Abstract:Heavy-Ion Cancer Therapy Device (HICTD) developed by the Institute of Modern Physics of the Chinese Academy of Sciences is an advanced tumor radiotherapy device for nuclear medicine. The time of flight (TOF) positron emission tomography (PET) located on the beam line, called In-Beam TOF-PET, is a key detector in HICTD, which is employed for obtaining time, energy and position information of gamma ray to implement real-time image reconstruction during the treatment. A high performance readout electronics system has been designed for In-Beam TOF-PET. The Data Acquisition Unit (DAQU), a key device of it has been implemented. The signals from the In-Beam TOF-PET are acquired and processed by the DAQU, including 8 energy chains and 2 time chains. A field programmable gate array (FPGA) is adopted as a processor on board for implementing pulse area information extracting algorithm, FPGA-based TDC, and etc. The precision pulsar test indicates that the DAQU system has intrinsic influences on the energy resolution better than 5.5 parts per thousand (full width at the half-maximum, FWHM) and time resolution of 255 ps (FWHM). The Na-22 source test indicates that the overall system of the in-beam TOF-PET has an energy resolution of 14% FWHM at 511 keV and coincidence timing resolution better than 1.12 ns. The good position identification ability is indicated by the flood map, all the 484 crystals in one of LYSO crystal block detectors can be clearly distinguished.
Simulation study of a brain PET scanner using TOF-DOI detectors equipped with first interaction position detection.

Abstract:. The aim of this study is to evaluate the performance characteristics of a brain positron emission tomography (PET) scanner composed of four-layer independent read-out time-of-flight depth-of-interaction (TOF-DOI) detectors capable of first interaction position (FIP) detection, using Geant4 application for tomographic emission(GATE). This includes the spatial resolution, sensitivity, count rate capability, and reconstructed image quality.. The proposed TOF-DOI PET detector comprises four layers of a 50 × 50 cerium-doped lutetium-yttrium oxyorthosilicate (LYSO:Ce) scintillator array with 1 mm pitch size, coupled to a 16 × 16 multi-pixel photon counter array with 3.0 mm × 3.0 mm photosensitive segments. Along the direction distant from the center field-of-view (FOV), the scintillator thickness of the four layers is 2.5, 3, 4, and 6 mm. The four layers were simulated with a 150 ps coincidence time resolution and the independent readout make the FIP detection capable. The spatial resolution and imaging performance were compared among the true-FIP, winner-takes-all (WTA) and front-layer FIP (FL-FIP) methods (FL-FIP selects the interaction position located on the front-most interaction layer in all the interaction layers). The National Electrical Manufacturers Association NU 2-2018 procedure was referred and modified to evaluate the performance of proposed scanner.. In detector evaluation, the intrinsic spatial resolutions were 0.52 and 0.76 mm full width at half-maximum (FWHM) at 0° and 30° incident-rays in the first layer pair, respectively. The reconstructed spatial resolution by the filter backprojection (FBP) achieved sub-millimeter FWHM on average over the whole FOV. The maximum true count rate was 207.6 kcps at 15 kBq mland the noise equivalent count rate (NECR_2R) was 54.7 kcps at 6.0 kBq ml. Total sensitivity was 45.2 cps kBqand 48.4 cps kBqat the center and 10 cm off-center FOV, respectively. The TOF and DOI reconstructions significantly improved the image quality in the phantom studies. Moreover, the FL-FIP outperformed the conventional WTA method in terms of the spatial resolution and image quality.. The proposed brain PET scanner could achieve sub-millimeter spatial resolution and high image quality with TOF and DOI reconstruction, which is meaningful to the clinical oncology research. Meanwhile, the comparison among the three positioning methods indicated that the FL-FIP decreased the image degradation caused by Compton scatter more than WTA.

Abstract:In order to elucidate an involvement of protein phosphatase type 1 (PP1) in the malignant transformation, we have isolated rat cDNA for NIPP-1, a nuclear inhibitor for PP1. Rat NIPP-1 protein conserved the PP1 binding domain and the nuclear targeting sequence. The mRNA was ubiquitously expressed with the highest expression in bone marrow and thymus. The rat NIPP1 mRNA levels were slightly, if at all, increased in some primary hepatomas, and significantly increased in rat ascites hepatomas. The levels of NIPP-1 mRNA were closely correlated with malignant phenotype of rat ascites hepatomas, suggesting that NIPP-1 is tightly linked to malignant phenotype in hepatomas.
Limited-angle TOF-PET for intraoperative surgical applications: proof of concept and first experimental data

Abstract:The intraoperative gamma probe (IPG) based on single gamma-ray detection remains the current gold standard modality for sentinel lymph node identification and tumor removal in cancer patients. However, IPGs do not meet the <5% false negative rate (FNR) requirement, a key metric suggested by the American Society of Clinical Oncology (ASCO). We aim to reduce FNR by using time of flight (TOF) PET detector technology in limited angle geometry system by using only two detector panels in coincidence. For proof of concept, we used two Hamamatsu TOF PET detector modules (C13500-4075YC-12) featuring 12× 12 arrays of 4.14× 4.14× 20 mm 3 LFS crystal pixels with 4.2 mm pitch and coupled one-one to silicon photomultiplier (SiPM) pixels. The measured detector coincidence timing resolution (CTR) was 271 ps FWHM for the whole detector. Simulation of a Compton-based detector for low-dose high-resolution time-of-flight positron emission tomography

Abstract:Two major challenges in time-of-flight positron emission tomography (TOF-PET) are low spatial resolution and high radioactive dose to the patient, both of which result from limitations in detection technology rather than fundamental physics. A new type of TOF-PET detector employing low-atomic number (low-Z) scintillation media and large-area, high-resolution photodetectors to record Compton scattering locations in the detector has been proposed as a promising alternative, but the minimum technical requirements for such a system have not yet been established. Here we present a simulation study evaluating the potential of a proposed low-Z detection medium, linear alkylbenzene (LAB) doped with a switchable molecular recorder, for next-generation TOF-PET detection. We developed a custom Monte Carlo simulation of full-body TOF-PET using the TOPAS Geant4 software package. By quantifying contributions and tradeoffs for energy, spatial, and timing resolution of the detector, we show that at reasonable combination of specifications, our likelihood-based identification of pairs of first interaction locations in the simulated detector identifies 87.1% of pairs with zero or negligible error, and correctly rejects 90% of all in-patient scatters. The same specifications give TOF-PET sensitivity of ~66.7% and PSF width 4.6 mm with clear contrast. A detector with these specifications provides a clear image of a brain phantom simulated at less than 1% of a standard radiotracer dose.

Characterization of TOF-PET Detectors Based on Monolithic Blocks and ASIC-Readout

Abstract:The aim of this work is to show the potential capabilities of monolithic crystals coupled to large SiPM arrays, to be considered as detector blocks for PET scanners enabling Time Of Flight (TOF) capabilities. Monolithic blocks allow one to decode the 3D photon impact position. This approach, along with TOF information, can be of high interest in clinical Positron emission tomography (PET) applications where a typical ring configuration is used. In this manuscript, we evaluate an ASIC- based readout for digitizing all signals coming from analog photosensors. Validation results with one-to-one coupling resulted in a Coincidence Time Resolution (CTR) of 202 ps FWHM. Providing timing resolution when using detectors based on monolithic crystals is however challenging. The wide distribution of scintillation light on the photosensors causes a poor SNR, which makes the system sensible to false triggering and to time walk errors. In this direction, we present a calibration method, designed to correct all recorded timestamps and also to compensate variations in time-paths among all channels. Thereafter, a CTR improvement nearing 45% is observed for all measurements. Moreover, we show a novel approach that describes the use of averaging methods to assign a single timestamp to each gamma impact. This approach results in a further improvement of the CTR in the range of 100 ps FWHM, reaching a time resolution of 585 ps FWHM when using a large 50x50x10 mm3 LYSO scintillator coupled to an 8x8 SiPM (6x6 mm2) array. These pilot studies show detector capabilities regarding TOF information when using monolithic scintillators.

Abstract:To increase spatial resolution and signal-to-noise ratio in PET imaging, we present in this paper the design and performance evaluation of a PET detector module combining both depth-of-interaction (DOI) and time-of-flight (TOF) capabilities. The detector module consists of a staggered dual-layer LYSO block with 2 mm × 2 mm × 7 mm crystals. MR-compatible SiPM sensors (MicroFJ-30035-TSV, SensL) are assembled into an 8 × 8 array. SiPM signals from both fast and slow outputs are read out by a 128-channel ASIC chip. To test its performance, a flood histogram is acquired with a 22Na point source on top of the detector, and the energy resolution and the coincidence resolving time (CRT) value for each individual crystal are measured. The flood histogram shows excellent crystal separation in both layers. The average energy resolution at 511 keV is 14.0 and 12.7% at the bottom and top layers, respectively. The average CRT of a single crystal is 635 and 565 ps at the bottom and top layers, respectively. In conclusion, the compact DOI–TOF PET detector module is of excellent crystal identification capability, good energy resolution and reasonable time resolution and has promising application prospective in clinical TOF PET, PET/MRI, and brain PET systems.

Abstract:In this study, we developed a prototype animal PET by applying several novel technologies to use solid-state photomultiplier (SSPM) arrays to measure the depth of interaction (DOI) and improve imaging performance. Each PET detector has an 8 × 8 array of about 1.9 × 1.9 × 30.0 mm(3) lutetium-yttrium-oxyorthosilicate scintillators, with each end optically connected to an SSPM array (16 channels in a 4 × 4 matrix) through a light guide to enable continuous DOI measurement. Each SSPM has an active area of about 3 × 3 mm(2), and its output is read by a custom-developed application-specific integrated circuit to directly convert analogue signals to digital timing pulses that encode the interaction information. These pulses are transferred to and are decoded by a field-programmable gate array-based time-to-digital convertor for coincident event selection and data acquisition. The independent readout of each SSPM and the parallel signal process can significantly improve the signal-to-noise ratio and enable the use of flexible algorithms for different data processes. The prototype PET consists of two rotating detector panels on a portable gantry with four detectors in each panel to provide 16 mm axial and variable transaxial field-of-view (FOV) sizes. List-mode ordered subset expectation maximization image reconstruction was implemented. The measured mean energy, coincidence timing and DOI resolution for a crystal were about 17.6%, 2.8 ns and 5.6 mm, respectively. The measured transaxial resolutions at the center of the FOV were 2.0 mm and 2.3 mm for images reconstructed with and without DOI, respectively. In addition, the resolutions across the FOV with DOI were substantially better than those without DOI. The quality of PET images of both a hot-rod phantom and mouse acquired with DOI was much higher than that of images obtained without DOI. This study demonstrates that SSPM arrays and advanced readout/processing electronics can be used to develop a practical DOI-measureable PET scanner.

Abstract:Time-of-flight (TOF) positron emission tomography (PET) has gained remarkable development recently due to the advances in scintillator, silicon photomultipliers (SiPM), and fast electronics. However, current clinical reconstruction algorithms in TOF-PET are still based on ordered-subset-expectation-maximization (OSEM) and its variants, which may face challenges in non-conventional imaging applications, such as fast imaging within short scan time. In this work, we propose an image-TV constrained optimization problem, and tailor a primal dual algorithm for solving the problem and reconstructing images. We collect list-mode data of a Jaszczak phantom with a prototype digital TOF-PET scanner. We focus on investigating image reconstruction from data collected within reduced scan time, and thus of lower count levels. Results of the study indicate that our proposed algorithm can 1) yield image reconstruction with suppressed noise, extended axial volume coverage, and improved spatial resolution over that obtained in conventional reconstructions, and 2) yield reconstructions with potential clinical utility from data collected within shorter scan time.
A multi-resolution TOF-DOI detector for human brain dedicated PET scanner

Abstract:Abstract Objective . We propose a single-ended readout, multi-resolution detector design that can achieve high spatial, depth-of-interaction (DOI), and time-of-flight (TOF) resolutions, as well as high sensitivity for human brain-dedicated positron emission tomography (PET) scanners. Approach . The detector comprised two layers of LYSO crystal arrays and a lightguide in between. The top (gamma ray entrance) layer consisted of a 16 × 16 array of 1.53 × 1.53 × 6 mm 3 LYSO crystals for providing high spatial resolution. The bottom layer consisted of an 8 × 8 array of 3.0 × 3.0 × 15 mm 3 LYSO crystals that were one-to-one coupled to an 8 × 8 multipixel photon counter (MPPC) array for providing high TOF resolution. The 2 mm thick lightguide introduces inter-crystal light sharing that causes variations of the light distribution patterns for high DOI resolution. The detector was read out by a PETsys TOFPET2 application-specific integrated circuit. Main result . The top and bottom layers were distinguished by a convolutional neural network with 97% accuracy. All crystals in the top and bottom layers were resolved. The inter-crystal scatter (ICS) events in the bottom layer were identified, and the measured average DOI resolution of the bottom layer was 4.1 mm. The coincidence time resolution (CTR) for the top–top, top-bottom, and bottom–bottom coincidences was 476 ps, 405 ps, and 298 ps, respectively. When ICS events were excluded from the bottom layer, the CTR of the bottom–bottom coincidence was 277 ps. Significance . The top layer of the proposed two-layer detector achieved a high spatial resolution and the bottom layer achieved a high TOF resolution. Together with its high DOI resolution and detection efficiency, the proposed detector is well suited for next-generation high-performance brain-dedicated PET scanners.

A Semi-Monolithic Detector providing intrinsic DOI-encoding and sub-200 ps CRT TOF-Capabilities for Clinical PET Applications

Abstract:Current clinical PET systems utilize detectors where the scintillator typically contains single elements of 3 - 6 mm width and about 20 mm height. While providing good TOF performance, this design limits the spatial resolution and causes radial astigmatism as the DOI remains unknown. We propose an alternative, aiming to combine the advantages of current detectors with the DOI capabilities shown for monolithic concepts, based on semi-monolithic scintillators (slabs). An array of 8 monolithic LYSO slabs of dimensions 3.9 x 32 x 19 mm3 was read out by a 64-channel photosensor containing digital SiPMs. The position estimation in the detector's monolithic and DOI direction was based on a calibration with a fan beam collimator and the machine learning technique gradient tree boosting (GTB). We achieved a positioning performance in terms of mean absolute error (MAE) of 1.44 mm and 2.12 mm for DOI considering a wide energy window of 300 - 700 keV. The energy resolution was determined to be 11.3%. We established both an analytical and machine-learning-based timing calibration approach and applied them for a first-photon trigger. The analytical timing calibration corrects for electronic and optical time skews leading to 240 ps coincidence resolving time (CRT) for a pair of slab-detectors. The CRT was significantly improved by utilizing GTB to predict the time difference based on specific training data and applied on top of the analytical calibration. We achieved 209 ps for the wide energy window and 198 ps for a narrow selection around the photopeak (411 - 561 keV). To maintain the detector's sensitivity, no filters were applied to the data during processing. Overall, the semi-monolithic detector provides attractive performance characteristics. Especially, a good CRT can be achieved while introducing DOI capabilities to the detector, making the concept suitable for clinical PET scanners.

Abstract:PET systems are widely used in clinical and preclinical molecular imaging applications. Various PET systems, including whole-body human and small animal PET, and dedicated systems such as breast and brain PET have been developed. A Standardized PET detector module with good performance will make it much easier and more efficient for different kind of PET scanner instrumentation. In this paper, we developed a compact MR-compatible PET detector with DOI and TOF capability. The detector module has staggered 15×15 +16 ×16 LYSO crystal array with single crystal size of 2×2×7mm3. The detector is coupled to SiPM array (MicroFJ-30035-TSV, SensL). The output signals of SiPMs are multiplexed by 64-channel ASIC chips and then digitized by 80MHz 12-bit ADC chips and TDC implemented inside FPGA to generate energy and timing information. The performance of the detector was preliminarily evaluated. A flood image was acquired and all crystals were clearly identified with an average energy resolution of 12.7% for 15×15 array and 14.2% for 16×16 array. About 300 ps coincidence timing resolution was achieved for a pair of single crystal. More accurate energy resolution and coincidence timing resolution for the detector module will be evaluated in the future. And the MR compatibility evaluation of this module is underway. Large quantities of detector blocks will be made and tested to verify the performance stability and reproducibility. Furthermore, we are developing standardized power and signal interface for better usability.
A Novel Sub-Millimeter Resolution Pet Detector with Tof Capability

Abstract:We have developed a PET detector module based on sub-millimeter lutetium oxyorthosilicate (LSO) crystal block and prototype Silicon Photomultiplier (SiPM) arrays with additional fast timing output. The LSO array consists of 20×20 crystals of which each pixel is 0.78 × 0.78×3 mm3 and the pitch is 0.8 mm due to the reflector between pixels. The FM SiPM array and FB SiPM array are arranged 4×4 with 4×4mm2 size for each pixel and 0.2mm gap between adjacent pixels. Through dual readout of both fast and standard output signals from this new SiPM, good energy and timing performance can be achieved at the same time. Through multiplexing and summing scheme and charge division resistor network, sixteen channels of fast output and sixteen channels of standard output are reduced to one fast timing signal and four energy signals, making the detector block compact and scalable in building PET scanners. The performance of FM and FB detector module was evaluated. 16×16 LSO crystals were resolved in the flood image and the edge crystals could be identified if a light guide was used. The average energy resolution of FM detector and FB detector were 15.3% ±1.4% and 11.3% ± 0.7%,respectively, at 511 KeV. Through time pick-off method of leading edge discriminator (LED), the average coincidence resolving timing (CRT) of the block against a 3x3x20mm3 coupled with a FB30035 pixel were 401± 10 ps for FM detector and 303 ± 16 ps for FB detector. The average block-to-block CRT were estimated to be 495 ps for FM detector and 324 ps for FB detector. The FB array has demonstrated a better performance due to its much higher PDE at 420nm wavelength than FM array. We conclude that the proposed design is feasible for developing sub-millimeter resolution TOF PET detectors.
High-resolution time-of-flight PET detector with 100 ps coincidence time resolution using a side-coupled phoswich configuration

Abstract:Photon time-of-flight (TOF) capability in positron emission tomography (PET) enables reconstructed image signal-to-noise ratio (SNR) improvement. With the coincidence time resolution (CTR) of 100 picosecond (ps), a five-fold SNR improvement can be achieved with a 40 cm diameter imaging subject, relative to a system without TOF capability. This 100 ps CTR can be achieved for a clinically relevant detector design (crystal element length ≥20 mm with reasonably high crystal packing fraction) using a side-readout PET detector configuration that enables 511 keV photon interaction depth-independent light collection efficiency and lower variance in scintillation photon transit time to the silicon photomultiplier (SiPM). In this study, we propose a new concept of TOF-PET detector to achieve high (<2 mm) resolution, using a 'side-coupled phoswich' configuration, where two crystals with different decay times (τd) are coupled in a side-readout configuration to a common row of photosensors. The proposed design was validated and optimized with GATE Monte Carlo simulation studies to determine an efficient detector design. Based on the simulation results, a proof-of-concept side-coupled phoswich detector design was developed comprising two LSO crystals with the size of 1.9 × 1.9 × 10 mm3 with decay times of 34.39 and 43.07 ns, respectively. The phoswich crystals were side-coupled to the same three 4 × 4 mm2 SiPMs and detector performances were evaluated. As a result of the experimental evaluation, the side-coupled phoswich configuration achieved CTR of 107 ± 3 ps, energy resolution of 10.5% ± 1.21% at 511 keV and >95% accuracy in identifying interactions in the two adjacent 1.9 × 1.9 × 10 mm3 crystal elements using the time-over-threshold technique. Based on our results, we can achieve excellent spatial and energy resolution in addition to ~100 ps CTR with this novel detector design.

Abstract:The purpose of this study was to show how advanced concepts of compact, lossless and "Time Of Flight" (TOF) capable electronics similar to those foreseen for the LHC and ILC experiments could be fairly and easily transferred to the medical imaging field through Positron Emission Tomography (PET). As a wish of explanation, the two overriding weaknesses of PET camera readout electronics, namely dead-time and timing resolution, were investigated analytically and with a Monte-Carlo simulator presently dedicated to this task. Results have shown there was room left for count rate enhancement through a huge decrease of the timing resolution well below the nanosecond. The novel electronics scheme suggested for PET in this paper has been partly inspired by the long experience led in High Energy Physics where the latter requirement is compulsory. Its structure entirely pipelined combined to a pixelation of the whole detector should allow dead-times to be suppressed, while the absence of devoted timing channel would remove the preponderant contributions to the timing resolution. To the common solution for timing would substitute an optimal filtering method witch clearly appears as a good candidate as timing resolution of a few tens of picoseconds may be achieved provided the shape of the signal is known and sufficient samples are available with enough accuracy. First investigations have yield encouraging results as a sampling frequency of 50 MHz with a 7 bits precision appears sufficient to ensure the 500ps coincidence timing resolution planed. At this point, there will be a baby step ahead to draw benefice from a TOF implementation to the design and the enormous noise variance enhancement that would come with.

Abstract:Abstract.Prostate cancer is the most common disease in men and the second leading cause of death from cancer. Generic large imaging instruments used in cancer diagnosis have sensitivity, spatial resolution, and contrast which are inadequate for the task of imaging details of a small organ such as the prostate. In addition, multimodality imaging can play a significant role in merging anatomical and functional details coming from simultaneous PET and MRI. Indeed, multiparametric PET/MRI was demonstrated to improve diagnosis, but it suffers from too many false positives. In order to address the above limits of the current techniques, we have proposed, built and tested, thanks to the TOPEM project funded by Italian National Institute of Nuclear Phisics, a prototype of an endorectal PET-TOF/MRI probe. In the applied magnification PET geometry, performance is dominated by a high-resolution detector placed closer to the source. The expected spatial resolution in the selected geometry is about 1.5mm FWHM and efficiency of a factor 2 with respect to what was obtained with the conventional PET scanner. In our experimental studies, we have obtained a timing resolution of ∼ 320 ps FWHM and at the same time a Depth of Interaction (DOI) resolution of under 1mm. Tests also showed that mutual adverse PET-MR effects are minimal. In addition, the matching endorectal RF coil was designed, built and tested. In the next planned studies, we expect that benefiting from the further progress in scintillator crystal surface treatment, in SiPM technology and associated electronics would allow us to significantly improve TOF resolution.
A Dual-Layer TOF-DOI Detector Block for Whole-Body PET

Abstract:We developed a dual-layer high-resolution detector block for whole-body positron-emission tomography systems. The top layer is a 13×13 array of lutetium yttrium orthosilicate (LYSO) crystals (2.85×2.85×10mm3) and the bottom layer a 13×13 array of lutetium gadolinium oxyorthosilicate (LGSO) crystals (2.85×2.85×10mm3). The LGSO layer was coupled to four Hamamatsu R11194HA photomultiplier tubes (PMTs) by a photomultiplier quadrant×sharing configuration. This block has both time-of-flight and depth-of-interaction functions. Using a Na-22 position source, the performance of this block was evaluated by coinciding with a reference detector which consisted of a 4×4×10mm3 LYSO crystal coupled to a Hamamatsu R9779 PMT. A total of 4.7% of the counts were mixed among the two layers. The LYSO layer had 11.4% energy resolution and 329ps coincidence time resolution.

Abstract:Organ-specific, targeted Field-of-View (FoV) Positron Emission Tomography (PET)/Magnetic Resonance Imaging (MRI) inserts are viable solutions for a number of imaging tasks where whole-body PET/MRI systems lack the necessary sensitivity and resolution. To meet the required PET detector performance of these systems, high count-rates and effective spatial resolutions on the order of a few mm, a novel two-axis patterned reflector foil pixelated scintillator crystal array design is developed and its proof-of-concept illustrated in-silico with the Monte Carlo radiation transport modelling toolkit Geant4. It is shown that the crystal surface roughness and phased open reflector cross-section patterns could be optimised to maximise either the PET radiation detector's effective spatial resolution, or count rate before event pile up. In addition, it was illustrated that these two parameters had minimal impact on the energy and time resolution of the proposed PET radiation detector design. Finally, it is shown that a PET radiation detector with balance performance could be constructed using ground crystals and phased open reflector cross-section pattern corresponding to the middle of the tested range.
