Abstract:This is a retrospective study to evaluate our early experience of using selective microcoil embolization in patients who had gastrointestinal (GI) haemorrhage. From December 2002 to December 2003, six patients with GI haemorrhage (upper GI, n = 1; lower GI, n = 5) underwent superselective microcoil embolization. Microcatheters were used to carry out embolizations in branches of the superior mesenteric artery. Microcoils were used in five patients and a combination of microcoils and embolospheres was used in one patient. Technical success (bleeding target devascularization) was achieved in all patients who showed active bleeding at the time of angiography. Two patients had recurrent bleeding within 24 h of embolization, of which one (16.7%) died. The other patient did not require active intervention as bleeding was minimal and resolved with conservative management. Satisfactory clinical success (no rebleeding after 30 days) was achieved in five patients. No clinical signs and symptoms of bowel ischaemia occurred in these patients. Follow-up colonoscopy carried out in two patients did not show any signs of ischaemia in the affected bowel segments. Superselective microcoil embolization is an effective and safe method of controlling and arresting bleeding in GI haemorrhage.
Imaging and Biomarkers: The Assesment of Pulmonary Embolism Risk and Early Mortality

Abstract:Background and Objectives: Pulmonary embolism (PE) incidence has been increasing in the last 10 years. Computed thoracic pulmonary angiography (CTPA) had a major role in PE diagnosis and prognosis. The main purpose of this study was as follows: the prognostic value of a CTPA parameter, pulmonary artery obstruction index (PAOI), in PE risk assessment and the predictive accuracy of biomarkers, D-dimer and cardiac Troponin T (c-TnT), in 7-day mortality. A second objective of the research was to investigate the relationship between imaging by PAOI and these biomarkers in different etiologies of PE. Materials and Methods: This study comprised 109 patients with PE, hospitalized and treated between February 2021 and August 2022. They had different etiologies of PE: deep vein thrombosis (DVT); persistent atrial fibrillation (AF); chronic obstructive pulmonary disease (COPD) exacerbation; COVID-19; and cancers. The investigations were as follows: clinical examination; D-dimer testing, as a mandatory method for PE suspicion (values ≥500 µg/L were highly suggestive for PE); c-TnT, as a marker of myocardial injury (values ≥14 ng/L were abnormal); CTPA, with right ventricle dysfunction (RVD) signs and PAOI. Treatments were according to PE risk: systemic thrombolysis in high-risk PE; low weight molecular heparins (LWMH) in high-risk PE, after systemic thrombolysis or from the beginning, when systemic thrombolysis was contraindicated; and direct oral anticoagulants (DOAC) in low- and intermediate-risk PE. Results: PAOI had a high predictive accuracy for high-risk PE (area under curve, AUC = 0.993). D-dimer and cTnT had a statistically significant relationship with 7-day mortality for the entire sample, p < 0.001, and for AF, p = 0.0036; COVID-19, p = 0.003; and cancer patients, p = 0.005. PAOI had statistical significance for 7-day mortality only in COVID-19, p = 0.045, and cancer patients, p = 0.038. The relationship PAOI-D-dimer and PAOI-c-TnT had very strong statistical correlation for the entire sample and for DVT, AF, COPD, and COVID-19 subgroups (Rho = 0.815-0.982). Conclusions: PAOI was an important tool for PE risk assessment. D-dimer and c-TnT were valuable predictors for 7-day mortality in PE. PAOI (imaging parameter for PE extent) and D-dimer (biomarker for PE severity) as well as PAOI and c-TnT (biomarker for myocardial injury) were strongly correlated for the entire PE sample and for DVT, AF, COPD, and COVID-19 patients.
Low Risk Meets High Stakes: Unraveling the Mystery of Low D-dimer Pulmonary Embolism

Abstract:Pulmonary embolisms (PEs) are potentially life-threatening emergencies that carry significant morbidity and mortality. Advances in treatment options and the safety of existing procedures have effectively reduced the long-term and short-term effects of the condition. Therefore, it is important to make an early diagnosis so that treatment options can be thoroughly explored. The D-dimer is an important tool in the early diagnosis of PEs. It is especially useful in ruling out the diagnosis in patients with a low to moderate suspicion of the disease. We present a case of a 22-year-old male who presented with exertional dyspnea, congestion, and rhinorrhea for one day and was noted to have persistent hypoxia and tachycardia. The influenza test was positive, and he was started on oseltamivir. Due to persistent hypoxia, a CT pulmonary angiogram was ordered and revealed filling defects in the left lower lobe segmental vessels suggestive of PE, as well as multifocal multilobar bilateral ground-glass opacities. He was initially treated with a heparin drip and subsequently switched to eliquis. After a significant improvement in his hypoxia, he was discharged home for outpatient follow-up, including a hypercoagulable workup. This case demonstrates that despite the usefulness of the D-dimer as a diagnostic tool for PEs, it cannot solely or fully replace the full gamut of screening tools used to determine the risk of PE. Although rare, false-negative scores do occur; therefore, the tool should always be used in conjunction with other scoring systems, physician gestalt, and within the specific clinical context.
Interaction of fl avokawain B with lysozyme : A photophysical and molecular simulation study

Abstract:Interaction of flavokawain B (FB), a therapeutic flavonoid with lysozyme (LYZ), was studied using various spectroscopic and molecular simulation techniques. The association constant, Ka of the binding reaction was determined to be 2.7970.16 10 M 1 at 25 1C based on fluorescence quenching titration results. Thermodynamic analysis of the binding data obtained at different temperatures along with molecular docking results suggested the involvement of hydrophobic and van der Waals forces, as well as hydrogen bonding in FB–LYZ interaction. The binding reaction between FB and LYZ was found to affect the microenvironment around protein fluorophores (Tyr and Trp) as revealed by intrinsic and threedimensional fluorescence results. A comparison of the LYZ thermograms, obtained by far-UV CD spectroscopy in the absence and the presence of FB, suggested improved protein thermal stability upon complexation with FB. Presence of metal ions was found to affect FB–LYZ interaction. Molecular docking predicted the formation of two hydrogen bonds between the oxygen atoms of FB and amino acid residues of LYZ (Asn-59 and Trp-63), located in the vicinity of the active site, in addition to various nonpolar contacts. Molecular dynamics studies showed that the complex reached equilibrium during simulation, indicating the stability of the FB–LYZ complex. & 2014 Elsevier B.V. All rights reserved.

Abstract:Perfusion index (PI) is a promising indicator for monitoring peripheral perfusion. The present study aimed to compare the efficiency of PI and PESI score in estimating the 30-day mortality and treatment needs of patients diagnosed with pulmonary embolism in the emergency department. This study was prospective and observational. The demographic features of the patients, comorbidities, vital signs, PESI score, PI, treatment applied to the patient and airway management, right ventricular diameter/left ventricular diameter ratio, length of hospital stay, outcome, and 30-day mortality were recorded. A total of 94 patients were included. All patients' vital signs and PI values were recorded on admission. The mean pulse rate ( p = 0.001) and shock index ( p = 0.017) values of deceased patients were statistically significantly higher, while the mean PI ( p = 0.034) was statistically significantly lower. PESI score and PI were statistically significant to predict the need for mechanical ventilation (PI, p = 0.004; PESI score, p < 0.001), inotropic treatment (PI, p = 0.047; PESI score p = 0.005), and thrombolytic therapy (PI, p = 0.035; PESI score p = 0.003). According to the ROC curve, the mortality prediction power of both PESI (AUC: 0.787, 95% CI 0.688-0.886, cutoff: 109.5, p < 0.001) and PI index (AUC: 0.668, 95% CI 0.543-0.793, cutoff: 1, p = 0.011) were determined as statistically significant. PI might be helpful in clinical practice as a tool that can be applied to predict mortality and treatment needs in PE.

Abstract:Introduction Pulmonary embolism (PE) remains a critical condition requiring timely diagnosis and treatment. The use of D-dimer, a fibrin degradation product, as a biomarker, combined with computed tomography pulmonary angiography (CTPA), is a common practice in diagnosing PE. Aim This study aims to increase diagnostic accuracy for PE by relating the D-dimer levels to the findings on CTPA. Specifically, it aims to calculate the sensitivity and specificity of D-dimer levels against CTPA results and also establish the association of D-dimer levels with the location of the PE. Methods This retrospective analysis was conducted at a tertiary care hospital, including patients who underwent CTPA and had D-dimer levels recorded over a one-year period. The total sample size was 124. D-dimer levels were categorized into four groups based on CTPA findings: Category 0 (no PE), Category I (peripheral PE), Category II (PE in lobar arteries), and Category III (central embolisms in the pulmonary trunk or arteries). Statistical analyses were performed to evaluate the correlation between D-dimer levels and CTPA findings, including sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV). Results The study found that Category 0 (no PE) had an average D-dimer of 3.6 mg/L, Category I (peripheral PE) had 4.3 mg/L, Category II (PE in lobar arteries) had 3.6 mg/L, and Category III (central embolisms) had 7.1 mg/L. The sensitivity of D-dimer in predicting PE was 1.0, and the specificity was 0.2. The PPV was 0.3208, and the NPV was 1.0. These findings indicate a significant correlation between elevated D-dimer levels and the presence of PE. Conclusion Integrating D-dimer levels with CTPA findings can improve diagnostic accuracy and efficiency for PE. Establishing reliable D-dimer cutoff values may help clinicians better stratify patient risk and make informed decisions about the need for imaging, thereby optimizing resource utilization and minimizing unnecessary CTPA scans. This study highlights the potential benefits of combining biomarker analysis with imaging results in the clinical management of PE.
Eosinophil-to-Monocyte Ratio as a Candidate for a Novel Prognostic Marker in Acute Pulmonary Embolism: Is it a Consumptive Mechanism?

Abstract:Background: The role of eosinophils in thrombotic processes is well known, and the prognostic value of eosinophil to monocyte ratio had been determined in patients with ST elevated myocardial infarction and acute ischemic stroke in recent studies. We aimed to evaluate the impact of the eosinophil-to-monocyte ratio on short- and long-term all-cause mortality in patients with pulmonary embolism, which is another clinical condition closely related to the thrombotic pathway. Methods: In this study, a total of 212 retrospectively evaluated patients with intermediate-high risk and high-risk pulmonary embolism who underwent catheter-directed therapies with ultrasound-assisted thrombolysis or rheolytic thrombectomy (Angiojet©) and intravenous thrombolytic treatment were included. Results: The median Pulmonary Embolism Severity Index score was 105 (86-128; interquartile range: 25-75, min-max: 35-250). The intermediate-high status and high-risk status were noted in 83.5% and 16.5% of the patients, respectively. All of the reperfusion strategies resulted in significant improvements in the measures of pulmonary arterial pressure and right ventricular strain. Death was recorded in 42 (18.6%) patients during the follow-up period (median 1029 days, interquartile range: 651-1358). Multiple Cox regression analysis revealed that a higher pulmonary embolism severity index score (from 85 to 128; hazard ratio = 3.00; 95% CI: 2.11-4.29; P < .001) and a lower eosinophil-to-monocyte ratio (from 0.02 to 0.24; hazard ratio = 0.56; 95% CI: 0.34-0.98; P = .032) were 2 independent predictors for long-term all-cause mortality. The eosinophil-to-monocyte ratio at the admission of less than 0.03 was documented to be associated with higher mortality (P < .001). Conclusion: Our results revealed that a lower eosinophil-to-monocyte ratio and a higher pulmonary embolism severity index score independently predict the long-term mortality in patients with intermediate-high- and high-risk pulmonary embolism.

Abstract:COVID-19 is a respiratory disease with severe cardiovascular complications, one of them is pulmonary embolism (PE). Studies have shown many contributing factors related to mortality of hospitalized COVID-19 patients that developed pulmonary embolism. This study aimed to find the association between coagulation parameters (PT and APTT), C-reactive protein (CRP), and D-dimer with in-hospital mortality of COVID-19 patients with suspected PE. COVID-19 patients in Ulin General Hospital Banjarmasin who met the clinical criteria of suspected PE, assessed with clinical scorings (Well’s score for PE and Padua prediction score for venous thromboembolism), and receiving anticoagulant therapy recorded in medical records were included in this study. A total of 91 patients were included, with most patients were female (59.3%), average age of 53.36 years, average BMI of 25.82 kg/m2, having normal electrocardiogram (79.1%), with history of hypertension (56%), and no histories of smoking (94.5%), diabetes mellitus (70.3%), heart failure (93.4%), coronary artery disease (96.7%), and chronic lung disease (95.6%). As much as 39.57% patients with suspected PE was deceased during the course of hospitalization. Unadjusted analysis showed significant increased levels of PT, APTT, and CRP, in deceased patients compared to those discharged alive (mean 14.62 vs 11.91 seconds, 42.94 vs 28.57 seconds, and 98.19 vs 47.01 mg/dL, respectively; p value < 0.01), and a non-significant increased level of D-dimer (mean 10.89 mg/L; p = 0.09). Multivariate logistic regression showed non-significant associations between PT, APTT, D-dimer, and CRP with in-hospital mortality (p values of 0.06, 0.14, 0.52, and 0.23, respectively). In conclusion, there is no association between coagulation parameters, CRP, and D-dimer levels with in-hospital mortality in COVID-19 patients with suspected PE. A further study with a larger sample size is needed to see the role of laboratory findings in predicting in-hospital mortality in COVID-19 patients with suspected PE.

Abstract:OBJECTIVE:To investigate the role of plasma D-dimer in differentiating fatal chest pain due either to acute myocardial infarction (AMI), acute pulmonary embolism (APE) or acute aortic dissection (AAD) in emergency department. METHODS:The data of a total of 445 consecutive patients complaining chest (back) pain and/or dyspnea who visited emergency department of Peking University Third Hospital from January 2011 to January 2012 were retrospectively analyzed. All cases were either diagnosed as AMI, APE or AAD finally. D-dimer concentrations were assessed and compared among different groups. The receiver operating characteristic curve (ROC curve) was established. The potency of D-dimer in distinguishing AMI, non-ST-segment elevation myocardial infarction (NSTEMI) by difference in values was assessed. RESULTS:Finally, 438 cases were enrolled, including 327 AMI [253 ST-segment elevation myocardial infarction (STEMI) and 74 NSTEMI], 76 APE and 35 AAD cases. The D-dimer concentrations in AMI group [0.21(0.15, 0.33) mg/L] were significantly lower than those in APE group [1.06 (0.86, 3.01) mg/L, Z=-11.416, P<0.001], AAD group [1.79 (0.83, 3.37) mg/L, Z=-8.715, P<0.001], APE/AAD group [1.15 (0.85, 3.13) mg/L, Z=-13.509, P<0.001]. The D-dimer concentrations in STEMI group were significantly lower than those in NSTEMI group [mg/L: 0.20 (0.15, 0.30) vs. 0.24 (0.17, 0.54), Z=-3.248, P=0.001]. The area under ROC curve (AUC) to discriminate AMI from APE/AAD was 0.929±0.015, and optimal value was 0.535 mg/L. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (PLR), negative likelihood ratio (NLR) with cut-off value of 0.535 mg/L were 86.2%, 93.7%, 97.6%, 69.8%,13.68, 0.15, while sensitivity, specificity, PPV, NPV, PLR, NLR with cut-off value of 0.5 mg/L were 84.4%, 93.7%, 97.5%, 67.1%, 13.40, 0.17. The AUC to discriminate NSTEMI from APE/AAD was 0.881±0.028. Sensitivity, specificity, PPV, NPV with cut-off value of 0.535 mg/L were 75.7%, 93.7%, 88.9%, 85.2%, while sensitivity, specificity, PPV, NPV with cut-off value of 0.5 mg/L were 70.3%, 93.7%, 88.1%, 82.5%, respectively. CONCLUSIONS:D-dimer is a better index in differentiating AMI from APE/AAD in emergency setting, guiding further examination and therapy, and increasing diagnosis efficiency.
Prognostic Impact of the Tricuspid Annular Plane Systolic Excursion/Pulmonary Arterial Systolic Pressure Ratio in Acute Pulmonary Embolism

Abstract:Background: Currently available risk stratification models for acute pulmonary embolism (PE) include hemodynamic status, cardiac biomarkers, right ventricle (RV) dysfunction on imaging, and clinical scores. Focusing on the length-tension relationship of the ventricle might have a superior predictive capability over RV dysfunction in terms of mortality and classification of patients with acute PE. In this study, our hypothesis suggests that the tricuspid annular plane systolic excursion (TAPSE)/systolic pulmonary artery pressure (sPAP) ratio has superior predictive capability for in-hospital mortality in patients with acute PE compared to TAPSE or sPAP as distinct measures. Methods: This single-center study comprised retrospectively evaluated 703 patients referred to our tertiary cardiovascular center with acute PE. We divided patients into quartiles based on the TAPSE/sPAP ratio. Different models were developed to quantify the predictive relationship between in-hospital death and echocardiographic measurements. A base model was created with variables including risk status and RV/LV ratio >1. Then, to evaluate the predictive contribution of each measurement; TAPSE/sPAP, TAPSE, and sPAP were sequentially added to the base model. After that, the performance of each model was evaluated. Results: Predictive and discriminative power was the highest in model containing TAPSE/sPAP. There was still a significant inverse association between TAPSE/sPAP and the risk of in-hospital death even after adjusting for risk status and RV/LV ratio >1. Receiver operating characteristic curve analysis for TAPSE/sPAP revealed the best cut-off value as 0.34. Conclusion: The outcomes of our study reveal that the ratio of TAPSE/sPAP serves as a more potent predictor of mortality than either of the 2 measurements taken separately. The interpretation and utilization of the TAPSE/sPAP cut-off value in acute PE can assist in identifying patients at risk of deterioration and guide the consideration of more intensive treatment options across all risk groups.
The predictive values of D-dimer for the early prognosis of the acute myocardial infarction (AMI): a review of 3134 AMI patients

Abstract:Objective To investigate whether D-dimer could be an early warning signal for patients with acute myocardial infarction (AMI),then may be helpful in risk accessment in emergency department.Methods Three thousand one hundred and thirty-four emergency AMI cases admitted to the hospital from January 1,2009 to December 31,2010 were retrospectively reviewed,and their age,gender,past medical history,D-dimer,MB isoenzyme of creatine kinase (CK-MB),cardiac troponin Ⅰ (cTnⅠ),the precursor of the N-terminal pro-brain natriuretic peptide (NT-proBNP),electrocardiogram (ECG) before treatment,and the CK-MB,cTnⅠ and echocardiography left ventricular ejection fraction (LVEF) 2 hours after the treatment of percutaneous coronary intervention (PCI) or thrombolytic drugs,whether died and time from onset to death were recorded.According to the results of multivariate logistic regression analysis,draw the receiver operating characteristic curve (ROC curve) and fitting curve,and analyzed the relativity between the D-dimer before the treatment and prognosis of myocardial infarction.Results Multivariate logistic regression analysis showed that age [odds ratio (OR) =1.109,95％ confidence interval (95％ CI)1.073 to 1.147,P=0.000],whether undergoing emergency PCI (OR=4.162,95％CI 1.980 to 8.748,P=0.000),D-dimer before treatment (OR=1.001,95％CI 1.000 to 1.002,P=0.001),the LVEF values (OR=0.946,95％CI 0.928 to 0.964,P=0.000) and cTnⅠ after 2 hours of treatment (OR=1.011,95％CI 1.004 to 1.018,P=0.002) were correlated to 28-day death.The ROC curves showed that age [area under curve (AUC) 0.796,P=0.000],whether undergoing emergency PCI (AUC 0.704,P=0.000),D-dimer before treatment (AUC 0.797,P=0.000) were positively correlated with the 28-day death; the LVEF values (AUC 0.261,P=0.000) were negative correlated with the 28-day death.The fitting curve of D-dimer before treatment,the time from onset to death and the LVEF showed no linear correlation.Conclusions D-dimer was correlated with the early prognosis of patients with myocardial infarction,but was not correlated with the time from onset to death.The rise in D-dimer at the early stage of AMI may be helpful to indicate the critical condition of the AMI patients.
Comparison of laboratory biomarkers for the prediction of in-hospital mortality and severity of acute pulmonary embolism: A multi-center study

Abstract:Objectives: To assess the specificity and sensitivity of prognostic biomarkers in individuals diagnosed with acute pulmonary embolism (PE). Methods: This study retrospectively enrolled 162 patients from the 741 patients who were hospitalized with acute PE and diagnosed using pulmonary computed tomography (CT) angiogram at 5 hospitals in Saudi Arabia between January 2015 and December 2019. Pulmonary embolism patients classified into survivor and non-survivor groups. Neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and red cell distribution width (RDW) were all recorded and were compared between the groups. The evaluation of mortality prediction, sensitivity, and specificity was carried out by employing receiver operating characteristic curves. Results: The variables NLR and RDW exhibited a statistically significant correlation with increased mortality and disease severity. A total of 8 patients among the 162 patients died. At the cut-off value of 5.5, NLR was showed an association with all-cause mortality, demonstrating a sensitivity of 75% and a specificity of 82%. At the cut-off value of 18.15, RDW was found to be significantly associated with all-cause mortality, displaying a sensitivity of 63% and a specificity of 88%. Conclusion: Multiple parameters have been implicated in the mortality and severity of PE. Our study revealed a statistically significant association between NLR, RDW, and PE mortality. These tests are easily accessible and may provide insights into the mortality associated with PE.

Abstract:Abstract Introduction Pulmonary embolism (PE) is a relatively common cardiovascular emergency. It can lead to a variety of outcomes, from potentially reversible right ventricular failure (by occluding the pulmonary arterial bed) to eventual death. The diagnosis of PE is often missed due to its nonspecific clinical presentation. Materials and Methods This is a descriptive cross-sectional study conducted at tertiary care center in India from October 1, 2016 to September 30, 2018, which includes 36 cases of PE. Institutional ethics committee clearance was obtained before start of the study. Written and informed consent was obtained from all patients. Patients were informed regarding purpose, procedures, risks, and benefits of the study in their own vernacular language. Results The age-wise distribution of patients in this study showed that most patients were up to 40 years amounting to 58.4% of cases. Sex-wise distribution also showed that most patients were male (66.7%). Fifty percent of the patients had no thrombosis on Color Doppler findings of the involved limb, and 31.25% had superficial venous thrombosis. Blood investigation revealed that 94.4% patients had troponin negative and 94.4% had D-dimer positive. This study also attempted to correlate the D-dimer findings and Well's score. It can be seen that as per Well's score, 13 patients were with high probability, 18 with moderate probability, and 5 with low probability. Conclusion This study states that D-dimer assays are highly sensitive and have a low rate of false-negative results. The assays have been found safe and effective when the clinical probability of PE is low. When the D-dimer result is negative in patients with low clinical probability of having PE, the presence of acute PE can be safely ruled out without diagnostic imaging.
Diagnostic value of D-dimer to fibrinogen ratio for pulmonary embolism in postpartum women

Abstract:Background: Pulmonary embolism is a common disease associated with high mortality and morbidity. Diagnosing pulmonary embolism is challenging due to diverse clinical presentations and the lack of specific biomarkers. The study aimed to investigate the diagnostic value on pulmonary embolism for postpartum women by D-dimer to fibrinogen ratio, and it combined with neutrophil-to-lymphocyte ratio or platelet-to-lymphocyte ratio. Methods: A total of 537 women with suspected pulmonary embolism were selected as the research subjects from the Shanghai First Maternity and Infant Hospital between 1 January 2019 and 31 October 2022. The D-dimer to fibrinogen ratio and it combined with neutrophil-to-lymphocyte ratio or platelet-to-lymphocyte ratio were applied to evaluate the clinical probability of pulmonary embolism, and the positive predictive value of both scores were calculated using computed tomography pulmonary arteriography as a gold standard. The diagnostic value of D-dimer to fibrinogen ratio, combined with neutrophil-to-lymphocyte ratio or platelet-to-lymphocyte ratio was evaluated by the area under the curve, sensitivity, specificity, and other indicators in the receiver operator characteristic curve. Results: Among the 502 women included for analysis, 194 (38.65%) were definitely diagnosed as pulmonary embolism. The positive predictive values of D-dimer to fibrinogen ratio and it combined with platelet-to-lymphocyte ratio or neutrophil-to-lymphocyte ratio were 70.1%, 50.5%, and 56.5%, respectively in the postpartum women, the area under the curve for the D-dimer to fibrinogen ratio and it combined with platelet-to-lymphocyte ratio or neutrophil-to-lymphocyte ratio were 0.606 (95%CI: 0.562-0.650), 0.624 (95%CI: 0.575-0.673), and 0.639 (95%CI: 0.592-0.686), respectively. The negative predictive values of D-dimer to fibrinogen ratio, it combined with platelet-to-lymphocyte ratio or neutrophil-to-lymphocyte ratio were 50.5%, 70.1%, and 69.8%, respectively. Conclusion: The diagnostic value of the D-dimer to fibrinogen ratio was higher than the D-dimer for the postpartum women with suspected pulmonary embolism. The combination of either the neutrophil-to-lymphocyte ratio or the platelet-to-lymphocyte ratio with D-dimer to fibrinogen ratio is an appropriate strategy to rule out pulmonary embolism.
Performance of the Simplified Pulmonary Embolism Severity Index in Predicting 30-Day Mortality after Acute Pulmonary Embolism: Validation from a Large-Scale Cohort.

Abstract:BACKGROUND:The performance of existing prognostic scores including the simplified Pulmonary Embolism Severity Index (sPESI) for short-term mortality of non-high-risk PE in Chinese population has not been widely validated.METHODS:Non-high-risk patients were included from the prospective cohort of the China pUlmonary Thromboembolism REgistry Study (CURES). The sPESI, RIETE, Geneva, modified FAST, and Bova score were validated. The discriminatory performance was measured by the area under the curve (AUC). We also compared the sensitivity, odds ratio, specificity, positive predictive value and negative predictive value of these scores.RESULTS:A total of 6,873 non-high-risk patients with acute PE were included and 241 (3.5 %) patients died within 30 days. Compared to the Geneva, modified FAST, and Bova score, the AUCs for predicting 30-day death of sPESI and RIETE score were higher at 0.712 (95 % CI, 0.680, 0.743) and 0.723 (95 % CI, 0.691, 0.755) respectively. The sPESI demonstrated the highest sensitivity at 0.809, while the RIETE score, Geneva, Modified FAST and BOVA score showed sensitivities of 0.622, 0.568, 0.477 and 0.502 respectively. A sPESI ⩾1 point was associated with a 4.7-fold increased risk of 30-day all-cause mortality (95 % CI, 3.427, 6.563, p < 0.001), while a RIETE score of ⩾1 point was associated with a 4.5-fold increased risk (95 % CI, 3.127, 6.341, p < 0.001). The Geneva score, modified FAST and Bova score showed inferior performance.CONCLUSIONS:The implementation of the fewer-parameter, easier-to-calculate sPESI in Chinese patients with PE can help to discriminate patients with extremely low risk of short-term mortality for home treatment or early discharge.

Abstract:Background: Pan-immuno-inflammation value (PIV) is a new and comprehensive index that reflects both the immune response and systemic inflammation in the body.Objective: The aim of this study was to investigate the prognostic relevance of PIV in predicting in-hospital mortality in acute pulmonary embolism (PE) patients and to compare it with the well-known risk scoring system, PE severity index (PESI), which is commonly used for a short-term mortality prediction in such patients.Methods: In total, 373 acute PE patients diagnosed with contrast-enhanced computed tomography were included in the study. Detailed cardiac evaluation of each patient was performed and PESI and PIV were calculated.Results: In total, 60 patients died during their hospital stay. The multivariable logistic regression analysis revealed that baseline heart rate, N-terminal pro-B-type natriuretic peptide, lactate dehydrogenase, PIV, and PESI were independent risk factors for in-hospital mortality in acute PE patients. When comparing with PESI, PIV was non-inferior in terms of predicting the survival status in patients with acute PE.Conclusion: In our study, we found that the PIV was statistically significant in predicting in-hospital mortality in acute PE patients and was non-inferior to the PESI.

Clinical value of combined CT pulmonary angiography and D-dimer in patients with acute pulmonary embolism in emergency department

Abstract:Objective To analyze the clinical value of combined CT pulmonary angiography (CTPA) and D-dimer in detecting pulmonary embolism patients with acute chest pain.Methods There were 155 patients (83 males and 72 females,age from 13-84 years,mean age of 55±16 years) with chest pain who underwent CTPA during Jan.1st,2015 to June 30,2017in our Department of Emergency were retrospectively studied.The presence and incidence of pulmonary embolism,clinically implicated concurrent lesions in patients without pulmonary embolism,and D-dimer value were recorded and analysed.Incidences of pulmonary embolism in positive and negative D-dimer groups were compared with Fisher's exact test.D-dimer values between pulmonary embolism patients and non-pulmonary embolism patients in abnormal D-dimer patients were compared with independent sample t test.With CTPA findings as reference standard,the diagnostic performance of D-dimer was calculated for diagnosing pulmonary embolism.Results Of 155 patients,50 patients were diagnosed as having pulmonary embolism (32.3％),while 105 patients (67.7％) had no pulmonary embolism.There were 152 patients had D-dimer measurement.With 0.5 mg/dL as cut-off value,128 patients had elevated D-dimer and 24 patients had normal D-dimer value.No pulmonary embolism was detected in CTPA in all 24 patients with normal D-dimer value (0/24),while 50 patients with pulmonary embolism (50/128,39.1％) and 78 patients without pulmonary embolism in CTPA had elevated D-dimer value.Statistical difference was found between the incidences of pulmonary embohsm in the positive and negative D-dimer groups (Fisher's exact test,P＜0.001).In abnormal D-dimer group,50 patients had pulmonary embolism in CTPA while 78patients had no pulmonary embolism in CTPA,no significant difference was found for their mean D-dimer values (6.11±5.59mg/dL vs 5.26±6.67 mg/dL;t=0.741,P=0.460).With CTPA findings as reference standard,the sensitivity,specificity,negative predictive value,and positive predictive value of D-dimer for diagnosing pulmonary embohsm were 100％,23.5％,39.1％,and 100％,respectively.Conclusions Combining the D-dimer measurements can improve the positive findings of pulmonary embolism in CTPA.
Characteristics of emergency patients with markedly elevated D-dimer levels

Abstract:Background: Markedly elevated D-dimer levels can occur in emergency patients with various clinical situations, and is likely to indicate the presence of coagulopathy, rapid differential diagnosis was crucial for them. Methods: D-dimer was detected in consecutive 813 patients entering the emergency department of our hospital, for the patients with D-dimer levels above 5.0 µg/mL, the final diagnoses and 28-day mortality were confirmed, and the levels of thrombomodulin (TM), thrombin-antithrombin complex (TAT) and plasmin-antiplasmin complex (PAP) on admission were detected. Results: There were 148 emergency patients with D-dimer levels higher than 5.0 µg/mL mainly due to sepsis, malignancy, trauma, venous thromboembolism (VTE), cerebrovascular accident, and so on. Both of the TM and TAT levels among these diagnoses were significantly different (p < 0.001). The elevated TM (>13.3 TU/mL) had a predictive value of 96.0% for excluding VTE, and the normal TM had a predictive value of 90.4% for excluding sepsis. The overall 28-day mortality of these patients with D-dimer >5.0 ug/mL was 14.2%, the TAT level on admission was independently associated with 28-day mortality (odds ratio 1.014, 95% CI 1.001-1.027, P = 0.030). Conclusions: The medical emergencies associated with markedly elevated D-dimer levels were revealed, specific markers of endothelial dysfunction and thrombin generation measured by automatic analyzer have the potential to distinguish diagnoses and predict outcomes in these patients.
Optimizing Use of High-Sensitivity Troponin for Risk-Stratification of Acute Pulmonary Embolism

Abstract:Background High-sensitivity troponin T (HS-TnT) may improve risk-stratification in hemodynamically stable acute pulmonary embolism (PE), but an optimal strategy for combining this biomarker with clinical risk-stratification tools has not been determined. Study Hypothesis We hypothesized that different HS-TnT cutoff values may be optimal for identifying (1) low-risk patients who may be eligible for outpatient management and (2) patients at increased risk of clinical deterioration who might benefit from advanced PE therapies. Methods Retrospective analysis of hemodynamically stable patients in the University of Michigan acute ED-PE registry with available HS-TnT values. Primary and secondary outcomes were 30-day mortality and need for intensive care unit-level care. Receiver operating characteristic curves were used to determine optimal HS-TnT cutoffs in the entire cohort, and for those at higher risk based on the simplified Pulmonary Embolism Severity Index (PESI) or imaging findings. Results The optimal HS-TnT cutoff in the full cohort, 12 pg/mL, was significantly associated with 30-day mortality (odds ratio [OR]: 3.94, 95% confidence interval [CI]: 1.48–10.50) and remained a significant predictor after adjusting for the simplified PESI (sPESI) score and serum creatinine (adjusted OR: 3.05, 95% CI: 1.11–8.38). A HS-TnT cutoff of 87 pg/mL was associated with 30-day mortality (OR: 5.01, 95% CI: 2.08–12.06) in patients with sPESI ≥1 or right ventricular dysfunction. Conclusion In this retrospective, single-center study of acute PE patients, we identified distinct optimal HS-TnT values for different clinical uses—a lower cutoff, which identified low-risk patients even in the absence of other risk-stratification methods, and a higher cutoff, which was strongly associated with adverse outcomes in patients at increased risk.
