Abstract:Background Vision Zero is a strategy to eliminate traffic fatalities and to promote equitable mobility options for all road users. Using a nationally representative survey, we aimed to estimate the prevalence of Vision Zero action plans or strategies in the USA. Methods Municipal officials were surveyed in 2021. In this cross-sectional study, we calculated the prevalence of Vision Zero plans or strategies and compared municipalities with adjusted prevalence ratios (PR) to account for region and sociodemographic characteristics. Results Among 1955 municipalities participating in the survey (question-specific response rate: 44.3%), the prevalence of a Vision Zero action plan or strategy was 7.7%; 70.5% responded no and 21.8% don’t know . Prevalence was 4.8% in small municipalities (1000–2499 residents), 20.3% in medium-large municipalities (50 000–124 999 residents; PR=4.1), and 37.8% in large municipalities (≥125 000 residents; PR=7.6). Conclusion The prevalence of Vision Zero plans and strategies across the USA is low. Additional adoption of Vision Zero plans and strategies could help address traffic fatalities.

Abstract:This paper analyses how Vision Zero (VZ) efforts in New York City (NYC) account for equity and social justice implications of road safety work. VZ policy documents, research literature, popular science and opinion articles on road safety work in the city were studied with a prime focus on equity and social justice. Twelve semi-structured interviews with stakeholders involved in road safety and transport planning in the city and at national level were conducted to gain an in-depth understanding of policy design, the adoption process, and the role of equity considerations in the city's road safety work. The results show that major equity and social justice issues arise in the adoption and implementation of VZ. These issues are primarily related to equity and fairness in the distribution of life saving interventions, the socio-economic impacts of road safety strategies, and the nature of community engagement in policy design and implementation. The findings point to a need for VZ practitioners to give due considerations to equity and social justice implications of VZ policies and strategies. Among others, it supports the need for understanding the nature of past equity and social justice problems in road safety and transport planning in the VZ policy design process. Moreover, the findings suggest the need for empirical studies on the socio-economic implications of VZ strategies and interventions.

Abstract:SUMMARY Prior published work from our laboratory concluded that there was a need for appropriate metabolic activity of iodine in breast tissue for normal growth and development. Results from studies in rats that were made iodine deficient showed histological changes in the breasts that were atypical and dysplastic. These tissue findings were further affected by the presence of estrogen and thyroxine. These changes parallel the iodine uptake of the tissues, thus representing a difference in the utilization of iodine by the mammary glands. Using an ion blockade agent, sodium perchlorate, breast tissues lacking iodine were evaluated by both endocrine and histological techniques. A dose-response series was com pleted that showed that perchlorate therapy for 8 weeks at 400 mg/100 ml produced breast blockade by a reduction in iodine uptake of greater than 52% of the control. At these levels, the histological experimentation showed atypia and some pleomorphism of the cells, particularly in the glands of the lobules. Blockade was less effective in estrogen treated groups. It is especially notable that both histologi cal changes and uptake reduction were greatest in those breasts that had been rendered euthyroid by thyroxine re placement, thus clearly indicating the necessity of iodine itself for maintenance of normal breast development. By this blockade the responses of iodine inadequacy in the breast were shown to cause abnormal tissue changes rela tive to the percentage of the block obtained.

Abstract:Today over 40 major metropolitan areas in the United States have formally adopted a new "Vision Zero" road design framework to prioritize roadway safety and eliminate traffic-related fatalities. This policy framework diffused across much of Europe and, more recently, the United States. In 2019, the City of San Francisco, California, implemented a series of traffic-calming interventions designed to lower speeds on left-hand turns and thereby reduce severe pedestrian and bicyclist injuries. This study uses a quasi-experimental design to evaluate this innovation. Results show that these safety measures reduced speeds between 1.4 and 1.9 miles per hour and that these reductions were sustained for at least 1 year. A conservative cost-benefit analysis suggests that these interventions have a large, positive return on investment, and that they are justified for any intersection with 2.5 or more pedestrian- or cyclist-involved crashes over a 5-year window.

Abstract:Imagine that, in 2050, not a single person in the United States dies in a traffic crash. This is the scenario described in this article, in which RAND researchers set forth a vision and strategy for achieving zero roadway deaths by 2050. The authors propose that a combination of three approaches can realize this scenario. The first is doubling down on programs and policies that have already been shown to be effective, including laws and enforcement, changes to roadway infrastructure designed to reduce traffic conflicts, reductions in speeds where crashes are likely, improvements to emergency response and trauma care, and more safety education and outreach. The second is accelerating advanced technology, beginning with advanced driver assistance systems (many of which are already in the market) and progressing up to fully automated vehicles. The third is prioritizing safety, which includes both (1) embracing a new safety culture that will lead Americans to think differently about our individual and collective choices and (2) widespread adoption of the "Safe System" approach, a paradigm shift in addressing the causes and prevention of roadway deaths and injuries. The authors conclude with a list of actions that key stakeholders-including professional engineering and planning organizations, public-sector organizations, safety advocates, vehicle manufacturers, technology developers, public health, emergency medical and trauma care organizations, and law enforcement and judicial system representatives-can take to bring about the changes needed to achieve zero roadway deaths by 2050.
The immediate effects of vision-zero corridor upgrades on pedestrian crashes in New York: A before-and-after spatial point process approach

Abstract:The long-term effects of the Vision-Zero (VZ) approach in Scandinavia are well documented. In contrast, information regarding the immediate effects of VZ at the starting phase upon gradual implementation is scarce. Taking New York City as the case study, we analyzed both the local and global effects of the Vision-Zero gradual implementation on pedestrian crashes in the early stage of implementation starting from 2014. The data analysis comprised 8,165 pedestrian injury crashes. Using location data, the crashes were matched to VZ infrastructure improvement location, start and completion dates. The experimental design included a treatment and two types of control conditions, and we controlled for well-known covariates including traffic exposure, land use, and risk-prone areas. We estimated a Geyer Saturation model and kernel density function for modeling the effect of Vision-Zero on crash intensity and dispersion two years before and after the implementation of Vision-Zero. The results reveal a significant global decrease of 6.1 % (p = 0.004) in pedestrian crash incidence in the treated sections compared with the control group two years after the treatment, and a greater dispersion of pedestrian injuries following the policy implementation.

Abstract:COMMITTEE ON TACTICAL COMBAT CASUALTY CARE The Committee on Tactical Combat Casualty Care (CoTCCC) was begun in 2001 as a US Special Operations Command biomedical research project. It was first established at the Naval Operational Medicine Institute and was supported by the Navy Bureau of Medicine and Surgery from fiscal year (FY) 2004 through FY 2009. In FYs 2007–2009, the Office of the Surgeon General of the Army and the US Army Institute of Surgical Research (USAISR) also helped to fund the activities of the CoTCCC. In 2007, because of the increasing visibility of TCCC in the conflicts in Iraq and Afghanistan, the Navy Medical Support Command proposed that the CoTCCC be considered for realignment at a more senior joint command. The CoTCCC was subsequently realigned to function as a subgroup of the Trauma and Injury Subcommittee of the Defense Health Board (DHB). The DHB is the senior external medical advisory group to the Secretary of Defense.

Abstract:Community engagement is a critical step to developing clean, equitable transportation systems in the transition to a decarbonized transport sector. Understanding the needs of the people who will use and be affected by new transportation infrastructure and services can help inform the fair distribution of potential burdens and benefits. A diverse set of communities must be selected for engagement to ensure multiple perspectives are considered. There is a need for consistent processes to systematically ensure community selection is inclusive. Important community groups have been left out of engagement, leading to biases in the findings from engagement efforts and ultimately, contributing to the inequitable transportation system we see today. There is a need for careful efforts up-front to identify and elevate the voices of those who have been and continue to be underrepresented in engagement processes. In this paper, we present a multifaceted approach for the selection of communities for engagement in transportation programs and policies. The approach consists of three integrated components: building problem context, analyzing data, and understanding community dynamics. We discuss each component and the iterations between them and apply the approach to a case study in Sonoma County, California, focused on the equitable transition to electric vehicles. Conducting this approach as a dedicated workstream in Sonoma County resulted in a diverse set of communities for future engagement efforts. This approach offers a shift in the way engagement efforts are conducted to prioritize diverse perspectives and ultimately, equitable transportation systems.

Abstract:Renewed interest in fission-fusion dynamics is due to the recognition that such dynamics may create unique challenges for social interaction and distinctive selective pressures acting on underlying communicative and cognitive abilities. New frameworks for integrating current knowledge on fissionfusion dynamics emerge from a fundamental rethinking of the term “fission-fusion” away from its current general use as a label for a particular modal type of social system (i.e., “fission-fusion societies”). Specifically, because the degree of spatial and temporal cohesion of group members varies both within and across taxa, any social system can be described in terms of the extent to which it expresses fission-fusion dynamics. This perspective has implications for socioecology, communication, cognitive demands, and human social evolution.
Higher-order structural determinants for expression of the ovalbumin gene family.

Abstract:The ovalbumin gene and the ovalbumin-related X and Y genes are expressed in the chicken oviduct in response to steroid hormones. These three genes are linked within a 100 kb domain of DNA which is preferentially sensitive to DNase I digestion in oviduct cell nuclei. No such preferential sensitivity to DNase is observed in nuclei isolated from other chicken tissues in which these genes are not transcribed. Thus, the DNase I sensitivity observed is correlated with the capacity for these genes to be expressed in oviduct. We have asked the question: are there specific signals in the DNA which are responsible for defining this domain or for conferring upon it the active, DNase I-sensitive, conformation? We have located DNA sequences belonging to a single repetitive DNA family, termed CR1, which are preferentially located in or near the boundary regions of the 100 kb domain. Therefore, these CR1 sequences are possible candidates for such a function. We have also searched for, but have not observed, any tissue-specific rearrangements of the DNA in the boundary regions of the domain. It is therefore unlikely that DNA rearrangements are involved in establishing the DNase I-sensitive domain in oviduct cells. However, we do note that a region at the far 3' end of the domain exhibits a cytidine methylation pattern which is highly variable among different chicken tissues. In particular, this region, which is approximately 30 kb downstream from the ovalbumin gene, is undermethylated in oviduct as compared to other hen tissues, and thus could be a control region involved in domain activation.
Major Traffic Safety Reform and Road Traffic Injuries Among Low-Income New York Residents, 2009–2021

Abstract:Objectives. To evaluate the effects of a comprehensive traffic safety policy—New York City’s (NYC’s) 2014 Vision Zero—on the health of Medicaid enrollees. Methods. We conducted difference-in-differences analyses using individual-level New York Medicaid data to measure traffic injuries and expenditures from 2009 to 2021, comparing NYC to surrounding counties without traffic reforms (n = 65 585 568 person-years). Results. After Vision Zero, injury rates among NYC Medicaid enrollees diverged from those of surrounding counties, with a net impact of 77.5 fewer injuries per 100 000 person-years annually (95% confidence interval = −97.4, −57.6). We observed marked reductions in severe injuries (brain injury, hospitalizations) and savings of $90.8 million in Medicaid expenditures over the first 5 years. Effects were largest among Black residents. Impacts were reversed during the COVID-19 period. Conclusions. Vision Zero resulted in substantial protection for socioeconomically disadvantaged populations known to face heightened risk of injury, but the policy’s effectiveness decreased during the pandemic period. Public Health Implications. Many cities have recently launched Vision Zero policies and others plan to do so. This research adds to the evidence on how and in what circumstances comprehensive traffic policies protect public health. Abstract:Purpose The United Nations Sustainable Development Goal (UN SDG) number 13 calls for urgent action to combat climate change impacts. Urban resilience planning documents often articulate partnerships and collaborations (P&Cs) as critical strategies. This study aims to identify the actors, the topics, processes, and visions of collaboration. Design/methodology/approach This paper explores the characterization of urban resilience partnerships via a thematic content analysis of resilience strategy documents produced in US cities through the 100 Resilient Cities Project. Findings P&Cs are defined broadly, taking several forms to meet various objectives. They act as boundary objects engaging across social groups, but the details of the P&Cs are rarely articulated, which is problematic for their implementation. P&Cs are commonly discussed in relation to the focus of the work; therefore, they less often define the specific actors, processes or transformative visions involved. Research limitations/implications This research focuses only on the resilience plans written in US cities, showing the perspective of US policymakers. Documents analyzed were produced via the Rockefeller Foundation's 100 Resilient Cities Project. Practical implications Understanding and categorizing the who, what, when and why of P&Cs for urban resilience provide a deeper understanding of how these strategies are being described and offer a starting point for tangibly actualizing partnerships and collaborations outside planning documents. Social implications To reach vital urban resilience goals, P&Cs must be designed and managed appropriately. Understanding the shortcoming of current P&C policies can help managers mitigate problems and find better approaches. Originality/value To the best of the author's knowledge, this paper is the first to analyze how P&Cs are being articulated and described in urban resilience plans. As urbanization continues and the effects of climate change escalate, city governments are finding themselves responsible for the resilience of large populations. To cope, increasing numbers of municipalities are developing urban resilience plans. These documents often articulate partnerships and collaborations (P&Cs) as critical strategies for enhancing resilience capacity and implementing resilience policies. Although specific case studies of resilience-related collaborative practices are well documented in urban resilience literature, little is known about the proliferation of P&C strategies collectively. Furthermore, questions remain regarding the characterization of resilience P&Cs by practitioners, including who is involved and what types of projects they undertake. Therefore, this analysis explores urban resilience P&Cs via a thematic content analysis of resilience strategy documents produced in 16 US cities through the 100 Resilient Cities Project. Results indicate that cities prioritize P&Cs in resilience policy implementation, but they often fall short in defining the key components of P&Cs that are vital to their success. The analysis exposes the most common actors, topics, processes and visions described in resilience P&Cs and makes recommendations for how urban resilience P&Cs can be improved in the future.
Endogeneity of pedestrian survival time and emergency medical service response time: Variations across disadvantaged and non-disadvantaged communities

Abstract:The Vision Zero-Safe Systems Approach prioritizes fast access to Emergency Medical Services (EMS) to improve the survivability of road users in transportation crashes, especially concerning the recent increase in pedestrian-involved crashes. Pedestrian crashes resulting in immediate or early death are considerably more severe than those taking longer. The time gap between injury and fatality is known as survival time, and it heavily relies on EMS response time. The characteristics of the crash location may be associated with EMS response and survival time. A US Department of Transportation initiative identifies communities often facing challenges. Six disadvantaged community (DAC) indicators, including economy, environment, equity, health, resilience, and transportation access, enable an analysis of how survival and EMS response times vary across DACs and non-DACs. To this end, this study created a unique and comprehensive database by linking DACs data with 2017-2021 pedestrian-involved fatal crashes. This study utilizes two-stage residual inclusion models with segmentation for DACs and non-DACs accounting for the endogenous relationship between EMS response and pedestrian survival time. The results indicate that EMS response time is higher and pedestrian survival time is lower in DACs than in non-DACs. A delayed EMS response time is associated with a greater reduction in survival time in DACs compared to non-DACs. Factors, e.g., nighttime and interstate crashes, contribute to higher EMS response time, while pedestrian drugs, driver speeding, and hit-and-run behaviors are associated with a greater reduction in survival time in DACs than non-DACs. The implications of the findings are discussed in the paper.
Vision Zero and Impaired Driving: Near and Longer-Term Opportunities for Preventing Death and Injuries

Abstract:Vision Zero involves the use of a systems approach to eliminate fatal and serious injuries from motor vehicle crashes by accommodating basic human limitations that lead to crashes through fundamental behavioral expectations, together with sound vehicle and road design. Alcohol-related crashes account for a significant proportion of motor vehicle crash death and injury and can be addressed in a safe road transport system. We look at near-term policy and program interventions that are known to motivate drivers to make safe drinking and driving decisions, and possibilities for using technology over the longer term to address risks resulting from driver impairment that is either inadvertent or willful high-risk behavior. From the Vision Zero perspective,"normal driving" refers to a situation where traffic and road users are operating as desired and planned. A driver in this normal driving envelope operates at a safe speed, wears a seat belt, focuses on the driving task, and is not impaired. A safe system accommodates human errors, mistakes, and misjudgments in the normal driving envelope. However, it may not be capable of compensating for deliberate violations and rule-breaking. A critical role of behavioral programs and policies is to motivate safe decisions by drivers and other road users and keep them in the normal driving envelope where they can be protected from unintentional errors by a safe system. While much progress has been made in developing and implementing impaired driving policies and programs, much potential remains in the their ability to motivate drivers to meet the fundamental expectations required in a safe system. Examples of behavioral programs and policies that have strong evidence of effectiveness but are underutilized in the U.S. include conducting periodic sobriety checkpoints, lowering the blood alcohol concentration limit for driving, and mandating the use of ignition interlock devices. While the specific interventions may differ, it is likely that the same situation of incomplete implementation of behavioral programs and policies - and consequent unrealized value to a comprehensive safe system - applies to many other nations. To reach the goal of zero deaths, a comprehensive Vision Zero program needs to address the problem of deliberate risk-taking, which can include driver impairment from alcohol or other causes and extend to dangerous and reckless driving. Advanced safety technologies offer a range of opportunities for this purpose. Cars available today and in the future will have a plethora of sensors that monitor circumstances inside and around the car. These systems can identify whether a driver is in their safe driving envelope and respond with interventions that are appropriate for the severity and nature of the risk. Interventions could range from those that are not perceivable to the driver, such as putting driver assist systems into active mode, to stronger steps such as limiting or preventing vehicle operation. Zero fatalities or serious injuries in motor vehicle crashes is possible with a systems approach that accommodates human errors and mistakes that occur with the normal driving envelope and incorporates effective responses to deliberate risk-taking outside of this envelope.

Abstract:As our cities grow, it is important to develop policies and streetscape designs that provide pedestrians with safe comfortable walking conditions and acknowledge the challenges involved in making urban places feel liveable and safe while understanding the critical role of streets around busy destinations. To understand these challenges at a nuanced, human level, new methods of citizen engagement are needed. This paper outlines the development and application of a new citizen perception collection method, using immersive virtual environments (IVE), coupled with an interactive emoji affective activation-pleasure grid and digital slider elements, embedded within an online e-participation survey to quantify, and rank the impact of individual (single-variable) urban design elements and safe system treatments on pedestrians’ perceptions of safety and place. The results demonstrate the effectiveness of this method for providing detailed, interrogable, scalable citizen perception data of a variety of urban street design elements and safe system treatments, which allows a statistical analysis of responses and prioritization of the most effective pedestrian-oriented interventions for maintaining or enhancing street vibrancy and liveability. Our IVE e-participation approach is an important contribution to forming a better understanding of streetscapes and provides a valuable method for urban designers and transport planners to prioritise different streetscape place and safety approaches.

Abstract:Despite advancements in our understanding of community engagement, questions remain as to how these efforts are applied in practice across specific contexts. First response is one such profession that has received minimal attention in the community engagement literature, even though the work of first responders frequently requires close engagement with communities to maintain safety and promote public health. To address this gap, we analyzed how first responders conceptualized community engagement during the COVID-19 pandemic. Through an analysis of semi-structured interviews collected from U.S. first responders ( n = 91), we developed a First Responder Ladder of Community Engagement informed by Arnstein's Ladder of Citizen Participation. We find that first responder community engagement during the COVID-19 pandemic predominantly focused on information sharing and offered minimal evidence of higher levels of engagement, highlighting the potential disconnect between theories and practice of community engagement. These findings advance our knowledge of the application, variations, and broader framings of community engagement approaches used by first responders, and other fields of practice.

What Lies Beneath? The Role of Community Engagement in Translating COVID-19 Research Findings to Policy-Makers

Abstract:Background: Community engagement is key to developing local and context-specific strategies for the prevention and control of COVID-19. However, expedited research design and approval in the early days of the pandemic may have limited the opportunities for community members to influence pandemic-related research. In this study, we sought to understand how a Community Engagement Group (CEG) could impact a large longitudinal COVID-19 research project (Optimise), when involved solely in the interpretation and knowledge translation phases of the research. Methods: Seven community members were recruited for the CEG, representing a diverse range of groups. Each month, Optimise data of topical importance were compiled into a draft report. The CEG discussed the draft report at their monthly meeting and members' contributions were incorporated into the final report for distribution to policy-makers. In this study, a document analysis was undertaken of ten consecutive reports produced between February and November 2021. Each report was compared pre- and post- the inclusion of CEG contributions, which were then analysed using thematic analysis. Results: Community engagement in the interpretation and knowledge translation phases of Optimise had positive impacts on reports for policy-makers, including grounding the empirical findings in broader community perspectives, identifying policy issues affecting different groups and contributing unique insights beyond the empirical findings. Overall, the CEG contributions demonstrated the complexity of lived experience lying beneath the empirical data. Conclusion: Community engagement in the translation of the Optimise findings resulted in research reports to policy-makers that were reflective of a broader range of community perspectives, and that provided potential solutions to emerging policy issues related to COVID-19. This study adds to the evidence base about the impact of community engagement in the later interpretation and knowledge translation phases of research, particularly in the context of reporting to policy-makers during a public health emergency.
