Abstract:Caesarean scar defect (CSD) can cause postmenstrual bleeding. Defect repair is an effective technique to improve this symptom, but there are still a few patients getting little improvement. This retrospective study evaluates the efficacy of scar repair and explores the factors associated with poor effect. In total, 123 patients were involved in the final analysis. All of them complained about menstruation period >7 days due to postmenstrual bleeding. Before surgery, 87.8% of patients had a menstruation period more than 10days and 20.3% had a period more than 15 days. After surgery, a normal menstruation period (< =7 days) was achieved in 46.3% (95%CI 37.3%-55.6%) of patients and a menstruation period lasting no more than 10 days was achieved in 74.8% (95%CI 66.2%-82.2%). Through multivariate logistic analysis, four factors were found dependently associated with poor effect (defined as menstruation period >10days after surgery): repeated caesarean section (OR 9.75, 95%CI 2.30-41.36, 0.002) was a risk factor, while defect volume >600 mm(3) (OR 0.14, 95%CI 0.03-0.56, 0.006), interval from caesarean section to symptom emerging >3 months (OR 0.25, 95%CI 0.07-0.94, 0.041) and straight or retroflexed uterus (OR 0.19, 95%CI 0.05-0.79, 0.022) were protective factors. IMPACT STATEMENT What is already known on this subject? Caesarean scar defect can cause postmenstrual bleeding. Defect repair can improve this symptom, but there are still a few patients getting little improvement after surgery. What do the results of this study add? Defect volume >600 mm3, interval from caesarean section to symptom emerging >3 months and straight or retroflexed uterus are protective factors of poor effect (defined as menstruation period >10 days after surgery), and repeated caesarean section is a risk factor. What are the implications of these findings for clinical practice and/or further research? These findings may help in counselling the patients and in medical decision. Further researches are needed to explore other factors associated with surgical effect and build prediction models.

Abstract:Postpartum hemorrhage (PPH) is one of the important risk factors leading to maternal mortality and intervention is essential. Oxytocin therapy is widely used clinically, but the effect is unsatisfactory. The efficacy of tranexamic acid (TXA) in hemostasis is notable, whereas its use in preventing PPH warrants exploration.To evaluate the effect of prophylactic administration of TXA on perioperative blood loss in women undergoing cesarean section by systematic review and meta-analysis of published studies.Bibliographic databases were screened from their inception to December 2022 to retrieve relevant studies. Study outcomes including blood loss during cesarean section, 2-h postpartum blood loss, total blood loss (during cesarean section and 2-h postpartum), and 6-h postpartum, as well as hemoglobin changes were extracted and compared.A total of 21 studies, nine randomized clinical trials and 12 cohort studies, involving 1896 patients given TXA prophylactically and 1909 patients given placebo or no treatment, were analyzed. Compared with the control group, the preoperative prophylactic intravenous administration of TXA significantly reduced the intraoperative (RCT: P < 0.00001, cohort studies: P < 0.00001), 2-h postpartum (RCT: P = 0.02, cohort studies: P < 0.00001) and total blood loss (RCT: P < 0.00001, cohort studies: P = 0.0002), and reduced the decline in hemoglobin (RCT: P < 0.00001, cohort studies: P = 0.0001), but did not significantly affect blood loss at 6-h postpartum (P = 0.05).Prophylactic intravenous TXA before cesarean section is helpful in preventing perioperative bleeding in women.http://www.crd.york.ac.uk/PROSPERO , identifier: CRD 42022363450.

Abstract:Aim The increasing incidence of cesarean sections has led to a higher prevalence of cesarean scar defects (CSDs), predominantly characterized by postmenstrual bleeding in affected women. CSD manifests in various forms, including isolated myometrial defects, intrauterine cystic protrusions, and extrauterine cystic extensions. The pathophysiological mechanisms underpinning CSD and its associated symptoms remain insufficiently understood. This review explores the pathogenesis of CSD, highlights its morphological characteristics with an emphasis on variable clinical diversity, and addresses the challenges for future research. Methods A comprehensive narrative review was conducted using electronic databases, including PubMed and Google Scholar, to identify 41 relevant literatures published up to December 31, 2023. Results The myometrium comprises two distinct layers—the inner and outer myometrium—with differences in their origin, structure, and function. Disruptions within these layers contribute to CSD development. Histopathologically, symptomatic CSD may be linked to uterine scar endometriosis, cystic adenomyosis, or endometrial cysts, suggesting that CSD, particularly those necessitating surgical intervention, are not limited to myometrial defects but may also involve iatrogenic endometriosis or adenomyosis, thereby exacerbating clinical symptoms. Conclusion This review provides an updated understanding of the histopathological features and classification of CSD, with an emphasis on elucidating its underlying pathogenesis.

Risk factors for massive hemorrhage during the treatment of cesarean scar pregnancy: a systematic review and meta-analysis

Abstract:Purpose Cesarean scar pregnancy (CSP) is one of the serious complications associated with cesarean delivery (CD). This meta-analysis aims to identify risk factors associated with massive hemorrhage during the CSP treatment. Methods Eight electronic databases were searched for case–control studies published before December 31th, 2018, which compared the possible factors causing massive bleeding during the CSP treatment. Quantitative synthesis was performed by RevMan 5.3. Sensitivity analysis and publication bias were performed by Stata 12.0. Results Total 20 case − control studies including 3101 CSP patients with previous CD met the inclusion criteria. Bleeding group had 573 patients and the control group had 2528 patients. The risk factors for massive bleeding during CSP treatment included multiple gravidities (MD = 0.15, 95% CI 0.03–0.28, P = 0.73), big maximum diameter of gestation sac (MD = 18.49 mm, 95%CI 15.34–21.65, P < 0.01), high gestational days (MD = 8.98 days, 95% CI 4.12–13.84, P < 0.01), high β-HCG level (MD = 21.39 IU/ml, 95% CI 7.36–35.41, P = 0.03; MD = 3.02 U/ml, 95% CI 0.21–5.84, P < 0.01) and rich blood flow around the lesion (OR = 6.73, 95% CI 3.93–11.51, P = 0.59). While, thick myometrium (MD = − 4.94 mm, 95% CI − 6.12 to − 3.75, P < 0.01) may be protective factor. Conclusions Multiple gravidities, big gestation sac, large gestational days, high serum β-HCG level, abundant blood supply to pregnancy sac and thin myometrium maybe the risk factors for massive bleeding during the CSP treatment.
Risk factors and influence of surgical technique on the risk of caesarean scar defect formation: A systematic review of the literature

Abstract:Objective: To determine the factors associated with an increased risk of cesarean scar defect formation. Methods: A systematic literature search was performed up to July 2022 in PubMed databases following the PRISMA recommendations. All available English-language clinical studies presenting one or more factors that may affect the risk of cesarean scar defect were included. Results: 39 studies meeting the selection criteria were identified. An association was found between the number of previous cesarean sections and a significant increase in the risk of cesarean scar defect formation. Regarding patient age, gestational age at cesarean section, birth weight and emergency context did not appear to influence the risk of cesarean scar defect. However, cesarean sections performed during labor advanced stages of labor, may increase the risk. The data remain too limited to conclude on the impact of BMI, flexion uterine, and pregnancy pathologies (gestational diabetes, preeclampsia, premature rupture of membranes), the use of oxytocic, or infectious and hemorrhagic complications. Regarding the surgical technique, the literature suggested that a lower hysterotomy is associated with an increased risk of scar defect. However, the single- or double-layer suture technique did not provide a change in risk, and the data were too limited to conclude on the impact of the type of thread or suture used. Conclusion: The present systematic review of the literature suggests that several factors may increase the risk of developing a cesarean scar defect, such as the number of previous cesarean sections, a cesarean section performed during advanced labor, and a lower hysterotomy. However, the current state of the literature does not allow definitive conclusions to be drawn on most other factors.

Abstract:Objectives: To determine the risk factors for development of caesarean scar defect (CSD), compare the efficacy of transvaginal ultrasound (TVS) and magnetic resonance imaging (MRI) for CSD assessment, and investigate the association between CSD size and clinical symptoms. Study design: One hundred and eighty-nine women with CSD and 378 women without CSD with a history of caesarean section (CS) at the Obsterics and Gynaecology Hospital of Fudan University between January 2008 and February 2016 were enrolled. The potential risk factors for CSD were investigated using multivariate logistic regression analysis. TVS and MRI were performed for CSD measurements, including residual myometrial thickness, and depth, length and width of CSD. Associations between CSD size by TVS/MRI and symptoms were evaluated. Results: CS time >= 85 min, peripartum fever or infection, and retroflexed uterus were risk factors for CSD, and age at last CS < 30 years, intraoperative blood loss <150 ml and double-layer closure were protective factors for CSD. Prolonged menstruation, dysmenorrhoea, chronic pelvic pain and infertility were the main clinical manifestations. Women with a larger CSD presented with more prolonged menstruation. Compared with TVS, measurements by MRI showed better prediction of the clinical symptoms of CSD. Conclusions: Various factors contribute to the development of CSD. Prevention of peripartum infection, reduction of CS time, reduction of blood loss and more careful uterine closure are needed to decrease the risk of developing CSD. MRI is a reliable method for the diagnosis and measurement of CSD, and can be utilized in clinical practice. (C) 2019 Elsevier B.V. All rights reserved.

Abstract:PurposeThe surgical technique for uterine closure following cesarean section influences the healing of the cesarean scar; however, there is still no consensus on the optimal technique regarding the closure of the endometrium layer. The aim of this study was to compare the effect of closure versus non-closure of the endometrium during cesarean section on the risk to develop uterine scar defect and associated symptoms.MethodsA randomized prospective study was conducted of women undergoing first elective cesarean section at a single tertiary medical center. Exclusion criteria included previous uterine scar, preterm delivery and dysmorphic uterus. Women were randomized for endometrial layer closure versus non-closure. Six months following surgery, women were invited to the ambulatory gynecological clinic for follow-up visit. 2-D transvaginal ultrasound examination was performed to evaluate the cesarean scar characteristics. In addition, women were evaluated for symptoms that might be associated with uterine scar defect. Primary outcome was defined as the residual myometrial thickness (RMT) at the uterine cesarean scar. Data are presented as median and interquartile range.Results130 women were recruited to the study, of them follow-up was achieved in 113 (86.9%). 61 (54%) vs. 52 (46%) of the women were included in the endometrial closure vs. non-closure groups, respectively. Groups were comparable for patient's demographic, clinical characteristics and follow-up time for postoperative evaluation. Median RMT was 5.3 (3.0–7.7) vs. 4.6 (3.0–6.5) mm for the endometrial closure and non-closure groups, respectively (p = 0.38). Substantially low RMT (< 2.5 mm) was measured in four (6.6%) women in the endometrial closure group and three (5.8%) of the women in the non-closure group (p = 0.86). All other uterine scar sonographic measurements, as well as dysmenorrhea, pelvic pain and intermenstrual bleeding rates were comparable between the groups.ConclusionClosure versus non-closure of the endometrial layer during cesarean uterine incision repair has no significant difference in cesarean scar characteristics and symptom rates at 6 months follow-up.

Abstract:Objective: To develop a model to predict the risk of postpartum hemorrhage (PPH) following cesarean delivery in women with a scarred uterus.Methods: A total of 4,637 pregnant women with scarred uterus who underwent a cesarean delivery at a large hospital between January 2014 and December 2017 were enrolled. The women were divided into PPH (n = 287) and non-PPH (n = 4,350) groups. A model to predict PPH (blood loss >= 1,000 mL within 24 h following cesarean delivery) was developed using multivariate logistic regression analysis. Receiver operating characteristic curve was drawn, and the area under curve (AUC) was calculated.Results: The incidence of PPH was 6.19% (287 of 4,637 women). Seven independent risk factors were associated with PPH: maternal age (odds ratio [OR] = 1.42, 95% confidence interval [CI]: 1.02-1.97), previous gravidity (OR = 1.24, 95% CI: 1.01-1.50), placental location (posterior wall of uterus, OR = 0.69, 95% CI: 0.47-1.02; other locations, OR = 1.21, 95% CI: 0.81-1.80), placenta previa (incomplete placenta previa, OR = 10.51, 95% CI: 5.99-18.42; complete placenta previa, OR = 31.65, 95% CI: 21.07-47.54), placenta accreta (OR = 6.39, 95% CI: 4.02-10.16), hypertensive disorders of pregnancy (OR = 2.27, 95% CI: 1.40-3.68), and fetal position (breech position, OR = 2.07, 95% CI: 1.19-3.60; transverse position, OR = 1.07, 95% CI: 0.48-2.41). A predictive model with AUC of 0.89 was developed (95% CI: 0.86-0.91, P < 0.001).Conclusions: A model was developed to predict PPH following cesarean delivery in women with a scarred uterus.

Abstract:(1) Background: A cesarean scar defect may cause localized inflammation of the endometrial tissue, and various researchers believe that the presence of a cesarean scar defect is associated with chronic endometritis. However, there is no report on the possible association between cesarean scar defects and chronic endometritis thus far. This study aimed to assess the role of having a cesarean scar defect in a person's susceptibility to chronic endometritis. (2) Methods: This retrospective propensity-score-matched study comprised 1411 patients with chronic endometritis that were admitted to Henan Provincial People's Hospital in China from 2020 to 2022. Based on whether a cesarean scar defect was present or not, all cases were assigned to the cesarean scar defect group or the control group. (3) Results: Of the 1411 patients, 331 patients with a cesarean scar defect were matched to 170 controls. All unbalanced covariates between groups were balanced after matching. Before matching, the prevalence of chronic endometritis in the cesarean scar defect group and in the control group was 28.8% and 19.6%, respectively. After correcting for all confounding factors, a logistic regression analysis showed that cesarean scar defect occurrence may increase the risk of chronic endometritis (odds ratio (OR), 1.766; 95% confidence interval (CI), 1.217–2.563; p = 0.003). After matching, the prevalence of chronic endometritis was 28.8% in the cesarean scar defect group and 20.5% in the control group. Thus, even after correcting for all confounding factors, the logistic regression analysis still showed that a cesarean scar defect remained an independent risk factor for chronic endometritis prevalence (OR, 1.571; 95% CI, 1.021–2.418; p = 0.040). The findings were consistent throughout the sensitivity analyses. (4) Conclusions: The present results suggest that the onset of a cesarean scar defect may increase the risk of chronic endometritis.

Abstract:The rate of caesarean section (CS) is increasing worldwide. Defects in uterine healing have a major gynaecological and obstetric impact (uterine rupture, caesarean scar defect, caesarean scar pregnancy, placenta accreta spectrum). The complex process of cellular uterine healing after surgery, and specifically after CS, remains poorly understood in contrast to skin wound healing. This literature review on uterine wound healing was mainly based on histological observations, particularly after CS. The primary objective of the review was to examine the effects of CS on uterine tissue at the cellular level, based on histological observations. The secondary objectives were to describe the biomechanical characteristics and the therapies used to improve scar tissue after CS. This review was performed using PRISMA criteria, and PubMed was the data source. The study included all clinical and animal model studies with CS and histological analysis of the uterine scar area (macroscopic, microscopic, immunohistochemical and biomechanical). Twenty studies were included: 10 human and 10 animal models. In total, 533 female humans and 511 female animals were included. Review articles, meeting abstracts, case series, case reports, and abstracts without access to full-text were excluded. The search was limited to studies published in English. No correlation was found between cutaneous and uterine healing. The histology of uterine scars is characterized by disorganized smooth muscle, fibrosis with collagen fibres and fewer endometrial glands. As for skin healing, the initial inflammation phase and mediation of some growth factors (particularly connective tissue growth factor, vascular endothelial growth factor, platelet-derived growth factor, tumour necrosis factor α and tumour necrosis factor β) seem to be essential. This initial phase has an impact on the subsequent phases of proliferation and maturation. Collagen appears to play a key role in the initial granulation tissue to replace the loss of substance. Subsequent maturation of the scar tissue is essential, with a decrease in collagen and smooth muscle restoration. Unlike skin, the glandular structure of uterine tissue could be responsible for the relatively high incidence of healing defects. Uterine scar defects after CS are characterized by an atrophic disorganized endometrium with atypia and a fibroblastic highly collagenic stromal reaction. Concerning immunohistochemistry, one study found a decrease in tumour necrosis factor β in uterine scar defects. No correlation was found between biomechanical characteristics (particularly uterine strength) and the presence of a collagenous scar after CS. Based on the findings of this review, an illustration of current understanding about uterine healing is provided. There is currently no validated prevention of caesarean scar defects. Various treatments to improve uterine healing after CS have been tested, and appeared to have good efficacy in animal studies: alpha lipoic acid, growth factors, collagen scaffolds and mesenchymal stem cells. Further prospective studies are needed.

Endoscopic Treatment of Previous Cesarean Scar Defect in Women with Postmenstrual Bleeding: A Retrospective Cohort Study

Abstract:Objective: To compare the incidence of postmenstrual bleeding after hysteroscopic resection versus laparoscopic repair of previous cesarean scar defect (PCSD).Materials and methods: Retrospective analysis of computerized patient records. For the diagnosis of PCSD, patients underwent transvaginal ultrasound first without and then with saline-assisted sonohysterography. Hysteroscopic PCSD resection was performed under sonographic guidance, while laparoscopic repair was guided by hysteroscopy for the confirmation of scar margins.Results: Records of 62 patients presenting with PCSD-related postmenstrual bleeding were included in analysis. Hysteroscopic surgery had significantly shorter operative time compared to the laparoscopic approach (Mean =30.9 vs 71.0 minutes; p < 0.001). Blood loss and hospital stay were significantly less (p < 0.001) in hysteroscopic resection (10.4 ± 4.6 ml and 2.1 ± 0.4 days) than in laparoscopic repair (36.6 ± 4 ml, and 4.6 ± 1 days). After surgical interventions, the postmenstrual bleeding was resolved or improved. The effectiveness rates of hysteroscopic resection and laparoscopic repair were 91.4% and 96.3%, respectively. Incidence of post-treatment postmenstrual bleeding was not significantly different between hysteroscopy and laparoscopy (OR= 1.29 [95% confidence interval 0.367, 4.86]; p = 0.662). Pretreatment postmenstrual bleeding was associated with time since cesarean section (B= −0.091 [−0.158, −0.023]; p = 0.01) and PCSD length (B = 0.502 [0.085, 0.919]; p = 0.019).Conclusion: Both hysteroscopic resection and laparoscopic repair of PCSD yield comparable efficacy in reducing postmenstrual bleeding. However, hysteoroscopic resection of PCSD is associated with comparatively shorter operative time, less blood loss, and shorter hospital stay.

Abstract:Background: Cesarean sections are increasingly likely to be applied; however, uterine scar defects (USD) often remain after delivery.The two existing treatment methods, hysteroscopic electric resection and vaginal surgery, are still controversial in terms of efficacy and safety.So, this paper to compares the effectiveness and safety of hysteroscopic electric resection and vaginal surgery in the treatment of USD after cesarean section.Methods: We performed a related literature search from main databases.According to the PICOS principles inclusion criteria were adult female USD patients to evaluate the efficacy of hysteroscopic resection and vaginal surgery for the treatment of USD, outcome data could be extracted to compare the efficacy and safety of the two procedures.Subsequently, according to the titles, abstracts, and full texts of the retrieved articles, studies that did not meet the inclusion criteria were eliminated.The RevMan 5.20 software was used for meta-analysis and Cochrane Risk of Bias 2 (RoB 2.0) was used to assess the risk of bias.The effectiveness and safety of hysteroscopic resection and vaginal surgery in the treatment of USD patients after cesarean section were compared.Results: Eight articles were finally included, with a total of 191 patients in the hysteroscopic electric resection group and 212 patients in the vaginal surgery group.Compared with hysteroscopic resection and vaginal surgery, there is less intraoperative blood loss [mean difference (MD) is -25.23,P<0.00001], shorter operation time (MD is -29.45,P<0.00001), and shorter hospital stay (MD is -1.87,P<0.00001), but menstrual improvement risk ratio (RR) is 0.71 (P=0.51) and diverticulum recovery RR is 0.60 (P=0.43)there were no significant differences.Discussion: Hysteroscopic electric resection provides a more satisfactory outcome than vaginal surgery in terms of intraoperative blood loss, operation time, and hospital stay.However, the sample size of the study was not large enough and some studies had high risk of bias, more large-sample multi-center high quality studies are needed for further comprehensive comparative analysis.

Abstract:Approximately 60% of patients undergoing Cesarean sections may develop Cesarean Scar Defect (CSD), presenting a significant clinical challenge amidst the increasing Cesarean section rates. This condition, marked by a notch in the anterior uterine wall, has evolved as a notable topic in gynecological research. The multifactorial origins of CSD can be broadly classified into labor-related factors, patients' physical conditions, and surgical quality. However, conflicting influences of certain factors across studies make it challenging to determine effective preventive strategies. Additionally, CSD manifests with diverse symptoms, such as abnormal uterine bleeding, dysmenorrhea, chronic pelvic pain, dyspareunia, secondary infertility, and Cesarean scar pregnancy. Some symptoms are often attributed to other diagnoses, leading to delayed treatment. The quandary of when and how to manage CSD also adds to the complexity. Despite the development of various therapies, clear indications and optimal methods for specific conditions remain elusive. This longstanding challenge has troubled clinicians in both identifying and addressing this iatrogenic disease. Recent studies have yielded some compelling consensuses on various aspects of CSD. This review aims to consolidate the current literature on every facet of CSD. We hope to raise awareness among clinicians about this clinical problem, encouraging more relevant research to unveil the complete picture of CSD.
Hysteroscopic examination and laparoscopic repair of uterine scar defect after cesarean section

Abstract:Objective: To explore the possible pathogenesis of uterine scar defect after cesarean section, and to investigate the effect of laparoscopic surgical repair on the defect. Methods: Fourteen patients of uterine scar defect after cesarean section who underwent hysteroscopic examination and laparoscopic repair in Obstetrics and Gynecology Hospital of Fudan University since Oct., 2009 were reviewed retrospectively and followed. Results: All the patients had a prolonged period, and the duration was (14.0±4.4) days (8-25 days), 4 cases had a history of infertility, and 4 cases complained of abdominal pain during menstruation. Hysterosalpingography (HSG) showed the width of the defect was (19.3±5.0) mm (13-30 mm), and the depth was (11.6±3.2) mm (5-19 mm), both significantly larger than thosed measured by trans-vaginal ultrasonic examination. The indications of cesarean section were uterine scar (previous section, 6 cases), premature rupture of fetal membranes (PROM, 3 cases), dystocia (3 cases), social factor (2 cases) and fetal distress (3 cases). In the previous cesarean section, the uterine was sutured by 1 layer of absorbable material in 7 cases, 2 layers in 4 cases, and unknown in the other 3 cases. Repair of the uterine defect was successfully achieved in all 14 patients. There was no intra-operative complications and none of the patients required blood transfusion. The mean operation time was (130±47) min (75-225 min), and the mean blood loss was (102±79) mL (30-300 mL). The cases was followed for an average of 4.1 months (2-12 months). In the 12 patients who had a recovery of menstruation, 11 patients had an obvious shortened period, and the duration was (8.6±3.2) days (6-10 days), and the clinic effective rate was 91.7% (11/12). Nine patients had trans-vaginal ultrasonic examination, and small defect was found in 2 patients (6 mm×2 mm and 7 mm×5 mm, respectively). Conclusions: HSG is better than transvaginal sonography to evaluate the defect before the surgery. Multiple cesarean section and PROM are the possible high risk factors of uterine scar defect after cesarean section. Laparoscopic surgical repair of the defect may be performed with satisfying effective rate.

Association Between First Caesarean Delivery and Adverse Outcomes in Subsequent Pregnancy: a Retrospective Cohort Study

Abstract:Background: Few studies have explored the association between a previous caesarean section (CS) and adverse perinatal outcomes in a subsequent pregnancy, especially in women who underwent a non-indicated CS in their first delivery. We designed this study to compare the perinatal outcomes of a subsequent pregnancy in women who underwent spontaneous vaginal delivery (SVD) or CS in their first delivery. Methods: This retrospective cohort study included women who underwent singleton deliveries at the International Peace Maternity and Child Health Hospital from January 2013 to December 2016. Data on the perinatal outcomes of all the women were extracted from the medical records. Multivariate logistic regression was conducted to assessed the association between CS in the first delivery and adverse perinatal outcomes in the subsequent pregnancy. Results: CS delivery in the subsequent pregnancy was more likely for women who underwent CS in their first birth than for women with previous SVD (97.3% versus 13.2%). CS in the first birth was also associated with a significantly increased risk of adverse outcomes in the subsequent pregnancy, especially in women who underwent a nonindicated CS. Adverse perinatal outcomes included pregnancy-induced hypertension [adjusted odds ratio (OR), 95% confidence interval (CI): 2.20, 1.59-3.05], gestational diabetes mellitus (1.82, 1.57-2.11), gestational anaemia (1.27, 1. 05-1.55), placenta previa (3.18, 2.15-4.71), placenta accreta (2.75, 1.75-4.31), and polyhydramnios (2.60, 1.57-4.31) in the mother and preterm delivery (1.37, 1.06-1.78), low birth weight (3.78, 2.07-6.90), macrosomia (5.04, 3.95-6.44), and neonatal jaundice (1.72, 1.39-2.14) in the baby. Conclusions: CS in the first delivery markedly increases the risk of repeated CS and maternal-fetal complications in the subsequent pregnancy, especially in women with a non-indicated CS.

Abstract:Surgical site infection (SSI) is one of the common postoperative complications after caesarean section for pregnant women. Previous studies have investigated the risk factors for SSI in pregnant women undergoing caesarean delivery. Whereas big differences in research results exist, and the correlation coefficients of different research results are quite different. A meta‐analysis was conducted to examine the risk factors related to SSI in pregnant women undergoing caesarean delivery. We searched English databases to collect case–control studies or cohort studies published between 1 January 2015 and 15 November 2023, including PubMed, Web of Science and ScienceDirect. The risk of bias of the included studies was assessed via Newcastle‐Ottawa Scale. The analysis was performed using RevMan 5.4.1 tool. A total of 24 articles (n = 581, 895) were selected in this meta‐analysis. The following risk factors were presented to be significantly correlated with SSI in pregnant women following caesarean delivery: smoking (odds ratio [OR] = 1.64, 95% confidence interval [CI] [1.31, 2.04]), previous caesarean section (OR = 1.46, 95% CI [1.18, 1.82]), multiple vaginal examinations (OR = 2.92, 95% CI [1.91, 4.46]), membrane rupture (OR = 1.68, 95% CI [1.19, 2.38]), hypertensive disorders (OR = 1.85, 95% CI [1.33, 2.57]), diabetes mellitus (OR = 1.36, 95% CI [1.18, 1.57]), high body mass index (OR = 1.57, 95% CI [1.35, 1.84]). Occurrence of SSI is influenced by a variety of factors. Thus, we should pay close attention to high‐risk subjects and take crucial targeted interventions to lower the SSI risk after caesarean section. Owing to the limited quality and quantity of the included studies, more rigorous studies with adequate sample sizes are needed to verify the conclusion.

Abstract:To evaluate the relationship between uterine cesarean scar diverticulum (CSD) and subsequent infertility in patients who underwent cesarean section, and determine the effects of pelvic fluid-releasing inflammations on infertility.A retrospective analysis was designed among patients with CSD who were admitted to our hospital from January 1, 2018 to December 31, 2019. A total of 60 patients with CSD and uterine fibroids or benign ovarian tumors who underwent cesarean section were included, and divided into the CSD group and control group. Baseline characteristics of all patients were collected, and the pelvic adhesion scores and the percents of tubal patency were evaluated. Furthermore, the postoperative clinical outcomes were followed up. The levels of inflammatory factors in pelvic fluid were tested using Elisa kits.Preoperative data indicated that the size of the uterine scar diverticulum was (1.68 ± 0.52) cm, the pelvic adhesion scores were higher in CSD group than control group (4.67 ± 0.90 vs 0.47 ± 0.90, P < .05), and 21 of 30 patients with unobstructed fallopian tubes. The levels of tumor necrosis factor-α, interleukin-1β, and interleukin-6 in patients with CSD were obviously higher than control group (P < .05). After the follow-up, the data displayed that no CSD was found in all patients, the time of menstrual period in patients with CSD was shortened to 7.80 ± 1.27 days, and the myometrial thickness at uterine scar was significantly increased (P < .05). Additionally, the pregnancy rate was increased, and 12 of 30 patients were repregnant. Correlation analysis showed that the levels of inflammatory factors (tumor necrosis factor-α, interleukin-1β, interleukin-6), the size of uterine scar diverticulum, and the myometrial thickness at uterine scar were significantly correlated with subsequent infertility (r = 0.307, 0.083, 0.147, 0.405, 0.291, P < .05).Uterine scar diverticulum repair could improve menstrual prolongation, increased the thickness of myometrium and repregnant rate. Subsequent infertility was positively correlated with uterine scar diverticulum and the levels of inflammatory factors.
