Abstract:Traditional Chinese medicine (TCM) documents have been handed down through the ages, containing rich theoretical knowledge and clinical experience. These unstructured data are the foundation for building the digital knowledge system of TCM. However, written in ancient Chinese, the TCM books have complex grammatical rules and terms which are different from modern medicine, inducing difficulty in entity annotation and recognition. In order to solve the problem of lacking labeled data, we construct a dataset with Wenbing Tiaobian, a classic work of TCM on the warm disease, identify six entities and annotate the book with the BIOES method. The BERT-BILSTM-CRF model is used to conduct experiments on the dataset with an F1 value of 91.4%. The results verify the effectiveness of the model in NER tasks and advance the construction of knowledge graphs in TCM.

Abstract:A new approach for recognizing single-channel EEG pattern induced by acupuncture on acupoint of Zusanli was presented. EEG was decomposed by ensemble empirical mode decomposition (EEMD) into a small number of intrinsic mode functions (IMFs) based on intrinsic local characteristic time scale. The power, standard deviation, and analytic amplitude of first two IMFs expressed the difference between normal EEG and EEG affected by acupuncture. The two EEG patterns could be classified by extreme learning machine (ELM) with faster and better classification performance compared with BP. Similar classification accuracy of different channels indicated the global EEG pattern affected by acupuncture.

Abstract:Inaccurate acupoint determination is one of the problems in moxibustion treatment of traditional Chinese medicine. Based on traditional Chinese medicine theory and computer stereo vision technology, we developed an intelligent treatment algorithm. Combined with attitude recognition framework, the algorithm completes automatic hole finding. Experimental results show that the algorithm performs well. Rotate the mannequin so that it is at a different angle from the camera field of view. The difference of measurement and location all met the clinical requirements.

Abstract:Introduction: Acupuncture and tuina, acknowledged as ancient and highly efficacious therapeutic modalities within the domain of Traditional Chinese Medicine (TCM), have provided pragmatic treatment pathways for numerous patients. To address the problems of ambiguity in the concept of Traditional Chinese Medicine (TCM) acupuncture and tuina treatment protocols, the lack of accurate quantitative assessment of treatment protocols, and the diversity of TCM systems, we have established a map-filling technique for modern literature to achieve personalized medical recommendations. Methods: (1) Extensive acupuncture and tuina data were collected, analyzed, and processed to establish a concise TCM domain knowledge base. (2)A template-free Chinese text NER joint training method (TemplateFC) was proposed, which enhances the EntLM model with BiLSTM and CRF layers. Appropriate rules were set for ERE. (3) A comprehensive knowledge graph comprising 10,346 entities and 40,919 relationships was constructed based on modern literature. Results: A robust TCM KG with a wide range of entities and relationships was created. The template-free joint training approach significantly improved NER accuracy, especially in Chinese text, addressing issues related to entity identification and tokenization differences. The KG provided valuable insights into acupuncture and tuina, facilitating efficient information retrieval and personalized treatment recommendations. Discussion: The integration of KGs in TCM research is essential for advancing diagnostics and interventions. Challenges in NER and ERE were effectively tackled using hybrid approaches and innovative techniques. The comprehensive TCM KG our built contributes to bridging the gap in TCM knowledge and serves as a valuable resource for specialists and non-specialists alike.
Relation Extraction Using Large Language Models: A Case Study on Acupuncture Point Locations

Abstract:In acupuncture therapy, the accurate location of acupoints is essential for its effectiveness. The advanced language understanding capabilities of large language models (LLMs) like Generative Pre-trained Transformers (GPT) present a significant opportunity for extracting relations related to acupoint locations from textual knowledge sources. This study aims to compare the performance of GPT with traditional deep learning models (Long Short-Term Memory (LSTM) and Bidirectional Encoder Representations from Transformers for Biomedical Text Mining (BioBERT)) in extracting acupoint-related location relations and assess the impact of pretraining and fine-tuning on GPT's performance. We utilized the World Health Organization Standard Acupuncture Point Locations in the Western Pacific Region (WHO Standard) as our corpus, which consists of descriptions of 361 acupoints. Five types of relations ('direction_of,' 'distance_of,' 'part_of,' 'near_acupoint,' and 'located_near') (n= 3,174) between acupoints were annotated. Five models were compared: BioBERT, LSTM, pre-trained GPT-3.5, fine-tuned GPT-3.5, as well as pre-trained GPT-4. Performance metrics included micro-average exact match precision, recall, and F1 scores. Our results demonstrate that fine-tuned GPT-3.5 consistently outperformed other models in F1 scores across all relation types. Overall, it achieved the highest micro-average F1 score of 0.92. This study underscores the effectiveness of LLMs like GPT in extracting relations related to acupoint locations, with implications for accurately modeling acupuncture knowledge and promoting standard implementation in acupuncture training and practice. The findings also contribute to advancing informatics applications in traditional and complementary medicine, showcasing the potential of LLMs in natural language processing.

Abstract:In recent years, Traditional Chinese Medicine (TCM) has attracted more and more attention due to its good therapeutic effect, low cost, and convenience. This research is also a part of the goal of the modernization of TCM. Based on the meridian electric potential acquisition system independently developed by our project team, in this paper, we designed the human body's meridian electric potential acquisition scheme. We use principal component analysis (PCA) to prove that the meridional potential signal is derived from the ECG signal. Then, Inception ResNet V2 was used to classify acupoints and nonacupoints. Finally, the classification accuracy rate reached 86.59045265, and the F1 score = 0.72161642. This shows that acupoints and nonacupoints can be distinguished by their surface potential.

Neural specificity of acupuncture stimulation from support vector machine classification analysis

Abstract:Acupoint specificity, as a crucial issue in acupuncture neuroimaging studies, is still a controversial topic. Previous studies have generally adopted a block-based general linear model (GLM) approach, which predicts the temporal changes in the blood oxygenation level-dependent signal conforming to the "on-off" specifications. However, this method might become impractical since the precise timing and duration of acupuncture actions cannot be specified a priori. In the current study, we applied a data-driven multivariate classification approach, namely, support vector machine (SVM), to explore the neural specificity of acupuncture at gall bladder 40 (GB40) using kidney 3 (KI3) as a control condition (belonging to different meridians but the same nerve segment). In addition, to verify whether the typical GLM approach is sensitive enough in exploring the neural response patterns evoked by acupuncture, we also employed the GLM method to the same data sets. The SVM analysis detected distinct neural response patterns between GB40 and KI3--positive predominantly for the GB40, while negative following the KI3. By contrast, group analysis from the GLM showed that acupuncture at these different acupoints can both evoke similar widespread signal decreases in multiple brain regions, and most of these regions were spatially overlapped, mainly distributing in the limbic and subcortical structures. Our findings may provide additional evidence to support the specificity of acupuncture, relevant to its clinical efficacy. Moreover, we also proved that GLM analysis is prone to be susceptible to errors and is not appropriate for detecting neural response patterns evoked by acupuncture stimulation.

Abstract:Acupuncture has a long history and definite effectiveness in traditional Chinese medicine. Manual acupuncture and electroacupuncture stimulate specific acupoints on meridians to obtain local and/or systemic biological effects, but the mechanism behind these effects remains unclear. In the present study, we used three-dimensional tissue-clearing technology to study four specific acupoints on the Ren meridian of mice, which is located within the anatomic landmark of the linea alba. The distribution, density, branching, and relationships between blood vessels and nerves were quantitated. The acupoints and meridian before and after electroacupuncture were visualized and analyzed together with the surrounding structures using topological Mapper methods. We found that capillary branching and sympathetic neurovascular interactions (SNIs) were denser in the CV 4 acupoint (Guanyuan) compared with surrounding non-acupoints. Furthermore, SNIs in CV 4 indicated capabilities in topological amplification, which was confirmed by high resolution in vivo dynamic vascular imaging. After electro or manual acupuncture, CV 4 showed different degrees of increased blood flow detected with NIR-II probe of LZ-1105, compared with little increased in non-acupoints. Enhancing traditional Chinese medicine diagnostics: Integrating ontological knowledge for multi-label symptom entity classification

Abstract:In traditional Chinese medicine (TCM), artificial intelligence (AI)-assisted syndrome differentiation and disease diagnoses primarily confront the challenges of accurate symptom identification and classification. This study introduces a multi-label entity extraction model grounded in TCM symptom ontology, specifically designed to address the limitations of existing entity recognition models characterized by limited label spaces and an insufficient integration of domain knowledge. This model synergizes a knowledge graph with the TCM symptom ontology framework to facilitate a standardized symptom classification system and enrich it with domain-specific knowledge. It innovatively merges the conventional bidirectional encoder representations from transformers (BERT) + bidirectional long short-term memory (Bi-LSTM) + conditional random fields (CRF) entity recognition methodology with a multi-label classification strategy, thereby adeptly navigating the intricate label interdependencies in the textual data. Introducing a multi-associative feature fusion module is a significant advancement, thereby enabling the extraction of pivotal entity features while discerning the interrelations among diverse categorical labels. The experimental outcomes affirm the model's superior performance in multi-label symptom extraction and substantially elevates the efficiency and accuracy. This advancement robustly underpins research in TCM syndrome differentiation and disease diagnoses.

Abstract:Acupuncture therapy, rooted in traditional Chinese medicine (TCM), plays a pivotal role in both disease treatment and preventive health care. A significant challenge within this realm is precise acupoint recommendations tailored to specific symptoms, with consideration of the intricate inherent relationships between the symptoms and acupoints. Traditional recommendation methods encounter another difficulty in grappling with the sparse nature of TCM data. To address these issues, we present a novel approach called the enhanced graph convolutional network with property fusion (PEGCN), which consists of two key components, the property feature graph encoder module and the enhanced graph convolutional network module. The former extracts property knowledge of acupoints to enrich their representations. The latter integrates the GCN structure and an attention mechanism to efficiently capture the underlying relationships between symptoms and acupoints. In this paper, we apply the PEGCN model to a real-world dataset related to acupuncture therapy, and the experimental results demonstrate its superiority over the baseline models in terms of the evaluation metrics, which include Precision@K, Recall@K, and NDCG@K. This finding suggests that our model effectively addresses the challenges associated with acupoint recommendations, offering an improved method for personalized treatments in the TCM context.

Automated information extraction model enhancing traditional Chinese medicine RCT evidence extraction (Evi-BERT): algorithm development and validation

Abstract:Background: In the field of evidence-based medicine, randomized controlled trials (RCTs) are of critical importance for writing clinical guidelines and providing guidance to practicing physicians. Currently, RCTs rely heavily on manual extraction, but this method has data breadth limitations and is less efficient. Objectives: To expand the breadth of data and improve the efficiency of obtaining clinical evidence, here, we introduce an automated information extraction model for traditional Chinese medicine (TCM) RCT evidence extraction. Methods: We adopt the Evidence-Bidirectional Encoder Representation from Transformers (Evi-BERT) for automated information extraction, which is combined with rule extraction. Eleven disease types and 48,523 research articles from the China National Knowledge Infrastructure (CNKI), WanFang Data, and VIP databases were selected as the data source for extraction. We then constructed a manually annotated dataset of TCM clinical literature to train the model, including ten evidence elements and 24,244 datapoints. We chose two models, BERT-CRF and BiLSTM-CRF, as the baseline, and compared the training effects with Evi-BERT and Evi-BERT combined with rule expression (RE). Results: We found that Evi-BERT combined with RE achieved the best performance (precision score = 0.926, Recall = 0.952, F1 score = 0.938) and had the best robustness. We totally summarized 113 pieces of rule datasets in the regulation extraction procedure. Our model dramatically expands the amount of data that can be searched and greatly improves efficiency without losing accuracy. Conclusion: Our work provided an intelligent approach to extracting clinical evidence for TCM RCT data. Our model can help physicians reduce the time spent reading journals and rapidly speed up the screening of clinical trial evidence to help generate accurate clinical reference guidelines. Additionally, we hope the structured clinical evidence and structured knowledge extracted from this study will help other researchers build large language models in TCM.
An Image-Based Facial Acupoint Detection Approach Using High-Resolution Network and Attention Fusion

Abstract:With the prevalence of Traditional Chinese Medicine (TCM), automation techniques are highly required to support the therapy and save human resources. As the fundamental of the TCM treatment, acupoint detection is attracting research attention in both academic and industrial domains, while current approaches suffer from poor accuracy even with sparse acupoints or require extra equipment. In this study, considering the decision-making knowledge of human experts, an image-based deep learning approach is proposed to detect facial acupoints by localising the centre of acupoints. In the proposed approach, high-resolution networks are selected as the backbone to learn informative facial features with different resolution paths. To fuse the learnt features from the high-resolution network, a resolution, channel, and spatial attention-based fusion module is innovatively proposed to imitate human decision, that is, focusing on the facial features to detect required acupoints. Finally, the heatmap is designed to integrally achieve the acupoint classification and position localisation in a single step. A small-scale real-world dataset is constructed and annotated to evaluate the proposed approach based on the authorised face dataset. The experimental results demonstrate the proposed approach outperforms other baseline models, achieving a 2.4228% normalised mean error. Most importantly, the effectiveness and efficiency of the proposed technical improvements are also confirmed by extensive experiments. The authors believe that the proposed approach can achieve acupoint detection with considerable high performance, and further support TCM automation.
Multi-target meridians classification based on the topological structure of anti-cancer phytochemicals using deep learning

Abstract:Ethnopharmacological relevance: Traditional Chinese medicine (TCM) meridian is the key theoretical guidance of prescription against tumor in clinical practice. However, there is no scientific and systematic verification of therapeutic action of herbs under meridians context. Several studies have determined the Chinese herbal medicine (CHM) phytochemicals for intrinsic attribute or meridians classification based on artificial intelligence (AI) tools. However, it is challenging to represent the complex molecular structures with large heterogeneity through the current technologies. In addition, the multiple correspondence between herbs and meridians has not been paid much attention. Aim of the study: We aim to develop an AI framework to classify multi-target meridians through the topological structure of phytochemicals. Materials and methods: A total of 354 anti-cancer herbs, their corresponding TCM meridians and 5471 ingredient compounds were collected from public databases of CancerHSP, ETCM, and Hit 2.0. The statistical analysis of herbal and compound datasets, clustering analysis of the associated cancers, and correlational analysis of meridian tropism were preliminary conducted. Then a deep learning (DL) hybrid model named GRMC consisting of graph convolutional network (GCN) and recurrent neural network (RNN) was employed to generate the meridian multi-label sequences based on molecular graph. Results: The curing herbs against tumors have tight relationships to lung, liver, stomach, and spleen meridians. These herbs behave different properties in curing certain cancer. Certain cancer types have co-occurrence such as ovarian, bladder and cervical cancer. Compounds have multitarget meridians with characteristics of higher-order correlations. Compared with the other state-of-the-art algorithms on the datasets and previous methods dealing with conventional fixed fingerprints of herbal compounds, the proposed GRMC has superior overall performance on testing dataset with the one error of 0.183, hamming loss of 0.112, mean averaged accuracy (MAA) of 0.855, mean averaged precision (MAP) of 0.891, mean averaged recall (MAR) of 0.812, and mean averaged F1 score (MAF) of 0.849. Conclusions: The proposed method can predict multi-targeted meridians through neural graph features in herbal compounds and outperforms several comparison methods. It could provide a basis for understanding the molecular scientific evidence of TCM meridians.

Abstract:The electronic medical records (EMRs) of traditional Chinese medicine (TCM) include a wealth of TCM knowledge and syndrome diagnosis information, which is crucial for improving the quality of TCM auxiliary decision-making. In practical diagnosis, one disease corresponds to one syndrome, posing considerable hurdles for the informatization of TCM. The purpose of this work was to create an end-to-end TCM diagnostic model, and the knowledge graph (KG) created in this article is used to improve the model's information and realize auxiliary decision-making for TCM disorders. We approached auxiliary decision-making for syndrome differentiation in this article as a multilabel classification task and presented a knowledge-based decision support model for syndrome differentiation (KDSD). Specifically, we created a KG based on TCM features (TCMKG), supplementing the textual representation of medical data with embedded information. Finally, we proposed fusing medical text with KG entity representation (F-MT-KER) to get prediction results using a linear output layer. After obtaining the vector representation of the medical record text using the BERT model, the vector representation of various KG embedded models can provide additional hidden information to a certain extent. Experimental results show that our method improves by 1% (P@1) on the syndrome differentiation auxiliary decision task compared to the baseline model BERT. The usage of EMRs can aid TCM development more efficiently. With the help of entity level representation, character level representation, and model fusion, the multilabel classification method based on the pretraining model and KG can better simulate the TCM syndrome differentiation of the complex cases.
Intelligent prescription-generating models of traditional chinese medicine based on deep learning

Abstract:Objective: This study aimed to construct an intelligent prescription-generating (IPG) model based on deep-learning natural language processing (NLP) technology for multiple prescriptions in Chinese medicine. Materials and Methods: We selected the Treatise on Febrile Diseases and the Synopsis of Golden Chamber as basic datasets with EDA data augmentation, and the Yellow Emperor's Canon of Internal Medicine, the Classic of the Miraculous Pivot, and the Classic on Medical Problems as supplementary datasets for fine-tuning. We selected the word-embedding model based on the Imperial Collection of Four, the bidirectional encoder representations from transformers (BERT) model based on the Chinese Wikipedia, and the robustly optimized BERT approach (RoBERTa) model based on the Chinese Wikipedia and a general database. In addition, the BERT model was fine-tuned using the supplementary datasets to generate a Traditional Chinese Medicine-BERT model. Multiple IPG models were constructed based on the pretraining strategy and experiments were performed. Metrics of precision, recall, and F1-score were used to assess the model performance. Based on the trained models, we extracted and visualized the semantic features of some typical texts from treatise on febrile diseases and investigated the patterns. Results: Among all the trained models, the RoBERTa-large model performed the best, with a test set precision of 92.22%, recall of 86.71%, and F1-score of 89.38% and 10-fold cross-validation precision of 94.5% ± 2.5%, recall of 90.47% ± 4.1%, and F1-score of 92.38% ± 2.8%. The semantic feature extraction results based on this model showed that the model was intelligently stratified based on different meanings such that the within-layer's patterns showed the associations of symptom–symptoms, disease–symptoms, and symptom–punctuations, while the between-layer's patterns showed a progressive or dynamic symptom and disease transformation. Conclusions: Deep-learning-based NLP technology significantly improves the performance of IPG model. In addition, NLP-based semantic feature extraction may be vital to further investigate the ancient Chinese medicine texts.

Method: This study adopted co-word analysis, cluster analysis and other bibliometric methods such as SATI3.2, Citespace and Gephi to visually demonstrate the annual publication volume, cooperation between institutions and co-authorship in the field of acupuncture-neuro immunoregulation research based on the data of more than 70 papers on related topics covering a period of 24 years from 1997 to 2020 in China. Results: The results show that there are three major research institutions in the field of acupuncture-neuro immunoregulation, namely Shanghai, Zhejiang major research institutions in the field of acupuncture-neuro immunoregulation, namely Shanghai, Zhejiang and Heilongjiang Universities of Traditional Chinese Medicine, as well as 7 clusters, including immune function, acupuncture therapy and endogenous opioid peptide. In the future research, a more comprehensive system will be constructed based on the gradually clear relationship between meridians and nerve regulation, humoral regulation, and immune regulation. Application practice will combine traditional acupuncture with modern medicine to create a variety of new therapies, such as electroacupuncture, electric acupuncture, and acupoint injection. Conclusion: The research focus will be expanded on how to improve the clinical efficacy of multiple combined therapies such as acupuncture for the prevention and treatment of diseases through the regulation of immune cells and immune molecules as well as the regulation of the "acupuncture-meridian- neuroendocrine-immune" network.

TCM-SD: A Benchmark for Probing Syndrome Differentiation via Natural Language Processing

Abstract:Traditional Chinese Medicine (TCM) is a natural, safe, and effective therapy that has spread and been applied worldwide. The unique TCM diagnosis and treatment system requires a comprehensive analysis of a patient's symptoms hidden in the clinical record written in free text. Prior studies have shown that this system can be informationized and intelligentized with the aid of artificial intelligence (AI) technology, such as natural language processing (NLP). However, existing datasets are not of sufficient quality nor quantity to support the further development of data-driven AI technology in TCM. Therefore, in this paper, we focus on the core task of the TCM diagnosis and treatment system -- syndrome differentiation (SD) -- and we introduce the first public large-scale dataset for SD, called TCM-SD. Our dataset contains 54,152 real-world clinical records covering 148 syndromes. Furthermore, we collect a large-scale unlabelled textual corpus in the field of TCM and propose a domain-specific pre-trained language model, called ZY-BERT. We conducted experiments using deep neural networks to establish a strong performance baseline, reveal various challenges in SD, and prove the potential of domain-specific pre-trained language model. Our study and analysis reveal opportunities for incorporating computer science and linguistics knowledge to explore the empirical validity of TCM theories.
Guidance for Acupuncture Robot with Potentially Utilizing Medical Robotic Technologies

Abstract:Acupuncture is gaining increasing attention and recognition all over the world. However, a lot of physical labor is paid by acupuncturists. It is natural to resort to a robot which can improve the accuracy as well as the efficacy of therapy. Several teams have separately developed real acupuncture robots or related technologies and even went to the stage of clinical trial and then achieved success commercially. A completed clinical practical acupuncture robot is not far from reach with the combination of existing mature medical robotic technologies. A hand-eye-brain coordination framework is proposed in this review to integrate the potential utilizing technologies including force feedback, binocular vision, and automatic prescription. We should take acupuncture prescription with artificial intelligence and future development trends into account and make a feasible choice in development of modern acupuncture.
