Abstract:Background: The odor identification test is culture-dependent. Objectives: This study aimed to develop a culturally adapted version of the 5-item Quick olfactory Sniffin' Sticks Test (Q-Stick) to adapt to the clinical environment in China. Methods: The study included 3 phases: (1) develop a culturally adapted version of Q-Stick by replacing unfamiliar odors in the original Q-Stick test (N = 344); (2) validate the culturally adapted version of Q-Stick against two widely used olfactory tests: the Sniffin' Sticks olfactory test and the 12-item Cross-Cultural Smell Identification Test (CC-SIT) (N = 286); 3) evaluate the test-retest reliability of the culturally adapted version of Q-Stick (N = 60). Results: After modification of the interference items, the correct recognition rate of leather was changed from 54.65% to 90.00%. The adapted version of the Q-Stick score significantly correlated with both the Sniffin' Sticks score (r = 0.7642, p < .0001) and CC-SIT score (r = 0.7403, p < .0001). Normal olfaction is indicated if the culturally adapted version of the Q-Stick score > 4 (specificity 70.00%, sensitivity 83.33%, Youden index 0.533) and Q-Stick score ≤ 4 indicates olfactory dysfunction. The 3-month test-retest reliability coefficient was as high as 0.96. Conclusions and significance: The culturally adapted version of Q-Stick is an effective and stable screening tool to identify patients with olfactory dysfunction in China.

Abstract:Abstract Clinical tests assessing olfactory performance have become indispensable for diagnosing olfactory dysfunction. As time and personnel resources are limited, it would be advantageous to have shorter protocols focusing on singular aspects of olfactory performance, such as odor identification. However, such a unidimensional approach is often inconclusive and needs further tests (and tools). Hence, new testing methods with high levels of sensitivity, specificity, and reproducibility are required for clinical practice. Here, we developed a S niffin’ S ticks o dor mix ture identification test method (SSomix), with emphasis on resource efficiency and simplicity of administration. SSomix consists of mixtures of two and three odors applied onto a piece of paper using 11 out of 16 items from the original Sniffin’ Sticks identification test kit. A total of 66 healthy subjects and 22 patients with olfactory dysfunction were included in the study. SSomix showed good to excellent test-retest reliability and validity. The area under the receiver operating characteristics curves indicated good diagnostic accuracy in identifying patients with reduced and severely impaired olfactory function. SSomix was a suitable downsizing of the original kit, especially regarding resource efficiency.

Abstract:BACKGROUND: The extended, 32-item version of the Sniffin' Sticks identification test was developed in order to create a precise tool enabling repeated, longitudinal testing of individual olfactory subfunctions.NEW METHOD: Odors of the previous test version had to be changed for technical reasons, and the odor identification test needed re-investigation in terms of reliability, validity, and normative values.RESULTS: In our study we investigated olfactory abilities of a group of 100 patients with olfactory dysfunction and 100 controls. We reconfirmed the high test-retest reliability of the extended version of the Sniffin' Sticks identification test and high correlations between the new and the original part of this tool. In addition, we confirmed the validity of the test as it discriminated clearly between controls and patients with olfactory loss.COMPARISON WITH EXISTING METHOD(S): The additional set of 16 odor identification sticks can be either included in the current olfactory test, thus creating a more detailed diagnosis tool, or it can be used separately, enabling to follow olfactory function over time. Additionally, the normative values presented in our paper might provide useful guidelines for interpretation of the extended identification test results.CONCLUSIONS: The revised version of the Sniffin' Sticks 32-item odor identification test is a reliable and valid tool for the assessment of olfactory function.

A computer-controlled olfactometer for a self-administered odor identification test

Abstract:Odor identification tests are widely used for the general screening of olfactory function. However, the administration of odor identification tests is often limited due to a lack of investigators’ time. Therefore, we attempted to design a computer-controlled olfactometer to present a self-administered odor identification test. The results produced by means of this olfactometer were evaluated in terms of validity and test–retest reliability. To test the validity, participants’ performance in the odor identification test using the olfactometer was compared with their performance in the odor identification test using the validated assessment of the “Sniffin’ Sticks” test. The ten-item odor identification test was performed two times using two different methods: (1) the self-administered test using the computer-controlled olfactometer and (2) the foreign-administered test using the “Sniffin’ sticks.” To examine test–retest reliability, 20 participants were asked to repeat these tests on a different day. Participants reached significantly higher scores on a foreign-administered odor identification test using the “Sniffin’ sticks” than on the olfactometer-based test; however, this effect was driven by two less correctly identified odors in the olfactometer-based test. The significant difference between both methods in the mean scores disappeared after excluding two odors from the analysis. In addition, both methods showed no significant difference in scores obtained during the first and second session, indicating that results were consistent between sessions. In conclusion, our findings demonstrate that the computer-controlled olfactometer designed in this study can be used for a self-administered odor identification test.

The “Sniffin' Kids” Test - A 14-Item Odor Identification Test for Children

Abstract:Tools for measuring olfactory function in adults have been well established. Although studies have shown that olfactory impairment in children may occur as a consequence of a number of diseases or head trauma, until today no consensus on how to evaluate the sense of smell in children exists in Europe. Aim of the study was to develop a modified "Sniffin' Sticks" odor identification test, the "Sniffin' Kids" test for the use in children. In this study 537 children between 6-17 years of age were included. Fourteen odors, which were identified at a high rate by children, were selected from the "Sniffin' Sticks" 16-item odor identification test. Normative date for the 14-item "Sniffin' Kids" odor identification test was obtained. The test was validated by including a group of congenital anosmic children. Results show that the "Sniffin' Kids" test is able to discriminate between normosmia and anosmia with a cutoff value of >7 points on the odor identification test. In addition the test-retest reliability was investigated in a group of 31 healthy children and shown to be ρ = 0.44. With the 14-item odor identification "Sniffin' Kids" test we present a valid and reliable test for measuring olfactory function in children between ages 6-17 years.

Abstract:BACKGROUND:Olfactory testing is a useful tool in the differential diagnosis of Parkinson's Disease (PD). Although fast and easy to use, the high intercultural variability of odor detection limits the world-wide use of the most common test sets. OBJECTIVE:The aim of this study was to test one of the most commonly used olfactory tests (Sniffin' Sticks 12-identification test) in an adapted version for a Chinese population of healthy subjects and PD patients. METHODS:For this purpose, cohorts of 39 Chinese and 41 German PD patients as well as 70 Chinese and 100 German healthy subjects have been examined both with the original and the adapted version of the Sniffin' Sticks test, the latter being designed according to the regional culture. RESULTS:The adapted Chinese version of the Sniffin' Sticks 12 identification test proved to discriminate Chinese PD patients from controls with a high specificity but relatively low sensitivity. Yet not all odor exchanges would have been necessary as the original odors including liquorice and coffee showed an equally high identification rate in the Chinese and German cohorts. CONCLUSIONS:The results showed that the newly adapted test could be used as a screening test for PD related olfactory dysfunction in a Chinese population. However further investigation will be necessary to optimize the selection of odors for the Chinese version of the test.

Abstract:IntroductionAssessment of olfactory performance is of high clinical interest in the contexts of smell loss as well as neurological diseases, and recently gained attention in obesity research. Available olfactory tests, especially for assessing olfactory sensitivity, are time-consuming and require high cognitive capacity. Therefore, we aimed to establish a short procedure for reliably testing olfactory sensitivity using a subtest of the “Sniffin’ Sticks” battery. Evaluation criteria are test duration, validity, and test-retest reliability.MethodsIn a preliminary study using a within-subject repeated-measures design, we measured olfactory sensitivity for n-butanol in 20 young and healthy participants. We compared sensitivity obtained with three different measures during two sessions in a pseudo-randomized order: a standard single-staircase three-alternative forced-choice procedure with seven reversals (SSP_7); an abbreviated version with five reversals (SSP_5); and an ascending presentation of 16 dilution steps from lowest to highest odor concentration (brief ascending procedure, BAP).ResultsCompared to the SSP_7, the BAP was 51%, and the SSP_5 26% shorter in duration. Both the BAP and SSP_5 scores were highly correlated with the SSP_7. The test-retest reliability in all three tests was similar to that typically reported in olfactory research.ConclusionThe abbreviated tests are valid measures of olfactory sensitivity. Especially, the BAP is as reliable as the standard method, but remarkably faster and easier to perform.ImplicationsThus, the short procedures bear potential for both research and clinical practice, especially for complex study designs with time constraints on olfactory testing and for patient populations with attention deficits.

Abstract:The cross-cultural adaptation and validation of the Sniffin`Sticks test for the Portuguese population is described. Over 270 people participated in four experiments. In Experiment 1, 67 participants rated the familiarity of presented odors and seven descriptors of the original test were adapted to a Portuguese context. In Experiment 2, the Portuguese version of Sniffin`Sticks test was administered to 203 healthy participants. Older age, male gender and active smoking status were confirmed as confounding factors. The third experiment showed the validity of the Portuguese version of Sniffin`Sticks test in discriminating healthy controls from patients with olfactory dysfunction. In Experiment 4, the test-retest reliability for both the composite score (r71 = 0.86) and the identification test (r71 = 0.62) was established (p<0.001). Normative data for the Portuguese version of Sniffin`Sticks test is provided, showing good validity and reliability and effectively distinguishing patients from healthy controls with high sensitivity and specificity. The Portuguese version of Sniffin`Sticks test identification test is a clinically suitable screening tool in routine outpatient Portuguese settings.

Abstract:Aims: This study aimed to design the Chinese Modified Olfactory Identification (CMOI) test based on the Sniffin' Sticks Olfactory Identification (SSOI) test by changing unfamiliar distractors and odors for more familiar ones for the Chinese population. Materials and methods: We recruited 200 healthy volunteers (103 males and 97 females, aged 18-65 years, mean age 35.04 years, SD 10.96); in a survey, 100 volunteers rated their familiarity with 121 odors, including all the SSOI test odor descriptors and common odors in Chinese daily life. The SSOI test was modified according to the survey results. The other 100 volunteers were tested three times using the SSOI test, the Modified Distractors Olfactory Identification (MDOI) test established by modifying distractors in the SSOI test, and the CMOI test developed by using familiar unpleasant odors to displace the odors with low correct recognition rates in the MDOI test. We compared the test scores of the volunteers during the modification process. Results: Volunteers were unfamiliar with 31 odor descriptors in the SSOI test; 23 distractors with low familiarity were displaced with more familiar distractors. The three odors with the lowest correct recognition rate in the MDOI test (apple, leather, and pineapple) were displaced with familiar unpleasant odors. The test scores were significantly higher in the CMOI test than in others (p < 0.0001); the correct recognition rate in the CMOI test was significantly higher than in the SSOI test (p < 0.01). Conclusion: The test scores in the CMOI test were significantly improved; it prevented choosing wrongly due to unfamiliarity with an odor and its distractors.

Abstract:The clinical diagnostics of olfactory dysfunction in children turns out to be challenging due to low attention span, insufficient linguistic development and lack of odor experiences. Several smell tests have been developed for adults. Most of these examinations take a relatively long time and require a high level of concentration. Therefore, the aim of the current study was to evaluate an odor discrimination and olfactory threshold test using the frequently used “Sniffin’ Sticks” in children and adolescents in a simplified two-alternative-forced-choice version (2AFC) and compare it to the original three-alternative-forced-choice test (3AFC). One-hundred-twenty-one healthy participants aged between 5 and 17 years took part in this study. Within each of the two sessions participants underwent olfactory testing using the modified 2AFC as well as the standard 3AFC method. A better test-retest reliability was achieved using the original 3AFC method compared to the modified 2AFC. This was true for the odor discrimination as well as the olfactory threshold. Age had a significant influence on both tests, which should be considered when testing young children. We discuss these findings with relation to the existing norms and recommend using the 3AFC version due to a better test-retest reliability to measure olfactory function in children.

Abstract:BACKGROUND:Olfactory dysfunction significantly reduces quality of life, with a prevalence as high as 20% in the general adult population. Odor identification (OI) tests are culturally dependent and widely used in clinical and epidemiological evaluations of olfaction. We aimed to develop a Chinese odor identification test (COIT) based on the Sniffin' Sticks identification test.METHODS:Patients (n=60) with olfactory disorders and healthy controls (n=404) were recruited in the Smell and Taste Center of a tertiary-care university hospital. Unfamiliar odors in the Sniffin' Sticks identification test were replaced to create a 16-item COIT, which was validated with a simplified Chinese version of the Cross-culture Smell Identification Test (CC-SIT) and Sniffin' Sticks. A test-retest reliability of COIT was also conducted.RESULTS:Six odors with a correct recognition rate <75% were replaced with familiar odors for Chinese. The COIT score significantly correlated with both Sniffin' Sticks (r=0.755 P<0.0001) and CC-SIT score (r=0.7462 P<0.0001). Based on the testing results of an additional 120 subjects, we concluded that scores of 12-16, 7-11, and 0-6 corresponded to normosmia, hyposmia, and anosmia, respectively. The 3-month test-retest-reliability coefficient was as high as 0.83.CONCLUSIONS:The COIT is an effective tool for assessing olfactory function in the Chinese population.
Multicenter investigation of 1,036 subjects using a standardized method for the assessment of olfactory function combining tests of odor identification, odor discrimination, and olfactory thresholds

Abstract:Abstract“Sniffin’ Sticks” is a test of nasal chemosensory performance that is based on penlike odor-dispensing devices. It is comprised of three tests of olfactory function: tests for odor threshold, discrimination and identification. Previous work has already established its test-retest reliability and validity in comparison to established measures of olfactory sensitivity. The results of this test are presented as a composite TDI score – i.e., the sum of results obtained for threshold, discrimination and identification measures. The present multicenter investigation aimed at providing normative values in relation to different age groups. To this end, 966 patients were investigated in 11 centers. An additional study tried to establish values for the identification of anosmic patients, with 70 anosmics investigated in five specialized centers where the presence of anosmia was confirmed by means of olfactory evoked potentials. For healthy subjects, the TDI score at the 10th percentile was 24.5 in subjects younger than 15 years, 30.3 for ages from 16 to 35 years, 28.8 for ages from 36 to 55 years and 27.5 for subjects older than 55 years. While these data can be used to estimate individual olfactory abilities in relation to a subject’s age, hyposmia was defined as the 10th percentile score of 16- to 35-year-old subjects. Our latter study revealed that none of 70 anosmics reached a TDI score higher than 15. This score of 15 is regarded as the cut-off value for functional anosmia. These results provide the basis for the routine clinical evaluation of patients with olfactory disorders using “Sniffin’ Sticks.”

Abstract:Abstract Background A precise and reliable test of the olfactory function is indispensable for the diagnosis of the olfactory disorder (OD). Despite of this, in a clinical context, often there is no place in daily routine for time-consuming procedures. This study aimed to examine if the assessment of olfactory function using the “Sniffin’ Sticks” is suitable for self-assessment. Methods Participants comprised 84 healthy control subjects (HC) and 37 OD patients. The “Sniffin’ Sticks” test battery consisting of odor threshold (T), discrimination (D) and identification (I) tests was used for self- and assisted assessments. To save time, we applied the 8-item wide step version of the T test and the 8-item D test, whereas the I task remained the same as the original version. The whole test included two sessions, with each session comprising a self-assessment part performed by the participants themselves, and an assisted-assessment part performed by the examiner. Results Sniffin’ Sticks self-assessment was efficient in distinguishing between self-reported HC subjects and OD patients ( p ’s 0.05). In the self-administered I and TDI tests, there was a moderate to excellent test–retest reliability (ICC = 0.51–0.93, p ’s < 0.01), and a strong to excellent correlation with the assisted assessment ( r = 0.71–0.92, p ’s < 0.01). However, the self-administered T and D tests only exhibited low to moderate test–retest reliability (ICC = 0.30–0.72, p ’s < 0.05) and correlations with the assisted test ( r = 0.31–0.62, p ’s < 0.05). Conclusions The Identification self-test is appropriate to be solely applied, and is therefore an easy-to-use alternative for olfactory screening in a larger segment of patients. The whole “Sniffin’ Sticks” self-test also shows good measurement properties and is therefore a suitable backup in clinical practice, but improvement is needed due to the simplified D and T self-test.

Abstract:In this article, in order to meet the demand of diagnosis of respiration odor, a new style laboratorial system of artificial optical nose was introduced, which was used to explored the possibility of diagnosing disease through respiration odor. This system consisted of gas-sensitive optic fiber sensor array and signal processing system. Utilizing the characteristics of optic fiber sensor of combing with lots of sensitive polymer easily, responding rapidly and high-precisionly to the gas and obtaining dynamic information of olfaction easily, a method of computing chemistry was applied to achieve quantification of sensitive polymers and fluorescent reagents, and develop the optic fiber sensor array with different selectivities. Besides, an olfactory mechanism was adopted to the artificial nose, which can separate the component and concentration information of mixed gases from the responsive signal data of the optic fiber sensor array using OSN-OB-DLNN (olfactory sensory neuron-olfactory bulb-delay line neural network) arithmetic by simulating biological olfaction. Based on the experimental results, it can be concluded that this laboratorial system of optical artificial nose sensors has the characteristic of bionics and prospect of application.
A practical test for retronasal odor identification based on aromatized tablets

Abstract:Background Olfactory perceptions elicited by odors originating from within the body (retronasal olfaction) play a crucial role in well-being and are often disrupted in various medical conditions. However, the assessment of retronasal olfaction in research and the clinical practice is impeded by the lack of commercially available tests and limited standardization of existing testing materials. New Method The novel ThreeT retronasal odor identification test employs 20 flavored tablets that deliver a standardized amount of odorous stimuli. The items represent common food- and non-food-related odors. Results The ThreeT test effectively distinguishes patients with olfactory dysfunction from healthy controls, achieving a specificity of 86% and sensitivity of 73%. Its scores remain stable for up to 3 months ( r =.79). Comparison with Existing Method ThreeT test exhibits a strong correlation with "Tasteless powders" measure of retronasal olfaction ( r =.78) and classifies people into healthy and patient groups with similar accuracy. Test-retest stability of ThreeT is slightly higher than the stability of "Tasteless powders" (r=.79 vs r=.74). Conclusions ThreeT is suitable for integration into scientific research clinical practice to monitor retronasal odor identification abilities.

Abstract:OBJECTIVE: Olfactory tests tailored for children are essential, as diagnosing olfactory dysfunction at these ages can be challenging. The 16-item "Sniffin' Sticks" is reliable and easy to perform. To the best of our knowledge, there is currently no validated olfactory test for the Portuguese pediatric population. This study aimed to adapt and validate the "Sniffin' Sticks" olfactory test for the Portuguese pediatric population.METHODS: Between August 2020 and September 2021, 354 children aged between 6 and 17years old enrolled in the study, 336 healthy children with a normal sense of smell, and 18 anosmic children with Kallmann syndrome. The study consisted of two parts. Firstly, the "Sniffin' Sticks" olfactory identification test was applied to healthy children and the odors with statistically significant low identification rates were excluded. A modified version of "Sniffin' Sticks" was defined and named "Sniffin' Kids-PT" test. Secondly, normative data were assessed and test-retest and validation tests were performed.RESULTS: Apple and Cloves odors were identified with a low rate and were excluded from the "Sniffin' Sticks" olfactory test. In the modified 14-item "Sniffin' Kids-PT", scoring <6 (from 6-8years old), <7 (from 9-11years old) or <8 (from 12-14years old and 15-17years old) was indicative of olfactory dysfunction. The test-retest reliability was good (r=0.81; P<0.001) and the differences between scores of healthy children and anosmic children were statistically significant (U213=13.00; P<0.001).CONCLUSION: The modified "Sniffin' Kids-PT" is a reliable test to discriminate between normosmia and olfactory dysfunction in Portuguese children over 5years old.

Abstract:Abstract The sense of smell allows for the assessment of the chemical composition of volatiles in our environment. Different factors are associated with reduced olfactory function, including age, sex, as well as health and lifestyle conditions. However, most studies that aimed at identifying the variables that drive olfactory function in the population suffered from methodological weaknesses in study designs and participant selection, such as the inclusion of convenience sample or only of certain age groups, or recruitment biases. We aimed to overcome these issues by investigating the Cooperative Health Research in South Tyrol (CHRIS) cohort, a population-based cohort, by using a validated odor identification test. Specifically, we hypothesized that a series of medical, demographic and lifestyle variables is associated with odor identification abilities. In addition, our goal was to provide clinicians and researchers with normative values for the Sniffin’ Sticks identification set, after exclusion of individuals with impaired nasal patency. We included 6,944 participants without acute nasal obstruction and assessed several biological, social, and medical parameters. A basic model determined that age, sex, years of education, and smoking status together explained roughly 13% of the total variance in the data. We further observed that variables related to medical (positive screening for cognitive impairment and for Parkinson’s disease, history of skull fracture, stage 2 hypertension) and lifestyle (alcohol abstinence) conditions had a negative effect on odor identification scores. Finally, we provide clinicians with normative values for both versions of the Sniffin’ Sticks odor identification test, i.e. with 16 items and with 12 items.

Abstract:Abstract“Sniffin’ Sticks” is a test of nasal chemosensory function that is based on pen-like odor dispensing devices, introduced some 10 years ago by Kobal and co-workers. It consists of tests for odor threshold, discrimination, and identification. Previous work established its test-retest reliability and validity. Results of the test are presented as “TDI score”, the sum of results obtained for threshold, discrimination, and identification measures. While normative data have been established they are based on a relatively small number of subjects, especially with regard to subjects older than 55 years where data from only 30 healthy subjects have been used. The present study aimed to remedy this situation. Now data are available from 3,282 subjects as compared to data from 738 subjects published previously. Disregarding sex-related differences, the TDI score at the tenth percentile was 24.9 in subjects younger than 15 years, 30.3 for ages from 16 to 35 years, 27.3 for ages from 36 to 55 years, and 19.6 for subjects older than 55 years. Because the tenth percentile has been defined to separate hyposmia from normosmia, these data can be used as a guide to estimate individual olfactory ability in relation to subject’s age. Absolute hyposmia was defined as the tenth percentile score of 16–35 year old subjects. Other than previous reports the present norms are also sex-differentiated with women outperforming men in the three olfactory tests. Further, the present data suggest specific changes of individual olfactory functions in relation to age, with odor thresholds declining most dramatically compared to odor discrimination and odor identification.

The Sniffin’ Sticks Odor Discrimination Memory Test: A Rapid, Easy-to-Use, Reusable Procedure for Testing Olfactory Memory

Abstract:Objectives: Neurodegenerative diseases can alter odor memory in addition to inducing quantitative impairment, and olfactory memory can be tested using the validated olfactory memory test with encapsulated odors (Odor Discrimination Memory Test [ODMT]). The aim of this study was to investigate the new Sniffin’ Sticks ODMT (SSODMT), which is comparable with the ODMT. Methods: The SSODMT was administered to 48 subjects (30 women, 18 men; mean age, 34.6 ± 16.3 years). Olfactory function was assessed using the 16-item Sniffin’ Sticks Identification Test. The ODMT was administered to a subgroup of 10 subjects to assess the comparability of the SSODMT and the ODMT. Test-retest reliability was studied in another 20 subjects (10 women, 10 men; mean age, 32.9 ± 16.4 years). Results: The mean SSODMT score was 10.7 ± 1.8 (range, 5-12). SSODMT test scores were significantly correlated with Sniffin’ Sticks Identification Test scores ( r 67 = 0.66, P < .001) and were significantly negatively correlated with subject age ( r 48 = −0.70, P < .001). SSODMT and ODMT scores were highly and significantly correlated, indicating that the test results were comparable ( r 10 = 0.75, P < .05). Test-retest analysis revealed a significant correlation ( r 20 = 0.68, P < .001). Conclusions: These results demonstrate the usefulness of the new SSODMT and its comparability with the ODMT. The SSODMT is an easy, rapid, and low-cost tool that is suitable for routine use in evaluating odor memory.
