Abstract:Introduction: The superior labial frenum corresponds to a thin and triangular anatomical structure that has its formation during the third month of intrauterine life, located from the tubercle of the upper lip to the alveolar process. Abnormal insertion of the roof-labial frenulum is usually indicated for surgical removal. If the surgical procedure is not performed, the frenum causes instability, the main problem being the diastema between the incisors, which many times cannot be treated only by orthodontics. The purpose of correction of the upper labial frenum consists of eliminating excess free tissue between the teeth, in addition to the purpose of improving aesthetics. Objective: To describe a case report of surgical treatment of the upper labial frenum performed on a pediatric patient at a Dental Specialties Center. Case report: Female patient, 11 years old, referred by the orthodontist with indication for surgical treatment of the upper labial frenum. On intraoral physical examination, the presence of persistent roof-labial frenum and diastema between the upper central incisors was observed. In view of the data obtained in the anamnesis and physical examination, we opted for the frenotomy technique with the repositioning of the brake. Intraoral antisepsis and facial antisepsis were performed, followed by terminal infiltrative anesthesia in the anterior superior alveolar nerves, bilaterally. Two oblique incisions were made close to the insertion of the roof-labial frenum and then the repositioning of the. Friction movements were performed between the incisors and simple stitches were used for suturing. After two weeks, the patient returned and the presence of tissue remnant was observed, which was removed using the same anesthetic technique and with the aid of Metzenbaum scissors, without the need for a new suture. Conclusion: Surgical treatments for labial frenum present excellent results and should be chosen according to the individual needs of each patient, always evaluating the risks, benefits and advantages and disadvantages.

Abstract:During or after the orthodontic closure, persistent diastemas are frequently the result of a high frenum attachment. A labial frenectomy is a complete removal of the frenum attachment, which typically attaches to the space between the upper two anterior teeth and the centre of the upper lip. It might be required if there is space between the teeth due to a frenulum positioned too high on the gums. Many surgical technique modifications, including Miller's technique, Z-plasty, and V-Y-plasty, have been established since the conventional classical frenectomy procedure was initially presented to cope with the difficulties associated with an aberrant labial frenum. This case report demonstrates that a Z-plasty approach was used to remove the 21-year-old female patient's high papillary-type labial frenum attachment and how orthodontic treatment led to the closure of the midline diastema. For several reasons, the frenectomy procedure with Z-plasty proved to be reliable and yielded outstanding aesthetic outcomes for the removal of the aberrant labial frenum connection. Understanding Z-plasty will enable primary intention-based tissue healing, reduce the risk of tissue contractures, shorten the patient's recovery, and enhance the patient's aesthetic outcomes.
Labial Frenectomy Using Conventional Scalpel Technique: A Case Report

Abstract:Frenum morphology is of pivotal concern while treating patients who desire improved aesthetics and for treating patients with midline diastemas, as it may lead to failure or relapse of orthodontic treatment. Frenum, a thick band of muscle fiber, may present with abnormalities in the shape, size, form, number, and thickness, which may cause gingival recession along with poor oral health. Frenectomy is adopted as the plan of action while treating abnormal frenum cases by resecting the frenum attachment to provide closure of the spacing between the upper anterior teeth, as in the case of midline diastema. Various forms and techniques of frenectomy have been adopted according to the type of frenum attachment and aesthetic concerns of the patients. Amidst the various treatment options available, conventional frenectomy using a scalpel has emerged as a viable solution for treating patients. The surgeons value and praise its exceptional precision and ease of use, and the patients prefer it for its affordability; hence, a functional and aesthetic outcome is achieved via this treatment. This report provides a comprehensive overview of a case of conventional frenectomy with a one-week follow-up.
Comparative study of cytologic and colposcopic findings in preclinical cervical cancer.

Abstract:Seventy-seven patients who had PAP smear cytology and colposcopic examination in a 2-year period between 1988 and 1989 were reviewed. Those with findings indicative or suspicious of malignancy were subjected to biopsy. All 50 patients thus biopsied were confirmed to have preclinical cancer on histological examination. Compared against histology, PAP smear cytology gave an accuracy rate of 90% (5 false negatives) and colposcopy gave an accuracy rate of 94% (3 false negatives). These results demonstrate that the two techniques are useful as screening tests for preclinical cervical cancer. The results are improved if they are used complementarily. However, there was poor specificity in the categorisation of cervical cancer by both methods. This was probably due to the subjectiveness of the two procedures. The study also raised the possibility of a higher incidence of preclinical cervical cancer in the Chinese ethnic group.

Abstract:Introduction: A modified frenal treatment for aberrant frenum is presented in this report to reduce scar tissue formation and maintain the attached gingiva. Description: The case report describes two cases in which a V-shaped incision removed the aberrant frenum and then the flaps of the frenum were sutured at the mid line. Results: The results showed reduced scar tissue in the mid line and the tissue healed with adequate attached gingiva. Take-away lessons: The modified frenotomy technique presented here is ideal for a large frenum that could expose the underlying connective tissue that could reduce the scar tissue formation.
Aesthetic Reconstruction of Philtrum Using De- Epidermized Scar Flap in Secondary Unilateral Cleft Lip

Abstract:Background: The presence of philtrum in the middle area of the upper lip is a pre-eminent feature of facial aesthetics. However, the philtrum is invariably damaged in cleft lips, and the damage persists even after primary surgery. We attempted to use de-epidermized scar tissue flaps, to raise the philtral ridge and dimple, in secondary unilateral cleft lips. Methods: A prospective study was planned to determine the effectiveness of this novel procedure in 11 patients with secondary unilateral cleft lip deformities, from 2011 to 2013. Pre-and post-operative photographs were taken and postoperative evaluation was performed between 6 and 23 months. Eminence of the philtral ridge was scored on a 5 point grading scale; the dimple was scored on a 3 point grading scale as well as the scars. Scores were recorded both preoperatively and postoperatively. Results: The results demonstrated a significant improvement in both, the philtral ridge and dimple, as indicated by the preoperative and postoperative scores (2.27 vs. 3.91, 1.36 vs. 2.03, respectively, all P<0.05). Five patients had a significant improvement in the appearance of scar, while appearance of the scar was acceptable to the remaining patients. Conclusions: The use of deepidermized scar tissue is an effective and simple method to construct the philtrum, in cases of secondary cleft lip.

Abstract:Rat hepatocyte plasmatic membrane damages caused by the administration of tetrachloromethane of heliotrine was investigated. Phospholipid content in plasmatic membranes decreased. Heliotrine caused complete and CCL4 twofold inhibition of Na, K-ATPase. Intraperitoneal administration of egg yolk phosphatidylcholine liposomes had a reparative effect on the damaged membranes consisting in the restoration of phospholipid content and enzyme activities.

Abstract:Ankyloglossia, commonly known as tongue-tie, is the most common disorder of tongue morphology characterized by aberrant attachment of the lingual frenum. This study aimed to provide a comprehensive literature review and evaluate the effectiveness of various laser wavelengths in the surgical treatment of patients with ankyloglossia. An electronic screening of PubMed and the Cochrane Central Register of Controlled Trials (CENTRAL) databases was conducted on 8 November 2021. The following search terms were used to review the available data on the subject of interest: (ankyloglossia OR tongue tie OR short lingual frenulum OR lingual frenectomy OR lingual frenulectomy OR lingual frenotomy OR lingual frenulotomy) AND laser. The use of lasers in ankyloglossia treatment resulted in shorter procedure time, reduced indications for general anesthesia, reduced administration of postoperative analgesics, fewer sutures or none needed, reduced postoperative bleeding, and improved healing. Despite many advantages, this method has its clinical limitations: it requires the use of expensive equipment; well-trained personnel familiar with lasers; and personal protective equipment for the patient, caregiver, operator, and assistant. The laser procedure does not eliminate the need for myofunctional exercises and work with a speech therapist.

Abstract:Various oral complications such as gingival recession, restricted lip movement and tooth malalignment are the result of an abnormal frenum. Management of these types of frenum is either frenectomy or frenotomy. Methods for performing frenectomies include the conventional scalpel technique, Z-plasty, Miller's technique, V-Y plasty, lasers, and electrocautery. This case report details the successful management of an abnormal frenum attachment using electrocautery to ease and reduce discomfort to the 19-year-old female patient, causing aesthetic concerns. For its precision, minimal bleeding and post-operative discomfort, electrocautery was chosen. This procedure was performed under local anaesthesia. There were favourable post-operative outcomes as the patient experienced minimal pain and rapid recovery from the surgical site. Significant improvement in gingival health was seen in the follow-up examination. This case demonstrates the efficacy of using electrocautery in managing abnormal frenum attachment while highlighting its benefits over traditional surgical methods for ease and reduced discomfort.

Abstract:Maxillary labial frenectomy is a surgical procedure aimed at addressing midline diastema, where a strip of tissue creating a gap between two upper front teeth is removed. Typically, this strip extends from the vestibule of the gingivobuccal sulcus to the attached gingiva. The procedure is often performed using a diode laser, offering benefits including simplicity and safety for the patients. It can remarkably improve overall aesthetics and decrease the chances of gingival recession. This case report highlights the successful management of a female patient who arrived complaining of double frenum attachment leading to poor musculature support due to which she experiences poor aesthetics and hampered oral hygiene maintenance.

Abstract:Background: The maxillary labial frenum (MLF) is a soft tissue fold connecting the upper lip to the alveolar process. Abnormal attachment can cause periodontal, functional, and esthetic problems. Differential diagnosis is important and can prevent unnecessary interventions. This study aims to summarize the current evidence on the assessment and management of abnormal MLF. Methods: A thorough review of the literature was conducted. Five online databases were searched for relevant peer-reviewed human studies. Article screening and data extraction were performed independently by two reviewers using predefined inclusion/exclusion criteria. Information about article type, study design, participants' characteristics, interventions, and outcomes was extracted and synthesized. Results: 52 articles met the review criteria. MLF is a dynamic structure characterized by a wide normal morphological variation. MLF assessment in infants has not been standardized. Studies in pre-adolescents reported a change in the thickness and position of the MLF observed over time, resulting in a lower prevalence of abnormal MLF morphology. Studies in adolescents and adults reported variable differential diagnosis criteria. Lasers appear as the most advantageous frenectomy modality. Conclusions: There is a need for more objective MLF diagnostic protocols and treatment guidelines, which could prevent unnecessary surgical interventions.

Abstract:Background In general, the width of the lower face is determined by four parts: the mandible, the masseter muscle, the buccal fat pad, and the subcutaneous fat. In this article, we introduce a modified surgical method to reshape the outline of round faces that have no significant overgrowth or eversion of the lower jawbone. Method The procedure includes dissection of the masseter muscle and partial lipectomy of the buccal fat pad through the same intraoral small incision. Results The patients were satisfied with the aesthetic outcome of both the profile and frontal view. Follow-up of more than 6 months shows no prospective complications. Conclusion This method yields an optimal aesthetic result in patients who have a round face without significant overgrowth or eversion of the lower jawbone. Abstract:Median diastema is a physiological occurrence that is frequently seen in the maxillary jaw. Therefore, a median diastema has been associated with a wide range of etiological variables, including thumb sucking, supernumerary teeth, tongue thrusting, some dentoalveolar discrepancy, and hypodontia. Patient esthetic and function demands are both negated by the labial frenum's abnormal location in relation to the maxillary anterior teeth, which results in diastema and gingival recession. Lasers are now being used in several fields of dentistry as an alternative to conventional scalpel operations. Frenectomy can be done through the use of electrosurgery, laser surgery, or the classic scalpel technique. This pathological frenum can be very well excised with a diode laser. Due to its applicability, sufficient coagulation, lack of suture requirements, and reduced discomfort and inflammation, the diode laser can be utilized in pediatric dentistry. High-connected midline diastema has remained a subject of debate when it comes to management and the right time to intervene and treat it. Both orthodontists and pediatric dentists agree that frenectomy should not be done after the closure of the orthodontist gap or before the appearance of the permanent canine teeth. However, several conditions, including the child's psychological status, parents' concerns, the closure's unpredictable effects on the future, and the expense of combined therapies, may lead to an early intervention for therapy during the primary or mixed dentition. In this specific scenario, a child who was eight years old underwent a diode laser frenectomy. After seven days, a follow-up examination showed normal position and attachment of the frenum and no signs of infection at the site of surgery.

Abstract:Background Large defects of the lower lip often require free tissue coverage. Fasciocutaneous free flaps have been traditionally used, but innervated muscular free flaps may fundamentally address orbicularis oris deficiency and improve dynamic competence. This review aimed to determine if innervated muscular free flaps provided benefit over fasciocutaneous free flaps for reconstruction of defects of more than 50% of the lower lip. Outcomes of interest included functional oral outcomes including oral competence, aesthetic outcomes, and patient-reported outcome measures. Methods Five databases (PubMed, Embase, Web of Science, CINAHL, and CENTRAL) were searched for variations of "lip" and "free flap." Two authors screened articles and included primary research of free flap reconstruction of more than 50% of the lower lip from any etiology in living humans with an English full-text available. Composite bony flaps were excluded. Oral competence and aesthetic satisfaction, reconstruction details, and complications were extracted. Proportional meta-analyses were used to synthesize results for fasciocutaneous free flaps, which were compared with those for muscular free flaps. Results Fifty-nine articles describing 242 patients were included. Muscular free flaps reported significantly higher proportional oral competence than fasciocutaneous free flaps (98 vs. 83%, p = 0.01). Aesthetic outcomes (98 vs. 97%, p = 0.22) and complications (17 vs. 18%, p = 0.79) were equivalent between fasciocutaneous and muscular free flaps. Conclusion Muscular free flaps may address the fundamental orbicularis oris defect that causes oral incompetence and seem to provide better functional results. Abstract:Background: Tumour resection or trauma often leaves a large defect in lips. Sometimes the defect can be closed directly, but most time the defect is so large that you have to design some flaps to close it. The defects could locate in upper lip or lower lip; in middle or in lateral of the lips; and the size of the defects are often different. There are a few surgical designs in textbooks, but they cannot meet each individual case.

Abstract:Background: An overlong upper lip or philtrum indicates aging. In the current study, a procedure for upper lip lift to alleviate this defect is illustrated and assessed for its esthetic effects and risks.Methods: A total of 84 Chinese patients with overlong upper lip underwent surgery for upper lip lift from 2012 to 2014. Skin excision height was designed on the basis of the golden proportion of the lower region of the face. The incision was made at the subnasal (inferior border of the columella-nostril-alar) region. After undermining the tissue between the subcutis and orbicularis oris, a "T"-shaped region of the muscle was resected. Both sides of the muscular flaps were developed and suspended upward to the columella base to lift the upper lip. The incision was then sutured delicately.Results: Of the 76 patients who were effectively followed up, recovery period, lift effect, visibility of the scar, vermilion shape, surgery-associated effects, any complications, and overall rejuvenation, among other outcomes, were evaluated at 1 week, 1 month, and 3-24 months postoperation. Assessment was performed by both objective measurements and subjective satisfaction: significant efficiency was achieved and high satisfaction, satisfaction, and dissatisfaction ratings were reported by 55 (72.4%), 18 (23.7%), and 3 (3.9%) patients, respectively.Conclusions: Upper lip lift yields a significant esthetic effect with few complications. Lack of information is one of the reasons why physicians have resisted the introduction of information systems in medical practice beyond auxiliary services. The author describes the functions of a clinical information system currently in use in a psychiatric inpatient hospital, offering an electronic medical record, office automation tools, knowledge databases and individual PC-programs. He discusses measures to preserve the confidentiality and integrity of the electronic data and shortterm experiences concerning benefits and risks of the system.
