Abstract:Genetic factors are the main causes in occurrence of birth defects. With deep research in the field of genomics and application of molecular biology technology, the carrier status, fetal genetic variation and postpartum screening are respectively detected from pre-pregnancy, pregnancy and post-natal screening (before onset of disease) under perfect three-level prevention and control system for birth defects. Prospective detection, early diagnosis and intervention can prevent the occurrence of birth defects related to genetic diseases at multiple levels. This article describes and analyzes current clinical application and existing challenge of molecular biology techniques in prevention of birth defects related to genetic diseases.
Chinese geneticists' views of ethical issues in genetic testing and screening: evidence for eugenics in China

Abstract:To identify Chinese geneticists' views of ethical issues in genetic testing and screening, a national survey was conducted. Of 402 Chinese geneticists asked to participate, 255 (63%) returned by mail anonymous questionnaires. The majority of respondents thought that genetic testing should be offered in the workplace for alpha-antitrypsin deficiency (95%) and the predisposition of executives to heart disease, cancer, and diabetes (94%); that genetic testing should be included in preemployment physical examinations (86%); that governments should require premarital carrier tests (86%), newborn screening for sickle cell (77%), and Duchenne muscular dystrophy (71%); and that children should be tested for genes for late-onset disorders such as Huntington disease (85%), susceptibility to cancers (85%), familial hypercholesterolemia (84%), alcoholism (69%), and Alzheimer disease (61%). Most believed that partners should know each other's genetic status before marriage (92%), that carriers of the same defective gene should not mate with each other (91%), and that women should have a prenatal diagnosis if medically indicated (91%). The majority said that in China decisions about family planning were shared by the couple (82%). More than half had views that, in China, there were no laws to prohibit disability discrimination (64%), particularly to protect people with adult polycystic kidney disease (57%), cystic fibrosis (56%), or genetic predisposition to other diseases (50%). To some extent, these results might provide a basis for a discussion of eugenics in China, particularly about China's Maternal and Infant Health Care Law (1994).

Abstract:Abstract Background To investigate the prevalence and prenatal diagnosis rate of chromosomal abnormalities (CA) in Zhejiang Province, China. Methods We estimated the annual changes in the detected prevalence of CA and prenatal diagnosis rate among 681,590 births in Zhejiang Province, China, between 2014 and 2020. Data were derived from the provincial birth defects surveillance system, which represents 30% of annual births in Zhejiang Province. The effect of maternal age was also evaluated. Results The detected prevalence of sex chromosomal abnormalities (1.70–7.30 per 10,000 births, Ptrend < 0.001) and microdeletion and microduplication (0.30–6.81 per 10,000 births, Ptrend < 0.001) gradually increased, contributing to an upward trend in overall CA (12.09–39.22 per 10,000 births). The diagnosis rate before 22 gestational weeks constantly increased from 20.8 to 70.1% for trisomy 21 (Ptrend = 0.003). The prevalence rate ratio for maternal age of ≥ 35 years was higher than that for maternal age of 25–29 years for trisomy 21 (5.40, 95% confidence interval [CI] 4.59–6.35) and sex chromosomal abnormalities (3.28, 95% CI 2.48–4.33). Conclusions The rising prevalence of CA in China may be attributable to the elevated maternal age and the innovation of prenatal diagnosis tools, Thus, studies should pay attention to the rare CA that were previously ignored, and select rational screening tools.

Abstract:An important part of China's "Healthy China 2030" planning is to lower the rate of birth defects. Because genetic factors contribute solely or collaboratively to about 80% of the occurrence of birth defects, genetic studies on birth defects can provide precise molecular targets for clinical screening, diagnosis and treatment. Genetic research on birth defects in China has developed by leaps and bounds since 1960s. At the same time, as related research achievements keep accumulating, translation of these scientific discoveries to clinical applications, with genetic counseling and testing as the core practices, has been developed and optimized. A close collaboration between genetic researches and clinical applications would provide reliable technical support for giving birth to more "healthy children" in China. This article firstly reviews China's history of genetic research on birth defects, then introduces current situation and hot topics of the research area at home and abroad and finally discusses about future trend and related clinical applications. In summary, an overall view is provided here for the readers to understand the development route of genetic research on birth defects in China.
Survey of Birth Defects and Discussion on Intervention Mode in Nanshan District of Shenzhen City

Abstract:Objective To obtain the current situation on screening of birth defects in the community of Nanshan district,Sheznhen,and find the intervention mode and method,which is suitable for the generalization of community,to prevent birth defects.Methods Screen on Mediterranean anemia,glucose-6-phosphate dehydrogenase deficiency,and congenital rubella syndrome for the married couple who is without birth and owns the census register,and screen on Down's syndrome and neural tube defects for pregnant women with a gestational period of 16~21 weeks.And provide consultation and guide about healthy birth for the high risk group.Results A percentage of 90.8% of investigated population had heard of birth defects,and the information was mainly gained by newspapers,magazine,TV,broadcast,propaganda,and leaflet.Although 74.4% of investigated population knows the screen on health birth,only 5.6% know 1 or 2 specific screening disease.There was a higher popular rate on haxard factors inducing birth defects for males than females.The percentage of carriers with the gene of Mediterranean anemia,glucose-6-phosphate dehydrogenase deficiency,and high risk group with Down's syndrome and neural tube defects was 7.64%,7.39%,9.83‰ and 3.73‰,respectively.Conclusion People in reproductive life are lack of relative knowledge to birth defects.It is necessary to perform integrative intervention of birth defects in community,and it is a feasible working mode that makes the use of three-tier service system of birth control and family planning to provide primary and secondary intervention on birth defects.

Abstract:Birth defects are a population health problem affecting the quality of birth population in China.Challenges exist with respect to reducing the risk of birth defects and the consequential disease burden in a cost-effective way.Therefore fundamental and applied researches to prevent birth defects should be further strengthened.These researches are expected to closely focus on the preventability of birth defects,such disease with a high risk,high prevalence,and disabling nature among China population;and development of population-based theoretic and technical countermeasures,in order to achieve the goals to effectively reduce the disease burden of population with birth defects.For fundamental researches,while continuing to study the genetic fundament and molecular mechanisms of major birth defects,we should employ a systematic biological point of view and the use of macro-and micro-studies combination,to strengthen the surveillance of birth defects and conduct prospective cohort studies;improve our understanding of the role of genetic factors,environmental factors,and their interaction in the occurrence of birth defects;identify the major environmental risk factors,genetic susceptibility,and epigenetic changes;and develop in-depth studies about the mechanism of birth defects.For the application of technology in prevention and control of birth defects,we should uphold the principle of prevention and the strategy of focusing on both prevention and treatment,targeting primary,secondary,and tertiary prevention schemes,to conduct comprehensive researches on birth defects prevention,screening,diagnosis,counseling,treatment and other applications of crucial technologies;and develop appropriate,demand-oriented systems to prevent and control in China setting,including pre-pregnancy risk assessment and counseling techniques,preventive technologies in primary prevention scheme of birth defect,prenatal screening and diagnostic techniques for major birth defects,screening techniques for genetic metabolic diseases among newborns,and early diagnosis and treatment technology for major congenital abnormalities.Meanwhile,we should strengthen the social science research and explore modeling of prevention and treatment of birth defects,in order to translate research results into practice and promote the implementation of national action plan for birth defects prevention.

Abstract:This paper describes the incidence of birth defects in China in recent years and demonstrates cost of illness of some birth defects. It highlights the dramatically heavy burdens to society due to birth defects, and shows that they are becoming an important public health issue. It analyses the opportunity and threats facing programmes to prevent and control birth defects by genetic services for the Chinese healthcare system. Some recommendations are proposed to improve genetic services, including political commitment, a tiered medical genetic services system, ethical guidelines, capacity building, consumers' KAP, R&D, and setting priorities.
The Effects of Preconception Examinations on Neural Tube Defects and the Primary Preventive Measures: Case-Control Study

Abstract:Background: Preventive measures for neural tube defects (NTDs) have been recommended for many years in China, but the compliance with these measures is unsatisfactory. This study aims to analyze the effects of preconception examinations on NTDs and its primary preventive measures.Methods: A 1:1 hospital-based case-control study was conducted. Four hundred and fifty-nine women who delivered or gestate infants/fetuses with NTDs from January 2006 to December 2008 were randomly selected and matched with women who delivered babies without obvious birth defects as controls in Shandong and Shanxi province. Multivariate conditional logistic regression was adopted.Results: Significant associations were shown between preconception examinations (OR = 0.461), health education (OR = 0.336), periconceptional folic acid supplement (OR = 0.295), periconceptional rational diet adjustment (OR = 0.278) and NTDs. In the case group, the rates of periconceptional folic acid supplement and health education conduction by women who had preconception examinations were significantly higher than that of those who had not, OR being 3.04 and 4.55, respectively (p<0.05). Among the preventive effects on NTDs, preconception examinations and other NTDs primary preventive measures had significant combined effects and the combined effects with periconceptional folic acid supplement were the greatest, with OR of 0.04.Conclusion: Preconception examinations have preventive effects on NTDs and can significantly improve the compliance of other NTDs primary preventive measures. In addition, preconception examinations and these measures have synergetic prevention effects, indicating the critical role played by preconception examinations on NTDs prevention.

Abstract:BackgroundCongenital heart defect is the leading malformation in China. There may have been changes in congenital heart defect incidence because of birth policy shift in China over past years. This study aimed to investigate the epidemiology, prenatal diagnosis, and outcomes of congenital heart disease to improve medical and policy decisions.MethodsData on cases of congenital heart disease identified during 2014-2018 were taken from the Zhejiang provincial birth defects surveillance system. Chi-square test, odds ratio (OR) and 95% confidence interval (CI) were used to explore epidemiology, prenatal diagnosis, and birth outcomes of congenital heart disease.ResultsThe average incidence of congenital heart disease was 16.0 per 1000 births, which increased by 62.2% during 2014-2018(chi (2)(trend)=181.41, P<0.001). However, the average critical congenital heart incidence was 1.6 per 1000 births, which remained stable over time. Women aged 20years (OR2.1, 95% CI 1.9-2.3) or >= 35years (OR 1.2, 95% CI 1.2-1.3) were at higher risk of having babies with congenital heart disease than women aged 21-34years. Women who gave birth in urban areas (OR 1.2, 95% CI 1.2-1.3), had a son (OR 1.3, 95% CI 1.3-1.4), or had multiple births (OR 4.0, 95% CI 3.7-4.4) were also at higher risk than those giving birth in rural areas, to girls, or single births, respectively. The three major subtypes of congenital heart disease were atrial septal defect (67.9%), patent ductus arteriosus (34.7%), and ventricular septal defect (6.4%). The prenatal detection rate of critical congenital heart disease was 90.0%, which was far higher than total congenital heart disease, at 22.2% (chi (2)=1687.67, P<0.001). There were 1457 (17.1%) stillbirths, 106 (1.2%) early neonatal deaths, and 6983 (81.7%) live births associated with congenital heart disease.ConclusionsThe high incidence of congenital heart disease in Zhejiang might be attributable to the large proportion of mild congenital heart disease. The incidence of critical congenital heart disease, the prenatal detection rate, and perinatal deaths from congenital heart disease are comparable to those in other studies.

Abstract:Carrier screening had been demonstrated as a powerful practice in preventing selected severe genetic disorders. This practice is expanding its scope and impact in the era of next-generation sequencing. Empirical and theoretical data support the utility of expanded carrier screening. The authors propose a comprehensive carrier screening program as a main component of the first-tier measure in preventing severe genetic disorders and birth defects in China. We discussed the key principles and important aspects to ensure the success of such a program. The authors believe this program will play a pivotal role in our endeavor for a healthier nation.
Screening for Congenital Heart Defects: Diversified Strategies in Current China

Abstract:BackgroundCongenital heart defects (CHD) is the most common type of birth defect and a leading cause of infant mortality in China. Detection of CHD during newborn is still challenging. The contradiction between the increasingly mature technology of diagnosis and treatment and the inability of early detection is the biggest current dilemma. A few pilot studies attempt to establish the universal screening for CHD in newborns; however, the rate of misdiagnosis is still high in most Chinese hospitals, especially in some undeveloped middle-western regions.Data sourcesBased on the recent publications on screening of congenital heart diseases in China. We reviewed the use of diversified screening strategies in current China.ResultsPrenatal diagnosis by fetal echocardiography and postnatal detection by pulse oximetry combined with clinical assessment are the useful methods for CHD screening in most areas. The altitude should be taken into account when using pulse oximetry in the middle-western areas of China, where the incidence of CHD maybe higher. Echocardiography is suitable for CHD screening in almost all areas but it could add to financial burden in the developing regions. Genetic analysis could assist clinical doctors to perform more earlier screening and give better counseling regarding the outcome. Due to disparities in economic and medical resources, the screening system should be carried out from multiple perspectives according to the present economic development. Notably, follow-up is an important issue in the screening of CHD, especially for the asymptomatic babies who discharged home. Policies should be formulated to address the epidemiology of CHD in deprived areas to better allocate medical resources and to develop local training programmes to screen and diagnose CHD.ConclusionsDiversified strategies are available in current China. The two-indicator method for CHD screening is recommended to be implemented in routine postnatal care. We can do more in screening for CHD in the future.
Prospective Prenatal Cell-Free DNA Screening for Genetic Conditions of Heterogenous Etiologies

Abstract:Prenatal cell-free DNA (cfDNA) screening uses extracellular fetal DNA circulating in the peripheral blood of pregnant women to detect prevalent fetal chromosomal anomalies. However, numerous severe conditions with underlying single-gene defects are not included in current prenatal cfDNA screening. In this prospective, multicenter and observational study, pregnant women at elevated risk for fetal genetic conditions were enrolled for a cfDNA screening test based on coordinative allele-aware target enrichment sequencing. This test encompasses the following three of the most frequent pathogenic genetic variations: aneuploidies, microdeletions and monogenic variants. The cfDNA screening results were compared to invasive prenatal or postnatal diagnostic test results for 1,090 qualified participants. The comprehensive cfDNA screening detected a genetic alteration in 135 pregnancies with 98.5% sensitivity and 99.3% specificity relative to standard diagnostics. Of 876 fetuses with suspected structural anomalies on ultrasound examination, comprehensive cfDNA screening identified 55 (56.1%) aneuploidies, 6 (6.1%) microdeletions and 37 (37.8%) single-gene pathogenic variants. The inclusion of targeted monogenic conditions alongside chromosomal aberrations led to a 60.7% increase (from 61 to 98) in the detection rate. Overall, these data provide preliminary evidence that a comprehensive cfDNA screening test can accurately identify fetal pathogenic variants at both the chromosome and single-gene levels in high-risk pregnancies through a noninvasive approach, which has the potential to improve prenatal evaluation of fetal risks for severe genetic conditions arising from heterogenous molecular etiologies. Changes in Maternal Age and Prevalence of Congenital Anomalies During the Enactment of China's Universal Two-Child Policy (2013-2017) in Zhejiang Province, China: an Observational Study.

Abstract:BACKGROUND:China implemented a partial two-child policy (2013) followed by a universal two-child policy (2015), replacing the former one-child policy mandated by the government. The changes affect many aspects of China's population as well as maternal and infant health, but their potential impact on birth defects (BDs) remains unknown. In this study, we investigated the associations of these policy changes with BDs in Zhejiang Province, China. METHODS AND FINDINGS:We used data from the BD surveillance system in Zhejiang Province, China, which covers 90 hospitals in 30 urban districts and rural counties, capturing one-third of the total births in this province. To fully consider the time interval between conception and delivery, we defined the one-child policy period as data from 2013 (births from October 2012 to September 2013), the partial two-child policy period as data from 2015 (births from October 2014 to September 2015), and the universal two-child policy period as data from 2017 (births from October 2016 to September 2017). Data from 2009 and 2011 were also used to show the changes in the proportion of births to women with advanced maternal age (35 years and older) prior to the policy changes. Main outcome measures were changes in the proportion of mothers with advanced maternal age, prevalence of BDs, rankings of BD subtypes by prevalence, prenatal diagnosis rate, and live birth rate of BDs over time. A total of 1,260,684 births (including live births, early fetal losses, stillbirths, and early neonatal deaths) were included in the analyses. Of these, 644,973 (51.16%) births were to women from urban areas, and 615,711 (48.84%) births were to women from rural areas. In total, 135,543 (10.75%) births were to women with advanced maternal age. The proportion increased by 85.68%, from 8.52% in 2013 to 15.82% in 2017. However, it had remained stable prior to policy changes. Overall, 23,095 BDs were identified over the policy changes (2013-2017). The prevalence of BDs during 2013, 2015, and 2017 was 245.95, 264.86, and 304.36 per 10,000 births, respectively. Trisomy 21 and other chromosomal defects increased in both risk and ranking from 2013 to 2017 (crude odds ratio [95% confidence interval] 2.13 [1.75-2.60], from ranking 10th to 5th, and 3.63 [2.84-4.69], from ranking 16th to 6th, respectively). The prenatal diagnosis rate increased by 3.63 (2.2-5.1) percentage points (P < 0.001), from 31.10% to 34.72%, and identification of BDs occurred 1.88 (1.81-1.95) weeks earlier (P < 0.001). The live birth rate for infants with BDs born before 28 gestational weeks increased from 1.29% to 11.45%. The major limitations of this observational study include an inability to establish causality and the possible existence of unknown confounding factors, some of which could contribute to BDs. CONCLUSIONS:In this study, we observed significant increases in maternal age and the prevalence of total and age-related anomalies following China's new two-child policy. Increases in live birth rate for infants with BDs born before 28 gestational weeks suggest that healthcare for very preterm births with BDs may be warranted in the future, as well as updating the definition of perinatal period.

Abstract:Objective To document attitude to birth defects in modern China, identifying enlightenment on modern birth defects medicine and social research. Methods Classifying and examining the historical materials on birth defects, reproduction, maternal and child health, collected from modern major medical periodicals and newspapers (e.g., Shen Bao). Results Birth defects were one of the important medical issues paid great emphasis by every country at the present time, and they were recorded in ancient and modern Chinese medical literature. Conclusion Modern China is in a period of social transition, with the dual-impact of both Chinese traditional and western culture. The recognition and disposition of birth defects have changed from tradition to modern medicine.

Abstract:The purpose of this paper is to explore population-based approaches for screening of common major birth defects based on Chinese family planning network.A population-based program for screening of thalassaemia,Down syndrome and major fetal malformations has been conducted in Guangzhou since 2007, based on public health education and community involvement.Active and full-range monitoring and service system was developed to follow up the pregnant women and to offer a series of genetic checks including genetic screening,counseling and voluntary prenatal diagnosis followed by option of pregnancy termination for affected fetuses.19,308 couples in total had received genetic checks and the screening coverage rate reached 73.75%.There were 30 fetuses detected with thalassemia major,20 with Down syndrome or with other severe chromosome abnormality,110 with severe fetal malformations.157 out of 160 whose fetuses with severe diseases voluntarily terminated the pregnancies,accounting for 8.13‰.To conclude,based on Chinese family planning network,active and full-range monitoring and service model can effectively reduce the birth of common major birth defects.
