Abstract:This study aimed to compare the effect of a bioceramic sealer (iRoot SP) and a resin-based sealer (AH Plus) on the outcome of root canal treatment in a 2-year follow-up. Seventy-six teeth with irreversibly or necrotic pulp were recruited. After instrumentation and disinfection, the root canals were obturated using warm vertical compaction with iRoot SP (n = 43) or AH Plus (n = 33). Patients were followed up by clinical and radiographic examination at 6 12 and 24 months with recall rates of 84.2%, 65.8% and 48.7%, respectively. During each recall session, the success rates were 80%, 85.2% and 85% in the iRoot SP group and 82.8%, 91.3% and 88.2% in the AH Plus group. The success rates of the two groups did not differ significantly (p > 0.05). The bioceramic sealer resulted in a similar clinical performance and success rate to the resin-based sealer in endodontic treatment during a 2-year follow-up.
In Vitro Study of Dentinal Tubule Penetration and Filling Quality of Bioceramic Sealer

Abstract:This study aimed to evaluate the dentinal tubule penetration and filling quality of a bioceramic sealer (iRoot SP). Forty-two roots of extracted adult lower incisors were selected. After instrumentation with Protaper Universal to F3, 40 roots were chosen and randomly divided into 4 groups, as follows: iRoot SP single cone group, iRoot SP warm vertical group, AH Plus single cone group, and AH Plus warm vertical group. Before root canal filling, sealers were mixed with Rhodamine B dye for visualization under confocal laser scanning microscope. All samples were sectioned at 2, 4, and 6 mm to apex. Then, the percentages of void areas, gap regions, and segments of sealer that penetrated into dentinal tubules in each section were calculated. Non-parametric test was used for statistical analysis (α = 0.05). We found that filling techniques and types of sealer had no statistically significant effects on the occurrence of voids and gaps. The segments of iRoot SP penetrated into dentinal tubules were statistically more than that of AH Plus in both single cone and warm vertical techniques at 2 mm to apex (P < 0.05). Regardless of the filling technique used, iRoot SP can achieve comparable filling quality and better dentinal tubules penetration than AH Plus. Considering the good bioactivity of iRoot SP, it may help improve the seal of root canal system.

Abstract:Background/purpose: The root fracture resistance of endodontically treated teeth is decreased significantly, and it is more likely to fracture. This study aimed to evaluate the ef-fect of a novel root canal sealer based on bioactive glass (BG) on root fracture resistance and explore its mechanism. Materials and methods: The BG-based root canal sealer (BG Sealer) was prepared by mixing a kind of bioactive glass (10.8% P2O5, 54.2% SiO2, 35% CaO, mol.%, named PSC), zirconia (ZrO2) powder, sodium alginate (SA) and phosphate solution (PS). A pH meter was used to measure the pH of simulated body fluid (SBF) after immersion with BG Sealer at different time. After preparing the samples of BG sealer with a diameter of 4 mm and a height of 6 mm, the compressive strength was tested by a universal testing machine. The scanning electron micro-scopy (SEM) and energy dispersive X-ray spectroscopy (EDS) were used to detect and analyze the mineral status of root canal systems filled with BG Sealer. The push out test was used to measure the push out bond strength of BG Sealer. The fracture resistance of root canals filled with BG Sealer was detected by the compressive loading test. Bioceramic root canal sealer iR-oot SP was set as the control group. Results: (1) Physicochemical properties: The pH value of SBF immersed with BG Sealer increased slightly up to 7.68, while the pH of SBF immersed with iRoot SP increased to 12.08. The compressive strength of the novel BG Sealer was 4.62 +/- 1.70 MPa, which was lower than that of iRoot SP (P < 0.05). Abstract:PURPOSE: This study evaluated the dentin bonding strength and biomineralization effect of a recently developed premixed calcium aluminate-based endodontic sealer (Dia-Root Bio Sealer) in comparison with existing calcium silicate-based sealers.METHODS: The root canals of 80 mandibular premolars were filled with Dia-Root Bio Sealer, Endoseal MTA, EndoSequence BC Sealer, and AH Plus Bioceramic Sealer. Medial and apical specimens were then obtained by sectioning. The push-out bond strength was measured using the medial specimens, and the failure mode was recorded. Intratubular biomineralization in the apical specimens was analyzed using scanning electron microscopy and energy-dispersive X-ray spectroscopy (EDS). The data were analyzed using one-way analysis of variance followed by the Tukey test (P < 0.05).RESULTS: The push-out bond strength of Dia-Root Bio Sealer was significantly higher than that of the other tested materials, and a cohesive failure pattern was observed in all groups. Dia-Root Bio Sealer also exhibited a significantly higher degree of biomineralization than the other groups, and EDS analysis indicated that the biomineralized precipitates were amorphous calcium phosphate.CONCLUSION: The results of this study indicate that Dia-Root Bio Sealer has the potential to be used as an adequate root canal sealer due to its favorable bonding performance.

Abstract:The adaptation of a sealer along with the periphery of the dentinal tubules of the root canal is studied. Various techniques have been used for the application of these sealers onto the canal wall for better adaptation but have not been compared to date. The purpose of the study was to comparatively evaluate the sealing ability of a bioceramic sealer with AH plus sealer with root canal dentin using three different techniques for the application of sealer. One hundred twenty extracted maxillary and mandibular anterior teeth were collected, disinfected, and decoronated at the cemento-enamel junction to maintain a standard working length for all samples. The establishment of the working length (40.10) and instrumentation was performed using a rotary instrument, along with a standard irrigation regimen. The teeth were then divided into two main groups according to the sealer used, i.e., Group A (AH Plus) and Group B (iRoot SP). These two main groups were categorized into three sub-groups depending on the technique of sealer placement, i.e., Subgroup 1 (master cone gutta-percha), Subgroup 2 (bidirectional spiral), and Subgroup 3 (passive ultrasonic activation). Out of the 20 samples, 15 samples were randomly allocated for the assessment of sealing ability using the routine dye extraction method, and to verify the results of the dye extraction method, a more advanced evaluation method, i.e., SEM evaluation, was utilized further. To this end, five random samples from each subgroup were allocated for SEM analysis. The obtained scores were then statistically analyzed using an ANOVA test and Post Hoc Tukey’s test. In the current study, statistical significance was seen among the three main groups and six subgroups with p-values < 0.005. Subgroup B3 performed significantly better than the other subgroups in both the dye extraction method as well as in SEM analysis. The highest microleakage was shown by subgroup A1; it also exhibited poor penetration of sealer in SEM evaluation. The bioceramic sealer (iRoot SP), when applied using passive ultrasonic activation, showed the best results in both the dye extraction method and the SEM evaluation.

Abstract:Introduction: The aim of the study was to evaluate the bacterial leakage of four different bioceramic root canal sealers and to compare it to the conventional epoxy resin-based sealer. Materials and Methods: The study sample consisted of 94 single-rooted human teeth with one round root canal, which were instrumented and randomly distributed into five experimental groups according to the root canal sealer: Group 1: TotalFill bioceramic sealer; Group 2: BioRoot root canal sealer; Group 3: mineral trioxide Aggregate (MTA) Fillapex; Group 4: MTA Plus; Group 5: epoxy resin-based sealer AH Plus and positive and negative control group. The prepared samples were fixed in a model designed for the Enterococcus faecalis leakage evaluation and were sterilized in plasma. The bacterial penetration through the filled root canals was checked by the color change in the lower tube during 60 days. The scanning electron microscopy was used to check the bond between the sealer and the dentine wall. Data were statistically analyzed. Results: Total Fill bioceramic sealer (BCS) group presented significantly lower number of samples that leaked (14.2%) compared to the AH Plus group (78.6%) ( P = 0.002) and MTA Plus group (50%) ( P = 0.046). There was no statistically significant difference between other groups ( P > 0.05). Conclusion: The TotalFill BCS root canal sealer provided better sealing ability compared to the AH Plus and the MTA Plus sealers.

Abstract:PURPOSE:To investigate the sealing ability of biological root canal sealer iRoot SP in over-root canal filling. METHODS：After completion of root canal preparation, 48 extracted human single-canal teeth were used. The ratio of mesio-distal root canal diameter to bucco-lingual diameter was less than 0.5. The teeth were randomly divided into group A, B and C. The root canals were obturated with root canal filling agent (group A), AH Plus (group B) and iRoot SP (group C), respectively; the apical leakage was evaluated by dye penetration method and transparent teeth technique. The data were analyzed using SPSS 19.0 software package. RESULTS:The depth of dye penetration in group A was significantly higher than that in group B and C (P<0.05); there was no significant difference in the depth of dye penetration between group B and group C (P>0.05). CONCLUSIONS:The apical sealing ability of iRoot SP is equivalent to AH Plus in oval-root canal, both are better than that of root canal filling agent．.
Physicochemical Properties of a Novel Bioceramic Silicone-Based Root Canal Sealer

Abstract:Background/purpose: With introduction into endodontics, bioceramic-based sealers have gained considerable popularity for excellent properties. The aim of this study was to investigate the physicochemical properties of a novel bioceramic silicone-based sealer, GuttaFlow Bioseal, and measure heat flow of setting reactions. Materials and methods: Film thickness, flow, working and setting time of Bioseal were compared with other 4 kinds of sealers: iRoot SP, AH Plus, RoekoSeal and GuttaFlow2. Differential scanning calorimetry test was performed to measure heat flow. Results: Bioseal demonstrated the highest film thickness of 44 mm, double to triple that of the other 4 sealers (P < 0.05). The highest flow was detected in iRoot SP and RoekoSeal, with values of 27.35 and 27.20 mm, while GuttaFlow2 and Bioseal had the lowest of 22.31 and 21.43 mm (P < 0.05). For each sealer, working time at 37 degrees C was shorter than that at 23 degrees C (P < 0.05). At 37 degrees C, Bioseal had the shortest working and setting time of 4.5 and 16.3 min, while iRoot SP showed the longest of 105.0 and 571.7 min (P < 0.05). Differential scanning calorimetry test revealed that setting process of all the tested sealers was exothermic. Bioseal reached an exothermic peak at 14 min, with almost 1.5 times peak intensity of GuttaFlow2 and RoekoSeal. Whereas iRoot SP and AH Plus reached an exothermic peak 5 h after mixing, with intensity 1/2 to 2/3 that of Bioseal. Conclusion: The novel bioceramic silicone-based sealer Bioseal showed intense and fast exothermic reactions with characteristic physicochemical properties. Evaluation of Sealer Remnants Using a Bioceramic Sealer Single-cone Technique after Post Space Preparation and Its Influence on the Adhesion of Fibre Posts in Vitro.

Abstract:Objective: To compare calcium silicate-based endodontic sealer and epoxy resin-based sealer remnants on root canal walls after post space preparation and their influence on the bond strength offibre posts fixed with a dual-cured resin cement. Methods: Thirty-six extracted single-root mandibular premolars were instrumented and divided randomly into two equal groups with different endodontic sealers. iRoot SP (Innovative BioCeramix, Vancouver, Canada) was employed in the experimental group and All Plus observed under an endodontic microscope after root canal therapy and post space preparation. Fibre posts were fixed with dual-cured resin cement. Specimens were taken at each third of the post space. The push-out bond strength was measured using a universal testing machine and fracture modes were assessed. Statistical analysis was performed using an independent samples t test and one-way analysis of variance. Results: There was no statistically significant difference in bond strength offibre posts between the control and experimental group (P > 0.05); however, sealer remnants were observed in 38.9% of the samples treated with iRoot SP and none of the samples treated with AH Plus. The major fracture mode in samples treated with iRoot SP was adhesive failure between the resin cement and the post, and no adhesive failure between the resin cement and dentine occurred at the site of the sealer remnant. The presence of iRoot SP remnants on the root canal walls after post space preparation did not interfere with bonding. Conclusion: iRoot SP is a viable option for root canal obturation before fibre post cementation.
The efficacy of premixed bioceramic sealers versus standard sealers on root canal treatment outcome, extrusion rate and post‐obturation pain: A systematic review and meta‐analysis

Abstract:Background Limited evidence is available regarding the superior clinical properties of bioceramic sealers comparted with traditional standard sealers. Objectives This review aimed to answer the following research questions: 'In healthy patients requiring a root canal treatment (P), what is the efficacy of premixed bioceramic sealers (I) compared with traditional root canal epoxy resin‐based sealers (C) in terms of survival, success rates (PICO1) sealer extrusion and resorption (PICO2) post‐obturation pain (PICO3) (O)?' Methods Authors independently searched three electronic databases: PubMed (including MEDLINE), Web of Science, Embase and Scopus up to 31 October 2023. This was accompanied by both grey literature and manual search. Detailed selection criteria were applied, namely mature permanent teeth requiring root canal treatment, premixed bioceramic sealer with gutta‐percha as an intervention group, a standard filling technique as control group and full‐text available in English. A random‐effect meta‐analysis was used to synthesize the body of evidence regarding the use of bioceramic sealers in root canal treatment and their impact on post‐obturation pain. Effect sizes were represented as relative risks on a logarithmic scale for binary outcomes and as mean differences for continuous outcomes. Results A total of 941 articles were identified. Fifteen Comparative clinical studies were finally included. Eleven were randomized clinical trials, and four were prospective clinical trials with control group. The follow‐up of these studies was not greater than 2 years. No publication bias was observed in any study. No significant differences were observed between the two groups in terms of survival and success rates. A small non‐significant lower risk of extrusion was observed for bioceramics. A small, non‐significantly lower post‐operative‐pain within 24‐h was observed when bioceramics were used. Discussion The majority of current evidence shows inconsistencies in reporting and is of short‐term duration. Robust prospective long‐term trials are needed in this area to better support future recommendations. Conclusion This systematic review is the first to analyse several clinical outcomes using premixed sealers. Included studies differed in terms of clinical protocol and operator expertise, but reported a similar outcome when comparing bioceramic versus standard sealers. Tooth survival, treatment outcome, post‐operative pain and periapical extrusion were similar and presented no significant differences between the two sealer types. Abstract:BACKGROUND: This study compared the solubility and the pH of different root canal sealers in vitro.MATERIAL AND METHODS: BioRoot™RCS, TotalFill BC Sealer, MTA Fillapex, SealapexTM, AH Plus, EasySeal, Pulp Canal Sealer™ and N2 were tested. Similar specimens were prepared using ring molds with an internal diameter of 20 ± 0,1 mm and a height of 1,5 ± 0,1 mm and digitally weighted to register the mass of each specimen before and after immersion in distilled water. Solubility was determined after 24 hours and statistically analysed using a one-way ANOVA test and post-hoc Tukey test. The pH value was measured by a digital pH meter after 3 and 24 hours from manipulation.RESULTS: BioRoot™RCS and TotalFill BC Sealer showed significantly higher solubility (P<0.05). All the remnant root canal sealers fulfilled the requirements of solubility of the International Standard Organization 6876 demonstrating a weight loss of less than 3%. BioRoot™RCS and Totalfill BC Sealer exhibited high alkaline pH over time (P<0.05); the alkalinity of the other tested cements was significantly lower.CONCLUSIONS: The prolonged alkalinity of bioceramic sealer matched the increase in solubility. This may encourage their biological and antimicrobial effects, but the ongoing solubility may impact their ability to prevent apical leakage. Key words:pH, root canal sealers, solubility.

Abstract:Obturation with a sealer material that provides an adequate apical sealing ability is required to prevent endodontic treatment failure due to microleakage. However, there are no sealers that meet all the physical and chemical properties to be able to hermetically seal the root canal system to date. Various sealer materials have been developed in recent years including the use of bioceramic materials which are claimed to have excellent biocompatibility to tissues. This study aimed to compare the apical sealing ability of bioceramic-based and zinc oxide eugenol (ZOE)-based sealer in root canal treatment. A total of 27 extracted mandibular premolars were decoronated to the standard root length of 14 mm. The root canals were prepared with a crown-down technique using manual instrument to file F3 (30/.09). The samples were then divided into three groups: obturation with bioceramic-based sealer (n=9); ZOE-based sealer (n=9); and control group (n=9). Microleakage was measured using a dye penetration method with 1% methylene blue and observed under stereomicroscope at x20 magnification. The mean of the maximum penetration length from the lowest to the highest was found in the bioceramic-based sealer group (0.825 mm), the ZOE-based sealer group (3.850 mm), and the control group (4.444 mm). One-way ANOVA test showed a significant difference in the maximum penetration length between the three groups (p<0.05). The post hoc LSD test showed a significant difference in the maximum penetration length between the bioceramic-based and ZOE-based sealer groups (p<0.001). Obturation with bioceramic-based sealer provides a better apical sealing ability than that with ZOE-based sealer.

Abstract:<h6>Background/purpose</h6><p>A perfect sealing of root canal system is essential for preventing ingress of bacteria from the oral environment. The purpose of this study was to evaluate the apical sealing ability of bioceramic (EndoSequence BC Sealer®) and epoxy resin-based (AH Plus®) sealers at 24 h, 7 days and 4 weeks.</p><h6>Materials and methods</h6><p>Forty two extracted human upper anterior teeth were sectioned to leave the root 15-mm long, then all the roots were instrumented using a set of ProTaper® rotary instruments. Four roots were selected randomly as controls, and the remaining 38 roots were randomly divided into 2 groups of 19 roots each: group 1: EndoSequence BC Sealer® and gutta-percha, and group 2: AH Plus® and gutta-percha using a multiple wave condensation technique. The apical sealing ability of the filled root canal was measured using the fluid filtration method with 200 mmHg (26.67 KPa) above atmospheric pressure at 24 h, 7 days and 4 weeks. Scanning electron microscopy (SEM) was used to assess the adaptation and penetration of the sealers. The apical microleakage between 2 groups was compared using Student's t-test. <em>P</em> < 0.05 was considered statistically significant.</p><h6>Results</h6><p>EndoSequence BC Sealer® had significantly better sealing ability than AH Plus® at all test periods (<em>P</em> < 0.001). SEM showed EndoSequence BC Sealer® had better penetration into dentinal tubules.</p><h6>Conclusion</h6><p>Bioceramic sealer could promote proper sealing of root canals obturated with multiple wave condensation.</p>

Abstract:Abstract Objectives To investigate in vitro effects of a nanoparticle bioceramic material, iRoot BP Plus, on stem cells from apical papilla (SCAP) and in vivo capacity to induce pulp-dentin complex formation. Materials and methods The sealing ability of iRoot BP Plus was measured via scanning electron microscopy (SEM). SCAP were isolated and treated in vitro by iRoot BP Plus conditioned medium, with mineral trioxide aggregate (MTA) conditioned medium and regular medium used as controls, respectively. Cell proliferation was assessed by BrdU labeling and MTT assay and cell migration was evaluated with wound healing and transwell assays. Osteo/odontogenic potential was evaluated by Alizarin red S staining and qPCR. Pulp-dentin complex formation in vivo was assessed by a tooth slice subcutaneous implantation model. Results iRoot BP Plus was more tightly bonded with the dentin. There was no difference in SCAP proliferation between iRoot BP Plus and control groups ( P > 0.05). iRoot BP Plus had a greater capacity to elevated cell migration ( P < 0.05) and osteo/odontogenic marker expression and mineralization nodule formation of SCAP compared with MTA groups ( P < 0.05). Furthermore, the new continuous dentine layer and pulp-like tissue was observed in the iRoot BP Plus group in vivo. Conclusions iRoot BP Plus showed excellent sealing ability, promoted the migration and osteo/odontogenesis of SCAP and induced pulp-dentin complex formation without affecting the cell proliferation, which indicated iRoot BP Plus was a promising coronal sealing material in REPs. Clinical relevance The coronal sealing materials play crucial roles for the outcomes of REPs. This study showed that iRoot BP Plus has good coronal sealing and promote pulp-dentin complex formation compared with MTA, providing experimental evidences for the clinical application of iRoot BP Plus as a promising coronal seal material in REPs.

Abstract:Abstract Background Research shows that nano-bioceramics can modulate the differentiation of dental stem cells. The novel ready-to-use calcium-silicate-based root-canal sealer iRoot SP is widely used in root filling. Accordingly, the aim of this study was to evaluate the effects of iRoot SP on proliferation and osteogenic differentiation in human stem cells from the apical papilla (hSCAPs). Methods hSCAPs were isolated and characterized in vitro, then cultured with various concentrations of iRoot SP extract. Cell proliferation was assessed by CCK-8 assay, and scratch-wound-healing assays were performed to evaluate cell-migration capacity. hSCAPs were then cultured in osteogenic medium supplemented with iRoot SP extracts. Alkaline phosphatase (ALP) activity assay was used to evaluate ALP enzyme levels. Alizarin red staining and cetylpyridinium chloride (CPC) assays were performed to assess calcified-nodule formation and matrix-calcium accumulation of hSCAPs. The mRNA and protein expression levels of the osteogenic markers OCN, OSX, Runx2, and DSPP were determined by qRT-PCR and Western blotting. The data were analyzed using one-way ANOVA and LSD-t tests. Results iRoot SP at low concentrations (2, 0.2, and 0.02 mg/mL) is nontoxic to hSCAPs. iRoot SP at concentrations of 0.02 and 0.2 mg/mL significantly increases cell-migration capacity. In terms of osteogenic differentiation, 0.2 mg/mL iRoot SP promotes intracellular ALP activity and the formation of mineralized nodules. Moreover, the expression of osteogenic markers at the mRNA and protein levels are upregulated by iRoot SP. Conclusion iRoot SP is an effective filling material for periapical bone regeneration.

Biological and physico-chemical properties of new root canal sealers

Abstract:BACKGROUND: The purpose of the present study was to compare the biological and the physico-chemical properties of bioceramic-based root canal sealers, calcium hydroxide-based, MTA-based and epoxy resin-based root canal sealers.MATERIAL AND METHODS: Two bioceramic-based sealers, one calcium hydroxide-based sealer, one MTA-based sealer and two epoxy resin-based sealers were tested.RESULTS: EasySeal and MTA Fillapex showed severe citotoxic activity, AH Plus and SealapexTM moderate cytotoxicity, BioRoot™ RCS and TotalFill BC Sealer were both cytocompatible. Except for TotalFill BC Sealer, all root canal sealers caused inhibition zones when tested with E. faecalis. The highest inhibition zone was observed for EasySeal, followed by AH Plus. BioRoot™ RCS, SealapexTM and MTA Fillapex showed the lowest inhibition zone. All the tested materials showed different degree of antibacterial activity by using direct contact test (DCT). The highest values were observed for BioRoot™ RCS, TotalFill BC Sealer and EasySeal, followed by MTA Fillapex and SealapexTM. Except for BioRoot RCS and TotalFill BC Sealer, all the root canal sealers fulfilled the requirements of the ISO 6876 standard, demonstrating a weight loss less than 3%. Bioroot RCS, TotalFill BC Sealer and SealapexTM exhibited high alkaline pH with an increase both for BioRoot™ RCS and TotalFill BC Sealer after 24 hours.CONCLUSIONS: The new bioceramic-based sealers showed acceptable physico-chemical properties, but BioRoot™ RCS and TotalFill BC Sealer seems to be too soluble, not respecting ISO 6876 requirements. The effect of sealer type and obturation technique on the fracture resistance of endodontically treated roots

Abstract:Objective: This study evaluated the effect of sealer type with various obturation techniques on the fracture resistance of root filled teeth. Materials and methods: Eighty mandibular premolars were collected, and root canals of 70 of them were prepared by Mtwo system up to 40/04. Thirty teeth were obturated using TotalFill BC sealer, and 30 using AH Plus sealer, with one of the obturation techniques (N = 10), namely cold lateral compaction (CLC), warm vertical compaction (WVC), and single cone (SC). Ten teeth were left without obturation as a positive control, and the other 10 teeth were not prepared serving as a negative control. Teeth were stored for 3 weeks, and the crowns were separated and the roots were subjected to fracture test using a universal testing machine. Results: Data was analyzed by ANOVA followed by Tukey's test. Fracture resistance of the TotalFill groups was significantly higher than the AH Plus groups in each obturation technique, and SC had the highest fracture resistance regardless of sealer used (P < 0.05). Obturation of the root canal by TotaFill BC sealer increased the calcium/phosphorous ratio in the dentin of the root. Conclusions: Obturation with TotalFill BC sealer improved the fracture resistance of the roots more than AH Plus sealer, and obturation with SC resulted in higher fracture resistance than CLC and WVC. Clinical relevance: Obturation of the root canal with Bioceramic sealer with single cone obturation technique could reduce the incidence of vertical root fracture.
