0.894), MAP (intergroup effect: F = 1.519, P = 0.222; time effect: F = 0.169, P = 0.954; interaction: F = 0.449, P = 0.773) and airway resistance (intergroup effect: F = 0.252, P = 0.617; time effect: F = 0.578, P = 0.679; interaction: F = 1.467, P = 0.212) of T0-T4 between two groups showed no significant difference. The Cst of T0-T4 between the two groups showed no significant difference in the intergroup effect (F = 0.311, P = 0.579) and the interaction (F = 0.364, P = 0.834), while the difference in the time effect was statistically significant (F = 120.546, P < 0.001). The PaO2/FiO2 of t0-t4 between the two groups showed no significant difference in the intergroup effect (F = 0.104, P = 0.748) and the interaction (F = 0.147, P = 0.964), while the difference in the time effect was statistically significant (F = 17.638, P < 0.001). The group factors and time factors were tested separately, and there were no significant differences in the HR, MAP, airway resistance, Cst, PaO2/FiO2 between the two groups at different time points (all P > 0.05). The Cst at T1-T4 and PaO2/FiO2 at t1-t4 in the two groups were significantly higher than those at T0/t0 (all P < 0.05). There were no significant differences in the tracheal intubation time [days: 6.75 (5.78, 8.33) vs. 7.00 (6.30, 8.45)] and mechanical ventilation time [days: 8.30 (6.70, 9.20) vs. 7.40 (6.80, 8.75)] between the lateral prone position ventilation group and the traditional PPV group (both P > 0.05). However, the incidences of PI [7.9% (3/38) vs. 27.0% (10/37)] and vomiting [10.5% (4/38) vs. 29.7% (11/37)] in the lateral prone position ventilation group were significantly lower than those in the traditional PPV group (both P < 0.05). Conclusions: Both lateral prone position ventilation and traditional PPV can improve Cst and oxygenation in patients with moderate to severe ARDS. The two types of prone position have little influence on HR, MAP and airway resistance of patients, and there is no difference in the influence on tracheal intubation time and mechanical ventilation time of patients. However, the lateral prone position ventilation mode can reduce the incidence of PI and vomiting, and is worthy of clinical promotion and application.

Abstract:The prone position is an immediately available and easily implemented procedure that was introduced more than 50 years ago as a method for improvement of gas exchange in patients with acute respiratory distress syndrome (ARDS). In the meantime, a survival advantage could also be shown in patients with severe ARDS, which led to the recommendation of the prone position for treatment of severe ARDS by expert consensus and specialist society guidelines. The continuing coronavirus disease 2019 (COVID-19) pandemic moved the prone position to the forefront of medicine, including the widespread implementation of the prone position for awake, spontaneously breathing nonintubated patients with acute hypoxemic respiratory insufficiency. The survival advantage is possible due to a reduction of the ventilator-associated lung damage. In this article, the physiological effects, data on clinical results, practical considerations and open questions with respect to the prone position are discussed.
Prone positioning for patients intubated for severe acute respiratory distress syndrome (ARDS) secondary to COVID-19: a retrospective observational cohort study

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3>The role of prone positioning in intubated subjects with acute respiratory distress syndrome caused by coronavirus disease 2019 (COVID-19) remains unclear.<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3>We conducted an observational cohort study of intubated patients admitted to our academic medical centre intensive care unit with COVID-19 between March 18-31 2020. Exclusion criteria were pregnancy, reintubation and previous prone positioning at a referring hospital. Patients that were placed in the prone position were followed up until hospital discharge. The primary outcome was oxygenation assessed by arterial oxygen tension/fraction of inspired oxygen ratio (PaO2/FIO2). Secondary outcomes included PaO2/FIO2 ratio improvement ≥20%. Treatment failure of prone positioning was defined as death or requirement for extracorporeal membrane oxygenation (ECMO).<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3>Forty-two subjects (29 males; mean age:58.5 [12.7] years) were eligible for analysis. Nine subjects were placed in the prone position only once, with 25 requiring prone positioning on ≥3 occasions. 31/42 (74%) subjects survived to discharge, with 5 requiring ECMO; 11/42 (26.2%) subjects died. Following the first prone positioning session, mean (SD) PaO2/FIO2 ratio increased from 17.9kPa (7.2) to 28.2kPa (12.2) (p<0.01). With repeated prone positioning sessions, 31/42 (73.8%) subjects who were discharged from hospital had PaO2/FIO2 ratio improvement of ≥20%, compared to 11/42 subjects who either died or required ECMO (odds ratio:XX (95% confidence intervals:xx-xx)).<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3>Patients with COVID-19 ARDS frequently responded to initial prone positioning with improved oxygenation. Subsequent prone positioning in subjects discharged from hospital was associated with greater improvements in oxygenation.

Abstract:The coronavirus disease (COVID-19) pandemic has significantly increased the number of patients with acute respiratory distress syndrome (ARDS), necessitating respiratory support. This strain on intensive care unit (ICU) resources forces clinicians to limit the use of mechanical ventilation by seeking novel therapeutic strategies. Awake-prone positioning appears to be a safe and tolerable intervention for non-intubated patients with hypoxemic respiratory failure. Meanwhile, several observational studies and meta-analyses have reported the early use of prone positioning in awake patients with COVID-19-related ARDS (C-ARDS) for improving oxygenation levels and preventing ICU transfers. Indeed, some international guidelines have recommended the early application of awake-prone positioning in patients with hypoxemic respiratory failure attributable to C-ARDS. However, its effectiveness in reducing intubation rate, mortality, applied timing, and optimal duration is unclear. High-quality evidence of awake-prone positioning for hypoxemic patients with COVID-19 is still lacking. Therefore, this article provides an update on the current state of published literature about the physiological rationale, effect, timing, duration, and populations that might benefit from awake proning. Moreover, the risks and adverse effects of awake-prone positioning were also investigated. This work will guide future studies and aid clinicians in deciding on better treatment plans.

Abstract:Background: Awake prone positioning has been broadly utilised for non-intubated patients with COVID-19-related acute hypoxaemic respiratory failure, but the results from published randomised controlled trials (RCTs) in the past year are contradictory. We aimed to systematically synthesise the outcomes associated with awake prone positioning, and evaluate these outcomes in relevant subpopulations. Methods: In this systematic review and meta-analysis, two independent groups of researchers searched MEDLINE, Embase, PubMed, Web of Science, Scopus, MedRxiv, BioRxiv, and ClinicalTrials.gov for RCTs and observational studies (with a control group) of awake prone positioning in patients with COVID-19-related acute hypoxaemic respiratory failure published in English from Jan 1, 2020, to Nov 8, 2021. We excluded trials that included patients intubated before or at enrolment, paediatric patients (ie, younger than 18 years), or trials that did not include the supine position in the control group. The same two independent groups screened studies, extracted the summary data from published reports, and assessed the risk of bias. We used a random-effects meta-analysis to pool individual studies. We used the Grading of Recommendations Assessment, Development, and Evaluation approach to assess the certainty and quality of the evidence. The primary outcome was the reported cumulative intubation risk across RCTs, and effect estimates were calculated as risk ratios (RR;95% CI). The analysis was primarily conducted on RCTs, and observational studies were used for sensitivity analyses. No serious adverse events associated with awake prone positioning were reported. The study protocol was prospectively registered with PROSPERO, CRD42021271285. Findings: A total of 1243 studies were identified, we assessed 138 full-text articles and received the aggregated results of three unpublished RCTs; therefore, after exclusions, 29 studies were included in the study. Ten were RCTs (1985 patients) and 19 were observational studies (2669 patients). In ten RCTs, awake prone positioning compared with the supine position significantly reduced the need for intubation in the overall population (RR 0·84 [95% CI 0·72-0·97]). A reduced need for intubation was shown among patients who received advanced respiratory support (ie, high-flow nasal cannula or non-invasive ventilation) at enrolment (RR 0·83 [0·71-0·97]) and in intensive care unit (ICU) settings (RR 0·83 [0·71-0·97]) but not in patients receiving conventional oxygen therapy (RR 0·87 [0·45-1·69]) or in non-ICU settings (RR 0·88 [0·44-1·76]). No obvious risk of bias and publication bias was found among the included RCTs for the primary outcome. Interpretation: In patients with COVID-19-related acute hypoxaemic respiratory failure, awake prone positioning reduced the need for intubation, particularly among those requiring advanced respiratory support and those in ICU settings. Awake prone positioning should be used in patients who have acute hypoxaemic respiratory failure due to COVID-19 and require advanced respiratory support or are treated in the ICU. Abstract:Objectives: To assess the effectiveness and safety of prone positioning in the treatment of neonatal respiratory distress syndrome (NRDS) using invasive respiratory support. Methods: A prospective study was conducted from June 2020 to September 2023 at Suining County People's Hospital, involving 77 preterm infants with gestational ages less than 35 weeks requiring invasive respiratory support for NRDS. The infants were randomly divided into a supine group (37 infants) and a prone group (40 infants). Infants in the prone group were ventilated in the prone position for 6 hours followed by 2 hours in the supine position, continuing in this cycle until weaning from the ventilator. The effectiveness and safety of the two approaches were compared. Results: At 6 hours after enrollment, the prone group showed lower arterial blood carbon dioxide levels, inspired oxygen concentration, oxygenation index, rates of tracheal intubation bacterial colonization, and Neonatal Pain, Agitation and Sedation Scale scores compared to the supine group (P<0.05). There were no significant differences between the groups in terms of pH, arterial oxygen pressure, positive end-expiratory pressure, duration of mechanical ventilation, accidental extubation, ventilator-associated pneumonia, air leak syndrome, skin pressure sores, feeding intolerance, and grades II-IV intraventricular hemorrhage (P>0.05). Conclusions: Compared to supine positioning, prone ventilation effectively improves oxygenation, increases comfort, and reduces tracheal intubation bacterial colonization in neonates requiring mechanical ventilation for NRDS, without significantly increasing adverse reactions.

Abstract:Perfusion, Ahead of Print. BackgroundThis study aimed to investigate the effects of prone position ventilation treatment on patients with acute respiratory distress syndrome (ARDS) after cardiac surgery.MethodsClinical data were collected from 93 hospitalized patients with acute respiratory distress syndrome (ARDS) in the intensive care unit (ICU) of cardiology from February 2021 to February 2023. Patients were divided into supine position group (n = 45) and prone position group (n = 48). The difference in 28-days survival rates, blood gas indicators, respiratory mechanics indicators, and adverse events before and after treatment was analyzed.ResultsWe found that within 28 days of admission, 8.33% of ARDS patients in prone position group and 11.11% in supine position group died of all causes (p > .05). After treatment, the levels of arterial PaO2 (103.25 ± 9.44 in prone position group and 91.62 ± 9.18 in supine position group), PaCO2 (30.26 ± 5.54 and 36.56 ± 6.37), blood LAC (1.35 ± 0.37 and 1.68 ± 0.42), oxygenation (232.23 ± 28.56 and 205.13 ± 31.34) and diffusion index (453.48 ± 63.30 and 395.18 ± 58.54) in both groups were improved (p < .001). Moreover, the increase in prone position group was more remarkable. After treatment, the respiratory mechanics indexes of the lung compliance as well as respiratory resistance were improved (p < .05). Moreover, the increase in supine position group was more remarkable (p < .05). The incidence of atelectasis in prone position group was lower than that in supine position group (p .05).ConclusionsTaken together, prone position ventilation in patients with ARDS after cardiac surgery improved blood gas indexes, hypoxemia, and respiratory mechanics indexes, as well as reduced the incidence of atelectasis.

Abstract:Background: Awake prone positioning (APP) is broadly implemented in patients with severe acute respiratory syndrome coronavirus 2 related disease [coronavirus disease 2019 (COVID-19)] admitted to hospital with severe respiratory distress syndrome. This prospective observational study aimed to explore the factors influencing the implementation of APP in patients with acute respiratory failure due to COVID-19.Methods: Patients with COVID-19, all hospitalized with positive X-ray findings and oxygen supplementation requirement, in the Respiratory Step-Down Unit of the Peking University Third Hospital between January 6th, 2023, and January 20th, 2023, were included in this study. Data regarding basic information, activities of daily living (ADLs) scores, oxygen therapy, vital signs, and duration of APP were collected to investigate the factors influencing prone positioning.Results: Among the 134 patients included, 55.2% showed an improvement in oxygen saturation 1 hour after APP. Logistic regression revealed that the pre-APP heart rate (HR) [odds ratio (OR) =1.032; P=0.046] and peripheral oxygen saturation (SpO(2)) (OR =0.720; P<0.001) were the associated factors of the improvement in SpO(2) after treatment. Multiple linear regression revealed that the ADL scores and pre-APP respiratory rate (RR) were the associated factors of the duration of prone positioning (P<0.01). The APP technical steering group effectively improved duration of APP.Conclusions: Patients with low SpO(2) and increased HR before treatment showed greater improvement in oxygen saturation. Patients with lower tolerance to ADL but lower RRs were those to demonstrate a longer duration of prone positioning. This is pointing towards establishing the most favorable time window for APP during the course of COVID-19: after the ADLs have already decreased, but before significant tachypnea has appeared.

Abstract:BACKGROUND: Previously prone positioning (PP) was described in addition to invasive mechanical ventilation and it has been known to reduced mortality and improve oxygenation in patients of ARDS. Recently novel timing of prone positioning was described with the use of high-frequency nasal cannula (HFNC) and noninvasive ventilation (NIV) in patients of acute respiratory distress syndrome (ARDS) to avoid the intubation. Here we would like to share a case of severe ARDS where prone positioning was used in a step further ahead.CASE DESCRIPTION: A 38-year-old gentleman presented with the complaints of progressive breathlessness, dry cough and fever for 7 days. Patient was diagnosed as a case of H1N1 pneumonia with severe ARDS. Patient was initially managed with invasive mechanical ventilation according to ARDS-Net protocol. Despite persistent hypoxia he was put on prone positioning for consecutive 4 days. Patient was extubated after 10 days of mechanical ventilation and put on HFNC in view of persistent high oxygen requirement. At this point of time, we attempted prone positioning in addition to HFNC. Patient was comfortable on prone position and put himself in the same condition for prolonged periods. His oxygenation showed a remarkable improvement from PaO2 of 63 (before prone positioning) to 136 mm Hg (after prone positioning). Oxygen supplementation was later tapered off and subsequently, he improved and was shifted to ward.CONCLUSION: Prone positioning is a harmless and still extremely effective intervention which can and should be utilized at all steps of ARDS-management.HOW TO CITE THIS ARTICLE: Lalwani LK, Sharma V, Chaudhry D, Singh PK. Incorporating Self-Efficacy Theory to Awake Prone Positioning in COVID-19 Patients: A Single-Center Perspective Study

Abstract:Objective: This study sought to examine whether the inclusion of self-efficacy theory could increase the effectiveness of awake prone positioning in COVID-19 patients. Method: A total of 80 COVID-19 patients admitted to the respiratory department of our hospital between December 2022 and February 2023 were randomly allocated into either the intervention group (n=40) or the control group (n=40). The control group received routine respiratory medicine treatment and underwent awake prone positioning, whereas the intervention group received additional self-efficacy theory intervention alongside awake prone positioning. The patients’ oxygen saturation level (SpO2) and respiratory rate (RR) were closely monitored at three different time points: before awake prone positioning, 30 minutes and 1 hour after awake prone positioning. Furthermore, the patients’ Modified Medical Research Council (mMRC) dyspnea scale and self-efficacy scores were evaluated on the first, second, and third days of awake prone positioning. The cumulative duration of awake prone positioning and patient satisfaction were recorded on the third day of the intervention. Results: The data indicate that the intervention group demonstrated significant improvements in both SpO2 and RR compared to the control group, both at 1 hour and at the end of awake prone positioning (P＜0.05). Moreover, the intervention group experienced a substantial reduction in the dyspnea index compared to the control group on the second and third day (P＜0.05). The intervention group also reported higher self-efficacy scores, cumulative duration of awake prone positioning ventilation on the third day, and satisfaction levels compared to the control group on the first, second, and third day (P<0.05). Conclusions: The incorporation of self-efficacy theory to awake prone positioning improves the short-term outcomes of COVID-19 patients. This patient-centered approach has the potential to increase patients’ self-efficacy and satisfaction levels, emphasizing the importance of incorporating patient-centered care in healthcare practices.

Abstract:Abstract Background Awake prone positioning (APP) is widely used in the management of patients with coronavirus disease (COVID-19). The primary objective of this study was to compare the outcome of COVID-19 patients who received early versus late APP. Methods Post hoc analysis of data collected for a randomized controlled trial (ClinicalTrials.gov NCT04325906). Adult patients with acute hypoxemic respiratory failure secondary to COVID-19 who received APP for at least one hour were included. Early prone positioning was defined as APP initiated within 24 h of high-flow nasal cannula (HFNC) start. Primary outcomes were 28-day mortality and intubation rate. Results We included 125 patients (79 male) with a mean age of 62 years. Of them, 92 (73.6%) received early APP and 33 (26.4%) received late APP. Median time from HFNC initiation to APP was 2.25 (0.8–12.82) vs 36.35 (30.2–75.23) hours in the early and late APP group ( p < 0.0001), respectively. Average APP duration was 5.07 (2.0–9.05) and 3.0 (1.09–5.64) hours per day in early and late APP group ( p < 0.0001), respectively. The early APP group had lower mortality compared to the late APP group (26% vs 45%, p = 0.039), but no difference was found in intubation rate. Advanced age (OR 1.12 [95% CI 1.0–1.95], p = 0.001), intubation (OR 10.65 [95% CI 2.77–40.91], p = 0.001), longer time to initiate APP (OR 1.02 [95% CI 1.0–1.04], p = 0.047) and hydrocortisone use (OR 6.2 [95% CI 1.23–31.1], p = 0.027) were associated with increased mortality. Conclusions Early initiation (< 24 h of HFNC use) of APP in acute hypoxemic respiratory failure secondary to COVID-19 improves 28-day survival. Trial registration ClinicalTrials.gov NCT04325906.

Abstract:Background/purpose: Acute respiratory distress syndrome (ARDS) is a serious disorder of intensive care unit patients. We evaluated the safety of continuous prone position ventilation (PRONE) and its effects on oxygenation and plasma cytokine concentrations in patients with ARDS caused by severe community-acquired pneumonia (CAP). Methods: This was a prospective observational clinical study conducted in a respiratory intensive care unit of a 1200-bed medical center in central Taiwan. Twenty-two patients with severe CAP and ARDS were included. They were treated by traditional supine ventilation (SUPINE, n = 11) or PRONE (n = 11) if they met the criteria for ARDS. Patients in the PRONE group were ventilated in prone position continuously for at least 72 hours. Plasma cytokines were collected and analyzed at baseline, 24 hours and 72 hours after enrolment. Serial PaO2/FiO2 and complications were evaluated. Results: Complications associated with PRONE were minor and self-limited. PRONE had higher PaO2/FiO2 ratio than SUPINE did at 48 hours after enrolment. The levels of plasma IL-6 concentration declined significantly with time in the PRONE group (p = 0.011). The levels of plasma IL-6 concentration at enrolment, 24 hours and 72 hours after enrolment also predicted the 14th day mortality of all patients. Conclusion: PRONE was a safe and effective maneuver for improving oxygenation in patients with severe CAP and ARDS. PRONE also influenced IL-6 expression in patients with severe CAP.
