Abstract:Periodontitis is a chronic inflammation that seriously affects oral health and quality of life for patients. The hydrogen peroxide (H 2 O 2 ) concentration in saliva and gingival crevicular fluid (GCF) can reflect the degree of periodontitis. In this work, a flexible electrochemical H 2 O 2 sensor was constructed by a screen-printed electrode (SPE) modified with a copper nanoparticle-anchored copper-based metal-organic framework (Cu ) and titanium carbide nanosheets (Ti 3 C 2 T x NSs). Notably, such an SPE sensor has favorable flexibility, which is appropriate for attaching exactly onto the human body with a varied profile. Furthermore, the nanohybrid obtains excellent electrocatalytic activity owing to the high surface area of small Cu NPs, hierarchical structure of Cu-MOF, accelerating electron transfer rate of Ti 3 C 2 T x NSs, and extraordinary synergistic effect of Cu and Ti 3 C 2 T x NSs. The sensor possesses prominent sensitivities of 254.9 μA mM -1 cm -2 (0.2-26.1 μM) and a detection limit of 84.5 nM. Regarding feasibility, the developed sensor can trace H 2 O 2 release at the cellular level. Moreover, it can distinguish between healthy people and patients with gingivitis and periodontitis by monitoring H 2 O 2 in saliva and GCF samples. Hence, the as-designed sensing platform is promising to serve as an efficient tool for early periodontitis indication.

Abstract:The development of facile, rapid and cost-effective strategies for sensitive detection of cancer biomarkers in human samples is of great significance for early diagnosis of malignant tumors related diseases. In this work, we develop a high-performance electrochemical biosensor based on highly active dual nanozyme amplified system, i.e., ultrathin two-dimension (2D) conductive metal–organic framework (C-MOF) nanosheets (NSs) decorated with high-density ultrafine gold nanoparticles (Au-NPs), and explore its application in sensitive detection of cancer biomarker H2O2 in live cells. The C-MOF NSs {i.e., Cu-HHTP (HHTP = 2,3,6,7,10,11-hexahydroxytriphenylene)-NSs} provide large surface area and abundant active open metal sites (Cu–O4), which could improve the catalytic activity of Cu-HHTP-NSs towards H2O2. Abstract:Oxidative stress plays an important role in the pathogenesis of many diseases, while the exact mechanism that hydrogen peroxide (H2O2) as one of the most abundant reactive oxygen species (ROS) exerts its influence on oxidative stress remains unclear. We developed a novel turn-on ratiometric electrochemical sensor for the detection of H2O2 in blood samples. The electrochemical probe 5-(1,2-dithiolan-3-yl)-N-(4-(4,4,5,5-tetramethyl-1,3,2-dioxaborolan-2-yl)phenyl)pent-anamide (BA) was designed and synthesized for the selective detection of H2O2 via a one-step amide reaction. Meanwhile, Nile Blue A (NB) was optimized as an internal reference molecule, thus enabling accurate quantification of H2O2 in a complex environment. BA and NB were then co-assembled onto a carbon fiber microelectrode (CFME) coated with Au cones. The oxidation peak current ratio between BA and NB demonstrated good linearity with the logarithm of the H2O2 concentration values ranging from 0.5 μM to 400 μM with a low detection limit of 0.02 μM. The developed sensor showed remarkable selectivity against potential interferences in whole blood samples, especially for ascorbic acid, uric acid, and dopamine. In combination with the unique characteristics of CFME, such as a small size and good biocompatibility, the microsensor was used for rapid analysis of one drop of whole blood sample. This sensor not only creates a new platform for the detection of H2O2 in whole blood samples, but also provides a new design strategy of other ROS analysis for early diagnosis of ROS-related diseases, drug discovery processes, and pathological mechanisms.

Development of a novel microfluidic biosensing platform integrating micropillar array electrode and acoustic microstreaming techniques

Abstract:Quantifying biomarkers at the early stage of the disease is challenging due to the low abundance of biomarkers in the sample and the lack of sensitive techniques. This article reports the development of a novel microfluidic electrochemical biosensing platform to address this challenge. The electrochemical sensing is achieved by utilizing a micropillar array electrode (μAE) coated with 3D bimetallic Pt-Pd nanotrees to enhance the sensitivity. A bubble-based acoustic microstreaming technique is integrated with the device to increase the contact of analyte molecules with the surface of electrodes to further enhance the electrochemical performance. The current density of Pt-Pd NTs/μAE with acoustic microstreaming is nearly 22 times that of the bare planar electrode in potassium ferrocyanide solution. The developed biosensor has demonstrated excellent sensing performance. For hydrogen peroxide detection, both the Pt-Pd NTs/μAE and acoustic microstreaming contribute to the sensitivity enhancement. The current density of the Pt-Pd NTs/μAE is approximatively 28 times that of the bare μAE. With acoustic microstreaming, this enhancement is further increased by nearly 1.6 times. The platform has a linear detection range of 5-1000 μM with a LOD of 1.8 μM toward hydrogen peroxide detection, while for sarcosine detection, the linear range is between 5 and 100 μM and LOD is 2.2 μM, respectively. Furthermore, the sarcosine biosensing shows a high sensitivity of 667 μA mM-1∙cm-2. Such a sensing platform has the potential as a portable device for high sensitivity detection of biomarkers.

Abstract:The development of high-efficient technologies for cancer biomarkers detection has attracted tremendous research effort for its great clinic significance. In this work, we designed a new type of flexible and robust nanohybrid microelectrode by modifying carbon fiber with dual nanoenzyme, i.e., AuPd alloy nanoparticles (AuPd-ANPs) decorated graphene quantum dots (GQDs) assembly, and explored its practical application in electrochemical sensing system for sensitive detection of cancer biomarker hydrogen peroxide (H2O2) in human breast cancer cells and tissue. For the preparation of dual nanoenzyme modified microelectrode, ionic liquid was used as the electrolyte for the effective electrodeposition of GQDs on carbon fiber substrate to form a close-packed assembly under a very negative potential, then the highly dense AuPd-ANPs were uniformly decorated on GQDs assembly by electrodeposition. In virtue of the structural merits and synergistic contribution of dual nanoenzyme in enhancing the electrocatalytic activity to H2O2, the resultant nanohybrid microelectrode exhibited good sensing performances for electrochemical detection of H2O2, including a high sensitivity of 371 μA cm-2 mM-1, a wide linear range from 1.0 μM to 18.44 mM, a low detection limit of 500 nM (a signal-to-noise ratio of 3:1), as well as good selectivity and biocompatibility, which could be used for real-time tracking H2O2 released from different types of human breast cells and in situ sensitive detection of H2O2 in clinical breast cancer tissue.

Abstract:ABSTRACT Eukaryotic initiation factor 4G (eIF4G) promotes mRNA recruitment to the ribosome by binding to the mRNA cap- and poly(A) tail-binding proteins eIF4E and Pap1p. eIF4G also binds eIF4A at a distinct HEAT domain composed of five stacks of antiparallel α-helices. The role of eIF4G in the later steps of initiation, such as scanning and AUG recognition, has not been defined. Here we show that the entire HEAT domain and flanking residues of Saccharomyces cerevisiae eIF4G2 are required for the optimal interaction with the AUG recognition factors eIF5 and eIF1. eIF1 binds simultaneously to eIF4G and eIF3c in vitro, as shown previously for the C-terminal domain of eIF5. In vivo, cooverexpression of eIF1 or eIF5 reverses the genetic suppression of an eIF4G HEAT domain Ts− mutation by eIF4A overexpression. In addition, excess eIF1 inhibits growth of a second eIF4G mutant defective in eIF4E binding, which was also reversed by cooverexpression of eIF4A. Interestingly, excess eIF1 carrying the sui1-1 mutation, known to relax the accuracy of start site selection, did not inhibit the growth of the eIF4G mutant, and sui1-1 reduced the interaction between eIF4G and eIF1 in vitro. Moreover, a HEAT domain mutation altering eIF4G moderately enhances translation from a non-AUG codon. These results strongly suggest that the binding of the eIF4G HEAT domain to eIF1 and eIF5 is important for maintaining the integrity of the scanning ribosomal preinitiation complex.

Abstract:A novel Fe-MoOx nanozyme, engineered with enhanced peroxidase (POD)-like activity through strategic doping and the creation of oxygen vacancies, is introduced to catalyze the oxidation of TMB with high efficiency. Furthermore, Fe-MoOx is responsive to single electron transfer (SET) and hydrogen atom transfer (HAT) mechanisms related to antioxidants and can serve as a desirable nanozyme for total antioxidant capacity (TAC) determination. The TAC colorimetric platform can reach a low LOD of 0.512 μM in solution and 24.316 μM in the smartphone-mediated RGB hydrogel (AA as the standard). As proof of concept, the practical application in real samples was explored. The work paves a promising avenue to design diverse nanozymes for visual on-site inspection of food quality.
Development of a nanozyme-based electrochemical catalyst for real-time biomarker sensing of superoxide and nitric oxide anions released from living cells and exogenous donors

Abstract:Developing quantitative biosensors of superoxide (O2•-) and nitric oxide (NO) anion is crucial for pathological research. As of today, the main challenge for electrochemical detection is to develop high-selectivity nano-mimetic materials to replace natural enzymes. In this study, the dendritic-like morphological structure of silver organic framework (Ag-MOF) was successfully synthesized via a solvothermal strategy. Owing to the introduction of polymeric composites results in improved electrical conductivity and catalytic activity, which promotes mass transfer and leads to faster electron efficiency. For monitoring the electrochemical signals of O2•- and NO, the Ag-MOF electrode substrate was produced by drop-coating, and composites were designed by cyclic voltammetric potential cycles. The designed electrode substrates demonstrate high sensitivity, wide linear concentrations of 1 nM-1000 μM and 1 nM-850 μM, and low detection limits of 0.27 nM and 0.34 nM (S/N = 3) against O2•- and NO. Aside from that, the sensor successfully monitored the cellular release of O2•-, and NO from HepG2 and RAW 264.7 living cells and has the potential to monitor exogenous NO release from donors of Diethylamine (DEA)-NONOate and sodium nitroprusside (SNP). Additionally, the developed system was applied to the analysis of O2•- and NO in real biological fluid samples, and the results were good satisfactory (94.10-99.57 ± 1.23%). The designed system provides a novel approach to obtaining a good electrochemical biosensor platform that is highly selective, stable, and flexible. Finally, the proposed method provides a quantitative way to follow the dynamic changes in O2•- and NO in biological systems.

Abstract:The exploration into nanomaterial-based nonenzymatic biosensors with superb performance in terms of good sensitivity and anti-interference ability in disease marker monitoring has always attained undoubted priority in sensing systems. In this work, we report the design and synthesis of a highly active nanocatalyst, i.e., palladium and platinum nanoparticles (Pt&Pd-NPs) decorated ultrathin nanoporous gold (NPG) film, which is modified on a homemade graphene paper (GP) to develop a high-performance freestanding and flexible nanohybrid electrode. Owing to the structural characteristics the robust GP electrode substrate, and high electrochemically catalytic activities and durability of the permeable NPG support and ultrafine and high-density Pt&Pd-NPs on it, the resultant Pt&Pd-NPs-NPG/GP electrode exhibits excellent sensing performance of low detection limitation, high sensitivity and anti-interference capability, good reproducibility and long-term stability for the detection of small molecular biomarkers hydrogen peroxide (H2O2) and glucose (Glu), and has been applied to the monitoring of H2O2 in different types of live cells and Glu in body fluids such as urine and fingertip blood, which is of great significance for the clinical diagnosis and prognosis in point-of-care testing.
Specific and Magnetic Covalent Organic Framework Confined Os Nanoclusterzyme for Interference‐Free and Ultrasensitive Biosensing

Abstract:By using ascorbic acid (AA) as deoxidizer, Fe3O4@COF@Os displays superior POD‐specific activity and negligible oxidase (OXD) and catalase (CAT) mimic activity. Therefore, Fe3O4@COF@Os can simultaneously separate and catalytic colorimetric analysis of targets in biological samples without background interference. The Fe3O4@COF@Os is adapted to detect the cancer biomarker PSA in serum and metabolic disease biomarker glucose in saliva with excellent sensitivity, specificity and recyclability. This POD‐specific nanozymes can offer a promising pretreatment‐free and POC strategy for rapid and accurate diseases diagnosis. The peroxidase (POD)‐mimic nanozymes can be a potential tool for point‐of‐care (POC) diseases diagnosis. However, the complex sample matrix and unspecific multiple‐enzyme activities of nanozyme can generate false background signal and decrease the diagnostic accuracy. Here, a magnetic and POD‐specific covalent organic framework (COF) confined osmium (Os) nanoclusterzyme Fe3O4@COF@Os is developed for biomarkers biosensing. First, found that the catalytic activity and specificity of Fe3O4@COF@Os can be regulated by the deoxidizers and the functional groups of COF ligands. By using ascorbic acid as deoxidizer and F functional COF ligands, Fe3O4@COF@Os displayed superior POD‐specific activity and negligible oxidase (OXD) and catalase (CAT) mimic activity. Therefore, Fe3O4@COF@Os can separate and analyze of targets in bio‐matrix without background interference. For application, Fe3O4@COF@Os based lateral flow assay is established for serum cancer biomarker prostate‐specific antigen, the detection limit is 3.83 pg mL−1. For clinical prostatic cancer samples, the accuracy is 100% with a correlation coefficient of 0.998 with commercial ELISA kit. For chronic disease biomarker sensing, salivary glucose can be quantified as low as 0.1 μm, with negligible background interference and favorable recyclability. This specific magnetic nanozymes can offer a promising pretreatment‐free and POC strategy for rapid and accurate diseases diagnosis.

Intelligent salivary biosensors for periodontitis: in vitro simulation of oral oxidative stress conditions

Abstract:One of the most common oral diseases affecting millions of people worldwide is periodontitis. Usually, proteins in body fluids are used as biomarkers of diseases. This study focused on hydrogen peroxide, lipopolysaccharide (LPS), and lactic acid as salivary non-protein biomarkers for oxidative stress conditions of periodontitis. Electrochemical analysis of artificial saliva was done using Gamry with increasing hydrogen peroxide, bLPS, and lactic acid concentrations. Electrochemical impedance spectroscopy (EIS) and cyclic voltammetry (CV) were conducted. From EIS data, change in capacitance and CV plot area were calculated for each test condition. Hydrogen peroxide groups had a decrease in CV area and an increase in percentage change in capacitance, lipopolysaccharide groups had a decrease in CV area and a decrease in percentage change in capacitance, and lactic acid groups had an increase of CV area and an increase in percentage change in capacitance with increasing concentrations. These data showed a unique combination of electrochemical properties for the three biomarkers. Scanning electron microscopy (SEM) with energy dispersive spectroscopy (EDS) employed to observe the change in the electrode surface and elemental composition data present on the sensor surface also showed a unique trend of elemental weight percentages. Machine learning models using hydrogen peroxide, LPS, and lactic acid electrochemical data were applied for the prediction of risk levels of periodontitis. The detection of hydrogen peroxide, LPS, and lactic acid by electrochemical biosensors indicates the potential to use these molecules as electrochemical biomarkers and use the data for ML-driven prediction tool for the periodontitis risk levels.
Effect of sulfur coverage on Fe(110) adhesion: a DFT study.

Abstract:The effect of adsorbed S at different coverages on the adhesion of Fe(110) surfaces in match and mismatch is examined using density functional theory (DFT). S is adsorbed in atop, bridge, and 4-fold hollow sites on one side of the interface in c(2 x 2) and p(1 x 1) arrangements, corresponding to coverages of 1/2 and 1 monolayer, respectively. The calculated adhesion energy values at different interfacial separations are fitted to the universal binding energy relation, and the effect of the S coverages on the adhesive strength is analyzed. The effect of relaxation of the interfaces at equilibrium is also investigated, and the resulting interfacial structures and related magnetic and charge density properties are compared.

Abstract:Quantitative analysis of up-regulated biomarkers in pathological tissues is helpful to tumor surgery yet the loss of biomarker extraction and time-consuming operation limited the accurate and quick judgement in preoperative or intraoperative diagnosis. Herein, an immobilization-free electrochemical sensing platform is developed by constraint coupling of electron transfer cascade on electrode-nanosensor interface. Specifically, electrochemical indicator (Ri)-labeled single-stranded DNA on electroactive nanodonor (polydopamine, PDA) can be responsively detached by formation of DNA complex through the recognition and binding with targets. By applying the oxidation potential of Ri, nanosensor collisions on electrode surface trigger a cascade redox cycling of PDA and Ri through synchronous electron transfer, which boost the amplification of current signal output. The developed nanosensor exhibit excellent linear response toward up-regulated biomarkers (miRNA-21, ATP, and VEGF) with low detection limits (32 fM, 386 pM, and 2.8 pM). Moreover, background influence from physiological interferent is greatly reduced by restricted electron transfer coupling on electrode. The practical applicability is illustrated in sensitive and highly repeatable profiling of miRNA-21 in lysate of tumor cells and tumor tissue, beneficial for more reliable diagnosis. This electrochemical platform by employing electron transfer cascades at heterogeneous interfaces offers a route to anti-interference detection of biomarkers in tumor tissues. Abstract:Real-time sensing of reactive oxygen species (ROS) and timely scavenging of excessive ROS in physiological environments are critically important in the diagnosis and prevention of ROS-related diseases. To solve the mismatch problem between conventional rigid ROS biosensors and biological tissues in terms of both modulus and composition, here, we present a flexible ferrocene-based hydrogel biosensor designed for oxidative stress detection and antioxidation treatment. The hydrogel was fabricated through a supramolecular assembly of ferrocene-grafted polyethylenimine (PEI-Fc), sodium alginate (SA), and polyvinyl alcohol (PVA). Multiple non-covalent interactions, including electrostatic interactions between PEI-Fc and SA, hydrophobic interactions and π–π stacking among ferrocene groups, and the PVA crystalline domain, synergistically improve the mechanical properties of the PVA/SA/PEI-Fc hydrogel. The flexible PVA/SA/PEI-Fc hydrogel biosensor exhibited a broad detection range for hydrogen peroxide (H 2 O 2 ), from 0 to 120 μM, using the differential pulse voltammetry method. Furthermore, the hydrogel demonstrated effective ROS scavenging and oxygen generation performance, desirable biocompatibility, and satisfactory antibacterial activity, making it suitable for biological interfaces. In vitro studies revealed that the PVA/SA/PEI-Fc hydrogel could monitor H 2 O 2 concentration in the proximity of inflammatory cells, and effectively scavenge ROS to protect cells from oxidative stress damage. This all-in-one multifunctional hydrogel, integrating both sensing and treatment functions, holds great promise for clinical applications in the diagnosis and management of ROS-related diseases.

Abstract:Hydrogen peroxide (H2O2) is a major messenger molecule in cellular signal transduction. It is critical to detect cellular secreted H2O2 in situ for clinical diagnosis and biomedical research. Therefore a simple, fast, sensitive, and cost-effective hydrogen peroxide detection method is needed. In this study, copper foam (CF) was used as copper source and conductive substrate, the electrochemical sensor SnO2@CuO/CF is prepared to detect H2O2 secreted by living cells. High resolution transmission electron microscopy (HRTEM), X-ray diffraction, Mott Schottky curves and X-ray photoelectron spectroscopy demonstrate the successful formation of SnO2@CuO heterostructure. Theoretical calculations confirm that SnO2@CuO heterostructure facilitates the adsorption of H2O2. The designed SnO2@CuO/CF sensor shows good sensing performance for H2O2. Under optimal conditions, the designed sensor shows a relatively wide linear range to H2O2 concentration from 1 μM to 13.88 mM, with high sensitivity (5100 μA·mM−1·cm−2) and low detection limit (0.13 μM). Cytotoxicity experiments prove that the sensor has good biocompatibility and the prepared sensor is successfully used to detect H2O2 released by HeLa cells. With superior sensitivity, anti-interference ability, and reproducibility, SnO2@CuO /CF shows the potential application of H2O2 monitoring in the field of biomedical and clinical diagnosis. This study provides a new platform for the development of highly sensitive H2O2 electrochemical sensors in practical applications.

Abstract:Hydrogen peroxide (H2O2) and hydrogen sulfide (H2S) play a key role in many cellular signaling pathways. The efficient and accurate in situ detection of H2O2 and H2S released from living cells has attracted extensive research interests. Herein, we presented a portable colorimetric and electrochemical dual-mode sensor for detection of cellular secreted H2O2 and H2S based on MOF-818 nanozyme. MOF-818 containing trinuclear copper centers not only possesses excellent peroxidase-like activity under 37 °C and acidic conditions, but also exhibits outstanding electrocatalytic reduction activity to H2O2 (reduction peak potential at −0.08 V vs Ag/AgCl). Meanwhile, the copper sites of MOF-818 can be coordinated with sulfide, reducing the catalytic activity of MOF-818. As a result, the colorimetric and electrochemical sensing of H2O2 and H2S were established based on “signal-on” and “signal-off” strategies, respectively. Additionally, the combination of a smartphone and a miniature electrochemical analyzer enables dual-mode colorimetric and electrochemical sensing platform for in situ detection of H2O2 and H2S released from live cells. This portable dual-mode sensor based on MOF nanozyme not only opens a new path for point-of-care testing, but also has potential applications in the field of cell biology and clinical diagnosis.
Cardiac venous endothelial dysfunction after myocardial ischemia and reperfusion in dogs.

Abstract:Endothelial dysfunction is a prominent occurrence in coronary arteries after myocardial ischemia and reperfusion. However, this has not been studied in coronary veins. Endothelium-dependent vasorelaxation was studied in cardiac venous rings obtained from dogs subjected to 60 min of coronary artery occlusion followed by 270 min of reperfusion, as well as from dogs subjected to sham ischemia-reperfusion. Myocardial ischemia resulted in a 96 +/- 2% decrease in coronary flow to the ischemic area 60 min after occlusion of the left anterior descending (LAD) coronary artery, which led to a significant degree of cardiac necrosis (i.e., 32.9 +/- 3.9% of area at risk). Cardiac venous rings isolated from sham ischemia-reperfusion dogs relaxed 68 +/- 3% to 200 microM ADP, 65 +/- 3% to 2 microM A23187, and 76 +/- 4% to 200 microM sodium nitrite (NaNO2). Corresponding values for cardiac venous rings isolated from ischemic-reperfused dogs were 32 +/- 3% for 200 microM ADP (P less than 0.01 vs. sham), 31 +/- 3% for 2 microM A23187 (P less than 0.01 vs. sham), and 74 +/- 3% for 200 microM NaNO2 (NS from sham). In rings from control dogs, vasorelaxation to ADP and A23187 was markedly inhibited by 4 mM NG-methyl-L-arginine (L-NMMA) and 10 methylene blue and restored after NG-nitro-L-arginine by 3 mM L-arginine. These results demonstrate that a significant degree of endothelial dysfunction occurred in cardiac venous rings after ischemia and reperfusion.(ABSTRACT TRUNCATED AT 250 WORDS)
Intermediate level training: A paradigm requiring reflective competence

Abstract:The role of echocardiography within the intensive care unit (ICU) has progressed dramatically in the past decade. It is now an expected part of the Intensivist’s armamentarium to diagnose and manage clinical conditions based on their ability to record and interpret ultrasound imaging. However, the acquisition of any new skill requires the responsibility to recognise the limitations of one’s knowledge, experience and expectations. We admitted a 64-year-old male to the ICU with a two-day history of vomiting, worsening renal function and hypotension. Initial transthoracic echo (TTE) was reported as showing; ventricular-septal-defect (VSD); dyskinesis of the IV septum; possible dissecting thoracic aortic aneurysm. However, on closer inspection of the parasternal long axis view, it was recognized that there was an abnormal echo-free space adjacent to the aorta (Figure 1); the Doppler jet originated from the echo-free space and not the left ventricle and occurred in diastole (Figures 2 and 3). The apical four-chamber view also demonstrated Doppler flow from the abnormal echo-free space to the right ventricle (RV) and right atrium (RA) (Figures 4 and 5). Transoesophageal echocardiogram (TOE) demonstrated the echo-free space to be pulsatile, representing a sinus of valsalva aneurysm (SOVA) originating near the right coronary cusp communicating with the RA/RV junction (Figure 6(a) and (b)). In a previous case, report of a ruptured SOVA being misdiagnosed as a VSD, it highlighted the need to demonstrate in which part of the cardiac cycle the abnormal jet is present (as above), however, the report also mentions the possibility of a coexisting VSD being masked by the ruptured SOVA, and another potential misdiagnosis being a coronary ateriovenous fistula. Our case and the case report highlight the learning-curve required when acquiring new skills, and the literature reinforces the danger of false positives (overcalling) and false-negatives (omission) as this skill is acquired . There is currently debate regarding the place for ‘intermediate level’ of training in echocardiography vs. simply ‘basic’ and ‘expert’; however, the desire for intermediate level training is not without risk. There has been discussion in the educational literature about the concept of the competency matrix (Figure 7). It states that a learner acquiring new skills begins at level of unconscious incompetence; however, this is not correct. Most learners commence a learning process knowing they do not know it. Therefore, they are consciously incompetent. It is as their skills progress and expectations of performance increase that learners are at risk of being unconsciously incompetent: they can actually regress to this level. Only with further training do they recognise this, and once again become consciously incompetent. The length of time learners spend in this stage depends on the strength of the stimulus to learn mastery. If a learner’s ceiling of desired mastery is ‘intermediate level’, then with no stimulus to achieve mastery at the ‘expert level’, the risk of ongoing unconscious incompetence is significant, leading to false-positives and false-negatives in interpretation. A practitioner who is at ‘intermediate level’ mastery needs to possess reflective competence to ensure that they recognise the constant potential of unconscious incompetence, and seek assistance when required. Our case demonstrated the requirement to utilise off-axis imaging and optimise Doppler settings. These skills require ‘expert level’ knowledge and experience. We are not arguing that ‘intermediate level’ training should be abandoned as a principle; however, it requires specific cognitive abilities of reflective competence to operate between the level of beginner and expert (Figure 7). Conscious incompetence: You become aware of how much you have to learn. You realise you require time and practice to progress. Conscious incompetence: You are starting to master the new skill, but you still have to actively think if you are doing it right.
