Abstract:Since the first report of totally endoscopic coronary artery bypass graft (CABG) using da Vinci Surgical System (Intuitive Surgical, Sunnyvale, CA, USA) in 1999, the robotic technology has been applied to internal thoracic artery (ITA) harvesting for more than 10 years. We present the experience of robotic ITA skeletonized harvesting in 200 cases using the da Vinci S or da Vinci SI Surgical System. The learning curves of ITA harvesting time and the results of ITA graft patency were evaluated. METHODS Patients Between January 2007 and August 2012, a total of 200 patients underwent totally robotic ITA takedown, including 154 males and 46 females aged 33-79 years (mean 58 years). The procedures of robotic-assisted CABG on beating heart were completed in two ways:1, 2 to harvest the ITA, followed by a minimally invasive direct coronary artery bypass graft (MIDCAB); and to harvest the ITA, followed by totally endoscopic coronary artery bypass graft (BH-TECAB). The performance of robotic ITA harvesting, MIDCAB and TECAB was approved by the Institutional Review Board of People's Liberation Army General Hospital and informed consent was obtained. Surgical methods After routine induction of anesthesia, double-lumen intubation was carried out for single right lung ventilation. Three 0.8 to 1.0-cm incisions were made in the 3rd, 5th and 7th intercostal spaces that located about 3 cm lateral of the midclavicular line. After deflation of the left lung, carbon dioxide insufflation was initialed through the cameral port and maintained at an average pressure of 6 to 8 mmHg (1 mmHg=0.133 kPa). The surgical cart of robotic surgical system docked to the camera and instrument ports. A 30-degree angle upward camera and micro-instruments were inserted. First, the fascia and muscles covering the ITA were transected along the entire length. The skeletonized ITA was developed from the lateral edge medially using blunt dissection and short bursts of low power monopolar cautery to mobilize the anterior attachments (Figure 1). Small intercostal branches were ligated with monopolar energy and the large intercostal branches were clipped for hemostasis. The entire length of ITA was dissected from the first intercostal branch to the diaphragm. The right ITA can be harvested after dissecting the anterior mediastinum. The left ITA was harvested followed the right ITA harvesting in double ITAs harvesting. The ITA harvesting time was defined from completed port placement to completed ITA detachment from thoracic wall. For calculation of the learning curve, regression analysis with logarithmic curve fit was used.Figure 1.: The robotic skeletonized ITA was harvested using monopolar cautery.RESULTS All the ITAs were harvested successfully in a skeletonized fashion. There were no major technical failures of the robotic system. One hundred and ninety-one left ITAs, 5 right ITAs and 4 double-ITAs were harvested. After robotic ITA harvesting, 120 patients underwent MIDCAB and 80 patients underwent BH-TECAB. The mean time of single ITA harvesting was (35.8±10.7) minutes (18 to 70). A significant learning curve was observed: y(min) = 58.0-5.3×ln(x), x = ITA takedown number; P<0.01, r2 = 0.33 (Figure 2). Harvesting time decreased from 41 minutes (median) in the first 30 cases to 29 minutes (median) in the last 30 cases.Figure 2.: The learning curve for robotic ITA harvesting. Followed the formula: y (min) = 58.0-5.3×ln (x).One hundred and ninety-five patients (97.5%) accepted the follow-up of graft patency before discharging, including 130 cases of CT angiography (CTA) scan and 65 patients coronary angiography. Both coronary angiography and CTA scan showed 100.0% graft patency. Unexpectedly, the angiography showed that left ITA graft in the middle segment developed a collateral branch in 2 patients. However, the patients were asymptomatic. One left ITA graft stenosis in TECAB group at 1year, and one in MIDCAB group at 2 years were found. The graft patency decreased as time went by. The ITA patency was 98.1% at 1 year, 97.8% at 2 years, respectively. And no more ITA occlusion was found at 3 to 5 years. DISCUSSION The superiority of ITA as a conduit has been well established for long-term patency and event-free survival. With the development of robotic cardiac surgery, the heart surgeon has had the possibility to perform ITA takedown totally endoscopically. Robotic harvesting of ITA is the first step toward a totally endoscopic coronary bypass graft. We have harvested the ITAs in 200 cases using da Vinci S or da Vinci SI surgical system including right ITA and bilateral ITAs. The skeletonized fashion was used in this group that can provide increased graft length as well as a larger anastomotic diameter compared with pedicled fashion. In our experiences, the skeletonized ITA harvesting using da Vinci System was feasible and safe, but more time consuming and demanding than traditional technique. With robotic assistance, the harvesting for bilateral ITAs was easier than conventional method. Robotic instrument allows the entire lengths of the bilateral ITAs to be harvested in a skeletonized fashion, resulting in greater conduit lengths and enabling multi-vessel grafting.3 After passing through a learning curve, ITAs takedown can be done in an acceptable time. In this study, the harvesting time was stable after about 30 cases and decreased with the cases number increasing. Long preparation times at the beginning may be explained by the complexity of this new system and the lack of clinical experience of the operation team.4 The harvesting time can reduce to an acceptable duration between 30 to 40 minutes after the learning curve. And there are several reports that the early clinical results were excellent. But the longer-term graft patency and clinical freedom from graft failure have remained a major concern. In our group, 97.5% patients underwent coronary angiography or CTA scan before discharge and the remaining patients (2.5%) refused to undergo follow up. After discharge, all patients were followed up by CTA scan, which is easily tolerated and accepted by the patients although conventional coronary angiography remains the gold standard for evaluation of graft patency.5 Our graft patency is comparable to the results for conventional CABG, and off-pump CABG (OPCAB) surgery. Limitations are as follows: This was a single center, single surgeon study of a highly selected group of patients. Other risk factors leading to ITA graft occlusion were not included in the current study. In conclusion, our study shows that robotic-enhanced ITA takedown is prerequisite for TECAB or MIDCAB operations and can be safely implemented. After passing through a learning curve, ITA takedown can be done in an acceptable time. It should be conducted by surgeons with extensive surgical experience in open techniques, and requires a stable and well-trained robotic team and overcoming a substantial learning curve.

Abstract:Introduction: Bilateral internal thoracic artery(ITA) grafting is associated with improved survival. However, many surgeons are reluctant to use this revascularization technique in patients after myocardial infarction (MI) due to the risk of sternal infection and the excellent survival benefit obtained with single ITA (SITA). Hypothesis: Long-term outcome of bilateral ITA (BITA) grafting, might be superior to single ITA and other conduits such as saphenous veins (SVG) and radial artery (RA) in patients with multi-vessels disease after recent MI. Methods: 871 patients who underwent BITA grafting after recent MI (<3 months prior to surgery), between 1996 and 2010, Were compared with 527 who underwent CABG with SITA and SVG or RA. Results: Patients undergoing SITA were older, more often female, more likely to have COPD, EF<30%, preoperative critical state, Diabetes, chronic renal failure (CRF), peripheral vascular disease (PVD) and emergency operation. In contrast, patients undergoing BITA grafting were more likely to have triple vessel disease and more often underwent CABG with three or more grafts and sequential grafts. Operative mortality of BITA patients was lower (3.0 % vs. 5.7%, in the SITA group p=0.01), Occurrences of sternal wound infection (3% vs 2%) and strokes (2.6% vs. 3%) were similar. Mean follow-up was 11.8 (95%CI 10.95-12.56) years. Kaplan Meier survival of BITA patients was better (70.3% vs.52.6% p<0.001). Propensity score matching was used to account for differences between groups in preoperative characteristics. The 293 matched pairs thus created had similar preoperative characteristics. Kaplan Meier 10 year survival (67.3% versus 59%, p=0.325 log-rank test), and the Cox adjusted survival of the matched groups did not reach statistical significance (HR 1.57, p=0.091). However, survival of non- emergent BITA patients was better (HR 1.256 p=0.038). Age, COPD, CHF, CRF, PVD and left main were independent predictors of decreased survival. Off-pump operations were associated with improved survival (Cox model) Conclusions: This study suggests that long-term outcome of arterial revascularization with BITA can be better than that of SITA in non-emergency cases after recent MI with proper patient selection

Abstract:A coherence-based, or interferometric approach to spectral analysis of charge-exchange recombination (CXR) emission radiation from high-temperature plasma probed or heated using energetic neutral beams, offers a number of advantages over wavelength-domain instruments. The spectral-line shift and broadening are obtained from measurements of the spectral coherence at a given fixed time delay. The coherence is monitored by first approximately isolating the spectral line of interest using an interference filter and subsequently imaging the spectral scene using a field-widened electro-optic path-delay-modulated polarization interferometer. Robot-Assisted MIDCAB Using Bilateral Internal Thoracic Artery: A Propensity Score-Matched Study With OPCAB Patients

Abstract:Objective: Robot-assisted minimally invasive direct coronary artery bypass (RA-MIDCAB) is an attractive strategy for coronary revascularization. Growing evidence supports the use of total arterial grafting in coronary surgery. We evaluated total arterial left-sided coronary revascularization with bilateral internal thoracic artery (BITA) using RA-MIDCAB and compared it with a propensity score-matched (PSM) off-pump CAB (OPCAB) surgery population. Methods: We retrospectively included all isolated OPCAB and RA-MIDCAB surgery using BITA without saphenous vein graft from January 1, 2015, to October 31, 2022. We analyzed all our RA-MIDCAB patients and performed PSM to compare them with our OPCAB population. Primary outcomes were major adverse cardiovascular and cerebrovascular events (MACCE) and mortality. Secondary outcomes were surgical parameters, length of hospital stay, and learning curve. Results: We included 601 OPCAB and 77 RA-MIDCAB procedures, which resulted in 2 cohorts of 54 patients after PSM. Mortality and MACCE survival analysis showed no significant difference. There was less blood transfusion in the RA-MIDCAB (16.7%) compared with the OPCAB group (38.9%; P = 0.02). We observed fewer intensive care unit (ICU) admissions (24.1% vs 96.6%), shorter ICU stay (0.78 ± 1.7 vs 1.91 ± 1.01 days), and shorter hospital stay (6.78 ± 2.4 vs 8.01 ± 2.5 days) in the RA-MIDCAB versus OPCAB group (P < 0.01). Surgery time decreased from 400.0 ± 70.8 to 325.0 ± 38.0 min with more experience in RA-MIDCAB BITA harvesting (P < 0.01). Conclusions: This is a first publication of 77 consecutive RA-MIDCAB BITA harvesting for left coronary artery system revascularization. This technique is safe in terms of MACCE and mortality. Additional advantages are shorter length of hospital stay, fewer ICU admissions, and less blood transfusion.

Abstract:Highlights Bilateral internal thoracic artery (BITA) grafting is one of the most effective methods of direct myocardial revascularization. However, its results are still contradictory, despite the worldwide trend to use more autogenous arterial conduits. This study involves more than fifteen years of data in the use of BITA in Kuzbass in coronary artery disease surgery. The findings presented in the course of the analysis confirm the need to use both internal mammary arteries more actively. Aim. To analyze long-term results of BITA grafting in terms of more than 15 years follow-up. Methods. The study included 232 patients who were admitted at the Cardiac Surgery Department for coronary artery bypass grafting (CABG). All patients were divided into 2 groups depending on usage of single or bilateral internal thoracic artery conduits. Both groups included an equal number of patients 116 (50%). Long-term results were assessed in terms of more than 15 years follow-up. Results. BITA grafting is comparable with CABG in such major adverse cardiac and cerebrovascular events as repeated myocardial revascularization, stroke, and in combined endpoint such as myocardial infarction (MI), stroke, repeated myocardial revascularization, or patient’s death within 15 years. However, the BITA group is superior to the CABG group in freedom from MI, all-cause deaths, showing a higher life expectancy after surgery (p = 0.011), and improved left ventricular ejection fraction and decreased functional class of angina. Moreover, in the present research it was confirmed that the severity of atherosclerosis in the coronary arteries is lower after bypassing with autogenous arterial conduits. Conclusion. BITA is safe procedure comparable with CABG, however, it has more advantageous long-term clinical and angiographic results.
Off-pump bilateral internal thoracic artery grafting in patients with left main coronary artery disease

Abstract:Abstract Background To compare postoperative outcomes in patients with left main coronary artery disease who underwent off-pump isolated coronary artery bypass grafting for multivessel disease using either skeletonized bilateral or single internal thoracic artery (ITA). Methods Among 1583 patients who underwent isolated coronary artery bypass grafting (CABG) in our hospital between 2002 and 2022, 604 patients with left main coronary artery disease underwent single ( n = 169) or bilateral ( n = 435) ITA grafting. We compared postoperative outcomes between the two groups after adjusting preoperative characteristics using inverse probability of treatment weighting. Results After adjustment using inverse probability of treatment weighting method, the sum of weights was 599.74 in BITA group and 621.64 in SITA group. There was no significant difference in postoperative deep sternal wound infection ( p = 0.227) and 30-day mortality ( p = 0.612). Follow-up was completed in 98.7% (596/604) of the patients, and the mean follow-up duration was 6.7 years. At 10 years, the overall survival following bilateral versus single ITA grafting was 71.2% and 60.6%, respectively (log-rank test, p = 0.040), and freedom from major adverse cardiac and cerebrovascular events (MACCE) was 63.3% and 46.3%, respectively (log-rank test, p = 0.008). In multivariate Cox proportional hazard models, bilateral ITA grafting was significantly associated with a lower risk of all-cause death (hazard ratio [HR]: 0.706, 95% confidence interval [CI]: 0.504–0.987; p = 0.042) and MACCE (HR: 0.671, 95% CI: 0.499–0.902; p = 0.008). Conclusions Bilateral skeletonized ITA grafting is associated with lower rates of all-cause death and MACCE than single ITA grafting in patients with left main coronary artery disease undergoing off-pump CABG.

Abstract:Perfusion, Ahead of Print. BackgroundImproved long term survival has demonstrated by grafting left internal thoracic artery (LITA) to the left anterior descending artery (LAD). This study investigated the prognostic benefits of BITA versus single LITA with common anastomotic configurations of BITA.MethodsElective patients age below 60 years, undertaken by a single surgeon, between 1992 and 2014 were explored. Cox regression models were fitted to investigate prognostic benefits of BITA. Specific comparisons were made with BITA subgroups who received LITA to LAD or RITA to LAD to establish long-term prognostic difference with the use of the RITA to LAD as compared to the LITA to LAD anastomosis. Comparisons were made with other BITA configurations to establish long-term difference in survival as a result of the actual targets bypassed with BITA grafts.ResultsBoth groups had similar baseline patient characteristics. Following risk adjustment, the use of a second ITA conferred a significant 48% reduction in long-term mortality to BITA group [HR 0.52 (95%CI: 0.36–0.74) p < .001]. The use of the RITA to LAD, instead of LITA, resulted in no difference in the long-term prognostic benefit. The targets grafted with second ITA conferred a significant prognostic benefit for common configurations it was used in for bypass surgery.ConclusionsThe use of BITA confers long term prognostic benefit to patients. Grafting RITA to LAD instead of more common LITA to LAD made no difference in long term prognosis when compared to CABG patients with single ITA. The use of a second ITA conduit in common configurations for CABG confers prognostic benefits irrespective of distal target grafted.

Robotic Beating-Heart Totally Endoscopic Coronary Artery Bypass: Impact of Chest Wall Dimensions in Single and Multivessel Bypass

Abstract:Objective: There can be anatomical constraints on patient selection for minimally invasive surgery. For example, robot-assisted coronary artery bypass was reported to be more challenging when patients had a cardiothoracic ratio >50% and a sternum-vertebra anteroposterior and transverse diameter ratio <0.45. We sought to examine the impact of chest wall anatomic parameters on surgical outcomes in our totally endoscopic coronary artery bypass (TECAB) procedures. Methods: We retrospectively reviewed patients who underwent robotic TECAB, all of whom had a preoperative chest radiograph at our institution from July 2017 to October 2021. The cohort was divided into 2 groups, which were patients undergoing single-vessel grafting using the left internal thoracic artery (ITA; group 1) and patients undergoing multivessel grafting with bilateral ITA grafts (group 2). We measured several anatomical parameters from the preoperative chest radiograph. Results: A total of 352 patients undergoing TECAB were retrospectively analyzed. After exclusions, 193 were included in this study. In group 1 (n = 91), no parameters correlated with operative time. In group 2 (n = 102), a significant negative correlation was observed between operative time and the sternum-vertebrae anteroposterior diameter (rs = -0.228, P = 0.022) and lung anteroposterior diameter (rs = -0.246, P = 0.013). To confirm these results in group 2, a propensity-matched analysis was performed and showed a statistically significant difference in surgical time based on chest anteroposterior diameters. Conclusions: In single-vessel robotic TECAB, chest wall anatomic dimensions measured on chest radiograph did not affect operative time. In multivessel cases with bilateral ITA grafts, larger anteroposterior diameter correlated with shorter operative times.
[Surgical Strategies and Long-Term Outcomes of Total Arterial Coronary Artery Bypass Grafting: a Series of 208 Patients].

Abstract:Objective: To examine the short and long-term clinical outcomes of total arterial coronary artery bypass grafting. Methods: Clinic data of 208 patients with left main and multiple vessel coronary artery disease and undertaken total arterial coronary artery bypass grafting from February 2009 to December 2019 in Department of Cardiac Surgery, Rui Jin Hospital, Shanghai Jiao Tong University School of Medicine were analyzed retrospectively. There were 188 males and 20 females with an age of (54.7±10.7) years (range: 32 to 79 years). The harvest of arterial conduits and grafting strategies were depended upon the individual patient characteristics and surgeon's experience. Left internal thoracic artery (LITA) was applied in 207 cases, right internal thoracic artery (RITA) in 38 cases (bilateral internal thoracic artery (BITA) in 37 cases), and radial artery (RA) in 187 cases (188 grafts). The graft number per case was 2.6±0.7 (range: 2 to 4). Surgical procedures was completed with off-pump technique in 98.1% patients (204/208). Subgroup analysis was carried out between subgroup BITA (n=37) and subgroup SITA (single ITA+RA) (n=171). The t test, χ(2) test or Fisher exact test were used to compare the clinic characteristics between the two subgroups. The Kaplan-Meier curve was used to estimate the rate of late mortality, major adverse cardiac cerebrovascular event (MACCE), and target vessel revascularization (TVR). A Cox proportional hazards model was used to identify the independent prognosis factors of late mortality. Results: The overall mortality within 30 days postoperatively was 1.4%(3/208). The incidences of perioperative MACCE, re-operation for bleeding and deep sternal wound infection (DSWI) were 1.9%(4/208), 0.5%(1/208) and 1.4%(3/208), respectively. Perioperative myocardial infarction and TVR were not observed. There was no significant difference of 30-day mortality, MACCE, bleeding and DSWI between subgroup BITA and SITA+RA (all P>0.05). In a follow-up period of (5.4±2.8)years (range: 0.2 to 10.9 years), the incidence of all-cause mortality at 1-, 5- and 10-year was 2.3%, 3.4% and 6.9%, respectively. The incidence of MACCE was 3.9%,11.2% and 28.5%, respectively. The rate of TVR was 0.4%, 3.7% and 11.9%, respectively. Age>65 was an independent prognosis factor of late mortality (HR=1.125, 95% CI:1.050 to 1.205, P<0.01). Conclusions: Total arterial coronary bypass grafting is safe and achievable with proper patient selection and surgical strategies. It significantly decreases the risks of late mortality and repeated revascularization.

Abstract:BACKGROUND: The efficacy of diagonal coronary artery stenting in patients undergoing robotic left internal thoracic artery-to-left anterior descending (LITA-to-LAD) anastomosis is not well defined. The objective of this study was to assess graft and stent patency in a single-stage hybrid revascularization with LITA-to-LAD anastomosis and PCI to a diagonal coronary artery.METHODS: From 2004 to 2014, a total of 25 patients consented to robotic-assisted LITA harvesting and a small left anterior thoracotomy for off-pump coronary artery bypass anastomosis onto the LAD along with concomitant PCI to the diagonal coronary artery. PCI to the diagonal coronary artery was performed in the same fluoroscopy-equipped hybrid operating room.RESULTS: Patients were on average 66 ± 11 years with 32% female. Pre-operative characteristics of these patients included 8% with a grade 3 or 4 left ventricle, 16% with a recent MI, and 92% with CCS III/IV symptoms. There were no death, one patient required an intra-aortic balloon pump, and one patient required re-operation for bleeding. The average ICU stay was 1.1 ± 0.53 days, and the average hospital stay was 4.6 ± 2.4 days. Fitzgibbon Grade A LITA-to-LAD patency at 6-month follow-up was 100%. As well, at 6-month follow-up the DES to the diagonal coronary artery had a patency rate of 96%.CONCLUSIONS: Single-stage hybrid revascularization strategy for bifurcating lesions of the LAD and diagonal coronary arteries with LITA-to-LAD anastomosis and PCI to a diagonal coronary artery appears to have acceptable clinical results with excellent 6-month angiographic patency results.

Abstract:Objective The optimal strategy for surgical revascularization in patients with impaired renal function is inconclusive. We compared early and late outcomes between bilateral internal thoracic artery (BITA) and single ITA (SITA) grafting in patients with renal dysfunction. Methods This is a retrospective analysis of all the patients with multivessel disease and impaired renal function (estimated glomerular filtration rate <60mL/min/1.73m 2 ) who underwent isolated coronary artery bypass graft (CABG) in our center during 1996–2011, utilizing either BITA or SITA revascularization. Results Of the 5301 patients with multivessel disease who underwent surgical revascularization during the study period, 391 were with impaired renal function: 212 (54.2%) underwent BITA, 179 (45.8%) underwent SITA. Patients who underwent BITA were less likely to have comorbidities. Statistically significant differences were not observed between the BITA and SITA groups in 30-day mortality (5.6% vs. 9.0%, p = 0.2) and in rates of early stroke, myocardial infarction, and sternal infection (4.5% vs. 6.1%, p = 0.467; 1.7% vs. 2.8%, p = 0.517; and 2.2% vs. 5.7%, p = 0.088, respectively). Long-term survival of the BITA group was better: median 8.36 vs. 4.14 years, p<0.001. In multivariable analysis, BITA revascularization was associated with decreased late mortality (HR = 0.704, 95% CI: 0.556–0.89, p = 0.003). In analysis of a matched cohort (134 pairs), early outcomes did not differ between the groups; however, in multivariable analysis, BITA revascularization was associated with decreased late mortality ‎‎(HR = 0.35 (95%CI 0.18–0.68), p = 0.002)‎. Conclusions BITA revascularization did not impact early outcome in patients with CRF, but demonstrated a significant protective effect on long-term survival ‎in the unmatched and matched cohorts.

Abstract:Objectives Long-term survival after coronary artery bypass grafting (CABG) appears superior with bilateral internal thoracic artery (BITA) grafting over grafting a single internal thoracic artery. BITA grafting is recommended in CABG without excessive risk of infection. However, there is concern about an increased risk of sternal wound infection when BITAs are harvested. A recent report demonstrated that sternal closure with rigid plate fixation provided better sternal healing and fewer complications. Thus, a retrospective study was performed to investigate whether BITA grafting can be safely performed in CABG with rigid plate fixation for patients at high risk of sternal wound infection. Methods From June 2017 to December 2018, a total of 31 patients who were deemed preoperatively to be at high risk of sternal wound infection underwent isolated CABG ± pulmonary vein isolation. BITAs were skeletonized, and the chest was closed with rigid plate fixation in all cases. The surgical results were retrospectively reviewed, and the incidence of sternal wound complications was evaluated. Results Each patient had at least 2 risk factors, and the average number of risk factors per patient was 3.1. A total of 14 patients (45%) had obesity, with mean BMI of 31 ± 8.1 kg/m , and 27 patients (86%) had diabetes mellitus, with mean HbA1c of 7.6 ± 1.5%. There was one patient with superficial sternal wound infection (3%) and one with wound dehiscence (3%); they were medically treated at the outpatient clinic without readmission. There was no incidence of deep sternal wound infection. Conclusions BITA grafting could be performed in high-risk patients without increasing sternal wound complications. Rigid sternal plating fixation may increase the feasibility of BITA grafting in high-risk patients.
Robotic Totally Endoscopic Off-Pump Unroofing of Myocardial Bridge: Early Experience and Midterm Outcomes

Abstract:Objective: Myocardial bridging (MB) occurs when a coronary artery, commonly the left anterior descending (LAD), has an intramyocardial course. In symptomatic patients who fail medical therapy, surgical unroofing can provide symptomatic relief by improving coronary blood flow. We present a series of patients undergoing robotic totally endoscopic beating-heart MB unroofing. Methods: There were 34 patients with an LAD-MB who failed medical therapy and underwent robotic totally endoscopic, off-pump unroofing between January 2017 and October 2023. Patients were evaluated by a multidisciplinary team and underwent provocative coronary angiography to confirm hemodynamic significance. We reviewed perioperative outcomes and contacted patients for midterm follow-up, including completion of a modified Seattle Angina Questionnaire (SAQ). Results: The mean age was 48 ± 8 years, and 56% were female patients. One patient had prior septal myectomy via sternotomy. All patients had significant dobutamine Pd/Pa reduction on preoperative coronary angiography. One patient had atrial fibrillation and underwent concomitant ablation with left atrial appendage ligation. The mean procedure time was 140 ± 69 min. All were completed totally endoscopically off-pump without intraoperative conversions. The mean MB length was 4.5 ± 1.4 cm, and the mean depth was 1.6 ± 0.9 cm. Of the patients, 76% were extubated in the operating room. The mean intensive care unit and hospital length of stay were 0.97 ± 0.58 and 1.73 ± 1.1 days, respectively. There were no mortalities or strokes. There was 1 postoperative take-back for bleeding. At midterm follow-up (19 ± 14 months), 28 patients completed the SAQ; 86% reported "much less angina" during activity compared with before surgery, and 93% reported taking no antianginal medication since surgery. Conclusions: In appropriate patients with hemodynamically significant LAD-MB who fail medical therapy, robotic beating-heart unroofing is possible with good outcomes. Further studies are warranted.

Abstract:BACKGROUND Outcomes of robotic-assisted reverse hybrid coronary revascularization (HCR) remain hindered. We aim to analyze mid-term clinical outcomes of robotic-assisted reverse HCR. METHODS All consecutive 285 patients that underwent reverse robotic-assisted HCR between 09/2005-07/2021 are included. Reverse HCR consists of percutaneous coronary intervention (PCI) with stent implantation in non-left anterior descending (LAD) coronary artery vessels performed within 30-days prior to robotic-assisted left internal thoracic artery (LITA) harvesting and LITA to LAD manual anastomosis through a 4-cm left minithoracotomy. Dual antiplatelet therapy was not interrupted on any patient. RESULTS Preoperatively, mean age was 70.2 (± 11.2) years old. Prior to surgery, a total of 168 patients received 1 stent, 112 patients 2 stents, and 5 patients 3 stents. Intraoperatively, mean OR time was 5.9 (± 1) hours, no case was converted to full sternotomy, while 9 (3.1%) patients received intraoperative blood products transfusions. Postoperatively, a low incidence of stroke 1 (0.3%), reoperation for bleeding 7 (2.4%), blood product transfusions 48 (16.8%), and hospital stay (4.8 days) was observed. At 30-day follow-up, 1 (0.3%) patient underwent PCI with stent on a surgical LITA-LAD anastomosis due to graft failure. Mean follow-up was 4.2-years. Reported mid-term outcomes included all-cause death in 31 patients (10.9%), MACCE in 102/285 (35.9%), nonfatal stroke in 2/285 (0.7%), MI in 17/285 (5.9%), and repeat intervention in 50/285 (17.5%) patients. CONCLUSION This single center study reports effective and safe clinical outcomes at mid-term follow-up of reverse HCR procedures for treatment of multivessel coronary artery disease.

Abstract:BackgroundReoperative cardiac surgery in patients with patent bilateral internal thoracic arteries (ITA) grafts is technically challenging.MethodsFrom 2008–2017, of 7,640 patients undergoing reoperative cardiac surgery, 116 (1.5%) had patent bilateral ITA grafts, including 28 with a right ITA crossing the midline. Mean age was 70±9.6 years, and 111 patients (96%) were male. Reoperations included isolated coronary artery bypass grafting (CABG; n=11), isolated valve (n=55), valve+CABG (n=26), and other procedures (n=24). Clinical details, intraoperative management, and perioperative outcomes were analyzed.ResultsAortic cannulation was central in 64 patients (56%) and via femoral or axillary artery in 50 (44%). Four patients (3.4%) had planned transection and reattachment of ITAs crossing the midline, and 4 (3.4%) had ITA injuries, all right ITAs, 3 crossing the midline; 3 were repaired with an interposition vein graft, and 1 was managed by translocating the right ITA as a Y-graft off another graft. Patent ITAs were managed by atraumatic occlusion during aortic clamping in 90 patients (78%) and by systemic cooling without ITA occlusion in 19. There were 6 operative deaths, all due to low cardiac output syndrome (5.2%), 4 strokes (3.4%), and 5 cases of new postoperative dialysis (4.3%).ConclusionsRisk of injury to bilateral ITA grafts during reoperation is high, and right ITAs crossing the midline present a particular risk of injury and should inform planning for primary CABG. Risk of low cardiac output syndrome underscores the challenge of ensuring adequate myocardial protection.

Abstract:Objectives: To explore the feasibility and safety of "2-staged" hybrid coronary revascularization using bilateral internal thoracic artery (BITA) for the treatment of multivessel coronary artery disease. Methods: Data of 65 patients who underwent "2-staged" hybrid technique (HCR) using BITA (BITA-HCR group) in our heart center because of multivessel coronary artery lesions during 2014.05-2017.05 were retrospectively analyzed. Results were compared with 96 patients who underwent "2-staged" HCR with single ITA (SITA-HCR group) by the same surgeon over the same time period. Results: There was no significant difference of preoperative characteristics between two groups. Operation time was significantly longer [(204.6±28.7) min vs (147.9±31.6) min, P<0.05], number of distal anastomoses was significantly higher [(2.2±0.5) vs (1.0±0.0), P<0.05], number of stents was significantly lower [(1.90±0.67) vs (2.40±0.49), P<0.05] in BITA-HCR group than in SITA-HCR group. Bleeding volume [(520.1±120.3) ml vs (532.2±350.3)ml, P>0.05], mechanical ventilation time [(7.7±3.2) h vs (6.9±2.3) h, P>0.05] and blood transfusion required [5(7.7%)vs 8(8.3%), P>0.05] were similar between the two groups. Conclusions: The "2-staged" hybrid procedure using BITA is safe and feasible for the revascularization of multiple coronary artery lesions.

Abstract:OBJECTIVE:To summary the first 14 cases undergoing internal mammary artery (IMA) harvest using da Vinci S system and minimally invasive direct coronary artery bypass grafting (MIDCAB) on beating heart.METHODS:The average age of patients was (60.4 +/- 10.1) years old. One case was female and 13 male. All the patients had a history of angina. The coronary angiography showed severe stenosis of anterior descending branch in all patients, of which 2 cases had diagonal and circumflex branch stenosis. Four case had myocardial infraction history. All the patients had good lung function and had no medical history of pleurisy. Without sternotomy, the camera cannula was placed in the left, 3 cm lateral to nipple in the 4th intercostal space (ICS). Da Vinci instrument arms were inserted through two 1 cm trocar incisions. The right instrument generally was positioned 4 to 6 cm cephalad to camera cannula in the 2nd or 3rd ICS. The left instrument arm was positioned 4 to 6 cm caudal to the camera cannula in the 5th or 6th ICS. Arm trocar sites were maintained 6 cm apart at chest entry. The internal mammary artery was harvested in routine methods. Thirteen cases underwent left internal mammary artery harvest, one case underwent right internal mammary artery harvest, one case underwent double internal mammary harvest. MIDCAB was performed on beating heart in 14 cases and 1 case accepted the totally endoscopic coronary artery bypass (TECAB).RESULTS:All cases were accomplished successfully without complications. The average time of ICU was 20 hours. Robotic surgery had less draining than the conventional coronary bypass.CONCLUSIONS:Totally robotic internal mammary artery harvest and beating heart coronary artery bypass is less invasive, more precise, safe and efficient.
