Abstract:BACKGROUND Esophageal bronchogenic cyst（EBC） is a rare congenital disease that is difficult to diagnose preoperatively, and treatment remains controversial.CASE SUMMARY We report a 53-year-old Chinese woman hospitalized in our hospital following the discovery of a submucosal protruding mass of the esophagus by upper endoscopy. A preliminary diagnosis of EBC was made by endoscopic ultrasonography（EUS）, and treatment was accomplished by endoscopic submucosal tunnel dissection（ESTD）. The pathological results verified the diagnosis. No scar changes or cystic lesion within the original lesion were found under EUS after a 3-mo follow-up.CONCLUSION EUS is valuable for the preliminary diagnosis of EBC and surveillance. ESTD is a safe and effective treatment for EBC. Further evaluation of complications and long-term follow-ups are required.
Endoscopic Submucosal Dissection for the Treatment of Large Gastric Submucosal Tumors Originating from the Muscularis Propria Layer: a Single Center Study.

Abstract:BACKGROUND AND AIMS:This retrospective study was designed to evaluate the safety and efficacy of endoscopic submucosal dissection (ESD) for the resection of gastric submucosal tumors (SMTs) originating from the muscularis propria (MP) layer at our institution.PATIENTS AND METHODS:From January 2010 to January 2014, a total of 49 patients underwent ESD for the resection of MP-originating gastric SMTs. The characteristics of the 49patients, clinical efficacy, safety of ESD, and post-ESD pathological diagnoses were evaluated retrospectively.RESULTS:ESD was successfully performed in 48 cases, and one case was converted to surgery. Pathological diagnosis included 43 gastrointestinal stromal tumors, 5 leiomyomas, and 1 schwannoma. During the procedure, 9 patients developed perforation. These patients recovered after conservative treatment. No residual or recurrent tumors were detected during the follow-up period.CONCLUSIONS:ESD seems to be a feasible, safe, effective and minimally invasive procedure, even for the resection of large gastric tumors originating from the MP. However, more studies should be carried out in the future.
Clinical analysis of endoscopic resection for submucosal tumors in the esophagogastric junction

Abstract:Objective: To investigate the clinical application of endoscopic dissection technique for submucosal tumors (SMTs) in the esophagogastric junction (EGJ). Methods: A total of 43 cases of SMTs in the EGJ were treated by endoscopic resection in the Endoscopy Center of the First Affiliated Hospital, College of Medicine, Zhejiang University between January 2010 and October 2013. The clinical and histopathologic features, surgical approach, en bloc resection rate, complications, and postoperative follow-up information were evaluated. Results: There were 19 males and 24 females with a mean age of 52.0±11.0 years old. The en bloc resection rate was 100%. The mean lesion size was 27.81±19.51 mm. The mean procedure time was 66.97±30.53 minutes and the mean bleeding volume during operation was 4.65±12.12 mL. Perforations occurred in 4 patients, pneumoperitoneum in 2 patients, and esophago-mediastinal fistula in 1 patient. All of them were successfully managed with conservational therapy. One patient with severe pneumothorax was treated with thoracic drainage. Pathologic examination showed that the lesions were leiomyoma (n = 19), gastrointestinal stromal tumor (n = 14), unknown pathological type of spindle cell tumor (n = 5) and intermuscular lipoma (n = 3), and mucinous cystadenoma (n = 1). No local recurrence and distant metastasis were noted during the follow-up. Conclusion: Various endoscopic resection methods based on endoscopic submucosal dissection seem to be safe and effective for SMTs in the EGJ. DOI:10.3781/j.issn.1000-7431.2014.04.008

Abstract:Heterotopic gastric mucosa (HGM) in the rectum is an extremely rare clinical entity which may be missed or misdiagnosed due to a lack of knowledge. In the present study, a 14-year-old girl visited our hospital due to a 5-year history of repeated hematochezia. Colonoscopy showed a solitary superficial depressed lesion approximately 5 cm in size and a concomitant 1.5 cm deep diverticulum in the rectum. Histological examination of the endoscopic biopsy showed typical ectopic gastric mucosa in the depressed lesion and inside the diverticulum. Narrow band imaging further confirmed the histological results. Endoscopic ultrasound indicated that the lesion originated from the mucosal layer, and partially involved the submucosal layer. Endoscopic submucosal dissection was performed in this patient due to the large size and shape of the lesion. No bleeding, perforation or other adverse events were observed. The presence of HGM in the diverticular cavity greatly increased the surgical difficulty. A literature review was also carried out in our study.

Abstract:Esophageal epiphrenic diverticulum (EED) in the distal 10 cm of the esophagus usually occurs in patients with esophageal motility disorders including achalasia. Per-oral endoscopic myotomy (POEM) is an effective and minimally invasive method that has developed into an important treatment for achalasia [1]. In 2016, we created submucosal tunneling endoscopic septum division (STESD) as a novel technique for treating Zenker’s diverticulum [2]. Here we present a case of achalasia with a large epiphrenic diverticulum that we cured with POEM and STESD together using only one submucosal tunnel.

Abstract:Early squamous cell carcinoma (SCC) in esophageal diverticula is rare, with an unclear pathogenesis [1]. Endoscopic septotomy is used to treat patients at high risk for surgical resection of a diverticulum [2], and endoscopic submucosal dissection (ESD) is recommended for selected early esophageal carcinoma [3]. ESD for early cancer in diverticula is challenging for endoscopists, as diverticula have characteristically thin walls and endoscopic treatment carries a higher risk of perforation [1] [4]. Here, we report a case of ESD curing early esophageal SCC in diverticula.
Guidewire-assisted piecemeal resection of a giant gastric tumor

Abstract:A 55-year-old woman underwent esophagogastroduodenoscopy, which revealed a large subepithelial lesion located at the gastroesophageal junction, presenting a "horseshoe" morphology ([Fig. 1]). A subsequent computed tomography (CT) scan and endoscopic ultrasonography indicated the lesion was a solid mass protruding into the lumen. The patient then underwent endoscopic submucosal resection, resulting in complete excision of the lesion. Due to the considerable size of the tumor, traditional snare-based extraction was unfeasible. Therefore, an innovative slicing technique was employed ([Video 1]). To implement this technique, a transparent cap was affixed to the distal end of the endoscope. A guidewire, ingeniously shaped into a semicircle ([Fig. 2]), was inserted into the working channel. Under direct visualization, the waist of the tumor was ensnared and repositioned anteriorly, aligned with the transparent cap. The guidewire was then meticulously retracted, enabling precise cold cutting of the tumor ([Fig. 3]). This process was iteratively executed, allowing sequential removal of divided tumor fragments ([Fig. 4]). Subsequent histopathological analysis confirmed a leiomyoma. This technique, while previously reported for disintegration of robust and oversized gastric bezoars [1], had not found prior application for excision of sizable, non-extractable gastric masses. With the development of endoscopic excision techniques, comprehensive resection of enormous gastric leiomyomas is now achievable [2] [3]. However, to prevent and manage post-resection intestinal obstruction from tumor migration and obtain definitive histopathological diagnosis, guidewire-assisted cold cutting emerges as an efficacious approach. An Esophageal Submucosal Tumor Treated with Submucosal Tunneling Endoscopic Resection: an Unexpected Result

Abstract:A 66-year-old female patient underwent esophagogastroduodenoscopy (EGD) in our center because of occasional dysphagia, and a large submucosal tumor (SMT) was identified 32–36 cm from the incisors, occupying nearly half of the luminal circumference (Figure 1A). Mini-probe endoscopic ultrasonography (EUS) revealed a hypoechoic submucosal lesion, measuring 20 15 mm and originating from the superficial muscularis propria layer (Figure 1B). It was covered by normal-appearing mucosa. The patient underwent EUS (SU-9000, Fujifilm, Japan) for further investigation (Figure 1C). EUS elastography showed high tissue stiffness with a predominantly blue pattern (Figure 1D) and there was no enhancement after injection of intravenous contrast. Overall, these findings were consistent with those of a submucosal tumor (SMT) of the lower esophagus originating in the muscularis propria, possibly a leiomyoma. The patient underwent en bloc resection of the tumor by submucosal tunneling endoscopic resection (STER). The first step of the procedure consisted of a submucosal injection 5 cm proximally to the lesion. A longitudinal mucosal incision and subsequent submucosal tunneling were performed using the Hybrid Knife (ERBE, Germany). Endoscopic en bloc resection was then carried out using both the Hybrid Knife and the IT Knife (Olympus, Japan). After retrieval of the endoscopic specimen measuring 40 x 20 mm (Figure 1E) and adequate hemostasis of the tunnel, the incision site was closed using hemostatic clips. However, histology revealed a poorly differentiated neuroendocrine carcinoma (small-cell carcinoma) invading the submucosal layer with lymphovascular invasion (Figure 1F). Immunohistochemistry staining was positive for CD56, synaptophysin, CgA and CK-PAN and negative for SSTR2, SSTR5, and p53. The Ki-67 score exceeded 60%. The patient was suggested to receive further surgical treatment. However, she did not seek any treatment. On a 3-month follow-up, investigation revealed distant metastases to both liver lobes and the left chest wall so the patient was referred for palliative care.
Submucosal tunneling endoscopic resection for large symptomatic submucosal tumors of the esophagus: A clinical analysis of 24 cases.

Abstract:Background/Aims: Studies evaluating submucosal tunneling endoscopic resection (STER) for the treatment of upper gastrointestinal submucosal tumors (SMTs) have recently increased. However, the efficacy and safety of STER for the treatment of large symptomatic SMTs in the esophagus have not been well investigated. The aim of the present study was to evaluate the efficacy and safety of STER for the treatment of large symptomatic SMTs in the esophagus. Methods: A total of 24 patients with large symptomatic SMTs in the esophagus who underwent STER in our hospitals between January 2015 and May 2018 were included in the study. The tumors were confirmed to be of muscularis propria layer origin. Treatment outcomes, complications, and follow-up results were retrospectively analyzed. Results: All 24 lesions were resected en bloc with STER. The mean maximum transverse diameter of the lesions was 4.7 (3.5-6.5) cm. The mean maximum longitudinal diameter of the lesions was 2.1 (1.5-3.0) cm. The mean duration from mucosal incision to complete mucosal closure was 65 (50-115) min. Postoperative pathological diagnosis confirmed 18 cases with leiomyomas, 4 cases with stromal tumors, and 2 cases with schwannomas. There were no major complications. There were no residual lesions or disease recurrence during follow-up. Conclusion: STER is safe and effective for the treatment of large symptomatic SMTs of muscularis propria layer origin in the esophagus.
Unusual Case of a Protruding Mass in the Esophagus Successfully Treated by Endoscopic Submucosal Dissection

Abstract:A 65-year-old man presented with a history of progressive dysphagia for 1 month. His physical examination and blood tests showed no abnormalities. He underwent upper gastrointestinal endoscopy three times, which showed a bulky protruding mass in the esophagus at 34 – 39 cm from the incisors ([Fig. 1]). Pathologic findings all revealed inflammatory hyperplasia of epithelial cells and subepithelial granulation tissue. A contrast-enhanced computed tomography (CT) scan showed an intraluminal mass in the lower esophagus ([Fig. 2]), with no evidence of mediastinal lymphadenopathy or any metastases in the chest or abdominal organs. After a multidisciplinary team discussion, we performed endoscopic submucosal dissection (ESD) ([Video 1]), and the lesion was successfully resected en bloc ([Fig. 3]).
Total removal of a large esophageal schwannoma by submucosal tunneling endoscopic resection: A case report and review of literature

Abstract:BACKGROUNDPrimary schwannoma is a rare submucosal tumor of the esophagus, which is most often benign, and surgery is the only effective treatment. So far, only a few cases have been reported. Herein, we reported a single case diagnosed with primary esophageal schwannoma that was totally removed by submucosal tunneling endoscopic resection (STER).CASE SUMMARYA 62-year-old man presented to the hospital with a history of resection of a malignant gastric tumor and mild dysphagia. Endoscopic examination revealed a large submucosal elevated lesion in the esophagus 25-30 cm from the incisors. Endoscopic ultrasonography detected a 45 mm x 35 mm x 31 mm hypoechoic lesion; chest computed tomography showed a mass of approximately 55 mm x 35 mm x 29 mm. A preliminary examination showed features suggestive of a stromal tumor. Pathological findings indicated esophageal schwannoma. Next, STER alone was performed to completely resect the mass, and the patient recovered well post-surgery. Afterward, the patient was discharged and showed no tumor recurrence at 33 mo of follow-up.CONCLUSIONEndoscopic resection is still an effective treatment for large esophageal schwannomas (> 30 mm) under meticulous morphological evaluation.
Submucosal Tunnel Endoscopic Resection for Extraluminal Tumors: a Novel Endoscopic Method for En Bloc Resection of Predominant Extraluminal Growing Subepithelial Tumors or Extra-Gastrointestinal Tumors (with Videos)

Abstract:Background and Aims: The management of subepithelial tumors with a predominant extraluminal growth pattern or extra-GI tumors can be challenging and traditionally requires a surgical resection that is not only invasive but may carry a significant risk of morbidity and mortality. We aimed to assess the feasibility, safety, and efficacy of a novel endoscopic technique termed submucosal tunnel endoscopic resection for extraluminal tumors (STER-ET). Methods: We prospectively enrolled patients who underwent STER-ET for GI subepithelial tumors with a predominant extraluminal growth pattern or extra-GI tumors located at the level of cardia or the proximal part of the lesser curvature of the stomach seen on cross-sectional imaging between January 2016 and March 2017. Results: Eight patients underwent STER-ET. The mean (+/- standard deviation) tumor size was 2.8 +/- 0.6 cm and 2.3 +/- 0.8 cm in longest and shortest dimension, respectively. The average procedure time was 67 +/- 4.4 minutes. The rates of curative en bloc resection and en bloc retrieval was 100% and 87.5%, respectively. On final histology, 6 tumors were GI stromal tumors, 1 was a schwannoma, and 1 was a foregut cyst. Five patients had capnoperitoneum during the procedure and required abdominal decompression. One patient had a small mucosotomy successfully treated with a hemostatic clip. There were no major adverse events or deaths. The median length of hospital stay was 3 days. There was no residual tumor on surveillance imaging after a mean follow-up period of 10.0 +/- 2.1 months. Conclusions: STER-ET is a novel technique that appears to be safe and effective in achieving a curative resection for GI subepithelial tumors with a predominantly extraluminal growth pattern or extraluminal tumors in a selected group of patients. However, larger studies are required to validate our finding.

Abstract:BACKGROUND: Submucosal tunneling endoscopic resection (STER) is regarded as a promising method for resection of submucosal tumors (SMTs); however, little is known about a comprehensive comparison of STER and thoracoscopic enucleation (TE). The aim of this study was to compare the clinical outcomes of STER and TE for large symptomatic SMTs in the esophagus and esophagogastric junction, as well as to analyze the factors that affect the feasibility and safety of STER. STUDY DESIGN: We enrolled 166 patients with large symptomatic SMTs in the esophagus and esophagogastric junction from September 2011 to March 2016 in this retrospective study. The clinicopathologic features and treatment results were collected and analyzed. RESULTS: En bloc resection was achieved in 84.6% of the patients in the STER group and 86.7% of the patients in the TE group (p = 0.708). Notably, the procedure time and hospital stay in the STER group were considerably shorter than those in the TE group. Tumor transverse diameter is a significant risk factor for piecemeal resection, adverse events, and technical difficulties. No recurrence or metastasis was found during a mean follow-up period of more than 2 years. CONCLUSIONS: Submucosal tunneling endoscopic resection is effective and safe for large SMTs in the esophagus and esophagogastric junction. This procedure has the advantage of being more minimally invasive with a shorter procedure time and hospital stay compared with TE. Submucosal tunneling endoscopic resection for tumors with a transverse diameter >= 3.5 cm and an irregular shape is associated with relatively high risk for piecemeal resection, adverse events, and technical difficulties. Abstract:Gastric submucosal tumor (SMT) originated from deep layers of the gastric wall can be resected by EFTR or STER. However, the lesions are usually in a tangent position, and the endoscopic view from gastric lumen is inherent poor. In addition, the highly movable tumor with limited endoscopic exposure increases the difficulty in the en bloc resection as well as safe hemostasis. Herein, we developed a modified method, called endoscopic intraperitoneal subserosal dissection (EISD), for the removal of gastric SMT with predominately extraluminal growth pattern. The specific steps of our technique were as follows. First, 1.5-cm gastric mucosal incision was created about 4-cm proximal to the lesion after submucosal injection. Subsequently, a short submucosal tunnel was created, and the gastric muscularis propria was then intentionally perforated about 3-cm away from the lesion. After locating the lesion on the serosa from the abdominal cavity, the tumor was carefully dissected from the serosa and the underlying muscularis propria without interruption of the tumor capsule. Subserosal injection was performed when necessary to create working space or to identify the layer. Caution should be taken to avoid the damage of the integrity of the mucosa during the dissection. The lesion was then removed after full dissection. Finally, after careful hemostasis, the tunnel entrance was closed. The video showed the two EISD cases of stromal tumor in the gastric body originated from the deep muscularis propria with predominantly extraluminal growth. The operating time was 70 and 65 minutes, respectively. The follow-up endoscopy and CT showed satisfactory healing with no residual tumor.
A case of early esophageal cancer with pharyngeal stenosis treated by endoscopic submucosal multi-tunnel dissection using an ultra-thin endoscope

Abstract:A 70-year-old man presented with early-stage esophageal cancer located in a semicircle in the upper esophagus. The hypopharynx was getting narrower owing to postoperative chemoradiotherapy for past pharyngeal cancer, and it was impossible to pass the pyriform sinus using a conventional scope. Therefore, we performed endoscopic submucosal multi-tunnel dissection, which has been shown to be effective for large esophageal cancers We used an ultra-thin endoscope (EG-L580NW7; Fujifilm, Tokyo, Japan) that could pass through the stenosis. The diameter of the endoscope was 5.8 mm and that of the working channel diameter 2.4 mm. We further used small-caliber endoscopic submucosal dissection (ESD) devices, the SOUTEN endoknife (Kaneka Medix, Tokyo, Japan) ([Fig. 1]) and the RAICHO2 hemostatic forceps (Kaneka Medix) ([Fig. 2]). The distal attachment placed on the ultra-thin endoscope tip was created by hand using transparent tape [1]. A local injection (25 gauge needle, 3 mm; TOP, Tokyo, Japan) was performed using glycerol. The electrosurgical unit (VIO 300 D; ERBE, Tübingen, Germany) was set to Endocut Mode I (effect 3, duration 2, interval 2) for mucosal incision and forced coagulation mode (effect 2, 45 W) for submucosal dissection. We created two tunnels and then resected the submucosa between the tunnels ([Fig. 3]) ([Video 1]). The ultra-thin endoscope made it possible to resect the esophageal lesions with pharyngeal stenosis [2]. But its thinness sometimes resulted in unsteady movement. Endoscopic submucosal multi-tunnel dissection allowed the endoscope itself to be fastened by a tunnel, making it possible to stabilize the maneuverability and the operative field even with an ultra-thin endoscope, allowing ESD to be completed ([Fig. 4]). There is no previous report of endoscopic submucosal multi-tunnel dissection using an ultra-thin endoscope. Using this technique, ESD can be performed on large esophageal lesions even in situations where only an ultra-thin endoscope is available. This procedure has the potential to reduce the risk of perforation and mediastinitis. The patient is under general anesthesia with endotracheal intubation. Prophylactic intravenous antibiotics are introduced 30 minutes before the procedure. A transparent cap is attached to the tip of the gastroscope and CO2 is used routinely during the operating procedure. The STESD procedure includes four steps (Figure 1). Submucosal injection is performed 3 cm proximal to the diverticular septum and a 1.5-2.0 cm longitudinal mucosal incision is performed at the tunnel entry. A submucosal longitudinal tunnel is created by using a technique similar to endoscopic submucosal dissection with a Hybrid knife (ERBE; Erbe Elektromedizin GmbH, Tubingen, Germany) between the mucosal and muscular layers. Tunneling is performed at both sides of the septum and ends 1-2 cm distal to the bottom of the diverticulum to ensure a satisfactory endoscopic view and enough working space for the myotomy. Precaution is taken to avoid injuring the overlying mucosa during tunneling. Under direct endoscopic view, cricopharyngeal muscle fibers of the septum are completely dissected down to the bottom of the diverticulum and further into the normal esophageal muscle. After careful hemostasis, the mucosal incision site is closed with several hemostatic clips. Any mucosal injury or perforation should also be clipped. Video 1 shows STESD for the treatment of ZD in a 45-year-old woman. This patient had complained of dysphagia for >10 years and showed worsening of symptoms over the past 2 years. Esophagogastroduodenoscopy and barium swallow revealed a ZD 18 cm from the incisors. The diverticulum was successfully treated by STESD with no procedure-related adverse events (Figure 2). Intravenous antibiotics were continued for 2 days postoperatively. Clear fluids were resumed at postoperative day 2 and the patient was discharged at postoperative day 3. Her symptoms resolved completely during follow-up. The 2-month follow-up esophagogastroduodenoscopy and barium swallow confirmed the disappearance of the diverticulum and recovery of the esophageal anatomy. STESD is a new branch of submucosal endoscopy that uses a submucosal tunnel as an operating space. Compared with a conventional endoscopic procedure, the major advantage of STESD is the maintenance of mucosal integrity when selectively dividing the muscular septum. This method will possibly reduce the risk of postoperative leakage and secondary infection. Even if mucosal injury or perforation occurs during STESD, usually it can be closed easily. Another potential advantage of STESD is that the muscle layer between the esophageal wall and diverticulum can be cut off completely. During conventional endoscopic treatment, there are significant concerns about aggressive dissection of the whole septum. The incompletely dissected septum is likely to result in a residual diverticulum and uncut cricopharyngeal muscle fiber, and may lead to residual symptoms or recurrence. In contrast, using the STESD technique, complete septum dissection can be performed, even into the normal esophageal muscle due to the maintenance of mucosal integrity. Owing to the narrow space, it is often very difficult to get adequate exposure of the septum and protection of the esophageal wall. STESD exploits the tunnel space to clearly expose orientation of the septal wall and the endoscope is stable inside the submucosal tunnel, which facilitates the procedure, providing full access to the myotomy, and further leading to an accurate and complete septum division. Abstract:OBJECTIVE:To evaluate the safety and efficacy of submucosal tunneling endoscopic septum division (STESD) for escophageal diverticulum.METHODS:Clinical data of six consecutive patients with symptomatic esophageal diverticula who received STESD in Endoscopy Center of Zhongshan Hospital, Fudan University from April 2016 to November 2016 were analyzed retrospectively. STESD was performed as following: mucosal entry was made 3 cm from the septum of esophageal diverticulum; submucosal tunnel was created towards the septum; after the satisfactory exposure of the septum, endoscopic division was made down to the bottom of the diverticulum; mucosal closure of the tunnel entry was made. The symptoms were scored using a system modified according to Eckardt score, namely dysphagia, heartburn, regurgitation, weight loss and retrosternal pain with each ranging from 0 to 3 (maximum score 15, minimum score 0, the higher the score, the more severe the symptoms).RESULTS:There were four males and two females with median age of 56.5 (range 50-67) years. Four patients were epiphrenic diverticula, and the other two were Zenker's diverticula. Median duration of disease was 2.5 years (range 5 months-29 years). No previous treatment was attempted. All the patients completed STESD successfully. The median septum division length was 2.5(1-4) cm. The median number of metallic clips for mucosal closure was 5(2-6). The median operation time was 51.5 (33-135) min. No major adverse events, such as perforation or bleeding were found in perioperative period. The median time of hospital stay was 5(3-9) days. All the patients had symptom relief after operation. One patient with Zenker's diverticulum reported foreign body sensation after operation and experienced relief two weeks afterwards. During a median follow-up time of 5(4-10) months, the median symptom score of 6 cases was 4.5 (1-13) before and 0.5 (0-4) after operation. The symptom scores went down to zero in 3 patients (preoperative scores 13, 1, 1, respectively), and down to 1 in 2 patients with main symptom of backflow (preoperative scores 5, 4, respectively). One patient with 29 years history of disease did not report obvious improvement in symptoms (preoperative and postoperative scores 5, 4, respectively).CONCLUSION:Submucosal tunneling endoscopic septum division is efficient and safe to relieve symptomatic esophageal diverticulum in short term.

Abstract:Background This study aimed to compare the clinical outcomes between submucosal tunneling endoscopic resection (STER) and endoscopic submucosal dissection (ESD) for large subepithelial esophageal lesions (SELs) and analyze risk factors for perforation and piecemeal resection. Methods The clinicopathological features and outcomes of endoscopic treatment of 56 patients with SELs with diameters >= 30 mm, diagnosed between June 2017 and December 2020, were reviewed in this retrospective cohort study. Patients were divided into two groups (ESD group and STER group). Results The complete resection rates of the STER and ESD groups were 88.1% and 78.6%, respectively (p = .398). The operation time of STER was longer than ESD (p = .03), while the hospital stay of STER was shorter than ESD (p = .02). The rate of major adverse events associated with ESD was considerably higher than STER group (p = .035). The extraluminal growth pattern was a risk factor for piecemeal resection, and ESD was an independent risk factor for perforation. Regarding tumors with extraluminal growth patterns, the ESD group's perforation rate was significantly higher than the STER group (p = .009). There were no recurrence or metastases found during a mean follow-up of 24.4 months. Conclusion The STER technique has advantages of shorter hospital stays and fewer major adverse events than ESD. The extraluminal growth pattern seems to be a risk factor for piecemeal resection in both ESD and STER. STER appears to be a preferable choice for large SELs with extraluminal growth patterns.

Abstract:Background: Submucosal tunneling endoscopic resection (STER) is widely applied for treatment of gastrointestinal submucosal tumors (SMTs) originating from the muscularis propria layer. However, the tumor location within the proximal esophagus makes STER a challenge for the endoscopists. The aim of this study was to summarize the technique skill and evaluate the outcomes of proximal esophageal STER. Study design: A total of 72 patients with SMTs in the proximal esophagus undergoing STER were included from February 2019 to March 2021. Imaging 3-dimensional reconstruction was used for patients with large SMTs. Clinicopathological, endoscopic, and follow-up data were collected and analyzed. Results: In this study, all the tumors were removed completely and no gross disease was remaining. The en bloc resection was achieved in 90.28% of patients, and the complications rate was 6.95%. Three-dimensional reconstruction was used for 30 patients (41.67%) with large SMTs (transverse diameter >2.0 cm). Based on statistical analysis, tumors with irregular shape and larger size were the significant contributors to piecemeal resection. Larger tumors increase the risk of long operation time, and irregular tumor shapes increase the risk of complications. The median hospitalization time was 4 days. All of the complications were cured by conservative treatment. A median follow-up of 12 months was available, and all patients were free from local recurrence or distant metastasis during the study period. Conclusions: STER is an effective and safe methodology for the resection of proximal esophageal SMTs. Tumor size and shape mainly impact the piecemeal resection rate, STER-related complications, and procedural difficulty.

Abstract:Lateral spreading lesions (LSLs) of the duodenum sharing morphological and possibly biological commonalities with laterally spreading tumors (LSTs) of the colon usually encountered in the second part. Piecemeal EMR is commonly used for large such lesions (≥20mm), but its recurrence rate is high without precise pathological evaluation. Endoscopic submucosal dissection (ESD) can achieve complete resection regardless of lesion size, but it remains technical challenging for most endoscopists to operate in the duodenum. Studies on ESD for large LSLs are few . We reported our experience on therapeutic outcomes of ESD for for laterally spreading lesions larger than 20mm located at the second part of the duodenum. Patients with a laterally spreading lesion at the second part of the duodenum and measuring ≥20mm were included from March 2017 to August 2019 in our hospital. We carried out ESD for them after plenary examinations and followed them regularly. A total of consecutive 19 eligible patients with 19 lesions were performed ESD by the same highly skillful endoscopist (Dr Meidong Xu).The average lesion size was 2.6cm (ranging from 2.0-4.0 cm). The mean procedure time was 58.6min (20-117min), All 19 lesions were resected completely in one piece without involvement of the horizontal and vertical margins. No delayed bleeding was observed. Despite prophylactic clipping, two patients occurred delayed perforation nearly 24 hours after the ESD procedure and both of them received emergent open surgery because of diffuse peritonitis. Finally, Each patient made a full recovery and was discharged 2-3 months later without further adverse event. NO local recurrence was observed during the follow-up periods. Endoscopic submucosal dissection for laterally spreading lesions larger than 20mm located at the second part of the duodenum achieved satisfactory therapeutic outcomes in our experience. However, it demands performance by a highly skilled endoscopists due to associated procedural difficulties and high risks of complications.
