Abstract:Background: Dental caries is the most common disease in childhood and has an impact on general health status. The topical application of fluoride varnishes has been used for the prevention and control of dental caries due to their high fluoride content, adhesion capacity, and safety. Silver has a varied application in medicine as well as in dentistry due to its anticaries, antimicrobial, and antirheumatic potentials. The introduction of nanosilver fluoride (NSF) was made with advancements in technology to overcome the drawbacks of silver diamine fluoride (SDF). Aim: To compare and evaluate the microhardness and microleakage of NSF varnish and sodium fluoride (NaF) varnish. Materials and methods: An in vitro comparative experimental study was carried out between synthesized NSF and commercially available NaF, with 20 samples in each group. The specimens were sectioned and subjected to microhardness evaluation using Vickers microhardness testing and the dye penetration method to evaluate the microleakage. Results: The average microhardness was found to be 230.7218 VMH for NSF (group I), 198.9841 VMH for NaF (group II), and 91.6120 VMH for group III. These differences were statistically significant when compared with each other (p = 0.002). In 50% of the samples, no dye penetration was seen in the NSF group, compared to the NaF varnish group, where 75% of the samples exhibited penetration onto the varnish interface or the varnish and tooth interface. Conclusion: Nanosilver fluoride proves to be an effective alternative to commercially available topical fluoride agents such as NaF. It has greater microhardness and lower microleakage than NaF and the control teeth. Clinical significance: Nanosilver fluoride varnish can be used as a cost-effective alternative to NaF varnish and SDF. Nano‐hydroxyapatite preparation for the remineralization of primary tooth enamel surface subjected to liquid medication: An observational study

Abstract:Background and Aims: Acid-induced demineralization may be caused by the consumption of liquid medications routinely administered to children. Therefore, different remineralizing agents, such as fluorides and nano-hydroxyapatite, have been added to oral care products to remineralize erosive lesions. This study was conducted to compare the effectiveness of 1% nano-hydroxyapatite suspension and 2% sodium fluoride solution on the surface texture of primary teeth enamel previously exposed to liquid drugs.Methods: Thirty posterior primary teeth were extracted and grouped depending on the remineralizing agent used: (A) nano-hydroxyapatite and (B) sodium fluoride. Groups A and B were subjected to liquid medication in two subgroups: cephalexin (cephalexin monohydrate) and ParAzar (acetaminophen), followed by remineralization with 1% nano-hydroxyapatite suspension and 2% sodium fluoride solution. An atomic force microscope was used to analyze the surface texture of the primary tooth enamel by measuring the roughness and waviness parameters of the tested surfaces. The examination was performed at baseline, 7 days after exposure to drugs, and after exposure to remineralizing agents.Results: A significant difference in enamel surface roughness between nano-hydroxyapatite-treated surfaces and sodium fluoride-treated surfaces was observed. A significant change was observed in the surface waviness of the primary enamel surface of sodium-fluoride-treated teeth that were subjected to cephalexin.Conclusion: Routine use of liquid medications could have a negative impact on primary enamel surface topography, because the primary tooth enamel is less mineralized than permanent tooth enamel. Our results show that compared with a 2% sodium fluoride solution, a 1% nano-hydroxyapatite suspension can be used for remineralizing and restoring defects of the enamel surface of primary teeth following exposure to liquid medication, thus reinforcing dental tissues with higher efficacy.

Abstract:OBJECTIVES: To compare remineralization potential of topical fluoride varnishes with added calcium phosphate-based delivery systems on artificial enamel caries.METHODS: Human enamel specimens (n = 60) were randomly divided into six groups: Group 0: Control (no varnish), Group 1: Duraphat® , Group 2: MI Varnish™ , Group 3: Embrace™ Varnish, Group 4: Enamel Pro® Varnish, and Group 5: Clinpro™ White Varnish. Specimens were immersed in demineralizing solution for 96 h to create artificial caries lesion. Each specimen was then immersed in artificial saliva for 6 h after fluoride varnish application and pH cycling was performed for 8 days. Specimens were evaluated using Knoop surface microhardness (KHN) and transverse microradiography (TMR). One-way anova test was performed to examine the effect of fluoride varnishes on change in percentage of surface hardness recovery (%SHR), change in lesion depth (ΔLD), and change in mineral loss (ΔΔZ) with the significance limit set at 5%.RESULTS: %SHR of enamel following treatment with Group 1 was significantly higher than Group 3 and Group 4. Conversely, ΔLD and ΔΔZ of Group 1 were significantly higher than Group 2, Group 3, and Group 4. No significant differences in %SHR, ΔLD, and ΔΔZ were found between Group 1 and Group 5.CONCLUSIONS: Duraphat® alone achieves significant remineralization of enamel carious lesions. Clinpro™ White Varnish had similar enamel remineralization effect as Duraphat® .

Abstract:Aim: Fluoride-Amorphous Calcium Phosphate and Biomimetic Nano-Hydroxyapatite for Enamel Remineralization; An In-Vitro Study of Surface Microhardness and Composition. Material and methods: Ninety-six extracted human premolars with sound buccal surface were divided using a randomization computer-generating software into four groups; Group I (control) sound untreated enamel, Group II (demineralized) demineralized to create white spot lesions, Group III (biom-n-HA) demineralized and then treated with biomimetic nanohydroxyapatite cream, and Group IV (F-ACP) demineralized and then treated using Fluoride-Amorphous Calcium Phosphate varnish. Each group was divided into two subgroups; subgroup "A" evaluated for mineral content using energy dispersive x-ray spectroscopy (EDX) and for surface microhardness using the Vickers microhardness test and Subgroup "B" evaluated for white spot lesion depth using a polarized light microscope (PLM). Results: The highest microhardness (VHN) was found in the (F-ACP) group (mean=428.61±54.43) and then in the (Biom-n-HA) group (mean=408.11±70.16) followed by the (Control) group (mean=402.13±53.40) with no significant difference between them and finally in the significantly different (Demineralized) group (mean=256.99±45.83). The weight percentage of Ca (30.29±1.04 and 33.44±1.07) and Ca/P ratio (1.87±0.06 and 2.03±0.05) were significantly different between Group III and Group IV respectively. PLM measurements in Group II (198.83μm), Group III (60.17μm), and Group IV (26.33μm) were significantly different. Conclusions: Both the (Biom-n-HA) cream and the (F-ACP) varnish showed promising results for enamel remineralization. The increased enamel surface microhardness was consistent with the mineral content and the changes in the birefringence.

Abstract:Abstract Aim: The purpose of this study was to evaluate the preventive effect of nanosilver fluoride (NSF) on intact enamel surface by using energy dispersive x-ray spectroscopy (EDX-analysis) to detect the superficial fluoride precipitation on the surface of intact human dental enamel. Methodology: In this study, we used energy-dispersive X-ray spectroscopy (EDX) to detect the precipitation of fluoride on the surface of flat polished enamel surface after NSF application. Results: Our data has shown that treatment with NSF for 2 minutes led to significant precipitation of fluoride on enamel surface as compared to untreated samples. The precipitated fluoride was detected 7 days after the varnish application. Conclusion: Nanosilver fluoride varnish application on the surface of sound unaffected enamel can be an effective caries preventive measure. Fluoride precipitation on the surface of enamel and its further incorporation in the crystal lattice will lead to the formation of less soluble fluorapatite crystals that are more resistant to the cariogenic bacterial demineralizing acids.
Comparative Evaluation of Longevity of Fluoride Release from Three Different Fluoride Varnishes: An Observational Study

Abstract:Introduction: The cycles of demineralization and remineralization result in a dynamic process of caries development. Caries are prevented by the shift in the balance from demineralization to remineralization at the tooth-oral fluid interface with the help of salivary fluoride levels (in parts per million). The advantages of fluoride varnish application over other substitutes like dentifrices, mouthrinses, gels, or foams are that varnishes are well tolerated by infants, young children, or children with special healthcare needs and have prolonged therapeutic effects. This study was formulated to evaluate and compare the longevity of fluoride release from different fluoride varnishes, namely Fluor Protector, Enamelast, and Enamel Pro varnish. Aim: To evaluate and compare the longevity of fluoride release from three different fluoride varnishes. Materials and methods: The study samples comprised 72 healthy permanent maxillary anterior teeth. The teeth were divided into four groups, with eighteen teeth in each group. The surfaces of all teeth were then covered by different colored nail varnish according to the respective group, except for a 3 × 3 mm window on the facial (labial) surface of the crown, where the test materials were applied according to their respective group. In group I, Fluor Protector varnish; in group II, Enamelast varnish; and in group III, Enamel Pro varnish was applied. Group IV was the control group; hence, no test material was applied. All specimens were then stored in plastic containers with a pH of 7.2 in artificial saliva at room temperature. The specimens were transferred into new plastic containers after 1 day, containing fresh artificial saliva, and solutions from previous plastic containers were taken for fluoride analysis. This process was repeated sequentially to analyze the amount of fluoride released in ppm from the specimens at the end of 1, 3, and 6 months. Statistical analysis: Analysis was performed on Statistical Package for the Social Sciences (SPSS) software (Windows version 22.0). Results: Conventionally used 1.5% ammonium fluoride varnish, that is, Fluor Protector varnish, showed the least release of fluoride (0.03 ± 0.0 ppm), while resin carrier-based 5% sodium fluoride varnish, that is, Enamelast varnish showed a good amount of fluoride release for 6 months continuously, that is, (0.16 ± 0.06 ppm) at last follow-up. Enamel Pro varnish, which has 5% sodium fluoride with amorphous calcium phosphate (ACP) formula, was found to be the best varnish as it released the maximum amount of fluoride in ppm in artificial saliva for up to 3 months (0.32 ± 0.08) but less than Enamelast varnish only at 6 months follow-up that is 0.09 ± 0.03 ppm. Conclusion: The present study concludes that based on the amount of fluoride released for 6 months duration, Enamel Pro varnish, followed by Enamelast varnish and Fluor Protector varnish, are advisable to apply for caries prevention. Assessment of Remineralization Ability of Different Fluoride Varnishes on Artificial Enamel Lesion of Primary Teeth - A Comparative Study

Abstract:Aim: The current study's aim was to evaluate the potential of various fluoride varnishes to remineralize artificial enamel lesions on primary teeth. Materials and methods: For the study, 90 deciduous molar teeth that were implicated for extraction were taken. A window region with a size of 3 × 3 mm was located in the middle of the tooth's coronal surface. The remaining portion was given a coat of nail polish and given time to dry. A demineralizing solution was used to create the artificial enamel lesion. The samples were randomized into three experimental groups at random, with 30 samples in each group. Group 1: Application of Clinpro White Varnish, Group 2: Application of Duraphat Varnish, and Group 3: Application of MI Varnish. After 14 days, samples from each group were examined under scanning electron microscopy at a magnification of 1000× to determine whether remineralization had occurred. Microhardness was also determined using a microhardness tester. Result: Before the intervention, the mean demineralized enamel lesion in the Clinpro White Varnish group was 134.44 ± 0.04, in the Duraphat Varnish group was 133.76 ± 0.12, and in the MI Varnish group was 133.89 ± 0.08. After intervention, the maximum remineralized area was found in the MI Varnish group (82.74 ± 0.18) followed by the Clinpro White Varnish group (101.43 ± 0.16) and the Duraphat Varnish group (104.11 ± 0.10). After the intervention, there was a statistically significant difference found between the groups. After the intervention, the maximum microhardness mean value improved in the MI Varnish group (257.03 ± 1.09 to 261.18 ± 1.20) followed by Clinpro White Varnish (258.78 ± 1.64 to 260.10 ± 1.22) and Duraphat Varnish group (255.24 ± 1.51 to 258.02 ± 0.89). Conclusion: According to the findings of the current investigation, all of the varnishes utilized in this in vitro experiment are capable of correcting early enamel defects on primary teeth. However, MI Varnish group was very effective compared to the Clinpro White Varnish group and the Duraphat Varnish group.

Abstract:Introduction: Er: YAG laser is contemplated as a preventive dental caries strategy when combined with topical fluoride varnish. Information regarding their combined effect on the enamel of primary teeth is still not established. Objectives: The present study aimed to evaluate the effect of combined treatment by Er: YAG laser irradiation prior to topical fluoride varnish application on the acid resistance of enamel in primary teeth compared to fluoride varnish by elemental analysis. Materials and Methods: Twenty caries-free exfoliated human primary molars were painted with nail varnish, leaving one window 4 x 4 mm on the buccal surface. The samples were assigned to 2 groups (10 each), according to the surface treatment: (G I) 5% sodium fluoride varnish (Enamel Pro® Varnish) only and (G II) Er: YAG laser irradiation followed by sodium fluoride varnish. The baseline value for each specimen was recorded before the test procedure. The enamel specimens underwent pH changes over 14 days. EDX was used to assess the mineral content of the specimens after pH challenge. Results: A significant increase occurred in the mean values of the Ca at%, P at%, and Ca/P ratio in the enamel in both groups compared to the baseline values (p≤0.001). No significant difference was found in the mean percent change (p ≥ 0.05) of the mineral content between the study groups. Conclusions: The combined treatment by laser and fluoride is as efficient as fluoride varnish alone for increasing the acid resistance of enamel in primary teeth.
A Comparison of the Fluoride 'Paint- On' vs Tray Application Techniques for Enamel Remineralisation

Abstract:Background: Fluoride gel treatment is not recommended for children < 6 years old due to its potential toxicity. Hence the aim of this study was to compare the effect of 1.23% acidulated-phosphate fluoride (APF) gel paint-on and the conventional tray application techniques on artificial, deciduous enamel carious lesions embedded on wearable appliances. Methods: In a randomised crossover study, the volunteer children (n = 29) wore mandibular removable appliances containing embedded tooth specimens with artificial carious lesions. The volunteers had 3 different treatment protocols: (I) 0.4 mL non-fluoride (control) gel, (II) 0.4 mL paint-on 1.23% APF gel or (III) 5 mL 1.23% APF gel, 4 minutes tray application. After 1 hour, the appliances were removed and the specimens underwent an in vitro, 14 days of pH-cycling. The mean percentage reduction in fluorescence (ΔF, %) at baseline (ΔF0) and after the pH-cycling (ΔF1) were determined using quantitative light-induced fluorescence-digital analysis. The mean ΔΔF (ΔF1-ΔF0) was calculated to compare the differences between groups. Results: The mean ΔΔF of groups I to III were -1.42 ± 1.49, 1.06 ± 2.11, and 1.12 ± 3.57 and -1.25 ± 1.44, 1.13 ± 1.84 and 1.44 ± 3.62 for the smooth surface and proximal surface lesions, respectively. The mean ΔΔF in the 2 treatment groups were significantly greater compared with the control group (P < .001). There was no significant difference in ΔΔF between the APF gel tray and paint-on groups either in the smooth surfaces, or the proximal surfaces (P = .629 and P = .613, respectively). Conclusion: Our study, for the first time, indicates that the paint-on application of APF gel or the tray application of APF had a similar enamel remineralisation effect. Clinically, this implies that, particularly in younger children, the paint-on application of fluoride is less cumbersome, and possibly more tolerable with a lesser likelihood of fluoride ingestion than the tray application technique. Effect of Laser Irradiance and Fluoride Varnish on Demineralization Around Dental Composite Restorations

Abstract:Introduction: This study aimed to assess the effects of CO2 and erbium-doped yttrium aluminum garnet (Er:YAG) lasers with and without fluoride varnish on demineralization around composite restorations. Methods: This in vitro experimental study evaluated 96 extracted human premolars. After preparation and restoration of class V cavities in the buccal surface of the teeth with composite resin, they were randomly divided into 8 groups of control, CO2 laser (L1), CO2 laser-NaF (L1F), NaF-CO2 laser (FL1), Er:YAG laser (L2), Er:YAG laser-NaF (L2F), NaF-Er:YAG laser (FL2) and NaF (F). The entire surface of the teeth, except for the restored cavity in the buccal surface and 1 mm around the margin, was coated with two layers of nail varnish. The teeth then underwent pH cycling for 10 days (3 hours in demineralizing solution and 21 hours in remineralizing solution) to artificially induce demineralization. The amount of calcium and phosphorous released into the cariogenic solution was quantified using atomic absorption spectroscopy and spectrophotometry. The Vickers hardness tester was used to measure the hardness of the tooth structure adjacent to composite restoration. Data were analyzed using one-way ANOVA and Tukey's test. Results: The four groups of L1F, FL1, FL2 and L2F showed minimum loss of calcium and phosphorous ions, and the mean hardness of FL1 and FL2 groups was higher than that of other groups. Conclusion: The CO2 and Er:YAG lasers alone have no significant effect on the resistance of tooth structure to cariogenic solution. However, they can exert a synergistic effect when used along with NaF varnish. Fluoride varnish applied prior to laser irradiation confers further resistance to the tooth structure and positively affects its hardness.
Comparative efficacy of dentifrice containing sodium monofluorophosphate + calcium glycerophosphate and non-fluoridated dentifrice: A randomized, double-blind, prospective study

Abstract:BACKGROUND: The efficacy of fluoridated dentifrices in caries prevention has been well documented and research into various formulations continues for a more effective dentifrice. This study evaluated the anti-caries and anti-plaque efficacy of a dentifrice containing sodium monofluorophosphate (1000 ppm) and calcium glycerophosphate, and compared it with a non-fluoridated dentifrice.MATERIALS AND METHODS: A total of 595 school children (12-15 years) were divided into test (302 children) and control (293 children) groups. The test group used the dentifrice containing sodium monofluorophosphate (1000 ppm) and calcium glycerophosphate, whereas the control group was given a placebo dentifrice. Oral examination for dental caries and plaque assessment was carried out at the start of the study and the children were followed up semiannually up to 18 months. Data were analyzed using repeated-measure analysis of variance (ANOVA) followed by one-way ANOVA.RESULTS: The values for decayed missing filled teeth (DMFT) increased from baseline to 18 month examination from 4.43±2.03 and 4.67±2.25 (P=0.175) to 5.84±2.29 and 5.13±2.30 (P=0.001) for control and test groups, respectively. Similarly, the increase in decayed missing filled surface (DMFS) values were from 6.42±4.10 and 7.06±4.77 (P=0.082) to 8.64±4.51 and 7.92±5.07 (P=0.095) for test and control groups, respectively. The mean DMFT and DMFS values increased for both the groups; however, the increase was less in test group as compared to control group. The baseline plaque score reduced from 2.94±0.72 and 2.91±0.72 (P=0.679), respectively, for control and test groups to 1.33±0.46 and 0.91±0.38 (P<0.001), respectively, at 18 month examination.CONCLUSION: Results revealed that the test dentifrice was effective in inhibiting the progression of plaque and control of dental caries as compared to the placebo dentifrice.
Comparison of Nanofluoridated Hydroxyapatite of Varying Fluoride Content for Dentin Tubule Occlusion

Abstract:Purpose: To evaluate the viability of a series of nano-fluoridated hydroxyapatite (nano-FHA) formulations of varying fluoride content for the occlusion of exposed dentin tubules, in comparison to nano-hydroxyapatite (nano-HA). Methods: Nano-FHA powders with varying levels of fluoride ion were synthesized to substitute hydroxyl ions (-OH) present in hydroxyapatite (HA). Nano-FHA were defined as 2nFHA,4nFHA,6nFHA, 8nFHA and 10nFHA, with the molar ratio of F- to Ca-2+ of 0.002,0.004,0.006,0.008 and 0.01, respectively. The powders were synthesized using a wet chemistry route, and characterized by transmission electron microscopy, X-ray diffraction, and Fourier transform infrared spectroscopy. The methylthiazol tetrazolium (MTT) assay was used to assess cell viability toward nano-FHA. According to the content of F-, the nano-nFHA and nano-HA powders were divided into six groups, with the artificial saliva as control. They were spread over the dentin surface for 1 minute and repeated three times per day for 7 consecutive days. After washing and brushing with distilled water for an additional 7 days, scanning electron microscopy was used to evaluate the in vitro plugging rate of the tubules and penetrating depth. Results: The 2nFHA, 4nFHA, 6nFHA, 8nFHA and 10nFHA powders were prepared and characterized. The 8nFHA had less crystallinity compared to 2nFHA,4nFHA, and 6nFHA.The MTT assay (from 12 to 120 hours) showed that the cell viability of the L-929 fibroblasts in the 2nFHA, 4nFHA, 6nFHA, 8nFHA, 10nFHA, and nano-HA groups ranged from 80.54 +/- 3.35% to 112.9 +/- 4.8%. Most of the nanoFHA powders successfully occluded dentin tubules. The plugging rate of 8nFHA was significantly higher than that of the 2nFHA, 4nFHA, 6nFHA and nano-HA groups. The penetrating depth of the 8nFHA group into the tubules was also significantly higher than that of the other nano-FHA and nano-HA group.
Influence of hydroxyapatite nanoparticles on the formation of calcium fluoride surface layer on enamel and dentine in vitro

Abstract:Topical application of different fluoride preparations is considered to be the gold standard of oral prophylaxis measures in preventive dentistry. Hydroxyapatite nanoparticles (nano-HAP) as well, have received considerable attention for dental use in the past few decades. The purpose of this in-vitro study was to analyze the interaction between nano-HAP and different fluoride preparations. In order to investigate the possibility to establish, in the presence of nano-HAP, reproducible calcium fluoride surface layers, specimens were visually examined with regard to the surface coverage's structure, morphology, homogeneity and stability. Test series based on enamel and dentine specimens, that were obtained from extracted bovine teeth, were conducted. Thereby, sodium fluoride, olaflur, elmex Fluid (10.000 ppm) and an aqueous nano-HAP solution (5%) served as test products and sterile water as reference. First, single application of nano- HAP and fluoride was tested. After 5 min of incubation in the test solution, the surface coverage was examined by scanning electron microscopy (SEM). Furthermore, samples were determined by energy dispersive X-ray spectroscopy (EDX) to identify the present elements of the surface layer, particularly fluoride. To test the calcium fluoride layer's persistence and stability, samples were exposed to the spray of a dental multifunctional syringe for 20 s using maximum pressure and maximum water supply. In the second application protocol, fluoride and nano-HAP were applied simultaneously and in the third application protocol they were used sequentially. SEM visualisation showed that the simultaneous or sequential addition of nano-HAP led to a distinct change in the surface layer's structure. Agglomerates of various sizes were formed, with obviously different morphology from the calcium fluoride globules, not covering the surface homogeneously and sprayed off with the multifunctional syringe easily. Application of pure fluoride compounds resulted in a more homogeneous calcium fluoride surface layer with higher persistence in comparison to the combination of fluoride and nano-HAP. Interaction between fluoride and nano-HAP clearly could be proved. On enamel as well as dentine surfaces, the combined application of nano-HAP and fluoride has a negative effect on the stability and persistence of the calcium fluoride surface precipitate.

Effect of adding sodium fluoride and nano-hydroxyapatite nanoparticles to the universal adhesive on bond strength and microleakage on caries-affected primary molars

Abstract:Evaluation of micro tensile bond strength (μTBS) and marginal leakage of sodium fluoride (NaF) and nano-hydroxyapatite (n-HA) modified universal adhesives (UAs) bonded using etch-and-rinse (ER) and self-etch (SE) bonding technique to the carious affected dentin (CAD). One hundred and twenty primary molars were prepared for CAD on the occlusal surface. The occlusal CAD surface was flattened and underwent a polishing procedure. The specimens were divided into six groups using a random allocation method based on the UAs applied and the mode of etching used (n = 20) Group A1: UAs (ER), Group B1: UAs (SE), Group A2: UAs (NaF) + ER, Group B2: UAs (NaF) + SE, Group A3: UA (n-HA) + ER and Group B3: UAs (n-HA) + SE. Composite restoration was placed and samples were thermocycled. Microleakage, μTBS, and failure mode assessment were performed using a dye penetration test, universal testing equipment, and stereomicroscope respectively. The μTBS and microleakage results (mean ± SD) were examined using analysis of the variance (ANOVA) and Tukey post hoc tests. Group B1 (UAs + SE) demonstrated the maximum scores of microleakage (25.14 ± 9.12 nm) and minimum recorded value of μTBS (14.16 ± 0.55 MPa). In contrast, Group A3 (UAs (n-HA) + ER) displayed a minimum value of marginal leakage (12.32 ± 6.33 nm) and maximum μTBS scores (19.22 ± 0.92 MPa). The outcomes of the intergroup comparison analysis showed that Group A2 (UAs (NaF) + ER), Group B2 (UAs (NaF) + SE), Group A3 (UA (n-HA) + ER) and Group B3 (UAs (n-HA) + SE) presented comparable outcomes of marginal seal outcomes and μTBS scores (p > 0.05). NaF and n-HA-modified UAs displayed favorable bond strength and minimum marginal leakage to the deciduous affected dentin surface.

Abstract:Gonadotropin releasing‐hormone‐1 (GnRH‐1) is expressed in mouse incisors during development. In this report, we identify (1) cell type(s) that express GnRH‐1 throughout tooth development, (2) the GnRH‐1 receptor, and (3) the role of GnRH‐1/GnRH‐1 receptor signaling in tooth maturation. Results show that GnRH‐1‐positive cells in dental epithelium differentiate and populate multiple tooth structures including ameloblast and papillary layers that are involved in enamel formation and mineralization. The GnRH‐1 receptor was present, and in vitro a GnRH‐1 antagonist attenuated incisor GnRH‐1 cell expression. In vivo, in mice lacking GnRH‐1 (−/−), the incisors were discolored, longer, and more curved compared to wildtype. Elemental analysis of calcium, phosphorus, and iron revealed changes in −/− incisors consistent with GnRH‐1 affecting movement of minerals into the dental matrix. In sum, in tooth development a signal transduction pathway exists for GnRH‐1 via the GnRH‐1 receptor and disruption of such signaling affects incisor growth and biomineralization. Efficacy of nano-hydroxyapatite on caries prevention—a systematic review and meta-analysis

Abstract:Abstract Introduction/objectives The review systematically explored in vivo or in situ studies investigating the efficacy of nano-hydroxyapatite (nHA) to reduce initiation of or to remineralize initial caries lesions. Data Prospective controlled (non-)randomized clinical trials investigating the efficacy of a nHA compared to any other (placebo) treatment or untreated/standard control. Sources Three electronic databases (Central Cochrane, PubMed-MEDLINE, Ovid EMBASE) were screened. Outcomes were, e.g., ICDAS score, laser fluorescence, enamel remineralization rate, mineral loss, and lesion depth. No language or time restrictions were applied. Risk of bias and level of evidence were graded using the Risk of Bias 2.0 tool and GRADE profiler. Study selection/results Five in vivo (and 5 in situ) studies with at least 633 teeth (1031 specimens) being assessed in more than 420 (95) patients were included. No meta-analysis could be performed for in vivo studies due to the high heterogeneity of the study designs and the variety of outcomes. In situ studies indicate that under demineralization conditions, NaF was able to hinder demineralization, whereas nHA did not; simultaneously, nHA did not differ from the fluoride-free control. In contrast, under remineralizing conditions, nHA and NaF show the same remineralizing potential. However, the level of evidence was very low. Furthermore, six studies showed a high risk of bias, and six studies were funded/published by the manufacturers of the tested products. Conclusion The low number of clinical studies, the relatively short follow-up periods, the high risks of bias, and the limiting grade of evidence do not allow for conclusive evidence on the efficacy of nHA. Clinical relevance No conclusive evidence on the efficacy of nHA could be obtained based on the low number of clinical studies, the relatively short follow-up periods, the high risks of bias, the limiting grade of evidence, and study conditions that do not reflect the everyday conditions.

A Scanning Electron Microscope Evaluation of the Efficacy of Different Fluoride-releasing Dental Restorative Materials to Prevent Enamel Demineralization: An In Vitro Study

Abstract:Aim: Aim of the present research was to investigate the effectiveness of various fluoride-releasing dental restorative agents in preventing demineralization of enamel. Materials and methods: Eighty human mandibular permanent molar teeth constituted the study group. All samples were subjected to storage in thymol, after which they were taken out to prepare alike proximal box in each. Inductions of artificial enamel surface lesions were done by placing the teeth in demineralizing solution for 96 hours. Subsequently, all 80 molars were randomly assigned to any of the four groups (i.e., 20 in every individual group) according to the restoration as group A: giomer (composite resin containing surface pre-reacted glass-ionomer fillers), group B: compomer (polyacid-modified composite resin), group C: resin-modified glass-ionomer cement (RMGIC), group D: fluoride-releasing composite. After this, the pH cycling was performed, and the samples were subjected to examination beneath scanning electron microscope (SEM). Results: Higher mean areas of remineralization were noted when RMGIC (96.34 ± 0.06) was used followed by the compomer (109.52 ± 0.17), giomer (118.39 ± 0.82), and the fluoride-releasing composite group (129.27 ± 0.31) in that order. A statistically significant difference was seen amid the investigational groups that utilized different restorative agents (p <0.001). A pairwise evaluation that was performed revealed that except for the giomer group and the compomer group, a statistically significant difference (p <0.001) was found among the experimental groups. Conclusion: This research infers that the RMGIC-treated samples exhibited significantly superior performance in preventing enamel demineralization in comparison to compomer, giomer as well as fluoride-releasing composites. Clinical significance: One among the highly frequently employed anticariogenic materials is fluorides. Owing to this characteristic, they are integrated into numerous restorative substances. Nevertheless, the quantity and speed of fluoride release differ in different agents, which translates to the efficacy of the restorative agent in avoiding demineralization about the restoration.

Abstract:Introduction. Nano-hydroxyapatite gel (NHG) has never been investigated for enamel remineralization. This study evaluated the effects of two concentrations of NHG on remineralization of an artificial carious lesion in comparison with nano-HA toothpaste (NHT) and fluoride varnish (FV). Materials and Methods. Carious lesions were prepared on 100 enamel samples and divided into 5 groups: FV, NHT, 20% NHG, and 30% NHG. One untreated (NT) group was left as control. The hardness of the surface was evaluated before, during, and after remineralization. Microhardness at various phases and the percent recovery of hardness (%HR) were determined and analyzed with ANOVA. Polarized-light micrographs (PLM) were evaluated for depth of the carious lesion. Results. Significantly different remineralization capability was indicated for tested agents ( p < 0.05 ). NHT was significantly capable of remineralization greater than NHG, FV, and NT ( p < 0.05 ). No noticeable difference in %HR between 20% NHG and 30% NHG ( p > 0.05 ) was found. Decreasing in the depth of caries lesion was notified by PLM as applying either NHT or NHG as greater than FV, with no reduction in the depth for NT. Conclusions. Nano-HA both in toothpaste and gel form was capable of remineralization better than fluoride varnish. Comparable remineralization of 20% versus 30% NHG was evidenced. NHG for both concentrations was recommended as a capable remineralizing agent for caries remineralization. Clinical Significance: This study indicated that an application of nano-HA gel is an attractive route to deliver the material and can be more effective and less toxic than conventional formulations and provide its effectiveness directly at the site of action, especially for a noncooperative young child and medicinally intimidated patients who may face with inconvenience in using toothbrush and toothpaste for hygiene control.
