Abstract:Most established clinical walking tests assess specific aspects of movement function (velocity, endurance, etc.) but are generally unable to determine specific biomechanical or neurological deficits that limit an individual's ability to walk. Recently, inertial measurement units (IMU) have been used to collect objective kinematic data for gait analysis and could be a valuable extension for clinical assessments (e.g., functional walking measures). This study assesses the reliability of an IMU-based overground gait analysis during the 2-min walk test (2mWT) in individuals with spinal cord injury (SCI). Furthermore, the study elaborates on the capability of IMUs to distinguish between different gait characteristics in individuals with SCI. Twenty-six individuals (aged 22–79) with acute or chronic SCI (AIS: C and D) completed the 2mWT with IMUs attached above each ankle on 2 test days, separated by 1 to 7 days. The IMU-based gait analysis showed good to excellent test–retest reliability (ICC: 0.77–0.99) for all gait parameters. Gait profiles remained stable between two measurements. Sensor-based gait profiling was able to reveal patient-specific gait impairments even in individuals with the same walking performance in the 2mWT. IMUs are a valuable add-on to clinical gait assessments and deliver reliable information on detailed gait pathologies in individuals with SCI.

Comparison of yellow dent and NutriDense corn hybrids in swine diets.

Abstract:Two experiments were conducted to verify the feeding value of NutriDense (ND) and Nutri-Dense Low-Phytate (NDLP) corn (Exseed Genetics LLC, BASF Plant Science, Research Triangle Park, NC) relative to that of yellow dent (YD) corn in swine diets. NutriDense corn is a high-protein, high-oil variety, and NDLP is a high-protein, high-oil, low-phytate variety. In Exp. 1, 315 nursery pigs that initially weighed 15.2 kg were used in a 21-d growth assay. Dietary treatments were arranged in a 3 x 3 factorial; main effects were corn source (YD, ND, and NDLP) and added fat (0, 3, or 6%, as-fed basis). Diets were formulated to contain 3.83 g of lysine/Mcal using calculated nutrient values. There were no corn source x fat interactions observed. Pigs fed YD, ND, and NDLP had ADG of 750, 734, and 738 g/d and G:F of 0.64, 0.66, and 0.65, respectively. No differences (P > 0.10) in ADG were observed among the three corn sources; however, pigs fed diets containing either ND or NDLP corn had decreased ADFI (P < 0.02) and improved G:F (P < 0.05) compared with pigs fed diets containing YD corn. Increasing dietary fat increased ADG (727, 746, and 748 g/d; linear, P < 0.04) and G:F (0.62, 0.66, and 0.68; linear, P < 0.01) and decreased ADFI (linear, P < 0.01). Using the NRC (1998) value for ME in YD corn, we calculated the energy value for ND and NDLP based on G:F differences compared with pigs fed YD corn. These data indicated the ME values for ND and NDLP corn are 4.5 and 2.5% greater (3,575 and 3,505 Kcal/kg), respectively, than for YD corn (3,420 Kcal/kg). In Exp. 2, 1,144 gilts (initial BW = 50.1 kg) were used in a commercial research facility to evaluate the effects of corn source (ND and YD) and added fat (0, 3, or 6%, as-fed basis) in a 2 x 3 factorial on pig performance and carcass traits. There was a corn source x fat interaction for ADFI and G:F. Increasing added fat resulted in greater changes in ADFI and G:F in pigs fed YD corn diets compared with those fed ND corn. Feeding ND corn increased ADG (main effect, P < 0.04), and greater percentages of added fat increased ADG (main effect; linear, P < 0.01). Results of Exp. 2 suggest that ND corn has 5.3% more ME than YD corn. The additional energy provided by ND corn improves G:F in both nursery and grow-finish pigs, and ND corn offers a means of formulating diets more concentrated in energy than YD corn.
Using Sensor Technology to Measure Gait Capacity and Gait Performance in Rehabilitation Inpatients with Neurological Disorders

Abstract:The aim of this study was to objectively assess and compare gait capacity and gait performance in rehabilitation inpatients with stroke or incomplete spinal cord injury (iSCI) using inertial measurement units (IMUs). We investigated how gait capacity (what someone can do) is related to gait performance (what someone does). Twenty-two inpatients (11 strokes, 11 iSCI) wore ankle positioned IMUs during the daytime to assess gait. Participants completed two circuits to assess gait capacity. These were videotaped to certify the validity of the IMU algorithm. Regression analyses were used to investigate if gait capacity was associated with gait performance (i.e., walking activity and spontaneous gait characteristics beyond therapy time). The ankle positioned IMUs validly assessed the number of steps, walking time, gait speed, and stride length (r ≥ 0.81). The walking activity was strongly (r ≥ 0.76) related to capacity-based gait speed. Maximum spontaneous gait speed and stride length were similar to gait capacity. However, the average spontaneous gait speed was half the capacity-based gait speed. Gait capacity can validly be assessed using IMUs and is strongly related to gait performance in rehabilitation inpatients with neurological disorders. Measuring gait performance with IMUs provides valuable additional information about walking activity and spontaneous gait characteristics to inform about functional recovery.
Validation of Non-Restrictive Inertial Gait Analysis of Individuals with Incomplete Spinal Cord Injury in Clinical Settings

Abstract:Inertial Measurement Units (IMUs) have gained popularity in gait analysis and human motion tracking, and they provide certain advantages over stationary line-of-sight-dependent Optical Motion Capture (OMC) systems. IMUs appear as an appropriate alternative solution to reduce dependency on bulky, room-based hardware and facilitate the analysis of walking patterns in clinical settings and daily life activities. However, most inertial gait analysis methods are unpractical in clinical settings due to the necessity of precise sensor placement, the need for well-performed calibration movements and poses, and due to distorted magnetometer data in indoor environments as well as nearby ferromagnetic material and electronic devices. To address these limitations, recent literature has proposed methods for self-calibrating magnetometer-free inertial motion tracking, and acceptable performance has been achieved in mechanical joints and in individuals without neurological disorders. However, the performance of such methods has not been validated in clinical settings for individuals with neurological disorders, specifically individuals with incomplete Spinal Cord Injury (iSCI). In the present study, we used recently proposed inertial motion-tracking methods, which avoid magnetometer data and leverage kinematic constraints for anatomical calibration. We used these methods to determine the range of motion of the Flexion/Extension (F/E) hip and Abduction/Adduction (A/A) angles, the F/E knee angles, and the Dorsi/Plantar (D/P) flexion ankle joint angles during walking. Data (IMU and OMC) of five individuals with no neurological disorders (control group) and five participants with iSCI walking for two minutes on a treadmill in a self-paced mode were analyzed. For validation purposes, the OMC system was considered as a reference. The mean absolute difference (MAD) between calculated range of motion of joint angles was 5.00°, 5.02°, 5.26°, and 3.72° for hip F/E, hip A/A, knee F/E, and ankle D/P flexion angles, respectively. In addition, relative stance, swing, double support phases, and cadence were calculated and validated. The MAD for the relative gait phases (stance, swing, and double support) was 1.7%, and the average cadence error was 0.09 steps/min. The MAD values for RoM and relative gait phases can be considered as clinically acceptable. Therefore, we conclude that the proposed methodology is promising, enabling non-restrictive inertial gait analysis in clinical settings.

Abstract:Abstract Background Inertial measurement units (IMUs) offer the ability to measure walking gait through a variety of biomechanical outcomes (e.g., spatiotemporal, kinematics, other). Although many studies have assessed their validity and reliability, there remains no quantitive summary of this vast body of literature. Therefore, we aimed to conduct a systematic review and meta-analysis to determine the i) concurrent validity and ii) test-retest reliability of IMUs for measuring biomechanical gait outcomes during level walking in healthy adults. Methods Five electronic databases were searched for journal articles assessing the validity or reliability of IMUs during healthy adult walking. Two reviewers screened titles, abstracts, and full texts for studies to be included, before two reviewers examined the methodological quality of all included studies. When sufficient data were present for a given biomechanical outcome, data were meta-analyzed on Pearson correlation coefficients (r) or intraclass correlation coefficients (ICC) for validity and reliability, respectively. Alternatively, qualitative summaries of outcomes were conducted on those that could not be meta-analyzed. Results A total of 82 articles, assessing the validity or reliability of over 100 outcomes, were included in this review. Seventeen biomechanical outcomes, primarily spatiotemporal parameters, were meta-analyzed. The validity and reliability of step and stride times were found to be excellent. Similarly, the validity and reliability of step and stride length, as well as swing and stance time, were found to be good to excellent. Alternatively, spatiotemporal parameter variability and symmetry displayed poor to moderate validity and reliability. IMUs were also found to display moderate reliability for the assessment of local dynamic stability during walking. The remaining biomechanical outcomes were qualitatively summarized to provide a variety of recommendations for future IMU research. Conclusions The findings of this review demonstrate the excellent validity and reliability of IMUs for mean spatiotemporal parameters during walking, but caution the use of spatiotemporal variability and symmetry metrics without strict protocol. Further, this work tentatively supports the use of IMUs for joint angle measurement and other biomechanical outcomes such as stability, regularity, and segmental accelerations. Unfortunately, the strength of these recommendations are limited based on the lack of high-quality studies for each outcome, with underpowered and/or unjustified sample sizes (sample size median 12; range: 2–95) being the primary limitation.

Complementing Clinical Gait Assessments of Spinal Cord Injured Individuals using Wearable Movement Sensors

Abstract:Today's standard clinical practice to assess the walking ability of patients with neurological disorders during rehabilitation is based on simple gait tests such as the six-minute walking test (6MWT). Since the outcome of these tests is the average walking speed only, the aim of this work was to show that the application of movement sensors during a standardized walking test for the population of spinal cord injured (SCI) patients provides additional information on gait quality not directly described by the average speed. Hence, gait features that are related to quantitative and qualitative aspects of gait were extracted from the ankle sensor recordings of 29 SCI subjects and 19 healthy controls performing the 6MWT. The subjects were clustered into groups based on these gait features, and six gait features were selected to demonstrate the key differences between the clusters. The correlation of these features to the outcome of the 6MWT is discussed with their implications on gait quality.

Abstract:The aim was to assess concurrent validity and test–retest reliability of spatiotemporal gait parameters from a thoracic-placed inertial measurement unit (IMU) in lab- (Phase One) and field-based (Phase Two) conditions. Spatiotemporal gait parameters were compared (target speeds 3, 5 and 7.5 m·s −1 ) between a 100 Hz IMU and an optical measurement system (OptoJump Next, 1000 hz) in 14 trained individuals (Phase One). Additionally, 29 English Premier League football players performed weekly 3 × 60 m runs (5 m·s −1 ; observations = 1227; Phase Two). Mixed effects modelling assessed the effect of speed on agreement between systems (Phase One) and test–retest reliability (Phase Two). IMU step time showed strong agreement (<0.3%) regardless of individual or running speed. Direction of mean biases up to 40 ms for contact and flight time depended on the running speed and individual. Step time, length and frequency were most reliable (coefficient of variation = 1.3-1.4%) but confounded by running speed. Step time, length and frequency derived from a thoracic-placed IMU can be used confidently. Contact time could be used if bias is corrected for each individual. To optimise test–retest reliability, a minimum running distance of 40 m is needed to ensure 10 constant-speed steps is gathered.

Abstract:Spinal cord injury (SCI) patients suffer from diverse gait deficits depending on the severity of their injury. Gait assessments can objectively track the progress during rehabilitation and support clinical decision making, but a comprehensive gait analysis requires far more complex setups and time-consuming protocols that are not feasible in the daily clinical routine. As using inertial sensors for mobile gait analysis has started to gain ground, this work aimed to develop a sensor-based gait analysis for the specific population of SCI patients that measures the spatio-temporal parameters of typical gait laboratories for day-to-day clinical applications. The proposed algorithm uses shank-mounted inertial sensors and personalized thresholds to detect steps and gait events according to the individual gait profiles. The method was validated in nine SCI patients and 17 healthy controls walking on an instrumented treadmill while wearing reflective markers for motion capture used as a gold standard. The sensor-based algorithm (i) performed similarly well for the two cohorts and (ii) is robust enough to cover the diverse gait deficits of SCI patients, from slow (0.3 m/s) to preferred walking speeds.

Abstract:The restoration of gait and mobility after stroke is an important and challenging therapy goal due to the complexity of the potentially impaired functions. As a result, precise and clinically feasible assessment methods are required for personalized gait rehabilitation after stroke. The aim of this study is to investigate the reliability and validity of a sensor-based gait analysis system in stroke survivors with different severities of gait deficits. For this purpose, 28 chronic stroke survivors (9 women, ages: 62.04 ± 11.68 years) with mild to moderate walking impairments performed a set of ambulatory assessments (3× 10MWT, 1× 6MWT per session) twice while being equipped with a sensor suit. The derived gait reports provided information about speed, step length, step width, swing and stance phases, as well as joint angles of the hip, knee, and ankle, which we analyzed for test-retest reliability and hypothesis testing. Further, test-retest reliability resulted in a mean ICC of 0.78 (range: 0.46–0.88) for walking 10 m and a mean ICC of 0.90 (range: 0.63–0.99) for walking 6 min. Additionally, all gait parameters showed moderate-to-strong correlations with clinical scales reflecting lower limb function. These results support the applicability of this sensor-based gait analysis system for individuals with stroke-related walking impairments.

Absolute and Relative Reliability of Spatiotemporal Gait Characteristics Extracted from an Inertial Measurement Unit among Senior Adults Using a Passive Hip Exoskeleton: A Test-Retest Study

Abstract:Background: Seniors wearing a passive hip exoskeleton (Exo) show increased walking speed and step length but reduced cadence. We assessed the test-retest reliability of seniors' gait characteristics with Exo. Methods: Twenty seniors walked with and without Exo (noExo) on a 10 m indoor track over two sessions separated by one week. Speed, step length, cadence and step time variability were extracted from one inertial measurement unit (IMU) placed over the L5 vertebra. Relative and absolute reliability were assessed using the intraclass correlation coefficient (ICC), standard error of measurement (SEM) and minimal detectable change (MDC). Results: The relative reliability of speed, step length, cadence and step time variability ranged from "almost perfect to substantial" for Exo and noExo with ICC values between 0.75 and 0.87 and 0.60 and 0.92, respectively. The SEM and MDC values for speed, step length cadence and step time variability during Exo and noExo were <0.002 and <0.006 m/s, <0.002 and <0.005 m, <0.30 and <0.83 steps/min and <0.38 s and <1.06 s, respectively. Conclusions: The high test-retest reliability of speed, step length and cadence estimated from IMU suggest a robust extraction of spatiotemporal gait characteristics during exoskeleton use. These findings indicate that IMUs can be used to assess the effects of wearing an exoskeleton on seniors, thus offering the possibility of conducting longitudinal studies.

Abstract:Inertial measurement units (IMUs) are possible alternatives to motion-capture systems (Mocap) for gait analysis. However, IMU-based system performance must be validated before widespread clinical use. Therefore, this study evaluated the validity of IMUs using statistical parametric mapping (SPM) for gait analysis. Ten healthy males (age, 30.10 ± 3.28 years; height, 175.90 ± 5.17 cm; weight: 82.80 ± 17.15 kg) participated in this study; they were asked to walk normally on a treadmill. Data were collected during walking at the self-selected speeds (preferred speed, 1.34 ± 0.10 m/s) using both Mocap and an IMU. Calibration was performed directly before each gait measurement to minimize the IMU drift error over time. The lower-extremity joint angles of the hip, knee, and ankle were calculated and compared with IMUs and Mocap; the hip-joint angle did not differ significantly between IMUs and Mocap. There were significant differences in the discrete (max, min, and range of motion) and continuous variables (waveform: 0–100%) of the knee and ankle joints between IMUs and Mocap, particularly on the swing phase (p < 0.05). Our results suggest that IMU-based data can be used confidently during the stance phase but needs evaluation regarding the swing phase in gait analysis.

Abstract:Background and objectives: Hereditary spastic paraplegia (HSP) causes progressive spasticity and weakness of the lower limbs. As neurologic examination and the clinical Spastic Paraplegia Rating Scale (SPRS) are subject to potential patient-dependent and clinician-dependent bias, instrumented gait analysis bears the potential to objectively quantify impaired gait. The aim of this study was to investigate gait cyclicity parameters by application of a mobile gait analysis system in a cross-sectional cohort of patients with HSP and a longitudinal fast progressing subcohort. Methods: Using wearable sensors attached to the shoes, patients with HSP and controls performed a 4 × 10 m walking test during regular visits in 3 outpatient centers. Patients were also rated according to the SPRS, and in a subset, questionnaires on quality of life and fear of falling were obtained. An unsupervised segmentation algorithm was used to extract stride parameters and respective coefficients of variation. Results: Mobile gait analysis was performed in a total of 112 ambulatory patients with HSP and 112 age-matched and sex-matched controls. Although swing time was unchanged compared with controls, there were significant increases in the duration of the total stride phase and the duration of the stance phase, both regarding absolute values and coefficients of variation values. Although stride parameters did not correlate with age, weight, or height of the patients, there were significant associations of absolute stride parameters with single SPRS items reflecting impaired mobility (|r| > 0.50), with patients' quality of life (|r| > 0.44), and notably with disease duration (|r| > 0.27). Sensor-derived coefficients of variation, on the other hand, were associated with patient-reported fear of falling (|r| > 0.41) and cognitive impairment (|r| > 0.40). In a small 1-year follow-up analysis of patients with complicated HSP and fast progression, the absolute values of mobile gait parameters had significantly worsened compared with baseline. Discussion: The presented wearable sensor system provides parameters of stride characteristics which seem clinically valid to reflect gait impairment in HSP. Owing to the feasibility regarding time, space, and costs, this study forms the basis for larger scale longitudinal and interventional studies in HSP.

Abstract:Given the high morbidity related to the progression of gait deficits in spinocerebellar ataxias (SCA), there is a growing interest in identifying biomarkers that can guide early diagnosis and rehabilitation. Spatiotemporal parameter (STP) gait analysis using inertial measurement units (IMUs) has been increasingly studied in this context. This study evaluated STP profiles in SCA types 3 and 10, compared them to controls, and correlated them with clinical scales. IMU portable sensors were used to measure STPs under four gait conditions: self-selected pace (SSP), fast pace (FP), fast pace checking-boxes (FPCB), and fast pace with serial seven subtractions (FPS7). Compared to healthy subjects, both SCA groups had higher values for step time, variability, and swing time, with lower values for gait speed, cadence, and step length. We also found a reduction in speed gain capacity in both SCA groups compared to controls and an increase in speed dual-task cost in the SCA10 group. However, there were no significant differences between the SCA groups. Swing time, mean speed, and step length were correlated with disease severity, risk of falling and functionality in both clinical groups. In the SCA3 group, fear of falling was correlated with cadence. In the SCA10 group, results of the Montreal cognitive assessment test were correlated with step time, mean speed, and step length. These results show that individuals with SCA3 and SCA10 present a highly variable, short-stepped, slow gait pattern compared to healthy subjects, and their gait quality worsened with a fast pace and dual-task involvement.

Validity and Reliability of Thoracic-Mounted Inertial Measurement Units to Derive Gait Characteristics During Running

J Strength Cond Res XX(X): 000–000, 2023—Inertial measurement units (IMUs) attached to the tibia or lumbar spine can be used to analyze running gait but, with team-sports, are often contained in global navigation satellite system (GNSS) units worn on the thoracic spine. We assessed the validity and reliability of thoracic-mounted IMUs to derive gait characteristics, including peak vertical ground reaction force (vGRF peak ) and vertical stiffness (K vert ). Sixteen recreationally active subjects performed 40 m run throughs at 3–4, 5–6, and 7–8 m·s −1 . Inertial measurement units were attached to the tibia, lumbar, and thoracic spine, whereas 2 GNSS units were also worn on the thoracic spine. Initial contact (IC) from a validated algorithm was evaluated with F1 score and agreement (mean difference ± SD ) of gait data with the tibia and lumbar spine using nonparametric limits of agreement (LoA). Test-retest error {coefficient of variation, CV (95% confidence interval [CI])} established reliability. Thoracic IMUs detected a nearly perfect proportion (F1 ≥ 0.95) of IC events compared with tibia and lumbar sites. Step length had the strongest agreement (0 ± 0.04 m) at 3–4 m·s −1 , whereas contact time improved from 3 to 4 (−0.028 ± 0.018 second) to 7–8 m·s −1 (−0.004 ± 0.013 second). All values for K vert fell within the LoA at 7–8 m·s −1 . Test-retest error was ≤12.8% for all gait characteristics obtained from GNSS units, where K vert was most reliable at 3–4 m·s −1 (6.8% [5.2, 9.6]) and vGRF peak at 7–8 m·s −1 (3.7% [2.5, 5.2]). The thoracic-spine site is suitable to derive gait characteristics, including K vert , from IMUs within GNSS units, eliminating the need for additional sensors to analyze running gait.

Validity and Sensitivity of an Inertial Measurement Unit-Driven Biomechanical Model of Motor Variability for Gait

Abstract:Motor variability in gait is frequently linked to fall risk, yet field-based biomechanical joint evaluations are scarce. We evaluated the validity and sensitivity of an inertial measurement unit (IMU)-driven biomechanical model of joint angle variability for gait. Fourteen healthy young adults completed seven-minute trials of treadmill gait at several speeds and arm swing amplitudes. Trunk, pelvis, and lower-limb joint kinematics were estimated by IMU- and optoelectronic-based models using OpenSim. We calculated range of motion (ROM), magnitude of variability (meanSD), local dynamic stability (λmax), persistence of ROM fluctuations (DFAα), and regularity (SaEn) of each angle over 200 continuous strides, and evaluated model accuracy (RMSD: root mean square difference), consistency (ICC2,1: intraclass correlation), biases, limits of agreement, and sensitivity to within-participant gait responses (effects of speed and swing). RMSDs of joint angles were 1.7-9.2° (pooled mean of 4.8°), excluding ankle inversion. ICCs were mostly good to excellent in the primary plane of motion for ROM and in all planes for meanSD and λmax, but were poor to moderate for DFAα and SaEn. Modelled speed and swing responses for ROM, meanSD, and λmax were similar. Results suggest that the IMU-driven model is valid and sensitive for field-based assessments of joint angle time series, ROM in the primary plane of motion, magnitude of variability, and local dynamic stability.
Evaluating the Sensoria Smart Socks Gait Monitoring System for Rehabilitation Outcomes

Abstract:Background: Gait monitoring is important for rehabilitation but clinic-based measurements provide a brief snapshot of gait patterns. Many consumer-based activity monitors facilitate continuous gait monitoring in daily life but may be subject to measurement errors. This study examines a novel consumer-based activity monitor that is worn on the feet, and therefore not susceptible to inaccurately detecting upper body movements as gait. Objective: To evaluate the clinical validity of Sensoria smart socks, a commercially available wireless gait monitoring technology. Design: A method comparison study between the GAITRite, a criterion standard for clinical gait monitoring, and Sensoria smart socks. Setting: Outpatient rehabilitation clinic. Participants: Thirty individuals capable of supervised ambulation, with or without assistive devices, were recruited through the use of flyers and emails. Data were analyzed for 29 participants. Fifteen participants had no neurologic diagnosis. Fourteen participants had a neurologic diagnosis that could result in gait impairments. Diagnoses included Parkinson disease, stroke, brain injury, developmental delay, and acoustic neuroma. Methods: Participants completed three gait trials with simultaneous measurements by the smart socks and the GAITRite. Main outcome measures: Measurements of step count, cadence, and velocity were compared between the two gait monitoring systems. Results: There was no significant difference in step count measurements between the two systems. Although there was a significant difference in cadence measurements for the total sample group, the mean difference fell within the GAITRite Standard Error of Measurement. There was no significant difference in velocity measurements for the total sample group and the mean difference fell within the GAITRite Standard Error of Measurement. Conclusion: These results support the clinical validity of the smart socks for measuring step number and velocity. Further investigation is warranted to determine the efficacy and clinical value of the smart socks system for measuring cadence and for monitoring gait over longer distances outside the clinic. Abstract:Abstract Background Inertial measurement units (IMUs) are useful in monitoring running and alerting running-related injuries in various sports settings. However, the quantitative summaries of the validity and reliability of the measurements from IMUs during running are still lacking. The purpose of this review was to investigate the concurrent validity and test–retest reliability of IMUs for measuring gait spatiotemporal outcomes and lower extremity kinematics of health adults during running. Methods PubMed, CINAHL, Embase, Scopus and Web of Science electronic databases were searched from inception until September 2021. The inclusion criteria were as follows: (1) evaluated the validity or reliability of measurements from IMUs, (2) measured specific kinematic outcomes, (3) compared measurements using IMUs with those obtained using reference systems, (4) collected data during running, (5) assessed human beings and (6) were published in English. Eligible articles were reviewed using a modified quality assessment. A meta-analysis was performed to assess the pooled correlation coefficients of validity and reliability. Results Twenty-five articles were included in the systematic review, and data from 12 were pooled for meta-analysis. The methodological quality of studies ranged from low to moderate. Concurrent validity is excellent for stride length (intraclass correlation coefficient (ICC) (95% confidence interval (CI)) = 0.937 (0.859, 0.972), p < 0.001), step frequency (ICC (95% CI) = 0.926 (0.896, 0.948), r (95% CI) = 0.989 (0.957, 0.997), p < 0.001) and ankle angle in the sagittal plane ( r (95% CI) = 0.939 (0.544, 0.993), p = 0.002), moderate to excellent for stance time (ICC (95% CI) = 0.664 (0.354, 0.845), r (95% CI) = 0.811 (0.701, 0.881), p < 0.001) and good for running speed (ICC (95% CI) = 0.848 (0.523, 0.958), p = 0.0003). The summary Fisher's Z value of flight time was not statistically significant ( p = 0.13). Similarly, the stance time showed excellent test–retest reliability (ICC (95% CI) = 0.954 (0.903, 0.978), p < 0.001) and step frequency showed good test–retest reliability (ICC (95% CI) = 0.896 (0.837, 0.933), p < 0.001). Conclusions Findings in the current review support IMUs measurement of running gait spatiotemporal parameters, but IMUs measurement of running kinematics on lower extremity joints needs to be reported with caution in healthy adults. Abstract:A dynamic gain equalizer (DGE) is required to automatically compensate for gain variations related to fiber amplifiers. This paper proposes a DGE with thermooptic (TO) phase shifters based on a single-filtered-arm interferometer. A design procedure was newly developed for the control of the TO phase shifters, which could be used to accurately control the intended attenuation profile in one adjustment. In this procedure, the Taylor series method was used as an optimization algorithm, and thermal crosstalk was taken into account. From the point of view of thermal crosstalk, the behavior of phase shifters in an array and the effect of three-dimensional (3-D) trenches formed around the phase shifters were precisely investigated. A DGE module based on 1.5%-/spl Delta/ planar lightwave circuits and the 3-D trench structure are demonstrated. The measured spectra were found to be in good agreement with arbitrarily designed profiles under open-loop control, even when there was some thermal crosstalk between phase shifters. The results indicate that the design procedure and the estimation of the thermal crosstalk are effective for accurate control of the DGE.
Feasibility of a Sensor-Based Technological Platform in Assessing Gait and Sleep of In-Hospital Stroke and Incomplete Spinal Cord Injury (iSCI) Patients

Abstract:Recovery of the walking function is one of the most common rehabilitation goals of neurological patients. Sufficient and adequate sleep is a prerequisite for recovery or training. To objectively monitor patients' progress, a combination of different sensors measuring continuously over time is needed. A sensor-based technological platform offers possibilities to monitor gait and sleep. Implementation in clinical practice is of utmost relevance and has scarcely been studied. Therefore, this study examined the feasibility of a sensor-based technological platform within the clinical setting. Participants (12 incomplete spinal cord injury (iSCI), 13 stroke) were asked to wear inertial measurement units (IMUs) around the ankles during daytime and the bed sensor was placed under their mattress for one week. Feasibility was established based on missing data, error cause, and user experience. Percentage of missing measurement days and nights was 14% and 4%, respectively. Main cause of lost measurement days was related to missing IMU sensor data. Participants were not impeded, did not experience any discomfort, and found the sensors easy to use. The sensor-based technological platform is feasible to use within the clinical rehabilitation setting for continuously monitoring gait and sleep of iSCI and stroke patients.
