Abstract:Background: Internet-based tele-audiology is expected to relieve the dilemma between the lack of resources and high demand of audiological care services. This paper presents a web services based, distributed pure-tone hearing assessment system that improves accessibility of traditionally underserved groups to audiology care. Methods: The system employs browser-server network architecture to connect patients to audiology specialists through a web server where all application software is hosted. Software on the server is designed with a three-tier approach which makes the system scalable to include other audiological services. Hearing test data are stored in a standard database and can potentially be integrated into established electronic medical records. On the remote patient side, off-the-shelf audiometers are adopted. The Internet connection of these audiometers can be flexibly configured either with or without a computer. Two aspects of the system were tested: (1) the clinical effectiveness of the system: double-blinded experiments were conducted to assess hearing ability of 30 subjects and paired t-tests were utilized to compare assessment results from the remote approach and the conventional setup; and (2) to analyze the system bandwidth requirements, data traffic among the server, the audiometer, and the audiologist terminal was examined with a network monitoring software (wireshark). Results: Paired t-test results have demonstrated that the remote hearing assessment is equivalent in effectiveness to its conventional counterparts at all tested frequencies (P values are in the range of [0.12, 0.94]), and the bandwidth required by the system is less than 1 Mbps, falling within the capacity of average commercial Internet service subscription. Conclusions: The project developed a remote hearing assessment system based on services on a web server. The system minimizes hardware and software requirements on the audiologist's computer and can be realized with regular Internet service subscription. Patient operations involved in hearing assessment are simple; making hearing test services more accessible to those otherwise may not be able to obtain the desired hearing care.
Can Videoconferencing Be a Viable Method to Measure Speech Perception?

Abstract:Purpose: Telehealth has proven effective for service delivery to remote and rural locations and was helpful during lockdowns when patients were unable to see clinicians in person. To assure the reliability of clinical services, the aim of the present study was to evaluate a telehealth protocol to measure speech perception skills through virtual meeting applications such as Zoom. Method: A total of 20 participants with normal hearing and cognition participated in the study. Participants' speech perception performance was measured in two sessions: one over a Zoom call and one in person in the laboratory. Speech perception was measured using Quick Speech-in-Noise Test, Bamford–Kowal–Bench Speech-in-Noise Test, and Words-in-Noise Test. Results: Statistical analysis revealed no significant differences between speech perception scores obtained over the Zoom and in-person methodologies for any of the tests. Additionally, our study found no significant difference in the scores obtained between wired and wireless headphones during Zoom calls. Conclusion: These results suggest that Zoom can be used as a reliable method to measure speech perception in young individuals with normal hearing using these three tests in situations where conventional methods cannot be utilized.

Automated Audiometry Using Apple iOS-Based Application Technology

Abstract:Objective The aim of this study is to determine the feasibility of an Apple iOS-based automated hearing testing application and to compare its accuracy with conventional audiometry. Study Design Prospective diagnostic study. Setting Academic medical center. Subjects and Methods An iOS-based software application was developed to perform automated pure-tone hearing testing on the iPhone, iPod touch, and iPad. To assess for device variations and compatibility, preliminary work was performed to compare the standardized sound output (dB) of various Apple device and headset combinations. Forty-two subjects underwent automated iOS-based hearing testing in a sound booth, automated iOS-based hearing testing in a quiet room, and conventional manual audiometry. Results The maximum difference in sound intensity between various Apple device and headset combinations was 4 dB. On average, 96% (95% confidence interval [CI], 91%-100%) of the threshold values obtained using the automated test in a sound booth were within 10 dB of the corresponding threshold values obtained using conventional audiometry. When the automated test was performed in a quiet room, 94% (95% CI, 87%-100%) of the threshold values were within 10 dB of the threshold values obtained using conventional audiometry. Under standardized testing conditions, 90% of the subjects preferred iOS-based audiometry as opposed to conventional audiometry. Conclusion Apple iOS-based devices provide a platform for automated air conduction audiometry without requiring extra equipment and yield hearing test results that approach those of conventional audiometry.

Abstract:Speech audiometry in noise based on sentence tests is an important diagnostic tool to assess listeners’ speech recognition threshold (SRT), i.e., the signal-to-noise ratio corresponding to 50% intelligibility. The clinical standard measurement procedure requires a professional experimenter to record and evaluate the response (expert-conducted speech audiometry). The use of automatic speech recognition enables self-conducted measurements with an easy-to-use speech-based interface. This article compares self-conducted SRT measurements using smart speakers with expert-conducted laboratory measurements. With smart speakers, there is no control over the absolute presentation level, potential errors from the automated response logging, and room acoustics. We investigate the differences between highly controlled measurements in the laboratory and smart speaker-based tests for young normal-hearing (NH) listeners as well as for elderly NH, mildly and moderately hearing-impaired listeners in low, medium, and highly reverberant room acoustics. For the smart speaker setup, we observe an overall bias in the SRT result that depends on the hearing loss. The bias ranges from +0.7 dB for elderly moderately hearing-impaired listeners to +2.2 dB for young NH listeners. The intrasubject standard deviation is close to the clinical standard deviation (0.57/0.69 dB for the young/elderly NH compared with 0.5 dB observed for clinical tests and 0.93/1.09 dB for the mild/moderate hearing-impaired listeners compared with 0.9 dB). For detecting a clinically elevated SRT, the speech-based test achieves an area under the curve value of 0.95 and therefore seems promising for complementing clinical measurements.

Abstract:Speech audiometry is one of the methods for evaluating hearing status, and recently it ’s tended to be implemented on mobile phones with the development of internet and smart devices. Although several applications include the function of speech audiometry, the effectiveness of this function was rarel y reported. In this work, we developed an Apple iOS-based application (Hearing Assistant) with functions of pure-tone audiometry and speech audiometry for people speaking Mandarin Chinese . The speech material was designed to reflect listeners ’ audibility at specified frequencies. The reliability of the speech audiometry was analyzed and discussed according to the users ’ data collected through this application . The results showed a significant but week correlation between pure-tone thresholds and scores of speech audiometry, indicating the speech audiometry can be used to discriminate impaired and normal hearing but it’s not accurate enough to evaluate hearing thresholds quan titatively at each test frequency.
Remote self-testing for adult cochlear implant users: the effect of wireless streaming on speech recognition in noise

Abstract:Objectives: Wireless sound transfer methods for cochlear implant sound processors have become popular for remote self-assessed hearing tests. The aim of this study was to determine (1) spectral differences in stimuli between different wireless sound transfer options and (2) the effect on outcomes of speech recognition tests in noise. Design: In study 1, the frequency response of different streaming options (Phonak Roger Select, Cochlear Mini Mic 2+, telecoil and Bluetooth connection) was measured by connecting headphones to CI sound processors. Study 2 followed a repeated measures design in which digits-in-noise (DIN) tests were performed using wireless streaming to sound processors from Cochlear, Advanced Bionics, and MED-EL. Study sample: 20 normal hearing participants. Results: Differences in frequency response between loudspeaker and wireless streaming conditions were minimal. We did not find significant difference in DIN outcome (F4,194 = 1.062, p = 0.376) between the wireless transfer options with the Cochlear Nucleus 7 processor. No significant difference in DIN outcomes was found between Bluetooth streaming and the loudspeaker condition for all of the three tested brands. The mean standard error of measurement was 0.72 dB. Conclusions: No significant differences in DIN test outcomes between wireless sound transfer and the reference method were found.

Abstract:Many hearing difficulties can be explained as a loss of audibility, a problem easily detected and treated using standard audiological procedures. Yet, hearing can be much poorer (or more impaired) than audibility predicts because of deficits in the suprathreshold mechanisms that encode the rapidly changing, spectral, temporal, and binaural aspects of the sound. The ability to evaluate these mechanisms requires well-defined stimuli and strict adherence to rigorous psychometric principles. This project reports on the comparison between a laboratory-based and a mobile system's results for psychoacoustic assessment in adult listeners with normal hearing. A description of both systems employed is provided. Psychoacoustic tests include frequency discrimination, amplitude modulation detection, binaural encoding, and temporal gap detection. Results reported by the mobile system were not significantly different from those collected with the laboratory-based system for most of the tests and were consistent with those reported in the literature. The mobile system has the potential to be a feasible option for the assessment of suprathreshold auditory encoding abilities.
Feasibility of efficient smartphone-based threshold and loudness assessments in typical home settings

Abstract:Ambient noise is a critical factor affecting the precision of mobile hearing tests conducted in home environments. Monitoring noise levels during out-of-booth measurements provides essential information about the suitability of the setting for accurate audiometric testing. When ambient noise is controlled, results are expected to be comparable to in-booth measurements. This study remotely conducted air-conduction pure-tone audiometry and adaptive categorical loudness scaling (ACALOS) tests at 0.25, 1, and 4 kHz using a smartphone, while an integrated microphone and a dosimeter app were used to quantify ambient noise levels. Additionally, a reinforced ACALOS (rACALOS) method was proposed to integrate threshold measurement into the ACALOS procedure. The rACALOS method not only improves the accuracy of threshold estimation but also increases efficiency by combining two independent procedures into a single, streamlined process. As a result, ambient noise levels were mostly below the maximum permissible level. Hearing tests conducted via smartphone demonstrated moderate-to-excellent reliability, with intraclass correlation coefficients (ICCs) exceeding 0.75, and strong validity, with biases of less than 1 dB. In simulations, the rACALOS method reduced the bias towards pre-assumed thresholds, and in behavioral experiments, it showed a stronger correlation with pure-tone audiometric thresholds than the baseline method. Overall, this study demonstrates that administering pure-tone audiometry and ACALOS tests at home is feasible, valid, efficient, and reliable when ambient noise is sufficiently low.

Abstract:Hearing loss prevalence is increasing, with an estimated 2.5 billion people affected globally by 2050. Scalable service delivery models using innovative technologies and task-shifting are World Health Organization priorities to improve access to hearing care, particularly in low- and middle-income countries. Smartphone-facilitated audiometry in the community using hearing aids covered by noise-attenuating ear cups ("in-situ") could support more accessible hearing care when provided by less trained individuals such as community health workers (CHWs). This study aimed to determine the validity of this method for potential hearing aid fitting. Study objectives included determining the maximum permissible ambient noise level (MPANL), inter-device reliability, clinical threshold accuracy, reliability, and performance in real-world settings.

Abstract:The importance of tele-audiology has been heightened by the current COVID-19 pandemic. The present article reviews the current state of tele-audiology practice while presenting its limitations and opportunities. Specifically, this review addresses: (1) barriers to hearing healthcare, (2) tele-audiology services, and (3) tele-audiology key issues, challenges, and future directions. Accumulating evidence suggests that tele-audiology is a viable service delivery model, as remote hearing screening, diagnostic testing, intervention, and rehabilitation can each be completed reliably and effectively. The benefits of tele-audiology include improved access to care, increased follow-up rates, and reduced travel time and costs. Still, significant logistical and technical challenges remain from ensuring a secure and robust internet connection to controlling ambient noise and meeting all state and federal licensure and reimbursement regulations. Future research and development, especially advancements in artificial intelligence, will continue to increase tele-audiology acceptance, expand remote care, and ultimately improve patient satisfaction.
Evaluation of Frequency-Lowering Algorithms for Intelligibility of Chinese Speech in Hearing-Aid Users

Abstract:Despite the attention being given to and the knowledge of the benefits of evaluating frequency-lowering algorithms for hearing-impaired people, the causality between these algorithms and their benefits is still not clear. This is aggravated by the fact that comparative research on the methodologies and skills required for fitting an appropriate algorithm to individual patients is lacking. Against this back-drop, the current study has attempted to make progress in this area. In this experiment, six experienced traditional hearing-aid users with severe impairments in the high frequencies were fitted with two different frequency-lowering methods, and weekly hearing tests were conducted to track the benefits of such methods. After the experiment, five of the listeners accepted the frequency-lowering algorithms. Both methods showed superior results when compared with the listeners’ own hearing aids in most of the tests, and the segmented compression algorithm was indicated to have better "anti-noise" quality and speech intelligibility improvement capabilities. A preference for the application of proportional compression to unvoiced speech was also found in this algorithm. Unlike in previous studies, all speech materials here are recorded in Chinese. Therefore, the results could also be used to evaluate the benefits of frequency-lowering to the intelligibility of Chinese speech for patients. As a result, an improvement was found, especially in the recognition of consonants. Moreover, no adverse effect was found in intonation recognition.
Do uHear? Validation of uHear App for Preliminary Screening of Hearing Ability in Soundscape Studies

Abstract:Studies involving soundscape perception often exclude participants with hearing loss to prevent impaired perception from affecting experimental results. Participants are typically screened with pure tone audiometry, the "gold standard" for identifying and quantifying hearing loss at specific frequencies, and excluded if a study-dependent threshold is not met. However, procuring professional audiometric equipment for soundscape studies may be cost-ineffective, and manually performing audiometric tests is labour-intensive. Moreover, testing requirements for soundscape studies may not require sensitivities and specificities as high as that in a medical diagnosis setting. Hence, in this study, we investigate the effectiveness of the uHear app, an iOS application, as an affordable and automatic alternative to a conventional audiometer in screening participants for hearing loss for the purpose of soundscape studies or listening tests in general. Based on audiometric comparisons with the audiometer of 163 participants, the uHear app was found to have high precision (98.04%) when using the World Health Organization (WHO) grading scheme for assessing normal hearing. Precision is further improved (98.69%) when all frequencies assessed with the uHear app is considered in the grading, which lends further support to this cost-effective, automated alternative to screen for normal hearing.
Personal experience with the remote check telehealth in cochlear implant users: from COVID-19 emergency to routine service

Abstract:Purpose: To critically illustrate the personal experience with using the "Remote Check" application which remotely monitors the hearing rehabilitation level of cochlear implant users at home and further allows clinicians to schedule in-clinic sessions according to the patients' needs. Methods: 12-month prospective study. Eighty adult cochlear implant users (females n = 37, males n = 43; age range 20-77 years) with ≥ 36 months of cochlear implant experience and ≥ 12 months of stable auditory and speech recognition level volunteered for this 12-month long prospective study. For each patient, at the beginning of the study during the in-clinic session to assess the stable aided hearing thresholds and the cochlear implant integrity and patient's usage, the "Remote Check" assessment baseline values were obtained. "Remote Check" outcomes were collected at different times in the subsequent at-home sessions, to identify the patients that had to reach the Center. Chi-square test has been used for statistical analysis of the comparison of the "Remote Check" outcomes and in-clinic session results. Results: "Remote Check" application outcomes demonstrated minimal or no differences between all sessions. The at-home Remote Check application reached the same clinical outcomes as the in-clinic sessions in 79 out 80 of participants (99%) with high statistical significance (p < 0.05). Conclusions: "Remote Check" application supported hearing monitoring in cochlear implant users that were not able to attend the in-clinic review during COVID-19 pandemic time. This study demonstrates that the application can be a useful routine tool also for clinical follow-up of cochlear implant users with stable aided hearing.
Screening for Hearing Impairment in Older Adults by Smartphone-Based Audiometry, Self-Perception, HHIE Screening Questionnaire, and Free-Field Voice Test: Comparative Evaluation of the Screening Accuracy With Standard Pure-Tone Audiometry

Abstract:Background Hearing impairment is the most frequent sensory deficit in humans, affecting more than 360 million people worldwide. In fact, hearing impairment is not merely a health problem, but it also has a great impact on the educational performance, economic income, and quality of life. Hearing impairment is therefore an important social concern. Objective We aimed to evaluate and compare the accuracy of self-perception, Hearing Handicap Inventory for the Elderly-Screening (HHIE-S) questionnaire, free-field voice test, and smartphone-based audiometry as tests for screening moderate hearing impairment in older adults in China. Methods In this study, 41 patients were recruited through a single otology practice. All patients were older than 65 years. Patients with otorrhea and cognitive impairment were excluded. Moderate hearing impairment was defined as mean hearing thresholds at 500, 1000, 2000, and 4000 Hz >40 dB hearing loss (pure-tone average > 40 dB hearing loss). All patients completed 5 hearing tests, namely, the self-perception test, HHIE-S questionnaire test, free-field voice test, smartphone-based audiometry test, and standard pure-tone audiometry by the same audiologist. We compared the results of these tests to the standard audiogram in the better-hearing ear. Results The sensitivity and the specificity of the self-perception test were 0.58 (95% CI 0.29-0.84) and 0.34 (95% CI 0.19-0.54), respectively. The sensitivity and the specificity of the HHIE-S questionnaire test were 0.67 (95% CI 0.35-0.89) and 0.31 (95% CI 0.316-0.51), respectively. The sensitivity and the specificity of the free-field voice test were 0.83 (95% CI 0.51-0.97) and 0.41 (95% CI 0.24-0.61), respectively. The sensitivity and the specificity of the smartphone-based audiometry test were 0.92 (95% CI 0.60-0.99) and 0.76 (95% CI 0.56-0.89), respectively. Smartphone-based audiometry correctly diagnosed the presence of hearing loss with high sensitivity and high specificity. Conclusions Smartphone-based audiometry may be a dependable screening test to rule out moderate hearing impairment in the older population.

Abstract:Technological advances have allowed researchers to conduct research remotely. Online auditory testing has received interest since the Covid-19 pandemic. A number of web-based developments have improved the range of auditory tasks during remote participation. Most of these studies have been conducted in young, motivated individuals who are comfortable with technology. Such studies have also used stimuli testing auditory perceptual abilities. Research on auditory cognitive abilities in real-world older adults is lacking. In this study, we assess the reproducibility of a range of auditory cognitive abilities in older adults, with a range of hearing abilities, who took part in in-person and online experiments. Participants performed a questionnaire-based assessment and were asked to complete two verbal speech-in-noise perception tasks, for digits and sentences, and two auditory memory tasks, for different sound features. In the first part of the study, 58 Participants performed these tests in-person and online in order to test the reproducibility of the tasks. In the second part, 147 participants conducted all the tasks online in order to test if previously published findings from in-person research were reproducible. We found that older adults under the age of 70 and those with a better hearing were more likely to take part in online testing. The questionnaire-based test had significantly better reproducibility than the behavioural auditory tests but there were no differences in reproducibility between in-person and online auditory cognitive metrics. Relationships between relationships with age and hearing thresholds in an in-person or online setting were not significantly different. Furthermore, important relationships between auditory metrics, evidenced in literature previously, were reproducible online. This study suggests that auditory cognitive testing may be reliably conducted online.
Reliability and Feasibility of Administering a Child Language Assessment via Telehealth

Abstract:Purpose: Despite the speed with which telehealth use advanced during the COVID-19 pandemic, evidence is needed to support the remote delivery of standardized assessments. This study investigated the reliability and feasibility of administering a standardized language assessment administered in real-world telehealth scenarios compared to in-person administration. Method: A total of 100 children between the ages of 3 and 12 years were administered one of three versions of the Clinical Evaluation of Language Fundamentals (CELF). Children were administered the CELF by the same licensed speech-language pathologists (SLPs) in person and using telehealth, with the order counterbalanced. Means for Core Language standard scores were compared between conditions and among devices. Descriptive statistics summarized the behavior and technology disruptions during administration as well as the results of parent and SLP telehealth perception surveys. Results: In-person and telehealth mean scores on all three versions of the CELF revealed no systematic differences of one condition under- or overestimating another. The incidence of child behavior disruptions was similar for both test administration conditions. Adaptations compensated for the rare technology disruptions. Despite no significant language score and behavior differences between testing conditions, parents reported they continued to prefer in-person assessments. SLP participants viewed telehealth overall positively but identified conditions in which they continued to prefer in-person delivery. Conclusions: This study provides evidence of minimal or no differences in scores and behavioral or technological disruptions between remote and in-person administration of the CELF core language assessments. SLP and parent participants' attitudes toward remote delivery of standardized tests appear to be evolving in a positive direction compared to previous studies. A Multicenter Validity Study of Four Smartphone Hearing Test Apps in Optimized and Home Environments

Abstract:This study explores whether patients can monitor their own hearing at home using applications ("apps") on their own smartphones. Based on these results, two apps (one from each of iOS and Android operating systems) could be recommended and may be useful for monitoring trends in hearing thresholds at home. Objective Pure tone audiometry (PTA) is the gold standard for hearing assessment. However, it requires access to specialized equipment. Smartphone audiometry applications (apps) have been developed to perform automated threshold audiometry and could allow patients to perform self‐administered screening or monitoring. This study aimed to assess the validity and feasibility of patients using apps to self‐assess hearing thresholds at home, with comparison to PTA. Methods A multi‐center, prospective randomized study was conducted amongst patients undergoing PTA in clinics. Participants were randomly allocated to one of four publicly‐available apps designed to measure pure tone thresholds. Participants used an app once in optimal sound‐treated conditions and a further three times at home. Ear‐specific frequency‐specific thresholds and pure tone average were compared using Pearson correlation coefficient. The percentage of app hearing tests with results within ±10 dB of PTA was calculated. Patient acceptability was assessed via an online survey. Results One hundred thirty‐nine participants submitted data. The results of two at‐home automated smartphone apps correlated strongly/very strongly with PTA average and their frequency‐specific median was within ±10 dB accuracy. Smartphone audiometry performed in sound‐treated and home conditions were very strongly correlated. The apps were rated as easy/very easy to use by 90% of participants and 90% would be happy/very happy to use an app to monitor their hearing. Conclusion Judicious use of self‐performed smartphone audiometry was both valid and feasible for two of four apps. It could provide frequency‐specific threshold estimates at home, potentially allowing assessments of patients remotely or monitoring of fluctuating hearing loss. Abstract:By 2050, 1 in 4 people worldwide will be living with hearing impairment. We propose a digital Speech Hearing Screener (dSHS) using short nonsense word recognition to measure speech-hearing ability. The importance of hearing screening is increasing due to the anticipated increase in individuals with hearing impairment globally. We compare dSHS outcomes with standardized pure-tone averages (PTA) and speech-recognition thresholds (SRT). Fifty participants (aged 55 or older underwent pure-tone and speech-recognition thresholding. One-way ANOVA was used to compare differences between hearing impaired and hearing not-impaired groups, by the dSHS, with a clinical threshold of moderately impaired hearing at 35 dB and severe hearing impairment at 50 dB. dSHS results significantly correlated with PTAs/SRTs. ANOVA results revealed the dSHS was significantly different (F(1,47) = 38.1, p < 0.001) between hearing impaired and unimpaired groups. Classification analysis using a 35 dB threshold, yielded accuracy of 85.7% for PTA-based impairment and 81.6% for SRT-based impairment. At a 50 dB threshold, dSHS classification accuracy was 79.6% for PTA-based impairment (Negative Predictive Value (NPV)-93%) and 83.7% (NPV-100%) for SRT-based impairment. The dSHS successfully differentiates between hearing-impaired and unimpaired individuals in under 3 min. This hearing screener offers a time-saving, in-clinic hearing screening to streamline the triage of those with likely hearing impairment to the appropriate follow-up assessment, thereby improving the quality of services. Future work will investigate the ability of the dSHS to help rule out hearing impairment as a cause or confounder in clinical and research applications.
