Abstract:Background: Ambulatory follow-up for all patients with heart failure (HF) is recommended within 7-14 days after hospital discharge to improve HF outcomes. This study objective was to examine post-discharge ambulatory follow-up of patients with comorbid diabetes and HF from a low-income population in primary and specialty care. Methods: Adults with diabetes and first hospitalizations for HF, covered by Alabama Medicaid in 2010-2019, were included and the claims analyzed for ambulatory care utilization (any, primary care, cardiology or endocrinology) within 60 days after hospital discharge. Restricted mean survival time regression, adjusted for socio-demographics and clinical factors, examined the time to first ambulatory visit. Multivariable-adjusted Poisson regression estimated prevalence ratios (PR) of primary and specialty care post-discharge visits. Results: Among 9,867 Medicaid-covered adults with diabetes and first hospitalization for HF, [mean age 53.7, SD 9.2 years, 47.4% African American, 10.9% Hispanic/other, 65.4% women], 26.5% had an ambulatory visit within 0-7 days, 15.1% - 8-14 days, 31.0% - 15-60 days and 26.8% - no visit; 71% saw primary care, 12% - cardiology and 1.6% - endocrinology. Compared to Non-Hispanic white adults, African American and Hispanic/other ethnicity adults were less likely to see primary care (adjusted PR 95% Confidence Interval [CI] 0.96 [0.91-1.00], and 0.91 [0.89-0.98], respectively), and their visits occurred later (by 1.8 day, p=0.0006 and by 2.8 days, p=0.0016, respectively). Having 4 or more additional comorbidities was associated with both primary care and cardiology visits (aPR [95% CI] 1.59 [1.35-1.87], and 1.45 [1.01-2.09], respectively). Conclusion: More than a half of Medicaid-covered adults with diabetes and HF in Alabama did not receive post-discharge ambulatory care follow-up as recommended by guidelines. African American and Hispanic/other ethnicity adults were less likely to receive recommended post-discharge ambulatory care for comorbid diabetes and HF.

Trends in Mortality After Incident Hospitalization for Heart Failure Among Medicare Beneficiaries

Abstract:Importance: Despite advances in treatment and care quality for patients hospitalized with heart failure (HF), minimal improvement in mortality has been observed after HF hospitalization since 2010. Objective: To evaluate trends in mortality rates across specific intervals after hospitalization. Design, setting, and participants: This cohort study evaluated a random sample of Medicare fee-for-service beneficiaries with incident HF hospitalization from January 1, 2008, to December 31, 2018. Data were analyzed from February 2023 to May 2024. Main outcomes and measures: Unadjusted mortality rates were calculated by dividing the number of all-cause deaths by the number of patients with incident HF hospitalization for the following periods: in-hospital, 30 days (0-30 days after hospital discharge), short term (31 days to 1 year after discharge), intermediate term (1-2 years after discharge), and long term (2-3 years after discharge). Each period was considered separately (ie, patients who died during one period were not counted in subsequent periods). Annual unadjusted and risk-adjusted mortality ratios were calculated (using logistic regression to account for differences in patient characteristics), defined as observed mortality divided by expected mortality based on 2008 rates. Results: A total of 1 256 041 patients (mean [SD] age, 83.0 [7.6] years; 56.0% female; 86.0% White) were hospitalized with incident HF. There was a substantial decrease in the mortality ratio for the in-hospital period (unadjusted ratio, 0.77; 95% CI, 0.67-0.77; risk-adjusted ratio, 0.74; 95% CI, 0.71-0.76). For subsequent periods, mortality ratios increased through 2013 and then decreased through 2018, resulting in no reductions in unadjusted postdischarge mortality during the full study period (30-day mortality ratio, 0.94; 95% CI, 0.82-1.06; short-term mortality ratio, 1.02; 95% CI, 0.87-1.17; intermediate-term mortality ratio, 0.99; 95% CI, 0.79-1.19; and long-term mortality ratio, 0.96; 95% CI, 0.76-1.16) and small reductions in risk-adjusted postdischarge mortality during the full study period (30-day mortality ratio, 0.88; 95% CI, 0.86-0.90; short-term mortality ratio, 0.94; 95% CI, 0.94-0.95; intermediate-term mortality ratio, 0.94; 95% CI, 0.92-0.95; and long-term mortality ratio, 0.95; 95% CI, 0.93-0.96). Conclusions and relevance: In this study of Medicare fee-for-service beneficiaries, there was a substantial decrease in in-hospital mortality for patients hospitalized with incident HF from 2008 to 2018, but little to no reduction in mortality for subsequent periods up to 3 years after hospitalization. These results suggest opportunities to improve longitudinal outpatient care for patients with HF after hospital discharge.

Abstract:Introduction: Safety-net hospitals disproportionately care for medically complex, socially vulnerable, and racial/ethnic minority populations. Prior to the Affordable Care Act, safety-net hospitals had higher mortality and readmission rates for common cardiovascular conditions than their non-safety-net counterparts, but it is unclear whether these disparities remain. Methods: We conducted a serial, cross-sectional study using data from 3265 acute-care hospitals in the US. Performance on 30-day mortality, readmission, and excess days in acute care (EDAC) measures for acute myocardial infarction (AMI), heart failure (HF), stroke, and coronary artery bypass graft surgery (CABG) was compared at safety-net hospitals versus non-safety-net hospitals in fiscal year (FY) 2020 (July 2016 to June 2019) using linear regression models that adjusted for hospital-level characteristics. Interaction terms for safety-net status and year were used to assess for differential changes between FY 2010 (July 2006 to June 2009) and FY 2020. Results: Thirty-day mortality rates at safety-net hospitals compared with non-safety-net hospitals were lower for HF (10.9% vs 11.5%; adjusted difference: -0.49%, 95% CI: -0.65% to -0.33%), higher for AMI (12.8% vs 12.6%; adjusted difference: 0.15%, 0.04% to 0.26%), and similar for CABG and stroke. Thirty-day readmission rates were modestly higher at safety-net sites for AMI (16.4% vs 16.0%; adjusted difference: 0.37%, 0.26% to 0.48%) and HF (22.4% vs 21.8%; 0.58%, 0.44% to 0.72%), and comparable for CABG. However, safety-net hospitals performed markedly worse on the 30-day EDAC measures for AMI (15.2 vs 3.4 excess days; adjusted difference: 10.8, 95% CI: 8.3 to 13.4) and HF (14.1 vs 3.9 excess days; adjusted difference: 10.2, 6.4 to 10.4). From FY 2010 to FY 2020, differences in 30-day mortality and readmission rates differentially improved at safety-net sites for HF, but not AMI. Conclusions: Thirty-day mortality rates for common cardiovascular conditions are now similar at safety-net and non-safety-net hospitals. However, large gaps in 30-day post-discharge acute care utilization indicate that policy initiatives are still needed to support under-resourced safety-net systems and the patients they serve.

Abstract:Introduction Geographic disparities in access to healthcare are linked to patient outcomes and healthcare utilization for many diseases. Geographic disparities may particularly impact HF outcomes because follow up after HF hospitalization is time-sensitive. Alternatively, the emergence of telemedicine may attenuate the magnitude of such associations. Hypothesis After index hospitalization for HF, patients who travel shorter distances to reach cardiology services are more likely to follow up within 14 days of discharge and have lower 24-month mortality. Methods Retrospective analysis of patients admitted to University of Pittsburgh Medical Center (UPMC) hospitals with primary diagnosis of HF from 2010-2023, limited to 27,097 index HF hospitalizations associated with in-person cardiology visit within 1 year of discharge and complete follow up data. Straight line distance was calculated between patients’ home and cardiology practice used for follow up. Outcomes were odds of 14 day post-discharge follow up using piecewise logistic regression with breakpoint at 30 miles, and odds of 24 month mortality using logistic regression. Results Of 27,097 index hospitalizations, 6347 (23%) had either in-person or telemedicine follow up within 14 days. Patients traveled average 11.8 (SD 36.8) miles for general cardiology follow-up and 17.7 (SD 34.0) miles for follow up with a HF subspecialist. Only 4,072 (15%) of patients went to the cardiology practice closest to their home. 14-day follow up had a non-linear association with distance. Under 30 miles, distance was positively associated with 14-day follow up; for each additional mile, patients had 1.01 odds of follow up (aOR=1.01, 1.00-1.01) . For distance greater than 30 miles, distance had a negative association with 14-day follow up (aOR=0.99, 0.98-0.99). Mortality had a positive linear association with distance; for each additional 10 miles of distance, odds of mortality were 1.04 higher (aOR 1.04, 1.01-1.05). Conclusion Patients traveled long distances to access care and many bypassed a closer provider for one further away. Under 30 miles, patients who lived further away were more likely to follow up, even after controlling for geographic covariates like area deprivation and neighborhood walk scores. This may reflect patient choice; patients living near multiple providers may select one further away based on personal preference. After 30 miles, distance may limit choice of provider and ability to follow up promptly. Efforts to increase post-discharge follow up may benefit from focus on expanding access for patients who live far away from cardiology providers.

Abstract:Circulation: Heart Failure, Ahead of Print. BACKGROUND:Disparities in guideline-based quality measures likely contribute to differences in heart failure (HF) outcomes. We evaluated between- and within-hospital differences in the quality of care across sex, race, ethnicity, and insurance for patients hospitalized for HF.METHODS:This retrospective analysis included patients hospitalized for HF across 596 hospitals in the Get With the Guidelines-HF registry between 2016 and 2021. We evaluated performance across 7 measures stratified by patient sex, race, ethnicity, and insurance. We evaluated differences in performance with and without adjustment for the treating hospital. We also measured variation in hospital-specific disparities.RESULTS:Among 685 227 patients, the median patient age was 72 (interquartile range, 61–82) and 47.2% were women. Measure performance was significantly lower (worse) for women compared with men for all 7 measures before adjustment. For 4 of 7 measures, there were no significant sex-related differences after patient-level adjustment. For 20 of 25 other comparisons, racial and ethnic minorities and Medicaid/uninsured patients had similar or higher (better) adjusted measure performance compared with White and Medicare/privately insured patients, respectively. Angiotensin receptor neprilysin inhibitor measure performance was significantly lower for Asian, Hispanic, and Medicaid/uninsured patients, and cardiac resynchronization therapy implant/prescription was lower among women and Black patients after hospital adjustment, indicating within-hospital differences. There was hospital-level variation in these differences. For cardiac resynchronization therapy implantation/prescription, 278 hospitals (46.6%) had ≥2% lower implant/prescription for Black versus White patients compared with 109 hospitals (18.3%) with the same or higher cardiac resynchronization therapy implantation/prescription for Black patients.CONCLUSIONS:HF quality measure performance was equitable for most measures. There were within-hospital differences in angiotensin receptor neprilysin inhibitor and cardiac resynchronization therapy implant/prescription for historically marginalized groups. The magnitude of hospital-specific disparities varied across hospitals.

Reliability of fine needle aspiration biopsy in large thyroid nodules.

Abstract:OBJECTIVE Fine needle aspiration biopsy provides one of the most important data that determines the treatment algorithm of thyroid nodules. Nevertheless, the reliability of fine needle aspiration biopsy is controversial in large nodules. The aim of this study was to evaluate the adequacy of fine needle aspiration biopsy in thyroid nodules that are four cm or greater. MATERIAL AND METHODS We retrospectively examined 219 patients files who underwent thyroidectomy for thyroid nodules that were greater than four centimeter between May 2007 and December 2012. Seventy-four patients with hyperthyroidism, and 18 patients without preoperative fine needle aspiration cytology were excluded from the study. Histopathologic results after thyroidectomy were compared with preoperative cytology results, and sensitivity and specificity rates were calculated. RESULTS False-negativity, sensitivity and specificity rates of fine needle aspiration biopsy of thyroid nodules were found to be 9.7%, 55.5%, and 85%, respectively. Within any nodule of the 127 patients, 28 (22.0%) had thyroid cancer. However, when only nodules of at least 4 cm were evaluated, thyroid cancer was detected in 22 (17.3%) patients. CONCLUSION In this study, fine needle aspiration biopsy of large thyroid nodules was found to have a high false-negativity rate. The limitations of fine-needle aspiration biopsy should be taken into consideration in treatment planning of thyroid nodules larger than four centimeters.

Abstract:Circulation: Heart Failure, Ahead of Print. BACKGROUND:Home-time is an emerging, patient-centered outcome that represents the amount of time a patient spends alive and outside of health care facility settings, comprising of hospitals, skilled nursing facilities, and acute rehabilitation centers. Studies evaluating home-time in the context of heart failure are limited, and the impact of quality improvement interventions on home-time has not been studied.METHODS:Medicare beneficiaries aged 65 years or older who were hospitalized for heart failure in the Get With the Guidelines-Heart Failure registry between 2019 and 2021 were included. Postdischarge home-time, mortality, and readmission rates at 30 days and 1 year were calculated with the goal of establishing baseline metrics before the initiation of IMPLEMENT-HF, a multicenter quality improvement program aimed at improving heart failure management.RESULTS:Overall, 66 019 patients were included across 437 sites. Median 30-day and 1-year home-time were 30 (18–30) and 333 (139–362) days, respectively. Only 22.1% of patients experienced 100% home-time in the year after discharge. Older patients spent significantly less time at home, with a median 1-year home-time of 302 (86–359) compared with 345 (211–365) days in patients over 85 and those between 65 and 74 years old, respectively (P<0.001). Black patients also experienced the least amount of home-time with only 328 (151–360) days at 1-year follow-up. Rates of heart failure readmission and all-cause mortality 1-year post-discharge were high at 29.8% and 37.0%, respectively.CONCLUSIONS:In this contemporary multicenter cohort, patients hospitalized with heart failure spent a median of 91.2% of their time in the year after discharge alive and at home, largely driven by high mortality rates. These findings serve as a preimplementation baseline for IMPLEMENT-HF, which will evaluate the impact of targeted heart failure initiatives on home-time and other clinical outcomes.

BackgroundThe AHA/ACC Adult Congenital Heart Disease guidelines recommend that most adults with congenital heart disease (CHD) follow-up with CHD cardiologists every 1 to 2 years because longer gaps in care are associated with adverse outcomes. This study aimed to determine the proportion of patients in North Carolina who did not have recommended follow-up and to explore predictors of loss to follow-up.MethodsPatients ages ≥18 years with a healthcare encounter from 2008 to 2013 in a statewide North Carolina database with an ICD-9 code for CHD were assessed. The proportion with cardiology follow-up within 24 months following index encounter was assessed with Kaplan-Meier estimates. Cox regression was utilized to identify demographic factors associated with differences in follow-up.Results2822 patients were identified. Median age was 35 years; 55% were female. 70% were white, 22% black, and 3% Hispanic; 36% had severe CHD. The proportion with 2-year cardiology follow-up was 61%. Those with severe CHD were more likely to have timely follow-up than those with less severe CHD (72% vs 55%, P < .01). Black patients had a lower likelihood of follow-up than white patients (56% vs 64%, P = .01). Multivariable Cox regression identified younger age, non-severe CHD, and non-white race as risk factors for a lower likelihood of follow-up by 2 years.Conclusion39% of adults with CHD in North Carolina are not meeting AHA/ACC recommendations for follow-up. Younger and minority patients and those with non-severe CHD were particularly vulnerable to inadequate follow-up; targeted efforts to retain these patients in care may be helpful.

Abstract:Circulation: Heart Failure, Ahead of Print. BACKGROUND:Timely heart failure (HF) diagnosis can lead to earlier intervention and reduced morbidity. Among historically marginalized patients, new-onset HF diagnosis is more likely to occur in acute care settings (emergency department or inpatient hospitalization) than outpatient settings. Whether inequity within outpatient clinician practices affects diagnosis settings is unknown.METHODS:We determined the setting of incident HF diagnosis among Medicare fee-for-service beneficiaries between 2013 and 2017. We identified sociodemographic and medical characteristics associated with HF diagnosis in the acute care setting. Within each outpatient clinician practice, we compared acute care diagnosis rates across sociodemographic characteristics: female versus male sex, non-Hispanic White versus other racial and ethnic groups, and dual Medicare-Medicaid eligible (a surrogate for low income) versus nondual-eligible patients. Based on within-practice differences in acute diagnosis rates, we stratified clinician practices by equity (high, intermediate, and low) and compared clinician practice characteristics.RESULTS:Among 315 439 Medicare patients with incident HF, 173 121 (54.9%) were first diagnosed in acute care settings. Higher adjusted acute care diagnosis rates were associated with female sex (6.4% [95% CI, 6.1%–6.8%]), American Indian (3.6% [95% CI, 1.1%–6.1%]) race, and dual eligibility (4.1% [95% CI, 3.7%–4.5%]). These differences persisted within clinician practices. With clinician practice adjustment, dual-eligible patients had a 4.9% (95% CI, 4.5%–5.4%) greater acute care diagnosis rate than nondual-eligible patients. Clinician practices with greater equity across dual eligibility also had greater equity across sex and race and ethnicity and were more likely to be composed of predominantly primary care clinicians.CONCLUSIONS:Differences in HF diagnosis rates in the acute care setting between and within clinician practices highlight an opportunity to improve equity in diagnosing historically marginalized patients.

Abstract:Background-It is unknown whether causes and temporal patterns of 30-day readmission vary between heart failure (HF) with preserved ejection fraction (HFpEF) and HF with reduced ejection fraction (HFrEF). We sought to address this question by examining a 5% national sample of Medicare beneficiaries. Methods and Results-We included individuals who experienced a hospitalization for HFpEF or HFrEF between 2007 and 2013. We identified causes of 30-day readmission based on primary discharge diagnosis and further classified causes of readmission as HF-related, non-HF cardiovascular-related, and non-cardiovascular-related. We calculated the cumulative incidence of these classifications for HFpEF and HFrEF in a competing risks model and calculated subdistribution hazard ratios of these classifications by comparing those with HFpEF and those with HFrEF. Among 60 640 Medicare beneficiaries, we identified 13 785 unique older adults hospitalized with HFpEF and 15 205 who were hospitalized with HFrEF. Noncardiovascular diagnoses represented the most common causes of 30-day readmission (HFpEF: 59%; HFrEF: 47%), a pattern that was observed for each week of the 30-day study period for both HFpEF and HFrEF participants. In comparing readmission diagnoses in an adjusted model, non-cardiovascular-related diagnoses were more common and HF-related diagnoses were less common in HFpEF participants. Conclusions-Non-cardiovascular-related diagnoses represented the most common causes of 30-day readmission following HF hospitalization for each week of the 30-day postdischarge period. HF diagnoses were less common among those with HFpEF compared with HFrEF. Future interventions aimed at reducing 30-day readmissions following an HF hospitalization would benefit from an increased focus on noncardiovascular comorbidity and interventions that target HFpEF and HFrEF separately.

Abstract:Background The use of recommended heart failure (HF) medications has improved over time, but opportunities for improvement persist among women and at rural hospitals. Objectives This study aims to characterize national trends in performance in the use of guideline-recommended pharmacologic treatment for HF at U.S. Department of Veterans Affairs (VA) hospitals, at which medication copayments are modest. Methods Among patients discharged from VA hospitals with HF between January 1, 2013, and December 31, 2019, receipt of all guideline-recommended HF pharmacotherapy among eligible patients was assessed, consisting of evidence-based beta-blockers; angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, or angiotensin receptor neprilysin inhibitors; mineralocorticoid receptor antagonists; and oral anticoagulation. Results Of 55,560 patients at 122 hospitals, 32,304 (58.1%) received all guideline-recommended HF medications for which they were eligible. The proportion of patients receiving all recommended medications was higher in 2019 relative to 2013 (OR: 1.54; 95% CI: 1.44-1.65). The median of hospital performance was 59.1% (Q1-Q3: 53.2%-66.2%), improving with substantial variation across sites from 2013 (median 56.4%; Q1-Q3: 50.0%-62.0%) to 2019 (median 65.7%; Q1-Q3: 56.3%-73.5%). Women were less likely to receive recommended therapies than men (adjusted OR [aOR]: 0.84; 95% CI: 0.74-0.96). Compared with non-Hispanic White patients, non-Hispanic Black patients were less likely to receive recommended therapies (aOR: 0.83; 95% CI: 0.79-0.87). Urban hospital location was associated with lower likelihood of medication receipt (aOR: 0.73; 95% CI: 0.59-0.92). Conclusions Forty-two percent of patients did not receive all recommended HF medications at discharge, particularly women, minority patients, and those receiving care at urban hospitals. Rates of use increased over time, with variation in performance across hospitals.

Abstract:Background: Understanding the relationship between neighborhood environment and cardiovascular outcomes is important to achieve health equity and implement effective quality strategies. We conducted a population-based cohort study to determine the association of neighborhood socioeconomic deprivation and 30-day mortality and readmission rate for patients admitted with common cardiovascular conditions. Methods and results: We examined claims data from fee-for-service Medicare beneficiaries aged ≥65 years between 2017 and 2019 admitted for heart failure, valvular heart disease, ischemic heart disease, or cardiac arrhythmias. The primary exposure was the Area Deprivation Index; outcomes were 30-day all-cause death and unplanned readmission. More than 2 million admissions were included. After sequential adjustment for patient characteristics (demographics, dual eligibility, comorbidities), area health care resources (primary care clinicians, specialists, and hospital beds per capita), and admitting hospital characteristics (ownership, size, teaching status), there was a dose-dependent association between neighborhood socioeconomic deprivation and 30-day mortality rate for all conditions. In the fully adjusted model for death, estimated effect sizes of residence in the most disadvantaged versus least disadvantaged neighborhoods ranged from adjusted odds ratio 1.29 (95% CI, 1.22-1.36) for the heart failure group to adjusted odds ratio 1.63 (95% CI, 1.36-1.95) for the valvular heart disease group. Neighborhood deprivation was associated with increased adjusted 30-day readmission rates, with estimated effect sizes from adjusted odds ratio 1.09 (95% CI, 1.05-1.14) for heart failure to adjusted odds ratio 1.19 (95% CI, 1.13-1.26) for arrhythmia. Conclusions: Neighborhood socioeconomic disadvantage was associated with 30-day mortality rate and readmission for patients admitted with common cardiovascular conditions independent of individual demographics, socioeconomic status, medical risk, care access, or admitting hospital characteristics.

Abstract:The banana (Musa) is one of the world’s most important crops and the most valuable fruit in the global market. In the search for varieties that are more pest- and disease-resistant plant breeders are increasingly looking to the wild progenitors,—as understanding its evolution is key to genetic improvement. The banana was also an important economic crop in prehistory although it is difficult to track its history of domestication and evolution due to preservation issues, the lack of reliable species identification criteria and limited archaeological evidence. Just two archaeobotanical studies of macro-remains and phytoliths, in New Guinea and Cameroon, have provided reliable identifications and interpretations to help our understanding of the origins and evolution of the banana. But to track the spread and growing importance of this plant in the diet, across the tropics and through time, we need to combine information drawn from botany, genetics, linguistics and archaeology.
Time taken for cardiology care and one-year results for primary care physicians' referrals for cardiology consultation in heart failure patients: clinical implications for heart failure management

Abstract:Background and Purpose The clinical trajectory of patients with heart failure (HF) is characterised by frequent episodes of decompensation requiring urgent medical assistance, hospitalisations, and an increased risk of mortality. HF clinical guidelines recommend early visits by health professionals after hospital discharge, yet little evidence exists regarding the associations between delayed medical attention and outcomes in HF patients. To assess the clinical impact of the time taken to receive cardiology care for referrals from primary care physicians (PCPs) to a cardiology department in HF patients, we selected 6,859 HF patients who visited the cardiology service at least once between 2010 and 2021. Methods Using an interrupted time series regression model, we analysed the impact of integrating e-consultations into the healthcare model (implemented in 2013) and evaluated the time taken to receive cardiology care, as well as all-cause, cardiovascular (CV), and HF-related hospital admissions and mortality. The primary objective of this analysis was to examine the associations between delays in cardiology care following a PCP referral and 1-year outcomes (HF-related, cardiovascular, and total hospitalisations; and HF-related, cardiovascular, and total mortality). Results Almost 50% were women, and the mean age was 77.8 years. Arterial hypertension (79.6%), diabetes (34%), atrial fibrillation (51.4%), ischemic heart disease (20.4%), and cerebrovascular disease were the most prevalent comorbidities. The mean time from PCP referral to cardiology consultation in the overall population was 22.3 days: 30 days in 13.0% of patients, respectively. One year after cardiology consultation, 68.6% of patients required assistance in an Emergency Department; 22.5% were hospitalised for any cause, 14.9% for cardiovascular reasons, and 9.2% for HF-related issues. Moreover, total mortality at 1-year was 8.6%, comprising 4.3% cardiovascular mortality and 1.3% HF-related deaths. Multivariate analysis of the relationship between the time taken to receive cardiology care and 1-year outcomes revealed a significant increased risk associated with a longer delay in cardiology assistance, particularly evident in the subgroup of patients with previous hospital admissions due to worsening HF (see Table). Conclusions Throughout the follow-up period, a reduced time taken to receive care was independently associated with better 1-year outcomes. We believe that our experience can contribute to the development of a more efficient ambulatory care pathway for HF patients, especially those at high risk, such as those with a history of hospitalisation due to worsening events. Our findings further support the clinical guideline recommendation for early contact by health professionals following hospital discharge, extending it to include PCP referrals for cardiology consultations.

Abstract:Introduction: Reducing heart failure (HF) admissions is one of the main objectives in disease control, due to its impact on prognosis and costs. The transitional models at discharge are imposed as a strategy capable of reducing hospitalizations, most of them based on specific hospital units. We analyzed the impact of the primary care (PC) post-discharge follow-up. Material and methods: Retrospective observational study at the referral area of a tertiary hospital. Linear regression analysis was performed between early follow-up from the PC center after HF admission rate and the 30-day all-cause readmission rate throughout 2021. Results: The degree of follow-up from PC after hospital discharge for heart failure is associated with fewer 30-day readmissions for all causes (Pearson's R=0.53, P=.02); with a decrease of 20%, similar to that observed when it is performed from other care facilities and which is maintained when adjusting for the complexity of the patients. Conclusions: PC heart failure post-discharge follow-up could be effective in reducing hospitalizations, and is complementary to that carried out by hospital units.
National Patterns of Remote Patient Monitoring Service Availability at US Hospitals and their Readmission Performance for Cardiovascular Conditions

Abstract:Background: Digital remote patient monitoring (RPM) enables longitudinal care outside traditional healthcare settings, especially in the vulnerable period after hospitalizations, with broad coverage of the service by payers. We sought to evaluate patterns of RPM service availability at US hospitals and the association of these services with 30-day readmissions for two key cardiovascular conditions, heart failure (HF) and acute myocardial infarction (AMI). Methods: We used contemporary national data from the American Hospital Association (AHA) Annual Survey to ascertain US hospitals offering RPM services for post-discharge or chronic care and used census-based county-level data to define the characteristics of the communities they serve. We linked these with hospitals benchmarked risk-standardized relative performance on readmissions (excess readmission ratio [ERR]) from CMS Hospital Quality Reports (2018-2022). We used mixed-effects multivariable regression to examine the association between RPM services at hospitals and hospital characteristics-adjusted ERR for HF and AMI. Results: There were 2,754 hospitals with CMS quality report data. Over five years of the study, there was a 38.3% increase in the number of hospitals offering RPM services, rising from 952 (42.0%) hospitals in 2018 to 1,237 (58.1%) in 2022. However, the availability of RPM services varied across different hospital groups with smaller, non-teaching hospitals, particularly those serving rural, low-income communities, and those located in the South, were less likely to offer RPM services. There was a consistent association between the availability of RPM services and better risk-standardized readmission performance for HF, with lower ERR for hospitals offering RPM compared with those not offering RPM (absolute difference, -0.016 [-0.023, -0.009], standardized difference, 24.6%, p<0.001). However, no such association was observed for AMI (-0.007 [-0.016, 0.002], standardized difference, 10.3%, p=0.19). Conclusions: In this national study of US hospitals, there has been a large increase in the availability of RPM services but with large variation among hospitals, with lower availability in hospitals serving low-income and rural communities. RPM services were associated with lower hospital readmission rates for HF but not AMI.

Abstract:The impact of hospital readmission reduction program (HRRP) on heart failure (HF) outcomes has been debated. Limited data exist regarding trends of HF readmission rates beyond 30 days from all-payer sources. The aim of this study was to investigate temporal trends of 30- and 90-day HF readmissions rates from 2010 to 2017 in patients from all-payer sources. The National Readmission Database was utilized to identify HF hospitalizations between 2010 and 2017. In the primary analysis, a linear trend in 30-day and 90-day readmissions from 2010 to 2017 was assessed. While in the secondary analysis, a change in aggregated 30- and 90-day all-cause and HF-specific readmissions pre-HRRP penalty phase (2010–2012) and post-HRRP penalties (2013–2017) was compared. Subgroup analyses were performed based on (1) Medicare versus non-Medicare insurance, (2) low versus high HF volume, and (3) HF with reduced versus preserved ejection fraction (heart failure with reduced ejection fraction and heart failure with preserved ejection fraction). Multiple logistic and adjusted linear regression analyses were performed for annual trends. A total of 6 669 313 index HF hospitalizations for 30-day, and 5 077 949 index HF hospitalizations for 90-day readmission, were included. Of these, 1 213 402 (18.2%) encounters had a readmission within 30 days, and 1 585 445 (31.2%) encounters had a readmission within 90 days. Between 2010 and 2017, both 30 and 90 days adjusted HF-specific and all-cause readmissions increased (8.1% to 8.7%, P trend 0.04, and 18.3% to 19.9%, P trend <0.001 for 30-day and 14.8% to 16.0% and 30.9% to 34.6% for 90-day, P trend <0.001 for both, respectively). Readmission rates were higher during the post-HRRP penalty period compared with pre-HRRP penalty phase (all-cause readmission 30 days: 18.6% versus 17.5%, P <0.001, all-cause readmission 90 days: 32.0% versus 29.9%, P <0.001) across all subgroups except among the low-volume hospitals. The rates of adjusted HF-specific and all-cause 30- and 90-day readmissions have increased from 2010 to 2017. Readmissions rates were higher during the HRRP phase across all subgroups except the low-volume hospitals.

Abstract:Introduction Environmental, socioeconomic and medical risk factors’ among patients with heart failure (HF) with reduced ejection fraction (HFrEF) may cause clinical and prognostic differences. We studied the racial and ethnic prognostic implications of HFrEF medical management among outpatients treated in our inner-city practice. Methods In a retrospective protocol, we studied patients referred to the HF clinic at our institution from October 2020 to October 2021. Clinical, hemodynamic, echocardiographic, socio-economic data and the use of guideline-directed medical therapy (GDMT) were collected. Patients were followed for a median of 16.7 months for HF hospitalizations, mortality as well as their composite. Results In the study period, 506 patients were included (67.4±14 years, 46% women, LVEF: 32.9±13%). 69(13%) patients were of white race, 99(20%) were Hispanics, 267(53%) were African Americans (AA), and 71(14%) patients were of other races. During follow-up, 155(31%) patients had HF admissions, 60(12%) died and 180(36%) had the combined outcomes. Figure 1A shows basic differences stratified by race. Regarding GDMT, AA and Hispanic patients showed the least use of individual GDMT and the least use of the full set of GDMT. Importantly, AA and Hispanics had more medication non-adherence, while AA showed the most clinic visits no shows. Interestingly, apart from more chronic kidney disease (CKD) in AA and Hispanics, other common medication side effects such as hypotension, bradycardia, hyperkalemia were not different between races. Importantly, more deaths, HF hospitalization, and combined outcomes (Figure 1 A to D) occurred in Hispanics and AA compared to other groups (Figure 1A). Survival analyses for the risk of combined outcomes showed that whites did not have different risks (HR: 1.7, 95% CI: 0.79 to 3.7, Figure), while Hispanics (HR: 2.1, 95% CI: 1.1 to 4.3) and AA (HR: 3.7, 95% CI: 2 to 6.9) had higher risk compared to other races. After adjusting for covariates (age, sex, individual GDMT, CRT, CKD, GFR, EF, medication non-adherence, and clinic no-shows), only AA continued to have elevated risk for combined outcomes (HR: 2.9, 95% CI: 1.5 to 5.6), while whites and Hispanics lost statistical significance. Conclusions Racial and ethnic disparities had prognostic significance in the outpatient management of HFrEF in terms of worse hospitalizations and mortality in the Hispanic and AA populations. Such findings may be attributed to patient factors (less medication and clinic visits non-adherance), and physician factors (stopping medications due to renal dysfunction). AA had worse outcomes even after adjusting for covariates suggesting further studies are needed to determine a more effective therapeutic approach in this population.
