Abstract:Background: Previous studies have demonstrated that Tc-99m-sestamibi (Tc-99m- MIBI) Single-Photon Emission Computed Tomography/Computed Tomography (SPECT/CT) imaging is an effective isotopic technique for locating the parathyroid in secondary hyperparathyroidism (SHPT). This study aimed to explore further the correlation between Tc-99m-MIBI SPECT/CT imaging and SHPT to demonstrate the value of Tc-99m-MIBI SPECT/CT in evaluating the degree of pathological hyperplasia of the parathyroid gland (PG). Methods: The demographics, surgical records, and follow-up information of 91 patients were recorded and analyzed. A total of 216 paraffin-embedded PGs of 54 patients were obtained and analyzed. Results: Patients with Tc-99m-MIBI negative PG(s) had significantly lower preoperative serum phosphorus and higher serum calcium levels at 6 months postoperatively compared to those with Tc-99m-MIBI positive PG(s) (P<0.05). We also found a higher total uptake ratio of the region of interest (URRI) and higher URRI max in the hypocalcemia group than in the non-hypocalcemia group. Both URRI total (P=0.003) and URRI max (P=0.028) were independent risk factors for hypocalcemia 6 months postoperatively. The URRI values of the PGs were significantly positively correlated with glandular weight (R-2=0.343, P<0.001), glandular volume (R-2=0.240, P<0.001), and degree of pathological hyperplasia (P<0.001). However, the URRI value of the PGs exhibited a notably weak correlation with proliferating cell nuclear antigen (PCNA) (R-2=0.035, P=0.006). The area under the receiver operating characteristic curve showed a URRI evaluative value of 0.771 for diffuse and nodular types in 216 PGs (P<0.001). We further evaluated 167 nodular-type PGs, distinguishing between nodular hyperplasia and a single nodule; the URRI evaluative value reached 0.819, which was higher than the volume or weight (P<0.001). Conclusions: The Tc-99m-MIBI SPECT/CT scintigraphy results were related to serum calcium levels at 6 months after total parathyroidectomy with autotransplantation (TPTX+AT), suggesting the occurrence of hypocalcemia (6 months after TPTX+AT). More importantly, this technique effectively evaluated the pathological hyperplasia of PGs preoperatively, and therefore, could assist surgeons in selecting the PGs with the lowest degree of hyperplasia intraoperatively.

Abstract:Importance The success of parathyroidectomy depends on accurate intraoperative localization and identification of all diseased glands in parathyroid exploration based on surgeon expertise to prevent persistent hyperparathyroidism. Near-infrared autofluorescence (NIRAF) imaging has recently emerged as a promising adjunctive intraoperative tool for localizing parathyroid glands; however, its potential utility in the assessment of parathyroid glands has yet to be established. Objective To analyze the differences in NIRAF signatures of parathyroid glands in single vs multiple glands in primary hyperparathyroidism (pHPT). Design, Setting, and Participants This prospective diagnostic study analyzed in vivo NIRAF images of parathyroid glands obtained during parathyroidectomies between November 18, 2019, and December 31, 2023, at a single tertiary referral center. Pixel intensities of the images were measured using third-party software. Patients who underwent parathyroidectomy for sporadic pHPT using a second-generation NIRAF imaging device were included. Patients with multiple endocrine neoplasm disorders were excluded. In vivo NIRAF images obtained during the procedures were analyzed. Exposure Near-infrared autofluorescence imaging during parathyroidectomy. Main Outcomes and Measures The primary outcomes were the autofluorescence intensity and heterogeneity of single adenomas and multigland disease (ie, double adenomas and 3- or 4-gland hyperplasia) in sporadic pHPT. Normalized autofluorescence intensity was calculated by dividing the mean pixel intensity of the parathyroid gland by the background tissue. A heterogeneity index was calculated by dividing the standard deviation by the mean pixel intensity of the gland. The secondary outcome was the visibility of each parathyroid gland on NIRAF imaging before it became apparent to the naked eye during exploration. Results A total of 1287 in vivo NIRAF images obtained from 377 patients (median [IQR] age, 66 [56-73] years; 299 female [79.3%]) were analyzed. Of all patients, 230 (61.0%) had a single adenoma, 91 (24.1%) had double adenomas, and 56 (14.9%) had 3- or 4-gland hyperplasia. A mean (SD) of 3.4 (1.1) parathyroid glands were identified in the procedures. A comparison of 581 diseased glands (45.1%) and 706 normal glands (54.9%) showed a lower median normalized autofluorescence intensity of 2.09 (95% CI, 1.07-4.01) vs 2.66 (95% CI, 1.43-4.20; effect size = 0.36) and higher heterogeneity index of 0.18 (95% CI, 0.07-0.41) vs 0.11 (95% CI, 0.01-0.27; effect size = 0.45), respectively. Of diseased glands, single adenomas (233 [40.1%]) vs double adenomas (187 [32.2%]) and 3- or 4-gland hyperplasia (161 [27.7%]) had a lower median autofluorescence intensity of 1.92 (95% CI, 1.02-4.44) vs 2.22 (95% CI, 1.10-3.97; effect size = 0.21), respectively. On receiver operating characteristic analysis, the optimal autofluorescence intensity threshold to differentiate between single adenomas vs multigland disease was 2.14, with a sensitivity of 64.4%, specificity of 58.1%, and area under the curve of 0.626. Conclusions and Relevance These findings suggest that parathyroid glands in single- vs multigland disease may exhibit different autofluorescence characteristics. Although the effect size was modest, the differences identified should be kept in mind when assessing the parathyroid glands during surgical exploration.

The value of 99mTc-sestamibi SPECT/CT over conventional SPECT in the evaluation of parathyroid adenomas or hyperplasia

Abstract:As SPECT/CT technology evolves, its applications and indications need to be evaluated clinically for more efficient and cost-effective use. This retrospective study evaluated the clinical value of simultaneously acquired (99m)Tc-sestamibi SPECT/CT versus conventional SPECT in diagnosing and locating parathyroid adenomas or hyperplasia in patients with primary hyperparathyroidism. Methods: Immediately and 60 minutes after intravenous administration of 740-925 MBq of (99m)Tc-sestamibi, static planar images of the neck and chest were obtained. SPECT/CT images were acquired 30 minutes after injection. Two experienced masked readers independently evaluated whether conventional SPECT images provided information beyond what was available from the planar images either by changing the diagnosis or by better locating the glands and whether the SPECT/CT images provided information beyond what was available from the planar plus conventional SPECT images. Forty-eight consecutive patients with a clinical diagnosis of primary hyperparathyroidism were included in the study. The 32 whose scans showed positive results underwent surgical resection and were examined histopathologically. Results: Planar and SPECT imaging, with or without CT fusion, identified 89% of the surgically confirmed diseased parathyroid glands. Use of SPECT/CT changed the diagnosis in only 1 patient (2%) from positive to negative and better located the glands in only 4 patients (8%). SPECT/CT was particularly helpful in locating the 2 ectopic parathyroid adenomas diagnosed in this cohort. Tracer retention in diseased glands did not correlate with histologic characteristics. Also, biochemical markers did not correlate with the scan findings. Conclusion: SPECT/CT has no significant clinical value additional to that of conventional SPECT for parathyroid imaging except in locating ectopic parathyroid glands. Eliminating the CT acquisition will spare patients the additional time, radiation exposure, and expense.

Abstract:SignificanceNear-infrared autofluorescence (NIRAF) utilizes the natural autofluorescence of parathyroid glands (PGs) to improve their identification during thyroid surgeries, reducing the risk of inadvertent removal and subsequent complications such as hypoparathyroidism. This study evaluates NIRAF's effectiveness in real-world surgical settings, highlighting its potential to enhance surgical outcomes and patient safety.AimWe evaluate the effectiveness of NIRAF in detecting PGs during thyroidectomy and central neck dissection and investigate autofluorescence characteristics in both fresh and paraffin-embedded tissues.ApproachWe included 101 patients diagnosed with papillary thyroid cancer who underwent surgeries in 2022 and 2023. We assessed NIRAF's ability to locate PGs, confirmed via parathyroid hormone assays, and involved both junior and senior surgeons. We measured the accuracy, speed, and agreement levels of each method and analyzed autofluorescence persistence and variation over 10 years, alongside the expression of calcium-sensing receptor (CaSR) and vitamin D.ResultsNIRAF demonstrated a sensitivity of 89.5% and a negative predictive value of 89.1%. However, its specificity and positive predictive value (PPV) were 61.2% and 62.3%, respectively, which are considered lower. The kappa statistic indicated moderate to substantial agreement (kappa = 0.478; P<0.001). Senior surgeons achieved high specificity (86.2%) and PPV (85.3%), with substantial agreement (kappa = 0.847; P<0.001). In contrast, junior surgeons displayed the lowest kappa statistic among the groups, indicating minimal agreement (kappa = 0.381; P<0.001). Common errors in NIRAF included interference from brown fat and eschar. In addition, paraffin-embedded samples retained stable autofluorescence over 10 years, showing no significant correlation with CaSR and vitamin D levels.ConclusionsNIRAF is useful for PG identification in thyroid and neck surgeries, enhancing efficiency and reducing inadvertent PG removals. The stability of autofluorescence in paraffin samples suggests its long-term viability, with false positives providing insights for further improvements in NIRAF technology.

Abstract:Objective: [18F]Fluorocholine positron emission tomography/computed tomography (PET/CT) is used frequently in addition to [99mTc]Tc-Sestamibi scintigraphy and ultrasonography for the location of hyperfunctioning parathyroid glands. The aim of this study is to evaluate the performance of quantitative criteria in [18F]fluorocholine PET/CT for localization of hyperfunctioning parathyroid glands. The secondary objective is to highlight a correlation between the detection rate of [18F]fluorocholine PET/CT and serum parathyroid hormone (PTH) level. Materials and methods: In two academic centers, we retrospectively included patients with biological hyperparathyroidism (HPT) and who had [18F]fluorocholine PET/CT. After a visual analysis, to measure the overall performance of [18F]fluorocholine PET/CT, a blind reading was carried out with standardized measurements of maximum standardized uptake value (SUVmax), liver ratio, thyroid ratio, and size ratio. We analyzed the quantitative criteria of [18F]fluorocholine PET/CT compared to the histological results, in particular to identify differences between adenomas and hyperplasias. We compared the performance of each quantitative criterion to the overall sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of [18F]fluorocholine PET/CT. The detection rate of hyperfunctioning parathyroid glands was calculated in subgroups of serum PTH level. Results: The quantitative criteria in [18F]fluorocholine PET/CT were measured for 120 patients (135 lesions). The areas under the receiver operating characteristic (ROC) curve representing SUVmax and liver ratio were significantly increased. The optimal cut-off values represented by the maximum Youden index was >4.12 for SUVmax and >27.4 for liver ratio. Beyond certain threshold values of SUVmax (>4.12) or liver ratio (>38.1), all the lesions were histologically proven adenomas. SUVmax and liver ratio were significantly higher for adenomas than for hyperplasias and differential diagnosis (p = 0.0085 and p = 0.0002). The positivity of [18F]fluorocholine PET/CT was correlated with PTH level. Detection rates were 55.56, 75.56, and 87.5%, respectively, for serum PTH < 70, 70 to 120, and >120 ng/ml. Conclusion: Semi-quantitative measurements (SUVmax and liver ratio) should be considered as additional tools in interpretation of [18F]fluorocholine PET/CT. These quantitative parameters have lower overall performance but higher specificity than overall visual analysis in identifying an adenoma. Above certain threshold values, all lesions are adenomas. [18F]fluorocholine PET/CT confirms excellent performance for the detection of hyperfunctional parathyroids. For serum PTH levels < 70 ng/ml, the detection rate of [18F]fluorocholine PET/CT is strongly decreased.

Abstract:Objective This study aimed to assess the diagnostic performance of 99m Tc-sestamibi SPECT/CT and 18 F-choline PET/CT in detecting hyperfunctioning parathyroid glands in patients undergoing surgery for primary hyperparathyroidism (PHPT). Methods A retrospective analysis was conducted on patients who underwent PHPT-related surgery between April 2019 and May 2022. The study focused on patients undergoing either 99m Tc-sestamibi SPECT/CT (81 patients) or 18 F-choline PET/CT (33 patients) scans before surgery to pinpoint hyperfunctioning parathyroid gland(s). In the majority of patients, 18 F-choline PET/CT was performed after negative or inconclusive findings on 99m Tc-sestamibi SPECT/CT. Pathohistological reports were utilized as the reference standard for evaluating the accuracy of the imaging findings. Results The study encompassed 83 patients (70 females, 84.3%) with an average age of 57.2 years (24–80 years). The pathohistological analysis identified a total of 98 glands. In a per-lesion analysis, the detection rate of 99m Tc-sestamibi SPECT/CT was 57% (95% CI 45.3–68.1), while the detection rate of 18 F-choline PET/CT was 90.3% (95% CI 74.3–98.0). Conclusion The results of our study showed the significant usefulness of 18 F-choline PET/CT in patients with negative or inconclusive results of 99m Tc-sestamibi SPECT/CT in accurately locating hyperfunctioning parathyroid glands in PHPT patients. ‡ Current address: University Clinical Hospital Mostar, Bijeli Brijeg bb, 88000 Mostar. Abstract:Objective:The purpose of this study was to evaluate the diagnostic value of clinical parameters and parathyroid scintigraphy forasymptomatic primary hyperparathyroidism (aPHPT), including quantitative parameter of laboratory and semiquantitative parameters derived from technetium-99m-metoxyisobutylisonitrile (Tc-99m-MIBI) single photon emission computed tomography/computed tomography (SPECT/CT). Patients and Methods:Two hundred and fourteen patients who had been diagnosed as PHPT and underwent surgical treatment were enrolled in this study. All patients were divided into two groups: aPHPT and symptomatic PHPT (sPHPT). Dual tracer Tc-99-pertechnetate/Tc-99m-MIBI, dual time point and tomography scintigraphy with Tc-99m-MIBI SPECT/CT were performed. Clinical parameters included basic information, serum calcium (Ca), phosphorus (P), parathyroid hormone (PTH) and alkaline phosphatase (ALP) levels. Semiquantitative parameters of parathyroid scintigraphy included the average counts of early parathyroid (PT1), late parathyroid (PT2), early thyroid (T-1) and late thyroid (T-2), the ratio of PT1/T-1 (R-1), the ratio of PT2/T-2 (R-2), parathyroid washout (PTW=(PT1-PT2)/PT1) and retention index (RI=(R-2-R-1)/R-1). P<0.05 was considered as statistically significant. Results: A total of 167 aPHPT patients (46 males, 121 females) were studied. One hundred and seventy four parathyroid glands were removed after operation: 146 (87.4%) lesions were adenoma, 12 (7.2%) lesions were hyperplasia and 1 (0.6%) lesion was adenocarcinoma. There were significant differences in previous history (P=0.000), echo of parathyroid (P=0.004), thyroid function (P=0.029), clinical course (Z=-3.422, P=0.001), Tc-99m-pertechnetat thyroid uptake (TcTU) (Z=-2.126, P=0.033), serum Ca level (t=-2.926, P=0.004) and serum PTH level (Z=-3.028, P=0.002) between a PHPT and sPHPT. For patients with aPHPT, there were significant differences for serum Ca level (t=2.832, P=0.005), R, (Z=-2.597, P=0.009) and RI (Z=-2.100, P=0.036) between adenoma and hyperplasia, and serum Ca level in aPHPT patients with adenoma was significantly higher compared with patients with hyperplasia. The areas under the curve (AUC) of clinical course, TcTU, serum Ca and PTH levels were 0.662,0.399,0.642 and 0.645 respectivelyfor differential diagnosis of aPHPT and sPHPT. The AUC of R-2 and RI were 0.737 and 0.692 respectively for differential diagnosis of adenoma and hyperplasia in patients with aPHPT. The sensitivity, negative predictive value (NPV) and accuracy for diagnosing aPHPT combined Tc-99m MIBI SPECT/CT with ultrasound (US) were significantly higher than dual tracer, dual time point, Tc-99m-MIBI SPECT/CT and US, which were 975%, 95.2% and 55.6% respectively. Conclusions:Laboratory inspection and semi-quantitative parameters of parathyroid scintigraphy had higher value for differential diagnoses of aPHPT and sPHPT, the same for adenoma and hyperplasia in patients with aPHPT. When combined Tc-99m-MIBI SPECT/CT with US, the diagnostic efficiency would be significantly improved.
Biochemical and pathologic factors affecting technetium-99m-methoxyisobutylisonitrile imaging results in patients with primary hyperparathyroidism

Abstract:Objective: Technetium 99 m methoxyisobutylisonitrile (Tc-99m MIBI) scintigraphy represents the most commonly utilized imaging modality for the detection of the diseased gland in patients with primary hyperparathyroidism (PHPT). In this study, we aimed to identify potential biological factors with an impact on MIBI sensitivity. Methods: A total of 147 patients with surgically confirmed parathyroid adenomas were assessed retrospectively. Data including medical history, biochemical and hormonal measurements, cervical US, Tc-99m MIBI scans as well as pathology reports were retrieved and recorded. Results: Of the 147 patients, there were a total of 77, 39, and 31 cases with a positive, negative, and suspicious parathyroid adenoma on Tc-99m MIBI scan, respectively. Serum calcium (Ca), parathyroid hormone (PTH) and 25 (OH) D levels were comparable among MIBI positive and negative patients [Ca: 11.5 ± 0.9 vs 11.3 ± 0.9 mg/dL (P = 0.42); PTH: 216 (146-347) vs 194 (140-317) pg/mL (P = 0.45); 25(OH)D: 8.4 (5.7-18.2) vs 10.0 (4.7-23.3) ng/mL (P = 0.64), respectively]. P-glycoprotein (P-gp) staining was negative in both groups. Also, pathological examination of tissue preparations revealed no difference in terms of the volume of the adenomas, incidence of cystic adenomas, cell-type dominance (oxyphilic cell), percent fat, and Ki-67 ratio in MIBI positive and negative groups. The rate of hyalinization was 13% in MIBI positive and 28% in MIBI negative subjects, the difference being statistically significant (P = 0.04). Conclusion: Presence of hyalinization in parathyroid adenomas was found to be negatively correlated with MIBI scan results.
Evaluation of Autofluorescence in Identifying Parathyroid Glands by Measuring Parathyroid Hormone in Fine-Needle Biopsy Washings

Abstract:Background Near-infrared autofluorescence imaging has potentially great value for assisting endocrine surgeons in identifying parathyroid glands and may dramatically change the surgical strategy of endocrine surgeons in thyroid surgery. This study is designed to objectively evaluate the role of near-infrared autofluorescence imaging in identifying parathyroid glands during thyroid surgery by measuring intraoperative parathyroid hormone in fine-needle aspiration biopsy washings. Methods This study was conducted at a tertiary referral teaching hospital in China from February 2020 to June 2020. Patients undergoing total thyroidectomy with or without neck lymph node dissection were consecutively included. The surgeon used near-infrared autofluorescence imaging to identify parathyroid glands during thyroid surgery and confirmed suspicious parathyroid tissues by measuring their intraoperative parathyroid hormone. Nanocarbon was injected into the thyroid gland if the thyroid autofluorescence intensity was too strong. The sensitivity and accuracy of near-infrared autofluorescence imaging and vision for identifying parathyroid glands, and the difference in autofluorescence intensity in various tissues were the main outcomes. Results Overall, 238 patients completed the trial. Based on the pathological and aIOPTH results, the sensitivity of near-infrared autofluorescence imaging for detecting parathyroid glands (568 of 596 parathyroid glands; 95.30%)was significantly higher than that of vision (517 of 596 parathyroid glands; 86.74%, P <.001). The accuracy of near-infrared autofluorescence imaging (764 of 841 tissues; 90.84%) was significantly higher than that of vision (567 of 841 tissues; 67.42%, P <.001) when the evaluations of certain tissues were inconsistent. There was a significant difference between the autofluorescence intensity of the parathyroid glands and that of the lymph nodes (74.19 ± 17.82 vs 33.97 ± 10.64, P <.001). Conclusion The use of near-infrared autofluorescence imaging, along with intraoperative parathyroid hormone and nanocarbon for the identification of parathyroid glands in thyroid surgery may increase the number of confirmed parathyroid glands. Using near-infrared autofluorescence imaging can effectively distinguish lymph nodes and parathyroid glands during lymph node dissection.

The poorly understood characteristic of parathyroid tissue to manifest autofluorescence (AF) under near-infrared (NIR) light has been promoted as an intraoperative adjunct in parathyroid surgery. This study sought to explore potential clinical correlates for AF and assess the clinical utility of AF in parathyroid surgery.</p>

Analysis of near-infrared autofluorescence imaging for detection of inadvertently resected parathyroid glands after endoscopic thyroidectomy

Abstract:Background: Preserving parathyroid function during thyroidectomy is crucial, but remains challenging. Real-time near-infrared autofluorescence (NIRAF) aids surgeons in intraoperative parathyroid gland (PTG) identification. However, its role in detecting PTGs unintentionally removed during surgery is unclear. Study design: This prospective study included adult patients undergoing endoscopic thyroidectomy. Surgeons identified and documented PTGs visually. Excised specimens underwent visual inspection and NIRAF imaging (PDE-Neo II). All fluorescent tissues were dissected and pathologically evaluated (reference standard). One scanned image per lobe was chosen to quantify autofluorescence (AF) intensity. Results: Overall, 95 patients underwent endoscopic thyroidectomies, with NIRAF imaging applied to 152 excised lobes. Of these, 19 lobes displayed a total of 23 spots with increased intensity. 175 specimens were sent for pathological evaluation, and 7 were confirmed to be parathyroid tissue. NIRAF demonstrated 100.0 % sensitivity and 90.5 % specificity for predicting parathyroid tissue, with 30.4 % positive predictive value, 100.0 % negative predictive value of and 90.9 % accuracy. Quantitatively normalized, the AF signal intensity was significantly higher in NIRAF-positive tissues than negative (4.3 vs 1.2 times, p < 0.0001). Additionally, the AF signal intensity in regions pathologically confirmed of parathyroid tissue was higher than non-parathyroid tissue (9.1 vs 2.1 times, p < 0.0001). Conclusion: This study suggests that NIRAF has high sensitivity and specificity for detecting inadvertently resected PTGs after endoscopic thyroidectomy, contributing to preservation efforts. However, NIRAF-positive tissues still require additional confirmation through multiple methods, emphasizing other examinations to verify that they are indeed parathyroid tissues. Further research is warranted to refine NIRAF imaging parameters.

Abstract:Background: The added benefit of using near-infrared autofluorescence (NIRAF) during total thyroidectomy (TT) remains controversial. This study investigated whether or not NIRAF results in improved patient outcomes postoperatively. Materials and Methods: We analyzed 1711 TT patients, reported in nine randomized controlled trials, following a systematic search of five databases. NIRAF was compared to the standard of care (naked eye with/without white light). Outcomes included parathyroid gland (PG) and calcium parameters and other clinical outcomes. For dichotomous outcomes, the log odds ratio (logOR) was calculated, and for continuous outcomes, the crude mean difference (MD) was measured. Sensitivity analysis was performed when heterogeneity was significant. The revised Cochrane risk of bias tool was used to assess the methodological quality. Results: Compared to the standard of care, the use of NIRAF was associated with a significant reduction in postoperative hypoparathyroidism [logOR=-0.31; 95% CI: -0.57: -0.05], inadvertent PG removal [logOR=-0.93; 95% CI: -1.60: -0.26], and postoperative hypocalcemia [logOR=-0.43 mmol/L; 95% CI: -0.77: -0.09]. It was also associated with significantly higher postoperative PTH levels [MD=4.78 pg/mL; 95% CI: 2.13: 7.43], PG identification rate [logOR=1.02; 95% CI: 0.31: 1.72], postoperative serum calcium [MD=0.05; 95% CI: 0.00: 0.09], and operative time [MD=9.38 minutes; 95% CI: 6.68: 12.09]. No difference was seen regarding PG autotransplantation, length of hospital stay, and hospitalization due to hypocalcemia. Seven trials had low risk and the remainder had some concerns. Conclusion: NIRAF is superior to the naked eye in identifying all four parathyroid glands during total thyroidectomy. The reduced risk of postoperative hypoparathyroidism and hypocalcemia reflected this preservation value. However, it was not associated with a change in the length of hospital stay. Although rare, the readmission rate due to hypocalcemia was similar across both methods.

Abstract:Objectives: To explore the diagnostic value of Tc-99m-Sestamibi multiple-phase static planar imaging and SPECT/CT in secondary hyperparathyroidism. Methods: This was a retrospective analysis of 40 cases of clinically diagnosed secondary hyperparathyroidism with multiple-phase static planar imaging, 15 min SPECT/CT fusion imaging, and ultrasonography. All cases were confirmed by pathological examination. Detection of lesions was compared between dual-phase and multi-temporal planar imaging, the detection rate of lesions using planar imaging, SPECT/CT, and ultrasonography was analyzed, and the characteristics of parathyroid tissue calcification detected by SPECT/CT were summarized. Results: The number of lesions detected was 69 by multi-temporal planar imaging and 57 by dual-phase imaging; scans at both 60 min and 90 min detected 12 lesions. SPECT/CT detected more lesions than planar imaging and ultrasonography; the sensitivities were 93.1%, 47.9%, and 70.1%, the accuracy was 88.7%, 47.6%, and 69.7%, respectively, and the specificity was 100%. Eighteen of 40 patients had parathyroid tissue calcification, 134 lesions in 32 patients exhibited calcification, and 90.6% of the 32 lesions were Tc-99m-Sestamibi positive. Conclusions: Multi-temporal planar imaging detected more lesions than dual-phase imaging. SPECT/CT plays an important role in the qualitative and quantitative evaluation and positioning of secondary hyperparathyroidism lesions.
18F-Fluorocholine PET/CT in the assessment of primary hyperparathyroidism compared with 99mTc-MIBI or 99mTc-tetrofosmin SPECT/CT: a prospective dual-centre study in 100 patients

Abstract:PurposeIn this prospective study we compared the accuracy of 18F-fluorocholine PET/CT with that of 99mTc-MIBI or99mTc-tetrofosmin SPECT/CT in the preoperative detection of parathyroid adenoma in patients with primary hyperparathyroidism. We also assessed the value of semiquantitative parameters in differentiating between parathyroid hyperplasia and adenoma.MethodsBoth 18F-fluorocholine PET/CT and 99mTc-MIBI/tetrofosmin SPECT/CT were performed in 100 consecutive patients with biochemical evidence of primary hyperparathyroidism. At least one abnormal focus on either 18F-fluorocholine or 99mTc-MIBI/tetrofosmin corresponding to a parathyroid gland or ectopic parathyroid tissue was considered as a positive finding. In 76 patients with positive findings on at least one imaging modality, surgical exploration was performed within 6 months, and the results were related to histopathological findings and clinical and laboratory findings at 3–6 months as the standard of truth. In 24 patients, no surgery was performed: in 18 patients with positive imaging findings surgery was refused or considered risky, and in 6 patients imaging was negative. Therefore, data from 82 patients (76 undergoing surgery, 6 without surgery) in whom the standard of truth criteria were met, were used in the final analysis.ResultsAll patients showed biochemical evidence of primary hyperparathyroidism with a mean serum calcium level of 2.78 ± 0.34 mmol/l and parathormone (PTH) level of 196.5 ± 236.4 pg/ml. The study results in 76 patients with verified histopathology and 3 patients with negative imaging findings were analysed. Three of six patients with negative imaging showed normalized serum PTH and calcium levels on laboratory follow-up at 3 and 6 months, and the results were considered true negative. In a patient-based analysis, the detection rate with 18F-fluorocholine PET/CT was 93% (76/82), but was only 61% (50/82) with 99mTc-MIBI/tetrofosmin SPECT/CT. In a lesion-based analysis, the sensitivity, specificity, positive predictive value, negative predictive value and overall accuracy of 18F-fluorocholine PET/CT in the detection of parathyroid adenoma were 93.7%, 96.0%, 90.2%, 97.4% and 95.3%, respectively, and of 99mTc-MIBI/tetrofosmin SPECT/CT were 60.8%, 98.5%, 94.1%, 86.3% and 87.7%, respectively. Although 18F-fluorocholine PET-positive adenomatous lesions showed higher SUVmax values than the hyperplastic glands (6.80 ± 3.78 vs. 4.53 ± 0.40) in the semiquantitative analysis, the difference was not significant (p = 0.236). The mean size (measured as the length of the greatest dimension) and weight of adenomas were 15.9 ± 7.6 mm (median 15 mm, range 1–40 mm) and 1.71 ± 1.86 g (median 1 g, range: 0.25–9 g), respectively. Among the analysed parameters including serum calcium and PTH and the size and weight of parathyroid adenomas, size was significantly different between patients with negative 99mTc-MIBI/tetrofosmin SPECT/CT and those with positive 99mTc-MIBI/tetrofosmin SPECT/CT (mean size 13.4 ± 7.6 mm vs. 16.9 ± 7.4 mm, respectively; p = 0.042).ConclusionIn this prospective study, 18F-fluorocholine PET/CT showed promise as a functional imaging modality, being clearly superior to 99mTc-MIBI/tetrofosmin SPECT/CT, especially in the detection and localization of small parathyroid adenomas in patients with primary hyperparathyroidism. SUVmax was higher in parathyroid adenomas than in hyperplasia. However, further evaluation of this modality is needed.

The Value of Tc-99m-MIBI Multi-Temporal SPECT and SPECT/CT in Patients after Total Parathyroidectomy with Autotransplantation

Abstract:234 Objectives: Total parathyroidectomy with autotransplantation(tPTX+AT) is a suitable procedure for renal hyperparathyroidism(HPT),which showed significantly often normalization of serum calcium and alkaline phosphatase as well as significant improvement of clinical signs like pruritus and muscle weakness.However,the rate of persistent or recurrent disease after tPTX+AT is high, and it is usually related to hyperplasia of the autografted tissue. 99mTc-MIBI scintigraphy is a reliable non-invasive exploratory tool in the detection of recurrent parathyroid disease in the neck or mediastinum.But little is known about its role in detecting the recurrence of autografted tissue. In this study,we aim to investigate the value of 99mTc-MIBI multi-temporal SPECT and SPECT/CT in detecting the localization of recurrence after tPTX+AT. Methods: The study group comprised 18 consecutive adult patients(4 males,14 females,age range: 33-66years) from December 2011 to July 2017 who have received tPTX+AT. All patients with suspected recurrence due to clinical or biochemical findings(eg.osteodynia,serum elevated level of parathormone) underwent 99mTc-MIBI multi-temporal SPECT and SPECT/CT of the neck, mediastinum and graft area. Multi-temporal SPECT images for these areas were acquired at early phase(15min)and delay phase(60min, 90min and 120min)after injection.SPECT/CT images were obtained at early phase using a 16-slice diagnostic CT system.Imaging results were confirmed by biopsy or further clinical and radiologic work-up. The detection rates for multi-temporal SPECT and SPECT/CT to identify hyperparathyroidism lesions were calculated. The tracer uptake by the lesions in the autograft area was assessed semi-quantitatively employing region of interest ratios to normal adjacent areas. Spearman’s rank correlation analysis was used to determine the association of the tracer uptake with serum levels of intact parathormone(iPTH) ,calcium and phosphorus. Results:99mTc-MIBI multi-temporal SPECT had detected 21 lesions including 16 in the graft site and 5 in the neck.Meanwhile, SPECT/CT had shown 24 lesions including 18 in the graft site and 6 in the neck.Two lesions in the autograft site were both missed by SPECT and SPECT/CT.One false-positive lesion proved by pathology was foreign body granuloma in the graft area,and pathological results were obtained in other 20 lesions,including 4 adenomas and 16 hyperplasias.The detection rates of hyperparathyroidism lesions by multi-temporal SPECT and SPECT/CT were 77.8%(21/27) and 88.9%(24/27),respectively,without statistical significance (P>0.05).In multi-phase SPECT,all 16 lesions in the graft sites showed the highest uptake of 99mTc-MIBI in 15min scan, and gradually reduced in the delayed scan. Five lesions located in the neck were detected in delayed imaging.The Spearman’s correlation analysis showed that the the tracer uptake had no significant correlation with levels of iPTH,calcium and phosphorus (P>0.05). Conclusion: In this retrospective study, 99mTc-MIBI multi-temporal SPECT and SPECT/CT images may provide helps for localization of the hyperparathyroidism glands accurately.It results that it is not necessary to perform delay imaging for autograft area.Future prospective studies in larger patient groups are desirable for confirming our findings.

Abstract:Background The aim of this study was to compare the autofluorescence of parathyroid glands (PGs) in-vivo and ex-vivo using two near-infrared autofluorescence(NIRAF) overlay imaging devices with different wavelengths of 785 nm and 805 nm. Methods This prospective study included six patients(five[83.3%] female and one[16.7%] male) who had undergone thyroid and/or parathyroid surgery and in whom NIRAF imaging of at least one PG was performed using two EleVision IR(Medtronic, Dublin, Ireland) devices with 785 nm and 805 nm excitation light with the same power of 3000mW in June 2023 in a tertiary referral care center. The PGs were first localized by the surgeon. Autofluorescence was subsequently imaged with both NIRAF overlay imaging devices. The distance, angle and automatic adjustment of infrared intensity(IR%) were documented using both NIRAF overlay imaging devices. Differences in autofluorescence patterns were noted. Results In total, 16 PGs were analyzed in surgeries of the thyroid glands(n=3; 50%) and in 3(50%) PG surgeries. The ideal distance for each measurement using either NIRAF overlay imaging device(wavelength 785 nm vs. 805 nm) was 10 cm with an angle of 90°. EleVision IR with a wavelength of 785nm and 805nm showed mean IR% values of 30.1±22.4(6-75) and 30.1±15.9(6-68). Stronger and homogeneous autofluorescence patterns of parathyroid adenoma were seen using the EleVision IR device with 805 nm compared to 785 nm wavelength. No other differences in autofluorescence were seen in-vivo or ex-vivo using either device. Conclusion NIRAF-based overlay imaging with a wavelength of 805 nm shows similar results in the localization of PGs as the device with 785 nm excitation light. Thus, it represents an equally feasible intraoperative tool for PGs.

Abstract:The aim of this review was to compare the efficiency of conventional ultrasound (CUS), contrast-enhanced ultrasound (CEUS), and 99mTc-MIBI scintigraphy in imaging parathyroid gland lesions in patients with primary hyperparathyroidism (PHPT). In this review, we attempted to present a clear view of the most reliable or precise technique for detecting parathyroid gland (PG) pathology in PHPT. Our study relied on secondary data based on the review of the extant literature on PHPT imaging. The research retrieved articles from the scholarly databases Google Scholar, Web of Science, and PUBMED/MEDLINE that examine the efficiency of CUS, CEUS, and 99mTc-MIBI scintigraphy in imaging PGs in PHPT patients. The study retrieved secondary data from past research identified using keyword and medical subject heading terms like "Ultrasound," "Contrast-Enhanced Ultrasound," "Sonography in parathyroid," "Primary hyperthyroidism," "Scintigraphy," and "Contrast-enhanced ultrasound in parathyroid disease." This study only relied on original contribution papers presented in English and published between 2011 and 2021. We found that CEUS has a comparable sensitivity to 99mTc-MIBI scintigraphy so that it can be used in patients who have a contraindication to scintigraphy or in a hospital without nuclear medicine facility. CEUS is able to detect very small size adenomas, whereas the detection using scintigraphy depends on the size of the diseased glands and the cytology.
Diagnostic performance of ultrasonography, dual-phase 99mTc-MIBI scintigraphy, early and delayed 99mTc-MIBI SPECT/CT in preoperative parathyroid gland localization in secondary hyperparathyroidism

Abstract:Abstract Background Secondary hyperparathyroidism (SHPT) usually requires parathyroidectomy when drug regimens fail. However, obtaining an exact preoperative map of the locations of the parathyroid glands is a challenge. The purpose of this study was to compare the diagnostic performance of US, dual-phase 99m Tc-MIBI scintigraphy, early and delayed 99m Tc-MIBI SPECT/CT in patients with SHPT. Methods Sixty patients with SHPT who were undergoing dialysis were evaluated preoperatively by US, dual-phase 99m Tc-MIBI scintigraphy, early and delayed 99m Tc-MIBI SPECT/CT. Postoperative pathology served as the gold standard. The sensitivity, specificity, and accuracy were determined for each method. Spearman correlation analysis was used to analyse the correlation of hyperplastic parathyroid calcification with serum alkaline phosphatase (ALP) and parathyroid hormone (PTH). Results A total of 229 lesions in 60 patients were pathologically confirmed to be parathyroid hyperplasia, with 209 lesions in typical sites, 15 lesions in the upper mediastinum and 5 lesions in the thyroid. A total of 88.33% (53/60) of patients had four lesions. US, early and delayed 99m Tc-MIBI SPECT/CT had significantly higher sensitivity and accuracy than dual-phase 99m Tc-MIBI scintigraphy ( P < 0.001). Furthermore, early 99m Tc-MIBI SPECT/CT had significantly higher sensitivity ( P < 0.001) and accuracy ( P = 0.001 and P < 0.001) than US and delayed 99m Tc-MIBI SPECT/CT. In patients with ectopic hyperplastic parathyroid glands, the sensitivity of early 99m Tc-MIBI SPECT/CT (90%) was significantly higher than that of US (55%) and dual-phase 99m Tc-MIBI scintigraphy (50%) ( P < 0.05). The Spearman correlation results showed a significant albeit weak association between calcification and both serum PTH and ALP ( P = 0.002). Conclusion The ability of early 99m Tc-MIBI SPECT/CT to detect hyperplastic parathyroid glands in patients with SHPT is superior to that of US, dual-phase 99m Tc-MIBI scintigraphy and delayed 99m Tc-MIBI SPECT/CT; furthermore, dual-phase 99m Tc-MIBI SPECT/CT is not essential.

Abstract:OBJECTIVE:To evaluate the efficiency of (99)Tc(m)-sestamibi (MIBI) in the diagnosis of parathyroid diseases with primary hyperparathyroidism.METHODS:One hundred and four cases with primary hyperparathyroidism underwent MIBI and parathyroidectomy between May 2010 to November 2013 were reviewed. With MIBI, single adenoma was found in 91 cases, two adenomas in 5 cases, carcinoma and hyperplasia in 3 cases respectively. Primary hyperparathyroidism was diagnosed based on the associated symptoms and signs in 93 patients, while it was indicated in 11 asymptomatic patients due to the presence of hypercalcemia in health examination. Parathroid hormone levels ranged from 98 to 2 800 ng/L, and serum calcium levels were between 2.56-4.23 mmol/L.RESULTS:The sensitivity of MIBI for total diseased parntyroids was 84.0% (100/119), and that for parathyroid adenoma, parnthyroid carcinoma, atypical adenoma and primary parathyroid hyperplasia was 87.1% (88/101), 3/3, 2/2 and 7/13, respectively.CONCLUSIONS:MIBI is efficient in the diagnosis of parathyroid diseases with primary hyperparathyroidism. The size and cystic degeneration of parathroid disease are two common factors decreasing MIBI reliability, and thyroid nodule is additional source leading to false-positive results.
