Abstract:Prediabetes is a critical health condition characterized by elevated blood glucose levels that fall below the threshold for Type 2 diabetes (T2D) diagnosis. Accurate identification of prediabetes is essential to forestall the progression to T2D among at-risk individuals. This study aims to pinpoint the most effective machine learning (ML) model for prediabetes prediction and to elucidate the key biological variables critical for distinguishing individuals with prediabetes. Utilizing data from the Canadian Primary Care Sentinel Surveillance Network (CPCSSN), our analysis included 6,414 participants identified as either nondiabetic or prediabetic. A rigorous selection process led to the identification of ten variables for the study, informed by literature review, data completeness, and the evaluation of collinearity. Our comparative analysis of seven ML models revealed that the Deep Neural Network (DNN), enhanced with early stop regularization, outshined others by achieving a recall rate of 60%. This model’s performance underscores its potential in effectively identifying prediabetic individuals, showcasing the strategic integration of ML in healthcare. While the model reflects a significant advancement in prediabetes prediction, it also opens avenues for further research to refine prediction accuracy, possibly by integrating novel biological markers or exploring alternative modeling techniques. The results of our work represent a pivotal step forward in the early detection of prediabetes, contributing significantly to preventive healthcare measures and the broader fight against the global epidemic of Type 2 diabetes.

Abstract:Diabetes, particularly Type 2 diabetes (T2D), poses a substantial global health burden, compounded by its associated complications such as cardiovascular diseases, kidney failure, and vision impairment. Early detection of T2D is critical for improving healthcare outcomes and optimizing resource allocation. In this study, we address the gap in early T2D detection by leveraging machine learning (ML) techniques on gene expression data obtained from T2D patients. Our primary objective was to enhance the accuracy of early T2D detection through advanced ML methodologies and increase the model's trustworthiness using the explainable artificial intelligence (XAI) technique. Analyzing the biological mechanisms underlying T2D through gene expression datasets represents a novel research frontier, relatively less explored in previous studies. While numerous investigations have focused on utilizing clinical and demographic data for T2D prediction, the integration of molecular insights from gene expression datasets offers a unique and promising avenue for understanding the pathophysiology of the disease. By employing six ML classifiers on data sourced from NCBI's Gene Expression Omnibus (GEO), we observed promising performance across all models. Notably, the XGBoost classifier exhibited the highest accuracy, achieving 97%. Our study addresses a notable gap in early T2D detection methodologies, emphasizing the importance of leveraging gene expression data and advanced ML techniques.

Exploring Nutritional Influence on Blood Glucose Forecasting for Type 1 Diabetes Using Explainable AI

Abstract:Type 1 diabetes mellitus (T1DM) is characterized by insulin deficiency and blood sugar control issues. The state-of-the-art solution is the artificial pancreas (AP), which integrates basal insulin delivery and glucose monitoring. However, APs are unable to manage postprandial glucose response (PGR) due to limited knowledge of its determinants, requiring additional information for accurate bolus delivery, such as estimated carbohydrate intake. This study aims to quantify the influence of various meal-related factors on predicting postprandial blood glucose levels (BGLs) at different time intervals (15 min, 60 min, and 120 min) after meals by using deep neural network (DNN) models. The prediction models incorporate preprandial blood glucose values, insulin dosage, and various meal-related nutritional factors such as intake of energy, carbohydrates, proteins, lipids, fatty acids, fibers, glycemic index, and glycemic load as input variables. The impact of input features was assessed by exploiting eXplainable Artificial Intelligence (XAI) methodologies, specifically SHapley Additive exPlanations (SHAP), which provide insights into each feature's contribution to the model predictions. By leveraging XAI methodologies, this study aims to enhance the interpretability and transparency of BGL prediction models and validate clinical literature hypotheses. The findings can aid in the development of decision-support tools for individuals with T1DM, facilitating PGR management and reducing the risks of adverse events. The improved understanding of PGR determinants may lead to advancements in AP technology and improve the overall quality of life for T1DM patients.

Abstract:Academic advances of AI models in high-precision domains, like healthcare, need to be made explainable in order to enhance real-world adoption. Our past studies and ongoing interactions indicate that medical experts can use AI systems with greater trust if there are ways to connect the model inferences about patients to explanations that are tied back to the context of use. Specifically, risk prediction is a complex problem of diagnostic and interventional importance to clinicians wherein they consult different sources to make decisions. To enable the adoption of the ever improving AI risk prediction models in practice, we have begun to explore techniques to contextualize such models along three dimensions of interest: the patients' clinical state, AI predictions about their risk of complications, and algorithmic explanations supporting the predictions. We validate the importance of these dimensions by implementing a proof-of-concept (POC) in type-2 diabetes (T2DM) use case where we assess the risk of chronic kidney disease (CKD) - a common T2DM comorbidity. Within the POC, we include risk prediction models for CKD, post-hoc explainers of the predictions, and other natural-language modules which operationalize domain knowledge and CPGs to provide context. With primary care physicians (PCP) as our end-users, we present our initial results and clinician feedback in this paper. Our POC approach covers multiple knowledge sources and clinical scenarios, blends knowledge to explain data and predictions to PCPs, and received an enthusiastic response from our medical expert.

Designing User-Centric Behavioral Interventions to Prevent Dysglycemia with Novel Counterfactual Explanations

Abstract:Monitoring unexpected health events and taking actionable measures to avert them beforehand is central to maintaining health and preventing disease. Therefore, a tool capable of predicting adverse health events and offering users actionable feedback about how to make changes in their diet, exercise, and medication to prevent abnormal health events could have significant societal impacts. Counterfactual explanations can provide insights into why a model made a particular prediction by generating hypothetical instances that are similar to the original input but lead to a different prediction outcome. Therefore, counterfactuals can be viewed as a means to design AI-driven health interventions to not only predict but also prevent adverse health outcomes such as blood glucose spikes, diabetes, and heart disease. In this paper, we design \textit{\textbf{ExAct}}, a novel model-agnostic framework for generating counterfactual explanations for chronic disease prevention and management. Leveraging insights from adversarial learning, ExAct characterizes the decision boundary for high-dimensional data and performs a grid search to generate actionable interventions. ExAct is unique in integrating prior knowledge about user preferences of feasible explanations into the process of counterfactual generation. ExAct is evaluated extensively using four real-world datasets and external simulators. With $82.8\%$ average validity in the simulation-aided validation, ExAct surpasses the state-of-the-art techniques for generating counterfactual explanations by at least $10\%$. Besides, counterfactuals from ExAct exhibit at least $6.6\%$ improved proximity compared to previous research.

Hybrid Explainable Artificial Intelligence Models for Targeted Metabolomics Analysis of Diabetic Retinopathy

Abstract:Background: Diabetic retinopathy (DR) is a prevalent microvascular complication of diabetes mellitus, and early detection is crucial for effective management. Metabolomics profiling has emerged as a promising approach for identifying potential biomarkers associated with DR progression. This study aimed to develop a hybrid explainable artificial intelligence (XAI) model for targeted metabolomics analysis of patients with DR, utilizing a focused approach to identify specific metabolites exhibiting varying concentrations among individuals without DR (NDR), those with non-proliferative DR (NPDR), and individuals with proliferative DR (PDR) who have type 2 diabetes mellitus (T2DM). Methods: A total of 317 T2DM patients, including 143 NDR, 123 NPDR, and 51 PDR cases, were included in the study. Serum samples underwent targeted metabolomics analysis using liquid chromatography and mass spectrometry. Several machine learning models, including Support Vector Machines (SVC), Random Forest (RF), Decision Tree (DT), Logistic Regression (LR), and Multilayer Perceptrons (MLP), were implemented as solo models and in a two-stage ensemble hybrid approach. The models were trained and validated using 10-fold cross-validation. SHapley Additive exPlanations (SHAP) were employed to interpret the contributions of each feature to the model predictions. Statistical analyses were conducted using the Shapiro–Wilk test for normality, the Kruskal–Wallis H test for group differences, and the Mann–Whitney U test with Bonferroni correction for post-hoc comparisons. Results: The hybrid SVC + MLP model achieved the highest performance, with an accuracy of 89.58%, a precision of 87.18%, an F1-score of 88.20%, and an F-beta score of 87.55%. SHAP analysis revealed that glucose, glycine, and age were consistently important features across all DR classes, while creatinine and various phosphatidylcholines exhibited higher importance in the PDR class, suggesting their potential as biomarkers for severe DR. Conclusion: The hybrid XAI models, particularly the SVC + MLP ensemble, demonstrated superior performance in predicting DR progression compared to solo models. The application of SHAP facilitates the interpretation of feature importance, providing valuable insights into the metabolic and physiological markers associated with different stages of DR. These findings highlight the potential of hybrid XAI models combined with explainable techniques for early detection, targeted interventions, and personalized treatment strategies in DR management.

Explainable Artificial Intelligence Paves the Way in Precision Diagnostics and Biomarker Discovery for the Subclass of Diabetic Retinopathy in Type 2 Diabetics

Abstract:Diabetic retinopathy (DR), a common ocular microvascular complication of diabetes, contributes significantly to diabetes-related vision loss. This study addresses the imperative need for early diagnosis of DR and precise treatment strategies based on the explainable artificial intelligence (XAI) framework. The study integrated clinical, biochemical, and metabolomic biomarkers associated with the following classes: non-DR (NDR), non-proliferative diabetic retinopathy (NPDR), and proliferative diabetic retinopathy (PDR) in type 2 diabetes (T2D) patients. To create machine learning (ML) models, 10% of the data was divided into validation sets and 90% into discovery sets. The validation dataset was used for hyperparameter optimization and feature selection stages, while the discovery dataset was used to measure the performance of the models. A 10-fold cross-validation technique was used to evaluate the performance of ML models. Biomarker discovery was performed using minimum redundancy maximum relevance (mRMR), Boruta, and explainable boosting machine (EBM). The predictive proposed framework compares the results of eXtreme Gradient Boosting (XGBoost), natural gradient boosting for probabilistic prediction (NGBoost), and EBM models in determining the DR subclass. The hyperparameters of the models were optimized using Bayesian optimization. Combining EBM feature selection with XGBoost, the optimal model achieved (91.25 ± 1.88) % accuracy, (89.33 ± 1.80) % precision, (91.24 ± 1.67) % recall, (89.37 ± 1.52) % F1-Score, and (97.00 ± 0.25) % the area under the ROC curve (AUROC). According to the EBM explanation, the six most important biomarkers in determining the course of DR were tryptophan (Trp), phosphatidylcholine diacyl C42:2 (PC.aa.C42.2), butyrylcarnitine (C4), tyrosine (Tyr), hexadecanoyl carnitine (C16) and total dimethylarginine (DMA). The identified biomarkers may provide a better understanding of the progression of DR, paving the way for more precise and cost-effective diagnostic and treatment strategies.
Proactive Identification of Patients with Diabetes at Risk of Uncontrolled Outcomes during a Diabetes Management Program: Conceptualization and Development Study Using Machine Learning

Abstract:Background: The growth in the capabilities of telehealth have made it possible to identify individuals with a higher risk of uncontrolled diabetes and provide them with targeted support and resources to help them manage their condition. Thus, predictive modeling has emerged as a valuable tool for the advancement of diabetes management. Objective: This study aimed to conceptualize and develop a novel machine learning (ML) approach to proactively identify participants enrolled in a remote diabetes monitoring program (RDMP) who were at risk of uncontrolled diabetes at 12 months in the program. Methods: Registry data from the Livongo for Diabetes RDMP were used to design separate dynamic predictive ML models to predict participant outcomes at each monthly checkpoint of the participants' program journey (month-n models) from the first day of onboarding (month-0 model) up to the 11th month (month-11 model). A participant's program journey began upon onboarding into the RDMP and monitoring their own blood glucose (BG) levels through the RDMP-provided BG meter. Each participant passed through 12 predicative models through their first year enrolled in the RDMP. Four categories of participant attributes (ie, survey data, BG data, medication fills, and health signals) were used for feature construction. The models were trained using the light gradient boosting machine and underwent hyperparameter tuning. The performance of the models was evaluated using standard metrics, including precision, recall, specificity, the area under the curve, the F1-score, and accuracy. Results: The ML models exhibited strong performance, accurately identifying observable at-risk participants, with recall ranging from 70% to 94% and precision from 40% to 88% across the 12-month program journey. Unobservable at-risk participants also showed promising performance, with recall ranging from 61% to 82% and precision from 42% to 61%. Overall, model performance improved as participants progressed through their program journey, demonstrating the importance of engagement data in predicting long-term clinical outcomes. Conclusions: This study explored the Livongo for Diabetes RDMP participants' temporal and static attributes, identification of diabetes management patterns and characteristics, and their relationship to predict diabetes management outcomes. Proactive targeting ML models accurately identified participants at risk of uncontrolled diabetes with a high level of precision that was generalizable through future years within the RDMP. The ability to identify participants who are at risk at various time points throughout the program journey allows for personalized interventions to improve outcomes. This approach offers significant advancements in the feasibility of large-scale implementation in remote monitoring programs and can help prevent uncontrolled glycemic levels and diabetes-related complications. Future research should include the impact of significant changes that can affect a participant's diabetes management.

Abstract:The aim of a recent research project was the investigation of the mechanisms involved in the onset of type 2 diabetes in the absence of familiarity. This has led to the development of a computational model that recapitulates the aetiology of the disease and simulates the immunological and metabolic alterations linked to type-2 diabetes subjected to clinical, physiological, and behavioural features of prototypical human individuals. We analysed the time course of 46,170 virtual subjects, experiencing different lifestyle conditions. We then set up a statistical model able to recapitulate the simulated outcomes. The resulting machine learning model adequately predicts the synthetic dataset and can, therefore, be used as a computationally-cheaper version of the detailed mathematical model, ready to be implemented on mobile devices to allow self-assessment by informed and aware individuals. Abstract:Background: Predictive modeling is a key component of solutions to many healthcare problems. Among all predictive modeling approaches, machine learning methods often achieve the highest prediction accuracy, but suffer from a long-standing open problem precluding their widespread use in healthcare. Most machine learning models give no explanation for their prediction results, whereas interpretability is essential for a predictive model to be adopted in typical healthcare settings. Methods: This paper presents the first complete method for automatically explaining results for any machine learning predictive model without degrading accuracy. We did a computer coding implementation of the method. Using the electronic medical record data set from the Practice Fusion diabetes classification competition containing patient records from all 50 states in the United States, we demonstrated the method on predicting type 2 diabetes diagnosis within the next year. Results: For the champion machine learning model of the competition, our method explained prediction results for 87.4% of patients who were correctly predicted by the model to have type 2 diabetes diagnosis within the next year. Conclusions: Our demonstration showed the feasibility of automatically explaining results for any machine learning predictive model without degrading accuracy.

Abstract:Type 2 diabetes mellitus (T2DM) often results in high morbidity and mortality. In addition, T2DM presents a substantial financial burden for individuals and their families, health systems, and societies. According to studies and reports, globally, the incidence and prevalence of T2DM are increasing rapidly. Several models have been built to predict T2DM onset in the future or detect undiagnosed T2DM in patients. Additional to the performance of such models, their interpretability is crucial for health experts, especially in personalized clinical prediction models. Data collected over 42 months from health check-up examinations and prescribed drugs data repositories of four primary healthcare providers were used in this study. We propose a framework consisting of LogicRegression based feature extraction and Least Absolute Shrinkage and Selection operator based prediction modeling for undiagnosed T2DM prediction. Performance of the models was measured using Area under the ROC curve (AUC) with corresponding confidence intervals. Results show that using LogicRegression based feature extraction resulted in simpler models, which are easier for healthcare experts to interpret, especially in cases with many binary features. Models developed using the proposed framework resulted in an AUC of 0.818 (95% Confidence Interval (CI): 0.812−0.823) that was comparable to more complex models (i.e., models with a larger number of features), where all features were included in prediction model development with the AUC of 0.816 (95% CI: 0.810−0.822). However, the difference in the number of used features was significant. This study proposes a framework for building interpretable models in healthcare that can contribute to higher trust in prediction models from healthcare experts.

Abstract:Detecting Type 2 Diabetes (T2D) and Prediabetes (PD) is a real challenge for medicine due to the absence of pathogenic symptoms and the lack of known associated risk factors. Even though some proposals for machine learning models enable the identification of people at risk, the nature of the condition makes it so that a model suitable for one population may not necessarily be suitable for another. In this article, the development and assessment of predictive models to identify people at risk for T2D and PD specifically in Argentina are discussed. First, the database was thoroughly preprocessed and three specific datasets were generated considering a compromise between the number of records and the amount of available variables. After applying 5 different classification models, the results obtained show that a very good performance was observed for two datasets with some of these models. In particular, RF, DT, and ANN demonstrated great classification power, with good values for the metrics under consideration. Given the lack of this type of tool in Argentina, this work represents the first step towards the development of more sophisticated models.

Abstract:Globally, diabetes affects 537 million people, making it the deadliest and the most common non-communicable disease. Many factors can cause a person to get affected by diabetes, like excessive body weight, abnormal cholesterol level, family history, physical inactivity, bad food habit etc. Increased urination is one of the most common symptoms of this disease. People with diabetes for a long time can get several complications like heart disorder, kidney disease, nerve damage, diabetic retinopathy etc. But its risk can be reduced if it is predicted early. In this paper, an automatic diabetes prediction system has been developed using a private dataset of female patients in Bangladesh and various machine learning techniques. The authors used the Pima Indian diabetes dataset and collected additional samples from 203 individuals from a local textile factory in Bangladesh. Feature selection algorithm mutual information has been applied in this work. A semi-supervised model with extreme gradient boosting has been utilized to predict the insulin features of the private dataset. SMOTE and ADASYN approaches have been employed to manage the class imbalance problem. The authors used machine learning classification methods, that is, decision tree, SVM, Random Forest, Logistic Regression, KNN, and various ensemble techniques, to determine which algorithm produces the best prediction results. After training on and testing all the classification models, the proposed system provided the best result in the XGBoost classifier with the ADASYN approach with 81% accuracy, 0.81 F1 coefficient and AUC of 0.84. Furthermore, the domain adaptation method has been implemented to demonstrate the versatility of the proposed system. The explainable AI approach with LIME and SHAP frameworks is implemented to understand how the model predicts the final results. Finally, a website framework and an Android smartphone application have been developed to input various features and predict diabetes instantaneously. In the setting of rejection or infection, the diagnosis of serum sickness is often difficult on clinical grounds, and biopsy may be an undesirable means of obtaining documentation. A sensitive enzyme-linked immunosorbent assay (ELISA) was developed to detect antibodies against ATGAM (Upjohn) and was utilized in 35 patients treated for rejection with ATGAM following clinical unresponsiveness to bolus methylprednisolone therapy. Serum sickness was diagnosed clinically in 7 of these patients (20%) during or immediately following ATGAM therapy. Significant titers of anti-ATGAM antibody were found in these 7 cases, as well as 5 additional cases (14%). Upon retrospective review, the diagnosis of serum sickness was also made in each of these 5 additional cases based upon the immunopathologic detection of horse immunoglobulin deposits in vessels of tissue (lung or kidney) examined. In 2 other cases where serologic testing was negative, there was some clinical suggestion of serum sickness that could not be documented pathologically. ELISA for anti-ATGAM antibodies was negative in testing 46 control patients and pooled normal sera. In one case, retrospective testing of pre-ATGAM therapy serum samples showed significant antihorse antibodies. Despite a negative skin test prior to administration, this patient developed symptoms of serum sickness 15 days after the onset of ATGAM therapy. Extremely high titers of antibody were detected 5 days earlier (10 days posttherapy), and the presence of horse immunoglobulin immune complexes in the kidney was documented following graft loss that occurred within 4 weeks. These findings indicate (1) serologic testing for anti-ATGAM antibodies using a sensitive ELISA method provides a quick, inexpensive, noninvasive means for the presumptive diagnosis of serum sickness in complicated clinical situations, and (2) screening of patient sera prior to administration of ATGAM may be useful in avoiding potential serum sickness reactions.

Abstract:Diabetes is a chronic, metabolic disease characterized by high blood sugar levels. Among the main types of diabetes, type 2 is the most common. Early diagnosis and treatment can prevent or delay the onset of complications. Previous studies examined the application of machine learning techniques for prediction of the pathology, and here an artificial neural network shows very promising results as a possible valuable aid in the management and prevention of diabetes. Additionally, its superior ability for long-term predictions makes it an ideal choice for this field of study. We utilized machine learning methods to uncover previously undiscovered associations between an individual's health status and the development of type 2 diabetes, with the goal of accurately predicting its onset or determining the individual's risk level. Our study employed a binary classifier, trained on scratch, to identify potential nonlinear relationships between the onset of type 2 diabetes and a set of parameters obtained from patient measurements. Three datasets were utilized, i.e., the National Center for Health Statistics' (NHANES) biennial survey, MIMIC-III and MIMIC-IV. These datasets were then combined to create a single dataset with the same number of individuals with and without type 2 diabetes. Since the dataset was balanced, the primary evaluation metric for the model was accuracy. The outcomes of this study were encouraging, with the model achieving accuracy levels of up to 86% and a ROC AUC value of 0.934. Further investigation is needed to improve the reliability of the model by considering multiple measurements from the same patient over time.

Abstract:In the healthcare sector, the application of deep learning technologies has revolutionized data analysis and disease forecasting. This is particularly evident in the field of diabetes, where the deep analysis of Electronic Health Records (EHR) has unlocked new opportunities for early detection and effective intervention strategies. Our research presents an innovative model that synergizes the capabilities of Bidirectional Long Short-Term Memory Networks-Conditional Random Field (BiLSTM-CRF) with a fusion of XGBoost and Logistic Regression. This model is designed to enhance the accuracy of diabetes risk prediction by conducting an in-depth analysis of electronic medical records data. The first phase of our approach involves employing BiLSTM-CRF to delve into the temporal characteristics and latent patterns present in EHR data. This method effectively uncovers the progression trends of diabetes, which are often hidden in the complex data structures of medical records. The second phase leverages the combined strength of XGBoost and Logistic Regression to classify these extracted features and evaluate associated risks. This dual approach facilitates a more nuanced and precise prediction of diabetes, outperforming traditional models, particularly in handling multifaceted and nonlinear medical datasets. Our research demonstrates a notable advancement in diabetes prediction over traditional methods, showcasing the effectiveness of our combined BiLSTM-CRF, XGBoost, and Logistic Regression model. This study highlights the value of data-driven strategies in clinical decision-making, equipping healthcare professionals with precise tools for early detection and intervention. By enabling personalized treatment and timely care, our approach signifies progress in incorporating advanced analytics in healthcare, potentially improving outcomes for diabetes and other chronic conditions.

Directive Explanations for Monitoring the Risk of Diabetes Onset: Introducing Directive Data-Centric Explanations and Combinations to Support What-If Explorations

Abstract:Explainable artificial intelligence is increasingly used in machine learning (ML) based decision-making systems in healthcare. However, little research has compared the utility of different explanation methods in guiding healthcare experts for patient care. Moreover, it is unclear how useful, understandable, actionable and trustworthy these methods are for healthcare experts, as they often require technical ML knowledge. This paper presents an explanation dashboard that predicts the risk of diabetes onset and explains those predictions with data-centric, feature-importance, and example-based explanations. We designed an interactive dashboard to assist healthcare experts, such as nurses and physicians, in monitoring the risk of diabetes onset and recommending measures to minimize risk. We conducted a qualitative study with 11 healthcare experts and a mixed-methods study with 45 healthcare experts and 51 diabetic patients to compare the different explanation methods in our dashboard in terms of understandability, usefulness, actionability, and trust. Results indicate that our participants preferred our representation of data-centric explanations that provide local explanations with a global overview over other methods. Therefore, this paper highlights the importance of visually directive data-centric explanation method for assisting healthcare experts to gain actionable insights from patient health records. Furthermore, we share our design implications for tailoring the visual representation of different explanation methods for healthcare experts.

Abstract:The progression of prediabetes towards diabetes was heterogeneous, suggesting a demand for individualized management. We aim to investigate whether stratifying participants with prediabetes according to their progression risk could affect the response to interventions. We developed a 1-year diabetes progression risk evaluating model with a panel of diabetes risk factors using a machine learning method (i.e., Bagging algorithm with 300 decision trees) in prediabetes participants in Pinggu Study (a prospective population-based survey in suburban Beijing, n=622). The model training phase was conducted on 2/3 randomly selected participants and the validation phase was conducted on other 1/3. This model successfully predicted the 1-year progression of prediabetes participants in Pinggu study (internal ROC AUC 1.000, external ROC AUC 0.883). In Beijing Prediabetes Reversion Program (BPRP, a multi-central RCT to evaluate the efficacy of lifestyle and/or pioglitazone on prediabetes reversion, n=1897), subjects were divided into tertiles as high, medium and low risk groups according to their progression probability predicted by the model. Pioglitazone plus intensive lifestyle significantly increased reversion and reduced diabetes progression only in high risk group.
