Abstract:Aim: The purpose of this study was to compare the effects of autologous platelet-rich fibrin (PRF) alone and PRF loaded with SIM on peri-implant bone changes and implant stability in patients undergoing implant rehabilitation. Settings and Design: This was a nonrandomized controlled split-mouth study Materials and Methods: The study included 8 males between the ages of 45 and 60 years. Each patient received two implants, one on each side of the arch. One side was treated with PRF alone and the other side with PRF loaded with SIM at the time of osteotomy. A cone-beam computed tomography was used to evaluate bone changes around the insertion of implant sites at 3, 6, and 12 months postoperatively. The secondary outcome included measuring implant stability using Osstell device at baseline and 3 months postinsertion. To compare groups at different time periods, data were examined using a two-way analysis of variance. Statistical Analysis Used: The results were compared between the groups using a two-way analysis of variance, followed by a post hoc Bonferroni test. To examine total bone changes and stability comparisons between the two groups at the end of the trial, an unpaired t-test was utilizedResults: The mean crestal bone-level changes in the SIM/PRF group were significantly lower than the PRF group, with a mean shift of 0.9788 ± 0.04853 versus 1.356 ± 0.0434, respectively (P < 0.0001). There was no significant difference between the two groups in implant stability. Conclusion: Peri-implant application of SIM/PRF resulted in less bone changes than PRF alone, which may prove to be beneficial for the long-term success of implants. SIM showed promising results in limiting peri-implant bone resorption providing new clinical application for SIM in dental implant rehabilitation.
The Effect of Platelet-Rich Fibrin on Soft Tissue and Crestal Bone in One-Stage Implant Placement in Fresh Extraction Sockets: A Randomized Controlled Clinical Trial

The placement of an implant immediately after tooth extraction helps to maintain the bone crest and leads to ideal implant positioning from a prosthetic point of view and has the potential of decreasing the total treatment time for patients. Aim and Objective: To evaluate the platelet rich fibrin with or without bone ceramic on soft issue and crestal bone in one stage implant placement in fresh extraction socket. Materials and Methods: A total of 30 patients were randomly assigned into two groups—each group comprising 15 patients based on platelet-rich fibrin with and without bone ceramic and one-stage implant placement in fresh extraction socket. All patients were prospectively evaluated clinically and radiographically. All the data were analyzed using a software statistical package for social sciences (SPSS) version 11.5 (Chicago, Illinois, USA). Results: During the 6-month interval, no implant was lost; the mean bone level at the implants increased in platelet-rich fibrin (PRF) with bone ceramic group and decreased in PRF group and papillary index increased in both the groups, whereas modified sulcus bleeding index and probing pocket depth (PPD) decreased in both the groups. Conclusion: It can be concluded that mean crestal bone levels increased in PRF with bone ceramic group, whereas decrease in PRF group and both the groups are statistically significant. When the papillary levels are compared between the two groups, there was no statistical significance, but increased papillary levels were found in both the groups. The mean PPD was decreased in both the groups, but there was no statistical significance between the two groups.

Abstract:Background: This clinical study investigates platelet-rich fibrin's (PRF) impact on dental implant stability, addressing global oral health challenges and limitations of traditional methods. Emphasizing osseointegration's pivotal role, the study explores PRF's potential in enhancing implant stability, assessing it through resonance frequency analysis (RFA) and implant stability quotient (ISQ). The hypothesis suggests PRF may improve both primary and secondary stability, aiming to uncover clinical benefits in dental implant procedures Materials and methods: The study involved 24 subjects from the Department of Periodontics outpatient clinics with a meticulously designed methodology. This included a pre-surgical protocol with oral prophylaxis, impressions, and cone-beam computed tomography (CBCT) analysis. PRF preparation utilized a minimally invasive venipuncture technique. Implant placement followed a two-stage surgical protocol, assessing primary stability with MEGA ISQ (Ostell). Post-surgery, patients received instructions and underwent recall for secondary stability after three months. Clinical parameters such as plaque index (PI), gingival index (GI), implant probing pocket depth (IPPD), sulcus bleeding index (SBI), and implant stability (IS) were systematically recorded. Robust statistical analyses, using IBM SPSS Statistics for Windows v20.0 (IBM Corp., Armonk, USA) software, incorporated Mann-Whitney U and Wilcoxon signed-rank tests for group and within-time point comparisons, with a significance level of p<0.05. This comprehensive study yields nuanced insights into the impact of PRF and implant procedures on key clinical parameters, contributing significantly to the field. Results: This study compared dental implants with and without PRF in 24 patients. Both groups showed significant improvements in the PI, GI, and SBI. The PRF group exhibited higher IS in the third and sixth months, while IPPD was lower in the PRF group in the sixth month. Conclusion: The findings of the study highlight a positive impact on implant stability contributing to better implant outcomes.

Abstract:Abstract Purpose To compare the survival and clinical performance of implants placed in sites previously augmented with autogenous bone grafts covered by either a platelet-rich fibrin (PRF) membrane (PRF group) or a standard procedure (gold standard) involving coverage of the autogenous bone graft with deproteinised bovine bone mineral and a resorbable collagen membrane (control group). Methods A total of 27 partially edentulous patients (test n = 14, control n = 13) with indication for staged lateral bone block augmentation and dental implant placement were included. Twenty-four months after crown placement (range: 14–32 months), patients were recalled for a final clinical and radiographic follow-up. Outcome measures were implant survival, implant crown survival, clinical parameters of the implant, peri-implant marginal bone level, marginal bone level of adjacent tooth surfaces, biological and technical complications and patient-related outcome measures. Results Two implants were lost in the control group (85% survival rate); none were lost in the PRF group (100% survival rate). None of the 26 initially placed implant crowns were lost, but one implant and therefore one implant crown were lost after 20 months. Consequently, the definitive implant crown survival was 92% (95% confidence interval (CI): 73–110%) in the control group and 100% in the PRF group. No statistical difference in implant survival rate ( p = 0.13) or implant crown survival was seen between the groups ( p = 0.28). The mean marginal bone level at the follow-up was 0.26 mm (95% CI: 0.01–0.50 mm) in the PRF group and 0.68 mm (95% CI: 0.41–0.96 mm) in the control group. The difference between the groups was − 0.43 mm (95% CI: − 0.80 to − 0.05 mm, p = 0.03), which was statistically significant ( p = 0.03). Both groups demonstrated similar healthy peri-implant soft tissue values at the final follow-up. Conclusion Although the current study is based on a small sample of participants, the findings suggest that the methodology of the PRF and the control group approach can both be used for bone augmentation with a similar outcome. A significant, but clinically irrelevant, higher peri-implant marginal bone level was registered in the PRF group than in the control group. Patients in both groups were highly satisfied with the treatment. Trial registration ClinicalTrials.gov Identifier: NCT04350749 . Registered 17 April 2020. Retrospectively registered.

Abstract:Introduction Dental extraction is a common oral surgery procedure, but it can lead to unpredictable changes in alveolar bone structure, which can complicate future rehabilitation with prostheses. Socket preservation aims to reduce bone and soft tissue loss after extraction and involves various techniques using different materials. Autogenous bone is considered the gold standard for grafts, while autologous platelet concentrates like platelet-rich fibrin (PRF) have been used for tissue healing. This study aims to radiographically assess the bone remodeling of freshly extracted socket filled with Activated Platelet Rich Fibrin (APRF) and PRF. Materials and Methods A randomized controlled study was conducted on 60 patients undergoing tooth extraction as atraumatic as possible. Patients were divided into two groups: one receiving APRF with Calcium Chloride as activating agent and the other PRF. Radiographic assessments were done using Kelley et al scoring criteria at various time intervals, and statistical analyses were performed to evaluate the results. Results In this study we found increased bone density in extracted socket filled with APRF at any given time interval compared to PRF which is statistically significant. The maxilla demonstrated better bone formation compared to the mandible. Gender, age, and site also influenced bone formation, with females showing differences in lamina dura and overall density. Discussion PRF is rich in growth factors, promoting osteoblast formation and angiogenesis. Activation with calcium chloride accelerates clot formation and the release of growth factors. This study's radiographic data suggests that APRF may be more effective than PRF in promoting socket preservation and bone formation.

Abstract:Background: Following the loss of a tooth, the new edentulous area of the ridge will undergo several adaptive modifications due to changes in function within and surrounding the socket. This bone resorption explains the need for socket preservation techniques in areas of esthetic concerns and functional demands. Demineralized freeze-dried bone allograft (DFDBA) possesses greater osteoinductive potential due to the exposure of bone morphogenetic protein (BMP-3)and collagen fibrils and can be used efficiently in socket preservation techniques. DFDBA yields better results when combined with an autologous platelet concentrate, such as platelet-rich fibrin. Therefore, we formulated this randomized controlled clinical trial to assess the clinical and radiovisiographical outcomes of platelet-rich fibrin (PRF) and DFDBAs for extraction socket preservation in humans at different time intervals. Materials and methods: This was a randomized controlled trial with 100 people as study subjects, and they were randomly divided into two groups: the test group (DFDBA and PRF placed in the extraction socket) and the control group (natural healing of the extraction socket). Clinical and radiographic evaluation using radiovisiography (RVG) was done at baseline, three-month, and six-month intervals. Cone-beam computed tomography (CBCT) was used at six months to determine the bone density in the test and control groups. Results: When compared from baseline to six months, the percentage change in clinical and RVG measurements for the test group was 15.96% (11.9064 mm) and 16.77% (12.1840 mm), respectively, whereas for the control group, it was 46.09% (14.0396 mm) and 47.61% (14.5716 mm), thus indicating lesser bone resorption in the test group as opposed to the control group. CBCT values also showed greater bone density for the test group (682.3120 HU) than the control group (503.8336 HU). Conclusion: This study demonstrates the advantages of DFDBA bone graft with PRF compared to natural healing in achieving socket preservation by maintaining the marginal and buccolingual bone levels.

Abstract:Background Finding a protocol that could prevent bone resorption and be implemented in clinical practice would be crucial in providing sufficient bone to replace missing teeth with implants. Purpose The study aimed to determine the effectiveness of different centrifugation platelet-rich fibrin (PRF) protocols in new bone formation and bone regenerative markers. Study Design, Setting and Sample This randomized clinical trial was conducted at Izmir Katip Çelebi Research Hospital, a population-based facility in Izmir. Study subjects were composed of patients who required extraction of anterior teeth. Exclusion criteria included periodontal disease, resorption of alveolar bone, defects, smoking, alcoholism, and systemic diseases. Independent Variable The independent variable was the PRF protocol. The subjects were randomly assigned to one of three groups: leukocyte platelet-rich fibrin (L-PRF), advanced platelet-rich fibrin (A-PRF) and control groups (healing naturally). Main Outcome Variable The primary outcome of interest was the percentage of new bone formation, determined by analyzing the staining intensity in histomorphometric assessments of bone samples collected 8 weeks after extraction. The secondary outcomes were regenerative effects measured by the immunohistochemical expression of markers such as osteocalcin, alkaline phosphatase, and proliferating cell nuclear antigen. Potential benefits were evaluated by clinical observations of pain, swelling, membrane visibility and healing. Covariates The covariates were age, sex and health conditions. Analyses Histologic comparative staining intensities and biomarkers expression between groups were evaluated by one way analysis of variance. A difference of P < .05 was considered statistically significant. Results The study included 57 subjects, with a mean age of 45 years (±5.6); 30 were male (53%) and 27 female (47%). The control group had a mean new bone formation of 32.68% (±2.5), the A-PRF group 61.37% (±3.0), and the L-PRF group 70.74% (±3.5) (P < .001). The A-PRF group showed significantly higher osteocalcin expression than the control group (P = .013). Alkaline phosphatase and proliferating cell nuclear antigen expression scores for PRF groups were significantly higher than the control group's (P = .001). Both groups demonstrated significantly lower pain scores, reduced gingival swelling, better membrane visibility, and healing compared to the control group. Conclusion and Relevance PRF enhanced bone formation rates, with L-PRF showing the most significant effect.

Abstract:Background. Platelet concentrates are currently being researched for their potential to enhance bone formation. Aim. The aim of this study is to quantitatively evaluate and compare bone regeneration in periapical lesions using a combination of platelet-rich plasma (PRP) + β-tricalcium phosphate (β-TCP) and platelet-rich fibrin (PRF) + β-TCP. Methods. Ten subjects with periapical lesions measuring 10-20 mm in relation to maxillary incisors indicated for periapical surgery were selected. Pre-operative bone density values at the periapical region were measured using CBCT. Root canal treatment was completed appropriately in all cases. Under adequate local anesthesia, periapical surgery was performed. The 10 subjects were distributed into two groups of 5 subjects in each group. Group I: Periapical bone defect filled with PRP + β-TCP and Group II: Periapical bone defect filled with PRF + β-TCP. The mucoperiosteal flaps were repositioned and sutured. Bone density evaluation of the periapical region was done 6 months and 1 year after surgery using cone beam computed tomography (CBCT). The Hounsfield unit (HU) values obtained were subjected to statistical analysis by Independent samples t-test for inter-group comparison. The intra-group comparison was done by repeated measure ANOVA and Bonferroni post hoc tests. Results. In Group I and Group II, a statistically significant difference was observed at the end of 6 months and 1 year, when compared to the pre-operative values (P=0.000). There was no statistically significant difference in bone density values between the PRP + β-TCP and PRF + β-TCPgroups at both 6 months and 1-year post-operative observation periods. Conclusion. PRP + β-TCP and PRF + β-TCP were equally effective in promoting bone regeneration and can be considered valuable bioactive surgical additives for enhancement of healing in periapical bone defects.
Histomorphometric evaluation of human extraction sockets treated with autologous fibrin, sticky bone or biphasic calcium phosphate

Abstract:It is essential to maintain the alveolar bone ridge to ensure the success of implant therapy. Platelet-rich fibrin (PRF) may benefit bone repair, but its quantitative microscopic results are still inconclusive. The aim of this study was to histomorphometrically analyze human dental alveoli after extraction treated with autologous fibrin, biphasic calcium phosphate or sticky bone. The sample consisted of healthy adult volunteer patients, with clinical and tomographic indication for single post-extraction graft of upper premolars for maintenance of the alveolar ridge and subsequent implantation. The 10 remaining patients in the study were divided into three groups according to the type of filling used in the dental socket: autologous PRF plug covered by a PRF membrane (G1), PRF associated with an alloplastic graft based on hydroxyapatite with beta tricalcium phosphate covered by a collagen membrane (G2) or alloplastic graft based on beta tricalcium phosphate covered by collagen membrane (control). After 8 months, bone biopsies were performed at the grafted sites and the patients underwent implant-prosthetic rehabilitation. Paraffin-embedded tissue blocks were routinely processed and sectionsfrom different depths were mounted in 3 slides and stained with HE. The histomorphometric evaluation analyzed 30 photomicrographs per block, quantifying the percentage presence of newly formed bone, connective tissue and remaining biomaterial using the ImageJ software. Parametric data enabled intergroup comparisons using ANOVA and Tukey’s post-hoc test for multiple comparison with statistical significance of 5% (p 0.05). There was a higher mean of newly formed bone in G1 (68.83%) compared to G2 (35.69%) and control (16.28%). There was greater presence of connective tissue in the control (61.56%). Remaining biomaterial was higher in G2 (15.75%), but did not differ statistically from the control. Bone regeneration obtained with PRF alone or sticky bone suggests the efficacy of these therapies, encouraging the clinical use of this blood concentrate in dental procedures.

Abstract:BackgroundPlatelet rich fibrin (PRF) has shown great potential in osteogenesis; however, some studies still question utilizing it as a grafting material. Thus, the aim of this review is to evaluate the effect of PRF when used in socket and ridge preservation procedures.MethodsElectronic searches through MEDLINE, EMBASE, and Cochrane, Science Citation Index Expanded databases and manual searches of unpublished data, academic theses, and journals were conducted up until July 2021. The outcomes were to assess the ability of PRF as a graft material to preserve bone width, height, and density after tooth extraction.ResultsTwelve studies were included in the review, using PRF showed significant results in all three outcomes when compared to no grafting at all, however when compared to other commonly used grafting materials it showed a lesser effect. On the other hand, most studies included reported mixing PRF with a graft material showed the best result. The meta-analysis also revealed the significant results in using PRF on the three outcomes.ConclusionThe meta-analysis of the studies included proved the beneficial effect of PRF in socket preservation surgeries alone or in combination with other graft materials, but further individual multi-centre randomized controlled studies with appropriate sample size are still needed to further confirm our findings.

Abstract:BACKGROUND: Simvastatin (SMV), a lipid lowering drug, can modulate the process of bone regeneration at the molecular and cellular levels. Its effect on the osseointegration of implants has been studied extensively on animals with assuring results with limited research on human subjects.AIM: To estimate the effect of simvastatin gel in the osseointegration of dental implants using bone scintigraphy, Materials and Methods: 20 participants with missing mandibular first molars and D2 type bone were assigned equally to Group A receiving 1.2% simvastatin and Group B receiving Placebo gels during the placement of implants. The participants were subjected to bone scintigraphy to determine the osteoblastic activity at baseline, 30th day and 90th day after implant placement.RESULTS: Group A revealed a significant increase in osteoblastic activity between baseline, day 30 and 90 (P<.05) with a higher mean of 100.06±21.644% on day 30. Group B revealed a significant increase in osteoblastic activity only between baseline and day 30, and baseline and day 90 (P<.05) whereas there was no difference between day 30 and 90 (P>.05) with a higher mean of 79.20±18.255% on day 30. Bivariate analysis at different time periods revealed a significant difference between groups A and B on day 30.CONCLUSION: Implants placed with 1.2% simvastatin gel showed enhanced osteoblastic activity on the fourth week of implant placement, indicating faster rate of osseointegration at an early stage.

Abstract:Introduction: Dental implants have become the treatment of option for replacing missing teeth. But dental implant placement needs adequate available bone for primary stability to attain successful osseointegration. In patients with inadequate bone volume, various bone augmentation procedures are available for facilitating implant placement. The mandibular ramus acts as an admirable source of autogenous bone for alveolar ridge augmentation procedures. Case Report: In the present case report a female patient with Siebert class III defect with respective to 46 was managed by implant placement along with autogenous bone grafting keeping in mind about the crown root ratio which could be a post-complication in anticipation. Conclusion: Thus, this article describes a case report of reconstruction of extensive vertical bone defect, using intraoral ramus block bone graft and autogenous A-PRF for implant site augmentation presented admirable results.

Abstract:Dental rehabilitation using implants sometimes lead to complications which ultimately leads to implant loss. With respect to peri‐implantitis, the non‐surgical mechanical therapy has not demonstrated to be equally effective. Therefore, surgical therapies have been proposed for treating peri‐implantitis. The aim of this procedure was to evaluate efficacy of using sticky bone - demineralized freeze dried bone allograft (DBDFA) + I-prf in management of a peri-implant defect. A full thickness flap was reflected, exposing the defect. Mechanical debridement of implant surface was done with Teflon curettes. After degranulation, implant surface was rinsed with 10% povidone iodine for 10 minutes. DFDBA + I-PRF was used to completely fill the defect and it was covered using a collagen membrane, Healiguide. 6 months post operatively, radiograph showed bone gain around the dental implant. DFDBA + Prf along with Healiguide shows promising results in treatment of peri-implantitis.

Comparative evaluation of the osteogenic capacity of second-generation platelet concentrates on dental pulp stem cells - An ex vivo study

Abstract:Introduction: Clinical evidence of platelet-rich fibrin (PRF) benefits on bone repair is still emerging, prompting researchers to experiment with different PRF formulations as osteoconductive scaffolds. Aims: This study compared the osteoconductive effects of injectable PRF (i-PRF) and leukocyte-rich PRF (L-PRF) on the differentiation of dental pulp stem cells (DPSCs) into osteoblasts. Materials and methods: Blood samples were collected from the volunteers to prepare L-PRF and i-PRF conditioned media (CM) by centrifugation. DPSCs were isolated from impacted third molars and cultured. Proliferation of DPSCs in response to L-PRF and i-PRF was assessed by MTT (3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) assay. Osteoinductive potential was evaluated through alkaline phosphatase (ALP) activity, alizarin red S (ARS) staining, growth factor levels (vascular endothelial growth factor [VEGF], transforming growth factor [TGF-beta]), and cytokine expression (interleukin 6 [IL-6], IL-8) after 7 days. Results: MTT assay results showed that both L-PRF and i-PRF increased DPSC proliferation relative to the control group. After 7 days in L-PRF and i-PRF CM, DPSCs exhibited increased ALP activity, higher red-colored calcium deposits with ARS staining, and elevated levels of VEGF and TGF-beta. In addition, higher concentrations of inflammatory cytokines IL-6 and IL-8 were observed in both L-PRF and i-PRF compared to the control. Conclusions: Using both L-PRF and i-PRF as scaffolds can enhance the osteoinductive ability of stem cells, offering a potential strategy for regenerative therapies.

Abstract:Background: Immediate implants have been commonly used for the single or bi-rooted teeth. However, it was a challenge and an intriguing dilemma with respect to molar teeth, due tothe presence of multiple factors such as: multiple root morphology,achieving implant stability and bone to implant contact.The aim of this study was to assess the efficiency of using A-PRF-autograft mixture around implants placed immediately in mandibular molar region from aclinical and radiographic point of view.Patient and methods: This study was carried out as a randomized controlled clinical trial in which twenty patients needing extraction of unrestored mandibular molars and placing of immediate implant, were selected. Patients were divided into two groups randomly; the study group where the resultant gap following the immediate insertion of a molar implant was filled with A-PRF-autograft mixtureand the control group where peri-implant gap was filled with autogenous bone graft alone.The studied variables were the degree of facial swelling, marginal bone height and the bone density around the immediately installed dental implants. The period for follow up was after six months. For statistical analysis,Mann- Whitney U test was used.Results: The study group showed better results in the clinical and radiographic studied variables. A significant difference was seen regarding the marginal bone height.Although, regarding bone density and swelling, more acceptable results have been seen within the study group, the results showed no statistically significant difference between both groups.Conclusion:The results assured the efficiency of using A-PRF-autograft mixturewith concurrent placement of immediate implants in the lower molar region and highlighted their promising effects on bone and soft tissue healing.

Abstract:Summary A study of the blood volume of the dog as measured simultaneously by the Dye T-1824, by carbon monoxide and by radioactive iron, Fe55, has shown that the values measured by the cell-tagging methods are distinctly lower than the value given by the dye method whenever the donor blood tagged with CO or Fe55 shows incompatibility with the blood of the recipient animal. However, when the bloods of the donor and the recipient animals show no incompatibility, the results by the three methods show improved agreement. These results suggest that the cell-dumpings occurring in the reaction of incompatibility, by making the smaller vessels inaccessible to the clumped cells, reduce the volume measured by the tagged cells.

Abstract:Objective To assess the outcome of bone graft material at alveolar bone augmentation sites combined with dental implants in postmenopausal women with compromised bone health by evaluating cone beam computed tomography (CBCT) scans at multiple time points. Materials and Methods CBCT scans were analyzed on 55 postmenopausal women with compromised bone health status to determine the fate of alveolar bone augmentation. CBCT scans were taken immediately after surgery and 9 and 24 months post-operatively. The patient's medication regimens and durations were recorded, and the pixel intensity value (PIV) was measured and standardized using scoring criteria and visual assessment. Statistical analyses included two-sample t-tests for continuous variables and Fisher's exact tests for categorical variables. Results Among the normal patients, 73% received a grade 2 visual score, and 27% received a grade 1 visual score. After 24 months, 45% of patients received a grade 2 score, and 27% received a grade 3 score. In the osteoporotic group receiving medication, 77% of participants received a grade 1 visual score at the 9-month postoperative evaluation, while 23% received a grade 2 score. At the 24-month assessment, 55% of patients received a grade 1 score, 41% received a grade 2 score, and only 5% received a grade 3 score. Notably, although the graft material did not remodel into native bone, it was a scaffold for implants in controlled osteoporotic patients. Conclusion The study's results show that the pixel intensity values of particulate graft materials are similar across the three different time points, suggesting that the graft material's pixel intensity value remains constant in postmenopausal women with osteoporosis. The study's limitations include a small sample size and a restricted 24-month follow-up period. This limited time frame may need to capture long-term changes or variations in graft materials adequately. Future Directions Future research should include a larger sample size and have a longer follow-up duration to provide a more comprehensive understanding of the change in graft materials between patients with normal and compromised bone health.

Abstract:Introduction: Replacement of lost teeth with implants is a well-known and accepted worldwide treatment. A healthy amount of bone surrounding the implant plays a vital role in osseointegration and is required for implant success. This study aimed to evaluate the efficacy of single-crown rehabilitation of subcrestal implants in terms of bone loss (BL) and overall success. Materials and methods: Twenty healthy patients requiring implant placement to replace single hopeless teeth or extracted teeth were recruited for this prospective study. In these 20 patients, the implants were placed at the subcrestal level. Bone levels around the implant placement (T0) and one year after loading (T1) were estimated in this study. A paired t-test was used for intra-group comparisons, multivariate linear regression analysis was conducted to analyze the effect of independent variables on BL, and a correlation test was used to correlate various variables. Statistical significance was maintained at a p value of 0.05. Results: The outcomes demonstrated statistically significant BL at the mesial and distal sides of the subcrestal implants at one-year follow-up (p<0.05). Age and sex were not significantly correlated with BL in any region (p>0.05). Brushing frequency, probing depth (PD), and bleeding index (BI) showed statistically significant effects on BL (p<0.05). A weak correlation was observed between age and other variables, with age and mesial BL showing a low correlation of 0.016. Conclusion: Within the parameters of this prospective study, it could be proposed that subcrestal implants caused significant BL. The PD, BI, and brushing frequency were significant predictors of BL.

Abstract:BACKGROUND: The preservation of post-extraction alveola, as part of guided bone regeneration, is recommended for the creation of a biological foundation for future implant-prosthetic or prosthetic production. Several graft materials are used, either independently or in a combination of each other. Advanced platelet-rich fibrin (A-PRF) as an autologous blood derivate in the recent years has been increasingly used because of its properties in angiogenesis, epithelialization, and hemostasis. Simple production, low cost, and non-use of anticoagulants are advantages for more mass use of it. CASE PRESENTATION: Two clinical cases are described, one is using a combination of A-PRF with xenograft, and the second clinical case is using a solo A-PRF, for socket preservation. CONCLUSION: The properties of A-PRF for preserving both bone volume and quality have been confirmed, highlighting its potential as a valuable therapeutic tool. Furthermore, the post-operative period following A-PRF treatment has been found to be free of significant discomfort and morbidity for patients, suggesting that this approach may represent a safe and effective means of promoting optimal clinical outcomes.
