Abstract:The aim of a recent research project was the investigation of the mechanisms involved in the onset of type 2 diabetes in the absence of familiarity. This has led to the development of a computational model that recapitulates the aetiology of the disease and simulates the immunological and metabolic alterations linked to type-2 diabetes subjected to clinical, physiological, and behavioural features of prototypical human individuals. We analysed the time course of 46,170 virtual subjects, experiencing different lifestyle conditions. We then set up a statistical model able to recapitulate the simulated outcomes. The resulting machine learning model adequately predicts the synthetic dataset and can, therefore, be used as a computationally-cheaper version of the detailed mathematical model, ready to be implemented on mobile devices to allow self-assessment by informed and aware individuals. The computational model used to generate the dataset of this work is available as a web-service at the following address: http://kraken.iac.rm.cnr.it/T2DM.

Abstract:Diabetes is a chronic, metabolic disease characterized by high blood sugar levels. Among the main types of diabetes, type 2 is the most common. Early diagnosis and treatment can prevent or delay the onset of complications. Previous studies examined the application of machine learning techniques for prediction of the pathology, and here an artificial neural network shows very promising results as a possible valuable aid in the management and prevention of diabetes. Additionally, its superior ability for long-term predictions makes it an ideal choice for this field of study. We utilized machine learning methods to uncover previously undiscovered associations between an individual's health status and the development of type 2 diabetes, with the goal of accurately predicting its onset or determining the individual's risk level. Our study employed a binary classifier, trained on scratch, to identify potential nonlinear relationships between the onset of type 2 diabetes and a set of parameters obtained from patient measurements. Three datasets were utilized, i.e., the National Center for Health Statistics' (NHANES) biennial survey, MIMIC-III and MIMIC-IV. These datasets were then combined to create a single dataset with the same number of individuals with and without type 2 diabetes. Since the dataset was balanced, the primary evaluation metric for the model was accuracy. The outcomes of this study were encouraging, with the model achieving accuracy levels of up to 86% and a ROC AUC value of 0.934. Further investigation is needed to improve the reliability of the model by considering multiple measurements from the same patient over time.

Predicting the Development of Type 2 Diabetes in a Large Australian Cohort Using Machine-Learning Techniques: Longitudinal Survey Study

Abstract:Background Previous conventional models for the prediction of diabetes could be updated by incorporating the increasing amount of health data available and new risk prediction methodology. Objective We aimed to develop a substantially improved diabetes risk prediction model using sophisticated machine-learning algorithms based on a large retrospective population cohort of over 230,000 people who were enrolled in the study during 2006-2017. Methods We collected demographic, medical, behavioral, and incidence data for type 2 diabetes mellitus (T2DM) in over 236,684 diabetes-free participants recruited from the 45 and Up Study. We predicted and compared the risk of diabetes onset in these participants at 3, 5, 7, and 10 years based on three machine-learning approaches and the conventional regression model. Results Overall, 6.05% (14,313/236,684) of the participants developed T2DM during an average 8.8-year follow-up period. The 10-year diabetes incidence in men was 8.30% (8.08%-8.49%), which was significantly higher (odds ratio 1.37, 95% CI 1.32-1.41) than that in women at 6.20% (6.00%-6.40%). The incidence of T2DM was doubled in individuals with obesity (men: 17.78% [17.05%-18.43%]; women: 14.59% [13.99%-15.17%]) compared with that of nonobese individuals. The gradient boosting machine model showed the best performance among the four models (area under the curve of 79% in 3-year prediction and 75% in 10-year prediction). All machine-learning models predicted BMI as the most significant factor contributing to diabetes onset, which explained 12%-50% of the variance in the prediction of diabetes. The model predicted that if BMI in obese and overweight participants could be hypothetically reduced to a healthy range, the 10-year probability of diabetes onset would be significantly reduced from 8.3% to 2.8% (P<.001). Conclusions A one-time self-reported survey can accurately predict the risk of diabetes using a machine-learning approach. Achieving a healthy BMI can significantly reduce the risk of developing T2DM.

Abstract:Abstract Diabetes, one of the top 10 causes of death worldwide, is associated with the interaction between lifestyle, psychosocial, medical conditions, demographic, and genetic risk factors. Predicting type 2 diabetes is important for providing prognosis or diagnosis support to allied health professionals, and aiding in the development of an efficient and effective prevention plan. Several works proposed machine-learning algorithms to predict type 2 diabetes. However, each work uses different datasets and evaluation metrics for algorithms’ evaluation, making it difficult to compare among them. In this paper, we provide a taxonomy of diabetes risk factors and evaluate 35 different machine learning algorithms (with and without features selection) for diabetes type 2 prediction using a unified setup, to achieve an objective comparison. We use 3 real-life diabetes datasets and 9 feature selection algorithms for the evaluation. We compare the accuracy, F-measure, and execution time for model building and validation of the algorithms under study on diabetic and non-diabetic individuals. The performance analysis of the models is elaborated in the article.

Abstract:Prediction of type 2 diabetes (T2D) occurrence allows a person at risk to take actions that can prevent onset or delay the progression of the disease. In this study, we developed a machine learning (ML) model to predict T2D occurrence in the following year (Y + 1) using variables in the current year (Y). The dataset for this study was collected at a private medical institute as electronic health records from 2013 to 2018. To construct the prediction model, key features were first selected using ANOVA tests, chi-squared tests, and recursive feature elimination methods. The resultant features were fasting plasma glucose (FPG), HbA1c, triglycerides, BMI, gamma-GTP, age, uric acid, sex, smoking, drinking, physical activity, and family history. We then employed logistic regression, random forest, support vector machine, XGBoost, and ensemble machine learning algorithms based on these variables to predict the outcome as normal (non-diabetic), prediabetes, or diabetes. Based on the experimental results, the performance of the prediction model proved to be reasonably good at forecasting the occurrence of T2D in the Korean population. The model can provide clinicians and patients with valuable predictive information on the likelihood of developing T2D. The cross-validation (CV) results showed that the ensemble models had a superior performance to that of the single models. The CV performance of the prediction models was improved by incorporating more medical history from the dataset.

Abstract:Background Between 2013 and 2015, the UK Biobank collected accelerometer traces from 103,712 volunteers aged between 40 and 69 years using wrist-worn triaxial accelerometers for 1 week. This data set has been used in the past to verify that individuals with chronic diseases exhibit reduced activity levels compared with healthy populations. However, the data set is likely to be noisy, as the devices were allocated to participants without a set of inclusion criteria, and the traces reflect free-living conditions. Objective This study aims to determine the extent to which accelerometer traces can be used to distinguish individuals with type 2 diabetes (T2D) from normoglycemic controls and to quantify their limitations. Methods Machine learning classifiers were trained using different feature sets to segregate individuals with T2D from normoglycemic individuals. Multiple criteria, based on a combination of self-assessment UK Biobank variables and primary care health records linked to UK Biobank participants, were used to identify 3103 individuals with T2D in this population. The remaining nondiabetic 19,852 participants were further scored on their physical activity impairment severity based on other conditions found in their primary care data, and those deemed likely physically impaired at the time were excluded. Physical activity features were first extracted from the raw accelerometer traces data set for each participant using an algorithm that extends the previously developed Biobank Accelerometry Analysis toolkit from Oxford University. These features were complemented by a selected collection of sociodemographic and lifestyle features available from UK Biobank. Results We tested 3 types of classifiers, with an area under the receiver operating characteristic curve (AUC) close to 0.86 (95% CI 0.85-0.87) for all 3 classifiers and F1 scores in the range of 0.80-0.82 for T2D-positive individuals and 0.73-0.74 for T2D-negative controls. Results obtained using nonphysically impaired controls were compared with highly physically impaired controls to test the hypothesis that nondiabetic conditions reduce classifier performance. Models built using a training set that included highly impaired controls with other conditions had worse performance (AUC 0.75-0.77; 95% CI 0.74-0.78; F1 scores in the range of 0.76-0.77 for T2D positives and 0.63-0.65 for controls). Conclusions Granular measures of free-living physical activity can be used to successfully train machine learning models that are able to discriminate between individuals with T2D and normoglycemic controls, although with limitations because of the intrinsic noise in the data sets. From a broader clinical perspective, these findings motivate further research into the use of physical activity traces as a means of screening individuals at risk of diabetes and for early detection, in conjunction with routinely used risk scores, provided that appropriate quality control is enforced on the data collection protocol.

Abstract:Prediction of type 2 diabetes (T2D) occurrence allows a person at risk to take preventive actions that can prevent or delay the progression of the disease. In this study, we aim to develop a machine learning (ML) model to predict T2D occurrence in the year Y+1 using the variables in the year Y. The dataset was collected from electronic health records between 2013 and 2018 at a medical agency. We utilized 169,024 instances from 80,692 patients with longitudinal data to build the ML model. Each instance has 1,444 variables. To construct a prediction model, key variables or features were first selected using ANOVA and chi-square tests, and the recursive feature elimination methods. Then we employed random forest (RF) and XGBoost algorithms based on these variables to predict the outcome as normal, prediabetes or diabetes. Selected variables were fasting plasma glucose (FPG), HbA1c, triglycerides, body mass index (BMI), r-GTP, gender, age, uric acid, smoking, drinking, physical activity and family history. The accuracy of the RF classifier in predicting the occurrence in next year was 73.3% while that of the XGBoost classifier was 73.8%. The proposed prediction model using ML can provide both clinicians and patients with valuable information on the immediate incident of T2D. In addition to traditional predictors of T2D (FPG, HbA1c, BMI, family history), it should also be noted that variables such as r-GTP, uric acid, triglycerides, and lifestyle factors in the year Y increased the accuracy up to 3% in predicting T2D occurrence in the year Y+1. Abstract:Purpose Though, type 2 diabetes (T2DM) is largely genetically heritable, several environmental factors could contribute to the occurrence of T2DM. The current employed machine learning (ML) algorithms to predict the risk of T2DM using the social determinants of health reported in the All of Us Research Program Data. Methods Data were sourced from the All of Us Research Program. All patients with a complete record of the social determinants of health survey were included in the analysis. The participants were categorized based on history of diabetes (case=1) and without history of diabetes (case=0). The major ML models tested were gradient boost model, RandomForest model, and support vector machines. The model performance measures include accuracy, precision, and recall. Feature importance was evaluated based on the mean decrease in accuracy score, an output from the best model. RESULTS Overall, the social determinants of health were able to improve the performance of the ML models to predict the risk of T2DM. The accuracy of the ML models was in the range of 88% to 92%. The sensitivity of all the models were more-than 90%. Also, important features out of the social determinants, were reported as predictors of T2DM. CONCLUSION The social determinants of health reported in the All of Us dataset were able to predict the risk of diabetes using machine learning algorithms. These factors could be used to screen patients with a risk of T2DM.

A novel mathematical model for predicting the benefits of physical activity on type 2 diabetes progression

Abstract:Despite the well-acknowledged benefits of physical activity for type 2 diabetes (T2D) prevention, the literature surprisingly lacks validated models able to predict the long-term benefits of exercise on T2D progression and support personalized risk prediction and prevention. To bridge this gap, we developed a novel mathematical model that formalizes the link between exercise and short- and long-term glucose-insulin dynamics to predict the benefits of regular exercise on T2D progression. The model quantitatively captured the dose-response relationship (larger benefits with increasing intensity and/or duration of exercise), it consistently reproduced the benefits of clinical guidelines for diabetes prevention, and it accurately predicted persistent benefits following interruption of physical activity, in line with real-world evidence from the literature. These results are encouraging and can be the basis for future development of decision support tools able to assist patients and clinicians in tailoring preventive lifestyle interventions.

Abstract:We present a new approach to population health, in which data-driven predictive models are learned for outcomes such as type 2 diabetes. Our approach enables risk assessment from readily available electronic claims data on large populations, without additional screening cost. Proposed model uncovers early and late-stage risk factors. Using administrative claims, pharmacy records, healthcare utilization, and laboratory results of 4.1 million individuals between 2005 and 2009, an initial set of 42,000 variables were derived that together describe the full health status and history of every individual. Machine learning was then used to methodically enhance predictive variable set and fit models predicting onset of type 2 diabetes in 2009-2011, 2010-2012, and 2011-2013. We compared the enhanced model with a parsimonious model consisting of known diabetes risk factors in a real-world environment, where missing values are common and prevalent. Furthermore, we analyzed novel and known risk factors emerging from the model at different age groups at different stages before the onset. Parsimonious model using 21 classic diabetes risk factors resulted in area under ROC curve (AUC) of 0.75 for diabetes prediction within a 2-year window following the baseline. The enhanced model increased the AUC to 0.80, with about 900 variables selected as predictive (p < 0.0001 for differences between AUCs). Similar improvements were observed for models predicting diabetes onset 1-3 years and 2-4 years after baseline. The enhanced model improved positive predictive value by at least 50% and identified novel surrogate risk factors for type 2 diabetes, such as chronic liver disease (odds ratio [OR] 3.71), high alanine aminotransferase (OR 2.26), esophageal reflux (OR 1.85), and history of acute bronchitis (OR 1.45). Liver risk factors emerge later in the process of diabetes development compared with obesity-related factors such as hypertension and high hemoglobin A1c. In conclusion, population-level risk prediction for type 2 diabetes using readily available administrative data is feasible and has better prediction performance than classical diabetes risk prediction algorithms on very large populations with missing data. The new model enables intervention allocation at national scale quickly and accurately and recovers potentially novel risk factors at different stages before the disease onset.

Abstract:ObjectiveDiabetes is a chronic fatal disease that has affected millions of people all over the globe. Type 2 Diabetes Mellitus (T2DM) accounts for 90% of the affected population among all types of diabetes. Millions of T2DM patients remain undiagnosed due to lack of awareness and under resourced healthcare system. So, there is a dire need for a diagnostic and prognostic tool that shall help the healthcare providers, clinicians and practitioners with early prediction and hence can recommend the lifestyle changes required to stop the progression of diabetes. The main objective of this research is to develop a framework based on machine learning techniques using only lifestyle indicators for prediction of T2DM disease. Moreover, prediction model can be used without visiting clinical labs and hospital readmissions.MethodA proposed framework is presented and implemented based on machine learning paradigms using lifestyle indicators for better prediction of T2DM disease. The current research has involved different experts like Diabetologists, Endocrinologists, Dieticians, Nutritionists, etc. for selecting the contributing 1552 instances and 11 attributes lifestyle biological features to promote health and manage complications towards T2DM disease. The dataset has been collected through survey and google forms from different geographical regions.ResultsSeven machine learning classifiers were employed namely K-Nearest Neighbour (KNN), Linear Regression (LR), Support Vector Machine (SVM), Naive Bayes (NB), Decision Tree (DT), Random Forest (RF) and Gradient Boosting (GB). Gradient Boosting classifier outperformed best with an accuracy rate of 97.24% for training and 96.90% for testing separately followed by RF, DT, NB, SVM, LR, and KNN as 95.36%, 92.52%, 90.72%, 90.20%, 90.20% and 77.06% respectively. However, in terms of precision, RF achieved high performance (0.980%) and KNN performed the lowest (0.793%). As far as recall is being concerned, GB achieved the highest rate of 0.975% and KNN showed the worst rate of 0.774%. Also, GB is top performed in terms of f1-score. According to the ROCs, GB and NB had a better area under the curve compared to the others.ConclusionThe research developed a realistic health management system for T2DM disease based on machine learning techniques using only lifestyle data for prediction of T2DM. To extend the current study, these models shall be used for different, large and real-time datasets which share the commonality of data with T2DM disease to establish the efficacy of the proposed system.
Predicting Type 2 Diabetes Metabolic Phenotypes Using Continuous Glucose Monitoring and a Machine Learning Framework

Abstract:Type 2 diabetes (T2D) and prediabetes are classically defined by the level of fasting glucose or surrogates such as hemoglobin A1c. This classification does not take into account the heterogeneity in the pathophysiology of glucose dysregulation, the identification of which could inform targeted approaches to diabetes treatment and prevention and/or predict clinical outcomes. We performed gold-standard metabolic tests in a cohort of individuals with early glucose dysregulation and quantified four distinct metabolic subphenotypes known to contribute to glucose dysregulation and T2D: muscle insulin resistance, beta-cell dysfunction, impaired incretin action, and hepatic insulin resistance. We revealed substantial inter-individual heterogeneity, with 44% of individuals exhibiting dominance in muscle or liver IR, and 16%, 13%, and 9% exhibiting dominance in beta-cell, incretin, or both, respectively. Further, with a frequently-sampled oral glucose tolerance test (OGTT), we developed a novel machine learning framework to predict metabolic subphenotypes using features from the dynamic patterns of the glucose time-series ("shape of the glucose curve"). The glucose time-series features identified insulin resistance, beta-cell deficiency, and incretin defect with auROCs of 95%, 89%, and 88%, respectively. These figures are superior to currently-used estimates. The prediction of muscle insulin resistance and beta-cell deficiency were validated using an independent cohort. We then tested the ability of glucose curves generated by a continuous glucose monitor (CGM) worn during at-home OGTTs to predict insulin resistance and beta-cell deficiency, yielding auROC of 88% and 84%, respectively. We thus demonstrate that the prediabetic state is characterized by metabolic heterogeneity, which can be defined by the shape of the glucose curve during standardized OGTT, performed in a clinical research unit or at-home setting using CGM. The use of at-home CGM to identify muscle insulin resistance and beta-cell deficiency constitutes a practical and scalable method by which to risk stratify individuals with early glucose dysregulation and inform targeted treatment to prevent T2D.

Machine Learning Reveals Time-Varying Microbial Predictors with Complex Effects on Glucose Regulation

Abstract:The incidence of type 2 diabetes (T2D) has been increasing globally, and a growing body of evidence links type 2 diabetes with altered microbiota composition. Type 2 diabetes is preceded by a long prediabetic state characterized by changes in various metabolic parameters. We tested whether the gut microbiome could have predictive potential for T2D development during the healthy and prediabetic disease stages. We used prospective data of 608 well-phenotyped Finnish men collected from the population-based Metabolic Syndrome in Men (METSIM) study to build machine learning models for predicting continuous glucose and insulin measures in a shorter (1.5 year) and longer (4 year) period. Our results show that the inclusion of the gut microbiome improves prediction accuracy for modeling T2D-associated parameters such as glycosylated hemoglobin and insulin measures. We identified novel microbial biomarkers and described their effects on the predictions using interpretable machine learning techniques, which revealed complex linear and nonlinear associations. Additionally, the modeling strategy carried out allowed us to compare the stability of model performance and biomarker selection, also revealing differences in short-term and long-term predictions. The identified microbiome biomarkers provide a predictive measure for various metabolic traits related to T2D, thus providing an additional parameter for personal risk assessment. Our work also highlights the need for robust modeling strategies and the value of interpretable machine learning. IMPORTANCE Recent studies have shown a clear link between gut microbiota and type 2 diabetes. However, current results are based on cross-sectional studies that aim to determine the microbial dysbiosis when the disease is already prevalent. In order to consider the microbiome as a factor in disease risk assessment, prospective studies are needed. Our study is the first study that assesses the gut microbiome as a predictive measure for several type 2 diabetes-associated parameters in a longitudinal study setting. Our results revealed a number of novel microbial biomarkers that can improve the prediction accuracy for continuous insulin measures and glycosylated hemoglobin levels. These results make the prospect of using the microbiome in personalized medicine promising.

Abstract:The study aimed to identify the most predictive factors for the development of type 2 diabetes. Using an XGboost classification model, we projected type 2 diabetes incidence over a 10-year horizon. We deliberately minimized the selection of baseline factors to fully exploit the rich dataset from the UK Biobank. The predictive value of features was assessed using shap values, with model performance evaluated via Receiver Operating Characteristic Area Under the Curve, sensitivity, and specificity. Data from the UK Biobank, encompassing a vast population with comprehensive demographic and health data, was employed. The study enrolled 450,000 participants aged 40–69, excluding those with pre-existing diabetes. Among 448,277 participants, 12,148 developed type 2 diabetes within a decade. HbA1c emerged as the foremost predictor, followed by BMI, waist circumference, blood glucose, family history of diabetes, gamma-glutamyl transferase, waist-hip ratio, HDL cholesterol, age, and urate. Our XGboost model achieved a Receiver Operating Characteristic Area Under the Curve of 0.9 for 10-year type 2 diabetes prediction, with a reduced 10-feature model achieving 0.88. Easily measurable biological factors surpassed traditional risk factors like diet, physical activity, and socioeconomic status in predicting type 2 diabetes. Furthermore, high prediction accuracy could be maintained using just the top 10 biological factors, with additional ones offering marginal improvements. These findings underscore the significance of biological markers in type 2 diabetes prediction.

Abstract:Abstract Type 2 Diabetes Mellitus (T2DM) is a significant public health problem globally. The diagnosis and management of diabetes are critical to reduce the diabetes complications including cardiovascular disease and cancer. This study was designed to assess the potential association between T2DM and routinely measured hematological parameters. This study was a subsample of 9000 adults aged 35–65 years recruited as part of Mashhad stroke and heart atherosclerotic disorder (MASHAD) cohort study. Machine learning techniques including logistic regression (LR), decision tree (DT) and bootstrap forest (BF) algorithms were applied to analyze data. All data analyses were performed using SPSS version 22 and SAS JMP Pro version 13 at a significant level of 0.05. Based on the performance indices, the BF model gave high accuracy, precision, specificity, and AUC. Previous studies suggested the positive relationship of triglyceride-glucose (TyG) index with T2DM, so we considered the association of TyG index with hematological factors. We found this association was aligned with their results regarding T2DM, except MCHC. The most effective factors in the BF model were age and WBC (white blood cell). The BF model represented a better performance to predict T2DM. Our model provides valuable information to predict T2DM like age and WBC.

Abstract:Diabetes is a large healthcare burden worldwide. There is substantial evidence that lifestyle modifications and drug intervention can prevent diabetes, therefore, an early identification of high risk individuals is important to design targeted prevention strategies. In this paper, we present an automatic tool that uses machine learning techniques to predict the development of type 2 diabetes mellitus (T2DM). Data generated from an oral glucose tolerance test (OGTT) was used to develop a predictive model based on the support vector machine (SVM). We trained and validated the models using the OGTT and demographic data of 1,492 healthy individuals collected during the San Antonio Heart Study. This study collected plasma glucose and insulin concentrations before glucose intake and at three time-points thereafter (30, 60 and 120 min). Furthermore, personal information such as age, ethnicity and body-mass index was also a part of the data-set. Using 11 OGTT measurements, we have deduced 61 features, which are then assigned a rank and the top ten features are shortlisted using minimum redundancy maximum relevance feature selection algorithm. All possible combinations of the 10 best ranked features were used to generate SVM based prediction models. This research shows that an individual's plasma glucose levels, and the information derived therefrom have the strongest predictive performance for the future development of T2DM. Significantly, insulin and demographic features do not provide additional performance improvement for diabetes prediction. The results of this work identify the parsimonious clinical data needed to be collected for an efficient prediction of T2DM. Our approach shows an average accuracy of 96.80% and a sensitivity of 80.09% obtained on a holdout set.

Abstract:Abstract: Diabetes is a chronic condition that could lead to a catastrophe in the world's healthcare system. 537 million people worldwide have diabetes, according to the International Diabetes Federation. By 2045, this number is anticipated to reach 783 million. A rise in blood glucose levels can result in diabetes. Numerous symptoms, which includes frequent urination, increased thirst, and increased appetite, are brought on by this raised blood glucose level. It is a significant contributor to heart failure, stroke, kidney failure, blindness, and amputations. The objective of the given study mainly is to develop the single collective system that combines result of many different types machine learning techniques, including Logistic Regression, Linear Regression, Support Vector Machine, and Random Forest, to more accurately predict diabetes in a patient. It collects the patient's records based on their pregnancy, blood sugar levels, blood pressure, insulin levels, body mass index, and many other factors. Each of the strategies will be used to determine the model's correctness, however, we found that the Support vector machine method had the highest accuracy (77%). The diabetes forecast is then made using the model with the highest accuracy.

Abstract:Detecting Type 2 Diabetes (T2D) and Prediabetes (PD) is a real challenge for medicine due to the absence of pathogenic symptoms and the lack of known associated risk factors. Even though some proposals for machine learning models enable the identification of people at risk, the nature of the condition makes it so that a model suitable for one population may not necessarily be suitable for another. In this article, the development and assessment of predictive models to identify people at risk for T2D and PD specifically in Argentina are discussed. First, the database was thoroughly preprocessed and three specific datasets were generated considering a compromise between the number of records and the amount of available variables. After applying 5 different classification models, the results obtained show that a very good performance was observed for two datasets with some of these models. In particular, RF, DT, and ANN demonstrated great classification power, with good values for the metrics under consideration. Given the lack of this type of tool in Argentina, this work represents the first step towards the development of more sophisticated models.

Abstract:Background: Type 2 diabetes disproportionately affects individuals of non-White ethnicity through a complex interaction of multiple factors. Therefore, early disease detection and prediction are essential and require tools that can be deployed on a large scale. We aimed to tackle this problem by developing questionnaire-based prediction models for type 2 diabetes prevalence and incidence for multiple ethnicities. Methods: In this proof of principle analysis, logistic regression models to predict type 2 diabetes prevalence and incidence, using questionnaire-only variables reflecting health state and lifestyle, were trained on the White population of the UK Biobank (n = 472,696 total, aged 37-73 years, data collected 2006-2010) and validated in five other ethnicities (n = 29,811 total) and externally in Lifelines (n = 168,205 total, aged 0-93 years, collected between 2006 and 2013). In total, 631,748 individuals were included for prevalence prediction and 67,083 individuals for the eight-year incidence prediction. Type 2 diabetes prevalence in the UK Biobank ranged between 6% in the White population to 23.3% in the South Asian population, while in Lifelines, the prevalence was 1.9%. Predictive accuracy was evaluated using the area under the receiver operating characteristic curve (AUC), and a detailed sensitivity analysis was conducted to assess potential clinical utility. We compared the questionnaire-only models to models containing physical measurements and biomarkers as well as to clinical non-laboratory type 2 diabetes risk tools and conducted a reclassification analysis. Findings: Our algorithms accurately predicted type 2 diabetes prevalence (AUC = 0.901) and eight-year incidence (AUC = 0.873) in the White UK Biobank population. Both models replicated well in the Lifelines external validation, with AUCs of 0.917 and 0.817 for prevalence and incidence, respectively. Both models performed consistently well across different ethnicities, with AUCs of 0.855-0.894 for prevalence and 0.819-0.883 for incidence. These models generally outperformed two clinically validated non-laboratory tools and correctly reclassified >3,000 additional cases. Model performance improved with the addition of blood biomarkers but not with the addition of physical measurements. Interpretation: Our findings suggest that easy-to-implement, questionnaire-based models could be used to predict prevalent and incident type 2 diabetes with high accuracy across several ethnicities, providing a highly scalable solution for population-wide risk stratification. Future work should determine the effectiveness of these models in identifying undiagnosed type 2 diabetes, validated in cohorts of different populations and ethnic representation.
