Abstract:Background: Following the loss of a tooth, the new edentulous area of the ridge will undergo several adaptive modifications due to changes in function within and surrounding the socket. This bone resorption explains the need for socket preservation techniques in areas of esthetic concerns and functional demands. Demineralized freeze-dried bone allograft (DFDBA) possesses greater osteoinductive potential due to the exposure of bone morphogenetic protein (BMP-3)and collagen fibrils and can be used efficiently in socket preservation techniques. DFDBA yields better results when combined with an autologous platelet concentrate, such as platelet-rich fibrin. Therefore, we formulated this randomized controlled clinical trial to assess the clinical and radiovisiographical outcomes of platelet-rich fibrin (PRF) and DFDBAs for extraction socket preservation in humans at different time intervals. Materials and methods: This was a randomized controlled trial with 100 people as study subjects, and they were randomly divided into two groups: the test group (DFDBA and PRF placed in the extraction socket) and the control group (natural healing of the extraction socket). Clinical and radiographic evaluation using radiovisiography (RVG) was done at baseline, three-month, and six-month intervals. Cone-beam computed tomography (CBCT) was used at six months to determine the bone density in the test and control groups. Results: When compared from baseline to six months, the percentage change in clinical and RVG measurements for the test group was 15.96% (11.9064 mm) and 16.77% (12.1840 mm), respectively, whereas for the control group, it was 46.09% (14.0396 mm) and 47.61% (14.5716 mm), thus indicating lesser bone resorption in the test group as opposed to the control group. CBCT values also showed greater bone density for the test group (682.3120 HU) than the control group (503.8336 HU). Conclusion: This study demonstrates the advantages of DFDBA bone graft with PRF compared to natural healing in achieving socket preservation by maintaining the marginal and buccolingual bone levels.
Alveolar Ridge Preservation in Severely Damaged Molar Socket Using a Polylactic Acid Membrane Without Primary Wound Closure: A Case Series

Abstract:Purpose: Untreated severely damaged molar sockets often complicate implant placement. This study evaluated membrane degradation, soft tissue healing, and alveolar crest dimensional changes after alveolar ridge preservation (ARP) in severely damaged molar sockets using a polylactic acid membrane (PLA) and xenograft without primary wound closure. Materials and Methods: A total of 14 damaged molar sockets in 12 patients were grafted with deproteinized bovine bone mineral, which was covered with a PLA membrane without primary wound closure. Wound healing was evaluated at set times. Three sets of CBCT data taken before surgery, at 1 to 2 weeks, and at 6 to 12 months after surgery were reconstructed and superimposed to analyze dimensional changes of the alveolar crest with volumetric imaging software. Results: Two weeks after ARP, no degradation of PLA membrane was present, and the xenograft had either no exfoliation (7 sites, 50%) or mild exfoliation (6 sites, 42.9%). One to 2 months following ARP, the membrane was partially degraded in 6 sites (42.9%) and completely degraded in 8 sites (57.1%). Nine sites (64.3%) presented with mild graft particle exfoliation, and four sites (28.6%) had granulation tissue covering the bone graft. All sites were fully covered by keratinized mucosa by 6 to 12 months. Compared with baseline measurements, the vertical bone graft height decreased by 2.2 +/- 2.2 mm, and central alveolar ridge thickness measured at the level of the virtual implant platform decreased by 5.0 +/- 5.1 mm after 6 to 12 months. Conclusion: The exposed PLA membrane maintains some barrier function for at least 3 to 4 weeks after ARP in severely damaged molar extraction sites. This technique allowed for implant placements without additional grafting. Future studies should include histologic assessment of the augmented bone and nongrafted control sockets.

Clinical Study of Tissue Preservation of Extraction Socket with Bio-Oss Collagen and Acellular Dermal Matrix

Abstract:OBJECTIVE:To evaluate the clinical and histological results of extraction socket preservation using Bio-Oss collagen and acellular dermal matrix (ADM).METHODS:From November 2005 to June 2006, a total of 11 cases underwent extraction socket preservation with Bio-Oss collagen and ADM in 14 extraction sites. Ten cases with 10 teeth undergoing extraction without bone graft served as control group. Clinical and histological examinations were conducted. Pre-and postoperative alveolar ridge width and attached gingival width were measured and statistically analyzed.RESULTS:All the grafted sockets healed uneventfully except one. Thirteen implants were placed 3 - 5 months after extraction. After 4 to 6 months permanent restorations were delivered. The mean follow-up time was 27.9 months (24 to 31 months). In the control group the sockets healed uneventfully, while hard and soft tissue collapsed obviously 3 months after extraction. No significant difference was found in the width of alveolar ridge and attached gingiva before and after extraction in the bone graft group (P > 0.05). The width of alveolar ridge before (7.69 + or - 0.48) mm and after extraction (7.68 + or - 0.48) mm and in the width of attached gingiva before (4.36 + or - 1.02) mm and after extraction (4.32 + or - 0.98) mm in the bone graft group (P > 0.05). Histological examination showed adequate new bone formation around Bio-Oss particles 3 months after operation. The specimen was free of inflammatory cells.CONCLUSIONS:It's effective to preserve the extraction sockets with Bio-Oss collagen and ADM and the short-term results were predictable.

Abstract:AimThe aim of this split-mouth randomized controlled study was to evaluate radiographic dimensional changes after tooth extraction in posterior sites treated with a ridge preservation technique or left for spontaneous healing. Materials and MethodsIn a total of 18 patients, tooth extraction in posterior sites of the upper and lower jaw was performed in a split-mouth design. The post-extraction sockets were randomly assigned to the following two treatment modalities: deproteinized bovine bone mineral (DBBM) with 10% collagen (DBBM-C) covered with a native bilayer collagen membrane (NBCM) (test group) and spontaneous healing (control group). Cone beam computed tomography (CBCT) scans were performed after extractions, 3 and 6months later. The following parameters were measured: the height of the buccal bone plate (BH), height of the palatal bone plate (PH), horizontal width of the extraction socket at 1mm, 3mm, and 5mm (HW-1, HW-3, HW-5), and the horizontal width (thickness) of the buccal bone plate at 1mm, 3mm, and 5mm (BHP-1, BHP-3, BHP-5). Statistical analysis was performed applying a nonparametric Wilcoxon signed-rank test. ResultsThe CBCT analysis showed a bone loss compared to baseline in test and control group. The measurements which have reached statistically significant differences at 6months were BH (test: -2.31% vs control: -13.11%), PH (test: -2.07% vs control: -15.32%), HW-1 (test: -17.14% vs control: -32.47%), and HW-3 (test: -11.65% vs control: -28.47%). ConclusionsThe posterior ridge preservation technique using DBBM-C covered with a NBCM is a valid approach reducing the amount of the radiographic loss in alveolar ridge dimensions.

Abstract:Objective To compare histologically the percentage of bone formation 12-20 weeks post-ridge augmentation using two different techniques. Background Tooth loss is associated with three-dimensional bone remodeling and ridge atrophy. Ridge preservation procedures can prevent alveolar bone volume loss. Different techniques and materials are used to preserve the alveolar ridge. Materials & Methods Computer-generated randomization software was used to assign two ridge preservation techniques for 11 extraction sites. In group I (type I bovine Achilles tendon collagen plugs with bioactive resorbable calcium apatite crystals (CPCAC) and ingroup II, cortico-cancellous bone chips (CCBC) mix, and an expanded polytetrafluoroethylene (ePTFE) barrier membrane were placed. The histomorphometric studies were performed using a computer-based image analysis system (IMAGE-J 1.4, National Institute of Health, Bethesda, MD, USA) to calculate the pixel area of bone tissue and the remaining bone graft material. The histomorphometric data were analyzed using a Student's t-test to compare the measurements between the two experimental groups. This parametric statistical test was employed to determine if there were any statistically significant differences in the quantitative histological parameters between the groups. Results: The sockets that received CPCAC showed a lower (31.89%) percentage of native bone surface area compared to the CCBC group (43.87%). However, the difference was not statistically significant (P< 0.05). In addition, group CPCAC showed evidence of foreign body reaction. Conclusion The CCBC graft covered with an ePTFE barrier may induce more bone formation with minimal inflammation in an extraction socket compared to a collagen plug with calcium apatite crystals. In addition, histological analysis of the CPCAC graft showed evidence of foreign body reaction, which indicates a negative clinical impact.

An open randomized controlled clinical trial to evaluate ridge preservation and repair using SocketKAP(™) and SocketKAGE(™) : part 2 - three-dimensional alveolar bone volumetric analysis of CBCT imaging

Abstract:Objectives: The aims of this study were (i) to evaluate the efficacy of ridge preservation and repair procedures involving the application of SocketKAP(™) and SocketKAGE(™) devices following tooth removal and (ii) to evaluate alveolar bone volumetric changes at 6 months post-extraction in intact sockets or those with facial wall dehiscence defects using 3-dimensional pre- and postoperative CBCT data. Materials and methods: Thirty-six patients required 61 teeth extracted. Five cohorts were established: Group A: Intact Socket Negative Control Group B: Intact Socket + SocketKAP(™) Group C: Intact Socket Filled with Anorganic Bovine Bone Mineral (ABBM) + SocketKAP(™) Group D: Facial Dehiscence Socket Negative Control Group E: Facial Dehiscence Socket Filled with ABBM + SocketKAP(™) + SocketKAGE(™) . Preoperative CBCT scans were obtained followed by digital subtraction of the test teeth. At 6 months post-extraction, another CBCT scan was obtained. The pre- and postoperative scans were then superimposed, allowing highly accurate quantitative determination of the 3D volumetric alveolar bone volume changes from baseline through 6 months. Results: Significant volumetric bone loss occurred in all sockets, localized mainly in the 0-3 mm zone apical to the ridge crest. For intact sockets, SocketKAP(™) + ABBM treatment led to a statistically significant greater percentage of remaining mineralized tissue volume when compared to negative control group. A significant difference favoring SocketKAP(™) + SocketKAGE(™) + ABBM treatment was observed for sockets with facial dehiscence defects compared to the negative control group. Conclusions: SocketKAP(™) , with ABBM, appears effective in limiting post-extraction volumetric bone loss in intact sockets, while SocketKAP(™) + SocketKAGE + ABBM appears effective in limiting post-extraction bone loss in sockets with dehiscence defects.
Healing patterns of alveolar bone following ridge preservation procedures

Abstract:Objectives: Examine the histomorphometric bone composition, following alveolar ridge preservation techniques and unassisted socket healing. Materials and methods: Forty-two patients (42) requiring a single rooted tooth extraction were randomly allocated into three groups (n = 14 per group): Group 1: Guided Bone Regeneration (GBR) using deproteinised bovine bone mineral (DBBM) and a porcine collagen membrane; Group 2: Socket Seal (SS) technique using DBBM and a porcine collagen matrix; Group 3: Unassisted socket healing (Control). Trephined bone biopsies were harvested following a 4-month healing period. Forty-two samples underwent Back-Scattered Electrons -Scanning Electron Microscopy (BSE-SEM) imaging, with 15 samples examined using Xray Micro-Tomography (XMT) (n = 6 for each GBR/SS and n = 3 Control). Images were analysed to determine the percentage (%) of connective tissue, new bone formation, residual DBBM particles and direct bone to DBBM particle contact (osseointegration). Results: BSE-SEM analysis demonstrated that new bone formation was higher in the Control (45.89% ± 11.48) compared to both GBR (22.12% ± 12.7/p < .004) and SS (27.62% ± 17.76/p < .005) groups. The connective tissue percentage in GBR (49.72% ± 9), SS (47.81% ± 12.57) and Control (47.81% ± 12.57) groups was similar. GBR (28.17% ± 16.64) and SS (24.37% ± 18.61) groups had similar levels of residual DBBM particles. XMT volumetric analysis indicated a lower level of bone and DBBM particles in all test groups, when matched to the BSE-SEM area measurements. Osseointegration levels (DBBM graft and bone) were recorded at 35.66% (± 9.8) for GBR and 31.18% (± 19.38) for SS. Conclusion: GBR and SS ARP techniques presented with less bone formation when compared to unassisted healing. GBR had more direct contact/osseointegration between the DBBM particles and newly formed bone.

Abstract:Abstract The mitochondrial genome of Pomacea canaliculata (Gastropoda: Ampullariidae) is the first complete mtDNA sequence reported in the genus Pomacea. The total length of mtDNA is 15,707 bp, which containing 13 protein-coding genes, 2 ribosomal RNAs, 22 transfer RNAs, and a 359 bp non-coding region. The A + T content of the overall base composition of H-strand is 71.7% (T: 41%, C: 12.7%, A: 30.7%, G: 15.6%). ATP6, ATP8, CO1, CO2, ND1-3, ND5, ND6, ND4L and Cyt b genes begin with ATG as start codon, CO3 and ND4 begin with ATA. ATP8, CO2-3, ND4L, ND2-6 and Cyt b genes are terminated with TAA as stop codon, ATP6, ND1, and CO1 end with TAG. A long non-coding region is found and a 23 bp repeat unit repeat 11 times in this region.

Abstract:© 2009 The Authors JEADV 2009, 23 , 954–982 Journal compilation © 2009 European Academy of Dermatology and Venereology about our published article: ‘Botulinum toxin type A for the treatment of inverse psoriasis’. We have done botulinurn toxin injections with needles of 5 mm in length with a grade of 30 ° on the cutaneous surface; this made possible a depth changeability of 1–2 mm that we thought insignificant for the local and systemic diffusion. There is no evidence in literature that the intradermic administration is more related to a clinical manifestation of botulinum-like syndrome than the intramuscular administration. The botulinum toxin therapy is a symptomatic therapy, and in the particular case of the psoriatic patients, the hypothesized modalities of the action are the blockage of the proalgogenic neuromediators release and the blockage of the cholinergie glandular activity, as described in the literature. The histological changes after the treatment were not to be foreseen because there weren’t any documented histological changes after the treatment in the patients affected with hyperhidrosis. The erythema area was defined with photographic controls and the subjective symptomatology (pain and itch) was evaluated on a visual analogue scale. We didn’t consider the Psoriasis Area Severity Index because of the limited extension of the lesions, and the score would have been extremely low to be significant for the evaluation of the seriousness of the inverse psoriasis. The patients treated with botulinum toxin for the district hyperhidrosis in extended areas, with the same modalities of the dilution and the dosage, never had a symptomatology similar to that of neuropathies with a damage of the little fibers. We underline that the botulinum toxin action is transitory, and it doesn’t damage the fibers but it reduces the neuromediators release. About the potential toxicity of the botulinum toxin, we did not find in the data of the literature, and of our experience in the observation of more than 4000 treatments of palmoplantar and axillary hyperhidrosis during the last 10 years, any severe adverse effect, cardiovascular, respiratory and neuromuscular alteration and any allergic reaction. It has to be underlined that the patients enlisted in our study were resistant to the traditional therapies and with an important local symptomatology. In these patients, the hypothesis of the application of the botulinum toxin therapy is still experimental; therefore, many studies are necessary to verify this and to compare the results of the traditional therapies.
A Randomized Controlled Trial Comparing Collagen Matrix to Hemostatic Gelatin Sponge as Socket Seal in Alveolar Ridge Preservation

Abstract:(1) Objectives: This study aimed to compare a collagen matrix to a hemostatic gelatin sponge as a socket seal in alveolar ridge preservation (ARP). (2) Methods: Systemically healthy patients planned for ARP at two sites with more than 50% of the buccal bone wall remaining after tooth extraction were eligible for inclusion. ARP involved socket grafting using collagen-enriched deproteinized bovine bone mineral. Sites were then randomly assigned to the test group (collagen matrix) or the control group (hemostatic gelatin sponge). The primary outcome was soft tissue thickness in the center of the site at 4 months, analyzed on cone-beam computed tomography. Secondary outcomes included the buccal and lingual soft tissue heights, horizontal bone loss, buccal soft tissue profile changes, wound dimensions, and Socket Wound Healing Score (SWHS). (3) Results: In total, 18 patients (12 females, 6 males) with a mean age of 57.3 years (SD 11.1) were included. Four months after ARP, the soft tissue thickness in the center of the site amounted to 2.48 mm (SD 0.70) in the test group and 1.81 mm (SD 0.69) in the control group. The difference of 0.67 mm (95% CI: 0.20–1.14) in favor of the collagen matrix was statistically significant (p < 0.009). The buccal soft tissue height was also statistically significantly higher for the collagen matrix (0.72 mm; 95% CI: 0.06–1.38; p = 0.034). A trend favoring the collagen matrix was found for the lingual soft tissue height (p = 0.066). No significant differences between the groups in terms of horizontal bone loss, buccal soft tissue profile changes, wound dimensions, and the SWHS were found. (4) Conclusions: The absence of significant differences in hard tissue outcomes suggests that both the collagen matrix and hemostatic gelatin sponge effectively sealed the extraction socket and supported bone preservation. However, the collagen matrix better maintained soft tissue dimensions. The clinical relevance of this finding with respect to the necessity for adjunctive soft tissue augmentation at the time of implant placement is yet to be studied.

Abstract:Alveolar Bone loss occurs frequently during the first six months after tooth extraction. Various studies have proposed different methods to reduce as much as possible the atrophy of the alveolar ridge after tooth extraction. Filling the socket with biomaterials after extraction can reduce the resorption of the alveolar ridge. We compared the height of the alveolar process at the mesial and distal aspects of the extraction site and the resorption rate was calculated after the application of HA/β-TCP or synthetic co-polymer polyglycolic - polylactic acid PLGA mixed with blood to prevent socket resorption immediately and after tooth extraction.

Kinetic disposition of flunixin meglumine in the camel (Camelus dromedarius).

Abstract:The kinetic disposition of flunixin meglumine was investigated in 5 camels after a single intravenous administration (1.1 mg/kg bwt). The plasma clearance was 1.012 ml.min-1.kg-1, the steady-state volume of distribution 0.168 l/kg and the elimination half-life 2.51 h. The results are discussed and compared with those reported in cattle and horses.

Abstract:Background: After tooth extraction, alveolar bone resorption is inevitable. This clinical phenomenon challenges dental surgeons aiming to restore esthetic and function. Alveolar ridge preservation can be applied to minimize dimensional changes with a new socket grafting material, an autogenous dentin graft, produced by mechanically and chemically processing natural teeth. This study assessed the safety and efficacy of using autogenous dentin biomaterial in alveolar ridge preservation. Materials and methods: Patients with nonrestorable maxillary anterior teeth bounded by natural sound teeth were included in this study. After a detailed clinical and tomographic examination, eligible participants were randomly allocated into two groups. The control group had spontaneous healing of extraction sockets. The study group had their extraction sockets filled with autogenous dentin biomaterial after processing their extracted retained roots with the KometaBio device. Standardized cone beam computed tomography (CBCT) scans were repeated four months later. A full-thickness mucoperiosteal flap reflection was achieved under local anesthesia to get core biopsies for histomorphometric analysis, and dental implants were placed at the same session. Results: A total of 32 eligible patients were included in this study (n = 16 in each group). Both groups had significantly higher facial soft tissue thickness after four months than baseline (p < 0.05). However, the study group showed statistically significant lesser dimensional changes than the control group according to the standardized CBCT scans. Furthermore, core biopsies confirmed an excellent remodeling of the autogenous dentin biomaterial in the study group. In comparison, only new thin bone trabeculae-filled sockets were in the control group. Conclusion: Autogenous dentin graft can be safely and successfully used for alveolar ridge preservation. Optimal graft remodeling histologically, better ridge dimensional stability, and uneventful wound healing support its clinical application. This trial is registered with TCTR20220615002.

After single tooth extraction in the maxillary incisor/premolar area, patients were randomly allocated to one of the following groups: 1) ridge preservation with a xenogeneic bone substitute covered with a collagen matrix (CM‐group), 2) ridge preservation with a xenogeneic bone substitute covered with a free palatal graft (PG‐group) or 3) spontaneous healing (control). Eight weeks after tooth extraction, implants were placed and clinical, profilometric and radiographic evaluations were performed. In addition, the need for further guided bone regeneration (GBR) at implant placement was assessed. The differences between the treatment groups were compared with the One‐way ANOVA or Kruskal‐Wallis test with the corresponding post hoc analysis. The proportions of the categorical parameters were compared with the Fisher´s exact test. ResultsSeventy‐five patients underwent early implant placement 8 weeks after tooth extraction and were evaluated. CM‐group (‐0.9 SD 0.6 mm) and PG‐group (‐1.0 SD 0.8 mm) showed less horizontal bone resorption compared to the control group (‐3.2 SD 2.1 mm) (p<.001). Moreover, the necessity of GBR at implant placement was significantly less in CM‐group (32%) and PG‐group (24%) when compared to control group (72%) (p=.001). Patients in CM‐group experienced less pain than PG‐group, one week after tooth extraction (p=.042). No significant differences were found regarding graft evaluation, post‐operative complications and soft tissue contour. ConclusionsRidge preservation using a xenogeneic bone substitute covered with a collagen matrix or a palatal graft, results in less bone resorption and fewer GBR procedures at early implant placement compared to spontaneous healing.

Abstract:OBJECTIVE:To evaluate bone formation in human extraction sockets with absorbed surrounding walls augmented with Bio-Oss and Bio-Gide after a 6-month healing period by histologic and histomorphometric analyses.METHODS:Six fresh molar tooth extraction sockets in 6 patients who required periodontally compromised moral tooth extraction were included in this study. The six fresh extraction sockets were grafted with Bio-Oss particle covered with Bio-Gide. The 2.8 mm×6.0 mm cylindric bone specimens were taken from the graft sites with aid of stent 6 months after the surgery. Histologic and histomorphometric analyses were performed.RESULTS:The histological results showed Bio-Oss particles were easily distinguished from the newly formed bone, small amounts of new bone were formed among the Bio-Oss particles, large amounts of connective tissue were found. Intimate contact between the newly formed bone and the small part of Bio-Oss particles was present. All the biopsy cylinders measurement demonstrated a high inter-individual variability in the percentage of the bone, connective tissues and Bio-Oss particles. The new bone occupied 11.54% (0-28.40%) of the total area; the connective tissues were 53.42% (34.08%-74.59%) and the Bio-Oss particles were 35.04% (13.92%-50.87%). The percentage of the particles, which were in contact with bone tissues, amounted to 20.13% (0-48.50%).CONCLUSION:Sites grafted with Bio-Oss particles covered with Bio-Gide were comprised of connective tissues and small amounts of newly formed bone surrounding the graft particles.
