Abstract:BackgroundColonoscopy is regarded as the gold standard for colorectal cancer screening and surveillance. However, previous studies have reported large numbers of polyps were missed during routine colonoscopy.AimsTo evaluate polyp miss rate in short-term repeated colonoscopy and explore the related risk factors.MethodsA total of 3695 patients and 12,412 polyps were included in our studies. We calculated the miss rate for polyps of different sizes, pathologies, morphologies and locations, and patients of different characteristics. Univariate and multivariate logistic regression analyses were performed to evaluate risk factors related to miss rate.ResultsThe polyp miss rate was 26.3% and the adenoma miss rate was 22.4% in our study. The advanced adenoma miss rate was 11.0% and the proportion of missed advanced adenomas in missed adenomas sized > 5 mm was up to 22.8%. Polyps sized < 5 mm had a significantly higher miss rate. The miss rate of pedunculated polyps was lower than that of flat or sessile polyps. Polyps in the right colon were prone to be missed than that in the left colon. For older men, current smokers, individuals with multiple polyps detected in the first colonoscopy, the risk of missing polyps was significantly higher.ConclusionNearly a quarter of polyps were missed during routine colonoscopy. Diminutive, flat, sessile, and right-side colon polyps were at higher risk of missing. The risk of missing polyps was higher in older men, current smokers, and individuals with multiple polyps detected in the first colonoscopy than their counterparts.

Abstract:Colonoscopic polypectomy is extremely effective in protecting against colorectal cancers, and recognition of adenomas by colonoscopy is of paramount importance in preventing colorectal cancer (CRC) and CRC-associated mortality. Incomplete polyp resection (IPR) occurs routinely in medical practice following polypectomy. We conducted a comprehensive literature search to identify studies that reported on the potential risk factors for IPR using MEDLINE, EMBASE, and PubMed. Publication time was limited between January 2004 and July 2021. Our search revealed assessments of the position, morphology, and histologic variation of the polyps—as well as the relevant skills that may interfere with IPR. The included studies showed that colonic polyps with a diameter >20 mm and proximal in location should be dealt with cautiously. For smaller polyps, cold-snare polypectomy appeared to be the most effective method used to accomplish comprehensive polyp removal. In addition, endoscopist experience in narrow-band imaging in polyp detection were also associated with IPR. Therefore, polypectomy should be performed cautiously if the polyp growth is expected to be cancerous and situated in the proximal portion of the colon or rectum, and modalities should be standardized so as to lower the potential risk for IPR.
Retrospective Study of Risk Factors for Colorectal Adenomas and Non-Adenomatous Polyps

Abstract:BACKGROUND:To clarify the risk factors of colorectal adenomas and non-adenomatous polyps by retrospective analysis.METHODS:Data were obtained from subjects who underwent health examination in the Health Promotion Centre of Sir Run Run Shaw Hospital of Zhejiang University from January 2016 to December 2018, and all of them completed colonoscopies. Statistical analysis was performed using R software.RESULTS:A total of 721 subjects were included in the analysis, of whom 58 (8.0%) were diagnosed as non-adenomatous polyps, 115 (16.0%) as adenomas, 3 (0.4%) as neuroendocrine neoplasms or adenocarcinomas. The gender ratio and prevalence of fatty liver in the non-adenomatous polyp group were different from that in the control group, and age, body mass index (BMI), waist circumference, blood glucose, serum uric acid, triglyceride, alanine transaminase and creatinine levels were higher, while the level of high-density lipoprotein was lower than control group (P<0.05). The sex ratio in the adenoma group was different from that in the control group, age, BMI, waist circumference, glycosylated hemoglobin, serum uric acid, triglyceride and creatinine levels were higher (P<0.05). Age was a risk factor for non-adenomatous polyps. Male and age were risk factors for adenomas.CONCLUSIONS:Age, body weight, waist circumference, blood lipid and blood glucose are related to the high incidence of colorectal adenomas and non-adenomatous polyps; fatty liver is related to the high incidence of colorectal non-adenomatous polyps; age is a risk factor for non-adenomatous polyps; age and male are risk factors for colorectal adenomas.
Miss rate of colorectal neoplastic polyps and risk factors for missed polyps in consecutive colonoscopies

Abstract:BACKGROUND/AIMS: Colonoscopic polypectomy is the best diagnostic and therapeutic tool to detect and prevent colorectal neoplasms. However, previous studies have reported that 17% to 28% of colorectal polyps are missed during colonoscopy. We investigated the miss rate of neoplastic polyps and the factors associated with missed polyps from quality-adjusted consecutive colonoscopies.METHODS: We reviewed the medical records of patients who were found to have colorectal polyps at a medical examination center of the Kangbuk Samsung Hospital between March 2012 and February 2013. Patients who were referred to a single tertiary academic medical center and underwent colonoscopic polypectomy on the same day were enrolled in our study. The odds ratios (ORs) associated with polyp-related and patient-related factors were evaluated using logistic regression analyses.RESULTS: A total of 463 patients and 1,294 neoplastic polyps were analyzed. The miss rates for adenomas, advanced adenomas, and carcinomas were 24.1% (312/1,294), 1.2% (15/1,294), and 0% (0/1,294), respectively. Flat/sessile-shaped adenomas (adjusted OR, 3.62; 95% confidence interval [CI], 2.40-5.46) and smaller adenomas (adjusted OR, 5.63; 95% CI, 2.84- 11.15 for ≤5 mm; adjusted OR, 3.18; 95% CI, 1.60-6.30 for 6-9 mm, respectively) were more frequently missed than pedunculated/sub-pedunculated adenomas and larger adenomas. In patients with 2 or more polyps compared with only one detected (adjusted OR, 2.37; 95% CI, 1.55-3.61 for 2-4 polyps; adjusted OR, 11.52; 95% CI, 4.61-28.79 for ≥5 polyps, respectively) during the first endoscopy, the risk of missing an additional polyp was significantly higher.CONCLUSIONS: One-quarter of neoplastic polyps was missed during colonoscopy. We encourage endoscopists to detect smaller and flat or sessile polyps by using the optimal withdrawal technique.
Pathological Analysis of Colonic Polyps and the Recurrence of Polyps after Polypectomy

Abstract:【Objective】To guide endoscope treatment and prognosis judgment, investigate the endoscopic characteristics of colonic polyps and cancerous polyps, analyze the recurrence of polyps after polypectomy and its influencing factors for comparison of pathological status of colonic polyps between before and after polypectomy. 【Methods】The endoscopic characteristics of colonic polyps which have accepted endoscopy from December 13th, 2009 to December 12th, 2010 were analyzed, and pathological diagnosis before and after polypectomy were compared. The recurrence rate of polyps after polypectomy was calculated and its affect factors were analyzed.【Results】 1401 patients with 3955 colonic polypus had received endoscopy in our hospital. The polyps in left hemicolon, the polyps diameter1.0 cm, the sessile and lobulated polyps was easy to cancerate (P 0.05). Pathological inconsistency was 33.7% between biopsy and polypectomy. The recurrence rate of polyps after polypectomy was 56.7% , 38.1% polyps recurred within 1 year and 78.2% polyps recurred within 2 years. Multiple polyps that were older than 50 years, the polyps diameters were bigger than 1.0cm and the adenomatous polyps were apt to recur (P 0.01).【Conclusion】Pathological difference exists significantly between before and after polypectomy, biopsy may miss malignant polyps. The recurrence of polyps is so high that the patients need intense follow-up.

Abstract:BackgroundMost screening colonoscopies require patients to miss work on the day of the procedure. Little is known about whether patients are taking additional time off from work, and the reasons for doing so.AimsThe purpose of this study was to assess the patterns and reasons for missed work related to screening colonoscopies.MethodsAll outpatient screening colonoscopy procedures performed at an academic medical center over 6 months were reviewed. Exclusions included procedures performed for other indications, patients age 65 or older, procedures performed on Monday or Friday, and patients who were not working. Patients were interviewed by telephone regarding missed work time and the reasons for doing so.ResultsSixty-eight patients met all inclusion criteria. Thirty-four percent missed work on more than the day of the procedure. Thirty-two percent took the day prior off, 10% took the day after off, and 9% took both days off. The reason for taking the day before the procedure off was uniformly anticipation of the bowel preparation. Of those who took the day after off, 57% did so as a precautionary measure after moderate sedation, while 43% had symptoms.ConclusionsOne third of working patients who undergo mid-week screening colonoscopies miss work on additional days to the procedure day. Unanticipated time missed from work could increase the indirect costs of screening colonoscopy.

Analysis of colonic miss polyps by routine video colonscopy

Abstract:Objective To explore miss rate of colonic polyps by routine video colonoscopy and size, type, location, and pathologic features of missed polyps. Methods We retrospectively analyzed the colonic polyps patients who were examined by tandem video colonoscopies within 120 days in the past 3 years from 2004 to 2006. Number of Miss polyps（NMP） equals to polyps found by the second examination（PFS） which minus polyps left in the fimt colonoscopy（PLF）. The number of basal polyps（NBP） equals to polyps found in the fimt colonscopy which plus NMP. Miss rate equals to NMP/NBP × 100%. Results There were 143 patients, age 63.0 ± 12.6（23 -82）years old, 92 of them were men. NMP was 126 and miss rate was 22. 5%. Among missed polyps, proportions of 〈 5mm, 5 -9mm and ≥ 10mm in diameter were 80. 2% , 18. 3% and 1.6% respectively, presented a significant difference（ P 〈 0.01 ） ; type Ⅰ , Ⅱ, Ⅲ and Ⅳ were 87.3% , 8.7% , 3.2% and 0. 8% respectively, the difference was significant （ P 〈 0.01 ）. Miss rates were different in different segment of colon （P 〈0. 01 ）, rectum, sigmoid, descending, transverse, hepatic flexure, ascending and cecum were 11.1% , 27.0% , 12. 7% , 19. 1% , 10. 3% , 15.1% and 4. 8% respectively. Missed advanced adenoma was 14. 8% in all missed adenomas. There was significant positive correlation between NMP and NBP（ r = 0. 674, P 〈 0. 01 ）. Conclusion The miss rate of colonic polyps is 22. 5 % by routine video colonoscopy. The most of missed polyps is 〈Smm in size and type Ⅰ ones. Different segment of colon has different miss rate. [Influences of Diagnostic Modes on an Early Diagnosis of Colorectal Cancer].

Abstract:OBJECTIVE:To explore the influences of diagnostic modes of colorectal cancer (CRC) on an early diagnosis of CRC.METHODS:A total of 405 cases were randomly collected from 1798 CRC patients registered at our hospital from January 2000 to December 2010. A retrospective chart review was undertaken for all identified cases. Besides the demographics and tumor features, TNM stage was obtained from medical records and pathological forms. Other collected data were as follows: (1) Type of clinical examinations leading to diagnosis. (2) Diagnostic duration: including patient duration (period from initial symptoms to consulting a doctor and hospitalization duration (period from patient first seeing a doctor to a confirmed CRC case). (3) Diagnostic delay and its rate: diagnostic delay was identified as the diagnostic duration of over 30 days. We compared the different delay periods of 31 - 60, 61 - 90, 91 - 150 and > 150 days and its corresponding tumor stages at diagnosis. (4) Misdiagnosis and its rate: that CRC subjects were diagnosed and treated as other diseases for at least 30 days was identified as misdiagnosis. The t and Mann-Whitney U tests were performed for the quantitative data and χ(2) test for the qualitative data.RESULTS:Among the study subjects, 67.2% (270/402) CRC cases were examined by colonoscopy, 17.4 % (70/402) cases by CT scan or B ultrasound. The median diagnostic duration of CRC was 90 days (25% percentile: 40 days, 75% percentile: 210 days). The median patient duration was 30 days and median hospital duration 10 days. The overall misdiagnostic rate of CRC was 27.9% (112/401), higher (39.7%, 48/121) in right sited CRC than in left sited CRC (22.9%, 63/275) (χ(2) = 11.7, P = 0.00). 77.7% (313/403) cases had > 30 days diagnostic delay, 50.8% (156/307) delay attributable to the patients, while 29.0% (89/307) attributable to hospitals and 20.2% (62/307) attributable to both. The diagnostic duration of early-stage CRC and advanced-stage CRC appeared to be in the same length. On the other hand, no difference of TNM stage at the initial diagnosis was found in groups with different diagnostic durations of 31 - 60, 61 - 90, 91 - 150 and > 150 days (all P > 0.05).CONCLUSION:Colonoscopy is the most effective in the diagnosis of CRC. The median diagnostic time of CRC is 90 days. The phenomenon of its diagnostic delay often occurs. However, the delay is not a major cause for its advanced stage and a poor outcome. Its inherent biological characteristics may be more important.
Factors influencing the detection rate of colorectal polyps and adenomas by colonoscopy

Abstract:Objective:To explore the diagnosis of colorectal polyps/adenomas by colonoscopy and its influencing factors.Methods:Patients who underwent colonoscopy at Peking University International Hospital from January 1, 2017 to December 31, 2019 were selected as study subjects. Gender, age, anesthesia, intestinal cleanliness, time of admission, examining physician, and diagnosis of polyps/adenomas were recorded. Univariate Chi-square test was used to analyze the relationship between the detection rate of polyps/adenomas and the above factors, and then the independent risk factors affecting the detection rate of polyps/adenomas were identified by Logistic multivariate analysis.Results:A total of 6679 patients were included in the study, including 3297 males and 3382 females, aged 55.98±9.79 years. The detection rate for colon polyps was 47.25%, and the detection rate for colorectal adenomas was 30.93%. Univariate analysis showed that gender, age group, anesthesia, intestinal cleanliness, time of endoscopic examination, and experience of examining physician affected the detection rate for colorectal adenomas. Multivariate analysis showed that gender, age, intestinal cleanliness, time of endoscopic examination, and experience of examining physician were factors affecting the detection rate of colorectal adenomas.Conclusion:The gender and age of patients are related to the detection of colon polyps/adenomas. It is necessary to make good bowel preparation and improve the experience of examining doctors, so as to improve the detection rate for colon polyps/adenomas.
Patient- and procedure-related factors affecting proximal and distal detection rates for polyps and adenomas: results from 1603 screening colonoscopies

Abstract:Background: Screening colonoscopy is less effective in reducing the incidence of proximal compared to distal colorectal cancer, presumably because of missed adenomas and advanced lesions during endoscopy. Thus, effectiveness and success of colorectal cancer (CRC) screening programs depend decisively on the quality of the endoscopic procedures. Methods: A retrospective analysis of 1603 average risk screening colonoscopies to calculate and to identify determinants of separate detection rates for proximally and distally located polyps, adenomas, and advanced adenomas was performed. Results: 56.1 % of 1603 individuals included were men, and the mean age was 60.2 ± 10.2 years. Distal detection rates were markedly higher compared to proximal detection rates for polyps (40.9 vs. 23.8 %), adenomas (21.3 vs. 16.2 %), and advanced adenomas (4.0 vs. 2.0 %). A gradual increase in detection rates with increasing age was found for proximal and distal localization. Gender difference was also seen for polyps and adenomas, but not for advanced adenomas. In multivariate analysis, age <65.0 years and female gender were independently associated with a lower separate polyp detection rate (PDR) and adenoma detection rate (ADR). The use of propofol was the only procedure-related variable significantly associated with higher polyp detection rate. Conclusion: Since age and gender affect detection rates of proximally and distally located polyps and adenomas, the requirement of a specific gender-related limit in total detection rates may be insufficient as a quality indicator for screening colonoscopies.

Abstract:BackgroundDespite thorough preoperative screening, 19–30% of synchronous polyps or adenomas are detected after colon cancer surgery. Remnant synchronous lesions require additional colonoscopy procedures or surgery.AimTo investigate factors of preoperative colonoscopy potentially affecting the detection of missed lesions in patients subjected to colorectal cancer surgery.MethodsOf 1147 colorectal cancer patients subjected to curative open or laparoscopic colectomy and colonoscopy at the Chungnam National University Hospital from January 2012 to December 2016, 518 patients underwent pre- and postoperative colonoscopy. The index colonoscopy was defined as the last preoperative endoscopy performed. We analyzed pre- and postoperative medical charts for colonoscopy and pathological data. The effects of patient, procedure, and tumor factors on the postoperative adenoma detection rate, advanced adenoma detection rate, and adenoma miss rate (AMR) were analyzed.ResultsThe overall AMR was 25.7% (95% confidence interval, 22.2–29.8%). Comparing optimal and non-optimal bowel preparation groups, the latter had greater postoperative polyp missed rate (PMR), AMR (p < 0.01), and AAMR (p = 0.272). The optimal preparation group allowed identification of more synchronous adenomas than in the fair (OR 5.72) and poor (OR 11.39) preparation groups. On univariate analysis, patient age and left-sided colectomy (p < 0.01) influenced AMR. Multivariate analysis showed that age, preoperative bowel preparation, and left colon resection influenced postoperative AMR.ConclusionA better quality of index colonoscopy had a positive effect on lowering the detection rate of postoperative adenoma. Older age and suboptimal bowel preparation at the index colonoscopy and left-sided colectomy had negative effects on lowering the postoperative AMR.

Real-word Evaluation of Differences in Bowel Preparation for Colonoscopy Between the Digestive and the Non-Digestive Physicians: A Retrospective Study

Abstract:IntroductionUsing real-world data, we compared the quality of bowel preparation instructed by the digestive and non-digestive physicians in outpatients for colonoscopy and identified potential risk factors.MethodsThis was a retrospective study based on real-world data, which were collected from the Ningbo First Hospital in China from December 2019 to October 2020. Outpatients included were classified into the digestive and the non-digestive physician groups according to the referring physician. The primary outcome was adequate bowel preparation measured by the Boston Bowel Preparation Scale (BBPS), namely, a BBPS score of 2 or higher in any colonic segment and a total score ≥ 6. Secondary outcomes included the total mean BBPS scores and possible risk factors associated with poor bowel preparation.ResultsThere were 671 outpatients included, with 392 in the digestive physician group and 279 in the non-digestive physician group. Adequate bowel preparation was 84.2% in the digestive physician group and 71.0% in the non-digestive physician group (odds ratio [OR]: 1.50, p < 0.001), and the latter had lower total mean BBPS scores (6.12 ± 1.33 vs. 6.66 ± 1.29, p < 0.001). The non-digestive physician was an independent risk factor according to the multivariate logistic regression analysis (OR: 0.45, p < 0.001).ConclusionThe quality of bowel preparations instructed by non-digestive physicians was inferior to digestive physicians, which was a factor potentially associated with poor bowel preparation (ClinicalTrials.gov number: NCT04738578).

Abstract:The acute activation of adrenal glucocorticoid synthesis by ACTH has long been believed to be mediated by cAMP as the major second messenger, although increases in cellular cAMP concentration have not been observed at low concentrations of ACTH. We found that steroidogenesis in bovine adrenal fasciculata-reticularis cells was activated by the addition of arachidonic acid or its 15-lipoxygenase metabolite, 15-hydroperoxyeicosatetraenoic acid. The cellular 15-lipoxygenase pathway was significantly activated by 1 pM ACTH, at which concentration no increase in cellular cAMP synthesis was observed. The 1 pM ACTH-induced stimulation of steroidogenesis was completely suppressed by a lipoxygenase inhibitor, AA-861. The stimulation was independent of the increase in cellular cAMP. These results show that the action of 1 pM ACTH on steroidogenesis may be mediated by the 15-lipoxygenase metabolite(s) as a solo second messenger. The addition of ACTH at concentrations higher than 10 pM increased both the 15-lipoxygenase activity and cellular cAMP synthesis. Under these conditions, the 15-lipoxygenase metabolite(s) and cAMP were shown to mediate the activation of steroidogenesis synergistically. The presence of a dual second messenger system could explain the stimulation of steroidogenesis by ACTH at physiological concentrations.
Effect and Safety Analysis of Day Endoscopic Surgery of Colorectal Polyps

Abstract:Objective To retrospectively analyze the safety of day surgery for colorectal polypectomy in gastroenterology department of our hospital. Methods 168 patients underwent day surgery for colorectal polypectomy in our hospital, and 238 patients underwent colorectal polypectomy through general hospitalization from June 2019 to February 2020. The efficacy, safety and hospitalization expenditure of the two groups were compared. Results The incidence of postoperative bleeding was 1.19%（2/168）. There was no significant difference in the incidence of postoperative bleeding between the two groups for polyps larger than 1cm（p=0.777）. The average length of stay and the average cost of hospitalization in the day surgery group were significantly lower than those in the patients with general hospitalization during the same period. Conclusion Day surgery is safe and effective for endoscopic resection of colorectal polyps larger than 1cm, which can reduce the medical burden of patients.
Clinical significance of colorectal polyp detection on colonoscopy insertion

Abstract:Background: Colorectal lesions are generally evaluated during the withdrawal phase of colonoscopy. Minimising the risk of missed lesions is crucial to determine an appropriate future surveillance colonoscopy interval.Objective: This study aimed to evaluate the clinical significance of detecting sigmoid colon lesions during the insertion phase.Methods: This retrospective study included 172 consecutive patients undergoing colonoscopy between October 2017 and April 2018. The total number of detected polyps, mean polyps per procedure, mean polyps per positive procedure, and histological and clinical characteristics of detected lesions were recorded. The primary endpoint was the difference in sigmoid colon polyp detection rates during insertion and withdrawal.Results: A total of 172 colonoscopies were performed for each patient and 322 lesions were detected. Sixty-two (19%) polyps were detected during insertion, 312 (97%) during withdrawal, and 52 (16%) during both insertion and withdrawal. Although all polyps except for those in the sigmoid colon could be detected during withdrawal, 10 of 87 (11%) polyps in the sigmoid colon could only be detected during insertion.Conclusions: In this study, attempts to detect polyps, even in the insertion phase, showed the clinical significance to decrease the risk of missed adenomatous polyps in the sigmoid colon.

Abstract:Background: Sedated colonoscopy has been increasingly selected. However, the effect of sedated colonoscopy on polyp/adenoma detection rate (PDR/ADR) remains controversial among studies. Methods: In this retrospective study, the medical records of 11 504 consecutive patients who underwent colonoscopy at our department from July 1, 2021 to December 31, 2022 were collected. Patients were divided into sedated and unsedated groups according to the use of intravenous sedation during colonoscopy. Overall PDR/ADR, right-side, transverse, and left-side colon PDR/ADR, and single and multiple PDR/ADR were calculated. By adjusting for age, gender, body mass index, inpatient, screening/surveillance, cecal intubation time, colonoscopy withdrawal time ≥6 min, and an endoscopist's experience ≥5 years, multivariate logistic regression analyses were performed to evaluate the association of sedated colonoscopy with overall PDR/ADR, right-side, transverse, and left-side colon PDR/ADR, and single and multiple PDR/ADR, where the absence of PDR/ADR was used as reference. Odds ratios (ORs) with their 95% confidence intervals (CIs) were calculated. Results: Overall, 2275 patients were included, of whom 293 and 1982 underwent sedated and unsedated colonoscopy, respectively. Multivariate logistic regression analyses showed that sedated colonoscopy was independently associated with lower overall PDR/ADR (OR = 0.640, 95% CI = 0.460-0.889, P = 0.008), right-side colon PDR/ADR (OR = 0.591, 95% CI = 0.417-0.837, P = 0.003), single PDR/ADR (OR = 0.659, 95% CI = 0.436-0.996, P = 0.048), and multiple PDR/ADR (OR = 0.586, 95% CI = 0.402-0.855, P = 0.005), but not transverse or left-side colon PDR/ADR. Conclusion: Sedated colonoscopy may not be beneficial in terms of overall PDR/ADR, right-side colon PDR/ADR, and number of polyps/adenomas. Thus, it should be selectively recommended. Additionally, it should be necessary to explore how to improve the quality of sedated colonoscopy.

Abstract:Background/aims: There have been few multicenter studies on colonic polyps conducted by primary medical institutions. This study examined the detection rate of colonic polyps in primary health care institutions and the related factors while following the guidelines. Methods: The medical records of 14,029 patients who underwent colonoscopy between January-June 2020 at 40 primary medical institutions in Korea were analyzed. High-risk adenoma was defined as advanced adenoma, carcinoma, or ≥3 adenomas. Results: Most patients (71.2%) aged ≥50 years underwent re-colonoscopy within 5 years (51.3%) for diagnostic purposes (61.3%) in Korean primary medical institutions. The detection rates of colon polyps, adenoma, advanced adenoma, high-risk adenoma, and carcinoma was 59.9%, 38.9%, 5.9%, 11.4%, and 0.3% in all subjects and 59.8%, 37.5%, 8.5%, 12.9%, and 0.3% in average-risk patients, respectively. The incidences of adenoma in average-risk patients increased significantly with age (30s/40s/50s: 20.1%/29.4%/43% for adenoma, 4.4%/6.7%/10.3% for advanced adenoma, and 5.6%/9.5%/14.6% for high-risk adenoma; p<0.05). Before 50 years of age, high-risk adenoma was detected in 9.1% of patients in the first-time screening group, and the significant risk factors were being male and ≥40 years of age. The detection rate of high-risk adenoma in the normal index colonoscopy group within 5 years was 9.0%. The significant risk factors included older age, male sex, positive fecal occult blood test, stool form changes, and nonspecific symptoms (gas and indigestion). Conclusions: More colonic adenoma studies targeting real-world clinical practice will be needed to revise the Korean guidelines for colorectal cancer screening and surveillance.
The value of auditing negative lower GI investigations preceding a final diagnosis of colorectal cancer

Abstract:Objective: To review all preceding 'negative' large bowel investigations in patients with a final diagnosis of colorectal cancer, and to examine whether delayed diagnosis was associated with worse outcome. Method: Details were gathered on all patients with a new diagnosis of colorectal adenocarcinoma presenting over 4.5 years. For each patient the hospital's clinical workstation and radiology and endoscopy databases were interrogated for all flexible sigmoidoscopies, colonoscopies and barium enemas during the 5 years prior to diagnosis. Results: Among the 570 patients, 28 (5%) had undergone colonoscopy and/or flexible sigmoidoscopy that had not shown colorectal cancer during the 5 years preceding final diagnosis, and a further 28 (5%) had undergone 'negative' barium enemas. Polyp surveillance might have missed four lesions destined to become malignant. Correspondingly there were three patients undergoing IBD surveillance found to have CRC, having had a negative complete colonoscopy within the preceding 5 years. Among patients undergoing de novo colonoscopy for diagnosis the true miss rate was only one patient per year. At August 2007, 29 (58%) of those with delayed diagnosis were still alive, compared with 216 (42%) of those diagnosed during initial investigation (chi2 = 5.04, P < 0.05). Conclusions: Colonoscopic miss rates are in line with previous studies. The application of simple clinical ground rules will avoid most pitfalls. The methodology described herein may assist in auditing the quality assurance of lower gastrointestinal diagnostic services. Despite the delay, late diagnosis was found to be associated with improved survival and a lower likelihood of metastatic disease.

Abstract:BACKGROUND:Colonoscopy, although a low-risk procedure, is not without associated adverse events. The rates of major adverse events such as perforation and bleeding after a colonoscopy are well reported. The rates of minor incidents following a colonoscopy, however, are less well examined. Recently the Centers for Medicare and Medicaid Services (CMS) started public reporting on the quality of outpatient endoscopy facilities by using a measure of risk-standardized rates of unplanned hospital visits within 7 days of colonoscopy.AIM:We intended to record and present the characteristics of our patient population who had an unplanned hospital visit within 7 days after undergoing colonoscopy in an outpatient setting.METHODS:This is a retrospective single-center observational study. During the study period of July 2018 to December 2019, we reviewed charts of all patients who returned to the emergency room within a week of undergoing an outpatient colonoscopy. Patient demographics, clinical data and details of colonoscopy were collected and analyzed.RESULTS:Of the 5344 outpatient colonoscopies performed, our post-colonoscopy emergency room visit rate was 1.05% (n=56). The mean age of the participants was 58 years and 55% were male; 32% of our patients reported gastrointestinal symptoms such as abdominal pain or gastrointestinal bleeding. Patients with gastrointestinal symptoms had a higher rate of polypectomies performed (36.4% vs 11.8%, P = 0.04) and reported higher illicit drug use (31.9% vs 5.9%, P = 0.02) compared with those with non-gastrointestinal complaints. After colonoscopy, 41% of the patients reported reasons for emergency room visits that were entirely unrelated to the procedure.CONCLUSION:Our study highlights that unplanned visits within 7 days of colonoscopy are not necessarily related to the procedure, and those that are, tend to be due to unavoidable patient factors. Hence the CMS measure may not be an accurate determinant of the quality of procedure or facility care delivered.
