Abstract:BACKGROUND: Histomorphological change of endplate may affect the nntritional transmission of intervertebral disk, eventually leading to intervertebral disc degeneration. OBJECTIVE: To observe the effects of endplate concavity variation on mechanical response of lumbar motion segment. DESIGN, TIME AND SETTING: Repeated measurement design, biomechanical analysis of finite element models, performed at the Laboratory of Biomechanics, Department of Orthopedics, the Second Affiliated Hospital of Medical College of Zhejiang University between January 2005 and January 2007. MATERIALS: SOMATOM SENSATION 16 spiral CT machine (Siemens, German) and ANSYS (Inc. Pennsylvania, USA).METHODS: A three-dimensional nonlinear geometrical and mechanical accurate finite element model of lumbar L<,4-5> segment was developed. Parametric studies were undertaken by studying endplate of three different concave angles, whereas disc angle, the gap of facet joint, finite element mesh density, and all other parameters were kept constant. Biomechanical parameters of three kinds of finite element models were tested under 5 different loading conditions, including vertical compression, flexion, extension, and anterior and posterior shearing loading. MAIN OUTCOME MEASURES: Endplate-intervertebral disc interface strain, intervertebral disc stiffness, nucleus pulposus pressure, annular fiber stress, radial disc bulge, stress in the annulus ground substance, stresses in posterior structure and facet contact force. RESULTS: The decrease in the endplate concavity stimulated by an increasing endplate concave angle would result in decreased strains of endplate-intervertebral disc interface, increased disc stiffness and nucleus pulposus pressure, decreased annular fiber stress, radial disc bulge and stress in the annulus ground substance, and simultaneously produce decreased facet contact force and stresses in posterior structure. CONCLUSION: The decrease of endplate concavity enhances the protective effects of the disc on vertebral body breakage. Small endplate deformation results from decreased endplate concavity would contribute to the reduction of nutritional diffusion to the intervertebral disc.

Abstract:Objective: To evaluate the effects of disc height and degeneration on mechanical behavior of human lumbar spine segments. Methods: A new effective CAD method was used to accurately establish three finite element models, including normal height disc (NHD) model, pure height decreased disc (PHDD) model and degeneration combined height decrease disc (DHDD) model. The biomechanical parameters of three finite element models were measured under axial compressive load. Results: The height and degeneration of disc had obviously effects on the mechanical behavior of lumbar spine segment in terms of axial displacement, posterolateral disc bulge, the maximum stress in the peripheral anulus fibers. The stress distribution differed between DHDD and non-DHDD model. Conclusions: The stiffness of disc obviously increases after height decreasing. The happening of disruption of inner anulus fibers is the lowest in DHDD model. The ratio between the gap of facet articulation and axial displacement are crucial to determine the load strength of disc. The degenerated disc plays an effective role for stress distribution.

Abstract:OBJECTIVE To explore the effects of intervertebral disc degeneration on the biomechanical behavior of the lumbar motion segment. METHODS A three-dimensional nonlinear finite element model of L4-L5 segment was established using CAD technique based on CT images. A normal disc model and three degenerative disc models were established by changing the parameters such as disc material properties and disc height. The effects of disc degeneration on the biomechanical properties, including stiffness, nucleus pressure, maximum von Mises stress in the annulus, and force of posterior structure were studied under two moment loads (flexion and extension) and for three different direct forces (compression, and anterior and posterior shear forces), and the stress distribution of vertebral body and end- plate was also analyzed. RESULTS The stiffness of the lightly degenerative disc model was decreased compared with that of the normal disc, while it was increased in the moderately and severely degenerative disc models when compared with the normal disc. The force of posterior structure in the lightly degenerative disc model was increased while the values of force of posterior structure in the moderately and severely degenerative disc models were decreased gradually. The maximum von Mises stress in the annulus increased, and the nucleus pressure decreased as the disc degeneration progressed. Moreover, the stress of intervertebral disc, vertebral body, and end-plate was distributed more peripherally. CONCLUSION Light degeneration of intervertebral disc leads to instability of lumbar spine, while the stability restores with further degeneration of disc. There is a negative correlation between the force of posterior structure and the load on the intervertebral disc. With the disc degeneration progressing, the intervertebral disc load pattern changes, the stress of intervertebral disc, vertebral body, and end-plate is concentrated peripherally.

Abstract:The annulus fibrosus (AF), a permeable, hydrated, and fiber-reinforced soft tissue, exhibits complex responses influenced by fluid pressure, osmotic pressure, and structural mechanics. Existing models struggle to comprehensively represent these intricate interactions and the heterogeneous solid responses within the AF. Additionally, the mechanisms driving differential damage accumulation between non-degenerative and degenerative intervertebral discs remain poorly understood. In this study, we introduce a biphasic-swelling damage model for the AF. We conceptually develop and rigorously validate this model through tissue-level tests employing various loading modes, consistently aligning model predictions with experimental data. Leveraging parametric geometric algorithms and custom Python scripts, we construct models simulating both non-degenerative and degenerative discs. Following calibration, we subject these models to viscous loading protocols. Our findings reveal the posterior AF's susceptibility to damage, contingent upon loading rate and water content. We elucidate the underlying mechanisms by examining the temporal evolution of fluid pressure, osmotic pressure, and the regionally dependent fiber network. This research presents a highly accurate model of the AF, providing valuable insights into disc damage. Future research endeavors should expand this model to incorporate ionic transport and diffusion, enabling a more profound exploration of intervertebral disc mechanobiology. This comprehensive model contributes to a better understanding of AF behavior and may inform therapeutic strategies for disc-related pathologies. STATEMENT OF SIGNIFICANCE: This research presents a comprehensive model of the annulus fibrosus (AF), a crucial component of the intervertebral disc that provides structural support and resists deformation. The study introduces a biphasic-swelling damage model for the AF and validates it through tissue-level tests. The model accounts for fluid pressure, osmotic pressure, and matrix mechanics, providing a more accurate representation of the AF's behavior. On the identification of the ultra-structural organization of elastic fibers and their effects on the integrity of annulus fibrosus

Abstract:Due to the complicated structure of the elastic fiber network in annulus fibrosus, existing in-silico studies either simplified or just overlooked its distribution pattern. Nonetheless, experimental and simulation results have proven that elastic fibers are of great importance to maintaining the structural integrity of annulus fibrosus and therefore to ensuring the load-bearing ability of intervertebral discs. Such needs call for a fine model. This work aims at developing a biphasic annulus fibrosus model by incorporating the accurate distribution pattern of collagen and elastic fibers. Both the structural parameters and intrinsic mechanical parameters were successfully identified using single lamella and inter-lamella microscopy anatomy and micromechanical testing data. The proposed model was then used to implement finite element simulations on various anterior and posterolateral multi-lamellae annulus fibrosus specimens. In general, simulation results agree well with available experimental and simulation data. On this basis, the effects of elastic fibers on the integrity of annulus fibrosus were further investigated. It was found that elastic fibers significantly influence the free swelling, radial stretching and circumferential shear performances of annulus fibrosus. Nonetheless, no significant effects were found for the circumferential stretching capability. The proposed biphasic model considers for the first time the distribution characteristics of elastic fibers at two scales, including both the principal orientations of all fiber families and the detailed distribution pattern within each family. Better understandings on the functions of collagen and elastic fibers can therefore be realized. To further enhance its prediction capability, the current model can be extended in the future by taking the fiber-matrix interaction as well as progressive damages into consideration.

Abstract:Degenerative disc degeneration (DDD) is the major cause of low back pain, which seriously affects the life of patients. Current surgical and conservative treatments only relieve the pain temporarily, yet fail to restore the normal biomechanics and functions of healthy spine. Indeed, high recurrence of disc herniation commonly happens after discectomy. Degenerative changes in biomechanical and structural properties of the intervertebral disc (IVD), including fissures in annulus fibrosus (AF) and volume loss of nucleus pulposus (NP), mainly contribute to DDD development. AF plays a critical role in the biomechanical properties of IVD as it structural integrity is essential to confine NP and maintain physiological intradiscal pressure under loading. Maintaining the homeostasis of AF and NP, and thereby IVD, requires regulation of their biomechanics, which is also involved in the onset and subsequent development of AF degeneration. Therefore, it is essential to understand the biomechanical changes of AF during degeneration, which can also provide valuable insights into the repair and regeneration of AF. In this review, we focus on the biomechanical properties of AF tissue associated with its homeostasis and degeneration, and discuss the biomechanical stimulus required for regeneration of AF. We also provide an overview of recent strategies to target and modulate cell mechanics toward AF regeneration.
Eruption combustion synthesis of NiO/Ni nanocomposites with enhanced properties for dye-absorption and lithium storage.

Abstract:Large-scale energy-efficient productions of oxide nanoparticles are of great importance in energy and environmental applications. In nature, volcano eruptions create large amounts of volcano ashes within a short duration. Inspired by such phenomena, we report herein our first attempt to achieve an artificial volcano for mass productions of various oxide nanoparticles with enhanced properties for energy and environmental applications. The introduction of NaF into the solution combustion synthesis (SCS), which is a generally adopted synthetic route for mass productions of various oxide nanoparticles, results in better particle dispersity and a drastic increase in specific surface area compared to the conventional SCS. In a fixed dosage of NaF, a new eruption combustion pattern emerges, which may be contributed to the more gas evolution, lower apparent density, and weaker interparticle force. The novel eruption combustion pattern observed in SCS provides a versatile alternative for SCS to control combustion behavior, microstructure, and property of the products. NiO/Ni nanocomposite yielded by the new approach shows an ideal dye-absorption ability as well as lithium storage capacity. The new SCS pattern reported in this paper is versatile, emerging in various systems of Ni-Co-O, Co-O, La-O, Ni-Co-O, Zn-Co-O, and La-Ni-O.

Abstract:SUMMARY Homografts of mouse skin in which cellular viability had been destroyed by repeated freezing and thawing or by treatment with cyanide but in which major structural changes had not occurred were accepted by the host without evidence of immune reaction, either by histologic criteria or by challenge with second-set viable grafts. Treated homografts evolved like similarly treated isografts. With adequate protection these dermal homografts persisted for long periods without necrosis, undergoing eventual resorption and replacement by scar tissue, rather than being revascularized and repopulated by host connective tissue cells. The acellular dermal homografts supported growth of isologous epithelium derived from grafts or from marginal ingrowth. These findings question the concept of cellular repopulation of non-viable autologous or homologous connective tissue grafts.
The effect of structural changes on the low strain rate behaviour of the intervertebral disc

Abstract:The annuus fibrosus (AF) and nucleus pulposus (NP) of the intervertebral disc (IVD) work in conjunction to dissipate spinal loads. In this study we have isolated the contribution of the NP to the overall response of the disc and investigated the effect of extreme structural changes to the disc on the mechanical behaviour. Linear stiffness, overall load range, hysteresis area and total energy were used to evaluate the impact of these changes on the spine and surrounding structures. Six porcine lumbar isolated disc specimens were tested in 6 DOFs with a 400 N compressive axial preload at low strain rates in three conditions: intact (IN), after total nucleotomy (NN) and after the injection of bone cement into the nuclear void (SN). The latter two conditions, NN and SN, were chosen to emulate the effect of extreme changes to the NP on disc behaviour. When comparing with intact specimens, significant changes were noted primarily in axial compression-extension, mediolateral bending and flexion-extension. NN and SN cases demonstrated significant increases in linear stiffness, overall load range and total energy for mediolateral bending and flexion-extension compared to the intact (IN) state. SN also demonstrated a significant increase in total energy for axial compression-extension, and significant decreases in the elastic contribution to total energy in all axes except flexion-extension. These changes to total energy indicate that surrounding spinal structures would incur additional loading to produce the same motion in vivo after structural changes to the disc.

Abstract:Annulus fibrosus (AF) injury, as a result of intervertebral disk degeneration (IVDD) or herniation, is very difficult to heal due to its avascular characteristics. Tissue engineering strategies have become increasingly promising to treat IVDD. However, in light of the fact that AF displays an anisotropic microstructure that results in gradient mechanical properties, fabrication of tissue engineered AF remains challenging. In this study, we fabricated micropatterned collagen scaffolds with angle-ply structure mimicking the microstructural features of AF. Following that, we cultured bone marrow mesenchymal stromal cells (BMSCs) in the scaffolds and applied cyclic stretching to them to mechanically stimulate the cells. We found that aligned cells under mechanical loading showed elevated expression of matrix anabolism-related genes and proteins. Further studies indicated that Caveolin-1 (CAV1) might mediate the nuclear translocation of Yes-associated protein (YAP) in response to external mechanical cues of microgroove-patterned membranes and mechanical loading. Micropatterned collagen membranes, with cells aligning at +/- 30 degrees along the longitudinal dimension, were rolled up together to form an angle-ply and multilayered tissue. After being implanted into the caudal vertebra in a rat model for 4 weeks, biomimetic tissues showed improved restoration of the disk. In general, our biomimetic collagen membranes exhibit great potential in terms of AF regeneration, with AF-like heterogeneous microstructures that satisfy the biomechanical property of the native disk.

Abstract:Background and objective: The intrinsic link between the compositional and structural attributes and the biomechanical functionality is evident in intervertebral discs. However, it remains unclear from a biomechanical perspective whether cartilage endplate (CEP) degeneration exacerbates intervertebral disc degeneration. Methods: This study developed and quantitatively validated four biphasic swelling-based finite element models. We then applied four quasi-static tests and simulated daily loading scenarios to examine the effects of CEP degradation. Results: Under free-swelling conditions, short-term responses were prevalent, with CEP performance changes not significantly impacting response proportionality. The creep test results showed the more than 50 % of the strain was attributed to long-term responses. Stress-relaxation testing indicated that all responses increased with disc degeneration, yet CEP degeneration's impact was minimal. Daily load analyses revealed that disc degeneration significantly reduces nucleus pulposus pressure and disc height, whereas CEP degeneration marginally increases nucleus pressure and slightly decreases disc height. Conclusions: Glycosaminoglycan content and CEP permeability are critical to the fluid-dependent viscoelastic response of intervertebral discs. Our findings suggest that CEP contributes to disc degeneration under daily loading conditions.

Abstract:Background and objective The annulus fibrosus is an essential part of the intervertebral disc, critical for its structural integrity. Mechanical deterioration in this component can lead to complete disc failure, particularly through tears development, with radial tears being the most common. These tears are often the result of both mechanical and biological factors. This study aims to numerically investigate the mechanisms of radial failure in the annulus tissue, taking into account the mechanical and age-dependent biological damage origins. A newly developed microstructure-based model was upgraded to predict damage evolution in the different annulus regions. Methods The study employs a computational model to predict mechanical failures in various annulus regions, using experimental data for comparison. The model incorporates age-dependent microstructural changes to evaluate the effects of biological aging on the mechanical behavior. It specifically includes a detailed analysis of the temporal changes in circumferential rigidity and failure strain of the annulus. Results The model demonstrated a strong ability to replicate the experimental responses of the different annulus regions to failure. It revealed that age-related microstructural changes significantly impact the rigidity and failure response of the annulus, particularly in the posterior regions and as well the anterior inner side. These changes increase susceptibility to rupture with aging. A correlation was also observed between the composition of collagen fibers, water content, and the annulus transversal response in both radial and axial directions. Conclusion The findings challenge previous assumptions, showing that age-dependent microstructural changes have a notable effect on the annulus mechanical properties. The computational model closely aligns with experimental observations, underscoring the determinant role of oriented collagen fibers in radial failure. This study enhances the understanding of annulus failure and provides a foundation for further research on the impact of aging on disc mechanical integrity and failure.

An Approach to Quantify Anisotropic Multiaxial Failure of the Annulus Fibrosus

Abstract:Tears in the annulus fibrosus (AF) of the intervertebral disk (IVD) occur due to multiaxial loading on the spine. However, most existing AF failure studies measure uniaxial stress, not the multiaxial stress at failure. Delamination theory, which requires advanced structural knowledge and knowledge about the interactions between the AF fibers and matrix, has historically been used to understand and predict AF failure. Alternatively, a simple method, the Tsai-Hill yield criteria, could describe multiaxial failure of the AF. This yield criteria uses the known tissue fiber orientation and an equation to establish the multiaxial failure stresses that cause failure. This paper presents a method to test the multiaxial failure stress of the AF experimentally and evaluate the potential for the Tsai-Hill model to predict these failure stresses. Porcine AF was cut into a dogbone shape at three distinct angles relative to the primary lamella direction (parallel, transverse, and oblique). Then, each dogbone was pulled to complete rupture. The Cauchy stress in the material's fiber coordinates was calculated. These multiaxial stress parameters were used to optimize the coefficients of the Tsai-Hill yield. The coefficients obtained for the Tsai-Hill model vary by an order of magnitude between the fiber and transverse directions, and these coefficients provide a good description of the AF multiaxial failure stress. These results establish both an experimental approach and the use of the Tsai-Hill model to explain the anisotropic failure behavior of the tissue.

Abstract:Intervertebral discs are soft biological tissues that have an important function in accommodating the movement of the spine during which the discs deform and act as shock absorbers to protect the spine from damage. Therefore, it is important to understand and model their mechanical properties to predict their response under various loading conditions. Towards this end, a simplified geometry of a single intervertebral disc, consisting of the nucleus pulposus within the anulus fibrosus, is modelled as a nonlinearly elastic solid. Pre-existing residual stresses in the unloaded state are taken into account in the radial and circumferential directions. In particular, the nucleus pulposus is considered to be isotropic, which becomes anisotropic with residual stress, while the anulus fibrosus consists of fibre families described by two standard reinforcing anisotropic models. The stress components associated with compressive and torsional deformations are determined analytically and illustrated for a simple constitutive model.

Shear mechanical properties of human lumbar annulus fibrosus

Abstract:Function, failure, and remodeling of the intervertebral disc are all related to the stress and strain fields in the tissue and may be calculated by finite element models with accurate material properties, realistic geometry, and appropriate boundary conditions. There is no comprehensive study in the literature investigating the shear material properties of the annulus fibrosus. This study obtained shear material properties of the annulus fibrosus and tested the hypothesis that these properties are affected by the amplitude and frequency of shearing, applied compressive stress, and degenerative state of the tissue. Cylindrical specimens with an axial orientation from seven nondegenerated and six degenerated discs were tested in torsional shear under dynamic and static conditions. Frequency sweep experiments over a physiological range of frequencies (0.1-100 rad/sec) at a shear strain amplitude of 0.05 rad were performed under three different axial compressive stresses (17.5, 25, and 35 kPa). At the largest compressive stress, shear strain sweep experiments (strain amplitude range: 0.005-0.15 rad at a frequency of 5 rad/sec) and transient stress-relaxation tests (shear strain range: 0.02-0.15 rad) were performed. The annulus fibrosus material was less stiff and more dissipative at larger shear strain amplitudes, stiffer at higher frequencies of oscillation, and stiffer and less dissipative at larger axial compressive stresses. The dynamic shear modulus, /G*/, had values ranging from 100 to 400 kPa, depending on the experimental condition and degenerative level. The shear behavior was also predominantly elastic, with values for the tangent of the phase angle (tandelta) ranging from 0.1 to 0.7. The annulus material also became stiffer and more dissipative with degenerative grade; however, this was not statistically significant. The results indicated that nonlinearities, compression/shear coupling, intrinsic viscoelasticity, and, to a lesser degree, degeneration all affect the shear material behavior of the annulus fibrosus, with important implications for load-carriage mechanisms in the intervertebral disc. These material complexities should be considered when choosing material constants for finite element models.
Unveiling interactions between intervertebral disc morphologies and mechanical behavior through personalized finite element modeling

Abstract:Introduction: Intervertebral Disc (IVD) Degeneration (IDD) is a significant health concern, potentially influenced by mechanotransduction. However, the relationship between the IVD phenotypes and mechanical behavior has not been thoroughly explored in local morphologies where IDD originates. This work unveils the interplays among morphological and mechanical features potentially relevant to IDD through Abaqus UMAT simulations. Methods: A groundbreaking automated method is introduced to transform a calibrated, structured IVD finite element (FE) model into 169 patient-personalized (PP) models through a mesh morphing process. Our approach accurately replicates the real shapes of the patient's Annulus Fibrosus (AF) and Nucleus Pulposus (NP) while maintaining the same topology for all models. Using segmented magnetic resonance images from the former project MySpine , 169 models with structured hexahedral meshes were created employing the Bayesian Coherent Point Drift++ technique, generating a unique cohort of PP FE models under the Disc4All initiative. Machine learning methods, including Linear Regression, Support Vector Regression, and eXtreme Gradient Boosting Regression, were used to explore correlations between IVD morphology and mechanics. Results: We achieved PP models with AF and NP similarity scores of 92.06\% and 92.10\% compared to the segmented images. The models maintained good quality and integrity of the mesh. The cartilage endplate (CEP) shape was represented at the IVD-vertebra interfaces, ensuring personalized meshes. Validation of the constitutive model against literature data showed a minor relative error of 5.20%. Discussion: Analysis revealed the influential impact of local morphologies on indirect mechanotransduction responses, highlighting the roles of heights, sagittal areas, and volumes. While the maximum principal stress was influenced by morphologies such as heights, the disc's ellipticity influenced the minimum principal stress. Results suggest the CEPs are not influenced by their local morphologies but by those of the AF and NP. The generated free-access repository of individual disc characteristics is anticipated to be a valuable resource for the scientific community with a broad application spectrum.
