Abstract:Traditional approaches to measurement in upper-limb therapy have gaps that electronic sensing and recording can help fill. We highlight shortcomings in current kinematic recording devices, and we introduce a wrist sensing device that performs multimodal sensing during single-axis rotation. Our goal is to characterize normative kinesthetic perception and real-world performance as a multimodal sensory "fingerprint" that can serve as a reference point for identifying deficit in persons affected by stroke, and then as a jumping point for later neuroscientific interrogation. We present an experiment involving psychophysical measurements of passive stimuli discrimination, matching adjustment acuity, and ADL performance in 11 neurologically-intact persons. We found that passive velocity sense and active position sense of healthy controls, measured by velocity discrimination and position matching respectively, correlated in rank with each other, but other score comparisons of acuity or task performance had no statistically significant correlations. We also found that participants differed in acuity between passive and active velocity sense, which supports current understanding about muscle spindle activation being modulated by conscious motor command. The potential for our null correlation results to reveal dissociable aspects of deficit is discussed, as well as implications for future neuroscientific study with more kinematic measures and larger datasets.

Abstract:Introduction Sensory deficits are common in neurological disorders. Proprioception is an example of a sensory system that could be impaired. In clinical populations, proprioception is preferably assessed as easily as possible. Techniques based on deep learning to precisely track body landmarks in simple video recordings are promising to analyze movement in clinical settings, outside the laboratory and without the need to attach markers. This study aims to validate deep learning based motion capture (DeepLabCut) with respect to laboratory-based 3D optoelectronic motion capture (gold standard) to assess proprioception by measuring the Joint Reproduction Error (JRE) during a knee joint position sense test [1]. Research question Can deep learning based motion capture (DeepLabCut) provide valid assessment of proprioception that is comparable to the gold standard (laboratory-based 3D optoelectronic motion capture)? Methods Proprioception was assessed with the eyes closed during 45 knee position sense tests of 11 typically developing children (6 girls, 5 boys, mean age 8.9 ± 1.0 years old, mean BMI 17.0 ± 2.6kg/m^2) by determining the difference between the criterion and reproduced angle (JRE, in degrees). The joint angles were determined with 2 setups; 1) 3D optoelectronic motion capture (Vicon, 10 cameras) using the ISB lower limb marker model [2] (26 markers), and 2) 2D deep learning based motion capture (DeepLabCut [3,4]) in sagittal video files using the pre-trained human model (MPII) with 55 manually labeled frames from videos of children, which were used only for network training and were excluded from the statistical analyses. Negative knee JREs correspond to a greater reproduced angle compared to the criterion angle. Linear mixed models were used to assess differences in JREs between DeepLabCut and Vicon. In addition, the mean knee JRE was calculated per subject and compared between Vicon and DeepLabCut using a paired t-test. Results There was no significant difference in the knee JRE measured with Vicon (-4.18°) and the knee JRE measured with DeepLabCut (-4.03°) (F=0.01, p = 0.91) (Fig. 1). The mean JRE over repeated trials per subject determined by using Vicon was not significantly different from the mean JRE determined by using DeepLabCut (t(10) = -0.11, p = 0.92). Discussion Measuring the knee JRE with DeepLabCut seemed valid with respect to the gold standard in typically developing children. Further research is needed and ongoing to assess the validity of DeepLabCut to measure the JRE in other joints than the knee (such as the ankle and the hip) and assess the reliability of measuring the JRE with DeepLabCut. Furthermore, we are working on validation of DeepLabCut to assess proprioception using the joint position sense tests in children with Cerebral Palsy. Deep learning based motion capture techniques could facilitate the implementation of proprioception assessment in clinical settings as there are no markers and no laboratory setting needed.

Multi-joint approach for assessing lower limb proprioception: reliability and precision in school-aged children

Abstract:Background: The Joint Position Reproduction (JPR) approach has been commonly used to assess joint position sense (JPS), however, no prior study investigated its psychometric properties in children. This study aimed to assess the reliability and precision of a newly developed multi–joint JPR protocol for assessing lower limb JPS in school-aged typically developing (TD) children. Methods: Ankle, knee and hip JPS was assessed in TD children (aged 5–12 years), on two different days, by a single rater using a standardized JPR protocol (re–identification of a passively placed target position of the ipsilateral joint). The mean and best error(JRE,°) between target and reproduction angle were calculated from three–dimensional(3D) kinematics for each tested joint on both sides for three trials. Furthermore, total, joint– and limb–JRE scores were provided for clinical use. For JPR–reliability, the Intraclass Correlation Coefficient(ICC,2.1) was reported. For JPR–precision, the standard error of measurement (SEM) was calculated. Results: 270 JPR trials were assessed in 15 TD children (8.6±1.2 years,8boys). The mean and best JRE, summarized for all joints for test and retest, was 3.7° and 2.5°, respectively. The ICC were poor to fair(0.01–0.44) for mean JRE, but fair to very good(0.46–0.77) for best JRE. The SEM ranged from 0.8°–1.8°, depending on the joint and side being tested. Conclusion: Evaluating ankle, knee and hip JPS in children, using passive JPR, is more reliable and precise when using the best JRE. This study highlights the need for a multi–joint JPR approach in research and clinics, and provides joint- and limb-specific SEM values. System identification: a feasible, reliable and valid way to quantify upper limb motor impairments

Abstract:Background: Upper limb impairments in a hemiparetic arm are clinically quantified by well-established clinical scales, known to suffer poor validity, reliability, and sensitivity. Alternatively, robotics can assess motor impairments by characterizing joint dynamics through system identification. In this study, we establish the merits of quantifying abnormal synergy, spasticity, and changes in joint viscoelasticity using system identification, evaluating (1) feasibility and quality of parametric estimates, (2) test-retest reliability, (3) differences between healthy controls and patients with upper limb impairments, and (4) construct validity. Methods: Forty-five healthy controls, twenty-nine stroke patients, and twenty cerebral palsy patients participated. Participants were seated with the affected arm immobilized in the Shoulder-Elbow-Perturbator (SEP). The SEP is a one-degree-of-freedom perturbator that enables applying torque perturbations to the elbow while providing varying amounts of weight support to the human arm. Participants performed either a 'do not intervene' or a resist task. Elbow joint admittance was quantified and used to extract elbow viscosity and stiffness. Fifty-four of the participants performed two sessions to establish the test-retest reliability of the parameters. Construct validity was assessed by correlating system identification parameters to parameters extracted using a SEP protocol that objectifies current clinical scales (Re-Arm protocol). Results: Feasibility was confirmed by all participants successfully completing the study protocol within ~ 25 min without reporting pain or burden. The parametric estimates were good with a variance-accounted-for of ~ 80%. A fair to excellent test-retest reliability was found ([Formula: see text]) for patients, except for elbow stiffness with full weight support ([Formula: see text]). Compared to healthy controls, patients had a higher elbow viscosity and stiffness during the 'do not intervene' task and lower viscosity and stiffness during the resist task. Construct validity was confirmed by a significant (all [Formula: see text]) but weak to moderate ([Formula: see text]) correlation with parameters from the Re-Arm protocol. Conclusions: This work demonstrates that system identification is feasible and reliable for quantifying upper limb motor impairments. Validity was confirmed by differences between patients and controls and correlations with other measurements, but further work is required to optimize the experimental protocol and establish clinical value.
Measuring Multi-Joint Upper Limb Proprioceptive Position Sense in Children with Unilateral Spastic Cerebral Palsy using 3D Kinematics

Abstract:Background: Proprioceptive position sense (PPS), which provides the awareness of the positions of one's limbs in space, is compromised in children with unilateral spastic cerebral palsy (USCP). Relationships between decreased PPS, skill acquisition, and performance are not well understood partly due to differences between measurement modalities of PPS. During assessments, individual joints are often constrained to a single plane of movement. We introduce ways to estimate PPS, simultaneously from multiple upper limb joints during unconstrained pose matching tasks, using 3D kinematics. Methods: Nineteen children with USCP, 14 age-matched typically developing children (TDC) and, 16 typically developed adults (TDA) were recruited. Participants performed contralateral upper limb pose matching tasks while blindfolded. Using 3D kinematic data extracted by a VICON Nexus motion-capture system, three ways of measuring matching performance were calculated: joint angle symmetry, joint distance symmetry, and orientation symmetry. Mann-Whitney U tests were used to statistically compare scores between groups. Spearman's rank correlation was used to investigate relationships between PPS symmetry measures and hand function in children with USCP. Results: Children with USCP presented significantly lower angle, distance, and orientation symmetries compared to TDC. TDA scores were not statistically significantly different compared to TDC. In children with USCP, orientation symmetry measures strongly correlated with several clinical hand function scores such as the box and blocks test. Conclusions:We introduced a new way to conveniently measure multi-joint PPS. Children with USCP presented lower PPS estimates compared to TDC. We also observed that orientation symmetry (limb coordination) strongly correlates with hand function.

Abstract:Proprioception is a critical component for motor functions and directly affects motor learning after neurological injuries. Conventional methods for its assessment are generally ordinal in nature and hence lack sensitivity. Robotic devices designed to promote sensorimotor learning can potentially provide quantitative precise, accurate, and reliable assessments of sensory impairments. In this paper, we investigate the clinical applicability and validity of using a planar 2 degrees of freedom robot to quantitatively assess proprioceptive deficits in post-stroke participants. Nine stroke survivors and nine healthy subjects participated in the study. Participants' hand was passively moved to the target position guided by the H-Man robot (Criterion movement) and were asked to indicate during a second passive movement towards the same target (Matching movement) when they felt that they matched the target position. The assessment was carried out on a planar surface for movements in the forward and oblique directions in the contralateral and ipsilateral sides of the tested arm. The matching performance was evaluated in terms of error magnitude (absolute and signed) and its variability. Stroke patients showed higher variability in the estimation of the target position compared to the healthy participants. Further, an effect of target was found, with lower absolute errors in the contralateral side. Pairwise comparison between individual stroke participant and control participants showed significant proprioceptive deficits in two patients. The proposed assessment of passive joint position sense was inherently simple and all participants, regardless of motor impairment level, could complete it in less than 10 minutes. Therefore, the method can potentially be carried out to detect changes in proprioceptive deficits in clinical settings.
Proprioception and its relationship with range of motion in hypermobile and normal mobile children

Abstract:To investigate differences in proprioception using four proprioceptive tests in children with and without hypermobility. Additionally, it was tested if the results on one proprioceptive test predict the results on the other tests. Of the children (8-11years), 100 were classified as normal mobile (Beighton score 0–4) and 50 as hypermobile (Beighton score 5–9). To test proprioception, in the upper extremity the unilateral and bilateral joint position reproduction tasks were used and for the lower extremity the loaded and unloaded wedges task. No differences were found in any of the proprioception tests between the two groups. Estimating the height of the wedges was easier in the loaded position (mean penalty in standing and sitting position, 4.78 and 6.19, respectively). Recalling the elbow position in the same arm resulted in smaller errors compared to tasks reproducing the position with the contralateral arm. Of the four angles used (110°, 90°, 70°, 50°), the position recall in the 90° angle had the smallest position error (1.8°). Correlations between the proprioception tests were weak (Loaded and Unloaded (r 0. 28); Uni and Bilateral (r 0.39), Upper and Lower extremity not significant). No indication of poorer proprioception was found in children with hypermobile joints compared to their normal mobile peers. Loading gives extra information that leads to fewer errors in the wedges task performed while standing, but this effect is independent of joint mobility. Proprioception test outcomes are dependent on the test used; upper extremity results do not predict lower extremity outcomes or vice versa.

Abstract:Bilateral proprioception includes the ability to sense the position and motion of one hand relative to the other, without looking. This sensory ability allows us to perform daily activities seamlessly, and its impairment is observed in various neurological disorders such as cerebral palsy and stroke. It can undergo experience-dependent plasticity, as seen in trained piano players. If its neural correlates were better understood, it would provide a useful assay and target for neurorehabilitation for people with impaired proprioception. We designed a non-invasive electroencephalography-based paradigm to assess the neural features relevant to proprioception, especially focusing on bilateral proprioception, i.e., assessing the limb distance from the body with the other limb. We compared it with a movement-only task, with and without the visibility of the target hand. Additionally, we explored proprioceptive accuracy during the tasks. We tested eleven Controls and nine Skilled musicians to assess whether sensorimotor event-related spectral perturbations in μ (8-12Hz) and low-β (12-18Hz) rhythms differ in people with musical instrument training, which intrinsically involves a bilateral proprioceptive component, or when new sensor modalities are added to the task. The Skilled group showed significantly reduced μ and low-β suppression in bilateral tasks compared to movement-only, a significative difference relative to Controls. This may be explained by reduced top-down control due to intensive training, despite this, proprioceptive errors were not smaller for this group. Target visibility significantly reduced proprioceptive error in Controls, while no change was observed in the Skilled group. During visual tasks, Controls exhibited significant μ and low-β power reversals, with significant differences relative to proprioceptive-only tasks compared to the Skilled group—possibly due to reduced uncertainty and top-down control. These results provide support for sensorimotor μ and low-β suppression as potential neuromarkers for assessing proprioceptive ability. The identification of these features is significant as they could be used to quantify altered proprioceptive neural processing in skill and movement disorders. This in turn can be useful as an assay for pre and post sensory-motor intervention research.

Abstract:Background: Assessment of bimanual movements, which are frequently impaired in children with cerebral palsy, is highly challenging in clinical practice. Instrumented measures have been developed to evaluate and help to understand impaired upper limb movement during bimanual tasks in these children. The aim of this review was to report instrumented measurement tools (3D motion analysis, sensors, etc.) used for bimanual task movement analysis, and the metrological properties of the measures in children with cerebral palsy. Methods: A systematic review was conducted (Prospero CRD42022308517). PubMed, Web of Science, Cochrane and Scopus databases were searched with relevant keywords and inclusion/exclusion criteria. Article quality and biomechanical methods were evaluated with a customized scale and metrological properties with the COSMIN checklist. Results: In total, 452 children, mostly with unilateral cerebral palsy, mean age 10.9 (SD 3.2) years, underwent quantitative bimanual assessments in the 31 included studies (mean quality score 22/32 points [SD 4.7]). The tools used were 3D motion analysis (n = 26), accelerometers (n = 2), and other instruments (cube, digitizer, etc.) (n = 3). Children performed 1-5 bimanual tasks in laboratory settings, mostly activities of daily living or game scenarios. Analyses focused mostly on spatiotemporal variables, 6 of which were specifically developed for bilateral measures (task completion time, goal synchronization, movement overlap time, interlimb coupling, continuous relative phase and asynchrony). These instrumented measurements had moderate to good discriminant and convergent validity, but reliability and responsiveness assessments were lacking. Conclusions: A large number of quantitative bimanual assessments involving different tools, bimanual tasks and specific variables developed to evaluate bimanual function were found. Development of other relevant variables and validation of these tools are needed to further determine their usefulness, both as research outcomes and to guide therapies in clinical practice. Future research, involving younger children and real-life assessments, will improve our understanding of bimanual function in children with cerebral palsy.
Assessing Proprioception in an Older Population: Reliability of a Protocol Based on Active Movement Extent Discrimination

Abstract:Proprioceptive ability - the sense of where body parts are located in space - is one of many factors thought to affect falls risk among the elderly. Active movement extent discrimination is an approach to measuring proprioception that is administered in an ecologically valid testing environment to better reflect the exercise of proprioceptive skills in daily life. The Active Movement Extent Discrimination Apparatus (AMEDA) was developed to objectively measure this proprioceptive discrimination. However, the current absolute identification testing protocol is cognitively demanding, and it yields results that are insufficiently reliable to assess performance at the individual level. The objectives of this pilot study were to test the reliability and feasibility of a proposed new AMEDA testing protocol and to explore how performance related to cognitive ability and any perceived dysfunction in the foot or ankle. We tested 42 participants (aged 19 - 94 years) three times on the ankle AMEDA using a newly developed protocol that asked participants to report whether a given angle of ankle inversion was shallower or deeper than the immediately preceding inversion. Participants also completed the Stroop test, as a measure of cognitive ability, and two validated questionnaires for identifying foot or ankle dysfunction (the Cumberland Ankle Instability Tool and the Foot and Ankle Ability Measure). The proportion of correct responses for the AMEDA test showed the expected sigmoid shape of the psychometric function as signal strength increased. The intraclass correlation coefficient measured over the three tests was 0.65 (95% confidence interval: 0.49 - 0.78), suggesting moderate reliability. We found a positive and statistically significant correlation between AMEDA performance and Stroop results but no relationship between the AMEDA score and questionnaire-measured foot or ankle dysfunction. This study confirmed that the alternative testing protocol was simple to administer and easily understood by participants.
Quantitative Assessment of Lower Limbs Gross Motor Function in Children with Cerebral Palsy Based on Surface EMG and Inertial Sensors.

Abstract:Taking advantage of motion sensing technology, a quantitative assessment method for lower limbs motor function of cerebral palsy (CP) based on the gross motor function measurement (GMFM)-24 scale was explored in this study. According to the motion analysis on GMFM-24 scale, we translated the assessment problem of GMFM-24 scale into a detection problem of different motion modes including static state, fall, step, turning, alternating gait, walking, running, lifting legs, kicking balls, and jumping. The surface electromyography (sEMG) electrodes and inertial sensors were adopted to capture motion data, and a framework integrating a series of detection algorithms was presented for the assessment of lower limbs gross motor function. Two groups of participants including 8 healthy adults and 14 CP children were recruited. A self-developed data acquisition equipment integrating 24 sEMG electrodes and 9 inertial units was adopted for data acquisition. A platform based on two laser beam sensors was used to perform cross-border detection. The parameters/thresholds of motion detection algorithms were determined by the data from healthy adults, and the lower limbs gross motor function evaluation was conducted on 14 CP children. The experimental results verified the feasibility and effectiveness of the proposed quantitative assessment method. Compared to the clinical assessment score based on GMFM-24 scale, 90.1% accuracy was obtained for evaluation of 303 tasks in 14 CP children. The objective motor function assessment method proposed has potential application value for the quantitative assessment of lower limbs motor function of CP children in clinical practice.
A new methodological approach to characterize selective motor control in children with cerebral palsy

Abstract:Introduction: Despite being a primary impairment in individuals with cerebral palsy (CP), selective motor control (SMC) is not routinely measured. Personalized treatment approaches in CP will be unattainable without the ability to precisely characterize the types and degrees of impairments in motor control. The objective of this study is to report the development and feasibility of a new methodological approach measuring muscle activation patterns during single-joint tasks to characterize obligatory muscle co-activation patterns that may underly impaired SMC. Methods: Muscle activation patterns were recorded during sub-maximal voluntary isometric contraction (sub-MVIC) tasks at the hip, knee, and ankle with an interactive feedback game to standardize effort across participants. We calculated indices of co-activation, synergistic movement, mirror movement, and overflow (indices range 0–2, greater scores equal to greater impairment in SMC) for each isolated joint task in 15 children – 8 with typical development (TD) (mean age 4.7 ± 1.0 SD years) and 7 with CP (mean age 5.8 ± 0.7 SD years). Indices were compared with Mann–Whitney tests. The relationships between the indices and gross motor function (GMFM-66) were examined with Pearson's r . Results: Mean indices were higher in the CP vs. the TD group for each of the six tasks, with mean differences ranging from 0.05 (abduction and plantarflexion) to 0.44 (dorsiflexion). There was great inter-subject variability in the CP group such that significant group differences were detected for knee flexion mirroring ( p = 0.029), dorsiflexion coactivation ( p = 0.021), and dorsiflexion overflow ( p = 0.014). Significant negative linear relations to gross motor function were found in all four indices for knee extension ( r = −0.56 to −0.75), three of the indices for ankle dorsiflexion ( r = −0.68 to −0.78) and in two of the indices for knee flexion ( r = −0.66 to −0.67), and ankle plantarflexion ( r = −0.53 to −0.60). Discussion: Indices of coactivation, mirror movement, synergy, and overflow during single-joint lower limb tasks may quantify the type and degree of impairment in SMC. Preliminary concurrent validity between several of the indices of SMC and gross motor function was observed. Our findings established the feasibility of a new methodological approach that quantifies muscle activation patterns using electromyography paired with biofeedback during single-joint movement.

Abstract:Background: The aim of the study was to assess the inter-rater and intra-rater agreement of measurements performed with the Luna EMG (electromyography) multifunctional robot, a tool for evaluation of upper limb proprioception in individuals with stroke. Methods: The study was conducted in a group of patients with chronic stroke. A total of 126 patients participated in the study, including 78 women and 48 men, on average aged nearly 60 years (mean = 59.9). Proprioception measurements were performed using the Luna EMG diagnostic and rehabilitation robot to assess the left and right upper limbs. The examinations were conducted by two raters, twice, two weeks apart. The results were compared between the raters and the examinations. Results: High consistency of the measurements performed for the right and the left hand was reflected by the interclass correlation coefficients (0.996-0.998 and 0.994-0.999, respectively) and by Pearson's linear correlation which was very high (r = 1.00) in all the cases for the right and the left hand in both the inter-rater and intra-rater agreement analyses. Conclusions: Measurements performed by the Luna EMG diagnostic and rehabilitation robot demonstrate high inter-rater and intra-rater agreement in the assessment of upper limb proprioception in patients with chronic stroke. The findings show that Luna EMG is a reliable tool enabling effective evaluation of upper limb proprioception post-stroke.

Abstract:Background: Disorders in the recovery of gait strategies in individuals with incomplete spinal cord injury (SCI) suggest difficulties in controlling lower limb intersegmental dynamics, which could relate to proprioceptive impairments. To probe discrete aspects of lower limb interjoint coordination, we present here a novel protocol to assess lower limb motor strategies and evaluate the influence of proprioceptive impairments following SCI. Methods: Twelve able-bodied controls and 16 participants with SCI performed lower limb pointing to three targets that involved combined hip and knee flexion, or hip or knee flexion only while standing, with either full or obstructed visual feedback. We quantified lower limb proprioceptive sense in individuals with SCI using a robotic gait device. We used motion analysis to determine lower limb joint angles and foot trajectory, computed inverse dynamics to quantify joint and intersegmental dynamics, and derived muscle torque as an indicator of the motor strategies produced to control the motion to each target. We used linear mixed-effects models to assess differences between the control and SCI groups on end-point performance and muscle torque, and to assess the relationship of muscle torque with end-point performance and proprioceptive sense. Results: Groups differed in motor strategies, but not end-point performance, when pointing to all three targets. Compared to controls, the SCI group had difficulty controlling knee muscle torque when performing the hip-flexion-only target (p = 0.008) or when flexing the hip and knee simultaneously (p = 0.0004). To complete the knee-flexion-only target, the SCI group had difficulties generating the required hip extensor muscle torque to maintain the thigh in neutral (p = 0.0001). These altered motor strategies in individuals with SCI were associated with proprioceptive impairments and end-point performance. Conclusion: This novel lower limb pointing task can identify disordered motor strategies in individuals with SCI, especially at the knee, and are associated with proprioceptive impairment. Variations of this paradigm can be employed to further understand differences in motor strategies between controls and individuals with SCI, and the impact of proprioceptive deficits.

Accuracy of Clinical Techniques for Evaluating Lower Limb Sensorimotor Functions Associated With Increased Fall Risk

Abstract:Background: In prior work, laboratory-based measures of hip motor function and ankle proprioceptive precision were critical to maintaining unipedal stance and fall/fall-related injury risk. However, the optimal clinical evaluation techniques for predicting these measures are unknown. Objective: To evaluate the diagnostic accuracy of common clinical maneuvers in predicting laboratory-based measures of frontal plane hip rate of torque development (Hip(RTD)) and ankle proprioceptive thresholds (AnkPRO) associated with increased fall risk. Design: Prospective, observational study. Setting: Biomechanical research laboratory. Participants: A total of 41 older subjects (aged 69.1 ± 8.3 years), 25 with varying degrees of diabetic distal symmetric polyneuropathy and 16 without. Assessments: Clinical hip strength was evaluated by manual muscle testing (MMT) and lateral plank time, defined as the number of seconds that the laterally lying subject could lift the hips from the support surface. Foot/ankle evaluation included Achilles reflex and vibratory, proprioceptive, monofilament, and pinprick sensations at the great toe. Main outcome measures: Hip(RTD), abduction and adduction, using a custom whole-body dynamometer. AnkPRO determined with subjects standing using a foot cradle system and a staircase series of 100 frontal plane rotational stimuli. Results: Pearson correlation coefficients (r) and receiver operator characteristic (ROC) curves revealed that LPT correlated more strongly with Hip(RTD) (r/P = 0.61/<.001 and 0.67/<.001, for abductor/adductor, respectively) than did hip abductor MMT (r/P = 0.31/.044). Subjects with greater vibratory and proprioceptive sensation, and intact Achilles reflexes, monofilament, and pin sensation had more precise AnkPRO. LPT of <12 seconds yielded a sensitivity/specificity of 91%/80% for identifying Hip(RTD) < 0.25 (body size in Newton-meters), and vibratory perception of <8 seconds yielded a sensitivity/specificity of 94%/80% for the identification of AnkPRO >1.0°. Conclusions: LPT is a more effective measure of Hip(RTD) than MMT. Similarly, clinical vibratory sense and monofilament testing are effective measures of AnkPRO, whereas clinical proprioceptive sense is not.
Association between upper limb clinical tests and accelerometry metrics for arm use in daily life in children with unilateral cerebral palsy

Abstract:Purpose: To evaluate the association between upper-limb (UL) clinical tests and UL accelerometry-derived metrics in children with unilateral Cerebral Palsy (CP). Methods: In this cross-sectional study, twenty children with unilateral CP and Manual Ability Classification System level I-III were included. Outcomes of the Assisting Hand Assessment, Box and Block-Test and accelerometry metrics were collected in the clinical setting and in daily life. UL asymmetry index (i.e., the ratio between the well-functioning UL and the affected UL use) was evaluated in different physical activity levels and relative use of UL was evaluated during daily living. Spearman's correlation was used to determine the association between UL clinical tests and accelerometry metrics in a clinical setting and in daily life. Results: The strongest negative association was between the Assisting Hand Assessment units and accelerometry metrics during the sedentary time in daily life (rs = -0.64). The asymmetries between ULs were highest during the child's sedentary time (asymmetry index: 45.15) compared to when the child was in light (asymmetry index: 23.97) or higher intensity physical activity (asymmetry index: 13.39). The children used both ULs simultaneously for 44% of the time during daily life. Conclusion: Accelerometry metrics may provide additional objective information to clinical tests by quantifying the amount of UL movements and the amount of asymmetry between the upper limbs in daily life.

Abstract:This study evaluated a dual wrist‐worn accelerometry and machine learning approach to measure functional use of the unilateral hand in children with cerebral palsy. Machine learning outperformed the traditional activity counts approach in predicting functional unilateral hand use and distinguishing non‐functional versus functional movements, but its performance decreased in the leave‐one‐subject‐out validation, suggesting more work is needed before it can be translated into practice. Nonetheless, wearable sensors and machine learning have potential for monitoring functional hand use in children with unilateral cerebral palsy. Aim To investigate wearable sensors for measuring functional hand use in children with unilateral cerebral palsy (CP). Method Dual wrist‐worn accelerometry data were collected from three females and seven males with unilateral CP (mean age = 10 years 2 months [SD 3 years]) while performing hand tasks during video‐recorded play sessions. Video observers labelled instances of functional and non‐functional hand use. Machine learning was compared to the conventional activity count approach for identifying unilateral hand movements as functional or non‐functional. Correlation and agreement analyses compared the functional usage metrics derived from each method. Results The best‐performing machine learning approach had high precision and recall when trained on an individual basis (F1 = 0.896 [SD 0.043]). Abstract:Unilateral spastic Cerebral Palsy (UCP) results from congenital brain injury, and Magnetic Resonance Imaging (MRI) has a role in understanding the etiology and severity of brain insult. In UCP, functional impairment predominantly occurs in the upper limb (UL) of the more affected side, where manual ability and dexterity are typically reduced. Also, mirror movements (MMs), are often present in UCP, with a further possible negative functional impact. This study aims to investigate the relationships among neuroanatomical characteristics of brain injury at MRI, manual functional impairment and MMs, in children with UCP. Thirty-five children with UCP participated in the study (20, M = 15, F, mean age 9.2 ± 3.5 years). Brain lesions at MRI were categorized according to the Magnetic Resonance Classification System (MRICS) and by using a semi-quantitative MRI (sqMRI) scale. Gross manual performance was assessed through Manual Ability Classification System (MACS) and the Box and Block Test (BBT), and MMs by Woods and Teuber scale, for both hands. Non-parametric correlation analyses were run to determine the relationship between neuroanatomical and functional features. Regression models were run to explore the contribution of neuroanatomical features and MMs to UL function. Correlation analyses revealed moderate to strong associations between sqMRI scores contralateral to the more affected side and UL functional impairment on MACS and BBT, with more severe brain injuries significantly correlating with poorer function in the more affected hand. No association emerged between brain lesion severity scores and MMs. MRICS showed no association with MACS or BBT, while a significant correlation emerged between MRICS category and MMs in the more affected hand, with brain lesion category that are suggestive of presumed earlier injury being associated with more severe MMs. Finally, exploratory regression analyses showed that neuroanatomical characteristics of brain injury and MMs contributed to the variability of UL functional impairment. This study contributes to the understanding of the neuroanatomical and neurological correlates of some aspects of manual functional impairment in UCP by using a simple clinical brain MRI assessment.

Abstract:BackgroundBilateral proprioception deficits were reported in stroke survivors. However, whether bilateral proprioception deficits exist in the ankle joint after stroke was unclear. Ankle proprioception is a significant predictor of balance dysfunction after stroke, and previous studies to date are lacking appropriate evaluation methods.ObjectivesWe want to determine whether the active movement extent discrimination apparatus (AMEDA) is a reliable tool for assessing ankle proprioceptive acuity in stroke survivors and the presence of deficits in ankle proprioception on the affected and unaffected sides in patients after stroke.MethodsBilateral ankle proprioception was assessed in 20 stroke patients and 20 age-matched healthy controls using AMEDA. Test-retest reliability was assessed using the intraclass correlation coefficient (ICC).ResultsThe ICC in the affected and unaffected sides was 0.713 and 0.74, respectively. Analysis of variance revealed significant deficits in ankle proprioception in subacute stroke survivors vs. healthy controls (F = 2.719, p = 0.045). However, there were no significant differences in proprioception acuity scores between the affected and unaffected sides in patients after stroke (F = 1.14, p = 0.331).ConclusionsStroke survivors had bilateral deficits in ankle proprioceptive acuity during active movements compared with age-matched healthy controls, underscoring the need to evaluate these deficits on both sides of the body and develop effective sensorimotor rehabilitation methods for this patient population. The AMEDA can reliably determine bilateral ankle proprioceptive acuity in stroke survivors.
