Abstract:<h3>Purpose/Objective(s)</h3> To evaluate the feasibility and safety of creating radiation treatment plans based on a convolutional neural network auto-contouring (AC) model for patients receiving radiation for localized prostate cancer with a radio-opaque rectal spacer hydrogel. <h3>Materials/Methods</h3> The deep learning model produces ACs for target volumes (prostate, proximal seminal vesicle), OARs (rectum, bladder, penile bulb, femoral heads), and rectal hydrogel spacers. ACs were compared to manual contours (MC) by MDs for clinical use. Individual volumes and overall performance (composite score) were previously evaluated and reported, qualitatively using a 1 (minor discrepancy), 2 (moderate discrepancy), 3 (significant discrepancy), and 4 (rejected, gross error) scale and quantitatively using Dice Similarity Coefficient (DSC) and mean distance to agreement. VMAT treatment plans were created on unedited AC volumes from five composite 1 (C1) cases and five composite 2 (C2) cases and dose volume metrics were computed for the final MCs. Dosimetry comparison was performed to assess the safety of delivering a treatment plan created from unedited ACs. Cases were planned to 50.4 Gy in 28 fractions to the SVs, with a SIB of 70 Gy the prostate. PTV expansions were CTV+5 mm (CTV+4 mm posteriorly). <h3>Results</h3> Of 62 initial cases evaluated, 11 were scored as a C1, 28 were scored as a C2, and 23 received a C3. 10 cases were randomly selected for treatment planning. For prostate (CTV P) in selected C1 and C2 cases, mean score was 1 and 1.8 and mean DSC was 0.919 and 0.876, respectively. Clinical goals for planning and dosimetric data from treatment plans are shown in Table 1. The mean V70 Gy for prostate CTV/PTV was 99.83%/90.05% for C1 cases and 99.46%/89.15% for C2 cases. All clinical goals for OARs were met in both cohorts. <h3>Conclusion</h3> 63% of all cases evaluated received a C1 or C2 score. Our results indicate the treatment plans generated from unedited ACs on these cases respected all clinical goals for OARs. Although C1 plans produced better PTV coverage than C2, almost all plans had sub-optimal PTV coverage, indicating the need for MDs to edit target volumes before treatment planning.
Automated Clinical Target Volume Delineation Using Deep 3D Neural Networks in Radiation Therapy of Non-small Cell Lung Cancer

Abstract:Background and purpose: Clinical targeted volume (CTV) delineation accounting for the patient-specific microscopic tumor spread can be a difficult step in defining the treatment volume. We developed an intelligent and automated CTV delineation system for locally advanced non-small cell lung carcinoma (NSCLC) to cover the microscopic tumor spread while avoiding organs-at-risk (OAR). Materials and methods: A 3D UNet with a customized loss function was used, which takes both the patients' respiration-correlated ("4D") CT scan and the physician contoured internal gross target volume (iGTV) as inputs, and outputs the CTV delineation. Among the 84 identified patients, 60 were randomly selected to train the network, and the remaining as testing. The model performance was evaluated and compared with cropped expansions using the shape similarities to the physicians' contours (the ground-truth) and the avoidance of critical OARs. Results: On the testing datasets, all model-predicted CTV contours followed closely to the ground truth, and were acceptable by physicians. The average dice score was 0.86. Our model-generated contours demonstrated better agreement with the ground-truth than the cropped 5 mm/8 mm expansion method (median of median surface distance of 1.0 mm vs 1.9 mm/2.0 mm), with a small overlap volume with OARs (0.4 cm(3) for the esophagus and 1.2 cm(3) for the heart). Conclusions: The CTVs generated by our CTV delineation system agree with the physician's contours. This approach demonstrates the capability of intelligent volumetric expansions with the potential to be used in clinical practice.

Abstract:Placental transfer of 14C-dimethyltubocurarine (14C-dmtc) was studied during Caesarean section. Liquid scintillation counting was used for determination of 14C activity from the maternal and foetal blood samples. Paper chromatography was used to confirm the stability of 14C-dmtc. In all cases 14C-dmtc was able to cross the placenta. Detectable amounts of 14C-dmtc were found in umbilical blood 2 min after injection into the mother. Six and 10 min after injection, the 14C concentration in the umbilical blood was 12% of the corresponding maternal value.
Automated plan generation for prostate radiotherapy patients using deep learning and scripted optimization

Abstract:Background and purpose: Treatment planning is a time-intensive task that could be automated. We aimed to develop a "single-click" workflow, fully deployed within a commercial treatment planning system (TPS), for autoplanning prostate radiotherapy treatment plans using predictions from a deep learning model (DLM). Materials and methods: Automatically generated treatment plans were created with a single script, executed from within a commercial TPS scripting environment, that performed two stages sequentially. Initially, a 3D dose distribution was predicted with a ResUNet DLM. The DLM was trained and validated using previously treated datasets (n = 120) which used 3D contours as inputs. Following this, dose predictions were converted into treatment plans by extracting dose-volume metrics from the predictions to use as objectives for the inverse optimizer within the TPS. An independent test dataset (n = 20) was used to evaluate the similarity between automated and clinical plans. Results: For planning target volumes, the median percentage difference and interquartile range between the automatically generated plans and clinical plans were 0.4% [0.2-1.1%] for the V100%, -0.5% [(-1.0)-(-0.2)%] for D99% and -0.5% [(-1.0)-(-0.2)%] for D95%. Bladder and rectum volume-at-dose objectives agreed within -6.1% [(-12.5)-0.9%]. The conversion of the DLM prediction into a treatment plan took 15 min [13-16 min]. Conclusions: An automatic plan generation workflow that uses a DL model with scripted optimization was fully deployed in a commercial TPS. Autoplans were compared to previously treated clinical plans and were found to be non-inferior.
Understanding and modeling human-AI interaction of artificial intelligence tool in radiation oncology clinic using deep neural network: a feasibility study using three year prospective data

Abstract:Background and purpose: Artificial intelligence (AI) based treatment planning tools are being implemented in clinic. However, human interactions with such AI tools are rarely analyzed. This study aims to comprehend human planner's interaction with the AI planning tool and incorporate the analysis to improve the existing AI tool. Materials and methods: An in-house AI tool for whole breast radiation therapy planning was deployed in our institution since 2019, among which 522 patients were included in this study. The AI tool automatically generates fluence maps of the tangential beams to create an AI plan. Human planner makes fluence edits deemed necessary and after attending physician approval for treatment, it is recorded as final plan. Manual modification value (MMV) maps were collected, which is the difference between the AI-plan and the final plan. Subsequently, a human-AI interaction (HAI) model using full scale connected U-Net was trained to learn such interactions and perform plan enhancements. The trained HAI model automatically modifies the AI plan to generate AI-modified plans (AI-m plan), simulating human editing. Its performance is evaluated against original AI-plan and final plan. Results: AI-m plan showed statistically significant improvement in hotspot control over the AI plan, with an average of 25.2cc volume reduction in breast V105% (p=0.011) and 0.805% decrease in Dmax (p<.001). It also maintained the same PTV coverage as the final plan, demonstrating the model has captured the clinic focus of improving PTV hot spots without degrading coverage. Conclusions: The proposed HAI model has demonstrated capability of further enhancing the AI plan via modeling human-AI tool interactions. This study shows analysis of human interaction with the AI planning tool is a significant step to improve the AI tool.

Automation of radiation treatment planning for rectal cancer

Abstract:Purpose: To develop an automated workflow for rectal cancer three-dimensional conformal radiotherapy (3DCRT) treatment planning that combines deep learning (DL) aperture predictions and forward-planning algorithms. Methods: We designed an algorithm to automate the clinical workflow for 3DCRT planning with field aperture creations and field-in-field (FIF) planning. DL models (DeepLabV3+ architecture) were trained, validated, and tested on 555 patients to automatically generate aperture shapes for primary (posterior-anterior [PA] and opposed laterals) and boost fields. Network inputs were digitally reconstructed radiographs, gross tumor volume (GTV), and nodal GTV. A physician scored each aperture for 20 patients on a 5-point scale (>3 is acceptable). A planning algorithm was then developed to create a homogeneous dose using a combination of wedges and subfields. The algorithm iteratively identifies a hotspot volume, creates a subfield, calculates dose, and optimizes beam weight all without user intervention. The algorithm was tested on 20 patients using clinical apertures with varying wedge angles and definitions of hotspots, and the resulting plans were scored by a physician. The end-to-end workflow was tested and scored by a physician on another 39 patients. Results: The predicted apertures had Dice scores of 0.95, 0.94, and 0.90 for PA, laterals, and boost fields, respectively. Overall, 100%, 95%, and 87.5% of the PA, laterals, and boost apertures were scored as clinically acceptable, respectively. At least one auto-plan was clinically acceptable for all patients. Wedged and non-wedged plans were clinically acceptable for 85% and 50% of patients, respectively. The hotspot dose percentage was reduced from 121% (σ = 14%) to 109% (σ = 5%) of prescription dose for all plans. The integrated end-to-end workflow of automatically generated apertures and optimized FIF planning gave clinically acceptable plans for 38/39 (97%) of patients. Conclusion: We have successfully automated the clinical workflow for generating radiotherapy plans for rectal cancer for our institution.

Abstract:Background Lung cancer poses a significant global health challenge. Adaptive radiotherapy (ART) addresses uncertainties due to lung tumor dynamics. We aimed to investigate a comprehensively and systematically validated offline ART regimen with high clinical feasibility for lung cancer. Methods This study enrolled 102 lung cancer patients, who underwent kV iterative cone‐beam computed tomography (iCBCT). Data collection included iCBCT and planning CT (pCT) scans. Among these, data from 70 patients were employed for training the UNet++ based deep learning model, while 15 patients were allocated for testing the model. The model transformed iCBCT into adaptive CT (aCT). Clinical radiotherapy feasibility was verified in 17 patients. The dosimetric evaluation encompassed GTV, organs at risk (OARs), and monitor units (MU), while delivery accuracy was validated using ArcCHECK and thermoluminescent dosimeter (TLD) detectors. Results The UNet++ based deep learning model substantially improved image quality, reducing mean absolute error (MAE) by 70.05%, increasing peak signal‐to‐noise ratio (PSNR) by 17.97%, structural similarity (SSIM) by 7.41%, and the Hounsfield Units (HU) of aCT approaching a closer proximity to pCT compared to kV iCBCT. There were no significant differences observed in the dosimetric parameters of GTV and OARs between the aCT and pCT plans, confirming the accuracy of the dose maps in ART plans. Similarly, MU manifested no notable disparities, underscoring the consistency in treatment efficiency. Gamma passing rates for intensity‐modulated radiation therapy (IMRT) and volumetric‐modulated arc therapy (VMAT) plans derived from aCT and pCT exceeded 98%, while the deviations in TLD measurements (within 2% to 7%) also exhibited no significant differences, thus corroborating the precision of dose delivery. Conclusion An offline ART regimen utilizing kV iCBCT and UNet++ based deep learning model is clinically feasible for lung cancer treatment. This approach provides enhanced image quality, comparable treatment plans to pCT, and precise dose delivery.

Abstract:Purpose: To investigate whether a deep learning-assisted contour (DLAC) could provide greater accuracy, inter-observer consistency, and efficiency compared with a manual contour (MC) of the clinical target volume (CTV) for non-small cell lung cancer (NSCLC) receiving postoperative radiotherapy (PORT).Materials and Methods: A deep dilated residual network was used to achieve the effective automatic contour of the CTV. Eleven junior physicians contoured CTVs on 19 patients by using both MC and DLAC methods independently. Compared with the ground truth, the accuracy of the contour was evaluated by using the Dice coefficient and mean distance to agreement (MDTA). The coefficient of variation (CV) and standard distance deviation (SDD) were rendered to measure the inter-observer variability or consistency. The time consumed for each of the two contouring methods was also compared.Results: A total of 418 CTV sets were generated. DLAC improved contour accuracy when compared with MC and was associated with a larger Dice coefficient (mean ± SD: 0.75 ± 0.06 vs. 0.72 ± 0.07, p < 0.001) and smaller MDTA (mean ± SD: 2.97 ± 0.91 mm vs. 3.07 ± 0.98 mm, p < 0.001). The DLAC was also associated with decreased inter-observer variability, with a smaller CV (mean ± SD: 0.129 ± 0.040 vs. 0.183 ± 0.043, p < 0.001) and SDD (mean ± SD: 0.47 ± 0.22 mm vs. 0.72 ± 0.41 mm, p < 0.001). In addition, a value of 35% of time saving was provided by the DLAC (median: 14.81 min vs. 9.59 min, p < 0.001).Conclusions: Compared with MC, the DLAC is a promising strategy to obtain superior accuracy, consistency, and efficiency for the PORT-CTV in NSCLC.

Abstract:This paper introduces a set of novel RRM philosophies: 1) the distributed Robin Hood philosophy, and 2) the self-organized proactive donation philosophy. By applying these philosophies with the soft-frequency reuse scheme, we proposed two novel inter-cell interference mitigation algorithms for the OFDMA based IMT advanced air interface: 1) distributed and dynamic power robbing (DDPR), and 2) self-organized and dynamic power shaping with proactive donation (SDPS/PD). Compared with the existing soft-frequency reuse schemes, our proposed algorithms are dynamic in the nature and more efficient and flexible. And for some scenarios, e.g. the user deployed wireless access points in a shopping center, the SDPS algorithm is the only possible interference mitigation solutions. Preliminary simulation results show that the DDPR algorithm can effectively reduce the average cell buffer size and average interference among the multi-cell environment.

Abstract:Background and Purpose To develop an artificial intelligence-based full-process solution for rectal cancer radiotherapy. Materials and Methods A full-process solution that integrates autosegmentation and automatic treatment planning was developed under a single deep-learning framework. A convolutional neural network (CNN) was used to generate segmentations of the target and the organs at risk (OAR) as well as dose distribution. A script in Pinnacle that simulates the treatment planning process was used to execute plan optimization. A total of 172 rectal cancer patients were used for model training, and 18 patients were used for model validation. Another 40 rectal cancer patients were used for an end-to-end evaluation for both autosegmentation and treatment planning. The PTV and OAR segmentation was compared with manual segmentation. The planning results was evaluated by both objective and subjective assessment. Results The total time for full-process planning without contour modification was 7 min, and an additional 15 min may require for contour modification and re-optimization. The PTV DICE similarity coefficient was greater than 0.85 for all 40 patients in the evaluation dataset while the DICE indices of the OARs also indicated good performance. There were no significant differences between the auto plans and manual plans. The physician accepted 80% of the auto plans without any further operation. Conclusion We developed a deep learning-based automatic solution for rectal cancer treatment that can improve the efficiency of treatment planning.

Artificial Intelligence-Assisted Delineation for Postoperative Radiotherapy in Patients with Lung Cancer: A Prospective, Multi-Center, Cohort Study

Abstract:Background:Postoperative radiotherapy (PORT) is an important treatment for lung cancer patients with poor prognostic features, but accurate delineation of the clinical target volume (CTV) and organs at risk (OARs) is challenging and time-consuming. Recently, deep learning-based artificial intelligent (AI) algorithms have shown promise in automating this process.Objective:To evaluate the clinical utility of a deep learning-based auto-segmentation model for AI-assisted delineating CTV and OARs in patients undergoing PORT, and to compare its accuracy and efficiency with manual delineation by radiation oncology residents from different levels of medical institutions.Methods:We previously developed an AI auto-segmentation model in 664 patients and validated its contouring performance in 149 patients. In this multi-center, validation trial, we prospectively involved 55 patients and compared the accuracy and efficiency of 3 contouring methods: (i) unmodified AI auto-segmentation, (ii) fully manual delineation by junior radiation oncology residents from different medical centers, and (iii) manual modifications based on AI segmentation model (AI-assisted delineation). The ground truth of CTV and OARs was delineated by 3 senior radiation oncologists. Contouring accuracy was evaluated by Dice similarity coefficient (DSC), Hausdorff distance (HD), and mean distance of agreement (MDA). Inter-observer consistency was assessed by volume and coefficient of variation (CV).Results:AI-assisted delineation achieved significantly higher accuracy compared to unmodified AI auto-contouring and fully manual delineation by radiation oncologists, with median HD, MDA, and DCS values of 20.03 vs. 21.55 mm, 2.57 vs. 3.06 mm, 0.745 vs. 0.703 (all P<0.05) for CTV, respectively. The results of OARs contours were similar. CV for OARs was reduced by approximately 50%. In addition to better contouring accuracy, the AI-assisted delineation significantly decreased the consuming time and improved the efficiency.Conclusion:AI-assisted CTV and OARs delineation for PORT significantly improves the accuracy and efficiency in the real-world setting, compared with pure AI auto-segmentation or fully manual delineation by junior oncologists. AI-assisted approach has promising clinical potential to enhance the quality of radiotherapy planning and further improve treatment outcomes of patients with lung cancer.
Machine learning automated treatment planning for online magnetic resonance guided adaptive radiotherapy of prostate cancer

Abstract:Background and purpose: No best practices currently exist for achieving high quality radiation therapy (RT) treatment plan adaptation during magnetic resonance (MR) guided RT of prostate cancer. This study validates the use of machine learning (ML) automated RT treatment plan adaptation and benchmarks it against current clinical RT plan adaptation methods. Materials and methods: We trained an atlas-based ML automated treatment planning model using reference MR RT treatment plans (42.7 Gy in 7 fractions) from 46 patients with prostate cancer previously treated at our institution. For a held-out test set of 38 patients, retrospectively generated ML RT plans were compared to clinical human-generated adaptive RT plans for all 266 fractions. Differences in dose-volume metrics and clinical objective pass rates were evaluated using Wilcoxon tests (p < 0.05) and Exact McNemar tests (p < 0.05), respectively. Results: Compared to clinical RT plans, ML RT plans significantly increased sparing and objective pass rates of the rectum, bladder, and left femur. The mean ± standard deviation of rectum D20 and D50 in ML RT plans were 2.5 ± 2.2 Gy and 1.6 ± 1.3 Gy lower than clinical RT plans, respectively, with 14 % higher pass rates; bladder D40 was 4.6 ± 2.9 Gy lower with a 20 % higher pass rate; and the left femur D5 was 0.8 ± 1.8 Gy lower with a 7 % higher pass rate. Conclusions: ML automated RT treatment plan adaptation increases robustness to interfractional anatomical changes compared to current clinical adaptive RT practices by increasing compliance to treatment objectives.
Automatic Treatment Planning Based on Three-Dimensional Dose Distribution Predicted from Deep Learning Technique.

Abstract:PURPOSE:To develop an automated treatment planning strategy for external beam intensity-modulated radiation therapy (IMRT), including a deep learning-based three-dimensional (3D) dose prediction and a dose distribution-based plan generation algorithm.METHODS AND MATERIALS:A residual neural network-based deep learning model is trained to predict a dose distribution based on patient-specific geometry and prescription dose. A total of 270 head-and-neck cancer cases were enrolled in this study, including 195 cases in the training dataset, 25 cases in the validation dataset, and 50 cases in the testing dataset. All patients were treated with IMRT with a variety of different prescription patterns. The model input consists of CT images and contours delineating the organs at risk (OARs) and planning target volumes (PTVs). The algorithm output is trained to predict the dose distribution on the CT image slices. The obtained prediction model is used to predict dose distributions for new patients. Then, an optimization objective function based on these predicted dose distributions is created for automatic plan generation.RESULTS:Our results demonstrate that the deep learning method can predict clinically acceptable dose distributions. There is no statistically significant difference between prediction and real clinical plan for all clinically relevant dose-volume histogram (DVH) indices, except brainstem, right and left lens. However, the predicted plans were still clinically acceptable. The results of plan generation show no statistically significant differences between the automatic generated plan and the predicted plan except PTV70.4 , but the difference is only 0.5% which is still clinically acceptable.CONCLUSION:This study developed a new automated radiotherapy treatment planning system based on 3D dose prediction and 3D dose distribution-based optimization. It is a promising approach for realizing automated treatment planning in the future.
Artificial Intelligence-Based Automated Treatment Planning of Postmastectomy Volumetric Modulated Arc Radiotherapy

Abstract:As a useful tool, artificial intelligence has surpassed human beings in many fields. Artificial intelligence-based automated radiotherapy planning strategies have been proposed in lots of cancer sites and are the future of treatment planning. Postmastectomy radiotherapy (PMRT) decreases local recurrence probability and improves overall survival, and volumetric modulated arc therapy (VMAT) has gradually become the mainstream technique of radiotherapy. However, there are few customized effective automated treatment planning schemes for postmastectomy VMAT so far. This study investigated an artificial intelligence based automated planning using the MD Anderson Cancer Center AutoPlan (MDAP) system and Pinnacle treatment planning system (TPS), to effectively generate high-quality postmastectomy VMAT plans. In this study, 20 patients treated with PMRT were retrospectively investigated, including 10 left- and 10 right-sided postmastectomy patients. Chest wall and the supraclavicular, subclavicular, and internal mammary regions were delineated as target volume by radiation oncologists, and 50 Gy in 25 fractions was prescribed. Organs at risk including heart, spinal cord, left lung, right lung, and lungs were also contoured. All patients were planned with VMAT using 2 arcs. An optimization objective template was summarized based on the dose of clinical plans and requirements from oncologists. Several treatment planning parameters were investigated using an artificial intelligence algorithm, including collimation angle, jaw collimator mode, gantry spacing resolution (GSR), and number of start optimization times. The treatment planning parameters with the best performance or that were most preferred were applied to the automated treatment planning method. Dosimetric indexes of automated treatment plans (autoplans) and manual clinical plans were compared by the paired t-test. The jaw tracking mode, 2-degree GSR, and 3 rounds of optimization were selected in all the PMRT autoplans. Additionally, the 350- and 10-degree collimation angles were selected in the left- and right-sided PMRT autoplans, respectively. The uniformity index and conformity index of the planning target volume, mean heart dose, spinal cord D 0.03cc , mean lung dose, and V 5Gy and V 20Gy of the lung of autoplans were significantly better compared with the manual clinical plans. An artificial intelligence-based automated treatment planning method for postmastectomy VMAT has been developed to ensure plan quality and improve clinical efficiency.

Abstract:Background/Objectives: To evaluate an end-to-end pipeline for normo-fractionated prostate-only and whole-pelvic cancer treatments that requires minimal human input and generates a machine-deliverable plan as an output. Methods: In collaboration with TheraPanacea, a treatment planning pipeline was developed that takes as its input a planning CT with organs-at-risk (OARs) and planning target volume (PTV) contours, the targeted linac machine, and the prescription dose. The primary components are (i) dose prediction by a single deep learning model for both localizations and (ii) a direct aperture VMAT plan optimization that seeks to mimic the predicted dose. The deep learning model was trained on 238 cases, and a held-out set of 86 cases was used for model validation. An end-to-end clinical evaluation study was performed on another 40 cases (20 prostate-only, 20 whole-pelvic). First, a quantitative evaluation was performed based on dose–volume histogram (DVH) points and plan parameter metrics. Then, the plan deliverability was assessed via portal dosimetry using the global gamma index. Additionally, the reference clinical manual plans were compared with the automated plans in terms of monitor unit (MU) numbers and modulation complexity scores (MCSv). Results: The automated plans provided adequate treatment plans (or minor deviations) with respect to the dose constraints, and the quality of the plans was similar to the manual plans for both localizations. Moreover, the automated plans showed successful deliverability and passed the portal dose verification. Despite higher median total MUs, no statistically significant correlation was observed between any of the gamma criteria tested and the number of MUs or MCSv. Conclusions: This study shows the feasibility of a deep learning-based fully automated treatment planning pipeline that generates high-quality plans that are competitive with manually made plans and are clinically approved in terms of dosimetry and machine deliverability.

Infants' Consumption of a New Food Enhances Acceptance of Similar Foods

Abstract:The number of feedings needed to increase intake of a novel target food was investigated, and whether exposure effects generalized to other foods in a sample of 4 to 7-month-old infants (N=39). Other foods varied in their similarity to the target food, including the same food prepared by another manufacturer, similar foods (other fruits for infants receiving a target fruit) and a different food (e. g. vegetables for infants receiving a target fruit). Infants were fed the target food once a day for 10 days. Intake was used to indicate acceptance. Results revealed that exposure dramatically increased infants' intake of the target food, from an average of 35-72 g. Intake of the different food was unchanged. Same and similar food intake increased with target food exposure. Intake of the target, same and similar foods nearly doubled to 60 g after one exposure to the target food. These rapid increases in intake contrast the slower changes seen in young children. Results for the other foods suggest that infants may have difficulty discriminating among many foods.
Fully automatic volumetric modulated arc therapy planning based on dose prediction combined with an iterative optimization algorithm

Abstract:Objective:To develope an automatic volumetric modulated arc therapy (VMAT) planning for rectal cancer based on a dose-prediction model for organs at risk(OARs) and an iterative optimization algorithm for objective parameter optimization.Methods:Totally 165 VMAT plans of rectal cancer patients treated in Peking University Cancer Hospital & Institute from June 2018 to January 2021 were selected to establish automatic VMAT planning. Among them, 145 cases were used for training the deep-learning model and 20 for evaluating the feasibility of the model by comparing the automatic planning with manual plans. The deep learning model was used to predict the essential dose-volume histogram (DVH) index as initial objective parameters(IOPs) and the iterative optimization algorithm can automatically modify the objective parameters according to the result of protocol-based automatic iterative optimization(PBAIO). With the predicted IOPs, the automatic planning model based on the iterative optimization algorithm was achieved using a program mable interface.Results:The IOPs of OARs of 20 cases were effectively predicted using the deep learning model, with no significantly statistical difference in the conformity index(CI) for planning target volume(PTV)and planning gross tumor volume(PGTV)between automatic and manual plans( P>0.05). The homogeneity index (HI) of PGTV in automatic and manual plans was 0.06 and 0.05, respectively( t=-6.92, P< 0.05). Compared with manual plans, the automatic plans significantly decreased the V30 for urinary bladder by 2.7% and decreased the V20 for femoral head sand auxiliary structure(avoidance)by 8.37% and 15.95%, respectively ( t=5.65, 11.24, P< 0.05). Meanwhile, the average doses to bladder, femoral heads, and avoidance decreased by 1.91, 4.01, and 3.88 Gy, respectively( t=9.29, 2.80, 10.23, P< 0.05) using the automatic plans. The time of automatic VMAT planning was (71.49±25.48)min in 20 cases. Conclusions:The proposed automatic planning based on dose prediction and an iterative optimization algorithm is feasible and has great potential for sparing OARs and improving the utilization rate of clinical resources.

Abstract:Objective. To demonstrate the feasibility of integrating fully-automated online adaptive proton therapy strategies (OAPT) within a commercially available treatment planning system and underscore what limits their clinical implementation. These strategies leverage existing deformable image registration (DIR) algorithms and state-of-the-art deep learning (DL) networks for organ segmentation and proton dose prediction. Approach: Four OAPT strategies featuring automatic segmentation and robust optimization were evaluated on a cohort of 17 patients, each undergoing a repeat CT scan. (1) DEF-INIT combines deformably registered contours with template-based optimization. (2) DL-INIT, (3) DL-DEF, and (4) DL-DL employ a nnU-Net DL network for organ segmentation and a controlling ROIs-guided DIR algorithm for iCTV segmentation. DL-INIT uses this segmentation alongside template-based optimization, DL-DEF integrates it with a dose-mimicking (DM) step using a reference deformed dose, and DL-DL merges it with DM on a reference DL-predicted dose. All strategies were evaluated on manual contours and contours used for optimization and compared with manually adapted plans. Key dose volume metrics like iCTV D98% are reported. Main results: iCTV D98% was comparable in manually adapted plans and for all strategies in nominal cases but dropped to 20 Gy in worst-case scenarios for a few patients per strategy, highlighting the need to correct segmentation errors in the target volume. Evaluations on optimization contours showed minimal relative error, with some outliers, particularly in template-based strategies (DEF-INIT and DL-INIT). DL-DEF achieves a good trade-off between speed and dosimetric quality, showing a passing rate (iCTV D98% > 94%) of 90% when evaluated against 2, 4 and 5mm setup error and of 88% when evaluated against 7mm setup error. While template-based methods are more rigid, DL-DEF and DL-DL have potential for further enhancements with proper DM algorithm tuning. Significance: Among investigated strategies, DL-DEF and DL-DL demonstrated promising within-10-minutes OAPT implementation results and significant potential for improvements.

A patient-specific auto-planning method for MRI-guided adaptive radiotherapy in prostate cancer

Abstract:Background and purpose: Fast and automated generation of treatment plans is desirable for magnetic resonance imaging (MRI)-guided adaptive radiotherapy (MRIgART). This study proposed a novel patient-specific auto-planning method and validated its feasibility in improving the existing online planning workflow. Materials and methods: Data from 40 patients with prostate cancer were collected retrospectively. A patient-specific auto-planning method was proposed to generate adaptive treatment plans. First, a population dose-prediction model (M0) was trained using data from previous patients. Second, a patient-specific model (Mps) was created for each new patient by fine-tuning M0 with the patient's data. Finally, an auto plan was optimized using the parameters derived from the predicted dose distribution by Mps. The auto plans were compared with manual plans in terms of plan quality, efficiency, dosimetric verification, and clinical evaluation. Results: The auto plans improved target coverage, reduced irradiation to the rectum, and provided comparable protection to other organs-at-risk. Target coverage for the planning target volume (+0.61 %, P = 0.023) and clinical target volume 4000 (+1.60 %, P < 0.001) increased. V2900cGy (-1.06 %, P = 0.004) and V1810cGy (-2.49 %, P < 0.001) to the rectal wall and V1810cGy (-2.82 %, P = 0.012) to the rectum were significantly reduced. The auto plans required less planning time (-3.92 min, P = 0.001), monitor units (-46.48, P = 0.003), and delivery time (-0.26 min, P = 0.004), and their gamma pass rates (3 %/2 mm) were higher (+0.47 %, P = 0.014). Conclusion: The proposed patient-specific auto-planning method demonstrated a robust level of automation and was able to generate high-quality treatment plans in less time for MRIgART in prostate cancer.
