Abstract:Background and study aim: Prior studies have suggested that music therapy can provide stress relief and analgesia. In this meta-analysis we focused on the effects of music therapy on patients undergoing gastrointestinal endoscopic procedures. Materials and methods: A literature search using the PubMed and Cochrane Library databases and a manual search led to the inclusion of six randomized controlled trials that examined the effects of music therapy on patients undergoing gastrointestinal endoscopic procedures. After data extraction, four separate meta-analyses were performed: in the three studies that did not use pharmacotherapy (group A), anxiety levels were used as a measure of efficacy; in the three studies in which pharmacotherapy was used (group B), sedation and analgesia requirements and procedure duration times were analyzed. Results: A total of 641 patients were included in the analysis. In group A, patients receiving music therapy exhibited lower anxiety levels (8.6% reduction, P = 0.004), compared with controls. In group B, patients receiving music therapy exhibited statistically significant reductions in analgesia requirements (29.7% reduction, P = 0.001) and procedure times (21% reduction, P = 0.002), and a reduction in sedation requirements that approached significance (15% reduction, P = 0.055), in comparison with controls. Conclusions: Music therapy is an effective tool for stress relief and analgesia in patients undergoing gastrointestinal endoscopic procedures.

Abstract:Combinations of immunosuppressive drugs are routinely used post-transplantation to prevent rejection and/or other complications and optimize outcomes. The prodrug ester mycophenolate mofetil (MMF) is frequently used in solid-organ and stem cell transplantation settings. A growing body of evidence supports therapeutic monitoring of this immunosuppressant to optimize its efficacy and reduce toxicity. Thus, pharmacodynamic monitoring of MMF is a strategy that could potentially improve patient outcomes. Pharmacodynamic measurements require evaluation of inosine-5'-monophosphate dehydrogenase (IMPDH) activity, the target enzyme of the active moiety mycophenolic acid. Various nonradioactive methods using chromatographic separations have been used to quantify xanthosine monophosphate, the catalytic product of the enzyme, to indirectly evaluate IMPDH activity. However, no methods have used mass spectrometry based detection, which provides more specificity and sensitivity. Here, we describe a liquid chromatography-coupled tandem mass spectrometry (LC-MS/MS) method for the quantification of xanthosine monophosphate and adenosine monophosphate (for normalization) in lysates of peripheral blood mononuclear cells (PBMCs) from hematopoietic stem cell transplant (HSCT) recipients. Linearity, precision, and accuracy were validated over a large range of concentrations for each compound. The method could measure analytes with high sensitivity, accuracy (93-116%), and reproducibility (CV < 7.5%). Its clinical application was validated in PBMC lysates obtained from healthy individuals (n = 43) and HSCT recipients (n = 19). This reliable and validated LC-MS/MS method could be a useful tool for pharmacodynamic monitoring of patients treated with MMF.

Abstract:Background: The authors aim to investigate the effect of music on hemodynamic fluctuations during induction of general anesthesia and reducing preoperative anxiety for women who underwent elective non-cardiac surgery. Methods: It is a multicenter, double-blind, randomized, parallel-group clinical trial. Patients were randomized 1:1 to either a Music Intervention group (MI) or a Control group (Control). The MI participants listened to their preferred music for more than 30 minutes in the waiting area. The State-Trait Anxiety Inventory (STAI) was used to measure anxiety levels in the groups, and hemodynamic parameters (Heart Rate [HR], Mean Arterial Pressure [MAP]) were continuously recorded before induction (T0), at loss of consciousness (T1), immediately before intubation (T2), and after intubation (T3). Intubation-related adverse events were also recorded. The primary outcome was the incidence of MAP changes more than 20 % above baseline during T0-T2. Results: A total of 164 patients were included in the final analyses. The incidence of MAP instability during T0-T2 was lower in the MI, and the 95 % Confidence Interval for the rate difference demonstrated the superiority of MI. HR instability was less frequent in MI participants both in T0-T2 and T2-T3. The overall incidence of preoperative anxiety was 53.7 % (88/164). After the music intervention, the mean score of STAI was significantly lower in the MI than in the Control, with a between-group difference of 8.01. Conclusions: Preoperative music intervention effectively prevented hemodynamic instability during anesthesia induction and significantly reduced preoperative anxiety in women undergoing elective non-cardiac surgery.

Abstract:Background & objective: Perioperative anxiety affects 25-80% of all surgical patients. It has a negative impact on postoperative recovery, potentially increasing morbidity and mortality. The anxiolytic effects of music therapy are well recognized, but its cognitive implications are unknown. We examined the effects of 440 Hz and 432 Hz music therapy on pre-operative anxiety and cognitive functions in patients scheduled for surgery. Methodology: The study took place in the operating room of our hospital. Following ethical committee approval, the study was launched. It was a prospective observational study comprising 66 ASA I, II, and III patients, between the ages of 18 and 65, scheduled to undergo surgery. They were divided into three groups: Control Group (CG), music therapy at 440 Hz (MG440), and music therapy at 432 Hz (MG432). Music therapy was administered to MG440 and MG432, but not to CG. Patients took the Montreal Cognitive Assessment (MoCA) and the Beck Anxiety Scale (BAS). MoCA and BAS were assessed before surgery, one hour and at 24 hours following surgery. Results: In both groups, preoperative music therapy significantly reduced anxiety (P < 0.001). Anxiety levels following surgery were comparable in all groups. Postoperative one-hour cognitive performance in music therapy groups was lower than in controls (P < 0.001), but increased and became significantly higher than in the control group at 24 h (P < 0.001). There was no substantial difference between the 440 Hz and 432 Hz groups. Conclusions: Preoperative music therapy reduced anxiety; it initially lowered postoperative cognitive performance; nevertheless, within 24 hours, therapy groups had enhanced cognitive skills when compared to controls. Abstract:Premedication with sedative drugs is one of the most preferred methods to reduce anxiety. Due to concerns about maternal and fetal side effects, their use in obstetric anesthesia is not preferred. The primary aim of the authors of this study is to investigate the effect of music on perioperative anxiety in pregnant. The patients were randomized into three groups; Group M was listened to music during the surgery, Group S isolated from ambient noise and Group C was the control group. Anxiety status was measured at the preoperative anesthesia examination, on the morning of surgery, and at the first hour postoperatively. Although we observed the lowest postoperative anxiety in Group M, it wasn't statistically significant. Music is an easy-to-apply and no-side-effect method. Therefore, we believe that more clinical studies with more patients are needed for clearer results.

Abstract:Effect of Music Therapy on Dental Anxiety in Periodontal Surgery Introduction: Pharmacological or non-pharmacological methods are applied to eliminate fear and anxiety problems related to dental treatments in many patients. As a non-invasive, non-pharmacological method with an anxiolytic effect, music therapy is preferred in the management of anxiety and fear. This study aimed to investigate the effect of music therapy on dental anxiety in patients scheduled to undergo periodontal surgery. Material and Method: The study included a total of 46 patients, 23 in the study group and 23 in the control group, who were scheduled to undergo periodontal surgery. During the surgical procedure, the patients in the study group listened to music using in-ear headphones that isolated ambient sounds. The patients in the control group wore in-ear headphones to isolate ambient sounds, but they did not listen to any music. All patients were asked to complete the Modified Dentistry Anxiety Scale (MDAS), the State-Trait Anxiety Inventory-State (STAI-S), and the Dental Fear Survey (DFS) before and after surgery. Systolic blood pressure (SBP) and diastolic blood pressure were measured as vital signs before and after surgery, and the Visual Analog Scale (VAS) scores were recorded twice (at the end of and 30 minutes after the surgical procedure). Results: In the study group, the post-test MDAS, STAI-S, and DFS measurement values showed statistically significant decreases compared to the pre-test measurement values (P<0.001, P=0.009, and P<0.001, respectively), but there was no statistically significant difference in the control group (P=0.093, P=0.923, and P=0.460, respectively). In both the study and control groups, the post-test VAS measurement values statistically significantly decreased compared to the pre-test measurement values (P=0.002 and P=0.019, respectively). While the post-test SBP values decreased statistically significantly in the study group compared to the pre-test measurement values (P=0.020), there was no statistically significant difference in the control group (P=0.705). No statistically significant difference was found between the pre-test and post-test measurement values of the DFS scores in the study or control group (P=0.083 and P=0.160, respectively). Clinical significance: This study is the first clinical study to evaluate the effect of music on anxiety in periodontal surgery simultaneously with four different scales. Music therapy was found to be effective in reducing dental anxiety and fear during periodontal surgery.

Abstract:BACKGROUND: A cardiac catheterization laboratory can be a frightening environment and music can be a supportive source of environmental sound that stimulates and maintains relaxation. However, the results of studies are conflicting in this regard.OBJECTIVES: The aim of this study was to investigate the effect of music therapy on patients' anxiety and hemodynamic parameters during percutaneous transluminal coronary angioplasty.PATIENTS AND METHODS: This was a randomized controlled trial, conducted in the Catheterization Laboratory Unit of Baqiyatallah Hospital, in Tehran, Iran. A sample of 64 patients, who were planned to undergo coronary angioplasty, was recruited. Patients were randomly allocated to either the control or the experimental groups. In the experimental group, patients received a 20 to 40-minute music therapy intervention, consisting of light instrumental music albums by Johann Sebastian Bach and Mariko Makino. Patients in the control group received the routine care of the study setting, which consisted of no music therapy intervention. Study data were collected by a demographic questionnaire, the Spielberger's State Anxiety Inventory, and a data sheet for documenting hemodynamic parameters. Chi-square, independent-samples t tests, paired-samples t-test and repeated measures analysis of variance were used to analyze the data.RESULTS: Before the intervention, the study groups did not differ significantly in terms of anxiety level and hemodynamic parameters. Moreover, the differences between the two groups, regarding hemodynamic parameters, were not significant after the intervention (P > 0.05). However, the level of post-intervention anxiety in the experimental group was significantly lower than the control group (32.06 ± 8.57 and 38.97 ± 12.77, respectively; P = 0.014). Compared with the baseline readings, the level of anxiety in the control group did not change significantly after the study (41.91 ± 9.88 vs. 38.97 ± 12.77; P = 0.101); however, in the experimental group, the level of post-intervention anxiety was significantly lower than the pretest readings (32.06 ± 8.57 vs. 41.16 ± 10.6; P = 0.001).CONCLUSIONS: Music therapy is a safe, simple, inexpensive, and non-invasive nursing intervention, which can significantly alleviate patients' anxiety during coronary angioplasty.

Abstract:A colonoscopy is a procedure commonly used for the diagnosis of colorectal diseases but may be associated with high levels of anxiety and discomfort, which can lead to complications during sedation and decreased patient comfort and cooperation. This study was conducted at a tertiary care hospital in Karachi to assess the effectiveness of music therapy on pre-procedural anxiety, sedation requirements, pain, and procedure duration among patients undergoing colonoscopies. This comparative study included 110 outpatients. The intervention arm received non-lyrical soft music through earphones before and during the colonoscopy procedure, while the control group did not receive any music. In the intervention arm, significant reductions were seen in pre-procedural anxiety scores (p < 0.001), physiological measures (p < 0.001), sedation doses (p < 0.001), and procedure durations (p < 0.05). Control arm showed an increase in heart rate and systolic blood pressure (p < 0.001). Pain scores were similar for both groups. Music therapy is an effective tool to reduce patient anxiety pre- procedure. It is a simple, safe, and noninvasive relaxing intervention that could be used as an adjunct to sedative medications before and during the colonoscopy procedure.

Abstract:In this special review edition ofMolecular Human Reproduction, we examine the impact of cell cleavage timing and morphology on the viability of preimplantation embryos. The concept of cleavage timing and morphology is brought into relevance by initially examining how cleavage planes, position and number in the early cleavage embryo impact developmental potential in different models, from flies to human. Finally, the historical use of cleavage assessment and morphology in human IVF in addition to the current clinical use of time-lapse imaging is discussed. Preimplantation embryo morphology and the timing of cleavage has been a mainstay for the clinical embryologist in human IVF. In the first reported case of IVF and embryo development in the human, Rock and Menkin (1944) made the following observation about the developing fertilized embryo: ‘One of these (embryo), when first seen in cleavage, consisted of one large blastomere and two smaller ones, each of the three containing a round, vesicular nucleus. The second egg from this same patient was in a similar stage, but part of the cytoplasm appeared fragmented, and soon proceeded to undergo rapid degenerative changes.’ Since that first observation, embryologists working in the field of IVF have relied largely on examining the morphology and timing of embryo development to select the best embryo. The underlying importance of these two observations has, however, not always been appreciated and the need of the cells within the embryo to cleave in the correct manner and to be in the right place at the right time has sometimes been overlooked by the clinical embryologist. The first two reviews (Ajduk and Zernicka-Goetz, 2016; Piliszek et al. 2016) examine the question of ‘Is the way a cell divides and where it goes important?’ In these reviews, we gain a better insight into how cleavage patterns impact molecular expression patterns and subsequent viability in species that have a more definitive animal and vegetal pole. We learn that despite the diversity in cleavage patterns between phyla, the molecular mechanisms regulating early embryonic divisions remain strongly conserved. The concept that cleavage division can lead to cells of not only different sizes but inherently different components is introduced and how this ultimately causes cells to follow distinct developmental paths. The observation of how the viability of a mouse embryo is affected by the pattern of segregation and inheritance of material from the daughter blastomere which undergoes at least one meridional division sheds light on the subtle importance of correct cleavage (Ajduk and Zernicka-Goetz, 2016). Piliszek et al. (2016) also discuss cell fate in mouse and human blastocysts again highlighting the importance of cells being in the correct place at the right time. This impacts the concept of just scoring blastocysts in human IVF and highlights the question: are we missing aspects of their development by just scoring expansion, inner cell mass and trophectoderm? The importance of cell positioning in the blastocyst also leads to questions of how much information we can glean from a trophectoderm biopsy when performing preimplantation genetic screening. In the third review, Gardner and Balaban (2016), the key features of the human preimplantation embryo are considered in light of historical data on the relationship between morphological characteristics and developmental potential. They argue that these observations have not only taught us about embryo potential, but also inform us about the impact of the culture environment on morphological traits and how key morphological qualities reflect aspects of embryo physiology. The importance of these observations has been an ongoing discussion for human embryos; however, it has now come under more scrutiny with the advent of video imaging or time-lapse systems. The final review from the group of Meseguer (Motato et al., 2016) discusses how timelapse systems have provided us with a multitude of data which are now being used to track cleavage patterns and timing more precisely. They show how time-lapse systems have taken the evaluation of embryo morphology and developmental kinetics to another level and how it is starting to impact clinical practice. Unfortunately, however, further studies are required to improve the quality of the current evidence. Our excitement in being able to visualize the preimplantation human embryo in its first few days of development using time-lapse systems is best summarized by Gardner and Balaban (2016), who stated that ‘it has reopened our eyes and given us a new vantage from which to view the beauty of the initial stages of human life’. Abstract:We report the synthesis of a hydrophilic copolymer with one polyethylene glycol (PEG) block and one β-cyclodextrin (β-CD) containing block by a "click" reaction between azido-substituted β-CD and propargyl flanking copolymer. (1)H NMR study suggested a highly efficient conjugation of β-CD units by this approach. The obtained copolymer was used as a host macromolecule to construct assemblies in the presence of hydrophobic guests. For assemblies containing a hydrophobic polymer, their size can be simply adjusted by simply changing the content of hydrophobic component. By serving as a guest molecule, hydrophobic drugs can also be loaded accompanying the formation of nanoparticles, and the drug payload is releasable. Therefore, the copolymer synthesized herein can be employed as a carrier for drug delivery.

Abstract:Background: As with any other invasive procedure, esophagogastroduodenoscopy (EGD) may lead to considerable anxiety in patients. This study aimed to investigate and compare the effects of sedated and non-sedated procedures on anxiety in patients undergoing EGD and to better recognize patient groups at risk for anxiety. Methods: In this prospective and 2-armed designed study, demographic data, including age, gender, comorbidities, height, weight, body mass index (BMI), and educational background, were collected. In this study, the Beck Anxiety Inventory (BAI) was administered to each patient before they were brought to the endoscopy unit. Subsequently, each patient who underwent EGD was telephoned on the seventh day after the procedure and the BAI was administered a second time. Results: Women population had higher pre-EGD and post-EGD BAI scores compared to men. No significant correlation was observed between educational background and BAI scores. Possible correlations between age, BMI, Charlson comorbidity index (CCI), and BAI scores were examined. There was a weak correlation between BMI and pre-EGD and post-EGD BAI scores. A strong and positive correlation was observed between the pre-EGD BAI score and post-EGD BAI and difference in BAI scores between groups (ΔBAI). Conclusion: Endoscopic procedures may cause anxiety in patients as with all other invasive procedures. Patients' compliance with the procedure and having a lower level of anxiety are very significant for diagnostic and, if necessary, therapeutic success. In this study, the patient gender was evaluated as a predictor of anxiety level, whereas educational background was not a predictor.

Abstract:Background The noise in an operating room may have a detrimental effect on human cognitive functions, and it may cause perioperative anxiety with prolonged exposure. The aim of this study was to investigate the effects of music therapy and use of earplugs and normal noise level in the operating room under general anesthesia of pediatric patients on hemodynamic parameters and postoperative emergence delirium. Methods One hundred and five pediatric patients were involved in this study. The patients were randomly divided into three groups. Group N was exposed to the ambient operating room noise, group S received earplugs from an independent anesthesiologist, and group M used a CD player. The preoperative anxiety levels of children were evaluated with the Modified Yale Preoperative Anxiety Scale (M-YPAS). Mean arterial pressure (MAP) and heart rate were recorded at 30-minute periods until the completion of surgery, end of surgery and postoperatively. During each measurement, noise level recordings were performed using sonometer. Pediatric Anesthesia Emergency Delirium (PAED) score was evaluated after postoperative extubation. Results M-YPAS was similar between groups. The MAP at 30 and 60 min intraoperatively, at end of surgery, and at 5, 10, and 15 min postoperatively was significantly lower in group S than in group N. There were no differences in heart rate among the groups. Postoperative PAED score was not significantly different among the groups. Conclusions The music therapy was not more effective than silence and operating noise room in reducing PAED score postoperatively in pediatric patients. Monaural beat stimulation is achieved by the superposition of amplitude-modulated signals of nearby frequencies to both ears simultaneously. Aim: The study aims to evaluate and compare anxiety level during restorative treatment using no music, monaural beats, and binaural auditory beats as audio distraction behavior guidance techniques in children aged 6–12 years. Settings and Design: Three-arm, parallel-group randomized study was conducted in the department of pediatric and preventive dentistry. Materials and Methods: The sample size of 45 participants was calculated and allocated into three groups as no music, monaural beats, and binaural auditory beats group. Teeth with Class I cavity in primary or permanent molars were prepared and restored. Anxiety was measured pre and postintervention using pulse oximeter and Venham’s picture test. Statistical Analysis Used: Data obtained were analyzed with SPSS software version 20.0 and were carried out using one-way analysis of variance and dependent t -test and significance value < 0.05 was considered. Results: Binaural auditory beats and monaural beats showed a better reduction in anxiety levels as per Venham’s picture test scores with statistically significant “ P ” value 0.0001 and 0.064, respectively. Likewise, binaural auditory beats showed better reduction in anxiety levels as per pulse rate scores with statistically significant “ P ” value 0.0001 as compared to conventional treatment technique. Conclusion: Binaural auditory beat was found to be the most effective followed by the monaural beat compared to the control group. Thus, binaural auditory beats and monaural beats can be used effectively as an essential aid to reduce anxiety in children.

Abstract:Moyamoya disease is characterized occlusion of Willis' artery ring and abnormal "moyamoya" vessels. By supplying sufficient blood flow to the distal area of the stenosis or occlusion, ischemic symptom could be improved and the risk of the hemorrhage are lessen. Encephalo-Myo-Synangiosis (EMS) can increase the blood supply from external carotid artery to the ischemic area just put the temporal muscle on the brain surface. A kind of growth factor are thought to be exist around the brain of the Moyamoya disease patient. Basic fibroblast growth factor (bFGF) contribute angiogenesis in vitro and in vivo. This effect is considered to grow tumors and many experiments are done to use it for therapy by blocking this effect. Few trials are done to utilize the angiogenetic effect for therapy. We studied the effect of the bFGF on angiogenesis after EMS by using rats. We operated EMS on 10 male SDrats. 0.1 microgram of the bFGF was poured on 5 rats between the brain and muscle. On the other 5 rats just saline was poured. One week after rats were sacrificed, new vessels were observed with light microscope and scanning electron microscope. Molding models were also observed. bFGF group grew larger new vessels between the brain and muscle than non bFGF group. On the surface of brain, bFGF had more larger vessels (diameter is over 6 um) and non bFGF group had more small vessels (less than 6 um). Total area of vessel of bFGF group was twice as large as that of non bFGF group. On molding models many closed end of vessels were seen and they were thought to be the growing vessels. In molding models, bFGF group also has larger vessels than non bFGF group. We could prove that bFGF promotes angiogenesis on EMS of the rats, and we also expect that bFGF help the supplying blood flow of the Moyamoya patient.

Abstract:Background: Dental anxiety is a significant and challenging problem in patients seeking dental treatment. During any dental procedure patients often experience severe pre-operative and intra-operative anxiety which makes the procedure more difficult. Anxiety during endodontic therapy often leads to increased pain perceptions and instability of the physiological parameter throughout treatment. Aim: The purpose of this study was to evaluate the influence of music as a nonpharmacologic aid on anxiety, pain perception and physiological parameters (heart rate, systolic blood pressure, and diastolic blood pressure) at three different time period - before, during, and after endodontic treatment. Methods: A total of 100 patients were recruited in the present study. Before starting the endodontic treatment, the interviewer administered the Modified Corah Dental Anxiety Scale (MCDAS) to assess the baseline level of anxiety and Visual Analog Scale (VAS) for pain assessment. Patients were randomly divided into 2 groups: the first one listened to the music during endodontic treatment and the second one did not. Before, during, and after the endodontic procedure, the physiological parameters (i.e. heart rate, systolic and diastolic blood pressure) were recorded. All the data were collected and statistically analyzed. Results: Direct contrasts between patients listening or not listening to music showed that all the measured physiological parameters improved considering the overall period (during and after the root canal therapy) in the group of patients listening to music (P < .05). Conclusion: Within the limitation of the study, music administered to subjects with different levels of anxiety during endodontic treatment significantly decreased pain levels and improved physiological parameters during the endodontic procedures. Clinical Significance: Music and medicine always work together and the soothing effects of sounds and musical frequencies make this union an extraordinary tool of synergistic care. Music therapy can be considered as a valid nonpharmacologic aid to manage anxiety during endodontic treatment.
A pilot study exploring the effectiveness of binaural beats at reducing anxiety associated with cadaveric dissection

Abstract:This study explored the effects of a novel intervention, binaural beats (BB), on anxiety levels of anatomy students during cadaveric dissections and its impact on the learning environment. The study was quasi-experimental, employing a purposive sampling strategy. State (SA) and trait (TA) anxiety levels were measured using the State-Trait Anxiety Inventory during two routine dissection sessions. SA was measured at three time points: at the start, after a 3-min control (silence)/intervention (10 Hz BB) session, and at the end of the dissection. A post-study survey was distributed to the participants to obtain qualitative feedback on their perceptions of BB use during cadaveric dissections. Thirteen female students participated in this study (mean age 19.62 ± 0.65 years). Both control and intervention measures significantly reduced SA levels, with BB demonstrating a greater effect size (Hedges' g = 0.98 for intervention vs. 0.76 for control). SA decreased significantly after the BB intervention (t(12) = 3.78, p = 0.003) and remained low throughout the session, in contrast to the control session, where the reduction post-measure was not sustained (t(12) = -0.64, p = 0.54). Feedback from the post-study survey indicated that most students found the BB intervention effective in reducing their anxiety levels before the dissection. The BB intervention was effective in reducing and maintaining students' SA levels during dissection. Students reported positive experiences with BB. Further investigations are warranted to determine whether the findings of this study can be generalized to larger and more heterogeneous populations.
