Abstract:Importance: Successful surgical correction of nasal tip position requires preoperative analysis. Perceived adequate nasal tip position depends on its relationship with other facial features. Nasal tip position can be affected by the subnasal contour, proportion of facial height, and relative facial size and shape. The relation of these factors to the nasolabial angle may not be as important as previously believed. Objective: To investigate and compare the factors affecting the ideal location of the tip of the nose in Asian patients using standard photographic measurements. Design: We analyzed measurements of profile photographs and compared different factors that affect nasal tip location, including the nasofrontal, nasolabial, nasomental, and Legan angles and the ratios of the dorsal height to tip projection, the radix height to tip projection, the glabella to subnasale, and the subnasale to mentum. Setting and participants: One hundred men, 100 women, 20 to 40 years old, seen for rhinoplasty at the Department of Otorhinolaryngology, Chosun University College of Medicine. Main outcome and measures: Contribution of different facial measurements to the ideal nasal tip position. Results: Nasolabial and nasomental angles exerted a statistically significant effect on ideal nasal tip position, whereas the Legane angle, the ratio of midface to lower face, and the ratio of nasal length to lower face did not show significant effects on nasal tip position. Conclusions and relevance: The nasolabial and nasomental angles have important effects on ideal nasal tip position and should be considered together during preoperative evaluation of the location of the nasal tip. Concurrent genioplasty should optimize rhinoplasty outcomes in appropriately selected patients. Level of evidence: NA.
The Ideal Nasal Tip Projection and Rotation Angles in Chinese: A Preference Analysis of the General Population

Abstract:Rhinoplasty to reshape the nasal tip is increasingly popular among Chinese women. Aesthetic standards vary across different ethnic groups and it is key to identify preferences for the ideal nasal tip in China to set surgical goals. Therefore, we administered an online survey to plastic surgeons and the public through social media to rank nasal tip images by aesthetic preference. Images were created from a single photograph to show various dimensions of nasal tip projection to nasal dorsum length ratio (NTP/NDL) and nasal labial angle (NLA). Preferences were compared by age, sex, living area, ethnic background, occupation, and history of plastic procedures on respondents' preferences. Overall, there were 703 respondents, including 441 (63%) women and 50 plastic surgeons. Nasal tip projection to nasal dorsum length ratio of 0.63 was ranked highest by all demographic groups, including women (47%), men (50%), and plastic surgeons (66%). Nasal labial angle of 106° was first choice overall and preferred by 34%, 34%, and 52%, respectively. Preferences followed a bell curve for NTP/NDL and NLA, with lower rates of preference as parameters diverged further from the first choice. The preference for NTP/NDL of 0.63 and NLA of 106° was conserved across surgeons, lay people, and all demographic groups. The authors suggest that these proportions could be used as reference for preoperative design in rhinoplasty.

Abstract:Objective The purpose of this study is to analyze the nasal morphological characteristics in Chinese adult, of Han nationality, by the measurements in sagittal plan and the proportionality between nose and facial structures, in order to provide aesthetic references for rhinoplasty, and individual surgical planning. Methods During 2017 November to December,258 healthy Han Youth volunteers were included. Three-dimensional (3D) models of the facial characteristic information were collected using Artec 3D scanner, by standardized procedures. The nasal and facial distances and angulations were measured, based on predesigned facial landmarks. Thereafter, the proportions of above measurements were calculated. Results There was no significant difference between genders in absolute prominence. As for relative prominence, male nose is more prominent, resulted in a more stereoscopic facial profile. The nasal basal plane of both male and female, was more anterior than the infraorbital points, with 7％ of the absolute degree of the infraorbital points. The range of the angle between the nasal basal plane and coronal plane was 2-4 degrees. Conclusions This study sets a database for the external nasal characteristics of the young adult in Chinese Han nationality, which could provide quantitative references for preoperative evaluation and individually surgical planning.
[Gastrointerstinal stromal tumors of the stomach--case reports].

Abstract:INTRODUCTION Gastrointestinal stromal tumors are the most common mesenchimal tumors of the gastrointestinal tract. Despite the enormous progress made in the field of diagnostics, most often diagnosis is made during the explorative laparotomy and postoperative immunohystochemical analysis of specimen. The surgical treatment is often indicated, although scarce preoperative diagnosis causes intraoperative dilemma concerning the level of radicality. MATERIAL AND METHODS In this paper we have analyzed two patients with gastric GIST. The first patient was 59 years old male, with preoperatively diagnosed colonic cancer. Intraoperatively besides the transverse colon cancer, we found intramural gastric tumor. This patient underwent subtotal gastrectomy and subtotal colectomy. The immunohystochemical analysis of gastric tumor proves benign GIST. The second patient was 50 year old male presented with repeated upper GI bleeding. The endoscopic ultrasound showed intramural tumor of the anterior gastric wall, with a visible blood vessel bleeding during endoscopy. After the resuscitation, we performed subtotal gastrectomy. The immunohystochemical analysis proved malignant GIST. DISCUSSION In the cases with inadequate preoperative diagnoses, the level of resection procedure is based on the size of tumor and the presence of necrosis and bleeding inside the tumor. Tumors larger than 5 cm in diameter with signs of necrosis and bleeding are parameters of malignant nature of GIST, therefore demanding a radical surgical treatment. CONCLUSION The surgical resection is a treatment of choice for gastrointestinal stromal tumors. It has been shown that adequate surgical resection correlates with high 5-years survival rates for patients with gastric GIST.

Abstract:Objective: To evaluate the influence of variations in the upper sulcus depth, nasal tip protrusion, and nasolabial angle on the esthetic perception of profiles. Design: A web-based survey study and was conducted in the Department of Orthodontics and Dentofacial Orthopaedics. This survey was conducted for a period of 10 months and included orthodontists practicing in India. Methods: This study included lateral cephalograms of the most esthetic profiles from a set of 30 profiles that were within Holdaways norms; the most esthetic profile was determined by a panel of two orthodontists and two general dentists. The most esthetic profile photograph will be transferred to a computer in Corel software. The outline of the profile will remain the same, with the inner aspect converted into a dark area (black); now, the profile is completely converted into facial silhouettes. This black facial silhouette is presented to avoid any distractions and bias. There will be a total of 27 silhouettes by various combinations of increased, decreased, and normal values for nasolabial angle, upper sulcus depth, and nasal tip protrusions. Results: Both the orthodontist and layman chose the most favored profile as a normal profile having a normal nasal tip, normal nasolabial angle, and normal upper sulcus depth, whereas among dentists, 50% chose the most favored profile in which the nasolabial angle was normal but the nasal tip and upper labial sulcus were decreased. When it comes to the least favored profiles, opinions varied between orthodontists, dentists, and laymen. Conclusion: The perception of facial esthetics has always been and still is varied between the layman, the dentist, and the orthodontists. Profiles with normal to sharp noses with normal and increased nasolabial angles were considered more attractive than the others. Profiles with fuller upper labial sulci were considered least attractive with other parameters changing.

Abstract:Abstract Due to its central location, the nose plays a prominent role in facial aesthetics. As tastes have shifted and techniques have advanced, the accepted “ideal” appearance and proportions of the nose have evolved over time. By assessing the aesthetics of the nasal dorsum through the use of lines and angles, one can more precisely elucidate a goal for the patient's postoperative nasal shape, which should, in turn, guide the surgeon to execute specific operative maneuvers needed to achieve that contour. In assessing the aesthetics of the nasal dorsum, practitioners calculate and observe aspects such as the paired dorsal aesthetic lines, the nasofrontal angle, and the nasofacial angle. There is also additional consideration given to nasal tip position as this must fit harmoniously with the shape of the dorsum. In contrast to the established aesthetic lines and angles, using nasal geometric polygons for the aesthetic evaluation and development of operative goals in rhinoplasty has recently been described in the literature. Constructed ideals, in the form of proportions, lines, and angles, should be used with caution, as there are many factors to consider in the aesthetic analysis of the nasal dorsum, including ethnic differences, and subjective and changing views of beauty.

Abstract:Background Nasal alarplasty is an important component of esthetic rhinoplasty in Asians. The two main surgical techniques that correct alar hypertrophy by reducing the height or length often leave external scars and are associated with a high relapse rate. Methods We developed a new technique, called three-dimensional（3 D） M-shaped resection, which corrects both the nasal alar height and length and simultaneously minimizes external scarring. We performed this procedure from January 2013 to September 2016 in 49 consecutive female patients diagnosed with saddle nose and nasal alar hypertrophy. Their mean age was 28.6（range, 18–40） years. All patients had previously undergone simple rhinoplasty. Nasal alar length and height, nostril length and width, and maximal nose width were analyzed preoperatively and postoperatively from photographs. Results After a mean of 9（range, 3–24） months of follow-up, surgery was considered successful in 46 women（94%） with good cosmetic effects. In three patients, nasal alar hypertrophy recurred（6 months postoperatively）. There were no early complications such as hematomas, infections, skin or mucosal necrosis, or wound dehiscence. The mean reductions postoperatively were 1.7 mm and 0.9 mm for nasal alar length and height, respectively, 1.6 mm for both nostril length and width, and 3.5 mm for nose width.Conclusion The 3 D M-shaped resection for nasal alar hypertrophy effectively reduced hypertrophy in 94% of patients for up to 24 months, producing minimal external scars and good cosmetic effects. It is a simple and convenient technique that is an effective and safe option for nasal alarplasty.

Abstract:Background Rhinoplasty enhances facial symmetry and functionality. However, the accurate and reliable quantification of nasal defects pre-surgery remains an ongoing challenge. Aim This study introduces a novel approach for defect quantification using 2D images and artificial intelligence, providing a tool for better pre-operative planning and improved surgical outcomes. Materials and Methods A pre-trained AI model for facial landmark detection was utilised on a dataset of 250 images of male patients aged 18 to 24 who underwent rhinoplasty for cosmetic nasal deformity correction. The analysis concentrated on 36 different distances between the facial landmarks. These distances were normalised using min-max scaling to counter image size and quality variations. Post-normalisation, statistical parameters, including mean, median, and standard deviation, were calculated to identify and quantify nasal defects. Results The methodology was tested and validated using images from different ethnicities and regions, showing promising potential as a beneficial surgical aid. The normalised data produced reliable quantifications of nasal defects(average 76.2%), aiding in pre-operative planning and improving surgical outcomes and patient satisfaction. Applications The developed method can be extended to other facial plastic surgeries. Furthermore, it can be used to create app-based software, assist medical education, and improve patient-doctor communication. Conclusion This novel method for defect quantification in rhinoplasty using AI and image processing holds significant potential in improving surgical planning, outcomes, and patient satisfaction, marking an essential step in the fusion of AI and plastic surgery.

Crooked Nose: Aesthetic and Functional Outcomes

Abstract:Purpose: To highlight a detailed analysis of aesthetic and functional rhinoplasty outcomes utilizing the most recent highly validated Standardized Cosmesis Health Nasal Outcomes Survey (SCHNOS) for Middle Eastern crooked nose patients. Methods: A longitudinal cohort study scrutinizing preoperative rhinoplasty patients' satisfaction retrospectively and their postoperative outcomes prospectively. The patients fulfilled the Arabic SCHNOS during postoperative follow-up. Preoperative and postoperative responses were recorded and then reviewed and analyzed. Results: This study included 41 patients with a mean age of 27.8 years. Females represented 41.5% of patients. About 73% of patients had a history of trauma. 90% of patients underwent primary procedures for crooked nose correction. Twelve patients (29.3%) underwent surgery for functional reasons, and twenty-six (63.4%) had it for both aesthetic and functional issues. There was a statistically significant difference regarding changes in SCHNOS regarding nasal obstruction scores (SCHNOS-O) and nasal cosmesis scores (SCHNOS-C) (p < 0.001). These findings coincided with a substantial reduction of all SCHNOS items postoperatively (p < 0.001). There was no statistically significant relationship between changes in (SCHNOS-O) or (SCHNOS-C) pre-and postoperatively and either age, sex, history of trauma, or type of surgery. However, a statistically significant difference was detected when assessing the relationship between changes in SCHNOS-O and the reason for surgery. Conclusion: A thorough knowledge of three-dimensional pathology and time-associated changes is required to achieve optimal aesthetic and functional outcomes for crooked nose patients. The use of highly validated questionnaires like SCHNOS in clinical practice is highly encouraged to modify and trace surgical techniques to the most appropriate and successful ones for the patients.

Abstract:Background: Rhinoplasty is conducted to address both cosmetic and functional objectives. It is not merely a cosmetic procedure, as it frequently aims to improve function by enhancing nasal breathing and removing congenital or acquired obstructions. The current study aims to determine the functional, cosmetic, and psychological outcomes of rhinoplasties. Methods: This is a descriptive cross-sectional study. The data were collected using an interview and an online Google form. A chi-square test was used to determine the relationship between qualitative variables and quantitative variables using frequency, percentage, and mean for quantitative variables. Each variable's P value is used to detect statistical significance. The significance level was set at a P value of less than 0.05. Results: Of the 234 patients enrolled in this study, 69.2% were women, whereas 30.8% were men, with a mean age of 22.86 ± 4.546 years. In terms of satisfaction, functional satisfaction accounted for about 59% of all satisfaction, cosmetic satisfaction was 63.2%, and psychological satisfaction was 70.9%. The primary reason for surgery was cosmetic (45%). Conclusion: The majority of enrolled participants were satisfied with their functional outcomes, whereas their satisfaction with psychological and cosmetic outcomes was moderate.
Unnecessary perioperative antibiotic prophylaxis in the placement of totally implanted central venous access ports should be avoided.

Abstract:Psychological assessment plays a crucial role in the preoperative phase of septorhinoplasty (SRPL), exerting potential influence on both pre- and postoperative aspects of quality of life (QoL). In this prospective study, SRPL patients were systematically screened using two validated questionnaires for body dysmorphic disorder (BDD) and one each for anxiety, depression, and somatic symptom disorder (SSD). Nasal perception was evaluated utilizing two disease-specific, self-report instruments: The Rhinoplasty Outcomes Evaluation (ROE) and the Functional Rhinoplasty Outcome Inventory-17 (FROI-17). A total of 95 patients were enrolled. Positive screening rates were 40.4 and 28.3% for BDD, 37.3% for elevated anxiety, 16.4% for depression, and 22.4% for SSD. Mean scores on the ROE and FROI indicated lower QoL in all positive screening groups. Statistically significant differences were observed for anxiety (FROI total score: 47.1 ± 14.6 to 37.8 ± 16.2; p = 0.021) and BDD (ROE: 43.7 ± 15.7 to 32.5 ± 15.5; p = 0.003 and FROI total score: 47.4 ± 15.3 to 37.8 ± 17.1; p = 0.014). The Hospital Anxiety and Depression Scale exhibited a significant correlation with the preoperative FROI score ( r = 0.34, p = 0.005). Patients with positive screenings for depression, anxiety, BDD, and SSD demonstrated compromised QoL during preoperative assessments compared with their negative counterparts, reflecting diminished nasal perception in both esthetic and functional dimensions. Abstract:Statement of the problem: The position of the chin and lips are important components in the lower third of the face and can be changed by orthodontic treatment. It seems that factors such as diversity in culture, ethnicity, and place of residence are among the factors affecting people's perception of beauty. Iran, as a vast country, contains different ethnicities and cultures, and of course, it is not exempt from this point of view. Purpose: Our purpose of conducting this study is to investigate the impact of the difference in living environment and culture on people's aesthetic perception. Orthodontists and oral surgeons can use these data to choose the best treatment plan for the patients according to their geographical zones. Material and method: A descriptive cross-sectional study was conducted to evaluate the perception of lips and chin position. A series of 25 profile images were prepared in 5 sets. Each set contained 5 profile images. Northern and Southern lay people and orthodontists were asked to evaluate the profile series of each set in 1 session and score them from 1 to 5: 1, very unattractive; 2, unattractive; 3, neither attractive nor unattractive; 4, attractive; or 5, very attractive. Results: 652 participants in 3 groups, including 16 orthodontists (10 men and 6 women), 318 lay people of the North of Iran (172 men and 138 women), and 318 lay people of the South of Iran (175 men and 139 women) participated in this study. Regardless of the chin position, normal lip position and slight changes of that (in both protruded and retruded positions) were more favorable for all three groups. The images with moderately retruded lips were scored as the least attractive by all three groups and orthodontist gave the lowest score to these profiles. Southern people could better tolerate moderately retruded lips than other two groups. In the fifth series, orthodontists preferred slightly and moderately protruded lips in comparison to other two groups of laypeople. Conclusion: Regardless of the chin position, normal and slightly (-2mm to +2mm) protruded and retruded lips were more favorable in all three groups. Southern people could better tolerate moderately retruded lips than the two other groups.

For this reason, it is crucial for surgeons to be aware of these potential impacts in order to achieve predictable and aesthetically pleasing results. In this article, we aim to provide a summary of the current evidence-based conclusions on the dynamic influence of the radix, dorsum, tip, and ala during rhinoplasty. By understanding the complex relationships between these structures, surgeons can make informed decisions and achieve the best possible outcomes for their patients.

Clinical features and therapy of acute thallium poisoning.

Abstract:Severe acute thallium poisoning in a young man is described. He presented with transient loss of consciousness and paraesthesiae of finger tips and lips, with a blood thallium concentration of 5750 micrograms/l (levels above 200 micrograms/l are toxic). He rapidly lost limb sensation and power and later required temporary mechanical ventilation and nasogastric feeding. The neurological sequelae one year afterwards are a flaccid paraparesis, cerebellar ataxia and mental impairment. Immediate cardiovascular complications included hypertension, sinus tachycardia, ECG abnormalities and an episode of ventricular fibrillation. We were able to assess the relative merits of different methods advocated for enhancing thallium excretion. Oral Prussian blue, forced diuresis and haemodialysis were found to be the most effective: 2000 mg of thallium were eliminated over 20 days, 820 mg over 46 days and 225 mg over 25 days respectively by these methods. Haemofiltration was ineffective. Diethyldithiocarbamate, a chelating agent, brought about a rise in serum thallium concentration accompanied by clinical deterioration and its use should be abandoned.

Abstract:Objectives To evaluate the impact of transfemale gender‐affirming rhinoplasty on nasal appearance as evaluated by common facial analysis measures. Methods Retrospective case series of patients undergoing gender‐affirming nasal surgery at a single tertiary care center from March 2016 to July 2022. Pre‐ and postoperative photographs were analyzed, using iris width to normalize the measurements. Parametric tests were applied to identify differences in facial analytical measures. Results A total of 33 male to female transgender patients met the inclusion criteria for study entry. Reductive rhinoplasty techniques were frequently utilized. Osteotomies and dorsal reduction were performed in 90.9% and 72.7% of cases, respectively. Tip width decreased on average by 2.3 mm. There was a mean increase of 10.0° in nasolabial angle, and 5.5° increase in nasofrontal angle. Among the 51.5% of patients who underwent alar base reduction, there was a mean decrease of 2.3 mm in alar base width, and 3.1 mm decrease in nasal width. Alar base excision and nasal tip grafting were utilized in 51.5% and 63.6% cases respectively. Conclusions Transfemale gender‐affirming rhinoplasty techniques consistently and predictably altered facial analytical measures, and were effective in aligning nasal appearance with desired gender identity.
