Abstract:PURPOSE:To investigate the effect of inflammation-based indexes in predicting radiation pneumonitis (RP) and prognosis in lung tumor patients treated with stereotactic body radiation therapy (SBRT). MATERIALS AND METHODS:The data of one hundred and seventy-two patients with 272 lung lesions from November 2015 to December 2020 were retrospectively analyzed. Pretreatment hematological indexes including platelet count, neutrophil count, and lymphocyte count were collected before treatment. Systemic immune-inflammation index (SII), neutrophil-to-lymphocyte ratio (NLR), and platelet-to-lymphocyte ratio (PLR) were calculated. The receiver operating characteristic (ROC) curve was established to predict the RP and overall survival of patients, and the Youden index was calculated to determine the cutoff values of SII, NLR, and PLR before radiotherapy. RESULTS:Pretreatment SII, NLR, and PLR could predict RP in lung tumor patients treated with SBRT, the optimal cutoff values of SII, NLR, and PLR were 355.38, 2.04, and 141.09, respectively. Pretreatment PLR could predict survival and the optimal cutoff value of PLR was 166.83, patients with a PLR > 166.83 predict worse overall survival (OS) (P < 0.001). The 1-year and 2-year OS for patients with a PLR ≤ 166.83 were 96.3% and 82.4%, while for those with a PLR > 166.83 were 82.0% and 58.5%, respectively. CONCLUSION:In lung tumor patients treated with SBRT, pretreatment SII, NLR, and PLR can effectively predict RP and a higher PLR predicts poor OS. These inflammation-based indexes could serve as reliable and convenient predictors to guide treatment for physicians in clinical practice.
Predicting Severe Radiation Pneumonitis in Patients with Locally- Advanced Non-Small Cell Lung Cancer after Thoracic Radiotherapy: Development and Internal Validation of a Nomogram Based on the Clinical, Hematological and Dose–volume Histogram Parameters

Abstract:Purpose Severe radiation pneumonitis (grade≥3 RP) remains an important dose-limiting toxicity after thoracic radiotherapy (RT). This study aimed to investigate risk factors for severe RP in patients with locally-advanced non-small cell lung cancer (NSCLC) after thoracic RT, develop a prediction model to identify high-risk groups and investigate impact of severe RP on overall survival (OS). Methods We retrospectively collected clinical, hematological and dosimetric factors from 351 stage-Ⅲ NSCLC patients after thoracic RT between 2018 and 2022. The primary endpoint was development of severe RP. The secondary endpoint was OS. Logistic regression and least absolute shrinkage and selection operator (LASSO) regression analysis were used to identify risk factors of severe RP. Nomogram was generated based on multivariate regression coefficients. Area under the ROC curve (AUC), calibration curve, and decision curve analysis (DCA) were conducted to validate the model. After a long-term follow-up, OS of patients with RP vs. non-RP and mild RP vs. severe RP groups was analyzed by Kaplan‒Meier method. Results ILD (p<0.001), percentage of contralateral lung volume receiving≥5Gy (contraV5) (P=0.013), percentage of ipsilateral lung volume receiving≥20Gy (ipsiV20)(P=0.039), pre-RT derived neutrophil lymphocyte ratio (dNLR) (P=0.015) and post-RT systemic inflammation response index (SIRI) (p=0.001) were showed to be independent predictors of severe RP and were included in the nomogram. ROC curves revealed the AUC of the nomogram was 0.782. Calibration curves showed favorable consistency, and DCA showed satisfactory positive net benefits of the model. Median follow-up time was 19.8 months (1.4-52.9 months), and cases who developed severe RP showed shorter OS than those developed mild RP (P=0.027). Conclusion We identified that ILD, contraV5(>11%), ipsiV20(>45%), pre-RT dNLR (>1.9) and post-RT SIRI (>3.4) could predict severe RP among patients with locally-advanced NSCLC receiving thoracic RT. Combining these indicators, a nomogram was first built and validated, showing its potential value in clinical practice.

Abstract:Background The study aims to identify the risk factors and develop a model for predicting grade ≥ 2 radiation pneumonitis (RP) for lung cancer patients treated with stereotactic body radiation therapy (SBRT). Materials and Methods Clinical data, dosimetric data, and laboratory biomarkers from 186 patients treated with lung SBRT were collected. Univariate and multivariate logistic regression were performed to determine the predictive factors for grade ≥ 2 RP. Three models were developed by using the clinical, dosimetric, and combined factors, respectively. Results With a median follow-up of 36 months, grade ≥ 2 RP was recorded in 13.4% of patients. On univariate logistic regression analysis, clinical factors of age and lung volume, dosimetric factors of treatment durations, fractional dose and V 10 , and laboratory biomarkers of neutrophil, PLT, PLR, and Hb levels were significantly associated with grade ≥2 RP. However, on multivariate analysis, only age, lung volume, fractional dose, V 10 , and Hb levels were independent factors. AUC values for the clinical, dosimetric, and combined models were 0.730 (95% CI, 0.660-0.793), 0.711 (95% CI, 0.641-0.775) and 0.830 (95% CI, 0.768-0.881), respectively. The combined model provided superior discriminative ability than the clinical and dosimetric models ( P <0.05). Conclusion Age, lung volume, fractional dose, V 10 , and Hb levels were demonstrated to be significant factors associated with grade ≥ 2 RP for lung cancer patients after SBRT. A novel model combining clinical, dosimetric factors, and laboratory biomarkers improved predictive performance compared with the clinical and dosimetric model alone. Micro- The age, lung volume, fractional dose, V 10 , and Hb levels were found to be independent factors for grade ≥ 2 RP by analyzing 186 patients with primary or metastatic lung tumor treated with SBRT. The age and Hb levels are novel predictors reported for the first time. A prediction model combining clinical, dosimetric factors and laboratory biomarkers was proposed and achieved an AUC value up to 0.830, higher than models reported in the literature.

Abstract:Introduction Patients with early non-small-cell lung cancer (NSCLC) have a relatively long survival time after stereotactic body radiation therapy (SBRT). Predicting radiation-induced pneumonia (RP) has important clinical and social implications for improving the quality of life of such patients. This study developed an RP prediction model by using 3-dimensional (3D) dosiomic features. The model can be used to guide radiation therapy to reduce toxicity. Methods Radiomic features were extracted from pre-treatment CT, dose-volume histogram (DVH) parameters and dosiomic features were extracted from the 3D dose distribution of 140 lung cancer patients. Four predictive models: (1) CT; (2) CT+DVH; (3) CT+Rtdose; and (4) Hybrid, CT+DVH+Rtdose, were trained to predict symptomatic RP by extremely randomized trees. Accuracy, sensitivity, specificity, and area under the receiver operator characteristic curve were evaluated. Result Results showed that the fraction regimen was correlated with symptomatic RP (p<0.001). The proposed model achieved promising prediction results. The performance metrics for CT, CT+DVH, CT+Rtdose, and Hybrid were as follows: accuracy: 0.786, 0.821, 0.821, and 0.857; sensitivity: 0.625, 1, 0.875, and 1; specificity: 0.8, 0.565, 0.5, and 0.875; and area under the receiver operator characteristic curve: 0.791, 0.809, 0.907, and 0.920, respectively. Conclusion Dosiomic features can improve the performance of the predictive model for symptomatic RP compared with that obtained with the CT+DVH model. The model proposed in this study can help radiation oncologists individually predict the incidence rate of RP.

Abstract:Abstract Background This study was designed to establish radiation pneumonitis (RP) prediction models using dosiomics and/or deep learning-based radiomics (DLR) features based on 3D dose distribution. Methods A total of 140 patients with non-small cell lung cancer who received stereotactic body radiation therapy (SBRT) were retrospectively included in this study. These patients were randomly divided into the training (n = 112) and test (n = 28) sets. Besides, 107 dosiomics features were extracted by Pyradiomics, and 1316 DLR features were extracted by ResNet50. Feature visualization was performed based on Spearman’s correlation coefficients, and feature selection was performed based on the least absolute shrinkage and selection operator. Three different models were constructed based on random forest, including (1) a dosiomics model (a model constructed based on dosiomics features), (2) a DLR model (a model constructed based on DLR features), and (3) a hybrid model (a model constructed based on dosiomics and DLR features). Subsequently, the performance of these three models was compared with receiver operating characteristic curves. Finally, these dosiomics and DLR features were analyzed with Spearman’s correlation coefficients. Results In the training set, the area under the curve (AUC) of the dosiomics, DLR, and hybrid models was 0.9986, 0.9992, and 0.9993, respectively; the accuracy of these three models was 0.9643, 0.9464, and 0.9642, respectively. In the test set, the AUC of these three models was 0.8462, 0.8750, and 0.9000, respectively; the accuracy of these three models was 0.8214, 0.7857, and 0.8571, respectively. The hybrid model based on dosiomics and DLR features outperformed other two models. Correlation analysis between dosiomics features and DLR features showed weak correlations. The dosiomics features that correlated DLR features with the Spearman’s rho |ρ| ≥ 0.8 were all first-order features. Conclusion The hybrid features based on dosiomics and DLR features from 3D dose distribution could improve the performance of RP prediction after SBRT.
Predicting radiation pneumonitis in lung cancer: a EUD-based machine learning approach for volumetric modulated arc therapy patients

Abstract:Purpose: This study aims to develop an optimal machine learning model that uses lung equivalent uniform dose (lung EUD to predict radiation pneumonitis (RP) occurrence in lung cancer patients treated with volumetric modulated arc therapy (VMAT). Methods: We analyzed a cohort of 77 patients diagnosed with locally advanced squamous cell lung cancer (LASCLC) receiving concurrent chemoradiotherapy with VMAT. Patients were categorized based on the onset of grade II or higher radiation pneumonitis (RP 2+). Dose volume histogram data, extracted from the treatment planning system, were used to compute the lung EUD values for both groups using a specialized numerical analysis code. We identified the parameter α, representing the most significant relative difference in lung EUD between the two groups. The predictive potential of variables for RP2+, including physical dose metrics, lung EUD, normal tissue complication probability (NTCP) from the Lyman-Kutcher-Burman (LKB) model, and lung EUD-calibrated NTCP for affected and whole lung, underwent both univariate and multivariate analyses. Relevant variables were then employed as inputs for machine learning models: multiple logistic regression (MLR), support vector machine (SVM), decision tree (DT), and K-nearest neighbor (KNN). Each model's performance was gauged using the area under the curve (AUC), determining the best-performing model. Results: The optimal α-value for lung EUD was 0.3, maximizing the relative lung EUD difference between the RP 2+ and non-RP 2+ groups. A strong correlation coefficient of 0.929 (P< 0.01) was observed between lung EUD (α = 0.3) and physical dose metrics. When examining predictive capabilities, lung EUD-based NTCP for the affected lung (AUC: 0.862) and whole lung (AUC: 0.815) surpassed LKB-based NTCP for the respective lungs. The decision tree (DT) model using lung EUD-based predictors emerged as the superior model, achieving an AUC of 0.98 in both training and validation datasets. Discussions: The likelihood of developing RP 2+ has shown a significant correlation with the advancements in RT technology. From traditional 3-D conformal RT, lung cancer treatment methodologies have transitioned to sophisticated techniques like static IMRT. Accurately deriving such a dose–effect relationship through NTCP modeling of RP incidence is statistically challenging due to the increased number of degrees-of-freedom. To the best of our knowledge, many studies have not clarified the rationale behind setting the α-value to 0.99 or 1, despite the closely aligned calculated lung EUD and lung mean dose MLD. Perfect independence among variables is rarely achievable in real-world scenarios. Four prominent machine learning algorithms were used to devise our prediction models. The inclusion of lung EUD-based factors substantially enhanced their predictive performance for RP 2+. Our results advocate for the decision tree model with lung EUD-based predictors as the optimal prediction tool for VMAT-treated lung cancer patients. Which could replace conventional dosimetric parameters, potentially simplifying complex neural network structures in prediction models.

Impact of normal lung volume choices on radiation pneumonitis risk prediction in locally advanced NSCLC radiotherapy

Abstract:This study is to evaluate the impact of different normal lung volumes on radiation pneumonitis (RP) prediction in patients with locally advanced non-small-cell-lung-cancer (LA-NSCLC) receiving radiotherapy. Three dosimetric variables (V20, V5, and mean lung dose (MLD)) were calculated using treatment plans from 442 patients with LA-NSCLC enrolled in NRG Oncology RTOG 0617. Three lung volumes were defined based on the treatment plan: total lung excluding gross-tumor-target (TL-GTV), total lung excluding clinical-target-volume (TL-CTV), and total lung excluding planning-target-volume (TL-PTV). Binary classification was used for clinical endpoints: no-RP2 (N = 377) for patients with acute RP grades 0 or 1, and RP2 (N = 65) for patients with acute grades 2 or higher. The impact of lung volume definition on RP prediction was investigated with statistical analyses and two supervised machine learning (ML) models: logistic regression (LR) and random forest (RF). Balanced data was generated for comparison to prediction obtained with the imbalanced data. Areas under curve (AUCs) and the Shapley Additive eXplanations (SHAP) were used to examine ML performance and explain the contribution of each feature to the prediction, respectively. Statistical analyses revealed that V20 and MLD were associated with RP but no difference among different lung volume definitions. Mean AUC values from 10-fold cross validation using imbalanced and balanced datasets were < 0.6 except that RF from the latter dataset yielded AUC values > 0.7. SHAP values indicated that MLD and V20 were the most prominent features in RP prediction. In all assessments, no difference among various volume definitions was found. Different lung volumes showed insignificant impact on RP prediction. Low AUC values suggest limited effectiveness of these dosimetric variables in predicting RP risk and more powerful predictors are needed.

Abstract:Purpose This study aimed to combine clinical/dosimetric factors and handcrafted/deep learning radiomic features to establish a predictive model for symptomatic (grade ≥2) radiation pneumonitis (RP) in lung cancer patients who received immunotherapy followed by radiotherapy. Materials and Methods This study retrospectively collected data of 73 lung cancer patients with prior receipt of ICIs who underwent thoracic radiotherapy (TRT). Of these 73 patients, 41 (56.2%) developed symptomatic grade ≥2 RP. RP was defined per multidisciplinary clinician consensus using CTCAE v5.0. Regions of interest (ROIs) (from radiotherapy planning CT images) utilized herein were gross tumor volume (GTV), planning tumor volume (PTV), and PTV-GTV. Clinical/dosimetric (mean lung dose and V5-V30) parameters were collected, and 107 handcrafted radiomic (HCR) features were extracted from each ROI. Deep learning-based radiomic (DLR) features were also extracted based on pre-trained 3D residual network models. HCR models, Fusion HCR model, Fusion HCR + ResNet models, and Fusion HCR+ ResNet + Clinical models were built and compared using the receiver operating characteristic (ROC) curve with measurement of the area under the curve (AUC). Five-fold cross-validation was performed to avoid model overfitting. Results HCR models across various ROIs and the Fusion HCR model showed good predictive ability with AUCs from 0.740-0.808 and 0.740-0.802 in the training and testing cohorts, respectively. The addition of DLR features improved the effectiveness of HCR models (AUCs from 0.826-0.898 and 0.821-0.898 in both respective cohorts). The best performing prediction model (HCR+ ResNet + Clinical) combined HCR & DLR features with 7 clinical/dosimetric characteristics and achieved an average AUC of 0.936 and 0.946 in both respective cohorts. Conclusions In patients undergoing combined immunotherapy/RT for lung cancer, integrating clinical/dosimetric factors and handcrafted/deep learning radiomic features can offer a high predictive capacity for RP, and merits further prospective validation.

A Novel Nomogram Model Based on Cone-Beam CT Radiomics Analysis Technology for Predicting Radiation Pneumonitis in Esophageal Cancer Patients Undergoing Radiotherapy

Abstract:Purpose We quantitatively analyzed the characteristics of cone-beam computed tomography (CBCT) radiomics in different periods during radiotherapy (RT) and then built a novel nomogram model integrating clinical features and dosimetric parameters for predicting radiation pneumonitis (RP) in patients with esophageal squamous cell carcinoma (ESCC). Methods At our institute, a retrospective study was conducted on 96 ESCC patients for whom we had complete clinical feature and dosimetric parameter data. CBCT images of each patient in three different periods of RT were obtained, the images were segmented using both lungs as the region of interest (ROI), and 851 image features were extracted. The least absolute shrinkage selection operator (LASSO) was applied to identify candidate radiomics features, and logistic regression analyses were applied to construct the rad-score. The optimal period for the rad-score, clinical features, and dosimetric parameters were selected to construct the nomogram model and then the receiver operating characteristic (ROC) curve was used to evaluate the prediction capacity of the model. Calibration curves and decision curves were used to demonstrate the discriminatory and clinical benefit ratios, respectively. Results The relative volume of total lung treated with ≥5 Gy (V5), mean lung dose (MLD), and tumor stage were independent predictors of RP and were finally incorporated into the nomogram. When the three time periods were modeled, the first period was better than the others. In the primary cohort, the area under the ROC curve (AUC) was 0.700 (95% confidence interval (CI) 0.568–0.832), and in the independent validation cohort, the AUC was 0.765 (95% CI 0.588–0.941). In the nomogram model that integrates clinical features and dosimetric parameters, the AUC in the primary cohort was 0.836 (95% CI 0.700–0.918), and the AUC in the validation cohort was 0.905 (95% CI 0.799–1.000). The nomogram model exhibits excellent performance. Calibration curves indicate a favorable consistency between the nomogram prediction and the actual outcomes. The decision curve exhibits satisfactory clinical utility. Conclusion The radiomics model based on early lung CBCT is a potentially valuable tool for predicting RP. V5, MLD, and tumor stage have certain predictive effects for RP. The developed nomogram model has a better prediction ability than any of the other predictors and can be used as a quantitative model to predict RP.

Abstract:Purpose: To establish a radiomics model using radiomics features from different region of interests (ROI) based on dosimetry-related regions in enhanced computed tomography (CT) simulated images to predict radiation pneumonitis (RP) in patients with non-small cell lung cancer (NSCLC). Methods: Our retrospective study was conducted based on a cohort of 236 NSCLC patients (59 of them with RP≥2) who were treated in 2 institutions and divided into the primary cohort (n = 182,46 of them with RP≥2) and external validation cohort (n = 54,13 of them with RP≥2). Radiomic features extracted from three ROIs were defined as the whole lung (WL), the dose volume histogram (DVH) of the lung V20 (V20_Lung) and the DVH of the V30 of lung minus the planning target volume (PTV) (V30 Lung-PTV). A total of 107 radiomics features were extracted from each ROIs. The U test, correlation coefficient and least absolute shrinkage and selection operator (LASSO) were performed for features selection. Six models based on different classification algorithms were developed to select the best radiomics model (R model).In addition, we built a dosimetry model then combined it with the best R model to create a mixed model (R+D model) The receiver operating characteristic (ROC) curve was delineated to assess the predictive efficacy of the models. Decision curve analysis could benefit from the model proposals through the assessment of clinical utility. Results: Among the three ROIs, the best R model constructed from the LightGBM algorithm demonstrated the strongest discriminative ability in the ROI of V30 Lung-PTV. The corresponding area under the curve (AUC) value was 0.930 (95 % confidence interval (CI): 0.829-0.941). The D model, R model and R+D model achieved AUC values of 0.798 (95 %CI: 0.732-0.865), 0.930 (95 %CI: 0.829-0.941) and 0.940 (95 %CI: 0.906-0.974) in primary cohort, and in external validation cohort, the AUC values were 0.793 (95 %CI:0.637-0.949), 0.887 (95 %CI:0.810-0.993), 0.951 (95CI%:0.891-1.000). Decision curve demonstrate that R+D model could benefit for patients through the assessment of clinical utility. Conclusion: The radiomics model was able to predict the acute RP more effectively in comparison with the traditional dosimetry model. Especially the radiomics model based on the V30 Lung-PTV region was able to achieve a higher accuracy when compared to the other regions.

Abstract:Purpose: Although intensity-modulated radiotherapy (IMRT) is now a preferred option for conventionally fractionated RT in lung cancer, the commonly used cutoff values of the dosimetric constraints are still mainly derived from the data using three-dimensional conformal radiotherapy (3D-CRT). We aimed to compare the prediction performance among different dosimetric parameters for acute radiation pneumonitis (RP) in patients with lung cancer received IMRT.Methods: A total of 236 patients treated with IMRT were retrospectively reviewed in two independent groups of lung cancer from January 2014 to August 2018. The primary endpoint was grade 2 or higher acute RP (RP2). Dose metrics were generated from the bilateral lung volume outside GTV (VdoseG) and PTV (VdoseP). The associations of RP2 with clinical variables, dose-volume parameters and mean lung dose (MLD) were analyzed by univariate and multivariate logistic regression. The power of discrimination among each predictor was assessed by employing the bootstrapped area under the receiver operating characteristic curve (AUC), net reclassification improvement (NRI), and the integrated discrimination improvement (IDI).Results: Thirty-four (14.4%) out of 236 patients developed acute RP2 after the end of IMRT. The clinical parameters were identified as less important predictors for RP2 based on univariate and multivariate analysis. In both studied groups, the significance of association was more convincing in V20P, V30P, and MLDP (smaller Ps) than V5G and V5P. The largest bootstrapped AUC was identified for the V30P. We found a trend of better discriminating performance for the V20P and V30P, and MLDP than the V5G and V5P according to the higher values in AUC, IDI, and NRI analysis. To limit RP2 incidence less than 20%, the V30P cutoff was 14.5%.Conclusions: This study identified the intermediate dose-volume parameters V20P and V30P with better prediction performance for acute RP2 than low-dose metrics V5G and V5P. Among all studied predictors, the V30P had the best discriminating power, and should be considered as a supplement to the traditional dose constraints in lung cancer treated with IMRT.

Abstract:Objective Radiation pneumonitis (RP) is the major dose-limiting toxicity of thoracic radiotherapy. This study aimed to developed a dual-omics (single nucleotide polymorphisms, SNP and dosiomics) prediction model for symptomatic RP. Materials and methods The potential SNPs, which are of significant difference between the RP grade ≥ 3 group and the RP grade ≤ 1 group, were selected from the whole exome sequencing SNPs using the Fisher's exact test. Patients with lung cancer who received thoracic radiotherapy at our institution from 2009 to 2016 were enrolled for SNP selection and model construction. The factorization machine (FM) method was used to model the SNP epistasis effect, and to construct the RP prediction model (SNP-FM). The dosiomics features were extracted, and further selected using the minimum redundancy maximum relevance (mRMR) method. The selected dosiomics features were added to the SNP-FM model to construct the dual-omics model. Results For SNP screening, peripheral blood samples of 28 patients with RP grade ≥ 3 and the matched 28 patients with RP grade ≤ 1 were sequenced. 81 SNPs were of significant difference (P < 0.015) and considered as potential SNPs. In addition, 21 radiation toxicity related SNPs were also included. For model construction, 400 eligible patients (including 108 RP grade ≥ 2) were enrolled. Single SNP showed no strong correlation with RP. On the other hand, the SNP-SNP interaction (epistasis effect) of 19 SNPs were modeled by the FM method, and achieved an area under the curve (AUC) of 0.76 in the testing group. In addition, 4 dosiomics features were selected and added to the model, and increased the AUC to 0.81. Conclusions A novel dual-omics model by synergizing the SNP epistasis effect with dosiomics features was developed. The enhanced the RP prediction suggested its promising clinical utility in identifying the patients with severe RP during thoracic radiotherapy.

Abstract:Purpose Stereotactic body radiation therapy (SBRT) is a standard treatment for early primary lung cancer patients. However, there are few simple models for predicting the clinical outcomes of these patients. Our study analyzed the clinical outcomes, identified the prognostic factors, and developed prediction nomogram models for these patients. Materials and Methods We retrospectively analyzed 114 patients with primary lung cancer treated with SBRT from 2012 to 2020 at our institutions and assessed patient’s clinical outcomes and levels of toxicity. Kaplan–Meier analysis with a log-rank test was used to generate the survival curve. The cut-off values of continuous factors were calculated with the X-tile tool. Potential independent prognostic factors for clinical outcomes were explored using cox regression analysis. Nomograms for clinical outcomes prediction were established with identified factors and assessed by calibration curves. Results The median overall survival (OS) was 40.6 months, with 3-year OS, local recurrence free survival (LRFS), distant disease-free survival (DDFS) and progression free survival (PFS) of 56.3%, 61.3%, 72.9% and 35.8%, respectively, with grade 3 or higher toxicity rate of 7%. The cox regression analysis revealed that the clinical stage, immobilization device, and the prescription dose covering 95% of the target area (D95) were independent prognostic factors associated with OS. Moreover, the clinical stage, and immobilization device were independent prognostic factors of LRFS and PFS. The smoking status, hemoglobin (Hb) and immobilization device were significant prognostic factors for DDFS. The nomograms and calibration curves incorporating the above factors indicated good predictive accuracy. Conclusions SBRT is effective and safe for primary lung cancer. The prognostic factors associated with OS, LRFS, DDFS and PFS are proposed, and the nomograms we proposed are suitable for clinical outcomes prediction.

Abstract:Radiation pneumonitis (RP) that develops early (i.e., within 3 mo) (RP Early ) after completion of concurrent chemoradiation (cCRT) leads to treatment discontinuation and poorer survival for patients with stage III non–small cell lung cancer. Since no RP Early risk model exists, we explored whether published RP models and pretreatment 18 F-FDG PET/CT–derived features predict RP Early . Methods: One hundred sixty patients with stage III non–small cell lung cancer treated with cCRT and consolidative immunotherapy were analyzed for RP Early . Three published RP models that included the mean lung dose (MLD) and patient characteristics were examined. Pretreatment 18 F-FDG PET/CT normal-lung SUV featured included the following: 10th percentile of SUV (SUV P10 ), 90th percentile of SUV (SUV P90 ), SUV max , SUV mean , minimum SUV, and SD. Associations between models/features and RP Early were assessed using area under the receiver-operating characteristic curve (AUC), P values, and the Hosmer–Lemeshow test (pHL). The cohort was randomly split, with similar RP Early rates, into a 70%/30% derivation/internal validation subset. Results: Twenty (13%) patients developed RP Early . Predictors for RP Early were MLD alone (AUC, 0.72; P = 0.02; pHL, 0.87), SUV P10 , SUV P90, and SUV mean (AUC, 0.70–0.74; P = 0.003–0.006; pHL, 0.67–0.70). The combined MLD and SUV P90 model generalized in the validation subset and was deemed the final RP Early model (RP Early risk = 1/[1+e (– x ) ]; x = –6.08 + [0.17 x MLD] + [1.63 x SUV P90 ]). The final model refitted in the 160 patients indicated improvement over the published MLD-alone model (AUC, 0.77 vs. 0.72; P = 0.0001 vs. 0.02; pHL, 0.65 vs. 0.87). Conclusion: Patients at risk for RP Early can be detected with high certainty by combining the normal lung's MLD and pretreatment 18 F-FDG PET/CT SUV P90 . This refined model can be used to identify patients at an elevated risk for premature immunotherapy discontinuation due to RP Early and could allow for interventions to improve treatment outcomes.

Development and Validation of a Random Forest Model for Predicting Radiation Pneumonitis in Lung Cancer Patients Receiving Moderately Hypofractionated Radiotherapy: a Retrospective Cohort Study

Abstract:Background: Radiation pneumonitis (RP) is a type of toxicity commonly associated with thoracic radiation therapy. We sought to establish a random forest (RF) model and evaluate its ability to predict RP in patients with non-small cell lung cancer (NSCLC) receiving moderately hypofractionated radiotherapy (hypo-RT). Methods: A total of 106 patients with stage II-IVa NSCLC who received moderately hypofractionated helical tomotherapy (2.3-3.0 Gy/fraction) at Zhongshan Hospital were included. All enrolled patients were divided chronologically into the training (67 patients) and validation (39 patients) groups. Higher than or equal to grade 2 RP was defined as the end point. Logistic regression and RF models were established and compared using the receiver operating characteristic (ROC) and a confusion matrix in the training and validation groups.Results: The cumulative incidence of the end point was 25.4% and 17.9% in the training and validation groups, respectively. Logistic regression models were constructed by dosage parameters of total lungs, ipsilateral or contralateral lungs, respectively. ROC analysis revealed that the dosimetric factors of total lungs yielded a superior classification performance than did that of the ipsilateral or contralateral lungs [area under the curve (AUC) =0.920, AUC =0.701, and AUC =0.661, respectively]. Furthermore, the RF model yielded a better prediction capacity than did the traditional logistic model based on the dosimetric factors of the total lungs (accuracy: 88.06%; precision: 84.62%; sensitivity: 64.71%; specificity: 96.00%). Moreover, the RF identified mean lung dose [MLD; mean decrease gini (MDG) =5.74], V20 (MDG =4.62), and V35 (MDG =3.08) of total lungs as the most common primary differentiators of RP.Conclusions: Our RF model established based on the dosimetric parameters of the total lungs could accurately predict the RP risk in patients with NSCLC treated with moderately hypofractionated tomotherapy.

Abstract:Purpose Stereotactic body radiotherapy (SBRT) is an important treatment modality for lung cancer patients, however, tumor local recurrence rate remains some challenge and there is no reliable prediction tool. This study aims to develop a prediction model of local control for lung cancer patients undergoing SBRT based on radiomics signature combining with clinical and dosimetric parameters. Methods The radiomics model, clinical model and combined model were developed by radiomics features, incorporating clinical and dosimetric parameters and radiomics signatures plus clinical and dosimetric parameters, respectively. Three models were established by logistic regression (LR), decision tree (DT) or support vector machine (SVM). The performance of models was assessed by receiver operating characteristic curve (ROC) and DeLong test. Furthermore, a nomogram was built and was assessed by calibration curve, Hosmer-Lemeshow and decision curve. Results The LR method was selected for model establishment. The radiomics model, clinical model and combined model showed favorite performance and calibration (Area under the ROC curve (AUC) 0.811, 0.845 and 0.911 in the training group, 0.702, 0.786 and 0.818 in the validation group, respectively). The performance of combined model was significantly superior than the other two models. In addition, Calibration curve and Hosmer-Lemeshow (training group: P = 0.898, validation group: P = 0.891) showed good calibration of combined nomogram and decision curve proved its clinical utility. Conclusions The combined model based on radiomics features plus clinical and dosimetric parameters can improve the prediction of 1-year local control for lung cancer patients undergoing SBRT.

A critique of the NCHS weight for height standard.

Abstract:The use of the NCHS standard to assess weight-for-height in children is equivalent to expressing the child's weight and height, in the ratio W/Hp, as a fraction of the NCHS median W/Hp for age, where the power p of height changes with age. It falls from a value of 3 at birth to 1.5 at 18 months, and then rises to 3 again by 11 years. Its discontinuous pattern of change in the second six months of life leads to potentially misleading assessments of weight-for-height in very tall or very short children. Weight for height standards in general, and the NCHS standard in particular, should not be used to assess weight-for-height in young children. The index W/H2, expressed as a fraction of the NCHS median W/H2 for age, should be used instead. The American growth standard published by the National Center for Health Statistics (Hamill et al. 1978; 1979) differs from several other widely used national standards, e.g. those of Britain and Holland (Tanner et al. 1966; Van Wieringen et al. 1971) in containing a weight for height standard. National standards generally provide reference figures of weight and height for age, but only with the NCHS standard can obesity (or less likely in the Western World, protein-energy malnutrition) be assessed solely from measurements of weight and height. Ideally of course, obesity should be assessed from body fat skinfolds, but they are difficult to measure reproducibly and this approach is not recommended for routine screening (Owen, 1982). The reasons why some studies include a weight for height standard, while others do not, have never been clearly spelt out. There is certainly a need for a simple method to assess overweight or underweight in childhood, which a weight for height standard appears to satisfy. On the other hand, various authors have expressed dissatisfaction with the behavior of weight for height standards in general, and have made efforts to provide alternative indices with better properties (Dugdale, 1971; McClaren and Read, 1975; Rolland-Cachera et al. 1982; Cronk et al. 1982). The authors of the NCHS report (Hamill et al. 1979) themselves say that the weight for height standard assumes age-independence: "... a useful statistical assumption that is not quite true." Against this should be set the one big advantage of the weight for height standard it can be used when children's ages are either unknown or unreliable, as is often the case in the Developing World. Abstract:PurposeFactors prediction in the development of radiation pneumonitis (RP) remains unclear. A meta-analysis about this was performed.MaterialsArticles were searched in February 2012 from PubMed, EMBASE, Cochrane Library and CNKI (Chinese Journal Full-text Database) using the keywords “lung cancer,” “radiation pneumonitis” or “radiation lung injury.” The outcome was the RP incidence. We pooled the data using RevMan 5.1 software and tested the statistical heterogeneity.ResultsWe included the following factors: age, gender, weight loss, smoking history, complications, performance status, pre-radiation therapy (RT) pulmonary function, TNM, histological type, tumor location, pre-RT surgery, RT combined with chemotherapy (RCT), RT/RCT combined with amifostine, plasma end/pre-RT TGF-β1 ratio and irradiation volume. The significant risk factors for RP ≥ grade 2 were patients with chronic lung disease, tumor located in the middle or lower lobe, without pre-RT surgery, RCT, plasma end/pre-RT TGF-β1 ratio ≥1 and gross tumor volume (GTV). Following factors were identified significant for RP, including tumor located not in the upper lobe, smokers, combined with chronic lung diseases or diabetes mellitus, low pre-RT pulmonary function, RCT, RT/RCT without amifostine and plasma end/pre-RT TGF-β1 ratio ≥1. Dose-volume parameters included the average of mean lung dose (MLD) of disease lung, GTV and V5, V10 (≥34 %), V20 (≥25 %), V30 (≥18 %) of bilateral lung.ConclusionsMore attention should be paid to the levels of patients’ pulmonary function, plasma TGF-β1 and dose-volume histogram (DVH). Rigorous studies are needed to identify the relationship between the above-mentioned factors and RP ≥grade 1 or 3.

Abstract:Purpose To develop a deep learning model that combines CT and radiation dose (RD) images to predict the occurrence of radiation pneumonitis (RP) in lung cancer patients who received radical (chemo)radiotherapy. Methods CT, RD images and clinical parameters were obtained from 314 retrospectively-collected patients (training set) and 35 prospectively-collected patients (test-set-1) who were diagnosed with lung cancer and received radical radiotherapy in the dose range of 50 Gy and 70 Gy. Another 194 (60 Gy group, test-set-2) and 158 (74 Gy group, test-set-3) patients from the clinical trial RTOG 0617 were used for external validation. A ResNet architecture was used to develop a prediction model that combines CT and RD features. Thereafter, the CT and RD weights were adjusted by using 40 patients from test-set-2 or 3 to accommodate cohorts with different clinical settings or dose delivery patterns. Visual interpretation was implemented using a gradient-weighted class activation map (grad-CAM) to observe the area of model attention during the prediction process. To improve the usability, ready-to-use online software was developed. Results The discriminative ability of a baseline trained model had an AUC of 0.83 for test-set-1, 0.55 for test-set-2, and 0.63 for test-set-3. After adjusting CT and RD weights of the model using a subset of the RTOG-0617 subjects, the discriminatory power of test-set-2 and 3 improved to AUC 0.65 and AUC 0.70, respectively. Grad-CAM showed the regions of interest to the model that contribute to the prediction of RP. Conclusion A novel deep learning approach combining CT and RD images can effectively and accurately predict the occurrence of RP, and this model can be adjusted easily to fit new cohorts.
