Abstract:Objectives This study was conducted to investigate the effect of music on anxiety, pain, and physiologic parameters in women undergoing brachytherapy. Data Sources The study was conducted with a randomized controlled design between June and December 2022. Music intervention was practiced to the patients in the experimental group (n = 30), while no practice was given to the control group (n = 25). In the study, the data were collected using the Descriptive Information Form, Hospital Anxiety and Depression Scale , visual analog scale, and vital signs (temperature, pulse rate, blood pressure, respiratory rate, SpO 2 ) recording form. Intergroup and intragroup averages were evaluated by the mixed-design analysis of variance. Variables with pre-post intervention designs were evaluated with the one-way analysis of covariance. There was no significant difference in physiological parameters between the groups ( P > . 05). Although the mean anxiety scores decreased in the music group and increased in the control group, there was no statistically significant difference ( P > .05). A significant difference was found in the pain levels of the patients in repeated measurements made in intragroup evaluations ( P < .001). The depression mean of the music intervention group was significantly lower than the control group ( P ≤ .05). Conclusion It was concluded that the music played during the brachytherapy process had positive effects on feeling less pain and management of depressive symptoms but did not affect anxiety and physiological parameters. Since each patient is different, the effects of music therapy can vary individually. Implications for Nursing Practice Nurses should apply different strategies to investigate session frequencies and durations for different patient groups and treatment stages in cancer care.

Abstract:Purpose: To evaluate the evidence from randomized clinical trials (RCTs) about the effect of music intervention in reducing patients' anxiety during breast biopsy. Methods: Electronic databases including PubMed, Cochrane Library, Scopus, and Web of Science were searched using the relevant MeSH terms. The inclusion criteria were all RCTs assessing the effect of music therapy versus no music in reducing anxiety during breast biopsy. The extracted outcomes were anxiety and pain during breast biopsy. They were pooled as mean difference (MD) with a 95% confidence interval (CI) in a fixed-effects model, using Review Manager 5.3 software for windows. The quality of included studies was assessed with the Cochrane risk of bias assessment tool (RoB 1.0). Then, the outcomes of our meta-analyses were independently evaluated by the Grading of Recommendations Assessment, Development and Evaluation (GRADE) to know the grade of their evidence. Results: The final analysis included five RCTs. We found a positive effect of music therapy in reducing anxiety levels compared with control group (MD = - 2.11; 95% CI (- 4.16 to - 0.06); p = 0.04). No difference between music and control groups regarding pain associated with breast biopsy (MD = 0.22; 95% CI (- 0.81 to 1.25); p = 0.68). The GRADE rating of our outcomes was low for anxiety levels and very low for pain during the biopsy. Conclusions: Music therapy could be an effective, simple, non-pharmacological option in relieving anxiety during breast biopsy; however, it had no effect on procedure-associated pain. More large and high-quality studies are needed to confirm our results.

Abstract:We report the synthesis of a hydrophilic copolymer with one polyethylene glycol (PEG) block and one β-cyclodextrin (β-CD) containing block by a "click" reaction between azido-substituted β-CD and propargyl flanking copolymer. (1)H NMR study suggested a highly efficient conjugation of β-CD units by this approach. The obtained copolymer was used as a host macromolecule to construct assemblies in the presence of hydrophobic guests. For assemblies containing a hydrophobic polymer, their size can be simply adjusted by simply changing the content of hydrophobic component. By serving as a guest molecule, hydrophobic drugs can also be loaded accompanying the formation of nanoparticles, and the drug payload is releasable. Therefore, the copolymer synthesized herein can be employed as a carrier for drug delivery.

Abstract:Introduction High-resolution ultrasonography devices have led to the increased detection of thyroid nodules and the need for fine-needle aspiration biopsy (FNAB). However, FNAB is an invasive procedure that can cause discomfort and pain. Music therapy has been used for centuries to alleviate pain, and our clinical trial was conducted to investigate its impact on pain scores during thyroid FNAB. Materials and methods We conducted a randomized, controlled clinical trial, including adult patients undergoing thyroid FNAB. We evaluated the non-pharmacological intervention of listening to music before and during thyroid FNAB. We used a slow, non-lyrical, flowing melody with a tempo of 60 to 80 beats per minute as a therapeutic intervention for managing anxiety and pain. It had low tones, minimal percussion, and a volume of around 60 decibels. The Beck Anxiety Inventory and Visual Pain Scale were used. Results The study included 529 patients assigned to either the music group (n=258, 48.7%) or the control group (n=271, 51.2%). The patients were categorized into subgroups based on either a single nodule (73.63%) or multiple nodules (24.18%). When all patients or single nodule and multiple data are examined separately, the intervention group showed statistically significantly lower pain scores than the control group. Conclusion Our study is among the first on this topic and the largest in the available literature to demonstrate that listening to music during FNAB significantly reduces pain and anxiety. Music therapy is an effective, safe, and noninvasive intervention that can improve patient care and reduce distress and pain.

Effect of music therapy on patients with rheumatic diseases

Abstract:Background: Music therapy seems to have a multitude of positive effects not only on mental health but also on organic pain. Some studies have showed its effectiveness on reducing pain for patients undergoing surgical procedures. However, data on its efficiency on patients' anxiety during rheumatology medical consultation for patients with chronic rheumatic diseases is lacking. Objectives: The purpose of the present study was to describe the impact of music therapy on the mood of patients followed for chronic rheumatic diseases. Methods: We conducted a cross-sectional study including patients followed for chronic rheumatic diseases seen in the outpatient rheumatology department during a period of three weeks. Patients selected were randomly assigned into two groups. Patients seen while listening to music "The Mozart Sonata" formed theG1 (n = 40). Patients seen without listening to music formed the group G2 (n = 30). Participants' anxiety levels were assessedbefore and immediately after medical consultations by collecting vital signs (heart rate and respiratory rate) and by the Spielberger State Trait Anxiety inventory questionnaire (STAI). Results: The mean age of 70 patients included was 54.36 ± 14.62 years. Females outnumbered males (62.9 %).The STAI scores were 38.44 (range, 25-60) and 34.51 (range, 22-52)respectively pre- and post-consultation. The medical consultation lasted for a mean of 24 ± 1.1 min in G1 and 20.63 ± 1.3 in G2. Sharp after the consultation, the mean Heart Ratewas significantly lower in the group with music therapy (64±1.5 in G1 versus 66.3 ± 1.3 in G2, p = 0.02) and the mean STAI decreased also significantly in tne G1 (34.72±1 in G1 versus 40.7 ± 5.2 in G2, p = 0.018). However, the decline of the mean Respiratory Ratewas not significant between both groups(16.1 ± 1 in G1 versus 16.96 ± 1.7 in G2, p = 0.42). Conclusion: Our study showed a significant lowering in anxiety level and heart rate in patients consulted while listening to music.This costless and available tool should be more used in our daily practice specially when treating patients with chronic painful diseases.

Abstract:Combinations of immunosuppressive drugs are routinely used post-transplantation to prevent rejection and/or other complications and optimize outcomes. The prodrug ester mycophenolate mofetil (MMF) is frequently used in solid-organ and stem cell transplantation settings. A growing body of evidence supports therapeutic monitoring of this immunosuppressant to optimize its efficacy and reduce toxicity. Thus, pharmacodynamic monitoring of MMF is a strategy that could potentially improve patient outcomes. Pharmacodynamic measurements require evaluation of inosine-5'-monophosphate dehydrogenase (IMPDH) activity, the target enzyme of the active moiety mycophenolic acid. Various nonradioactive methods using chromatographic separations have been used to quantify xanthosine monophosphate, the catalytic product of the enzyme, to indirectly evaluate IMPDH activity. However, no methods have used mass spectrometry based detection, which provides more specificity and sensitivity. Here, we describe a liquid chromatography-coupled tandem mass spectrometry (LC-MS/MS) method for the quantification of xanthosine monophosphate and adenosine monophosphate (for normalization) in lysates of peripheral blood mononuclear cells (PBMCs) from hematopoietic stem cell transplant (HSCT) recipients. Linearity, precision, and accuracy were validated over a large range of concentrations for each compound. The method could measure analytes with high sensitivity, accuracy (93-116%), and reproducibility (CV < 7.5%). Its clinical application was validated in PBMC lysates obtained from healthy individuals (n = 43) and HSCT recipients (n = 19). This reliable and validated LC-MS/MS method could be a useful tool for pharmacodynamic monitoring of patients treated with MMF.
Effects of Listening to Music on the Comfort of Chemotherapy Patients

Abstract:The symptoms of an illness that requires chemotherapy and the corresponding effects of such treatment exacerbate the pain and discomfort that patients typically experience. Listening to music may help patients cope with chemotherapy symptoms, thereby contributing to their physical ease and well-being. Seventy patients who were receiving treatment at the outpatient chemotherapy unit were invited to participate in this work. During chemotherapy sessions and the week after the sessions, the patients listened to music with headphones. The occurrence of chemotherapy symptoms such as pain, tiredness, nausea, depression, anxiety, drowsiness, lack of appetite, not feeling well, and shortness of breath in the intervention group was statistically significant after listening to music ( p < .05). Improvements in total general comfort, as well as physical, psychospiritual, and sociocultural comfort, were also statistically significant ( p < .05). These findings indicate that listening to music effectively reduces the severity of chemotherapy symptoms and enhances the comfort of patients receiving the treatment.

Abstract:ABSTRACT Background Percutaneous renal biopsy (PRB) may subject patients to emotional distress and pain before and during the biopsy. The aim of this study was to evaluate the effects of complementary/non-pharmacological interventions such as music therapy (MT) on anxiety, pain and satisfaction in renal patients undergoing PRB. Methods A prospective, single-centre, single-blind, randomized controlled two-arm trial was conducted. Patients ≥18 years of age, hospitalized at the Nephrology, Dialysis and Transplantation Unit (Bari, Italy) and scheduled for PRB were screened. Participants were assigned to standard treatment (CG) or to the music therapy (MT) intervention group. Participants in the MT group received standard care and an MT intervention by a certified music therapist qualified in guided imagery and music. The CG patients received the standard of care. MT and CG patients were subjected to identical measurements (pre/post) of the parameters in the State Trait Anxiety Inventory Y1 (STAI-Y1), visual analogue scale for pain (VAS-P) and satisfaction (VAS-S) and heart rate variability. Results A statistically significant difference in the anxiety scores after PRB between MT and CG patients (STAI-Y1 35.4 ± 6.2 versus 42.9 ± 9.0) was observed. MT also had strong and significant effects on VAS-P compared with CG (5.0 ± 1.4 versus 6.3 ± 1.3, respectively; P < .001) and VAS-S (7.8 ± 1.0 versus 6.0 ± 0.9, respectively; P < .001). Decreased activity of the sympathetic nervous system and increased activity of the parasympathetic nervous system was observed after PRB in the MT group. Conclusion Our study supports the use of MT to mitigate the psychological anxiety, pain and sympathetic activation associated with PRB.

Abstract:Effect of Music Therapy on Dental Anxiety in Periodontal Surgery Introduction: Pharmacological or non-pharmacological methods are applied to eliminate fear and anxiety problems related to dental treatments in many patients. As a non-invasive, non-pharmacological method with an anxiolytic effect, music therapy is preferred in the management of anxiety and fear. This study aimed to investigate the effect of music therapy on dental anxiety in patients scheduled to undergo periodontal surgery. Material and Method: The study included a total of 46 patients, 23 in the study group and 23 in the control group, who were scheduled to undergo periodontal surgery. During the surgical procedure, the patients in the study group listened to music using in-ear headphones that isolated ambient sounds. The patients in the control group wore in-ear headphones to isolate ambient sounds, but they did not listen to any music. All patients were asked to complete the Modified Dentistry Anxiety Scale (MDAS), the State-Trait Anxiety Inventory-State (STAI-S), and the Dental Fear Survey (DFS) before and after surgery. Systolic blood pressure (SBP) and diastolic blood pressure were measured as vital signs before and after surgery, and the Visual Analog Scale (VAS) scores were recorded twice (at the end of and 30 minutes after the surgical procedure). Results: In the study group, the post-test MDAS, STAI-S, and DFS measurement values showed statistically significant decreases compared to the pre-test measurement values (P<0.001, P=0.009, and P<0.001, respectively), but there was no statistically significant difference in the control group (P=0.093, P=0.923, and P=0.460, respectively). In both the study and control groups, the post-test VAS measurement values statistically significantly decreased compared to the pre-test measurement values (P=0.002 and P=0.019, respectively). While the post-test SBP values decreased statistically significantly in the study group compared to the pre-test measurement values (P=0.020), there was no statistically significant difference in the control group (P=0.705). No statistically significant difference was found between the pre-test and post-test measurement values of the DFS scores in the study or control group (P=0.083 and P=0.160, respectively). Clinical significance: This study is the first clinical study to evaluate the effect of music on anxiety in periodontal surgery simultaneously with four different scales. Music therapy was found to be effective in reducing dental anxiety and fear during periodontal surgery.

Abstract:PurposeThe purpose of this study was to compare the impact of music therapy (MT) versus music medicine (MM) interventions on psychological outcomes and pain in cancer patients and to enhance understanding of patients’ experiences of these two types of music interventions.MethodsThis study employed a mixed methods intervention design in which qualitative data were embedded within a randomized cross-over trial. Thirty-one adult cancer patients participated in two sessions that involved interactive music making with a music therapist (MT) and two sessions in which they listened to pre-recorded music without the presence of a therapist (MM). Before and after each session, participants reported on their mood, anxiety, relaxation, and pain by means of visual analogue and numeric rating scales. Thirty participants completed an exit interview.ResultsThe quantitative data suggest that both interventions were equally effective in enhancing target outcomes. However, 77.4 % of participants expressed a preference for MT sessions. The qualitative data indicate that music improves symptom management, embodies hope for survival, and helps connect to a pre-illness self, but may also access memories of loss and trauma. MT sessions helped participants tap into inner resources such as playfulness and creativity. Interactive music making also allowed for emotional expression. Some participants preferred the familiarity and predictability of listening to pre-recorded music.ConclusionsThe findings of this study advocate for the use of music in cancer care. Treatment benefits may depend on patient characteristics such as outlook on life and readiness to explore emotions related to the cancer experience.

Relief of Procedural Pain in Critically Ill Patients by Music Therapy: A Randomized Controlled Trial

Abstract:Background: This study aimed to determine the effectiveness of music listening for procedural pain relief using two different observational pain tools during endotracheal suctioning. Materials and Methods: This study was a randomized controlled trial. The sample of the study included 98 patients with mechanical ventilation support who met the selection criteria. The patients were randomly assigned to control and music therapy groups. Patients in the control group were routinely suctioned as usual. Patients in the music group received music therapy 20 min before, during, and 20 min after endotracheal suctioning. The primary outcome was the pain relief during suctioning. Results: Forty patients in each arm completed the study. Pain scores in the Critical Care Pain Observation Tool and Behavioral Pain Scale were lower in the music group than in the control group during endotracheal suctioning (group: F = 14.85, p = 0.000; F = 9.04, p = 0.000, respectively). It was also found to be a significant interaction effect between the groups and time (group × time: F = 17.35, p = 0.000; F = 18.00, p = 0.000, respectively). Conclusion: The Critical Care Pain Observation Tool and Behavioral Pain Scale in the current study generally demonstrated similar pain scores during the painful procedure. Our findings support that music therapy may act as a nonpharmacological therapy to relieve procedural pain in patients on mechanical ventilation.

Abstract:A colonoscopy is a procedure commonly used for the diagnosis of colorectal diseases but may be associated with high levels of anxiety and discomfort, which can lead to complications during sedation and decreased patient comfort and cooperation. This study was conducted at a tertiary care hospital in Karachi to assess the effectiveness of music therapy on pre-procedural anxiety, sedation requirements, pain, and procedure duration among patients undergoing colonoscopies. This comparative study included 110 outpatients. The intervention arm received non-lyrical soft music through earphones before and during the colonoscopy procedure, while the control group did not receive any music. In the intervention arm, significant reductions were seen in pre-procedural anxiety scores (p < 0.001), physiological measures (p < 0.001), sedation doses (p < 0.001), and procedure durations (p < 0.05). Control arm showed an increase in heart rate and systolic blood pressure (p < 0.001). Pain scores were similar for both groups. Music therapy is an effective tool to reduce patient anxiety pre- procedure. It is a simple, safe, and noninvasive relaxing intervention that could be used as an adjunct to sedative medications before and during the colonoscopy procedure.
The effect of preoperative music therapy on perioperative anxiety and cognitive functions in adult patients in Erzincan, Turkey: a cohort study

Abstract:Background & objective: Perioperative anxiety affects 25-80% of all surgical patients. It has a negative impact on postoperative recovery, potentially increasing morbidity and mortality. The anxiolytic effects of music therapy are well recognized, but its cognitive implications are unknown. We examined the effects of 440 Hz and 432 Hz music therapy on pre-operative anxiety and cognitive functions in patients scheduled for surgery. Methodology: The study took place in the operating room of our hospital. Following ethical committee approval, the study was launched. It was a prospective observational study comprising 66 ASA I, II, and III patients, between the ages of 18 and 65, scheduled to undergo surgery. They were divided into three groups: Control Group (CG), music therapy at 440 Hz (MG440), and music therapy at 432 Hz (MG432). Music therapy was administered to MG440 and MG432, but not to CG. Patients took the Montreal Cognitive Assessment (MoCA) and the Beck Anxiety Scale (BAS). MoCA and BAS were assessed before surgery, one hour and at 24 hours following surgery. Results: In both groups, preoperative music therapy significantly reduced anxiety (P < 0.001). Anxiety levels following surgery were comparable in all groups. Postoperative one-hour cognitive performance in music therapy groups was lower than in controls (P < 0.001), but increased and became significantly higher than in the control group at 24 h (P < 0.001). There was no substantial difference between the 440 Hz and 432 Hz groups. Conclusions: Preoperative music therapy reduced anxiety; it initially lowered postoperative cognitive performance; nevertheless, within 24 hours, therapy groups had enhanced cognitive skills when compared to controls. Abstract:BACKGROUND AND PURPOSE: Cancer patients often suffer from anxiety and depression. Various methods are used to alleviate anxiety and depression, but most of them have side effects. Music therapy can be used as a noninvasive method to reduce anxiety and depression. This study aimed to examine the effect of music therapy on anxiety and depression in patients with cancer.MATERIALS AND METHODS: This quasi-experimental study was conducted attaching hospitals in Urmia city. A total number of sixty patients with depression and anxiety were recruited using random sampling method and divided into two groups of control and intervention. Patients in intervention group listened to light music at least 20 min per day for 3 days. The degree of patients' anxiety and depression was assessed by Hospital Anxiety and Depression Scale at baseline and 3 days after music therapy. Data were analyzed by SPSS version 13 using t-test, Pearson, and ANOVA tests.RESULTS: The results showed no significant differences between demographic variable of intervention and control groups. Our findings indicated a significant decrease in the level of depression and anxiety among intervention group. There were significant relationships between anxiety, depression, and sex (<i>P</i> < 0.001, r = 0.42) as well as education (<i>P</i> = 0.003, r = 0.37).CONCLUSION: This study revealed positive effects of music therapy on decreasing level of depression and anxiety in patients with cancer. Therefore, it is recommended to include music therapy in the nursing care.

Abstract:Aim This study was conducted to determine the effects of music therapy on non‐stress test results and anxiety levels in high‐risk pregnant women. Methods This randomized controlled study was conducted in pregnant women who were hospitalized in the High‐Risk Pregnancy Service of Meram Medical Faculty Hospital, Konya, Turkey, between April 1 and July 31, 2021. Abstract:BACKGROUND: A cardiac catheterization laboratory can be a frightening environment and music can be a supportive source of environmental sound that stimulates and maintains relaxation. However, the results of studies are conflicting in this regard.OBJECTIVES: The aim of this study was to investigate the effect of music therapy on patients' anxiety and hemodynamic parameters during percutaneous transluminal coronary angioplasty.PATIENTS AND METHODS: This was a randomized controlled trial, conducted in the Catheterization Laboratory Unit of Baqiyatallah Hospital, in Tehran, Iran. A sample of 64 patients, who were planned to undergo coronary angioplasty, was recruited. Patients were randomly allocated to either the control or the experimental groups. In the experimental group, patients received a 20 to 40-minute music therapy intervention, consisting of light instrumental music albums by Johann Sebastian Bach and Mariko Makino. Patients in the control group received the routine care of the study setting, which consisted of no music therapy intervention. Study data were collected by a demographic questionnaire, the Spielberger's State Anxiety Inventory, and a data sheet for documenting hemodynamic parameters. Chi-square, independent-samples t tests, paired-samples t-test and repeated measures analysis of variance were used to analyze the data.RESULTS: Before the intervention, the study groups did not differ significantly in terms of anxiety level and hemodynamic parameters. Moreover, the differences between the two groups, regarding hemodynamic parameters, were not significant after the intervention (P > 0.05). However, the level of post-intervention anxiety in the experimental group was significantly lower than the control group (32.06 ± 8.57 and 38.97 ± 12.77, respectively; P = 0.014). Compared with the baseline readings, the level of anxiety in the control group did not change significantly after the study (41.91 ± 9.88 vs. 38.97 ± 12.77; P = 0.101); however, in the experimental group, the level of post-intervention anxiety was significantly lower than the pretest readings (32.06 ± 8.57 vs. 41.16 ± 10.6; P = 0.001).CONCLUSIONS: Music therapy is a safe, simple, inexpensive, and non-invasive nursing intervention, which can significantly alleviate patients' anxiety during coronary angioplasty.

Abstract:Pollen tubes transport two immotile sperm cells in their cytoplasm during growth through the pistil, delivering one to the egg cell to form the zygote and one to the central cell to create the endosperm. Pollen tubes grow through the pistil’s complex extracellular environment and target ovules by following a gradient of small protein attractants, which are secreted by two specialized synergid cells within the ovule. This image shows Arabidopsis thaliana pollen tubes growing in vitro, and illustrates how pollen tubes gain the capacity to locate and enter ovules only after interaction with the pistil tissue. When pollen grains expressing red fluorescent protein (bottom) are placed directly on media in vitro, they exhibit random growth and are unable to target ovules (shown in white; middle). In contrast, when pollen expressing green fluorescent protein (top) is grown through an explant of stigma and style, pollen tubes grow more rapidly and efficiently target and enter ovules. Pollen transcriptional programs are rapidly induced during growth through the pistil, which prepares the pollen tube for interaction with the synergid cells. The molecular components that allow perception of the chemoattractants, which are similarly transcriptionally regulated, are beginning to be identified. Pollen growth in the pistil initiates pollen tube differentiation, maximizing the capacity of pollen to deliver sperm.
