Abstract:Background: The association between ambient temperature and mortality has been well documented worldwide. However, limited data are available on nonfatal health outcomes, such as emergency department visits (EDVs), particularly from China.Objectives: To examine the temperature-EDV association in 12 Chinese cities; and to assess the modification effects by region, gender and age.Methods: Daily meteorological data and non-accidental EDVs were collected during 2011-2014. Poisson regression with distributed lag non-linear model was applied to examine the temperature-lag-EDV association in each city. The effect estimates were pooled using multivariate meta-analysis at the national and regional level. Stratified analyses were performed by gender and age-groups. Sensitivity analyses adjusting for air pollution and relative humidity were conducted.Results: A total of 4,443,127 EDVs were collected from the 12 cities. Both cold and hot temperatures were associated with increased risk of EDVs, with minimum-mortality temperature located at 64th percentile of temperature. The effect of cold temperature appeared on day 2 and persisted until day 30, causing a cumulative relative risk (RR) of 1.80 (1.54, 2.11). The effect of hot temperature appeared immediately and lasted until day 3, with a cumulative RR of 1.15 (1.03,1.29). The effect of temperature on EDVs was similar in male and female but was attenuated with increasing age. The effect of cold temperature on EDVs was greater in southern areas of the country whereas the hot effect was greater in northern cities. The association was robust to a large range of sensitivity analyses.Conclusions: In China, there is a U-shaped association between temperature and risk of EDVs that is independent of air pollution and humidity. The temperature-EDV association varies with latitude and age-groups but is not affected by gender. Forecasting models for hospital emergency departments may be improved if temperature is included as an independent predictor. (C) 2017 Elsevier Ltd. All rights reserved.
Dynamic Total Order Protocol Replacement

Abstract:The dynamic total order protocol replacement mechanism proposed in this paper allows an application to change, in run-time, the total order broadcast protocol it is using. In a conventional setting, without such a replacement mechanism, the application should be stopped, reconfigured and then restarted, in order to be adapted to a changing environment. The problem of this stop-and-restart approach is that it stops the normal operation of the application, reducing its availability. A dynamic replacement mechanism allows the application to switch from the current total order protocol it is using to a different one that performs better under the new operation conditions. Moreover, the switch should be performed in a transparent manner, from both the application and its users. This paper presents a software architecture for dynamically switching total order protocols, including the switching algorithm, its correctness justification and an experimental evaluation of its performance.

Abstract:BACKGROUND: There are few multi-city studies on the association between temperature and mortality in basin climates. This study was based on the Sichuan Basin in southwest China to assess the association of basin temperature with non-accidental mortality in the population and with the temperature-related mortality burden.METHODS: Daily mortality data, meteorological and air pollution data were collected for four cities in the Sichuan Basin of southwest China. We used a two-stage time-series analysis to quantify the association between temperature and non-accidental mortality in each city, and a multivariate meta-analysis was performed to obtain the overall cumulative risk. The attributable fractions (AFs) were calculated to access the mortality burden attributable to non-optimal temperature. Additionally, we performed a stratified analyses by gender, age group, education level, and marital status.RESULTS: A total of 751,930 non-accidental deaths were collected in our study. Overall, 10.16% of non-accidental deaths could be attributed to non-optimal temperatures. A majority of temperature-related non-accidental deaths were caused by low temperature, accounting for 9.10% (95% eCI: 5.50%, 12.19%), and heat effects accounted for only 1.06% (95% eCI: 0.76%, 1.33%). The mortality burden attributable to non-optimal temperatures was higher among those under 65 years old, females, those with a low education level, and those with an alternative marriage status.CONCLUSIONS: Our study suggested that a significant association between non-optimal temperature and non-accidental mortality. Those under 65 years old, females, and those with a low educational level or alternative marriage status had the highest attributable burden.

Abstract:BACKGROUND: Previous studies examining temperature-mortality associations in China focused on a single city or a small number of cities. A multi-city study covering different climatic zones is necessary to better understand regional differences in temperature risk on mortality in China.METHODS: Sixty-six communities from 7 regions across China were included in this study. We first used a Distributed Lag Non-linear Model (DLNM) to estimate community-specific effects of temperature on non-accidental mortality during 2006-2011. A multivariate meta-analysis was then applied to pool the estimates of community-specific effects.RESULTS: A U-shaped curve was observed between temperature and mortality at the national level in China, indicating both low and high temperatures were associated with increased mortality risk. The overall threshold was at about the 75th percentile of the pooled temperature distribution. The relative risk was 1.61 (95% CI: 1.48-1.74) for extremely cold temperature (1st percentile of temperature), and 1.21 (95% CI: 1.10-1.34) for extreme hot temperature (99th percentile of temperature) at lag0-21 days. The temperature-mortality relationship is different for different regions. Compared with north China, south China had a higher minimum mortality temperature (MMT), and there was a larger cold effect in the more southern parts of China and a more pronounced hot effect in more northern parts.CONCLUSIONS: Both cold and hot temperatures increase mortality risk in China, and the relationship varies geographically. Our findings suggest that public health policies for climate change adaptation should be tailored to the local climate conditions.

Abstract:Background: Temperature-related mortality risks have mostly been studied in urban areas, with limited evidence for urban–rural differences in the temperature impacts on health outcomes. Objectives: We investigated whether temperature–mortality relationships vary between urban and rural counties in China. Methods: We collected daily data on 1 km gridded temperature and mortality in 89 counties of Zhejiang Province, China, for 2009 and 2015. We first performed a two-stage analysis to estimate the temperature effects on mortality in urban and rural counties. Second, we performed meta-regression to investigate the modifying effect of the urbanization level. Stratified analyses were performed by all-cause, nonaccidental (stratified by age and sex), cardiopulmonary, cardiovascular, and respiratory mortality. We also calculated the fraction of mortality and number of deaths attributable to nonoptimum temperatures associated with both cold and heat components. The potential sources of the urban–rural differences were explored using meta-regression with county-level characteristics. Results: Increased mortality risks were associated with low and high temperatures in both rural and urban areas, but rural counties had higher relative risks (RRs), attributable fractions of mortality, and attributable death counts than urban counties. The urban–rural disparity was apparent for cold (first percentile relative to minimum mortality temperature), with an RR of 1.47 [95% confidence interval (CI): 1.32, 1.62] associated with all-cause mortality for urban counties, and 1.98 (95% CI: 1.87, 2.10) for rural counties. Among the potential sources of the urban–rural disparity are age structure, education, GDP, health care services, air conditioners, and occupation types. Conclusions: Rural residents are more sensitive to both cold and hot temperatures than urban residents in Zhejiang Province, China, particularly the elderly. The findings suggest past studies using exposure–response functions derived from urban areas may underestimate the mortality burden for the population as a whole. The public health agencies aimed at controlling temperature-related mortality should develop area-specific strategies, such as to reduce the urban–rural gaps in access to health care and awareness of risk prevention. Drivers of associations between daytime-nighttime compound temperature extremes and mortality in China

Abstract:Background: Temperature extremes are anticipated to become more frequent and more intense under the context of climate change. While current evidence on health effects of compound extreme temperature event is scarce. Methods: This nationwide cross-sectional study collected daily data on weather and mortality for 161 Chinese districts/counties during 2007-2013. A quasi-Poisson generalized linear model was first applied to assess effects of daytime-only, nighttime-only and compound daytime-nighttime heat wave (and cold spell) on cause-specific mortality. Then a random-effect meta-analysis was used to produce pooled estimates at national level. Stratification analyses were performed by relative humidity, individual and regional characteristics. Results: Here we show that mortality risks of compound daytime-nighttime temperature extremes are much higher than those occurring only in the daytime or nighttime. Humid weather further exaggerates the mortality risk during heat waves, while dry air enhances the risk during cold weather. People who are elderly, illiterate, and those with ischemic heart disease and respiratory disease are particularly vulnerable to extreme temperature. At the community-level, population size, urbanization rate, proportion of elderly and PM2.5 are positively associated with increased risks associated with heat waves. Temperature, humidity and normalized difference vegetation index are positively associated with the effects of cold weather, with an opposite trend for latitude and diurnal temperature range. Conclusions: This nationwide study highlights the importance of incorporating compound daytime-nighttime extreme temperature events and humid conditions into early warning systems and urban design/planning.
The Temporal Variation in the Effects of Extreme Temperature on Respiratory Mortality: Evidence from 136 Cities in China, 2006–2019

Abstract:Background: In the context of climate change and urbanization, the temporal variation of the adverse health effect of extreme temperature has attracted increasing attention. Methods: The meteorological data and the daily death records of mortality from respiratory diseases of 136 Chinese cities were from 2006 to 2019. Heat wave and cold spell were selected as the indicator events of extreme high temperature and extreme low temperature, respectively. The generalized linear model and time -varying distributed lag model were used to perform a two -stage time -series analysis to evaluate the temporal variation of the mortality risk associated with extreme temperature in the total population, sub -populations (sex- and agespecific) and different regions (climatic zone and relative humidity level). Results: During the study period, relative risk ( RR ) of respiratory mortality associated with heat wave decreased from 1.22 (95 % CI : 1.07 -1.39) to 1.13 (95 % CI : 1.01 -1.26) in the total population, and RR of respiratory mortality associated with cold spell decreased from 1.30 (95 % CI : 1.14 -1.49) to 1.17 (95 % CI : 1.08 -1.26). The impact of heat wave reduced in the males ( P = 0.044) and in the females as with cold spell ( P < 0.001). The respiratory mortality risk of people over 65 associated with cold spell decreased (P = 0.040 for people aged 65 -74 and P < 0.001 for people over 75). The effect of cold spell reduced in cities from tropical or arid zone ( P = 0.035). The effects of both heat wave and cold spell decreased in cities with the relative humidity in the first quartile ( P = 0.046 and 0.010, respectively). Conclusion: The impact of heat wave on mortality of respiratory diseases decreased mainly in males and cities with the lowest relative humidity, while the impact of cold spell reduced in females, people over 65 and tropical and arid zone, suggesting adaptation to extreme temperature of Chinese residents to some extent.
Individual-level and community-level effect modifiers of the temperature-mortality relationship in 66 Chinese communities

Abstract:Objectives: To examine the modification of temperature-mortality association by factors at the individual and community levels. Design and methods: This study investigated this issue using a national database comprising daily data of 66 Chinese communities for 2006-2011. A 'threshold-natural cubic spline' distributed lag non-linear model was utilised to estimate the mortality effects of daily mean temperature, and then examined the modification of the relationship by individual factors (age, sex, education level, place of death and cause of death) using a meta-analysis approach and community-level factors (annual temperature, population density, sex ratio, percentage of older population, health access, household income and latitude) using a meta-regression method. Results: We found significant effects of high and low temperatures on mortality in China. The pooled excess mortality risk was 1.04% (95% CI 0.90% to 1.18%) for a 1°C temperature decrease below the minimum mortality temperature (MMT), and 3.44% (95% CI 3.00% to 3.88%) for a 1°C temperature increase above MMT. At the individual level, age and place of death were found to be significant modifiers of cold effect, while age, sex, place of death, cause of death and education level were effect modifiers of heat effect. At the community level, communities with lower socioeconomic status and higher annual temperature were generally more vulnerable to the mortality effects of high and low temperatures. Conclusions: This study identifies susceptibility based on both individual-level and community-level effect modifiers; more attention should be given to these vulnerable individuals and communities to reduce adverse health effects of extreme temperatures.
Hourly temperature variability and mortality in 31 major Chinese cities: Effect modification by individual characteristics, season and temperature zone

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>In the context of ongoing climate change, temperature variability (TV) has been considered as an important trigger of death. However, evidence of association between mortality and hourly temperature variability (HTV) is scarce at the multi-city level, and the time window of health effects of HTV is lack of investigation. This study aims at quantifying the mortality risk and burden of HTV and exploring subpopulations susceptible to HTV from a large-scale multi-city perspective.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Data on daily number of deaths and meteorology were collected for 31 Chinese major cities during 2007–2013. HTV was calculated as the standard deviation of hourly temperature within a few days. The optimal exposure period of HTV was chosen according to multiple scientific criteria. A quasi-Poisson regression combined with distributed lag nonlinear model was used to assess the city-specific HTV-mortality associations. Then, meta-analysis was further applied to pool city-specific effect estimates. Finally, we calculated the fraction of mortality attributable to HTV. Stratification analyses were conducted by individual characteristics (i.e. age, sex, and educational attainment), season, and region.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>HTV calculated in a relatively long-time window like 18 d (HTV<sub>0–17</sub>) could capture the impact of HTV adequately. Per 1 °C raise of HTV<sub>0–17</sub> associated with 1.38% (95%CI: 0.77, 1.99) increase of non-accidental mortality. During the study period, 5.47% (95%CI: 1.06, 9.64) of non-accidental mortality could be attributed to HTV. The females, the elderly, and individuals with low education level were more susceptible to HTV than their counterparts, respectively. Moreover, a stronger HTV-mortality association was observed in individuals who live in warmer season and temperature zone.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>HTV is associated with a considerable mortality burden, which may be modified by season, geographic and individual-level factors. Our findings highlight the practical importance of establishing early warning systems and promoting health education to mitigate the impacts of temperature variability.</p>

Abstract:<b>What is already known about this topic?</b> High temperature is a well-recognized public health threat and may increase mortality risks, especially mortality risks involving diseases of the circulatory system. <b>What is added by this report?</b> Using a six-year time series analysis, the differences of daily mean, maximum, minimum temperature were explored in assessing the health effects of high temperatures in nationwide and at climatic-zone level, and population groups susceptible to high temperatures were identified. <b>What are the implications for public health practice?</b> This study suggests that the daily mean temperature is the optimal indicator for high temperature exposure in heat-related health risk assessments and early warnings. The policy measures of heat-related public health protection should be made considering regional distribution, sensitive diseases, and vulnerable populations.

Abstract:Global climate change increased air temperature variability and enhanced the frequency and intensity of extreme weather events, such as heat waves and cold spells with adverse impacts on public health. In this study, we examined the relationships of the daily air temperature with mortality in Shanghai in 2003, a record hot year. We found V-shaped associations between cause-specific mortality and daily air temperature. The temperature-mortality relationship well manifests in three temperature measures, but with varied temperature thresholds for different age groups and mortality categories. Two heat waves and one cold spell were identified in 2003 and brought out excess mortality. The first heat wave lasting for 19 days had a significant impact on total non-accidental, cardiovascular and respiratory deaths compared to the corresponding reference period. The second heat wave lasting for 14 days have resulted in excess mortality in three categories of mortality but without statistical significance. The cold spell lasting for 7 days only had a significant impact on total non-accidental and cardiovascular mortality. We also found the elderly are more sensitive to temperature variation. Our results suggest that air temperature is a significant factor influencing human mortality, particularly for the elderly.

Temperature-related Mortality in 17 Large Chinese Cities: How Heat and Cold Affect Mortality in China.

Abstract:Few multicity studies have been conducted to investigate the acute health effects of cold and hot temperatures in China. We aimed to examine the relationship between temperature and daily mortality in 17 large Chinese cities. We first calculated city-specific effect of temperature using time-series regression models combined with distributed lag nonlinear models; then we pooled the city-specific estimates with the Bayesian hierarchical models. The cold effects lasted longer than the hot effects. For the cold effects, a 1 degrees C decrease from the 25th to 1st percentiles of temperature over lags 0-14 days was associated with increases of 1.69% [95% posterior intervals (PI): 1.01%, 2.36%], 2.49% (95% PI: 1.53%, 3.46%) and 1.60% (95% PI: 0.32%, 2.87%) in total, cardiovascular and respiratory mortality, respectively. For the hot effects, a 1 degrees C increase from the 75th to 99th percentiles of temperature was associated with corresponding increases of 2.83% (95% PI: 1.42%, 4.24%), 3.02% (95% PI: 1.33%, 4.71%) and 4.64% (95% PI: 1.96%, 7.31%). The latitudes, number of air conditioning per household and disposable income per capita were significant modifiers for cold effects; the proportion of the elderly was a significant modifier for hot effects. This largest epidemiological study of temperature to date in China suggested that both cold and hot temperatures were associated with increased mortality. Abstract:The main and added effect from heat co-existed in China. ● Both of the main and added effect could increase the mortality risk of population. ● Females, the elderly, the less educated and inland residents were more vulnerable. Increases in ambient temperatures and the frequency of extreme heat events constitute important burdens on global public health. However, evidence on their effects on public health is limited and inconclusive in China. In this study, data on daily deaths recorded in 33 Chinese cities from 2007 to 2013 was used to evaluate the effect of heat on mortality in China. In addition to the definition of a heatwave established by the China Meteorological Administration, we combined four city-specific relative thresholds (90th, 92.5th, 95th, and 97.5th percentiles) of the daily mean temperature during the study period and three durations of ≥ 2, ≥ 3, and ≥ 4 days, from which 13 heatwave definitions were developed. Then, we estimated the main and added effects of heat at the city level using a quasi-Poisson generalized additive model combined with a distributed lag nonlinear model. Next, the estimates for the effects were pooled at the national level using a multivariable meta-analysis. Subgroup analysis was performed according to sex, age, educational attainment, and spatially stratified heterogeneity. The results showed that the mortality risk increased from 22.3% to 37.1% due to the effects of the different heatwave definitions. The added effects were much lower, with the highest increase of 3.9% (95% CI: 1.7%–6.1%) in mortality risk. Females, the elderly, populations with low educational levels, and populations living inland in China were found to be the most vulnerable to the detrimental effects of heat. These findings have important implications for the improvement of early warning systems for heatwaves.

Abstract:Chronic respiratory diseases cause huge death burden worldwide, especially in developing countries. The influence of ambient temperature on respiratory mortality has attracted growing concerns in the background of climate change. Daily cause-specific respiratory deaths and meteorological variables were collected from 16 Chinese cities during 2007–2013. We first applied the distributed lag non-linear model for each city to calculate the percentage change in respiratory mortality risk with a 1 °C change in moderate and extreme temperature range. Then, random-effect meta-analytical approach was used to produce the pooled effects. Individual-level and city-level effect modifications were separately assessed by the stratified analysis and mixed effects meta-regression model. Both cold and heat effects were associated with respiratory mortality. The pooled excess risk was 4.40% (95%CI: 2.75–6.08%), 2.23% (1.19–3.28%), 2.64% (1.81–3.47%), and 4.21% (3.10–5.33%) for 1 °C change among extreme cold, moderate cold, moderate heat, and extreme heat, respectively. Positive heat and cold effects were found for COPD, asthma, and pneumonia, among which asthma was most vulnerable to extreme cold temperatures, with the effect estimate of 6.52% (2.73–10.46%). The heat effects were much higher in females than males. Slightly higher risks were also observed in the elderly during the heat, and those with lower education level during the heat and extreme cold. City-level modifiers included green coverage rates and diurnal temperature range. Ambient heat and cold temperatures are positively associated with respiratory mortality in China. Individual and city-specific effect modifiers should be taken into consideration in policy-making.
All-cause Mortality Attributable to Long-Term Changes in Mean Temperature and Diurnal Temperature Variation in China: a Nationwide Quasi-Experimental Study

Abstract:Previous studies have demonstrated an association between short-term exposure to ambient temperature and mortality. However, the long-term effects of elevated temperature and temperature variability on mortality have remained somewhat elusive in epidemiological studies. We conducted a comprehensive epidemiological study utilizing Chinese population census data from 2000 and 2010. Census-derived demographic and socioeconomic factors were paired with temperature data from the European Re-Analysis Land Dataset across 2823 counties. We employed a difference-in-difference approach to quantitatively examine the relationship between all-cause mortality and annual exposure to mean temperature and diurnal temperature range (DTR). Additionally, we evaluated the potential effects of socioeconomic and environmental covariate modifications on this relationship and calculated the attributable mortality. Lastly, we projected excess deaths attributable to annual temperature exposure under various shared socioeconomic pathways (SSPs, e.g. SSP126, SSP370, and SSP585). For each 1 °C rise in annual mean temperature and DTR, the mortality risk could increase by 6.12% (95% CI: 0.84%, 11.69%) and 7.72% (95% CI: 3.75%, 11.84%), respectively. Counties with high labor-force ratios and high NO _2 and O _3 concentrations appeared to be sensitive to the annual mean temperature and DTR. Climate warming from 2000 to 2010 may have resulted in 5.85 and 14.46 additional deaths per 10 000 people attributable to changes in annual mean temperature and DTR, respectively. The excess mortality related to changes in annual mean temperature and DTR is expected to increase in the future, with special attention warranted for long-term temperature changes in Southwest China. Our findings indicate that long-term mean temperature and DTR could significantly impact mortality rates. Given the spatial heterogeneity of increased mortality risk, the formulation of region-specific strategies to tackle climate change is crucial.
High Temperatures and Emergency Department Visits in 18 Sites with Different Climatic Characteristics in China: Risk Assessment and Attributable Fraction Identification.

Abstract:Background: Health impacts of high temperatures on hospital emergency department visits (EDVs) have been less reported, especially from developing countries. Objectives: To investigate high temperature-EDVs relationship in various regions with different climatic characteristics, to explore the regional differences, to identify vulnerable populations, and to provide scientific evidence for climate change adaptation strategies in China. Methods: Daily data on weather, air pollution and EDVs were collected from 18 sites in China from June to August during 2014-2017. A quasi-Poisson generalized additive regression model was applied to examine the high temperature-EDVs relationship in each site. Site-specific risks of EDVs were pooled using a random effect meta-analysis model. Stratified analyses were performed by gender, age-groups, cause-specific EDVs and regions. Attributable fractions of EDVs due to high temperatures were calculated in different regions. Results: 1 degrees C increase in daily mean temperature was associated with 1.07% (95% CI, 0.46-1.67%) increase in EDVs across all study regions. The negative health effects from high temperatures were worse for the people living in southern China, in subtropical monsoon climate zone or in counties, with percentage change of 1.96% (95% CI, 0.92-3.02%), 1.35% (95% CI, 0.95-1.76%) and 1.41% (95% CI, 0.48-2.34%), respectively. People under 18 were more vulnerable to high temperatures. Exposure to high temperatures increased EDVs risks from endocrine, respiratory, and digestive diseases and injury. The attributable fraction due to high temperatures was 8.64% for overall EDVs, 11.70% for the people living in southern China, 10.80% for people living in subtropical monsoon climate zone and 12.65% for the county population. Conclusions: Exposure to high temperatures resulted in extra burden to China's already overloaded hospital emergency departments. More resources are needed to meet increasing demands and effective preventative measurements are warranted to tackle such a challenge. Further studies should pay more attention to both heat and cold-related EDVs risks and socioeconomic cost for better climate change adaptation.

Abstract:Background: Associations between temperature and health outcomes have been documented worldwide. However, few studies have examined the impact of temperature on mortality in China. Aims: To examine the temperature-mortality relationship and estimate the current and future temperature-related mortality in three major Chinese cities (Beijing, Shanghai, and Guangzhou). Methods: We collected daily mortality, weather and air pollution data in the three cities. We used distributed lag nonlinear model to analyze the lag structure of temperature's effect on mortality, and then estimated the current and future excess temperature-related mortality rate. Results: The association between temperature and mortality was U- or J-shaped, with increased death risk for both hot and cold temperatures in these cities. The effect of cold temperature lasted longer than that of hot temperature. Under the future scenarios of temperatures increase by 1?C, 3?C, and 5?C, the estimated temperature-related mortality will increase since the decreases in the cold-related mortality rate will not offset the increases in the heat-related mortality rate. Among the three cities, Guangzhou is predicted to have the largest temperature-related mortality increase: 4% for a 1?C increase, 16% for a 3?C increase, and 35% for a 5?C increase. Conclusions: Our findings may have implication for the climate policy of China because these temperature-related health impacts might be a strong inducement for the Chinese government to act to combat climate change.

Abstract:OBJECTIVE:To explore the impact of the socio-economic factors on the temperature-mortality association in different cities in southern China.METHODS:Daily mortality registration data, meteorological data and air pollution data of the cities as Changsha and Kunming during 2006-2009, and cities as Guangzhou and Zhuhai during 2006-2010, were collected to explore modifying effects, stratified by age, gender, education and place of death, of socio-economic factors on the association between temperature and mortality, by distributed lag non-linear model. The accumulative effect of temperature-mortality were separately analyzed in each city, under the high temperature (0-3 days) and low temperature (0-20 days) situation. The association between temperature and mortality was evaluated by general linear threshold model. The above process was firstly adopted to analyze the impact in single city and then Meta analysis was applied to analyze the impact in several cities by effect-combine.RESULTS:The relationship between temperature and mortality in the four cities showed nonlinearity. The minimum mortality risk was separately 23.5 °C, 20.5 °C, 25.0 °C and 26.0 °C in Changsha, Kunming, Guangzhou and Zhuhai. The results of effect-combine showed that low-temperature (RR = 1.67, 95%CI:1.54-1.80) has a higher gross effect than high-temperature (RR = 1.11, 95%CI:1.01-1.18) on population. With the age increasing, risk of death increased both under high and low temperature situation, and the effect of low temperature was greater (RR = 1.83, 95%CI:1.65-2.04) for the elderly than it of high temperature (RR = 1.17, 95%CI:1.03-1.33). The mortality risk among females (cold and hot effects(95%CI) were 1.75(1.57-1.97) and 1.11(0.99-1.25), respectively)was higher than it among males (cold and hot effects(95%CI) were 1.59(1.45-1.77) and 1.11(1.03-1.19), respectively). Whereas the mortality risk on higher education population was significantly higher (cold and hot effects (95%CI) were 1.89(1.48-2.45)and 1.34(1.19-1.48), respectively) than it on other educated people.CONCLUSION:Age, gender, educational level and place of death showed modifying effects on the association between temperature and mortality. The elderly, women and highly educated people were vulnerable to the temperature influence on mortality.

Abstract:Abstract Background Climate change caused by environmental pollution is the most important one of many environmental health hazards currently faced by human beings. In particular, the extreme temperature is an important risk factor for death from respiratory and circulatory diseases. This study aims to explore the meteorological-health effect and find out the vulnerable individuals of extreme temperature events in a less developed city in western China. Method We collected the meteorological data and data of death caused by respiratory and circulatory diseases in Mianyang City from 2013 to 2019. The nonlinear distributed lag model and the generalized additive models were combined to study the influence of daily average temperature (DAT) on mortality from respiratory and circulatory diseases in different genders, ages. Results The exposure-response curves between DAT and mortality from respiratory and circulatory diseases presented a nonlinear characteristic of the “V” type. Cumulative Relative Risk of 30 days ( CRR 30 ) of deaths from respiratory diseases with 4.48 (2.98, 6.73) was higher than that from circulatory diseases with 2.77 (1.96, 3.92) at extremely low temperature, while there was no obvious difference at extremely high temperature. The health effects of low temperatures on the respiratory system of people of all ages and genders were persistent, while that of high temperatures were acute and short-term. The circulatory systems of people aged < 65 years were more susceptible to acute effects of cold temperatures, while the effects were delayed in females and people aged ≥65 years. Conclusion Both low and high temperatures increased the risk of mortality from respiratory and circulatory diseases. Cold effects seemed to last longer than heat did.
