Influence of Different Mouthwashes on the Efficacy of Fluoridated Dentifrices in Prevention of Enamel Erosion: An In Vitro Study

Abstract:Aim: The purpose of the current study was to evaluate the impact of three various mouthwashes on the effectiveness of fluoride dentifrices in preventing enamel erosion. Materials and methods: A total of 120 sound intact human premolar teeth which were extracted for orthodontic treatment were selected for the study. A 3 × 3 mm window section was positioned in the middle of the coronal surface of the tooth in order to define the study area. Each sample was placed in a solution of 1% citric acid (pH 3.5) for 10 minutes in order to produce an eroded surface. All samples were divided into two main groups (60 samples each) as follows: Group A for sodium fluoride dentifrices and group B for stannous fluoride dentifrices, again it is subdivided into: CHX: Chlohex ADS®, EO: Listerine®, CPC: Colgate® Plax (20 samples in each subgroup). After that, samples underwent the pH cycling model for 5 days. Samples were examined for surface loss using a scanning electron microscope. Results: In sodium fluoride dentifrices group, before intervention, the surface loss was 3.12 ± 1.03 in CHX group, 3.08 ± 1.20 in EO group, and 3.09 ± 0.96 in CPC group. After intervention, the less surface loss found with CHX group (2.18 ± 0.84), followed by CPC (2.34 ± 0.74) and EO group (2.46 ± 0.97). In stannous fluoride dentifrices group, before intervention, the surface loss in CHX group was 3.26 ± 1.19, in EO group, it was 3.18 ± 1.31, and in CPC group, it was 3.22 ± 1.06. After intervention, the less surface loss found with CHX: group (1.90 ± 0.54), followed by CPC (2.24 ± 0.28) and EO group (2.38 ± 0.20). Conclusion: The present study concluded that the fluoride dentifrices' preventive effects against tooth surface loss were unaffected by a different mouthwashes with varying compositions and major constituents. In terms of erosion, fluoridated toothpaste containing stannous fluoride was found to provide better surface loss protection than sodium fluoride. Clinical significance: Primary prevention and the eradication of contributing causes are the greatest strategies for preventing erosion. Simultaneously, antibacterial agent in the mouthwashes may help in enhancing the effect of fluoride in the enamel, owing to their high affinity for teeth structures. Therefore, in addition to cause-related treatment, further efforts to reduce tooth tissue loss are also necessary.
Comparative evaluation of remineralization potential of novel bioactive agents on eroded enamel lesions: A single-blinded in vitro study

Abstract:Objective: The escalating prevalence of noncarious tooth wear stands as a critical concern in the backdrop of evolving lifestyles and dietary patterns. Dental erosion, a progressive condition induced by both endogenous and exogenous acidic influences, directly impacts enamel integrity, resulting in surface loss. The contemporary surge in carbonated beverage consumption further exacerbates this erosive milieu, underscoring the urgency for dental practitioners to adopt meticulous treatment strategies. Existing literature underscores a noteworthy 94% reduction in tooth erosion risk for individuals abstaining from sweetened soft beverages, emphasizing the imperative for a well-devised remineralization protocol to counter demineralized surfaces. Methodology: Seventy-three enamel specimens were taken. Forty samples were subjected to pre-operative hardness testing, and five samples were subjected to baseline EDX evaluation followed by grouping of samples (Group 1 = control Group; Group 2 = casein phosphopeptide-amorphous calcium phosphate fluoride [CPP-ACPF] Group; Group 3 = Biomin F Group; and Group 4 = self-assembling peptide [SAP] P-114 Group). A demineralization-remineralization cycle was carried out for 5 days followed by testing through Vickers Microhardness Tester, EDX Evaluation, and Scanning Electron Microscopy (SEM) Imaging. Statistical analysis was performed using one-way analysis of variance followed by intergroup analysis using Tukey's post hoc test with SPSS software 25.0 version. Results: The mean percentage change in microhardness values was 30.05% in Group 1, 24.21% in Group 2, 18.85% in Group 3, and 12.08% in Group 4. The mean Ca/P ratio of samples tested through EDAX was 2.20 at baseline, 1.40 in Group 1 (Control Group), 1.62 in Group 2 (CPP-ACPF), 1.82 in Group 3 (Biomin F), and 2.01 in Group 4 (SAP-P114). Postintervention values were statistically significant from baseline values in both parameters. Conclusion: Curodont Protect exhibits superior efficacy, offering valuable insights for future in vivo studies and clinical applications. The multifaceted evaluation, encompassing microhardness testing, SEM analysis, and EDXS assessment, contributes to a nuanced interpretation of the agents' impact, paving the way for informed decisions in clinical practice and future research endeavors.

Abstract:Aims and Objectives: The aim of this in vitro study was to evaluate the remineralization effect of calcium glycerophosphate (CaGP) in fluoride mouth rinse on permanent enamel eroded by a soft drink. Materials and Methods: Forty sound permanent premolars were embedded in self-curing acrylic resin and immersed in Coca-Cola to create erosive lesions. The teeth were divided into four groups (n = 10): Group I artificial saliva; Group II sodium fluoride; Group III sodium fluoride + sodium monofluorophosphate; and Group IV sodium monofluorophosphate + CaGP. The specimens in the assigned groups underwent pH cycling for ten days. The baseline, after erosion, and after remineralization surface microhardness (SMH) values were determined. The data were analyzed by one-way analysis of variance (ANOVA). The mean SMH value between groups and one-way repeated measures ANOVA for the mean SMH value within each group and Bonferroni’s for multiple comparisons at a 95% confidence level were determined. The average SMH was used and calculated as the percentage recovery of SMH. Results: After being eroded by the cola soft drink, the mean SMH values in all groups were significantly decreased. After remineralization, Group I had the lowest %SMHR. The %SMHR of Groups II, III, and IV were significantly higher than Group I (P < 0.001). However, there were no significant differences among Groups II, III, and IV (P > 0.05). Conclusions: Fluoride mouth rinse with and without CaGP showed similar efficacies in remineralizing eroded permanent enamel.

Three-Dimensional Surface Texture Analysis of Fluoride’s Effect on Enamel Erosion

Abstract:Enamel erosion has become a common clinical finding that often impairs dental esthetics and function. In the current study, we aimed to implement the three-dimensional surface texture analysis (3DST) method to explore the protective effect of fluoride on surface texture prior to erosive conditions. For each of the 50 teeth used in this study, the polished buccal enamel surface was divided into three separate areas: the first area was untouched polished enamel, the two other surfaces were immersed in 0.3% citric acid for 30 s. One was treated with high-concentration (19,000 ppm) sodium fluoride (NaF) solution prior to acid attack, and the other had no treatment prior to acid exposure. Enamel surface texture and step height measurements were obtained using a high-resolution disk scanning confocal microscope, and SEM images were also acquired. Surfaces treated with fluoride showed fewer variations in 3-D surface texture parameters than the eroded surface compared to the control group (p = 0.001). This was in accordance with the SEM descriptive images. The findings indicate that pre-fluoridated enamel areas were less affected by the acid and showed similar features to the untouched enamel. Moreover, a protective effect of the fluoride treatment against irreversible enamel damage was noted as the surface loss (step-height) was significantly reduced (p = 0.03). The study showed that 3DST analysis is a valuable methodology for detecting and quantifying subtle differences between the surfaces. When exploring the combination of all surface texture parameters, it was revealed that the pre-fluoridated eroded enamel surfaces showed considerable similarity to the untouched enamel.

A Scanning Electron Microscope Evaluation of the Efficacy of Different Fluoride-releasing Dental Restorative Materials to Prevent Enamel Demineralization: An In Vitro Study

Abstract:Aim: Aim of the present research was to investigate the effectiveness of various fluoride-releasing dental restorative agents in preventing demineralization of enamel. Materials and methods: Eighty human mandibular permanent molar teeth constituted the study group. All samples were subjected to storage in thymol, after which they were taken out to prepare alike proximal box in each. Inductions of artificial enamel surface lesions were done by placing the teeth in demineralizing solution for 96 hours. Subsequently, all 80 molars were randomly assigned to any of the four groups (i.e., 20 in every individual group) according to the restoration as group A: giomer (composite resin containing surface pre-reacted glass-ionomer fillers), group B: compomer (polyacid-modified composite resin), group C: resin-modified glass-ionomer cement (RMGIC), group D: fluoride-releasing composite. After this, the pH cycling was performed, and the samples were subjected to examination beneath scanning electron microscope (SEM). Results: Higher mean areas of remineralization were noted when RMGIC (96.34 ± 0.06) was used followed by the compomer (109.52 ± 0.17), giomer (118.39 ± 0.82), and the fluoride-releasing composite group (129.27 ± 0.31) in that order. A statistically significant difference was seen amid the investigational groups that utilized different restorative agents (p <0.001). A pairwise evaluation that was performed revealed that except for the giomer group and the compomer group, a statistically significant difference (p <0.001) was found among the experimental groups. Conclusion: This research infers that the RMGIC-treated samples exhibited significantly superior performance in preventing enamel demineralization in comparison to compomer, giomer as well as fluoride-releasing composites. Clinical significance: One among the highly frequently employed anticariogenic materials is fluorides. Owing to this characteristic, they are integrated into numerous restorative substances. Nevertheless, the quantity and speed of fluoride release differ in different agents, which translates to the efficacy of the restorative agent in avoiding demineralization about the restoration.
The Efficiency of Fluoride Bioactive Glasses in Protecting Enamel Surrounding Orthodontic Bracket

Abstract:Objectives: The aim of this study was to evaluate the protective effect of using four different fluoride bioactive enamel sealers against an acidic erosion challenge. Materials and methods: A sample of 50 freshly extracted sound upper premolars had their buccal surface bonded to 50 orthodontic brackets using Transbond PLUS color change adhesive; the first four groups had four compositions of fluoride bioactive glasses based on 37 mol% SiO2, 43.9-53.9 mol% CaO, 6.1 mol% P2O5 and CaF2, and 0-10 mol% of Na2O applied to their surfaces and the fifth group served as control (which was not treated by any bioactive sealer). All specimens were challenged by 1% citric acid for 18 minutes which was stirred by a magnetic stirrer. The enamel surfaces next to the orthodontic brackets were examined by SEM. The Wilcoxon signed-rank test was used to compare the area covered by the fluoride bioactive pastes before/after erosion (p < 0.05). Samples from the layer formed on top of the examined teeth were tested before/after erosion to be examined by the attenuated total reflectance Fourier-transform infrared spectroscopy (FTIR/ATR). Results: The FTIR/ATR test showed that fluoride bioactive pastes' applications resulted in the formation of a hydroxyapatite-rich layer; the SEM analysis showed that the aforementioned layer significantly resisted erosion challenge when compared to the control group (p < 0.05). Conclusions: Fluoride bioactive pastes can efficiently protect the enamel surfaces next to orthodontic brackets from acidic erosion challenges.

Abstract:BACKGROUND: Erosion of the teeth is a chronic irreversible process leading to loss of surface enamel and even the dentin, in turn causing sensitivity and pain. Increased consumption of carbonated beverages remains a major cause for dental erosion. However, many of the so called safe beverages that are consumed may also have sufficiently low pH to cause dental erosion. One of the parameters to measure the dental erosion is estimation of hardness and surface roughness. Thus, this study aims to evaluate the difference in hardness and surface roughness of enamel and cementum using three beverages namely (carbonated drink, lime soda, lime juice) in deciduous and permanent teeth.MATERIAL AND METHODS: Ten permanent and three deciduous teeth samples each were kept in lime juice, lime soda, carbonated beverage and tap water. The VHN using Vickers hardness tester and Ra value using surface profilometer were assessed at baseline, 1 day and 10 days.RESULTS: At the end of 10 days the decrease in hardness of enamel of permanent teeth was maximum for teeth immersed in carbonated beverage followed by lime soda and lime juice. However, in the deciduous teeth it was observed that the VHN drop was maximum at 1 day in relation to teeth immersed in carbonated beverage followed by lime juice and lime soda. The hardness of cementum decreased significantly at the end of ten days both in deciduous as well as permanent teeth.CONCLUSIONS: The present study shows that many of the most commonly used beverages like lime juice and lime soda have a sufficiently low pH to cause erosion of the enamel surface as well as that of cementum of both deciduous and permanent teeth. Though protective mechanisms do exist in the oral cavity to neutralize the acids present in these beverages, continuous usage of these beverages leads to irreversible damage to the tooth structure. Abstract:Objectives This study assesses the application of the focus variation 3D microscopy for the evaluation of dental erosion and fluoride treatment for prevention of enamel erosion in vitro. Methods Human dental enamel disks were treated with Prevident 5000® (PV, n=15) for 1 week and compared with a reference group (PN, n=15) after orange juice erosion in vitro. A focus variation 3D scanning microscope (IFM) and a stylus type profilometer (SSP) were used to evaluate the erosion depths on enamel. 3D topographic images were taken with vertical resolutions of 0.1 and 0.02μm. Scratch marks depths from SSP were measured on IFM images. Measurements were compared between the SSP and IFM and between the two study groups. Results The SSP and IFM measurements of eroded enamel surfaces showed similar trends between the two methods and between the two study groups. The SSP and the IFM measurements were statistically significantly different but correlated with each other. PV group showed consistently lower erosion depth than PN in all profile measures using both SSP and IFM. The stylus tip created scratch marks that were significantly different in depths between the eroded and the reference surfaces in both groups. Conclusions The focus variation 3D microscopy is a powerful tool in evaluating surface topography associated with enamel erosion and in assessing the treatment effects of anti-erosive therapies. Topical treatment with Prevident 5000® significantly increased enamel resistance to erosion by orange juice and should be considered as a treatment choice in patients susceptible to acidic dental erosion.
The Impact of Selected Fluoridated Toothpastes on Dental Erosion in Profilometric Measurement

Abstract:Background: Some fluoridated toothpastes, available commercially, are described to have a protective effect against dental erosion. Objectives: To evaluate the influence of the selected marketed toothpastes on the human enamel exposed to acid beverages. Material and methods: Enamel specimens from extracted human teeth were prepared (n = 40). Specimens were randomly divided into 10 experimental groups, 4 specimens each, which were subjected to acid challenge for 10 min using orange juice (pH 3.79) or Pepsi Cola (pH 2.58) and then immersed for 2 min into a slurry of five marketed toothpastes with distilled water (1 : 3 w/w). The tested toothpastes contained 1450 or 5000 ppm fluoride, CPP-ACP with 900 ppm fluoride, 1450 ppm fluoride with potassium nitrate 5%, all of them as sodium fluoride, and 700 ppm fluoride as amine and sodium fluoride with 3500 ppm SnCl2. Enamel roughness (Ra parameter) by contact profilometer at baseline and after exposure onto soft drinks and slurry was measured. Results: Exposure to both beverages caused a similar increase of enamel surface roughness. After the specimens immersion into slurries of toothpastes with 1450 or 5000 ppm fluoride, 1450 ppm fluoride with potassium nitrate 5% and CPP-ACP with 900 ppm fluoride the significant decrease of Ra values were found, reaching the baseline values. However, toothpaste with 700 ppm fluoride and 3500 ppm SnCl2 did not cause any fall in Ra value, probably due to other mechanism of action. Conclusions: Within the limitation of the study we can conclude that the sodium fluoride toothpastes are able to restore the surface profile of enamel exposed shortly to acidic soft drinks.
In vitro evaluation of fluoride treatment on remineralization of eroded enamel of human primary tooth-A study of microhardness

Abstract:Objective To evaluate in vitro the efficacy of fluorides to promote the reminerization of human primary tooth enamel eroded by a commercial carbonated soft drink.Methods Sixteen retained secondary primary molars were collected at clinic and were processed into enamel slabs.The enamel slabs were first immersed into a carbonated acidic drink for 10min,twice a day for 5 days,then they were randomly divided into 4 groups for further treatments:①Group A treated with artificial saliva as the control group;②Group B treated with 2% sodium fluoride;③Group C treated with 1.23% fluoride gelatin;④Group D treated with saturated calcium phosphate solution with 10 ppm fluoride.The treatments were given 2 min per day for 5 days.The surface microhardness(SMH)of the enamel slabs was determined prior to the erosive challenge(baseline),after the erosive challenge,and after in situ remineralization.One-way ANOVA was used for statistical analyses.Results The SMH of the enamel slabs was decreased significantly after immersion into the carbonated acidic drink.The SMH of the enamel slabs treated with 2% Sodium Fluoride or 1.23% fluoride felatin was significantly increased compared with that of the control(P=0.031 and P=0.017 respectively)while there was no significant difference between these two groups.The SMH of the enamel slabs treated with saturated calcium phosphate solution did not increase significantly compared with that of the control(P>0.05).Conclusion The present microhardness studies showed that in vitro treatment with high fluoride-containing reagents(2% sodium fluoride and 1.23% fluoride gelatin)could significantly promote remineralization of eroded human enamel.However,mineral solution with very low fluoride concentration did not show any significant reminerization effect in this study.
Fluoride-amorphous calcium phosphate and biomimetic nano-hydroxyapatite for enamel remineralization: An in-vitro study of surface microhardness and composition

Abstract:Aim: Fluoride-Amorphous Calcium Phosphate and Biomimetic Nano-Hydroxyapatite for Enamel Remineralization; An In-Vitro Study of Surface Microhardness and Composition. Material and methods: Ninety-six extracted human premolars with sound buccal surface were divided using a randomization computer-generating software into four groups; Group I (control) sound untreated enamel, Group II (demineralized) demineralized to create white spot lesions, Group III (biom-n-HA) demineralized and then treated with biomimetic nanohydroxyapatite cream, and Group IV (F-ACP) demineralized and then treated using Fluoride-Amorphous Calcium Phosphate varnish. Each group was divided into two subgroups; subgroup "A" evaluated for mineral content using energy dispersive x-ray spectroscopy (EDX) and for surface microhardness using the Vickers microhardness test and Subgroup "B" evaluated for white spot lesion depth using a polarized light microscope (PLM). Results: The highest microhardness (VHN) was found in the (F-ACP) group (mean=428.61±54.43) and then in the (Biom-n-HA) group (mean=408.11±70.16) followed by the (Control) group (mean=402.13±53.40) with no significant difference between them and finally in the significantly different (Demineralized) group (mean=256.99±45.83). The weight percentage of Ca (30.29±1.04 and 33.44±1.07) and Ca/P ratio (1.87±0.06 and 2.03±0.05) were significantly different between Group III and Group IV respectively. PLM measurements in Group II (198.83μm), Group III (60.17μm), and Group IV (26.33μm) were significantly different. Conclusions: Both the (Biom-n-HA) cream and the (F-ACP) varnish showed promising results for enamel remineralization. The increased enamel surface microhardness was consistent with the mineral content and the changes in the birefringence.
Effect of cheese and casein phosphopeptide-amorphous calcium phosphate on erosive lesions of primary teeth enamel following exposure to amoxicillin and ibuprofen syrups: An in vitro study

Abstract:Background: The acidic component of liquid medicinal syrups used by pediatric patients may cause erosion and partial demineralization. This study aimed to evaluate the effect of cheese and casein phosphopeptide-amorphous calcium phosphate (CPP-ACP) on erosive lesions of primary teeth enamel following exposure to amoxicillin and ibuprofen syrups. Materials and methods: In this in vitro study, 60 noncarious deciduous molars were used. After measuring the surface microhardness of the samples, they were randomly separated into two groups and immersed in either amoxicillin or ibuprofen for 1 min three times per day. CPP-ACP, cheese, and artificial saliva were then applied to each of the three subgroups (n = 10). After each immersion time, 10 min of therapy was given. Between treatment intervals, the samples were kept in artificial saliva. The microhardness was remeasured after 1 week. Data were analyzed using SPSS software through repeated-measures ANOVA (α = 0.05). Results: All samples' microhardness reduced considerably after immersion in liquid pharmaceuticals (amoxicillin [84.9 kgf/mm2] and ibuprofen [75.1 kgf/mm2]), but increased significantly following exposure to therapeutic solutions. There was no difference between the amoxicillin-cheese and amoxicillin-CPP-ACP subgroups (P = 0.975). A statistically insignificant difference was found between the ibuprofen group and the ibuprofen-CPP-ACP subgroup (P = 0.499). Conclusion: As a result, cheese and CPP-ACP can be utilized to remineralize erosive lesions caused by amoxicillin or ibuprofen exposure.
The ability of 1.23% acidulated phosphate fluoride gel to inhibit simulated endogenous erosion in tooth roots

Abstract:Background: Endogenous dental erosion is that produced by contact of gastric acids with tooth structure. It may affect exposed root cementum/dentine as well as coronal enamel, causing marked loss of mineral. The aim of this study was to determine whether 1.23 per cent acidulated phosphate fluoride gel, if applied to the surface cementum at certain intervals during an erosive acid challenge, could provide any protection against demineralization. Materials and methods: Roots of preserved extracted human teeth were painted with a water and acid resistant varnish, leaving two windows (3x1mm) of exposed dentine. These were placed in a solution containing 0.06MHCl and 2.2mMCaHPO4, which has been shown to simulate gastric acid when it meets the tooth surface. The roots were placed in the erosive solution unprotected (controls), or subject to APF application for four minutes prior to and every 10, 30 or 120 minutes during the erosive challenge. Roots were removed at either 6 or 12 hours, washed thoroughly and cut into 120microm thick sections. Depths of demineralization were measured using an optical graticule under polarized light microscopy. Results: A high level of protection was provided when the roots were coated with APF gel every 10 or 30 minutes. Conclusions: APF gel will partially inhibit endogenous erosion of roots for up to 30 minutes if applied, for example, the night before a morning reflux episode. This should be considered along with other erosion control or reduction procedures for patients suffering from the effects of endogenous erosion.
In vitro effect of fluoride gels supplemented with nano-sized sodium trimetaphosphate on enamel erosive wear

Abstract:Objective To assess the protective effect of fluoride (F) gels supplemented with micrometric or nano-sized sodium trimetaphosphate (TMPmicro and TMPnano, respectively) against enamel erosion in vitro. Methods Bovine enamel blocks (n=140) were selected according to their surface hardness, and randomly divided into seven groups (n=20/group), according to the gels tested: Placebo (without F/TMP), 4,500 μg F/g (4500F), 9,000 μg F/g (9000F), 4500F plus 2.5% TMPnano (2.5% Nano), 4500F plus 5% TMPnano (5% Nano), 4500F plus 5% TMPnano (Micro 5%) and 12,300 μg F/g (Acid gel). Blocks were treated once during one minute with the gels, and submitted to erosive (ERO, n=10/group) or erosive plus abrasive (ERO+ABR, n=10/group) challenges 4 times/day, for 90 seconds for each challenge (under reciprocating agitation), during consecutive 5 days. Blocks were analyzed by profilometry, and by surface (SH) and cross-sectional hardness (∆KHN). Data were submitted to two-way ANOVA, and Fisher's LSD test (p<0.05). Results For ERO, both TMPnano-containing gels promoted enamel wear significantly lower than Placebo and 4500F, reaching levels similar to both positive controls (9000F and acid gel); significantly lower softening was observed for enamel treated with 4500F+5% Micro and 4500F+2.5% Nano. Also, the lowest ∆KHN values were observed for 4500F+2.5% TMPnano among the TMP-containing gels. For ERO+ABR, the lowest enamel wear was achieved by the use of 4500F+5% Nano among all gels, including both positive controls; lower softening was observed for Placebo and 9000F groups. Conclusion The addition of 5% nano-sized TMP to a low-fluoride gel produced superior protective effects for enamel under both challenges conditions, when compared with micrometric TMP, reaching values similar to or superior than both positive controls, respectively for ERO and ERO+ABR. Clinical significance The supplementation of low-F gels with TMP was shown to significantly improve their effects on enamel erosive wear, and the use of nano-sized TMP further enhances this protective action.

Abstract:Objective To evaluate the effectiveness of a dentifrice with 5000ppm fluoride in preventing dental erosion by orange juice in situ in comparison to a control dentifrice with 1450ppm fluoride. Methods This was a double-blind and randomized clinical study with a cross-over design. Sixteen subjects wore an intra-oral appliance containing two enamel disks with an exposed surface of approximately 2mm×5mm. Enamel disks in the study group were treated with a dentifrice with 5000ppm fluoride and in the control group with 1450ppm fluoride. The subjects rinsed with slurries of study dentifrices for one minute before immersing the enamel disks in 250ml orange Juice four times in an 8-h period daily. The treatment procedure was repeated for three 5-day phases for each dentifrice. Enamel erosion was measured after each 5-day treatment phase using a focus-variation 3D scanning microscopy. Medians and inter-quartile ranges (IQR) of mean erosion depth were compared between the groups. Results The mean erosion depths of enamel varied greatly amongst the subjects. Enamel treated with 5000ppm fluoride had less erosion (median 5.7μm, IQR 4.5μm) as compared to the control (median 12.6μm, IQR 12.3μm) after 15 days of fluoride treatment and erosive challenge cycles (p<0.05). Conclusions Enamel treated with 5000ppm fluoride had significantly improved resistance to erosion by orange juice. Periodic application of 5000ppm fluoride may be beneficial in individuals at risk of acidic erosion associated with soft drink consumptions.

Abstract:The aim of this study was to determine the effectiveness of high calcium milk and casein phosphopeptide-amorphous calcium phosphate (CPP-ACP) on enamel erosion caused by chlorinated water. Thirty-six bovine enamel samples without wear or caries 3x4 mm in size were placed in acrylic blocks. All specimens were randomly allocated into 3 groups (n=12/group): CPP-ACP in the form of paste, Anlene concentrated milk and a control (no treatment). All specimens were soaked in chlorinated water (pH =5.0) at room temperature for 72 hours following by soaking in artificial saliva for 30 minutes. Then, microhardness was determined using a microhardness tester. Data were analyzed using a one-way ANOVA and paired t-test. The microhardness value change in the control group was significantly higher than the other groups. No significant differences were seen between the 2 study groups. High calcium milk and CPP-ACP enhanced remineralization of enamel erosion caused by chlorinated water.

Abstract:The aim of this study was to evaluate the erosion-inhibiting effect of two toothpastes on the development of erosion-like lesions, by a confocal laser scanning microscope (CLSM). Forty human enamel blocks were divided into five groups (n = 8), in accordance to evaluate the GC MI Paste Plus and Oral B with stannous fluoride, applied as slurries and associated with toothbrush. Specimens were submitted to an erosion challenge from citric acid (0.5%, pH = 2.8), for 5 min, six times a day, alternating in artificial saliva immersions. Reference group was not exposed to treatment. Part of specimens (Groups 02 and 03) was exposed twice daily just to slurries, for 2 min, therefore specimens from Groups 04 and 05 were also abraded, for 30 s. The enamel surfaces were morphological characterized using CLSM images, with mineral loss being measured using the resulting 3D images referenced to an un-challenged portion of the sample. Step values were compared using the one-way ANOVA test. CLSM was shown to be a viable, noncontact, and simple technique to characterize eroded surfaces. The statistical difference in the step size was significant between the groups (P = 0.001) and using multiple comparisons a statistically significant protective effect of toothpastes was shown when these were applied as slurries. Although groups submitted to tooth brush showed mineral loss similar to reference control group, due to the damages of abrasion associated.

Abstract:The primary objective of this in vitro study was to investigate the erosive potential of enamel under the use of clear aligners (CA), by simulating in vivo conditions experienced by patients who do not remove their CA during the consumption of acidic beverages. In addition, the difference in erosion protection conferred by artificial and human saliva was also evaluated. Sound-extracted human premolars ( n = 20) had half of their surfaces protected with acid-resistant nail polish and were randomly distributed into two experimental groups ( n = 10): teeth immersed in human saliva or artificial saliva. All teeth had half of their lingual surfaces enclosed by a CA device. The erosive challenges consisted of individual immersion of each sample in citrus acid three times a day, intermediated by immersion in human saliva or artificial saliva for 2 h, during ten days of the erosive protocol. The enamel mineral content was analyzed by high-resolution microtomography. The differential mineral concentration profiles were obtained by subtracting the profile of the mineral concentration of the exposed area and enamel under the CA area from the respective sound area (control). In addition, enamel wear and enamel volume loss were measured. Scanning electron microscopy (SEM) was also performed to analyze the enamel surface. Data were analyzed by two-way ANOVA, followed by the Student-Newman-Keuls test. The enamel wear was higher in teeth immersed in artificial saliva, when compared to human saliva ( p < 0.001). The volume loss of the exposed enamel area was lower for tooth immersed in human saliva than in artificial saliva ( p < 0.001), during the acid challenge protocol. The use of CA during acid challenges promoted wear and mineral loss of dental enamel, being these changes more pronounced on the enamel surface under the CA. These results open a new path for the development of further studies adopting clinical protocols that promote more accurate responses in the clinical practice during orthodontic treatment.

Abstract:ABSTRACT Background: White lesions, also known as white spots, are the earliest signs of tooth decay. At this stage, noninvasive preventive treatments, such as fluoride application, can help to slow down or even reverse the progression of white lesions. For decades, fluoride has held the spotlight as the go-to agent in oral preventive care, demonstrating significant remineralizing power. Recent scientific literature reveals a growing interest in alternative products as natural substances that demonstrate potential remineralizing effects on tooth enamel. This in vitro study aimed to evaluate the potential remineralizing effect of calcium sodium phosphosilicate (CSP), ginger, and turmeric. Materials and Methods: This in vitro study was designed to evaluate the remineralizing effects of various materials on the enamel of extracted human teeth. The aim was to compare the effectiveness of these different substances in promoting enamel remineralization. Enamel blocks were prepared and were divided into seven experimental groups: CSP, fluoride toothpaste, nonfluoride toothpaste, turmeric, ginger, fluoride varnish, and distilled water. In this study, sample mineralization is assessed through the quantitative photo-induced fluorescence test before demineralization, after demineralization, and after remineralization. Statistical analysis of the data was conducted using a one-factor analysis of variance test. The significance level was set at P < 0.05. Results: The best results were obtained by groups treated with turmeric ( P < 0.0001), Enamelast fluoride varnish ( P < 0.0001), and NovaMin ( P < 0.01), as compared to other remineralizing treatments. Conclusion: While fluoride has undeniably revolutionized oral preventive care, the exploration of alternative products as natural substances and innovative compounds in recent scientific literature signals a shift in focus toward safer and potentially more diverse options.
Preventive dental erosion with silver diamine fluoride: an in vitro study

Abstract:Objectives To evaluate the erosion preventive effect of 38% silver diamine fluoride (SDF) solution in enamel and dentin of human permanent teeth. Methods Ninety enamel and ninety dentin blocks were prepared from permanent molars and allocated into three groups. Gp-SDF received a one-off application of 38% SDF solution. Gp-SNF received a one-off application of a solution containing 800ppm stannous chloride and 500ppm fluoride. Gp-DW received a one-off application of deionized water. The blocks were submitted to acid challenge at pH 3.2, 2 minutes, 5 times/day for 7 days. All blocks were immersed in human saliva between cycles for one hour. The crystal characteristics, percentage of surface microhardness loss (%SMHL), surface loss, and elemental analysis and surface morphology were examined by X-ray diffraction (XRD), microhardness test, non-contact profilometry, and energy-dispersive X-ray spectroscopy (EDS) and scanning electron microscopy (SEM), respectively. Data of %SMHL and surface loss were analyzed by one-way ANOVA. Results XRD spectra revealed that fluorapatite and silver compounds formed in Gp-SDF, while fluorapatite and stannous compounds formed in Gp-SNF. Gp-DW presented only hydroxyapatite. The median (interquartile range) of %SMHL in Gp-SDF, Gp-SNF and Gp-DW were 27.86(3.66), 43.41(2.45), and 46.40(3.54) in enamel ( p< 0.001), and 14.21(1.57), 27.99(1.95), and 33.18(1.73) in dentin, respectively ( p <0.001). The mean (standard deviation, μm) of surface loss of Gp-SDF, Gp-SNF, and Gp-DW were 2.81(0.59), 4.28(0.67), and 4.63(0.64) in enamel ( p <0.001) and 4.13(0.69), 6.04(0.61), and 7.72(0.66) in dentin, respectively ( p <0.001). SEM images exhibited less enamel corruption and more dentinal tubular occlusion in Gp-SDF compared to Gp-SNF and Gp-DW. EDS analysis showed silver was detected in Gp-SDF while stannous was detected in the dentin block of Gp-SNF. Conclusion 38% SDF yielded superior results in protecting enamel and dentin blocks from dental erosion compared to SNF and DW. Clinical significance Topical application of 38% SDF is effective in preventing dental erosion in human enamel and dentin.
