Abstract:The U-shaped association between health outcomes and ambient temperatures has been extensively investigated. However, such analyses cannot fully estimate the mortality burden of climate change because the features of the association (e.g., minimum mortality temperature) vary with human adaptation; thus, they are not generalizable to different locations. In this study, we assumed that humans could adapt to regular temperature variations; and thus examined the all-cause mortality attributable to temperature anomaly (TA), an indicator widely utilized in climate science to measure irregular temperature fluctuations, across 115 cities in the United States (US). We first used quasi-Poisson regressions to obtain the city-specific TA-mortality associations, then used meta-regression to pool these city-specific estimates. Finally, we calculated the number of TA-related deaths using the uniform pooled association, then compared it to the estimates from city-specific associations, which had been controlled for adaptation. Meta-regression showed a U-shaped TA-mortality association, centered at a TA near 0. According to the pooled association, 0.579 % (95 % confidence interval [CI]: 0.465–0.681 %), 0.394 % (95 % CI: 0.332–0.451 %), and 0.185 % (95 % CI: 0.107–0.254 %) of all-cause deaths were attributable to all anomalous temperatures (TA ≠ 0), anomalous heat (TA > 0), and anomalous cold (TA < 0), respectively. At the city level, heat-related deaths estimated from the pooled association were in good agreement with heat-related deaths estimated from the city-specific associations (R2 = 0.84). However, the cold-related deaths estimated from the two methods showed a weaker correlation (R2 = 0.07). Our findings suggest that TA constitutes a generalizable indicator that can uniformly evaluate deaths attributable to anomalous heat in distinct geographical locations.

Abstract:This study utilizes China's records of non-accidental mortality along with twenty-five simulations from the NASA Earth Exchange Global Daily Downscaled Projections to evaluate forthcoming heat stress and heat-related mortality across China across four distinct scenarios (SSP1-2.6, SSP2-4.5, SSP3-7.0, and SSP5-8.5). The findings demonstrate a projected escalation in the heat stress index (HSI) throughout China from 2031 to 2100. The most substantial increments compared to the baseline (1995–2014) are observed under SSP5-8.5, indicating a rise of 7.96°C by the year 2100, while under SSP1-2.6, the increase is relatively modest at 1.54°C. Disparities in HSI growth are evident among different subregions, with South China encountering the most significant elevation, whereas Northwest China exhibits the lowest increment. Projected future temperatures align closely with HSI patterns, while relative humidity is anticipated to decrease across the majority of areas. The study's projections indicate that China's heat-related mortality is poised to surpass present levels over the forthcoming decades, spanning a range from 215% to 380% from 2031 to 2100. Notably, higher emission scenarios correspond to heightened heat-related mortality. Additionally, the investigation delves into the respective contributions of humidity and temperature to shifts in heat-related mortality. At present, humidity exerts a greater impact on fluctuations in heat-related mortality within China and its subregions. However, with the projected increase in emissions and global warming, temperature is expected to assume a dominant role in shaping these outcomes. In summary, this study underscores the anticipated escalation of heat stress and heat-related mortality across China in the future. It highlights the imperative of emission reduction as a means to mitigate these risks and underscores the variances in susceptibility to heat stress across different regions.

Projections of Heatwave-Attributable Mortality under Climate Change and Future Population Scenarios in China

Abstract:Background In China, most previous projections of heat-related mortality have been based on modeling studies using global climate models (GCMs), which can help to elucidate the risks of extreme heat events in a changing cli-mate. However, spatiotemporal changes in the health effects of climate change considering specific regional characteristics remain poorly understood. We aimed to use credible climate and population projections to estimate future heatwave-attributable deaths under different emission scenarios and to explore the drivers underlying these patterns of changes. Methods We derived climate data from a regional climate model driven by three CMIP5 GCM models and calculated future heatwaves in China under Representative Concentration Pathway (RCP) (2).6, RCP4.5, and RCP8.5. The future gridded population data were based on Shared Socioeconomic Pathway 2 assumption with different fertility rates. By applying climate zone-specific exposure-response functions to mortality during heatwave events, we projected the scale of heatwave-attributable deaths under each RCP scenario. We further analyzed the factors driving changes in heatwave-related deaths and main sources of uncertainty using a decomposition method. We compared differences in death burden under the 1.5 degrees C target, which is closely related to achieving carbon neutrality by mid-century. Findings The number of heatwave-related deaths will increase continuously to the mid-century even under RCP2.6 and RCP4.5 scenarios, and will continue increasing throughout the century under RCP8.5. There will be 20,303 deaths caused by heatwaves in 2090 under RCP2.6, 35,025 under RCP4.5, and 72,260 under RCP8.5, with half of all heatwave-related deaths in any scenario concentrated in east and central China. Climate effects are the main driver for the increase in attributable deaths in the near future till 2060, explaining 78% of the total change. Subsequent population decline cannot offset the losses caused by higher incidence of heatwaves and an aging population under RCP8.5. Although health loss under the 1.5 degrees C warming scenario is 1.6-fold higher than the baseline period 1986-2005, limiting the temperature rise to 1.5 degrees C can reduce the annual mortality burden in China by 3,534 deathsin 2090 compared with RCP2.6 scenarios. Interpretation With accelerating climate change and population aging, the effects of future heatwaves on humanhealth in China are likely to increase continuously even under a low emission scenario. Significant health benefitsare expected if the optimistic 1.5 degrees C goal is achieved, suggesting that carbon neutrality by mid-century is a critical tar-get for China's sustainable development. Policymakers need to tighten climate mitigation policies tailored to localconditions while enhancing climate resilience technically and infrastructurally, especially for vulnerable elderly people. Non-optimum Temperature-Related Mortality Burden in China: Addressing the Dual Influences of Climate Change and Urban Heat Islands

Abstract:Under the dual effects of climate change and urban heat islands (UHI), non-optimum temperature-related mortality burdens are complex and uncertain, and are rarely discussed in China. In this study, by applying city-specific exposure-response functions to multiple temperature and population projections under different climate and urbanization scenarios, we comprehensively assessed the non-optimum temperature-related mortality burdens in China from 2000 to 2050. Our results showed that temperature-related deaths will decrease from 1.19 million in 2010 to 1.08–1.17 million in 2050, with the exception of the most populous scenario. Excess deaths attributable to non-optimal temperatures under representative concentration pathway 8.5 (RCP8.5) were 2.35% greater than those under RCP4.5. This indicates that the surge in heat-related deaths caused by climate change will be offset by the reduction in cold-related deaths. As the climate changes, high-risk areas will be confronted with more severe health challenges, which requires health protection resource relocation strategies. Simultaneously, the net effects of UHIs are beneficial in the historical periods, preventing 3493 (95% CI: 22–6964) deaths in 2000. But UHIs will cause an additional 6951 (95% CI: −17,637–31,539, SSP4-RCP4.5) to 17,041 (95% CI: −10,516–44,598, SSP5-RCP8.5) deaths in 2050. The heavier health burden in RCP8.5 than RCP4.5 indicates that a warmer climate aggravates the negative effects of UHIs. Considering the synergistic behavior of climate change and UHIs, UHI mitigation strategies should not be developed without considering climate change. Moreover, the mortality burden exhibited strong spatial variations, with heavy burdens concentrated in the hotspots including Beijing-Tianjin Metropolitan Region, Yangtze River Delta, Chengdu-Chongqing City Group, Guangzhou, Wuhan, Xi'an, Shandong, and Henan. These hotspots should be priority areas for the allocation of the national medical resources to provide effective public health interventions.
Projection of Mortality Burden Attributable to Nonoptimum Temperature with High Spatial Resolution in China

Abstract:The updated climate models provide projections at a fine scale, allowing us to estimate health risks due to future warming after accounting for spatial heterogeneity. Here, we utilized an ensemble of high-resolution (25 km) climate simulations and nationwide mortality data from 306 Chinese cities to estimate death anomalies attributable to future warming. Historical estimation (1986-2014) reveals that about 15.5% [95% empirical confidence interval (eCI):13.1%, 17.6%] of deaths are attributable to nonoptimal temperature, of which heat and cold corresponded to attributable fractions of 4.1% (eCI:2.4%, 5.5%) and 11.4% (eCI:10.7%, 12.1%), respectively. Under three climate scenarios (SSP126, SSP245, and SSP585), the national average temperature was projected to increase by 1.45, 2.57, and 4.98 °C by the 2090s, respectively. The corresponding mortality fractions attributable to heat would be 6.5% (eCI:5.2%, 7.7%), 7.9% (eCI:6.3%, 9.4%), and 11.4% (eCI:9.2%, 13.3%). More than half of the attributable deaths due to future warming would occur in north China and cardiovascular mortality would increase more drastically than respiratory mortality. Our study shows that the increased heat-attributable mortality burden would outweigh the decreased cold-attributable burden even under a moderate climate change scenario across China. The results are helpful for national or local policymakers to better address the challenges of future warming.

Abstract:Abstract Recent studies have reported a variety of health consequences of climate change. However, the vulnerability of individuals and cities to climate change remains to be evaluated. We project the excess cause-, age-, region-, and education-specific mortality attributable to future high temperatures in 161 Chinese districts/counties using 28 global climate models (GCMs) under two representative concentration pathways (RCPs). To assess the influence of population ageing on the projection of future heat-related mortality, we further project the age-specific effect estimates under five shared socioeconomic pathways (SSPs). Heat-related excess mortality is projected to increase from 1.9% (95% eCI: 0.2–3.3%) in the 2010s to 2.4% (0.4–4.1%) in the 2030 s and 5.5% (0.5–9.9%) in the 2090 s under RCP8.5, with corresponding relative changes of 0.5% (0.0–1.2%) and 3.6% (−0.5–7.5%). The projected slopes are steeper in southern, eastern, central and northern China. People with cardiorespiratory diseases, females, the elderly and those with low educational attainment could be more affected. Population ageing amplifies future heat-related excess deaths 2.3- to 5.8-fold under different SSPs, particularly for the northeast region. Our findings can help guide public health responses to ameliorate the risk of climate change.

Abstract:Background Future warming is projected to increase the heat-related mortality burden, especially for vulnerable populations. However, most previous studies focused on non-accidental morbidity or mortality, with far less research on heat-related accidental events. Methods We collected individual accidental death records among all residents in Chinese mainland from June to August during 2013-2019. Accidental deaths were further divided into several subtypes by different causes. We used an individual-level, time-stratified, case-crossover study design to estimate the association between daily mean temperature and accidental deaths, and estimate its variation in seven geo-climatic zones, age (5-64, 65-74, ≥75), and sex (male, female). We then estimated the temperature-related excess accidental deaths under global warming scenarios of 1.5, 2, and 3°C. Findings A total of 711,929 accidental death records were included in our study. We found that higher temperatures were associated with increased risks of deaths from the total accidental events and four main subtypes, including traffic, falls, drowning, and unintentional injuries. We also found that younger individuals (ages 5-64) and males faced a higher risk of heat-related mortality due to total accidents, traffic incidents, and drowning. For future climate scenarios, even under the 1.5°C climate change scenario, 6,939 (95% eCI (empirical Confidence Interval): 6,818-7,067) excess accidental deaths per year are attributed to higher summertime daily temperature over mainland China, and the number of accidental deaths would increase by 16.71% and 33.59% under the 2°C and 3°C climate change scenarios, respectively. For residents living in southern coastal and northwest inland regions, the projected increase in accidental death is higher. Conclusions This nationwide study confirms that higher summer temperatures are linked to an increased risk of accidental deaths. Younger age groups and males face a higher risk. This indicates that current estimates of the health effects of climate change might be underestimated, particularly for younger populations.

Abstract:BackgroundExtreme temperature events have increased in frequency and intensity globally in recent decades, but few studies have considered the additional effects of heat waves. This study evaluated and compared the excess mortality of characteristic populations under different definitions of high temperature in Nanjing, Suzhou, Yancheng in the future.MethodsThis study projected the extreme temperature by global climate models from 2020 to 2050 of three cities under four SSP scenarios and used DLNM to evaluate the relationship between attributable mortality and temperature. This paper combined with future climate change and population change to explore the attributable mortality of future characteristic populations under different definitions of high temperature.ResultsThe health burden on elderly population will be much greater in the future than 22 present, relative to the rest of the population. Under different definitions of high 23 temperature, added effects of heat waves on mortality were higher for females versus 24 males, the elderly versus young residents.ConclusionThe definition of heat wave plays an important role in the relationship 26 between heat wave and mortality. Therefore, the selection of appropriate heat wave 27 definitions is important for designing local heat warning systems.

Abstract:BACKGROUND:Heatwaves have significant adverse effects on human health. The frequency, duration, and intensity of heatwaves are projected to increase dramatically, in the context of global warming. However, there are few comprehensive assessments of the health impact of heatwaves considering different definitions, and their characteristics under climate change scenarios.OBJECTIVE:We aimed to compare future excess mortality related to heatwaves among different definitions under climate change, population, and adaptation scenarios in China and further explore the mortality burden associated with heatwave characteristics.METHODS:Daily data during 2010-2019 were collected in Guangzhou, China. We adopted nine common heatwave definitions and applied quasi-Poisson models to estimate the effects of heatwaves and their characteristics' impact on mortality. We then projected the excess mortality associated with heatwaves and their characteristics concerning climate change, population, and adaptation scenarios.RESULTS:The relative risks of the nine common heatwave definitions ranged from 1.05 (95% CI: 1.01, 1.10) to 1.24 (95% CI: 1.13, 1.35). Heatwave-related excess mortality will consistently increase in the future decades considering multiple heatwave definitions, with more rapidly increasing rates under the Shared Socioeconomic Path5-8.5 and non-adaptability scenarios. Regarding heatwave characteristics, the intensity is the main factor involved in the threat of heatwaves. The increasing trend of characteristic-related mortality burden is similar to that of heatwaves, and the mortality burden caused by the duration of the heatwaves was the largest among all characteristics.CONCLUSIONS:This study provides a comprehensive picture of the impact of heatwaves and their characteristics on public health under various climate change scenarios, population changes, and adaptive assumptions. The results may provide important public health implications for policymakers in planning climate change adaptation and mitigation policies, and implementing specific plans.
Projecting heat-related cardiovascular mortality burden attributable to human-induced climate change in China

Abstract:Background: Cardiovascular disease (CVD) has been found to be particularly vulnerable to climate change and temperature variability. This study aimed to assess the extent to which human-induced climate change contributes to future heat-related CVD burdens. Methods: Daily data on CVD mortality and temperature were collected in 161 Chinese communities from 2007 to 2013. The association between heat and CVD mortality was established using a two-stage time-series design. Under the natural forcing, human-induced, and combined scenarios, we then separately projected excess cause-/age-/region-/education-specific mortality from future high temperature in 2010-2100, assuming no adaptation and population changes. Findings: Under shared socioeconomic pathway with natural forcing scenario (SSP2-4.5-nat), heat-related attributable fraction of CVD deaths decreased slightly from 3.3% [95% empirical confidence interval (eCI): 0.3, 5.8] in the 2010s to 2.8% (95% eCI: 0.1, 5.2) in the 2090s, with relative change of -0.4% (95% eCI: -0.8, 0.0). However, for combined natural and human-induced forcings, this estimate would surge to 8.9% (95% eCI: 1.5, 15.7), 14.4% (95% eCI: 1.5, 25.3), 21.3% (95% eCI: -0.6, 39.4), and 28.7% (95% eCI: -3.3, 48.0) in the 2090s under SSP1-2.6, SSP2-4.5, SSP3-7.0, and SSP5-8.5 scenarios, respectively. When excluding the natural forcing, the number of human-induced heat-related CVD deaths would increase from approximately eight thousand (accounting for 31% of total heat-related CVD deaths) in the 2010s to 33,052 (68%), 63,283 (80%), 101,091 (87%), and 141,948 (90%) in the 2090s under SSP1-2.6, SSP2-4.5, SSP3-7.0, and SSP5-8.5 scenarios, respectively. Individuals with stroke, females, the elderly, people living in rural areas, and those with lower education level would exhibit heightened susceptibility to future high temperature. In addition, Southern and Eastern regions of China were expected to experience a faster increase in heat-related attributable fraction of CVD deaths. Interpretation: Human activities would significantly amplify the future burden of heat-related CVD. Our study findings suggested that active adaptation and mitigation measures towards future warming could yield substantial health benefits for the patients with CVD. Abstract:The main and added effect from heat co-existed in China. ● Both of the main and added effect could increase the mortality risk of population. ● Females, the elderly, the less educated and inland residents were more vulnerable. Increases in ambient temperatures and the frequency of extreme heat events constitute important burdens on global public health. However, evidence on their effects on public health is limited and inconclusive in China. In this study, data on daily deaths recorded in 33 Chinese cities from 2007 to 2013 was used to evaluate the effect of heat on mortality in China. In addition to the definition of a heatwave established by the China Meteorological Administration, we combined four city-specific relative thresholds (90th, 92.5th, 95th, and 97.5th percentiles) of the daily mean temperature during the study period and three durations of ≥ 2, ≥ 3, and ≥ 4 days, from which 13 heatwave definitions were developed. Then, we estimated the main and added effects of heat at the city level using a quasi-Poisson generalized additive model combined with a distributed lag nonlinear model. Next, the estimates for the effects were pooled at the national level using a multivariable meta-analysis. Subgroup analysis was performed according to sex, age, educational attainment, and spatially stratified heterogeneity. The results showed that the mortality risk increased from 22.3% to 37.1% due to the effects of the different heatwave definitions. The added effects were much lower, with the highest increase of 3.9% (95% CI: 1.7%–6.1%) in mortality risk. Females, the elderly, populations with low educational levels, and populations living inland in China were found to be the most vulnerable to the detrimental effects of heat. These findings have important implications for the improvement of early warning systems for heatwaves.

Abstract:Climate change is one of the biggest health threats of the 21st century. Although China is the biggest developing country, with a large population and different climate types, its projections of large-scale heat-related excess mortality remain understudied. In particular, the effects of climate change on aging populations have not been well studied, and may result in significantly underestimation of heat effects. In this study, we took four climate change scenarios of Tier-1 in CMIP6, which were combinations of Shared Socioeconomic Pathways (SSPs) and Representative Concentration Pathways (RCPs). We used the exposure-response functions derived from previous studies combined with baseline age-specific non-accidental mortality rates to project heat-related excess mortality. Then, we employed the Logarithmic Mean Divisia Index (LMDI) method to decompose the impacts of climate change, population growth, and aging on heat-related excess mortality. Finally, we multiplied the heat-related Years of Life Lost ( YLL ) with the Value of a Statistical Life Year ( VSLY ) to quantify the economic burden of premature mortality. We found that the heat-related excess mortality would be concentrated in central China and in the densely populated south-eastern coastal regions. When aging is considered, heat-related excess mortality will become 2.8–6.7 times than that without considering aging in 2081–2100 under different scenarios. The contribution analysis showed that the effect of aging on heat-related deaths would be much higher than that of climate change. Our findings highlighted that aging would lead to a severe increase of heat-related deaths and suggesting that regional-specific policies should be formulated in response to heat-related risks.

Impact of Climate Change on Heat-Related Mortality in Jiangsu Province, China.

Abstract:A warming climate is anticipated to increase the future heat-related total mortality in urban areas. However, little evidence has been reported for cause-specific mortality or nonurban areas. Here we assessed the impact of climate change on heat-related total and cause-specific mortality in both urban and rural counties of Jiangsu Province, China, in the next five decades. To address the potential uncertainty in projecting future heat-related mortality, we applied localized urban- and nonurban-specific exposure response functions, six population projections including a no population change scenario and five Shared Socioeconomic Pathways (SSPs), and 42 temperature projections from 21 global-scale general circulation models and two Representative Concentration Pathways (RCPs). Results showed that projected warmer temperatures in 2016-2040 and 2041-2065 will lead to higher heat-related mortality for total non-accidental, cardiovascular, respiratory, stroke, ischemic heart disease (IHD), and chronic obstructive pulmonary disease (COPD) causes occurring annually during May to September in Jiangsu Province, China. Nonurban residents in Jiangsu will suffer from more excess heat-related cause-specific mortality in 2016-2065 than urban residents. Variations across climate models and RCPs dominated the uncertainty of heat-related mortality estimation whereas population size change only had limited influence. Our findings suggest that targeted climate change mitigation and adaptation measures should be taken in both urban and nonurban areas of Jiangsu Province. Specific public health interventions should be focused on the leading causes of death (stroke, IHD, and COPD), whose health burden will be amplified by a warming climate.

Abstract:In the context of climate change, most of the global regions are facing the threat of high temperature. Influenced by tropical cyclones in the western North Pacific Ocean, high temperatures are more likely to occur in central China, and the economic losses caused by heat are in urgent need of quantification to form the basis for health decisions. In order to study the economic burden of high temperature on the health of Wuhan residents between 2013 and 2019, we employed meta-analysis and the value of statistical life (VSL) approach to calculate the relative risk of high temperature health endpoints, the number of premature deaths, and the corresponding economic losses in Wuhan City, China. The results suggested that the pooled estimates of relative risk of death from high temperature health endpoints was 1.26 [95% confidence interval (CI): 1.15, 1.39]. The average number of premature deaths caused by high temperature was estimated to be 77,369 (95% CI: 48,906–105,198) during 2013–2019, and the induced economic losses were 156.1 billion RMB (95% CI: 92.28–211.40 billion RMB), accounting for 1.81% (95% CI: 1.14–2.45%) of Wuhan's annual GDP in the seven-year period. It can be seen that high temperature drives an increase in the premature deaths, and the influence of high temperature on human health results in an economic burden on the health system and population in Wuhan City. It is necessary for the decision-makers to take measures to reduce the risk of premature death and the proportion of economic loss of residents under the impacts of climate change.

Abstract:Background: Associations between temperature and health outcomes have been documented worldwide. However, few studies have examined the impact of temperature on mortality in China. Aims: To examine the temperature-mortality relationship and estimate the current and future temperature-related mortality in three major Chinese cities (Beijing, Shanghai, and Guangzhou). Methods: We collected daily mortality, weather and air pollution data in the three cities. We used distributed lag nonlinear model to analyze the lag structure of temperature's effect on mortality, and then estimated the current and future excess temperature-related mortality rate. Results: The association between temperature and mortality was U- or J-shaped, with increased death risk for both hot and cold temperatures in these cities. The effect of cold temperature lasted longer than that of hot temperature. Under the future scenarios of temperatures increase by 1?C, 3?C, and 5?C, the estimated temperature-related mortality will increase since the decreases in the cold-related mortality rate will not offset the increases in the heat-related mortality rate. Among the three cities, Guangzhou is predicted to have the largest temperature-related mortality increase: 4% for a 1?C increase, 16% for a 3?C increase, and 35% for a 5?C increase. Conclusions: Our findings may have implication for the climate policy of China because these temperature-related health impacts might be a strong inducement for the Chinese government to act to combat climate change.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Numerous studies have been conducted to project temperature-related mortality under climate change scenarios. However, most of the previous evidence has been limited to the total or non-accidental mortality, resulting in insufficient knowledge on the influence of climate change on different types of disease.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objectives</h3><p>We aimed to project future temperature impact on mortality from 16 causes under multiple climate change models in a coastal city of China.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>We first estimated the baseline exposure-response relationships between daily average temperature and cause-specific mortality during 2009–2018. Then, we acquired downscaled future temperature projections from 28 general circulation models (GCMs) under two Representative Concentration Pathway (RCP4.5 and RCP8.5). Finally, we combined these exposure-response associations with projected temperature to estimate the change in the temperature-related death burden in different future decades in comparison to the 2010 s, assuming no demographic changes and population acclimatization.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>We found a consistently decreasing trend in cold-related mortality but a steep rise in heat-related mortality among 16 causes under climate change scenarios. Compared with the 2010 s, the net change in the fraction of total mortality attributable to temperature are projected to −0.54% (95% eCI: −1.69% to 0.71%) and −0.38% (95% eCI: −2.73% to 2.12%) at the end of the 21st century under RCP4.5 and RCP8.5, respectively. However, the magnitude of future cold and heat effects varied by different causes of death. A net reduction of future temperature-related death burden was observed among 10 out of 15 causes, with estimates ranging from −5.02% (95% eCI: −17.42% to 2.50%) in mental disorders to −1.01% (95% eCI: −5.56% to 3.28%) in chronic lower respiratory disease. Conversely, the rest diseases are projected to experience a potential net increase of temperature-related death burden, with estimates ranging from 0.44% (95% eCI: −4.40% to 6.02%) in ischemic heart disease and 4.80% (95% eCI: −0.04% to 9.84%) in external causes.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Our study indicates that the mortality burden of climate change varied greatly by the mortality categories. Further investigations are warranted to comprehensively understand the impacts of climate change on different types of disease across various regions.</p>

Abstract:Exposure to ambient high temperature has been associated with numerous health outcomes. It remains largely unclear whether the heat effect may vary in the course of the warm season. This study aimed to explore the intraseasonal variation of the association between heat exposure and mortality risk during the warm season in Shandong province between 2013 and 2018. Daily data on mortality and meteorological conditions were collected from each of the 1822 sub-districts in Shandong Province during the 2013–2018 warm seasons (June–September). Conditional logistic regression with a distributed lag non-linear model was used to fit the heat-mortality association over lag 0–10 days from early (June–July) to late (August–September). The extreme heat effect was defined as the cumulative odds ratio (OR) at the 97.5th percentile of the temperature range compared to the minimum mortality temperature (MMT). Stratified analysis was conducted by sex, age-group, educational level, and cause of mortality. A J-shaped association between ambient heat and mortality risk was consistently observed across the warm season, with the MMT located at 22.5 °C. The extreme heat effect (OR) decreased significantly from 3.52 (95%CI: 3.10, 4.00) in the early warm season to 2.85 (95%CI: 2.48, 3.28) in the late warm season ( P = 0.028). Females, elderly aged over 75 years, individuals with junior high school education or lower, and cardiorespiratory patients were more vulnerable to heat exposure compared to other population subgroups, especially in the early warm season. In conclusion, this study observed a J-shaped association between ambient warm temperature and mortality risk, with the effect size of extreme heat decreasing substantially from the early to the late warm season. The findings highlight the importance of developing health promotion strategies to offset the adverse impact of early heat.
