Abstract:Objective The treatment of laryngotracheal stenosis is a major therapeutic challenge. Various treatments include observation, medical management, and surgical management. The most effective surgical management is resection and reconstruction. To the authors' knowledge, no reports have described the use of xenogenic acellular dermal matrix (ADM) for laryngotracheal stenosis. Methods A 27-year-old man presented with hemoptysis of the neck due to a traffic accident. Emergency orotracheal intubation was performed. Tracheostomy was then performed under local anesthesia. Computed tomography revealed fractures of the right thyroid cartilage and posterior arc of the cricoid cartilage and stenosis of the subglottis and first and second tracheal rings. We used a composite hyoid-sternohyoid osseomuscular flap with xenogenic ADM and a straight silicone tube as a lumen stent to reconstruct the laryngotracheal stenosis. Results Surgical recovery was uneventful. The tracheotomy opening was changed to a metal tube 5 days postoperatively. Four months postoperatively, the silicone tube was endoscopically removed under local anesthesia. The patient was decannulated 20 days later. The patient satisfied with his voice, respiration, and deglutition at the 16-month postoperative follow-up. Conclusion The use of ADM for laryngotracheal stenosis may reduce the growth of granulation tissues and promote the repair process.

Abstract:Abstract OBJECTIVES Benign (laryngo-)tracheal stenosis is a relatively rare pathology and its surgical treatment is performed only at few specialized centres. This study aims to investigate outcomes after (laryngo-)tracheal resection–anastomosis, to explore potential risk factors for postoperative complications and to assess whether, over a 33-year period, there were major changes in surgical indications, techniques or outcomes. METHODS Retrospective, single-centre review of all consecutive patients who underwent tracheal or laryngo-tracheal resection/anastomosis for benign pathologies from 1990 to 2023. RESULTS Overall, 211 patients underwent tracheal (149 patients, 70.6%) and laryngo-tracheal (62 patients, 29.4%) resection–anastomosis. Of these, 195 patients (93.8%) were affected by iatrogenic stenosis, while 13 (6.2%) suffered from idiopathic stenosis. The median length of stenosis was 25 mm (interquartile range 1–3, 20–30). The overall morbidity rate was 27.5%, while major morbidity occurred in 10.5% of cases. One patient (0.5%) died in the postoperative period. Glottic oedema (17 patients, 8.1%), granulations (12 patients, 5.7%) and restenosis (10 patients, 4.7%) were the main complications. The only independent risk factor for postoperative complications was the length of the resected airway (P = 0.019). In the latest half of the study period, an older median age was observed, and no patient with idiopathic tracheal stenosis underwent surgery. Postoperative outcomes were comparable between surgical eras. CONCLUSIONS Surgical treatment of (laryngo-)tracheal stenosis is challenging and should be performed by specialized centres. In our experience, morbidity and mortality rates were satisfactory, and in most cases, patients could breathe without tracheostomy. The length of the stenosis was the most significant risk factor for postoperative complications.

[Surgical Management of Laryngotracheal Stenosis: a Retrospective Study of 53 Cases].

Abstract:OBJECTIVE:To analyze the etiology, means of diagnosis and surgical approaches for the treatment of laryngotracheal stenosis (LTS), and to evaluate their effects on the outcome of surgical management of LTS.METHODS:Fifty-three patients with benign LTS were retrospectively studied for the clinical trial in Peking University, First Hospital, ENT Head & Neck Department between April 1989 and April 2005. The follow-up ranged from 1 to 16 years. The surgical approaches included endoscopic removal of the obstruction with or without CO(2) Laser and open surgical techniques. Open surgical techniques included scar resection, arytenoidectomy, laryngoplasty with hyoid bone and end-to-end anastomosis. A number of grafts (cartilage, hyoid bone) and intraluminal stents were used. Statistical analysis was performed by the chi(2) test.RESULTS:The overall success was achieved in 38 of 53 patients (71.7%). Three cases were lost of follow-up and 12 cases were lived with endotracheal tube in place. As for the surgical approach, 19 cases (35.8%) were done by open surgical technique and 34 cases (64.2%) by endoscopic approach. The successful rates were 73.7% (14/19) and 70.6% (24/34) respectively. There were 31 cases of isolated stenosis with a successful rate of 87.1% (27/31) and 22 cases of multiple sites with a successful rate of 50.0% (11/22). Isolated stenosis had a statistically better result than multiple stenosis through the chi(2) test (chi(2) = 8.727, P = 0.003). As for the length of stenosis, 47 cases were less than 3 cm with a successful rate of 80.8% (38/47), 6 cases were longer than 3 cm with no success. The stenosis of less than 3 cm had a statistically better result than longer stenosis through the chi(2) test (chi(2) = 17.140, P = 0.000). According to the Cotton grading system of LTS, there were 28 cases of Grade I with a successful rate of 96.4% (27/28) and 25 cases of Grade II - IV with a successful rate of 44.0% (11/25). Grade I stenosis according to Cotton grading system of LTS had better prognosis than Grade II - IV (chi(2) = 17.891, P = 0.000). As for the stents, finger cot usage of one case was successful, while laryngeal stents had a successful rate of 72.2% (13/18) and T tube of 25.0% (1/4). Six cases of hyoid graft with sternohyoid muscle flap were suess.CONCLUSIONS:Surgical approaches of LTS should be chosen according to the site, length and diameter of stenosis. The operative efficacy ranged from case to case, while shorter stenosis and isolated stenosis might be easier to handle than multiple stenosis. The use of stents and interposition grafts were also helpful in some cases.

Stenosis location was in the tracheal upper-third in 108 (98.2%), the middle-third in 1 (0.9%), and the lower-third in 1 (0.9%). Stenosis length ranged from 1 to 5.6 cm. Follow-up ranged from 1 to 60 months (two years for the most). Most frequent complications were transitory dysphagia in 13 (11.3%), granuloma formation in 8 (7.3%), dehiscence or air leak in 5 (4.5%), and wound infection in 4 (3.6%). Restenosis rate ranged from 2% to 25%. There was no perioperative mortality. CONCLUSIONS Tracheal resection and primary anastomosis in post-intubation tracheal stenosis appear to be safe and effective in the short and mid-terms; however, the very low level of evidence found prevents definitive conclusions.

Abstract:Abstract Background Surgical treatment for airway stenosis necessitates personalized techniques based on the stenosis location and length, leading to favorable surgical outcomes. However, there is limited literature on functional outcomes following laryngotracheal surgery with an adequate number of patients. Material and Methods We conducted a retrospective analysis of patients who underwent laryngotracheal surgery at the Department of Thoracic Surgery, Medical University of Vienna, from January 2017 to June 2021. The study included standardized functional assessments before and after surgery, encompassing spirometry, voice measurements, swallowing evaluation, and subjective patient perception. Results The study comprised 45 patients with an average age of 51.9 ±15.9 years, of whom 89% were female, with idiopathic being the most common etiology (67%). Procedures included standard cricotracheal resection (CTR) in 11%, CTR with dorsal mucosal flap in 49%, CTR with dorsal mucosal flap and lateral cricoplasty in 24%, and single-stage laryngotracheal reconstruction in 16%. There were no in-hospital mortalities or restenosis cases during the mean follow-up period of 20.8±13.2 months. Swallowing function remained intact in all patients. Voice evaluations showed a decrease in fundamental vocal pitch (203 (81-290) Hz vs. 150 (73-364) Hz, p < 0.001) and dynamic voice range (23.5 ±5.8 semitones vs. 17.8 ±6.7 semitones, p < 0.001). However, no differences in voice volume were observed (60.0 ±4.1 dB vs. 60.2 ±4.8 dB, p = 0.788). The overall predicted voice profile changed from R0B0H0 to R1B0H1. Conclusion Laryngotracheal surgery proves effective in fully restoring breathing capacity while preserving vocal function. Even in cases of high-grade and complex airway stenosis necessitating laryngotracheal reconstruction, favorable functional outcomes can be achieved.

Abstract:Abstract Introduction This study reports our experience with open reconstructive surgery in patients with laryngotracheal stenosis (LTS) following prolonged intubation and/or tracheostomy in the context of COVID-19. Methods All patients underwent a preoperative endoscopic airway assessment. Posterior glottic lesions were graded according to the Bogdasarian classification, subglottic-tracheal lesions according to the Cotton-Myers classification and postoperative complications reported by the Clavien–Dindo classification. We report postoperative outcomes and functional results in this patient subset. Results We include 14 patients diagnosed to have post COVID LTS, one female and 13 males. This group was compared with a control group, diagnosed with LTS following prolonged intubation. In the COVID group, mean age of patients at the time of the airway surgery was 52.1 ± 16.8 years (range: 13.7–76.3). More than half the patients were multi-morbid with hypertension and obesity being the most common conditions. Eleven patients had multi-site stenoses. Open surgical interventions performed were tracheal resection and anastomosis, laryngotracheal reconstruction and extended cricotracheal resection, and postoperative complications were seen in 12 (85.6%) patients. 70% patients with pre-existing tracheostomy were decannulated. Oral swallowing was not tolerated in one-fifth of the patients and a significant number of them have poor voice quality. Conclusion Post-COVID pandemic, airway surgeons are seeing an increased number of patients with complex LTS, and we report significant postoperative complications in this patient subset. Decannulation rates, voice and swallowing results are poor in patients with glottic involvement as compared to isolated tracheal stenosis.

Abstract:Seventy-five cases of laryngotracheal stenosis treated between 1981 and 1991 were reviewed to determine the effectiveness of surgical treatment on the basis of site and severity of stenosis. Decannulation and absence of exertional dyspnea were the criteria of successful management. The treatment methods used were endoscopic laser incision and dilatation, expansion laryngotracheoplasty, and segmental resection. Endoscopic procedures were effective in treating thin (< 1 cm) stenoses in the subglottis and trachea. Laryngotracheoplasty was most effective in treating thick stenoses of the glottis and subglottis. Tracheal stenoses were most effectively treated by segmental resections. Bronchoscopic dilatation in the management of benign (non-transplant) tracheobronchial stenosis

Abstract:Background: Tracheobronchial stenosis in the adult patient is a recognized postoperative complication of sleeve resection or lung transplantation, but also occurs in medical conditions such as sarcoidosis, tuberculosis, postintubation/tracheostomy or post-radiation. Aims: To assess the response of bronchoscopic dilatation in the management of benign (non-transplant) tracheobronchial stenosis and the longevity of symptomatic relief. Methods: Eight patients underwent bronchoscopic dilatation for benign (non-transplant) tracheobronchial stenosis. The indications were post-tuberculous bronchostenosis (n = 3), post-tracheostomy/endotracheal intubation strictures (n = 3), postradiation bronchostenosis (n = 1) and narrowing of the tracheal lumen following a muscle flap surgery for tracheoesophageal fistula (n = 1). Results: Dilatation alone was effective in the management of four patients (50%). Two patients had stent placement postdilatation, one patient had tracheal resection and primary anastomosis and one patient had laser ablation for restenosis followed by balloon dilatation. All patients had clinical improvement. One patient was successfully weaned off mechanical ventilation and extubated. There was no procedure-related mortality and all patients were alive and well at the time of reporting, with a mean duration since procedure of 123 +/- 105 (range 8-340) weeks. The complications observed were granuloma formation at the site of laser excision and restenosis, each in one patient. Conclusions: Bronchoscopic dilatation is a safe and effective modality in the initial assessment and management of benign tracheobronchial stenosis. Stent placement and Nd:YAG laser therapy complement a dilatation procedure in the combined bronchoscopic treatment of benign tracheobronchial stenosis.
Ventilation Strategy and Anesthesia Management in Patients with Severe Tracheal Stenosis Undergoing Urgent Tracheal Stenting

Abstract:BackgroundStenting of airway stenosis is a common procedure in specialized centers. The aim of this study was to summarize our clinical experience in ventilation strategy and anesthesia management of patients undergoing urgent tracheal stenting. MethodsClinical data of 22 patients with severe tracheal stenosis who underwent urgent endoscopic placement of a tracheal stent during a 2-year period were retrospectively reviewed. The efficacy and safety of different ventilation strategies and veno-arterial extracorporeal membrane oxygenation (ECMO), individualized based on the cause and location of tracheal narrowing, were evaluated. ResultsSufficient ventilation was successfully established in all patients; ECMO was used in five patients with stenosis in the mid-trachea who were unable to tolerate conventional intubation; a laryngeal mask airway (LMA) was used in five patients with post-intubation tracheal stenosis; a cuffed tracheal tube was used in eight patients with lower tracheal stenosis; and low-frequency jet ventilation in rigid bronchoscopy was used in four patients with mid- or lower tracheal stenosis. Tracheal stents were successfully placed and there were significant improvements in dyspnea. There were significant increases in the partial pressure of carbon dioxide in patients ventilated with the LMA and cuffed tracheal tube. There was no hypoxia during the operative period. ConclusionEstablishment of effective airway ventilation in patients with severe tracheal stenosis should be based on the cause, location, and severity of tracheal narrowing. Veno- arterial ECMO may be considered in patients with severe stenosis, if they are judged unable to tolerate conventional ventilation or jet ventilation.

Abstract:Subglottic stenosis (SGS) is a common cause of obstructed airway in children, and the treatment of pediatric SGS, especially congenital SGS, remains a challenge for the otolaryngologist.Objective: To analyze the outcomes of endoscopic management in young children with SGS.Methods: We performed a retrospective review of treatment with endoscopic balloon dilation (EBD) or EBD combined with endoscopic anterior cricoid split (EACS) for young SGS children, from December 2008 to December 2015. The ages of patients ranged from 2 days to 12 years, median age was 5 months. The grade of them ranged from II to IV.Results: For acute acquired SGS, 19 cases received EBD alone and the other 3 cases received EBD and EACS, the success rate was about 95.5%; For chronic acquired SGS, EBD and EACS was performed in 6 patients with a success rate of 66.7%; For congenital SGS, EBD and EACS was performed in 28 patients with a success rate of 85.7%. Overall, the success rate of endoscopic management in 56 young children was about 87.5%. Besides, No procedure-related complications were observed in any patients.Conclusions: Endoscopic surgical technique offers a safe and effective approach for treatment of young children with SGS, especially in congenital SGS. Single-Stage Laryngotracheal Reconstruction in Children: A Review of 200 Cases

Abstract:OBJECTIVE: We reviewed our experience with pediatric single-stage laryngotracheal reconstruction (SSLTR) to identify factors that affect postoperative outcomes, including the need for reintubation and tracheostomy. STUDY DESIGN: Retrospective chart review was done. RESULTS: In total, 190 children underwent 200 SSLTRs; 29% were reintubated, and 15% required postoperative tracheostomy. Currently, 96% are decannulated. The use of anterior and posterior costal cartilage grafting, age less than 4 years, sedation for more than 48 hours, a leak pressure around the endotracheal tube at greater than 20 cm H 2 O, and moderate/severe tracheomalacia significantly increased the rate of reintubation. The duration of stenting did not affect outcomes. Children with anterior and posterior grafts and those with moderate or severe tracheomalacia were more likely to need a postoperative tracheostomy. CONCLUSION: SSLTR can be effective for the treatment of pediatric laryngotracheal stenosis. Diligent preoperative assessment of the patient and the patient's airway and close postoperative care are important to the success of this operation. (Otolaryngol Head Neck Surg 2000;123:430-4.)
High Tracheal Resection With Intralaryngeal Extension as an Alternative to Cricotracheal Resection for Treatment of Subglottic Stenosis

Abstract:Objective: Some patients with subglottic stenosis (SGS) require open airway reconstruction, which traditionally involves resection of the anterior cricoid cartilage. As an alternative, we present a novel technique: cricoid-sparing high tracheal resection with excision of subglottic stenotic tissue from below. A novel set of posterior circumferential cricoid sutures is used to reline the exposed cricoid plate. Study design: Retrospective chart review. Setting: Single tertiary care center. Methods: The surgical technique is described and illustrated. A chart review was performed for all patients who underwent surgery between January 1, 2016, and June 30, 2021. Results: Fourteen patients (100% female) underwent this airway resection and reconstruction surgery during the study time period. After a mean follow-up of 12 months, no patients required tracheostomy. Twelve of 14 patients (86%) had durable airways. Two patients (14%) have required repeated endoscopic procedures for recurrent stenosis and are considered treatment failures. None exhibited postoperative dysfunction to suggest posterior cricoarytenoid muscle injury. Thirteen patients (93%) had a postoperative normal voice or only transient dysphonia. One patient had permanent unilateral vocal fold paralysis. Conclusion: Cricoid-sparing high tracheal resection is a safe and effective alternative to cricotracheal resection, especially for predominantly posterior SGS. The cricoid cartilage and cricothyroid muscles are left undisturbed, potentially decreasing the risk of postoperative dysphonia, namely lowered fundamental frequency. This is especially meaningful in the setting of a predominantly female patient population.

Abstract:A case of "milary" Crohn's disease in a 53-year-old woman is described. There were numerous epithlioid-cell, non-caseous serosal granulomas in the jejunal and terminal ileal regions. There were, furthermore, immediately adjoining the epithelioid-cell granulomas, granuloma-like collections of brown fat tissue, a previously undescribed coexistence.

Abstract:Congenital tracheal stenosis (CTS) is a rare life‐threatening condition. Comorbidities such as various cardiac defects, prematurity, and genetic disorders would complicate the treatment options. Early surgical treatment with a cardiopulmonary bypass machine provides favorable outcomes in large, experienced centers. Objectives To investigate demographic data and airway management techniques for patients with congenital tracheal stenosis (CTS) during the preoperative, intraoperative, and postoperative periods. Study Design A retrospective chart review. Methods This study was a retrospective case series at a single tertiary care pediatric medical center. It encompassed all children diagnosed with CTS who underwent slide tracheoplasty from January 2001 through December 2018. Exclusion criteria were acquired stenosis, tracheomalacia, patients without a confirmed diagnosis of CTS, and those with missing data. Patient demographics and details of airway management were collected and analyzed. Results A total of 148 patients met the inclusion criteria. The most common etiology of CTS was congenital tracheal rings (90.5%). The median age at surgery was 9 months (interquartile range [IQR] 3.3–35.4), and the median stenosis length was 4.0 cm (IQR 3.5–5.4). The most frequent comorbidities were genetic diseases, prematurity, and preoperative intubation. The most common related cardiac comorbidities were pulmonary artery sling, patent ductus arteriosus, atrial septal defect, ventricular septal defect, and tetralogy of Fallot. Most of the patients with CTS exhibited 50% to 80% stenosis. Most of the patients underwent thoracic approach slide tracheoplasty. Airway management was primarily accomplished using an oral or nasal endotracheal tube preoperatively, cardiopulmonary bypass during surgical repair, and an oral or nasal endotracheal tube during closure and the postoperative period. Conclusions Slide tracheoplasty is an effective treatment for congenital tracheal stenosis. Coordinated airway management between the anesthesia and surgical teams is crucial. Diagnosis and Treatment of Airway Stenosis after Lung Transplantation

Abstract:Objective To analyze the diagnosis and treatment of airway stenosis in a consecutive series of bronchial anastomosis after lung transplantation in our center.Methods We performed a retrospective study on 100 cases of lung transplants in our center from September 2002 to December 2010.Seventy-two cases were subjected to single lung transplants (SLT), and twenty-eight to bilateral sequential single lung transplantation (BSSLT). There were totally 128 bronchial anastomoses.All recipients received long-term follow-up to monitor the lung function.Lung CT and fibrobronchoscopic examinations were done when necessary. Results Twenty-five cases with 37 bronchial anatomoses were died.A total of 12 airway stenosis occurred in 10 cases (12/128,9.4 ％).Four cases underwent telescopic anastomosis and 6 cases underwent end-to-end anastamosis.Mean diagnosis time was 60.1 35.6 days post-operation (ranging from 15-120 days,median 59 days).There were 8 cases of unilateral airway stenosis (3 on the left,and 5 on the right) and 2 cases of bilateral airway stenosis.The number of simple airway stenosis was 3,that of exophytie granulation tissue was 8,and that of bronchus intermedius stenosis was 1.Culture of bacteria by fibrobronchoscopy with protected specimen brush revealed:3 strains of Pseudomonas aeruginosa,2 strains of Klebsiella pneumoniae,2 strains of Aspergillus,1 strain of Escherichia Coli. 10 cases were treated with fiberoptic bronchoscopic balloon dilation:5 cases with high-frequency electrotome,4 cases with stent placement,and 1 case with argon plasma coagulation (APC).Seven cases were cured or improved and 3 cases died.Conclusion Airway stenosis after lung transplantation remains a major problem.The fiberoptic bronchoscopic procedure is the gold standard to diagnose. The preferred treatment is fiberoptic bronchoscopic balloon under expansion,and other approaches include high-frequency electrotome,APC and stem placement,etc.

Abstract:In adults, the glottis is the narrowest part of the larynx, whereas, in newborns, it is the subglottic region. The subglottic region has a diameter of around 4–5 mm in infants and 3 mm in premature infants. It is crucial to acknowledge that even a 1 mm circumference swelling or edema in the subglottic area of an infant can lead to a substantial 60% obstruction of the airway. Therefore, the treatment of airway issues in pediatric patients may require more immediate and complex interventions compared to adults (Marston AP et al., Clin Perinatol. 2018;45:787–804). Airway stenosis can be congenital or acquired (intubation for a prolonged period, infection, tumors, etc.) and most commonly occurs within the subglottis (Marston AP et al., Clin Perinatol. 2018;45:787–804). About 5% of subglottic stenosis cases are congenital, making it the third most common cause of congenital airway obstruction. Congenital subglottic stenosis is usually milder than the acquired type and tends to improve as the child grows. The Myer-Cotton rating scale is the most commonly used method for diagnosing subglottic stenosis. It classifies the degree of stenosis based on visual inspection during laryngoscopy. It consists of four grades: Grade I - 0 to 50% decrease in lumen surface; Grade II - 51 to 70% decrease; Grade III - 71 to 99% decrease, and Grade IV - no evidence of a detectable lumen. However, patients with advanced stage (3–4) according to the Myer-Cotton classification of subglottic stenosis may require emergency intervention (Marston AP et al., Subglottic Stenosis. Clin Perinatol. 2018 Dec;45:787–804). While the ultimate therapeutic goal is to provide a stable and safe airway with endoscopic or open surgical methods, the need for tracheotomy may occur in emergencies where surgery is not feasible. Balloon laryngoplasty is a highly effective, low-risk alternative or adjunct to traditional reconstructive procedures in children with subglottic or laryngeal stenosis (Wentzel JL et al., Laryngoscope. 2014;124:1707–12, Wenzel AM et al., Eur Arch Otorhinolaryngol, 2018;275:2325–2331) We present the management of a 26-month-old patient with stage 3 subglottic stenosis who has developed pectus excavatum and growth retardation due to inspiratory stridor and respiratory distress since birth. This case serves as an educational example for clinicians, as it demonstrates the successful management of a patient despite a late diagnosis and challenging airway.
