Abstract:Objectives Airway reconstruction for laryngo tracheal stenosis (LTS) improves dyspnoea. There is little evidence relating to impact upon voice and swallowing. We explored voice and swallowing outcomes in adults with LTS before and after reconstructive surgery. Design Outcome measures were collected pre‐reconstructive surgery, two‐weeks post‐surgery and up to 4‐6 months post‐surgery. Setting Tertiary referral centre. Participants With ethical approval, twenty consecutive adult (≥18 years) LTS patients undergoing airway reconstruction were prospectively recruited. Main outcome measures These included physiological values (maximum phonation time (MPT) and fundamental frequency; penetration aspiration score, residue score), clinician‐reported (GRBAS, functional oral intake score, 100ml Water Swallow Test) and patient‐reported outcomes (Voice Handicap Index‐10, Reflux Symptoms Index, Eating Assessment Tool, Dysphagia Handicap Index). Results The observational study identified patient‐reported and clinician‐reported voice and swallow difficulties pre‐ and post‐surgery; median and interquartile range are reported at each time point: Voice Handicap Index‐10 23 (8‐31); 20.5 (9‐33.5), 24.5 (12.5‐29); Dysphagia Handicap Index 9 (0‐37); 13 (7‐44); 15 (4‐34); GRBAS grade 1(1‐2); 2 (1‐2.5); 2(1‐2); 100ml Water Swallow Test volume score 16.7 (11.1‐20); 14.3 (12.5‐16.7); 16.7 (14.3‐20.0); 100ml Water Swallow Test capacity score 16.3 ± 9.0; 11.0 ± 4.1; 12.5 ± 2.6. Conclusions We present the first prospective data on voice and swallowing outcomes in adults with LTS before and after reconstructive surgery. The variability of the outcomes was higher than expected but importantly, for many the voice and swallow outcomes were not within normal limits before surgery. The clinical value of the study demonstrates the need for individual assessment and management of LTS patients' voice and swallowing.

Abstract:Abstract OBJECTIVES Benign (laryngo-)tracheal stenosis is a relatively rare pathology and its surgical treatment is performed only at few specialized centres. This study aims to investigate outcomes after (laryngo-)tracheal resection–anastomosis, to explore potential risk factors for postoperative complications and to assess whether, over a 33-year period, there were major changes in surgical indications, techniques or outcomes. METHODS Retrospective, single-centre review of all consecutive patients who underwent tracheal or laryngo-tracheal resection/anastomosis for benign pathologies from 1990 to 2023. RESULTS Overall, 211 patients underwent tracheal (149 patients, 70.6%) and laryngo-tracheal (62 patients, 29.4%) resection–anastomosis. Of these, 195 patients (93.8%) were affected by iatrogenic stenosis, while 13 (6.2%) suffered from idiopathic stenosis. The median length of stenosis was 25 mm (interquartile range 1–3, 20–30). The overall morbidity rate was 27.5%, while major morbidity occurred in 10.5% of cases. One patient (0.5%) died in the postoperative period. Glottic oedema (17 patients, 8.1%), granulations (12 patients, 5.7%) and restenosis (10 patients, 4.7%) were the main complications. The only independent risk factor for postoperative complications was the length of the resected airway (P = 0.019). In the latest half of the study period, an older median age was observed, and no patient with idiopathic tracheal stenosis underwent surgery. Postoperative outcomes were comparable between surgical eras. CONCLUSIONS Surgical treatment of (laryngo-)tracheal stenosis is challenging and should be performed by specialized centres. In our experience, morbidity and mortality rates were satisfactory, and in most cases, patients could breathe without tracheostomy. The length of the stenosis was the most significant risk factor for postoperative complications.

Abstract:Abstract Background Surgical treatment for airway stenosis necessitates personalized techniques based on the stenosis location and length, leading to favorable surgical outcomes. However, there is limited literature on functional outcomes following laryngotracheal surgery with an adequate number of patients. Material and Methods We conducted a retrospective analysis of patients who underwent laryngotracheal surgery at the Department of Thoracic Surgery, Medical University of Vienna, from January 2017 to June 2021. The study included standardized functional assessments before and after surgery, encompassing spirometry, voice measurements, swallowing evaluation, and subjective patient perception. Results The study comprised 45 patients with an average age of 51.9 ±15.9 years, of whom 89% were female, with idiopathic being the most common etiology (67%). Procedures included standard cricotracheal resection (CTR) in 11%, CTR with dorsal mucosal flap in 49%, CTR with dorsal mucosal flap and lateral cricoplasty in 24%, and single-stage laryngotracheal reconstruction in 16%. There were no in-hospital mortalities or restenosis cases during the mean follow-up period of 20.8±13.2 months. Swallowing function remained intact in all patients. Voice evaluations showed a decrease in fundamental vocal pitch (203 (81-290) Hz vs. 150 (73-364) Hz, p < 0.001) and dynamic voice range (23.5 ±5.8 semitones vs. 17.8 ±6.7 semitones, p < 0.001). However, no differences in voice volume were observed (60.0 ±4.1 dB vs. 60.2 ±4.8 dB, p = 0.788). The overall predicted voice profile changed from R0B0H0 to R1B0H1. Conclusion Laryngotracheal surgery proves effective in fully restoring breathing capacity while preserving vocal function. Even in cases of high-grade and complex airway stenosis necessitating laryngotracheal reconstruction, favorable functional outcomes can be achieved.

The Outcomes of Endoscopic Management in Young Children with Subglottic Stenosis.

Abstract:Subglottic stenosis (SGS) is a common cause of obstructed airway in children, and the treatment of pediatric SGS, especially congenital SGS, remains a challenge for the otolaryngologist.Objective: To analyze the outcomes of endoscopic management in young children with SGS.Methods: We performed a retrospective review of treatment with endoscopic balloon dilation (EBD) or EBD combined with endoscopic anterior cricoid split (EACS) for young SGS children, from December 2008 to December 2015. The ages of patients ranged from 2 days to 12 years, median age was 5 months. The grade of them ranged from II to IV.Results: For acute acquired SGS, 19 cases received EBD alone and the other 3 cases received EBD and EACS, the success rate was about 95.5%; For chronic acquired SGS, EBD and EACS was performed in 6 patients with a success rate of 66.7%; For congenital SGS, EBD and EACS was performed in 28 patients with a success rate of 85.7%. Overall, the success rate of endoscopic management in 56 young children was about 87.5%. Besides, No procedure-related complications were observed in any patients.Conclusions: Endoscopic surgical technique offers a safe and effective approach for treatment of young children with SGS, especially in congenital SGS. [Surgical Management of Laryngotracheal Stenosis: a Retrospective Study of 53 Cases].

Abstract:OBJECTIVE:To analyze the etiology, means of diagnosis and surgical approaches for the treatment of laryngotracheal stenosis (LTS), and to evaluate their effects on the outcome of surgical management of LTS.METHODS:Fifty-three patients with benign LTS were retrospectively studied for the clinical trial in Peking University, First Hospital, ENT Head & Neck Department between April 1989 and April 2005. The follow-up ranged from 1 to 16 years. The surgical approaches included endoscopic removal of the obstruction with or without CO(2) Laser and open surgical techniques. Open surgical techniques included scar resection, arytenoidectomy, laryngoplasty with hyoid bone and end-to-end anastomosis. A number of grafts (cartilage, hyoid bone) and intraluminal stents were used. Statistical analysis was performed by the chi(2) test.RESULTS:The overall success was achieved in 38 of 53 patients (71.7%). Three cases were lost of follow-up and 12 cases were lived with endotracheal tube in place. As for the surgical approach, 19 cases (35.8%) were done by open surgical technique and 34 cases (64.2%) by endoscopic approach. The successful rates were 73.7% (14/19) and 70.6% (24/34) respectively. There were 31 cases of isolated stenosis with a successful rate of 87.1% (27/31) and 22 cases of multiple sites with a successful rate of 50.0% (11/22). Isolated stenosis had a statistically better result than multiple stenosis through the chi(2) test (chi(2) = 8.727, P = 0.003). As for the length of stenosis, 47 cases were less than 3 cm with a successful rate of 80.8% (38/47), 6 cases were longer than 3 cm with no success. The stenosis of less than 3 cm had a statistically better result than longer stenosis through the chi(2) test (chi(2) = 17.140, P = 0.000). According to the Cotton grading system of LTS, there were 28 cases of Grade I with a successful rate of 96.4% (27/28) and 25 cases of Grade II - IV with a successful rate of 44.0% (11/25). Grade I stenosis according to Cotton grading system of LTS had better prognosis than Grade II - IV (chi(2) = 17.891, P = 0.000). As for the stents, finger cot usage of one case was successful, while laryngeal stents had a successful rate of 72.2% (13/18) and T tube of 25.0% (1/4). Six cases of hyoid graft with sternohyoid muscle flap were suess.CONCLUSIONS:Surgical approaches of LTS should be chosen according to the site, length and diameter of stenosis. The operative efficacy ranged from case to case, while shorter stenosis and isolated stenosis might be easier to handle than multiple stenosis. The use of stents and interposition grafts were also helpful in some cases.

Abstract:OBJECTIVE: We reviewed our experience with pediatric single-stage laryngotracheal reconstruction (SSLTR) to identify factors that affect postoperative outcomes, including the need for reintubation and tracheostomy. STUDY DESIGN: Retrospective chart review was done. RESULTS: In total, 190 children underwent 200 SSLTRs; 29% were reintubated, and 15% required postoperative tracheostomy. Currently, 96% are decannulated. The use of anterior and posterior costal cartilage grafting, age less than 4 years, sedation for more than 48 hours, a leak pressure around the endotracheal tube at greater than 20 cm H 2 O, and moderate/severe tracheomalacia significantly increased the rate of reintubation. The duration of stenting did not affect outcomes. Children with anterior and posterior grafts and those with moderate or severe tracheomalacia were more likely to need a postoperative tracheostomy. CONCLUSION: SSLTR can be effective for the treatment of pediatric laryngotracheal stenosis. Diligent preoperative assessment of the patient and the patient's airway and close postoperative care are important to the success of this operation. (Otolaryngol Head Neck Surg 2000;123:430-4.)

Abstract:Background Tracheobronchomalacia (TBM) is characterized by excessive dynamic airway collapse. Severe TBM can be associated with substantial morbidity. Children with secondary TBM associated with esophageal atresia/tracheoesophageal fistula (EA/TEF) and vascular‐related airway compression (VRAC) demonstrate clinical improvement following airway pexy surgery. It is unclear if children with severe primary TBM, without secondary etiologies (EA/TEF, vascular ring, intrinsic pulmonary pathology, or complex cardiac disease) demonstrate clinical improvement following airway pexy surgery. Materials and Methods The study cohort consisted of 73 children with severe primary TBM who underwent airway pexy surgery between 2013 and 2020 at Boston Children's Hospital. Pre‐ and postoperative symptoms as well as bronchoscopic findings were compared with Fisher exact test for categorical data and Student's t‐test for continuous data. Results Statistically significant improvements in clinical symptoms were observed, including cough, noisy breathing, prolonged respiratory infections, pneumonias, exercise intolerance, cyanotic spells, brief resolved unexplained events (BRUE), and noninvasive positive pressure ventilation (NIPPV) dependence. No significant differences were seen regarding oxygen dependence, ventilator dependence, or respiratory distress requiring NIPPV. Comparison of pre‐ and postoperative dynamic bronchoscopy findings revealed statistically significant improvement in the percent of airway collapse in all anatomic locations except at the level of the upper trachea (usually not malacic). Despite some initial improvements, 21 (29%) patients remained symptomatic and underwent additional airway pexies with improvement in symptoms. Conclusion Airway pexy surgery resulted in significant improvement in clinical symptoms and bronchoscopic findings for children with severe primary TBM; however, future prospective and long‐term studies are needed to confirm this benefit.

Intermediate-Term Outcomes of Slide Tracheoplasty in Pediatric Patients With Ring-Sling Complex

Abstract:Background: This study was intended to determine the intermediate-term outcomes after slide tracheoplasty (STP) in patients with ring-sling complex. Methods: All children undergoing STP between 2009 and 2018 were included. The patients' baseline characteristics, perioperative management details, and follow-up evaluations were reviewed retrospectively. Results: Median age was 1.2 years (range, 2 months to 9.2 years). Seventy-eight patients had additional cardiovascular anomalies and 7 had unilateral pulmonary hypoplasia. Carina involved in the stenotic lesion was present in 42 patients and carinal compression occurred in 58. Twenty-five (21.6%) patients had a stenosis extending into the main bronchus. Type 2, typically having aberrant tracheobronchial patterns, accounted for nearly two-thirds of the patients according to Wells' classification. There were 7 in-hospital deaths and 8 late deaths. Median follow-up was 2.1 years (range, 1.2 months to 9.2 years). Type 2B patients had a higher incidence of malacia and restenosis. In multivariate analysis, the additional cardiovascular anomaly was significantly associated with postoperative tracheomalacia (P = .037). Carinal stenosis (P = .034) was significantly associated with recurrent stenosis. Bronchial stenosis with concomitant carinal stenosis and compression was significantly associated with postoperative tracheomalacia, restenosis, and mortality (for all comparisons, P < .001). Conclusions: Intermediate outcomes of STP in patients with ring-sling complex is satisfactory. Type 2B is a heterogenous subset patients with more severe tracheobronchial anomalies who will benefit from recognition preoperatively and close surveillance postoperatively.
Long term Follow Up of Severe Laryngomalacia Patients Following CO2 LASER Supraglottoplasty

Abstract:Laryngomalacia is the most common cause of stridor. It is believed to be due to delayed development of the supraglottic structures. On the basis of presentation, it has been divided into 3 grades. Management is dependent upon the grades: Mild and moderate laryngomalacia are managed conservatively, while CO2 LASER supraglottoplasty is the surgical management of choice for severe disease. In this study we evaluated the outcomes of supraglottopasty in long term follow up. It was a retrospective study which included patients with severe laryngomalacia who have undergone supraglottoplasty for the dates from July 2013 to July 2018. Weight and height of the patients were taken during the follow up visit after one year and have been compared with that of the normal children of comparable age using the pediatrics growth charts. Follow up was done by telephonic conversation to evaluate the status of other symptoms associated with laryngomalacia. Total of 44 patients were included in the study, 47.7% and 52.3% females and males respectively. Weight for age, weight for height and height for age were normal in 80.9%, 80.9% and 76.1% of patients respectively after supraglottoplasty. Of the patients studied, stridor was resolved in 80.9%, early tiredness during play was resolved in 43.2%, swallowing was normal in 85.7% and there was no aspiration in 90.5%. It can be concluded that patients with severe laryngomalacia improved with respect to airway symptoms, as well as, weight and height on long term follow up after CO2 LASER supraglottoplasty. It is important to evaluate the general status of the patients as a whole to truly assess the success of the surgery in addition to the airway symptoms.

Oncological Outcomes of Surgical Management for T2N0M0 Glottic Laryngeal Squamous Cell Carcinoma

Abstract:ObjectivesThe research aimed to evaluate the clinical treatment outcomes of T2N0M0 glottic laryngeal squamous cell carcinoma (LSCC) patients who underwent laryngectomy.MethodsRetrospective review of 533 T2N0M0 glottic LSCC patients.ResultsFive-year cancer-specific survival (CSS) rate was 90.0%, and the overall survival (OS) rate was 89.1%. No statistically difference was found between the patients who have undergone total laryngectomy (5-year disease-free survival[DFS] = 80.7%, and the CSS = 86.7%) and those who have had partial laryngectomy (the 5-year DFS = 85.3%, and CSS = 91.1%). There was no difference in the CSS and DFS rates between patients with negative margins and those with positive margins following postoperative radiotherapy (PORT) +/- chemotherapy (the CSS: 90.8% vs. 81.8%, p = 0.458 and 5-year DFS: 84.6% vs. 79.5%, p = 0.371). Patients who underwent vertical partial laryngectomy (VPL) had better survival (5-year OS was 91.9%, and the CSS was 92.8%) than those who underwent cricohyoidoepiglottopexy (CHEP) or cricohyoidopexy (CHP) (the 5-year OS = 83.8%, p = 0.022 and CSS = 84.9%, p = 0.038).ConclusionsSurgery remains the gold standard for treating T2N0M0 glottic LSCC patients because it can achieve satisfactory oncological outcomes. Regarding the systemic conditions, the effect of partial laryngectomy is similar to that of total laryngectomy. Moreover, partial laryngectomy preserves the function of the larynx. VPL may be superior to CHP/CHEP, depending upon the invasiveness of the tumor.

Abstract:Objectives: Multicenter studies reporting outcomes following tracheostomy in children with congenital heart disease are limited, particularly in patients with single ventricle physiology. We aimed to describe clinical characteristics and outcomes in a multicenter cohort of patients with single ventricle physiology who underwent tracheostomy before Fontan operation. Design: Multicenter retrospective cohort study.SETTING: Twenty-one tertiary care pediatric institutions participating in the Collaborative Research from the Pediatric Cardiac Intensive Care Society. Patients: We reviewed 99 children with single ventricle physiology who underwent tracheostomy before the Fontan operation at 21 institutions participating in Collaborative Research from the Pediatric Cardiac Intensive Care Society between January 2010 and December 2020, with follow-up through December 31, 2021. Interventions: None. Measurements and Main Results: Death occurred in 51 of 99 patients (52%). Cox proportional hazard analysis was performed to determine factors associated with death after tracheostomy. Results are presented as hazard ratio (HR) with 95% CIs. Nonrespiratory indication(s) for tracheostomy (HR, 2.21; 95% CI, 1.14–4.32) and number of weeks receiving mechanical ventilation before tracheostomy (HR, 1.06; 95% CI, 1.02–1.11) were independently associated with greater hazard of death. In contrast, diagnosis of tricuspid atresia or Ebstein’s anomaly was associated with less hazard of death (HR, 0.16; 95% CI, 0.04–0.69). Favorable outcome, defined as survival to Fontan operation or decannulation while awaiting Fontan operation with viable cardiopulmonary physiology, occurred in 29 of 99 patients (29%). Median duration of mechanical ventilation before tracheostomy was shorter in patients who survived to favorable outcome (6.1 vs. 12.1 wk; p < 0.001), and only one of 16 patients with neurologic indications for tracheostomy and 0 of ten patients with cardiac indications for tracheostomy survived to favorable outcome. Conclusions: For children with single ventricle physiology who undergo tracheostomy, mortality risk is high and should be carefully considered when discussing tracheostomy as an option for these children. Favorable outcomes are possible, although thoughtful attention to patient selection and tracheostomy timing are likely necessary to achieve this goal.

Abstract:Tracheostomy is a common decision parents of critically ill children are asked to consider. The prevalence of tracheostomy placement in the United States is increasing, with nearly 5,000 pediatric tracheostomies placed annually (<a class="ejp-citation-link js-ejp-citation-link">1</a>). The decision to pursue tracheostomy is not one to be taken lightly. It is associated with real benefits and, unfortunately, many burdens and complications (<a class="ejp-citation-link js-ejp-citation-link">2</a>). Studies have reported a 6–15% mortality rate years following tracheostomy placement, an average of four hospitalizations and a 38.8% complication rate in the 2-year period following tracheostomy placement (<a class="ejp-citation-link js-ejp-citation-link">3–5</a>). At our institution, the inpatient mortality rate after tracheostomy placement and prior to initial discharge is 11%, whereas the mortality rate for all children in our PICU is 2–3%. In addition to high mortality rates, these patients comprise 20% of PICU admissions (<a class="ejp-citation-link js-ejp-citation-link">6</a>), consume high proportions of critical care resources, have prolonged lengths of stays in the ICU and often require transition to a subacute facility prior to returning home due to intense parental training (<a class="ejp-citation-link js-ejp-citation-link">7</a>). The potential complications and long-term implications make it imperative parents are resolute in their decision to pursue this life-altering procedure for their child.

Abstract:BACKGROUND: Airway anomalies, symptoms and interventions are commonly reported in children with oesophageal atresia with tracheoesophageal fistula (OA/TOF). The purpose of this study was to assess the incidence of these airway pathologies and those requiring interventions in the long-term.METHODS: A retrospective case note review of all patients admitted to the Neonatal Unit at the Royal Hospital for Children, Glasgow between January 2000 and December 2015 diagnosed with OA/TOF. Included patients had a minimum of 5 years follow-up.RESULTS: 121 patients were identified. 118 proceeded to OA/TOF repair. 115 patients had long-term follow-up data. Ninety-five (83%) children had one or more airway symptom recorded. Thirty-six (31%) neonates underwent airway endoscopy at the time of their initial OA/TOF repair. Forty-six (40%) children underwent airway endoscopy at a later date due to airway symptoms. Airway pathologies identified included airway malacia, thirty-two (28%), subglottic stenosis, eleven (10%), tracheal pouch, twenty-five (22%), laryngeal cleft, seven (6%) and recurrent fistula, five (4%). Airway interventions included endoscopic division of tracheal pouch, ten (9%), tracheostomy, seven (6%), aortopexy, six (5%), repair of recurrent fistula, five (4%), endoscopic repair of laryngeal cleft, three (3%) and four (3%) required open airway reconstruction for subglottic stenosis. One child (1%) remains tracheostomy dependent.CONCLUSIONS: Long-term airway pathologies are common in children with OA/TOF. Many of these are remediable with surgical intervention. Clinicians should be cognisant of this and refer to Airway Services appropriately.

Improving patient care and quality of life after laryngectomy/glossectomy

Abstract:Patients undergoing total laryngectomy/glossectomy are left without a voice at least temporarily in the early postoperative stage, since their larynx and/or tongue has been removed. Those patients rendered permanently speechless usually acquire a form of mechanical speech through electronic devices. However, the life-changing nature of this surgery should not be underrated as effectively these people have lost their normal voice. It can be argued that this patient group is also without a voice in a political sense - theirs seems to be a forgotten cancer, as other forms of cancer appear to attract greater attention for research priorities and funding. In some ways this research attempts to redress the balance through exploring the patient experience postoperatively, in particular the issues related to communication for this patient group. The results also highlight the emotional trauma experienced by patients and the mechanisms healthcare staff employ to support these patients. This research is multidisciplinary, involving patients and all those health workers that come into contact with them on the ward. The patient experience forms a major part of this article, allowing their stories to form much of the content. The results draw attention to deficiencies in service delivery that, as a direct result of this research, have been and are being addressed so that patients' care is improved and their quality of life is restored. Such change has been driven by patient comment such as: 'But then when you wake up after that operation that's the pits when you are laying there and you think "I'm dead". And then when you wake up properly that's the bit I always say you wish you were dead. That's when you wish you had died.'

Abstract:This systematic review and meta‐analysis investigated the ideal treatment for pediatric posterior glottic stenosis (PGS). Posterior cricoid split with cartilage graft and mucosal advancement flaps can be performed for high PGS grades. Endoscopic and open techniques are efficacious with high decannulation rates and improvement in airway obstruction, with endoscopic surgeries offering a less invasive approach. Objectives To evaluate the outcomes of open and endoscopic posterior cricoid split grafting and mucosal flaps for the treatment of pediatric posterior glottic stenosis (PGS). Data Sources COCHRANE Library, CINAHL, PubMed, and Scopus databases. Methods Studies were grouped by treatment method. Outcomes included decannulation rate, improvement in airway obstruction, remaining vocal symptoms, and subsequent airway procedures. A meta‐analysis of continuous measures and proportions (%) with 95% confidence interval (CI) was conducted. Results Nineteen studies with 223 patients were included, with 72 patients having isolated PGS. Twelve studies (n = 23) reported endoscopic repair (group 1), while seven studies (n = 49) reported open techniques (group 2). Most patients in group 1 and group 2 had grade IV PGS (55.9% [26.1%–83.5%] and 56.9% [30.5%–80.8%], respectively). Post‐operatively, 70.8% [43.5%–91.7%] of group 1 patients had tracheostomies with 83.4% [62.1%–95.5%] subsequently being decannulated, whereas 97.3% [89.1%–99.8%] of group 2 patients had tracheostomies post‐operatively with 90.2% [79.2%–96.5%] subsequently being decannulated. Group 1 and group 2 had 87.1% [70.5%–96.3%] and 84.4% [64.9%–95.5%] improvement in airway obstruction, respectively. Conclusion There are multiple open and endoscopic surgical options for pediatric PGS. Posterior cricoid split with cartilage graft and mucosal advancement flaps can be performed for high PGS grades.
