Abstract:The positioning of the patient on the fracture table is critical for the successful reduction and operative fixation of intertrochanteric hip fractures. However, this manipulation is challenging with patients who have undergone amputations of their legs. A 97-year-old man presented to the emergency department with symptom of right hip pain following a mechanical fall. He had a below-knee amputation on his right leg following a traffic accident as a 19-year-old and had a below-knee patellar tendon bearing prosthesis fitted to his lower limb for mobility. Radiographs of his pelvis revealed a displaced intertrochanteric fracture of the right side femur. The patient was positioned on a fracture table, as in the standard procedure. The method of inverting the traction boot to accommodate the flexed knee and stump described by Al-Harthy could be used to provide traction and rotational control. Internal fixation was performed using a short femoral nail. Postoperatively, the patient could walk with full weight bearing using a prosthesis on his affected limb. The method of inverting the traction boot to accommodate the flexed knee and stump can be used safely and effectively to achieve and maintain fracture reduction during fixation of intertrochanteric fractures for patients with a below-knee amputated limb.
Improved anterolateral approach to treatment of terminal femoral fractures

Abstract:Objective To explore the clinical effects of internal fixation with plate by the improved anterolateral approach to treatment of terminal femoral fractures.Methods Internal fixation with plate by improved anterolateral approach was applied to 78 patients with complex comminuted bone fractures of terminal femora from August 2008 to January 2012.Results During the follow-up of 10 months to 2 years,73 patients reached complete healing without screw relaxation,plate rupture and malformation(e.g.limb shortening and knee varus or eversion);the other 5 ones who suffered from nonunion reached complete healing by the second stage bone grafting.The excellent and good rate of knee joint function was up to 91 %.Conclusion With simple procedure,reliable fixation,integral protection of knee extensor device,mini-operative damage and propitiousness to functional recovery of knee joint,the internal fixation with plate by the improved anterolateral approach is a relatively good therapeutic approach to the complex comminuted bone fractures of terminal femora.
Treatment of Comminuted Trochanteric Fractures and Non-Union of Trochanteric Osteotomy in Revision Total Hip Arthroplasty.

Abstract:OBJECTIVE:To explore the clinical efficacy of the treatment of comminuted trochanteric fractures and trochanteric osteotomy non-union in revision total hip arthroplasty with tension-band fixation.METHODS:A retrospective review of 295 revision total hip operations performed between 1992 and 1998 was undertaken. Twenty hips of 19 patients with comminuted fractures or nonunion of the greater trochanter were stabilized with tension band technique. Multiple 2.0 mm k-wires and tension-band wires were placed through the intact cortex distally and the abductor tendon proximally in the pattern "8".RESULTS:The average follow-up was 30 months. The Harris Hip Score improved on average from 45 preoperatively to 89 at follow-up. Sixteen hips with intra-operative trochanteric fracture through osteolytic bone and four hips with symptomatic trochanteric nonunion were approached with tension band fixation. Perioperative loss of fixation in one patient required a repeated surgery. The same fixation at the second operation achieved an uneventful healing. Two patients had a 2 cm proximal migration of one K-wire without loss of bony fixation. The trochanteric fractures healed with no further proximal wire migration. One patient had loss of fixation with trochanteric escape at 6 weeks post-operatively. The patient has abductor weakness with Trendleburg limp but without pain. On average, radiographic examination showed that healing occurred at 16.6 weeks postoperatively. Six patients developed grade 1 heterotopic ossification and two patients grade 3. All were asymptomatic. None of the 19 patients experienced a dislocation during the follow-up.CONCLUSIONS:Tension-band fixation for greater trochanter can enhance the success rate of revision total hip arthroplasty without a deficient abductor mechanism.
Neck of femur fracture fixation in a bilateral amputee: an uncommon condition requiring an improvised fracture table positioning technique

Abstract:While neck of femur fractures are common it is rare to see this injury in a bilateral leg amputee. Special consideration needs to be given to the management of these patients. We report the case of a 58-year-old man with bilateral leg amputation who presented to the emergency department with left hip pain following a fall. A fracture of the left neck of femur with extension into the femoral shaft was diagnosed. Internal fixation was planned with a dynamic hip screw. Standard fracture table setup, which allows for traction of the fractured limb and positioning of the contralateral limb such that anteroposterior and lateral X-rays can be obtained, was not possible in this case due to the amputations. We highlight considerations that need to be made in positioning a bilateral amputee for neck of femur fracture fixation and also highlight an improvised technique that can be utilised by other surgeons.

Abstract:Introduction: Closed anatomical reduction of per-trochanteric fractures requires 3-axis control of the orthopedic traction table (OTT) which is challenging in patients having suffered a previous ipsilateral Gritti-Stokes amputation. Case Report: A 52-year-old man, known by Gritti-Stokes amputated (GSA) 10 years before, was admitted to our trauma center after a fall from his height. We describe a simple method for positioning a previously ipsilateral GSA patient in an OTT to reduce displaced a femoral per-trochanteric fracture site and to maintain it during surgery. The proposed method attaches non-adhesive bands to the stump, reinforced by the wire guide package to increase rigidity and allow 3-axis control of the OTT. Conclusion: This method can be performed in a standard trauma operating room, it only uses materials that were planned to be used during surgery, and consequently, it does not increase operative costs. Abstract:We reviewed medical records of patients with unstable intertrochanteric fractures (AO/OTA type 31-A2, A3) treated with percutaneous non-contact bridging (NCB) plates and with gamma nails and compared operative data and outcomes. Fifty-two patients received NCB plates (27 males and 25 females; median age, 77.0 years) and 36 received gamma nails (15 males and 21 females; median age, 77.0 years). Fluoroscopy time in the NCB plate group was 34.0 s (IQR: 30.0, 37.5), and in the gamma nail group was 45.0 s (IQR: 39.0, 51.5, p < 0.001). Hospital stay in the NCB plate group was significantly shorter than the gamma nail group (15.0 days [IQR: 13.0, 16.5] vs. 17.0 days [IQR: 14.0, 19.0], respectively; p = 0.004). Harris hip scores were similar between the groups at 6 months and 1 year postoperatively. The percutaneous NCB plate provides satisfactory results for the treatment of unstable intertrochanteric fractures.

Abstract:Abstract Background The aim of this study was to compare the clinical results between double reverse traction repositor and traction table used for the treatment of unstable intertrochanteric femur fracture. Methods This retrospective study included 95 patients with AO/OTA 31- A2 and A3 proximal femur fracture, who underwent double reverse traction repositor or traction table facilitated Asian proximal femoral nail antirotation (PFNA-II) nailing. The demographics, duration of operation, blood loss, part loading time after surgery, the period of union of fracture, complication were assessed. Clinical and radiological outcomes were evaluated. Results There were no significant differences in respect to demographics and fracture characteristics. Duration of patient positioning and total operative time were significant longer in traction table group than that in double reverse traction repositor group(p<0.001). No differences were found intraoperative blood loss, part loading time after surgery, fracture healing time and Harris Hip Score between two groups. Conclusion When treating unstable intertrochanteric fractures, double reverse traction repositor is superior to tract table in respect to operative time and duration of patient position, despite an additional ipsilateral anterior superior iliac spine (ASIS)incision and drilling of ASIS and femur condyle.

Abstract:Displaced acetabular fractures with medial and cranial displacement of the femoral head commonly require an anterior approach for reduction and stabilization. Restoration of the femoral head to its native position under the reduced acetabular dome is a primary goal of surgery. We present a surgical technique for applying traction to the proximal femur using the Bookwalter retractor system during the repair of acetabular fractures when using an anterior approach. By placing traction in line with the femoral neck, the femoral head is moved to a more anatomical position allowing acetabular fracture fragments to be reduced unimpeded and the femoral head may be used as a reconstructive template. We review a case series of 116 patients treated using this technique and report the short- and long-term radiographic and clinical results of treatment.

Abstract:BACKGROUND Prior to the 1900s, femoral-shaft fractures were treated with various types of splinting. In 1907 and 1909, Steinmann and Kirschner, respectively, developed the first traction treatment modalities with the use of pins or wires under tension. A single pin is usually placed either at the distal femur or proximal tibia when applying skeletal traction for treating femoral fractures. Complications associated with skeletal traction include pin track infection and loosening,1 which may require repositioning of the pin and inefficient traction forces applied across the fracture. Pin fixation problems are particularly severe in patients with osteoporosis, which can be a relative contra-indication to external fixation and presents a challenge for any fixation technique.2 TECHNIQUE We propose a novel technique in applying skeletal traction. Two Denham pins are placed into the proximal tibia 2.5 cm posterior to and 2.5 cm distal to tibial tubercle (Fig. 1). The pins are then connected with a Hoffmann external flexator connector producing a stable construct. Traction is then applied to both pins through free weights attached to a pulley at the end of the bed (Fig. 2). This technique was used effectively in elderly osteoporotic patients who had sustained femoral supracondylar fragility fractures and were not fit for operative fixation of the fractures (Fig. 3). There was no incidence of pin loosening or pin-site infection noted.

Abstract:Abstract Background Intertrochanteric fracture is a common fracture suffered by elderly patients. Total hip arthroplasty (THA) is regarded as a salvage operation to restore hip joint function after fixation failure, which remains somewhat controversial due to some clinical potential issues. Methods 18 elderly patients (average age 70.3 years) each with intertrochanteric fracture fixation failure treated with THA between September 2013 and October 2016 were retrospectively analyzed. Internal fixation treatments involved 5 patients who had received proximal femoral nail anti-rotation, 7 who received locking proximal femur plates and 6 who received dynamic hip screws. All patients were treated with THA using biological acetabular prosthesis and hip arthroplasty (HA) coating skillet femoral prosthesis, with the greater trochanter fixed using wire or steel when necessary. Patients’ Harris scores pre- and post-treatment, SF-36 Health Questionnaire score and digital radiology (DR) were used for joint prostheses initial stability and survival evaluation. Results 15 patients completed follow-up periods ranging between 19 and 54 months (mean 26.2 months; 1 patient died from a pulmonary embolism, 1 patient died from pulmonary heart disease 1 year after surgery and 1 patient withdrew for personal reasons). There were no joint infections, periprosthetic fractures or dislocations. The average Harris score increased significantly, from 32.68 ± 12.04 points before surgery to 91.08 ± 5.9 points at 24 months post-treatment. SF-36 scores were significantly increased. Conclusion THA as salvage treatment for failed internal fixation of intertrochanteric femoral fractures in elderly patients significantly reduced hip pain and restored joint function, and early clinical outcomes were satisfactory.

Abstract:Traction tables are used in numerous procedures about the hip and femur, including fracture fixation, hip arthroscopy, and less invasive arthroplasty. The use of a traction table is not without risks, however, and significant complications have been described, including injury to the perineal integument and soft tissues, neurologic impairment, and iatrogenic compartment syndrome of the well leg. The orthopaedic surgeon who uses a traction table for the surgical management of femur fracture must be familiar with the associated potential dangers and risks and must develop a plan to avoid traction table-associated complications, such as use of a radiolucent flat-top operating table for obese patients, adequate patient positioning, and the minimum possible surgical time.

Abstract:The treatment of subtrochanteric fractures is a challenge for orthopaedic trauma surgeons. Three positions have been described previously: supine on a fracture table, supine on a flat radiolucent table, and the lateral decubitus position on a flat radiolucent table. Each one has its advantages and limitations. In this article we describe a prone position for intramedullary nailing of subtrochanteric femoral fractures. This position has the advantages including: 1) an easy approach to reduce and maintain the reduction of fracture by adjusting only the leg plate on injured side, 2) perfect intraoperation fluoroscopic imaging on both anteroposterior view and lateral view, and 3) an easy approach to establish an appropriate entry point even in obese patients.

Abstract:We present the case of an 80-year-old Malaysian gentleman who sustained a displaced intracapsular neck of femur (NOF) fracture and underwent a modified SPAIRE (Sparing Piriformis and Internus, Repair Externus) approach for total hip replacement (THR). The standard approach used in our hospital for THR in NOF fractures is the modified Hardinge approach to the hip. We used this modified SPAIRE approach as this patient lives in a 'Mahjong' center and always sits cross-legged on the floor. Therefore, he is at increased risk of an anterior dislocation. This approach is a modification of the standard SPAIRE approach popularized by the Exeter Hip Unit, United Kingdom. In this report, we describe the modification of the SPAIRE approach that has not been described before and the outcome for this patient.

Abstract:PurposeFractures of the lower extremity, particularly of the femur and acetabulum, may be difficult to immobilize with splinting alone. These injuries may be best stabilized with the application of various types of skeletal traction. Often, traction is applied percutaneously in an emergent setting, making the knowledge of both superficial and deep anatomy crucial to successful placement.MethodsReview was performed via PubMed search as well as referencing the Orthopaedic literature. Relevant articles to the anatomy of the knee, ankle and calcaneus as they pertain to traction placement were referenced in compiling the optimal recommendations for traction placement.ConclusionBy palpating and marking superficial landmarks and observing specific anatomic relationships, safe application of traction pins can be performed while minimizing iatrogenic injury to vital anatomic structures, and avoiding intra-articular placement which could potentially lead to joint infection.

Abstract:Objective: To compare two positioning approaches in the surgical treatment of unstable intertrochanteric femoral fractures fixed by proximal femoral nailing, the supine versus lateral decubitus position Methodology: This randomized prospective comparative study on 26 patients with unstable intertrochanteric fractures was carried out from January 2020 and June 2022. We randomly divided patients into two groups: group A (13 patients) were operated using the traction table in the supine position for implant insertion, and group B (13 patients) were operated using the lateral decubitus position. We compared both groups regarding the setup time, operative time, tip-to-apex distance, collodiaphyseal angle, time for fluoroscopic time exposure, and radiologic reduction of the fracture using Chang criteria. Results: The setup, fluoroscopic exposure, and surgical times were shorter in the lateral decubitus group with statistical significance. The collodiaphyseal angles, the tip-to-apex distance, and the radiological fracture reduction were not significantly different between two groups. Conclusions: The lateral decubitus positioning is superior to the supine position in setup, fluoroscopic exposure, and surgical times

Abstract:PurposeAugmenting fracture fixation with intramedullary fibular graft has seen several applications such as in comminuted proximal humerus fractures, and femoral neck and shaft nonunion. The purpose of this study was to report the first case series demonstrating the novel application of an intramedullary fibular allograft strut and dual locking plates on complicated periprosthetic fracture of distal femur.MethodsThe patient demographics, weight-bearing status, and radiographic and clinical outcomes of six patients, who were treated for periprosthetic distal femur fractures using an intramedullary fibular allograft strut and dual locking plate, between January 2018 and April 2020 were analyzed.ResultsPostoperative range of knee motion showed median 10' (range 10–20) of flexion contracture and 90' (range 80–110) of further flexion. Postoperative joint function was assessed according to the Kolmert functional criteria, with "excellent to good" in angular deformity and "Good to fair" result in range of motion and pain. RUSH score was assessed, and median score was 24 (range 18–26). All patients were able to ambulate without assistance 6 months after surgery.ConclusionIn summary, the application of fibular strut graft and dual locking plate might be an option in complicated distal femur fracture above total knee arthroplasty. Albeit with an available number, such application showed promise as an intramedullary and cortical support for both reduction and maintenance of correction alignment, length, and prevention of further angular deformity.Level of evidence IV: Surgical technique and Cases-series.

Abstract:Lactose/H+ symport in Escherichia coli is catalysed by a hydrophobic transmembrane protein encoded by the lacY gene that has been purified to homogeneity, reconstituted into proteoliposomes and shown to be completely functional as a monomer. Circular dichroic studies and hydropathy profiling of the amino-acid sequence of this 'lac' permease suggest a secondary structure in which the polypeptide consists of 12 hydrophobic segments in alpha-helical conformation that traverse the membrane in zig-zag fashion connected by shorter, hydrophilic domains with most of the charged residues and many of the residues commonly found in beta-turns. Support for certain general aspects of the model has been obtained from other biophysical studies, as well as biochemical, immunological and genetic approaches. Oligonucleotide-directed, site-specific mutagenesis is currently being utilized to probe the structure and function of the permease. Application of the technique provides an indication that Arg302 (putative helix IX), His322 (putative helix X) and Glu325 (putative helix X) may be sufficiently close to hydrogen-bond and that these residues play a critical role in lactose-coupled H+ translocation, possibly as components of a charge-relay type of mechanism. In contrast, Cys residues, which were long thought to play a central role in the mechanism of lactose/H+ symport, do not appear to be involved in either substrate binding or H+ translocation.
