Abstract:BACKGROUND: Histomorphological change of endplate may affect the nntritional transmission of intervertebral disk, eventually leading to intervertebral disc degeneration. OBJECTIVE: To observe the effects of endplate concavity variation on mechanical response of lumbar motion segment. DESIGN, TIME AND SETTING: Repeated measurement design, biomechanical analysis of finite element models, performed at the Laboratory of Biomechanics, Department of Orthopedics, the Second Affiliated Hospital of Medical College of Zhejiang University between January 2005 and January 2007. MATERIALS: SOMATOM SENSATION 16 spiral CT machine (Siemens, German) and ANSYS (Inc. Pennsylvania, USA).METHODS: A three-dimensional nonlinear geometrical and mechanical accurate finite element model of lumbar L<,4-5> segment was developed. Parametric studies were undertaken by studying endplate of three different concave angles, whereas disc angle, the gap of facet joint, finite element mesh density, and all other parameters were kept constant. Biomechanical parameters of three kinds of finite element models were tested under 5 different loading conditions, including vertical compression, flexion, extension, and anterior and posterior shearing loading. MAIN OUTCOME MEASURES: Endplate-intervertebral disc interface strain, intervertebral disc stiffness, nucleus pulposus pressure, annular fiber stress, radial disc bulge, stress in the annulus ground substance, stresses in posterior structure and facet contact force. RESULTS: The decrease in the endplate concavity stimulated by an increasing endplate concave angle would result in decreased strains of endplate-intervertebral disc interface, increased disc stiffness and nucleus pulposus pressure, decreased annular fiber stress, radial disc bulge and stress in the annulus ground substance, and simultaneously produce decreased facet contact force and stresses in posterior structure. CONCLUSION: The decrease of endplate concavity enhances the protective effects of the disc on vertebral body breakage. Small endplate deformation results from decreased endplate concavity would contribute to the reduction of nutritional diffusion to the intervertebral disc.
The Effect of Disc Height and Degeneration on the Mechanical Behavior of Lumbar Spine:A Finite Element Analysis

Abstract:Objective: To evaluate the effects of disc height and degeneration on mechanical behavior of human lumbar spine segments. Methods: A new effective CAD method was used to accurately establish three finite element models, including normal height disc (NHD) model, pure height decreased disc (PHDD) model and degeneration combined height decrease disc (DHDD) model. The biomechanical parameters of three finite element models were measured under axial compressive load. Results: The height and degeneration of disc had obviously effects on the mechanical behavior of lumbar spine segment in terms of axial displacement, posterolateral disc bulge, the maximum stress in the peripheral anulus fibers. The stress distribution differed between DHDD and non-DHDD model. Conclusions: The stiffness of disc obviously increases after height decreasing. The happening of disruption of inner anulus fibers is the lowest in DHDD model. The ratio between the gap of facet articulation and axial displacement are crucial to determine the load strength of disc. The degenerated disc plays an effective role for stress distribution.
Annular Defects Impair the Mechanical Stability of the Intervertebral Disc

Abstract:Study Design: A biomechanical study. Objectives: The purpose of this study was to investigate the effects of cruciform and square incisions of annulus fibrosus (AF) on the mechanical stability of bovine intervertebral disc (IVD) in multiple degrees of freedom. Methods: Eight bovine caudal IVD motion segments (bone-disc-bone) were obtained from the local abattoir. Cruciform and square incisions were made at the right side of the specimen's annulus using a surgical scalpel. Biomechanical testing of threedimensional 6 degrees of freedom was then performed on the bovine caudal motion segments using the mechanical testing and simulation (MTS) machine. Force, displacement, torque and angle were recorded synchronously by the MTS system. P value <.05 was considered statistically significant. Results: Cruciform and square incisions of the AF reduced both axial compressive and torsional stiffness of the IVD and were significantly lower than those of the intact specimens (P <.01). Left-side axial torsional stiffness of the cruciform incision was significantly higher than a square incision (P <.01). Neither incision methods impacted flexional-extensional stiffness or lateralbending stiffness. Conclusions: The cruciform and square incisions of the AF obviously reduced axial compression and axial rotation, but they did not change the flexion-extension and lateral-bending stiffness of the bovine caudal IVD. This mechanical study will be meaningful for the development of new approaches to AF repair and the rehabilitation of the patients after receiving discectomy.

Abstract:SUMMARY Homografts of mouse skin in which cellular viability had been destroyed by repeated freezing and thawing or by treatment with cyanide but in which major structural changes had not occurred were accepted by the host without evidence of immune reaction, either by histologic criteria or by challenge with second-set viable grafts. Treated homografts evolved like similarly treated isografts. With adequate protection these dermal homografts persisted for long periods without necrosis, undergoing eventual resorption and replacement by scar tissue, rather than being revascularized and repopulated by host connective tissue cells. The acellular dermal homografts supported growth of isologous epithelium derived from grafts or from marginal ingrowth. These findings question the concept of cellular repopulation of non-viable autologous or homologous connective tissue grafts.
Anchorage of Annulus Fibrosus Within the Vertebral Endplate with Reference to Disc Herniation

Abstract:To describe the microarchitecture of the annulus-endplate region with reference to disc herniation.Twenty-five motion segments (L1-2 to L5-S1) were harvested from five cadaveric spines (four males, one female). Superior and inferior annulus-endplate junctions were divided into anterior, lateral and posterior regions. Tissue blocks were fixed, decalcified, and embedded in paraffin. Thin sections (4 µm) were stained with H&E and Masson's trichrome for light microscopy.Annulus fibrosus presents a laminated structure, with lamellae becoming wider and more disordered close to the nucleus. Each lamella comprises parallel bundles of collagen fibers. On entering the hyaline cartilage endplates, fiber bundles from the outer annulus often change direction, branch into sub-bundles separated by hyaline cartilage matrix, and no longer exhibit the undulating "crimp" structure of the annulus. Their three-dimensional multileaf morphology appears to provide direct mechanical anchorage in vertebral bone. Calcification is present at the annulus-cartilage and cartilage-bone interfaces, especially posteriorly. In the inner annulus and nucleus, annulus fibers branch and merge obliquely with the hyaline cartilage matrix. The diameter of collagen fiber bundles and sub-bundles increased significantly in the outer annulus compared with middle annulus, and increased at lower spinal levels, but no significant differences were observed between superior and inferior endplates.The branching of annulus collagen fiber bundles within the endplates increases the interface area with surrounding matrix, reducing shear stresses and (probably) increasing strength. Calcification may also increase strength. These observations can explain why disc herniations often contain cartilage and bone fragments from the endplate.
Biomechanical evaluation of a novel intervertebral disc repair technique for large box-shaped ruptures

Abstract:Objective: The purpose of this study was to analyze the feasibility of repairing a ruptured intervertebral disc using a patch secured to the inner surface of the annulus fibrosus (AF). Different material properties and geometries for the patch were evaluated. Methods: Using finite element analysis, this study created a large box-shaped rupture in the posterior-lateral region of the AF and then repaired it with a circular and square inner patch. The elastic modulus of the patches ranged from 1 to 50 MPa to determine the effect on the nucleus pulposus (NP) pressure, vertical displacement, disc bulge, AF stress, segmental range of motion (ROM), patch stress, and suture stress. The results were compared against the intact spine to determine the most suitable shape and properties for the repair patch. Results: The intervertebral height and ROM of the repaired lumbar spine was similar to the intact spine and was independent of the patch material properties and geometry. The patches with a modulus of 2-3 MPa resulted in an NP pressure and AF stresses closest to the healthy disc, and produced minimal contact pressure on the cleft surfaces and minimal stress on the suture and patch of all models. Circular patches caused lower NP pressure, AF stress and patch stress than the square patch, but also caused greater stress on the suture. Conclusion: A circular patch with an elastic modulus of 2-3 MPa secured to the inner region of the ruptured annulus fibrosus was able to immediately close the rupture and maintain an NP pressure and AF stress similar to the intact intervertebral disc. This patch had the lowest risk of complications and produced the greatest restorative effect of all patches simulated in this study.

Abstract:The annuus fibrosus (AF) and nucleus pulposus (NP) of the intervertebral disc (IVD) work in conjunction to dissipate spinal loads. In this study we have isolated the contribution of the NP to the overall response of the disc and investigated the effect of extreme structural changes to the disc on the mechanical behaviour. Linear stiffness, overall load range, hysteresis area and total energy were used to evaluate the impact of these changes on the spine and surrounding structures. Six porcine lumbar isolated disc specimens were tested in 6 DOFs with a 400 N compressive axial preload at low strain rates in three conditions: intact (IN), after total nucleotomy (NN) and after the injection of bone cement into the nuclear void (SN). The latter two conditions, NN and SN, were chosen to emulate the effect of extreme changes to the NP on disc behaviour. When comparing with intact specimens, significant changes were noted primarily in axial compression-extension, mediolateral bending and flexion-extension. NN and SN cases demonstrated significant increases in linear stiffness, overall load range and total energy for mediolateral bending and flexion-extension compared to the intact (IN) state. SN also demonstrated a significant increase in total energy for axial compression-extension, and significant decreases in the elastic contribution to total energy in all axes except flexion-extension. These changes to total energy indicate that surrounding spinal structures would incur additional loading to produce the same motion in vivo after structural changes to the disc.
Minimal Invasive Annulotomy for Induction of Disc Degeneration and Implantation of Poly (Lactic-Co-glycolic Acid) (PLGA) Plugs for Annular Repair in a Rabbit Model

Abstract:Background The rabbit disc model is useful for the study of intervertebral disc (IVD) degeneration and experimental therapeutic interventions. The annulotomy-induced disc models present several drawbacks, particularly an excessive disruption of disc integrity and a rapidly disc degeneration; therefore, this study sought to establish a minimal invasive annulotomy for induction of disc degeneration model, combined to annulus repair using implantation of a PLGA (poly lactic-co-glycolic acid) plug. Methods New Zealand white rabbits ( n = 24) received annular injuries in three discs (L3/4, L4/5 and L5/6). The experimental discs were randomly assigned to four groups: (a) annular defect with a 1.8 mm diameter mini-trephine; (b) annular puncture by 16G needle; (c) annular defect with a PLGA plug implanted by press-fit fashion; (d) uninjured L2/3 disc served as control. Postsurgical x-ray, MRI examination, and real-time PCR analysis were performed at 1, 3 and 6 months. Gross morphology and histology were evaluated at postoperative 6 months. Results Radiographic examinations showed a slow, progressive disc space narrowing and a significant degree of disc degeneration on MRI grade in the injured discs at 6 months in all rabbits. Histological examinations and aggrecan, Col1A1, Col2A1 and matrix metalloprotease (MMP)-3 mRNA expression confirmed the disc degeneration, supporting the imaging results. The PLGA implantation reduced the marked loss of T2-weighted signal intensity seen at MRI in the injured discs and slowly decreased the disc height index (DHI) over the follow-up period. HE/Safranin O staining showed that annular defect was replaced by regenerated connective tissue with significant loss of proteoglycan content. Conclusions The minimally invasive approach for the creation of annular defects by an appropriately sized mini-Trephine is a suitable option for the study of disc degeneration in a rabbit model. Implantation of a suitable PLGA plug induced a successful repair of the annulus fibrosus within the degenerated disc, and retarded the degenerative process in the annular injury model.

Abstract:Background: Growth of nerve fibers has been shown to occur in a rabbit model of intravertebral disc degeneration (IVD) induced by needle puncture. As nerve growth may underlie the process of chronic pain in humans affected by disc degeneration, we sought to investigate the factors underlying nerve ingrowth in a minimally invasive annulotomy rabbit model of IVD by comparing the effects of empty disc defects with those of defects filled with poly(lactic-co-glycolic acid)/fibrin gel (PLGA) plugs.Methods: New Zealand white rabbits (n = 24) received annular injuries at three lumbar levels (L3/4, L4/5, and L5/6). The discs were randomly assigned to four groups: (a) annular defect (1.8-mm diameter; 4-mm depth) by mini-trephine, (b) annular defect implanted with a PLGA scaffold containing a fibrin gel, (c) annular puncture by a 16G needle (5-mm depth), and (d) uninjured L2/3 disc (control). Disc degeneration was evaluated by radiography, MRI, histology, real-time PCR, and analysis of proteoglycan (PG) content. Nerve ingrowth into the discs was assessed by immunostaining with the nerve marker protein gene product 9.5.Results: Injured discs showed a progressive disc space narrowing with significant disc degeneration and proteoglycan loss, as confirmed by imaging results, molecular and compositional analysis, and histological examinations. In 16G punctured discs, nerve ingrowth was observed on the surface of scar tissue. In annular defects, nerve fibers were found to be distributed along small fissures within the fibrocartilaginous-like tissue that filled the AF. In discs filled with PLGA/fibrin gel, more nerve fibers were observed growing deeper into the inner AF along the open annular track. In addition, innervations scores showed significantly higher than those of punctured discs and empty defects. A limited vascular proliferation was found in the injured sites and regenerated tissues.Conclusions: Nerve ingrowth was significantly higher in PLGA/fibrin-filled discs than in empty defects. Possible explanations include (i) annular fissures along the defect and early loss of proteoglycan may facilitate the ingrowth process and (ii) biodegradable PLGA/fibrin gel may promote adverse growth of nerves and blood vessels into deeper parts of injured disc. The rabbit annular defect model of disc degeneration appears suitable to investigate the effects of nerve ingrowth in relation to pain generation.

Abstract:Intervertebral disc herniation is not a common injury in the adolescent population, but the correlation between trauma and herniation warrants concern. Previous research demonstrated the capacity for rapid internal pressurization to reduce the mechanical integrity of the intervertebral disc's annulus fibrosus, even in the absence of fracture. The purpose of this study was to modify previous internal pressurization procedures towards a more transferable injury model, then investigate the capacity for these procedures to damage the mechanical integrity of the annulus fibrosus. Porcine cervical motion segments with intact facet joints were confined between a vice and force plate under 300 N of static compression, then a single, manual, rapid internal pressurization was delivered. Posterolateral annulus samples were extracted and situated in a 180° peel test configuration, exposing the interlamellar matrix of samples to separations of 0.5 mm/s, until complete separation of the sample occurred. Multilayer tensile testing was performed on superficial and mid-span samples of annulus by applying uniaxial tension of 1 %/s to 50 % strain. Compared to unpressurized controls, rapid pressurization causing fracture resulted in reduced lamellar adhesion and increased toe-region stress and strain properties in the annulus. Morphological assessment reported similar fracture patterns between endplate fractures achieved in the present experiment and endplate fractures documented in human patients. Mechanical plus morphological results suggest that rapid internal pressurization resulting in endplate fracture may represent a potent mechanism for subsequent damage to the intervertebral disc.
Minimally Invasive Anterior Lumbar Interbody Fusion for Adjacent Segment Disease after Posterior Lumbar Fusion

Abstract:Posterior lumbar fusion is a popular technique for managing lumbar spine degenerative diseases including disc hernia, canal stenosis, scoliosis, spondylosis and degenerative spondylolisthesis. One of the commonest complications is adjacent segment disease (ASD). About 4.7% patients who have undergone lumbar fusion need revision surgery for ASD1. There are various revision procedures; they include anterior and posterior approaches. Due to the previous posterior procedure, the risks of a posterior revision approach are obviously high and include dural tear injury, nerve root injury, and secondary damage to posterior structures. Thus, a retroperitoneal approach to performing minimally invasive anterior lumbar interbody fusion (ALIF) would avoid all the risks mentioned above and might therefore be a better treatment option2, 3. A 45-year-old female had undergone posterior lumbar interbody fusion at the L3–4 to L4–5 level and posterolateral fusion of the L5S1 level 6 months previously for degenerative disease of the lumbar spine in a local hospital with an initially satisfactory outcome. However, five months after this surgery, she had developed progressive low back pain with numbness and tingling throughout her right leg. Careful physical examination showed tenderness on the right side of the L5 spinous process. She had a positive Lasègue sign and straight leg raising test in the right leg. There was also decreased sensation over the lateral aspect of the leg, lateral malleolus and lateral aspect of the foot. Plantar flexion was weaker in her right foot than in her left. The ankle jerk was reduced on the right side. Routine laboratory investigations were normal. X-ray films revealed that L3–S1 had been fixed with pedicle screws and there were cages in the L3–4 and L4–5 interspaces. Osteolysis was noted around the pedicle screws in the S1 vertebral body. MRI revealed disc herniation on the right side at the L5S1 level. Therefore, the preoperative diagnosis was disc herniation of L5S1 and ASD (L5S1). ALIF with an integrated titanium plate and a poly(ether-ether-ketone) (PEEK) cage (SynFix-LR, Synthes, Solothurn, Switzerland) was planned. To avoid vessel injury and identify an appropriate operating area, computed tomography angiography (CTA) of the abdominal aorta was performed preoperatively. After induction of general anesthesia, the patient was positioned supine on a table with a break at the level of the lumbar spine to exaggerate lumbosacral lordosis. A Pfannenstiel incision was made between the level of the umbilicus and symphysis pubis. The left side of the rectus sheath was dissected parallel to the midline and the left rectus abdominis was retracted laterally, after which the retroperitoneal space was entered by retracting all the periosteum to the right4. After the L5S1 level had been exposed and fluoroscopically verified, a SynFrame Retractor System (Synthes) was used to create a stable surgical field5. Before performing discectomy, the median sacral vessels were carefully ligatured or electrocoagulated. Then an adequate discectomy was performed6 and the nerve root was carefully decompressed. The adjacent vertebral endplates were scratched appropriately. After a trial cage had been tested, an appropriately sized cage with allograft was inserted into the intervertebral space. The cage was anchored by four diverging angle-stable locking screws into the adjacent vertebrae and its position was fluoroscopically confirmed. The operation time was 110 min with an estimated blood loss of 120 mL. The duration of hospitalization was 7 days. With the improved PEEK cage integrated with a titanium locking plate and four angle-stable screws (SynFix-LR, Synthes), stand-alone ALIF can meet the biomechanical needs of lumbar fusion7, 8. The improved cage is biomechanically superior to cages with separate plates9 and has a zero profile construct, which reduces the rates of vessel and visceral organ injury. The SynFrame Retractor System provides a more stable operative field and allows a smaller skin incision, reducing the risk of vessel and organ injury during the procedure5. A preoperative CTA of the abdominal aorta and careful handling of median sacral vessels increase the safety of this procedure. This technique facilitates more direct discectomy and more complete nerve decompression and involves a larger cage which results in higher fusion rates10. As a revision for ASD after posterior lumbar fusion, ALIF offers direct anterior column support without disturbing the posterior scars around the dural sac and nerve roots. Biomechanical Effect of C 5 /C 6 Intervertebral Reconstructive Height on Adjacent Segments in Anterior Cervical Discectomy and Fusion - A Finite Element Analysis

Abstract:Objective: To investigate the biomechanical effect of different intervertebral reconstructive heights on adjacent segments following C 5 /C 6 anterior cervical discectomy and fusion (ACDF) through ﬁ nite element analysis. Methods: A ﬁ nite element model of intact C 4 – C 7 segments was developed and validated for the present study. Five additional C 4 – C 7 postoperative models were constructed with 100%, 125%, 150%, 175%, and 200% of the benchmark height of C 5 /C 6 on the basis of the intact model. The changes in intradiscal pressure (IDP) and range of motion (ROM) of adjacent segments before and after reconstruction of C 5 /C 6 were analyzed. Results: For the upper adjacent segment (C 4 /C 5 ), the IDPs under the different loading conditions all increased after reconstruction. The maximum IDPs were 0.387, 0.489, 0.491, and 0.472 MPa under ﬂ exion, extension, axial rotation, and lateral bending, respectively, observed at the reconstructive height of 200%. The minimum IDPs were observed at 150% reconstructive height under all loading conditions except extension, and were 57, 86 and 81% of the maximum IDPs under ﬂ exion, axial rotation, and lateral bending, respectively. The minimum IDP under extension occurred when the reconstructive height is 125% of the benchmark height. For the lower adjacent segment (C 6 /C 7 ), the IDPs of postoperative models under all loading conditions also increased compared to the preoperative model. The maximum IDPs after reconstruction under ﬂ exion, extension, axial rotation, and lateral bending were 0.402, 0.411, 0.461, and 0.497 MPa, respectively, when the height of the reconstruction was 200% of the benchmark. The minimum IDPs were observed after a reconstruction at 150% of the benchmark, and were 59%, 85%, 82%, and 81% of the maximum IDPs under ﬂ exion, extension, axial rotation, and lateral bending loading conditions. Conclusions: The reconstructive height is an important factor affecting the IDP and the ROM of adjacent segments after ACDF. To delay the adjacent segment disease, an intervertebral reconstructive height of 150% is an appropriate height in C 5 /C 6 ACDF.

Abstract:Objective: Characterize 3D remodeling of the rat intervertebral disc (IVD) following acute annular injury via in vivo micro-computed tomography (µCT), ex vivo contrast-enhanced (CE)-µCT, and histology.Design: Female Lewis rats (N = 4/group) underwent either sham surgery or anterior annular puncture to L3-L4 and L5-L6 (n = 8 IVDs/group) to induce IVD degeneration. Rats were allowed ad libidum cage activity before and after surgery and underwent in vivo µCT scanning at baseline and every 2 weeks post-op for 12 weeks to characterize longitudinal changes in IVD height. At 12 weeks, lumbar spines were dissected and underwent CE-µCT scanning to characterize endpoint glycosaminoglycan distribution and nucleus pulposus (NP) volume ratio. Spines were processed for safranin-O-stained sagittal histology, and IVD degeneration was graded via the Rutges scale.Results: Puncture IVDs exhibited loss of IVD height at all time points from 4 weeks onward compared to Sham—the most severe height loss occurred posteriorly, with significant changes also occurring in the NP and laterally. Puncture IVDs exhibited higher CE-µCT attenuation, indicative of lower glycosaminoglycan content, and reduced NP volume ratio compared to Sham. Histologically, Puncture IVDs had higher Rutges damage scores and exhibited reduced NP cellularity and hydration, disorganized annulus fibrosus (AF) lamellae with evidence of the stab tract, and indistinct AF-NP border compared to Sham.Conclusions: Characterization of the complex, 3D alterations involved in the onset and early progression of IVD degeneration can foster greater understanding of the pathoetiology of IVD degeneration and may inform future studies assessing more sensitive diagnostic techniques or novel therapies.

Abstract:Three experiments with rats explored the differential outcome effect (DOE) using a pavlovian magazine approach conditioning preparation. Experiment 1 compared groups trained on a biconditional discrimination (AX+, AY-, BX-, BY+) with differential or nondifferential outcomes, and Experiment 2 examined this using an ambiguous occasion setting task (e.g., AX+, X-, Y+, AY-). In both experiments, subjects trained with differential outcomes learned the tasks better than subjects trained with nondifferential outcomes. Furthermore, subjects given differential outcome training learned the positive occasion setting component of the ambiguous task more efficiently than the negative occasion setting component, although both were enhanced by differential outcome training. Experiment 3 demonstrated that the ambiguous occasion setting task was reversed more readily when the target-outcome relations (as opposed to the modulator-outcome relations) were maintained during the reversal phase. These data suggest that an acquired distinctiveness effect may be responsible for the DOE in pavlovian learning.
Innovative hydrogel-patch combination for large annulus fibrosus defects: a prospective approach to address herniation recurrence

Abstract:Background context: Large annulus fibrosus (AF) defects often lead to a high rate of reherniation, particularly in the medial AF region, which has limited self-healing capabilities. The increasing prevalence of herniated discs underscores the need for effective repair strategies. Purpose: The objectives of this study were to design an AF repair technique to reduce solve the current problems of insufficient mechanical properties and poor sealing capacity. Study design: In vitro biomechanical experiments and finite element analysis. Methods: The materials used in this study were patches and hydrogels with good biocompatibility and sufficient mechanical properties to withstand loading in the lumbar spine. Five repair techniques were assessed in this study: hydrogel filler (HF), AF patch medial barrier (MB), AF patch medial barrier and hydrogel filler (MB&HF), AF patch medial-lateral barrier (MLB), and AF patch medial-lateral barrier and hydrogel filler (MLB&HF). The repair techniques were subjected to in vitro testing (400 N axial compression and 0-500 N fatigue loading at 5Hz) and finite element analysis (400 N axial compression) to evaluate the effectiveness at repairing large AF defects. The evaluation included repair tightness, spinal stability, and fatigue resistance. Results: From the in vitro testing, the failure load of the repair techniques was in the following order HF <MB <MB&HF <MLB <MLB&HF. Both HF and MB groups failed to effectively increase intervertebral disc (IVD) stiffness, resulting in a reduction in spatial stability. The MLB, MB&HF, and MLB&HF groups partially restored IVD stiffness, with MLB&HF showing the most effective recovery (-24.13% ± 3.59%). From the finite element models, incorporating a hydrogel filler was best able to maintain the IVD height. Patch repair alone could not adequately reduce the high AF stress due to AF injury, but with hydrogel support, stress was substantially low and more uniformly distributed. All repair techniques demonstrated reduced stress around the damaged area on the AF, in comparison to the unrepaired model. The NP pressure in the HF group was closest to the intact group, and the patch repair reduced the NP pressure. The maximum patch deformation and suture stress were ranked as MB >MLB >MB&HF >MLB&HF. Conclusions: The combined use of patches and hydrogels exhibited promising mechanical properties postdiscectomy, providing a promising solution for addressing large AF defects and improving disc stability. Clinical significance: This study introduces a promising method for repairing large annular fissure (AF) defects after disc herniation, combining patch repair with a hydrogel filler. These techniques hold potential for developing clinical AF repair products to address this challenging issue.
An Approach to Quantify Anisotropic Multiaxial Failure of the Annulus Fibrosus

Abstract:Tears in the annulus fibrosus (AF) of the intervertebral disk (IVD) occur due to multiaxial loading on the spine. However, most existing AF failure studies measure uniaxial stress, not the multiaxial stress at failure. Delamination theory, which requires advanced structural knowledge and knowledge about the interactions between the AF fibers and matrix, has historically been used to understand and predict AF failure. Alternatively, a simple method, the Tsai-Hill yield criteria, could describe multiaxial failure of the AF. This yield criteria uses the known tissue fiber orientation and an equation to establish the multiaxial failure stresses that cause failure. This paper presents a method to test the multiaxial failure stress of the AF experimentally and evaluate the potential for the Tsai-Hill model to predict these failure stresses. Porcine AF was cut into a dogbone shape at three distinct angles relative to the primary lamella direction (parallel, transverse, and oblique). Then, each dogbone was pulled to complete rupture. The Cauchy stress in the material's fiber coordinates was calculated. These multiaxial stress parameters were used to optimize the coefficients of the Tsai-Hill yield. The coefficients obtained for the Tsai-Hill model vary by an order of magnitude between the fiber and transverse directions, and these coefficients provide a good description of the AF multiaxial failure stress. These results establish both an experimental approach and the use of the Tsai-Hill model to explain the anisotropic failure behavior of the tissue.

Abstract:BACKGROUND CONTEXT In clinical practice, acute trauma and chronic degeneration of the annulus fibrosus (AF) can promote further degeneration of the intervertebral disc (IVD). Therefore, it is critical to understand the AF repair process and its consequences on IVD. However, the lack of cost-effective and reproducible in vivo animal models of AF injury has limited research development in this field. PURPOSES The purpose of this study was to establish and evaluate the utility of a novel animal model for full-thickness AF injury. Three foci were proposed: (1) whether this new modeling method can cause full-layer AF damage; (2) the repair processes and pathological changes in the damaged area after AF injury, and (3) the morphological and histological changes in the IVD are after AF injury. STUDY DESIGN/SETTING In vivo rat AF injury model with characterization of AF damage repair, IVD degeneration. METHODS A total of 72,300 g male rats were randomly assigned to one of the two groups: experimental or sham. Annulus fibrosus was separated layer by layer under the microscope with a #11 blade up to the AF- nucleus pulpous (NP) junction. The repair process of the horizontal AF and morphological changes in the sagittal IVD were evaluated with HE staining. Sirius red staining under polarized light. Immunofluorescence was conducted to analyze changes in the expression of COL1 and COL3 in the AF injury area and 8-OHdg, IL-6, MMP13, FSP1, and ACAN in the IVD. The disc height and structural changes after AF injury were measured using X-ray and contrast-enhanced micro-CT. Additionally, the resistance of the AF to stretching was analyzed using three-point bending. RESULTS Annulus fibrosus-nucleus pulpous border was identified to stably induce the full-thickness AF injury without causing immediate NP injury. The AF repair process after injury was slow and expressed inflammation factors continuously, with abundant amounts of type III collagen appearing in the inner part of the AF. The scar at the AF lesion had decreased resistance to small molecule penetration and weakened tensile strength. Full-thickness AF injury induced disc degeneration with loss of disc height, progressive unilateral vertebral collapse, and ossification of the subchondral bone. Inflammatory-induced degeneration and extracellular matrix catabolism gradually appeared in the NP and cartilage endplate (CEP). CONCLUSIONS We established a low-cost and reproducible small animal model of AF injury which accurately replicated the pathological state of the limited AF self-repair ability and demonstrated that injury to the AF alone could cause further degeneration of the IVD. CLINICAL RELEVANCE This in vivo rat model can be used to study the repair process of the AF defect and pathological changes in the gradual degeneration of IVD after AF damage. In addition, the model provides an experimental platform for in vivo experimental research of potential clinical therapeutics.

Abstract:OBJECTIVE To determine Leptin levels in supernatant fluid of culture of human umbilical vein endothelial cells (ECV-304) after being challenged by lipopolysaccharide (LPS) and calcium ion vector A23187, and to explore the possible relation between Leptin release and cytosolic phospholipase A(2) (cPLA(2)) activity in an inflammatory cell model. METHODS ECV-304 cells were cultured in vitro. Experiment 1: the cells were divided into seven groups: blank control group, LPS 5, 10, 20 μg/ml stimulation groups, A23187 0.1, 1.0, 10.0 μmol/L stimulation groups. The supernatants were collected at 6, 12 and 24 hours.Experiment 2: according to the results of experiment 1, the cells were divided into eight groups: blank control group, LPS 20 μg/ml stimulation group, the inhibitor of cPLA2 AACOCF3 0.1, 1.0, 10.0 μmol/L plus LPS stimulation groups, the inhibitor of mitogen-activated protein/extracellular signal-regulated protein kinase kinase 1/2 (MEK1/2) U0126 0.1, 1.0, 5.0 μmol/L plus LPS stimulation groups, with AACOCF3 or U0126 added 1 hour before the addition of LPS, and the supernatants were collected 24 hours after the addition of LPS. Leptin level was determined by radioimmunoassay. RESULTS Experiment 1: with increase in LPS concentration and prolongation of time, Leptin release was decreased gradually. After 24 hours of interaction the concentration of Leptin (ng/ml) in LPS 20 μg/ml group was decreased significantly compared with the blank control group (0.540±0.109 vs. 0.823±0.048,P<0.05). However, A23187 had no significant effect on Leptin release. Experiment 2: LPS rendered cells to release less Leptin (ng/ml: 0.558±0.069 vs. 0.825±0.067,P<0.05); by adding AACOCF3 or U0126 in different concentration before adding LPS rendered the cells to release more Leptin (ng/ml), and it showed concentration-dependent (the AACOCF3 0.1, 1.0, 10.0 μmol/L groups were 0.673±0.135, 0.723±0.055, 0.797±0.062, respectively; the U0126 0.1, 1.0, 5.0 μmol/L groups were 0.698±0.112, 0.862±0.184, 0.935±0.145, respectively). The release of Leptin in AACOCF3 1.0 μmol/L, 10.0 μmol/L and U0126 1.0 μmol/L, 5.0 μmol/L groups was significantly higher than LPS 20 μg/ml stimulation group (all P<0.05). CONCLUSION There is a possible relation between Leptin release and cPLA 2 activity in inflammatory cells induced by LPS.
