Abstract:1) Ten 3-niethoxy-phenyl-metabolites of dihydroxy-phenylalanine, dopamine, norepinephrine, epinephrine, and N-methyl-epinephrine were investigated in urine from 10 patients with functional neural tumors. The acids, alcohols and amines were determined with paper chromatography and the amino-acid 3-methoxy-4-hydroxy-phenylalanine with high voltage electrophoresis combined with chromatography. All' these ten above-mentioned metabolites are stable and easy to determine compared with the unstable catechol metabolites themselves.2) The acids, 3-methoxy-4-hydroxy-man-delic acid and/or 3-methoxy-4-hydroxy-phe-nylacetic acid, are overexcreted by all these 9 cases of functional neural tumors. Very high values of these acids point to longlasting generalized metastases.3) The alcohols, 3-methoxy-4-hydroxy-phenyl-glycol and 3-methoxy-4-hydroxy-phenyl-ethanol, vary mainly with the acids and with age. Analysis of Transcranial Focused Ultrasound Beam Profile Sensitivity for Neuromodulation of the Human Brain

Abstract:Objective. While ultrasound is largely established for use in diagnostic imaging and heating therapies, its application for neuromodulation is relatively new and not well understood. The objective of the present study was to investigate issues related to interactions between focused acoustic beams and brain tissues to better understand possible limitations of transcranial ultrasound for neuromodulation. Approach. A computational model of transcranial focused ultrasound was constructed and validated against bench top experimental data. The models were then incrementally extended to address and investigate a number of issues related to the use of ultrasound for neuromodulation. These included the effect of variations in skull geometry and gyral anatomy, as well as the effect of transmission across multiple tissue and media layers, such as scalp, skull, CSF, and gray/white matter on ultrasound insertion behavior. In addition, a sensitivity analysis was run to characterize the influence of acoustic properties of intracranial tissues. Finally, the heating associated with ultrasonic stimulation waveforms designed for neuromodulation was modeled. Main results. Depending on factors such as acoustic frequency, the insertion behavior of a transcranial focused ultrasound beam is only subtly influenced by the geometry and acoustic properties of the underlying tissues. Significance. These issues are critical for the refinement of device design and the overall advancement of ultrasound methods for noninvasive neuromodulation.

Abstract:Objective: Transcranial focused ultrasound (tFUS) is an emerging neuromodulation approach that has been demonstrated in animals but is difficult to translate to humans because of acoustic attenuation and scattering in the skull. Optimal dose delivery requires subject-specific skull porosity estimates which has traditionally been done using CT. We propose a deep learning (DL) estimation of skull porosity from T1-weighted MRI images which removes the need for radiation-inducing CT scans. Approach: We evaluate the impact of different DL approaches, including network architecture, input size and dimensionality, multichannel inputs, data augmentation, and loss functions. We also propose back-propagation in the mask (BIM), a method whereby only voxels inside the skull mask contribute to training. We evaluate the robustness of the best model to input image noise and MRI acquisition parameters and propagate porosity estimation errors in thousands of beam propagation scenarios. Main results: Our best performing model is a cGAN with a ResNet-9 generator with 3D 64x64x64 inputs trained with L1 and L2 losses. The model achieved a mean absolute error of 6.9% in the test set, compared to 9.5% with the pseudo-CT of Izquierdo et al. (38% improvement) and 9.4% with the generic pixel-to-pixel image translation cGAN pix2pix (36% improvement). Acoustic dose distributions in the thalamus were more accurate with our approach than with the pseudo-CT approach of both Burgos et al. and Izquierdo et al, resulting in near-optimal treatment planning and dose estimation at all frequencies compared to CT (reference). Significance: Our DL approach porosity estimates with ~7% error, is robust to input image noise and MRI acquisition parameters (sequence, coils, field strength) and yields near-optimal treatment planning and dose estimates for both central (thalamus) and lateral brain targets (amygdala) in the 200-1000 kHz frequency range.

Abstract:Transcranial ultrasound stimulation (TUS) has emerged as a promising technique for non-invasive neuromodulation, but current systems lack the precision to target deep brain structures effectively. Here, we introduce an advanced TUS system that achieves unprecedented precision in deep brain neuromodulation. The system features a 256-element, helmet-shaped transducer array operating at 555 kHz, coupled with a stereotactic positioning system, individualised treatment planning, and real-time monitoring using functional MRI. In a series of experiments, we demonstrate the system's ability to selectively modulate the activity of the lateral geniculate nucleus (LGN) and its functionally connected regions in the visual cortex. Participants exhibited significantly increased visual cortex activity during concurrent TUS and visual stimulation, with high reproducibility across individuals. Moreover, a theta-burst TUS protocol induced robust neuromodulatory effects, with decreased visual cortex activity observed for at least 40 minutes post-stimulation. These neuromodulatory effects were specific to the targeted LGN, as confirmed by control experiments. Our findings highlight the potential of this advanced TUS system to non-invasively modulate deep brain circuits with high precision and specificity, offering new avenues for studying brain function and developing targeted therapies for neurological and psychiatric disorders. The unprecedented spatial resolution and prolonged neuromodulatory effects demonstrate the transformative potential of this technology for both research and clinical applications, paving the way for a new era of non-invasive deep brain neuromodulation.

Abstract:Purpose: As a non-invasive method for brain diseases, transcranial focused ultrasound (tFUS) offers higher spatial precision and regulation depth. Due to the altered path and intensity of sonication penetrating the skull, the focus and intensity in the skull are difficult to determine, making the use of ultrasound therapy for cancer treatment experimental and not widely available. The deficiency can be effectively addressed by numerical simulation methods, which enable the optimization of sonication modulation parameters and the determination of precise transducer positioning. Methods: A 3D skull model was established using binarized brain CT images. The selection of the transducer matrix was performed using the radius positioning (RP) method after identifying the intracranial target region. Simulations were performed, encompassing acoustic pressure (AP), acoustic field, and temperature field, in order to provide compelling evidence of the safety of tFUS in sonication-induced thermal effects. Results: It was found that the angle of sonication path to the coronal plane obtained at all precision and frequency models did not exceed 10° and 15° to the transverse plane. The results of thermal effects illustrated that the peak temperatures of tFUS were 43.73°C, which did not reach the point of tissue degeneration. Once positioned, tFUS effectively delivers a Full Width at Half Maximum (FWHM) stimulation that targets tumors with diameters of up to 3.72 mm in a one-off. The original precision model showed an attenuation of 24.47 ± 6.13 mm in length and 2.40 ± 1.42 mm in width for the FWHM of sonication after penetrating the skull. Conclusion: The vector angles of the sonication path in each direction were determined based on the transducer positioning results. It has been suggested that when time is limited for precise transducer positioning, fixing the transducer on the horizontal surface of the target region can also yield positive results for stimulation. This framework used a new transducer localization method to offer a reliable basis for further research and offered new methods for the use of tFUS in brain tumor-related research.

Abstract:Transcranial focused ultrasound enables precise and non-invasive manipulations of deep brain circuits in humans, promising to provide safe and effective treatments of various neurological and mental health conditions. Ultrasound focused to deep brain targets can be used to modulate neural activity directly or localize the release of psychoactive drugs. However, these applications have been impeded by a key barrier—the human skull, which attenuates ultrasound strongly and unpredictably. To address this issue, we have developed an ultrasound-based approach that directly measures and compensates for the ultrasound attenuation by the skull. No additional skull imaging, simulations, assumptions, or free parameters are necessary; the method measures the attenuation directly by emitting a pulse of ultrasound from an array on one side of the head and measuring with an array on the opposite side. Here, we apply this emerging method to two primary future uses—neuromodulation and local drug release. Specifically, we show that the correction enables effective stimulation of peripheral nerves and effective release of propofol from nanoparticle carriers through an ex vivo human skull. Neither application was effective without the correction. Moreover, the effects show the expected dose-response relationship and targeting specificity. This article highlights the need for precise control of ultrasound intensity within the skull and provides a direct and practical approach for addressing this lingering barrier.

Abstract:ObjectiveUltrasonic neuromodulation as a safe and non-invasive brain stimulation method that delivers a low-intensity, focused ultrasound to nervous system tissue in a targeted area of the brain. The objective of this study is to numerically investigate the ultrasound wave propagation and the energy distribution within the brain tissues using customized single element focused ultrasound transducers (SEFT), targeting the hippocampus.MethodsA high resolution detailed human head model with seven tissue types was constructed from magnetic resonance imaging (MRI). A full-wave finite-difference time-domain simulation platform, Sim4life, was then used to simulate a 3D non-linear ultrasound wave equation to the specific region of interest, the hippocampus. Three customized SEFT were used to test the effect of transducer positions, and another customized transducer was used to compare the sensitivity effect on heterogeneous and homogeneous brain models. Finally, the sensitivity and performance of low intensity focusing ultrasound stimulation were evaluated.ResultsAn optimized application of SEFT was customized to deliver 100 W/m2 intensity of energy deposition at the hippocampus region. About 85.65% of the generated volume beam was delivered to the targeted hippocampus region and the beam overlap parameter was affected by different transducer positions. Deflection angle changes of SEFT at the range of ±5% did not have a significant effect on energy delivery and position displacement. Only 0.5% of peak pressure change was observed between heterogeneous and homogeneous brain models. The sensitivity analysis also showed that the sound speed is the most influential acoustic parameter.SignificanceThis study demonstrated that ultrasound neuromodulation targeting the depth brain tissue of the hippocampus could be a potential and promising alternative method to some non-acoustic brain stimulation modalities. In the numerical study of ultrasound brain stimulations, ultrasound parameters and the brain model need to be properly determined to simulate the ultrasonic neuromodulations.

Abstract:Ultrasound is a mechanical wave that can non-invasively penetrate the skull to deep brain regions to activate neurons. Transcranial focused ultrasound neuromodulation is a promising approach, with the advantages of noninvasiveness, high-resolution, and deep penetration, which developed rapidly over the past years. However, conventional transcranial ultrasound's spatial resolution is low-precision which hinders its use in precision neuromodulation. Here we focus on methods that could increase the spatial resolution, gain modulation efficiency at the focal spot, and potential mechanisms of ultrasound neuromodulation. In this paper, we summarize strategies to enhance the precision of ultrasound stimulation, which could potentially improve the ultrasound neuromodulation technic.
An element-wise approach for simulating transcranial MRI-guided focused ultrasound thermal ablation

Abstract:This work explored an element-wise approach to model transcranial MRI-guided focused ultrasound (TcMRgFUS) thermal ablation, a noninvasive approach to neurosurgery. Each element of the phased array transducer was simulated individually and could be simultaneously loaded into computer memory, allowing for rapid calculation of the pressure field for different phase offsets used for beam steering and aberration correction. We simulated the pressure distribution for 431 sonications in 32 patients, applied the phase and magnitude values used during treatment, and estimated the resulting temperature rise. We systematically varied the relationship between CT-derived skull density and the acoustic attenuation and sound speed to obtain the best agreement between the predictions and MR temperature imaging (MRTI). The optimization was validated with simulations of 396 sonications from 40 additional treatments. After optimization, the predicted and measured heating agreed well (R2: 0.74 patients 1-32; 0.71 patients 33-72). The dimensions and obliquity of the heating in the simulated temperature maps correlated well with the MRTI (R2: 0.62, 0.74 respectively), but the measured heating was more spatially diffuse. The energy needed to achieve ablation varied by an order of magnitude (3.3-36.1 kJ). While this element-wise approach requires more computation time up front, it can be performed in parallel. It allows for rapid calculation of the three-dimensional heating at the focus for different phase and magnitude values on the array. We also show how this approach can be used to optimize the relationship between CT-derived skull density and acoustic properties. While the relationships found here need further validation in a larger patient population, these results demonstrate the promise of this approach to model TcMRgFUS.

Abstract:Focused ultrasound (FUS) is a non-invasive and non-ionizing technique which deploys ultrasound waves to induce bio-effects. When paired with acoustically active particles such as microbubbles (MBs), it can open the blood brain barrier (BBB) to facilitate drug delivery otherwise inhibited due to the presence of BBB. One of the parameters that affects the FUS beam propagation is the beam incidence angle on the skull. Prior work by our group has shown that, as incidence angles deviate from 90°, FUS focal pressures attenuate and result in a smaller BBB opening volume. The incidence angles calculated in our prior studies were in 2D and used skull information from CT. The study presented herein develops methods to calculate incidence angle in 3D in non-human primate (NHP) skull fragments using harmonic ultrasound imaging without using ionizing radiation. Our results show that ultrasound harmonic imaging is capable of accurately depicting features such as sutures and eye-sockets of the skull. Furthermore, we were able to reproduce previously reported relationships between the incidence angle and FUS beam attenuation. We also show feasibility of performing ultrasound harmonic imaging in in-vivo non-human primates. The all-ultrasound method presented herein combined with our neuronavigation system stands to increase more widespread adoption of FUS and render it accessible by eliminating the need for CT cranial mapping.

Abstract:Non-invasive ultrasound neuromodulation (USNM) is a powerful tool to explore neural circuits and treat neurological disorders. Due to the heterogeneity of the skull and regional variations in modulation and treatment objectives, it is necessary to develop an efficient and spatially controllable neuromodulation approach. Recently, transcranial focused ultrasound (tFUS) combined with external biomicro/nanomaterials for brain stimulation has garnered significant attention. This study focused on tFUS combined with perfluoropentane (PFP) nanodroplets (NDs) to improve the efficacy and spatial controllability of USNM. The developed two-stage variable pulse tFUS sequence that include the acoustic droplet vaporization (ADV) pulse for vaporizing PFP NDs into microbubbles (MBs) and the USNM sequence for inducing mechanical oscillations of the formed MBs to enhance neuronal activity. Further, adjusting the acoustic pressure of the ADV pulse generated the controllable vaporization regions, thereby achieving spatially controllable neuromodulation. The results showed that the mean densities of c-fos+ cells expression in the group of PFP NDs with ADV (109 ± 19 cells/mm2) were significantly higher compared to the group without ADV (37.34 ± 8.24 cells/mm2). The acoustic pressure of the ADV pulse with 1.98 MPa and 2.81 MPa in vitro generated the vaporization regions of 0.146 ± 0.032 cm2 and 0.349 ± 0.056 cm2, respectively. Under the same stimulation conditions, a larger vaporization region was also obtained with higher acoustic pressure in vivo, inducing a broader region of neuronal activation. Therefore, this study will serve as a valuable reference for developing the efficient and spatially controllable tFUS neuromodulation strategy.

Abstract:Focused ultrasound (FUS) is an emerging noinvasive technique for neuromodulation in the central nervous system (CNS). To evaluate the effects of FUS-induced neuromodulation, many studies used behavioral changes, functional magnetic resonance imaging (fMRI) or electroencephalography (EEG). However, behavioral readouts are often not easily mapped to specific brain activity, EEG has low spatial resolution limited to the surface of the brain and fMRI requires a large importable scanner that limits additional readouts and manipulations. In this context, functional ultrasound imaging (fUSI) holds promise to directly monitor the effects of FUS neuromodulation with high spatiotemporal resolution in a large field of view, with a comparatively simple and flexible setup. fUSI uses ultrafast Power Doppler Imaging (PDI) to measure changes in cerebral blood volume, which correlates well with neuronal activity and local field potentials. We designed a setup that aligns a FUS transducer with a linear array to allow immediate subsequent monitoring of the hemodynamic response with fUSI during and after FUS neuromodulation. We established a positive correlation between FUS pressure and the size of the activated area, as well as changes in cerebral blood volume (CBV) and found that unilateral sonications produce bilateral hemodynamic changes with ipsilateral accentuation in mice. We further demonstrated the ability to perform fully noninvasive, transcranial FUS-fUSI in nonhuman primates for the first time by using a lower-frequency transducer configuration.

Abstract:Transcranial focused ultrasound therapy is becoming a viable tool for treatment of brain related disorders. The technique has been used for treatment of essential tremors and chronic neuropathic pain with good results. In addition, the technique has a broad scope of potential applications including: tumor ablation, localized drug delivery, thrombolysis, and neurostimulation. Performing an efficient focused ultrasound treatment delivery requires planning. Accurate computational models could, in principle, be utilized to provide auxiliary guidelines for such planning. In this work, a computational model is presented for simulating the propagation of ultrasound in transcranial setting. The effects of ultrasound propagation through soft tissue and bone are modeled by coupling wave equations of fluid and solid. The coupled model is numerically evaluated by using a hybrid simulation technique that couples finite difference method with a grid method. The resulting computational model is utilized to simulate focused ultrasound field in clinical patient treatment setting. Effect of absorption of heat into the brain tissue is further simulated by using the bioheat equation. Simulation results are compared with magnetic resonance thermometry performed during the clinical treatments for evaluation of validity of the computational model.
Functional ultrasound (fUS) imaging of displacement-guided focused ultrasound (FUS) neuromodulation in mice

Abstract:Focused ultrasound (FUS) stimulation is a promising neuromodulation technique with the merits of non-invasiveness, high spatial resolution, and deep penetration depth. However, simultaneous imaging of FUS-induced brain tissue displacement and the subsequent effect of FUS stimulation on brain hemodynamics has proven challenging thus far. In addition, earlier studies lack in situ confirmation of targeting except for the magnetic resonance imaging-guided FUS system-based studies. The purpose of this study is 1) to introduce a fully ultrasonic approach to in situ target, modulate neuronal activity, and monitor the resultant neuromodulation effect by respectively leveraging displacement imaging, FUS, and functional ultrasound (fUS) imaging, and 2) to investigate FUS-evoked cerebral blood volume (CBV) response and the relationship between CBV and displacement. We performed displacement imaging on craniotomized mice to confirm the in targeting for neuromodulation site. We recorded hemodynamic responses evoked by FUS and fUS revealed an ipsilateral CBV increase that peaks at 4 s post-FUS. We saw a stronger hemodynamic activation in the subcortical region than cortical, showing good agreement with the brain elasticity map that can also be obtained using a similar methodology. We observed dose-dependent CBV response with peak CBV, activated area, and correlation coefficient increasing with ultrasonic dose. Furthermore, by mapping displacement and hemodynamic activation, we found that displacement colocalizes and linearly correlates with CBV increase. The findings presented herein demonstrated that FUS evokes ipsilateral hemodynamic activation in cortical and subcortical depths and the evoked hemodynamic responses colocalized and correlate with FUS-induced displacement. We anticipate that our findings will help consolidate accurate targeting as well as an understanding of how FUS displaces brain tissue and affects cerebral hemodynamics.

Abstract:Background and objective: Transcranial focused ultrasound (tFUS) is an emerging non-invasive therapeutic technology that offers new brain stimulation modality. Precise localization of the acoustic focus to the desired brain target throughout the procedure is needed to ensure the safety and effectiveness of the treatment, but acoustic distortion caused by the skull poses a challenge. Although computational methods can provide the estimated location and shape of the focus, the computation has not reached sufficient speed for real-time inference, which is demanded in real-world clinical situations. Leveraging the advantages of deep learning, we propose multi-modal networks capable of generating intracranial pressure map in real-time. Methods: The dataset consisted of free-field pressure maps, intracranial pressure maps, medical images, and transducer placements was obtained from 11 human subjects. The free-field and intracranial pressure maps were computed using the k-space method. We developed network models based on convolutional neural networks and the Swin Transformer, featuring a multi-modal encoder and a decoder. Results: Evaluations on foreseen data achieved high focal volume conformity of approximately 93% for both computed tomography (CT) and magnetic resonance (MR) data. For unforeseen data, the networks achieved the focal volume conformity of 88% for CT and 82% for MR. The inference time of the proposed networks was under 0.02 s, indicating the feasibility for real-time simulation. Conclusions: The results indicate that our networks can effectively and precisely perform real-time simulation of the intracranial pressure map during tFUS applications. Our work will enhance the safety and accuracy of treatments, representing significant progress for low-intensity focused ultrasound (LIFU) therapies.
Effects of chronic administration of polybrominated biphenyls on parameters associated with hepatic drug metabolism.

Abstract:Parameters relating to hepatic microsomal drug metabolism were measured in vitro following 2 weeks exposure of rats to polybrominated biphenyls (PBB) as a dietary supplement at 4.69, 18.75, 75 and 300 ppm. The mixture of PBB was a potent inducer of hepatic microsomal drug metabolism. The pattern of induction seen after PBB exposure was compared with that produced by phenobarbital and 3-methyl-cholanthrene and the pattern of induction observed shared similarities with both of these agents.

Abstract:Transcranial ultrasound stimulation (TUS) has been clinically applied as a neuromodulation tool. Particularly, the phase array ultrasound can be applied in TUS to non-invasively focus on the cortex or deep brain. However, the vital phase distortion of the ultrasound induced by the skull limits its clinical application. In the current study, we aimed to develop a hybrid method that combines the ultrashort echo time (UTE) magnetic resonance imaging (MRI) sequences with the prDeep technique to achieve focusing ventral intermediate thalamic nucleus (VIM). The time-reversal (TR) approach of the UTE numerical acoustic model of the skull combined with the prDeep algorithm was used to reduce the number of iterations. The skull acoustic model simulation therapy process was establish to valid this method's prediction and focus performance, and the classical TR method were considered as the gold standard (GS). Our approach could restore 75% of the GS intensity in 25 iteration steps, with a superior the noise immunity. Our findings demonstrate that the phase aberration caused by the skull can be estimated using phase retrieval techniques to achieve a fast and accurate transcranial focus. The method has excellent adaptability and anti-noise capacity for satisfying complex and changeable scenarios.
