Abstract:There are a very few people who have the ability to "see" the surroundings by the echoes, which is called echolocation. The study of the brain mechanism of echolocation can not only help to improve the blind assistance device, but also provides a window into the research of brain's plasticity. In this paper, we developed a wearable system to transform the spatial information captured by camera into a voice description and fed it back to blind users which is inspired by echolocation. After our online virtual scene training, users can easily discriminate object location in the camera's view, motion of the objects, even shape of the objects. Compared with natural echolocation, it's easier to learn and be applied in daily life. In addition, the device achieves high spacial resolution. In this study, two trained blind subjects and two non-trained sighted subjects were tested by using functional Magnetic Resonance Imaging (fMRI). We obtain the fMRI images of the subjects' brain activity when they were listening to the sound of the wearable prototype. Intriguingly, we find that after training with the blind assistance system, the blind' visual area of the brain have been activated when they are dealing with the acoustic feedback from the device.

Abstract:ABSTRACTIn this paper we present a novel concept of hybrid auditory feedback in mobility assistance for people with visual disabilities in indoor environment. Hybrid auditory feedback is a gradual con-version of sound from speech-only to non-speech (i.e., spearcons) based on the sound repetitiveness and the users' frequency of the travelled route. Using a within-subject design, eight participants carried out a task using a mobility assistant application and followed a same route for few days. Preliminary results suggest that hybrid sounds in auditory feedback are more effective than non-speech and are pleasant compared to speech-only.

Abstract:Spatial hearing relies on the encoding of perceptual sound location cues in space. It is critical for communicating in background noise, and understanding where sounds are coming from (sound localization). Although there are some monoaural spatial hearing cues (i.e., from one ear), most of our spatial hearing skills require binaural hearing (i.e., from two ears). Cochlear implants (CIs) are often the most appropriate rehabilitation for individuals with severe-to-profound hearing loss, with those aged 18 years of age and younger typically receiving bilateral implants (one in each ear). As experience with bilateral hearing increases, individuals tend to improve their spatial hearing skills. Extensive research demonstrates that training can enhance sound localization, speech understanding in noise, and music perception. The BEARS (Both Ears) approach utilizes Virtual Reality (VR) games specifically designed for young people with bilateral CIs to train and improve spatial hearing skills. This paper outlines the BEARS approach by: (i) emphasizing the need for more robust and engaging rehabilitation techniques, (ii) presenting the BEARS logic model that underpins the intervention, and (iii) detailing the assessment tools that will be employed in a clinical trial to evaluate the effectiveness of BEARS in alignment with the logic model.

Abstract:Older children and teenagers with bilateral cochlear implants often have poor spatial hearing because they cannot fuse sounds from the two ears. This deficit jeopardizes speech and language development, education, and social well-being. The lack of protocols for fitting bilateral cochlear implants and resources for spatial-hearing training contribute to these difficulties. Spatial hearing develops with bilateral experience. A large body of research demonstrates that sound localisation can improve with training, underpinned by plasticity-driven changes in the auditory pathways. Generalizing training to non-trained auditory skills is best achieved by using a multi-modal (audio-visual) implementation and multi-domain training tasks (localisation, speech-in-noise, and spatial music). The goal of this work was to develop a package of virtual-reality games (BEARS, Both EARS) to train spatial hearing in young people (8-16 years) with bilateral cochlear implants using an action-research protocol. The action research protocol used formalized cycles for participants to trial aspects of the BEARS suite, reflect on their experiences, and in turn inform changes in the game implementations. This participatory design used the stakeholder participants as co-creators. The cycles for each of the three domains (localisation, spatial speech-in-noise, and spatial music) were customized to focus on the elements that the stakeholder participants considered important. The participants agreed that the final games were appropriate and ready to be used by patients. The main areas of modification were: the variety of immersive scenarios to cover age range and interests, the number of levels of complexity to ensure small improvements were measurable, feedback, and reward schemes to ensure positive reinforcement, and an additional implementation on an iPad for those who had difficulties with the headsets due to age or balance issues. The effectiveness of the BEARS training suite will be evaluated in a large-scale clinical trial to determine if using the games lead to improvements in speech-in-noise, quality of life, perceived benefit, and cost utility. Such interventions allow patients to take control of their own management reducing the reliance on outpatient-based rehabilitation. For young people, a virtual-reality implementation is more engaging than traditional rehabilitation methods, and the participatory design used here has ensured that the BEARS games are relevant.
Auditory Feedback for Standing Balance Improvement in Virtual Reality

Abstract:Virtual Reality (VR) users often experience postural instability, i.e., balance problems, which could be a major barrier to universal usability and accessibility for all, especially for persons with balance impairments. Prior research has confirmed the imbalance effect, but minimal research has been conducted to reduce this effect. We recruited 42 participants (with balance impairments: 21, without balance impairments: 21) to investigate the impact of several auditory techniques on balance in VR, specifically spatial audio, static rest frame audio, rhythmic audio, and audio mapped to the center of pressure (CoP). Participants performed two types of tasks - standing visual exploration and standing reach and grasp. Within-subject results showed that each auditory technique improved balance in VR for both persons with and without balance impairments. Spatial and CoP audio improved balance significantly more than other auditory conditions. The techniques presented in this research could be used in future virtual environments to improve standing balance and help push VR closer to universal usability.

Abstract:We developed a novel virtual reality [VR] platform with 3-dimensional sounds to help improve sensory integration and visuomotor processing for postural control and fall prevention in individuals with balance problems related to sensory deficits, such as vestibular dysfunction (disease of the inner ear). The system has scenes that simulate scenario-based environments. We can adjust the intensity of the visual and audio stimuli in the virtual scenes by controlling the user interface (UI) settings. A VR headset (HTC Vive or Oculus Rift) delivers stereo display while providing real-time position and orientation of the participants' head. The 3D game-like scenes make participants feel immersed and gradually exposes them to situations that may induce dizziness, anxiety or imbalance in their daily-living.

Abstract:Psychiatric News reported several deaths of psychiatrists at the hands of their patients in 1982. This year a psychiatry resident was murdered by a patient in her office. Two years ago an intern was the fatal victim of a former psychiatric patient as he worked in a Kansas City emergency room. These striking reports do little to describe accurately the incidence and severity of assaults in hospitals. There is a need for valid, reliable data concerning potential dangers to psychiatrists, other medical staff, and patients in medical care settings. It is important to know the baselines from which to measure deviations in assault incidence, to discuss what is being done to whom, and to prevent and manage such situations. Unfortunately, most common sense approaches to this problem have limited usefulness. Bromides such as "paranoids are more dangerous" or "female staff hardly ever get assaulted" may have some merit in certain situations; however, our group is now fairly certain that one should put only very limited credence in any of them. One reason for the limited value of these sayings has to do with the great range of health care settings in which violence may occur. In addition, different ways of defining and reporting assaultive incidents significantly alter the form and content of one's results. Recent studies have reported rates of significant assault as low as 25 cases per 27,000 patients per ten years. I Most studies have found a higher incidence. 2 -4 In a large survey of clinicians, one of us (W.H.R.) found data to agree with other investigators that up to one-half of all psychiatrists experience at least one serious assault during their careers. S Tardiff and Sweillam 4 report that about 7.5 percent of patients in the New York State hospital system have committed at least one assault in the hospital. Finally, unpublished data from the Veterans Administration indicates some 12,000 assaultive incidents were reported within the system over a recent five-year period.

Abstract:Functional upper-limb prosthetic training can improve users performance in controlling prostheses and has been incorporated into occupational therapy for individuals in need. In recent years, virtual reality (VR) and augmented reality (AR) technologies have been shown to be promising avenues to improve the convenience of rehabilitative prosthesis training systems. However, it is uncertain if the comprehensive efficacy and effectiveness of VR or AR assistive tools are adequate compared to conventional prosthetic tools and if not, whether enhancements can be made through incorporation of other technical paradigms. This work first presents a mixed reality system we developed for prosthesis control and training. Five able-bodied subjects are involved to perform three-dimensional object manipulation tasks in analogous AR and VR environments. Multiple evaluation metrics are applied to assess subjects performances within the two paradigms. Based on the comparative analysis, we find that VR-based environment promotes more efficient motion along with higher task completion rate and path efficiency while AR paradigm allows subjects to perform motor tasks with shorter time consumed. Another study is conducted to evaluate the efficiency and feasibility of AR-facilitated prosthesis control system compared to that in real-world and if any technical additions can be applied to improve the AR-based system. Three able-bodied subjects were engaged in the experiment to perform object manipulation tasks in a) physical environment, b) AR-without-bypass environment, and c) AR-with-bypass environment. Based on the results obtained from the assessment, we conclude that while our AR-based system modestly lags behind the effectiveness of physical systems, the study conducted using a bypass prosthesis suggests that AR system has the potential to improve the efficacy of prosthesis control.

Abstract:Video-capture virtual reality (VR) systems are gaining popularity as intervention tools. To date, these platforms offer visual and audio feedback but do not provide haptic feedback. We contend that adding haptic feedback may enhance the quality of intervention for various theoretical and empirical reasons. This study aims to integrate haptic-tactile feedback into a video capture system (GX VR), which is currently applied for rehabilitation. The proposed multi-modal system can deliver audio-visual as well as vibrotactile feedback. The latter is provided via small vibratory discs attached to the patient's limbs. This paper describes the system, the guidelines of its design, and the ongoing usability study.

Abstract:Movement sonification has emerged as a promising approach for rehabilitation and motion control. Despite significant advancements in sensor technologies, challenges remain in developing cost-effective, user-friendly, and reliable systems for gait detection and sonification. This study introduces a novel wearable personalised sonification and biofeedback device to enhance movement awareness for individuals with irregular gait and posture. Through the integration of inertial measurement units (IMUs), MATLAB, and sophisticated audio feedback mechanisms, the device offers real-time, intuitive cues to facilitate gait correction and improve functional mobility. Utilising a single wearable sensor attached to the L4 vertebrae, the system captures kinematic parameters to generate auditory feedback through discrete and continuous tones corresponding to heel strike events and sagittal plane rotations. A preliminary test that involved 20 participants under various audio feedback conditions was conducted to assess the system's accuracy, reliability, and user synchronisation. The results indicate a promising improvement in movement awareness facilitated by auditory cues. This suggests a potential for enhancing gait and balance, particularly beneficial for individuals with compromised gait or those undergoing a rehabilitation process. This paper details the development process, experimental setup, and initial findings, discussing the integration challenges and future research directions. It also presents a novel approach to providing real-time feedback to participants about their balance, potentially enabling them to make immediate adjustments to their posture and movement. Future research should evaluate this method in varied real-world settings and populations, including the elderly and individuals with Parkinson's disease.

Abstract:Physical rehabilitation is essential to recovery from joint replacement operations. As a representation, total knee arthroplasty (TKA) requires patients to conduct intensive physical exercises to regain the knee's range of motion and muscle strength. However, current joint replacement physical rehabilitation methods rely highly on therapists for supervision, and existing computer-assisted systems lack consideration for enabling self-monitoring, making at-home physical rehabilitation difficult. In this paper, we investigated design recommendations that would enable self-monitored rehabilitation through clinical observations and focus group interviews with doctors and therapists. With this knowledge, we further explored Virtual Reality(VR)-based visual presentation and supplemental haptic motion guidance features in our implementation VReHab, a self-monitored and multimodal physical rehabilitation system with VR and vibrotactile and pneumatic feedback in a TKA rehabilitation context. We found that the third point of view real-time reconstructed motion on a virtual avatar overlaid with the target pose effectively provides motion awareness and guidance while haptic feedback helps enhance users' motion accuracy and stability. Finally, we implemented \systemname to facilitate self-monitored post-operative exercises and validated its effectiveness through a clinical study with 10 patients.

Abstract:Human maintain their body balance by sensorimotor controls mainly based on information gathered from vision, proprioception and vestibular systems. When there is a lack of information, caused by pathologies, diseases or aging, the subject may fall. In this context, we developed a system to augment information gathering, providing the subject with warning audio-feedback signals related to his/her equilibrium. The system comprises an inertial measurement unit (IMU), a data processing unit, a headphone audio device and a software application. The IMU is a low-weight, small-size wireless instrument that, body-back located between the L2 and L5 lumbar vertebrae, measures the subject's trunk kinematics. The application drives the data processing unit to feeding the headphone with electric signals related to the kinematic measures. Consequently, the user is audio-alerted, via headphone, of his/her own equilibrium, hearing a pleasant sound when in a stable equilibrium, or an increasing bothering sound when in an increasing unstable condition. Tests were conducted on a group of six older subjects (59y-61y, SD = 2.09y) and a group of four young subjects (21y-26y, SD = 2.88y) to underline difference in effectiveness of the system, if any, related to the age of the users. For each subject, standing balance tests were performed in normal or altered conditions, such as, open or closed eyes, and on a solid or foam surface The system was evaluated in terms of usability, reliability, and effectiveness in improving the subject's balance in all conditions. As a result, the system successfully helped the subjects in reducing the body swaying within 10.65%-65.90%, differences depending on subjects' age and test conditions.

Using Virtual Reality to Assess Auditory Performance

Abstract:Auditory assessments aim to quantify auditory abilities that impact real-world listening, such as replicating patient complaints of listening difficulty in specific situations or understanding how auditory systems perform in natural scenes. This goal suggests that assessments should be as realistic as possible. However, the need to quantify specific abilities requires greater experimental control than achievable in most natural settings. This fundamental trade-off between realism and control has existed since perception science began. But emerging technologies in virtual and augmented reality promise to bring the real world into the lab–or vice versa–and break this trade-off. Implemented correctly, these technologies offer new approaches to quasi-realistic auditory assessment with improved multisensory consistency, more natural tasks, and greater participant engagement that will make assessment more reliable, valid, and fun. Traditional laboratory-based testing remains the gold standard for auditory assessment in the clinic and in basic research. But several features of this approach limit the real-world validity of laboratory tasks. First, these tasks are rife with multisensory conflict. Booth walls and desktop displays of the testing environment are visually distracting and inconsistent with the auditory stimuli listeners are asked to judge, whereas real-world listening involves a consistent multisensory context that can reduce uncertainty and support task performance even when non-auditory cues are not explicitly informative about the task. Removing that support increases demands on participants' memory and mental imagery—cognitive skills that can confound attempts to measure sensory abilities. Even in explicitly multisensory studies, the spatial arrangement of auditory and visual stimuli are often poorly matched: Visuals may appear on a desktop display, while sounds appear inside the head or from a distant loudspeaker. In contrast, real-world auditory and visual features are aligned in all directions and remain so as a person moves around, producing a natural experience of immersion within the sensory world. Second, laboratory-based tasks are behaviorally unrealistic, typically presenting explicit feedback about an isolated task dimension such as localization or identification. But real-world listening combines multiple task dimensions and implicit feedback. Conversation in a crowded restaurant, for example, requires simultaneous localization, segregation, and identification of target speech, along with the evaluation of facial cues that might indicate which talker asked an important question but not the content of the question itself. Virtual reality (VR) and other immersive technologies have been gaining attention as new modes for computer-based entertainment, gaming, communication, and work. VR head-mounted displays (HMDs) present video and audio content (typically a simulated 3D environment) customized for the individual user. Low-latency tracking of the user's position in space is used to update the display in close to real time, allowing direct interaction with immersive multisensory experiences. At Boys Town National Research Hospital's Spatial Hearing Laboratory, we have been using commercial HMDs to explore the benefits of VR for auditory assessment. The focus has been on enhancing auditory assessment rather than on assessing the performance of virtual experiences (e.g., 3D audio) themselves. As such, our approach has been to add immersive multisensory elements without giving up the control made possible by existing unisensory methods and apparatus. For computer-based assessment, this is accomplished by software commands sent to and from a dedicated process that controls VR-based interactions with the participant. Across multiple investigations, we have noted several exciting potential benefits: Improving Multisensory Consistency. Replacing the sound booth with even a simple virtual scene can eliminate distracting visual elements and provide better consistency between visual and auditory elements, which can help participants understand the auditory stimulus even when visual cues are not directly informative. Working in my lab at Vanderbilt University, Travis Moore, AuD, PhD, conducted a binaural localization study in which listeners indicated the direction of a perceived sound by turning their heads in a simple immersive scene—a circular array of 360 balloons (one per degree azimuth). On each trial, they turned to face the balloon closest to where they heard a target sound and pressed a button to pop the selected balloon with a simple silent animation. Supporting Tremor Rehabilitation Using Optical See-Through Augmented Reality Technology

Abstract:Tremor is a movement disorder that significantly impacts an individual's physical stability and quality of life, and conventional medication or surgery often falls short in providing a cure. Rehabilitation training is, therefore, used as an auxiliary method to mitigate the exacerbation of individual tremors. Video-based rehabilitation training is a form of therapy that allows patients to exercise at home, reducing pressure on rehabilitation institutions' resources. However, it has limitations in directly guiding and monitoring patients' rehabilitation, leading to an ineffective training effect. This study proposes a low-cost rehabilitation training system that utilizes optical see-through augmented reality (AR) technology to enable tremor patients to conduct rehabilitation training at home. The system provides one-on-one demonstration, posture guidance, and training progress monitoring to achieve an optimal training effect. To assess the system's effectiveness, we conducted experiments comparing the movement magnitudes of individuals with tremors in the proposed AR environment and video environment, while also comparing them with standard demonstrators. Participants wore a tremor simulation device during uncontrollable limb tremors, with tremor frequency and amplitude calibrated to typical tremor standards. The results showed that participants' limb movement magnitudes in the AR environment were significantly higher than those in the video environment, approaching the movement magnitudes of the standard demonstrators. Hence, it can be inferred that individuals receiving tremor rehabilitation in the AR environment experience better movement quality than those in the video environment. Furthermore, participant experience surveys revealed that the AR environment not only provided a sense of comfort, relaxation, and enjoyment but also effectively guided them throughout the rehabilitation process.

The vestibular implant: A probe in orbit around the human balance system

Abstract:The primary goal of the vestibular implant is to restore the vestibular function in patients with a disabling bilateral vestibular loss for whom there is currently no available treatment. The prototype developed by our team is a hybrid system consisting of a modified cochlear implant incorporating additional vestibular electrodes. Therefore, in addition of delivering sound information it is also capable of delivering motion information to the central nervous system using electrical stimulation. To date, thirteen patients have been implanted with such vestibular implant prototypes. For ethical reasons, only deaf ears were implanted and all patients experienced a clinical benefit from the hearing rehabilitation. The recent demonstration of partial restoration of the vestibulo-ocular and the vestibulo-collic reflexes in implanted patients suggests that gaze stabilization and postural control, fundamental functions of the balance system, can be artificially restored using a vestibular implant. This allows us to glimpse a useful clinical application in a near future. In parallel, we show how the vestibular implant provides a unique opportunity to explore the integration of the vestibular sensory input into the multisensory, multimodal balance system in humans, since it is able to selectively stimulate the vestibular system.

Abstract:Abstract Background Hand amputation can have a truly debilitating impact on the life of the affected person. A multifunctional myoelectric prosthesis controlled using pattern classification can be used to restore some of the lost motor abilities. However, learning to control an advanced prosthesis can be a challenging task, but virtual and augmented reality (AR) provide means to create an engaging and motivating training. Methods In this study, we present a novel training framework that integrates virtual elements within a real scene (AR) while allowing the view from the first-person perspective. The framework was evaluated in 13 able-bodied subjects and a limb-deficient person divided into intervention (IG) and control (CG) groups. The IG received training by performing simulated clothespin task and both groups conducted a pre- and posttest with a real prosthesis. When training with the AR, the subjects received visual feedback on the generated grasping force. The main outcome measure was the number of pins that were successfully transferred within 20 min (task duration), while the number of dropped and broken pins were also registered. The participants were asked to score the difficulty of the real task (posttest), fun-factor and motivation, as well as the utility of the feedback. Results The performance (median/interquartile range) consistently increased during the training sessions (4/3 to 22/4). While the results were similar for the two groups in the pretest, the performance improved in the posttest only in IG. In addition, the subjects in IG transferred significantly more pins (28/10.5 versus 14.5/11), and dropped (1/2.5 versus 3.5/2) and broke (5/3.8 versus 14.5/9) significantly fewer pins in the posttest compared to CG. The participants in IG assigned (mean ± std) significantly lower scores to the difficulty compared to CG (5.2 ± 1.9 versus 7.1 ± 0.9), and they highly rated the fun factor (8.7 ± 1.3) and usefulness of feedback (8.5 ± 1.7). Conclusion The results demonstrated that the proposed AR system allows for the transfer of skills from the simulated to the real task while providing a positive user experience. The present study demonstrates the effectiveness and flexibility of the proposed AR framework. Importantly, the developed system is open source and available for download and further development.

An Ontological Approach to XBRL Financial Statement Reporting

Abstract:As a standard for the exchange, transmission, and reporting of accounting and financial data, XRBL goes a long way toward standardization. However, as the extent of contemporary financial markets is now global, XBRL reporting must transcend or accommodate differences in reporting standards. While XBRL has been of great help in standardizing the presentation of the U.S. GAAP and IASB standards, reconciling meaning between these standards is still a manual and error-prone affair. As a wide variety of stakeholders, spanning countries and cultures, will likely take part in digesting XBRL-formatted financial reports; how will they participate at an appropriate level that is meaningful for them? This paper focuses on an ontological approach towards solving this problem by offering an XBRL ontology architecture for translation between XBRL formats. The architecture is explained, evaluation criteria offered, and future research towards realizing artifacts which use this architecture are proffered.
Augmented Reality Prosthesis Training Setup for Motor Skill Enhancement

Abstract:Adjusting to amputation can often time be difficult for the body. Post-surgery, amputees have to wait for up to several months before receiving a properly fitted prosthesis. In recent years, there has been a trend toward quantitative outcome measures. In this paper, we developed the augmented reality (AR) version of one such measure, the Prosthetic Hand Assessment Measure (PHAM). The AR version of the PHAM - HoloPHAM, offers amputees the advantage to train with pattern recognition, at their own time and convenience, pre- and post-prosthesis fitting. We provide a rigorous analysis of our system, focusing on its ability to simulate reach, grasp, and touch in AR. Similarity of motion joint dynamics for reach in physical and AR space were compared, with experiments conducted to illustrate how depth in AR is perceived. To show the effectiveness and validity of our system for prosthesis training, we conducted a 10-day study with able-bodied subjects (N = 3) to see the effect that training on the HoloPHAM had on other established functional outcome measures. A washout phase of 5 days was incorporated to observe the effect without training. Comparisons were made with standardized outcome metrics, along with the progression of kinematic variability over time. Statistically significant (p<0.05) improvements were observed between pre- and post-training stages. Our results show that AR can be an effective tool for prosthesis training with pattern recognition systems, fostering motor learning for reaching movement tasks, and paving the possibility of replacing physical training.

Abstract:Sensory augmentation using spatial sound presented in augmented-reality (AR) can assist people with low vision or blindness in navigating their environment (Katz et al., 2012). Nonetheless, in many situations, the poor quality of the binaural sound rendered using current tool sets limits the potential capability of the assistive technology. In particular, acoustic environments with near-field sources and reflections pose significant challenges. In this work, we provide a reference binaural room impulse response (BRIR) dataset with near-field sources and reflections and an associated shorelining psychophysical task that is useful for the evaluation of AR spatial audio. The dataset consists of 12∼small loudspeakers arranged on a 3-by-4 grid in a complex acoustic environment. BRIR measurements are recorded using the Head and Torso Simulator (HATS) for 17∼different receiver positions with a 5o angular resolution. Room impulse response measurements are also recorded using the Eigenmike for each of the 17 receiver positions. Using the Razer Anzu smart glasses to render the binaural AR spatial audio, we compare psychophysical performance on the shorelining navigation task using the recorded dataset and various existing binaural AR tool sets.
