Abstract:Abstract Biological ages predicted by biochemical markers (biomarkers) outperform other measures in predicting a variety of aging outcomes. Several have been developed in recent studies, and there is evidence that each may independently predict mortality. While the included biomarkers are disease-associated, it is unclear what aspects of aging are captured. We aimed to understand and quantify genetic predisposition to accelerated biological ages, determined based on two measures, PhenoAge (9 biomarkers plus chronological age, Levine et al. 2018) and BioAge (7 biomarkers plus chronological age, Levine 2013). We performed genome-wide scans using the UK Biobank data (n=107,460 for PhenoAge, n=98,446 for BioAge). The SNP-based (single nucleotide polymorphism) heritability estimates were 14.45% and 12.39% for PhenoAge and BioAge, respectively. Both shared the strongest signal in the APOE region, with opposite associations with e2 and e4 alleles. e2 was associated with younger BioAge but older PhenoAge. e4 was associated with older BioAge but younger PhenoAge. BioAge was highly genetically correlated with its element of systolic blood pressure (rg=0.84) and the genetic correlation between PhenoAge and red blood cell distribution width was 0.65. Previous genome-wide association study findings of the top hits suggest that BioAge mostly captures cardiac aging but PhenoAge has more to do with inflammatory aging. The results are consistent with SNP clusters by associations with a broad range of aging traits, including an independent cluster with SNPs near the APOE. Genetic risk scores will be created to quantify the genetic predisposition and will be tested for associations with numerous aging traits.

Abstract:IMPORTANCE Previous studies have shown an association between actual age at menarche and risk of all-cause mortality; however, the results are inconsistent, and no study has analyzed the joint associations between genetic susceptibility and actual age at menarche with the risk of mortality in prospective cohorts.OBJECTIVES To investigate joint associations of actual age and genetically determined age at menarche with risk of all-cause mortality.DESIGN, SETTING, AND PARTICIPANTS This prospective cohort study was conducted using data from the UK Biobank population across the United Kingdom from March 13, 2006, to October 1, 2010. A total of 264 546 women aged between 39 and 71 years with actual menarcheal age were included in this study; 246 676 of these women had genetic data available. Actual age at menarche was obtained from the touchscreen questionnaire at recruitment from 2006 to 2010. Genetically determined age at menarche was assessed by a genetic risk score. Statistical analysis was performed from August 22 to December 12, 2019.EXPOSURE Age at menarche.MAIN OUTCOMES AND MEASURES A multivariable Cox proportional hazards regression model was used to assess associations of actual or genetically determined age at menarche with risk of all-cause mortality.RESULTS The mean (SD) age of the study population at baseline was 56.4 (8.0) years, and the mean (SD) age at menarche included in the analyses was 13.0 (1.6) years. During a median of 9.0 years (range, 8.3-9.7 years) of follow-up, 7761 deaths were documented among the women with actual age at menarche, and 7054 deaths were documented among the women with genetically determined age at menarche. Both the actual age at menarche and the genetically determined age at menarche showed a U-shaped association with the risk of all-cause mortality (lowest actual age [<12 years] vs reference age [15 years]: hazard ratio [HR], 1.16 [95% CI, 1.07-1.26]; highest actual age [>= 16 years] vs reference age [15 years]: HR, 1.17 [95% CI, 1.05-1.31]; P <.001 for quadratic trend; genetic risk score [GRS] of 1 vs reference score [GRS of 4]: HR, 1.10 [95% CI, 1.01-1.19; GRS of 6 vs reference score [GRS of 4]: HR, 1.09 [95% CI, 1.00-1.18]; P =.03 for quadratic trend). Significant interactions were also found between actual age at menarche and genetically determined age at menarche with all-cause mortality (HR of mortality associated with age of menarche <12 year was 1.24 [95% CI, 1.10-1.40] in the GRS of 1 group and 1.44 [95% CI, 1.21-1.72] in the GRS of 6 group; P =.001 for interaction). Women with mismatch of actual age and genetically determined age at menarche had the highest mortality risks; participants with the lowest genetic risk score and the highest age at menarche had an HR of 2.12 (95% CI, 1.58-2.83), and participants with the highest GRS and the lowest age at menarche had an HR of 1.44 (95% CI, 1.21-1.72).CONCLUSIONS AND RELEVANCE The results suggest that both actual age and genetically determined age at menarche exhibit U-shaped associations with all-cause mortality. Women with mismatch of actual age and genetically determined age at menarche may have the highest risk of all-cause mortality.
Associations of combined phenotypic aging and genetic risk with incident cancer: A prospective cohort study

Abstract:Background: Age is the most important risk factor for cancer, but aging rates are heterogeneous across individuals. We explored a new measure of aging-Phenotypic Age (PhenoAge)-in the risk prediction of site-specific and overall cancer. Methods: Using Cox regression models, we examined the association of Phenotypic Age Acceleration (PhenoAgeAccel) with cancer incidence by genetic risk group among 374,463 participants from the UK Biobank. We generated PhenoAge using chronological age and nine biomarkers, PhenoAgeAccel after subtracting the effect of chronological age by regression residual, and an incidence-weighted overall cancer polygenic risk score (CPRS) based on 20 cancer site-specific polygenic risk scores (PRSs). Results: Compared with biologically younger participants, those older had a significantly higher risk of overall cancer, with hazard ratios (HRs) of 1.22 (95% confidence interval, 1.18–1.27) in men, and 1.26 (1.22–1.31) in women, respectively. A joint effect of genetic risk and PhenoAgeAccel was observed on overall cancer risk, with HRs of 2.29 (2.10–2.51) for men and 1.94 (1.78–2.11) for women with high genetic risk and older PhenoAge compared with those with low genetic risk and younger PhenoAge. PhenoAgeAccel was negatively associated with the number of healthy lifestyle factors (Beta = –1.01 in men, p<0.001; Beta = –0.98 in women, p<0.001). Conclusions: Within and across genetic risk groups, older PhenoAge was consistently related to an increased risk of incident cancer with adjustment for chronological age and the aging process could be retarded by adherence to a healthy lifestyle. Abstract:Epigenetic age (EA) is recognized as a valuable biomarker of aging. The discrepancy between epigenetic and chronological age (CA), termed epigenetic age acceleration (EAA), has been associated with increased mortality from various age-related diseases, including cancer. However, previous studies primarily focused on the EA in blood cells, resulting in a notable gap in understanding the EA in cancer-relevant tissues and its relationship to cancer development. To examine if genetic risks for seven distinct cancers are associated with EAA in cancer-relevant tissues and blood cells, we utilized Illumina EPIC-array-based DNA methylation data and whole genome sequencing (WGS)-derived genotype data of 659 normal tissue and 42 whole blood samples from 315 European-ancestry subjects in the Genotype-Tissue Expression (GTEx) project. EA was estimated across 30 female breast, 167 colon, 41 kidney, 163 lung, 118 ovary, 96 prostate, 44 testis, and 42 whole blood samples via seven established epigenetic clocks: HorvathAge, GrimAge, GrimAge2, HannumAge, PhenoAge, Zhang-BLUP-Age, and Zhang-EN-Age. EAA was calculated as the residual of EA regressed on CA. For each participant, a polygenic risk score (PRS) was computed to assess the genetic risk for each cancer, using validated cancer-specific PRSs for European populations from the Polygenic Score Catalogue. Linear regression analysis was performed to evaluate the association between cancer PRS and EAA, adjusting for body mass index, sex (where applicable), cigarette smoking, and WGS platform and library construction protocol. All EA estimates of cancer-relevant tissues and blood cells exhibited significant correlations with CA (maximum P=2.44 × 10−3). Among them, GrimAge, known for its strong predictive capacity for mortality, had the most pronounced correlation with CA, with Spearman coefficients ranging from 0.80 in blood cells to 0.98 in breast tissues. A higher prostate cancer PRS was significantly associated with increased EAA in normal prostate tissues measured by PhenoAge (β=2.49; P=0.01), Zhang-BLUP-Age (β=1.71; P=0.02), HannumAge (β=1.36; P=0.04), and GrimAge (β=0.73; P=0.04). In addition, a higher PRS for renal cell carcinoma showed a significant association with increased GrimAge acceleration in normal kidney tissues (β=0.82; P=0.02). Moreover, breast and lung cancer PRSs were significantly associated with increased EAA in blood cells. Specifically, breast cancer PRS was associated with Zhang-BLUP-Age acceleration (β=0.99; P=0.04), while lung cancer PRS showed associations with HannumAge (β=1.78; P=0.01) and Zhang-EN-Age acceleration (β=0.94; P=0.02). Our results suggest a potential connection between genetic susceptibility to certain cancers and accelerated aging in relevant tissues and/or blood cells before cancer onset. Impact of Accelerated Biological Aging and Genetic Variation on Esophageal Adenocarcinoma: Joint and Interaction Effect in a Prospective Cohort.

Abstract:Accelerated biological aging may be associated with increased risk of esophageal adenocarcinoma (EAC). However, its relationship with genetic variation, and its effect on improving risk population stratification, remains unknown. We performed an exposome association study to determine potential associated factors associated with EAC. To quantify biological age and its difference from chronological age, we calculated the BioAge10 and Biological Age Acceleration (BioAgeAccel) based on chronological age and nine biomarkers. Multivariable Cox regression models for 362,310 participants from the UK Biobank with a median follow-up of 13.70 years were performed. We established a weighted polygenic risk score (wPRS) associated with EAC, to assess joint and interaction effects with BioAgeAccel. Four indicators were used to evaluate their interaction effects, and we fitted curves to evaluate the risk stratification ability of BioAgeAccel. Compared with biologically younger participants, those older had higher risk of EAC, with adjusted HR of 1.79 (95%CI: 1.52-2.10). Compared with low wPRS and biologically younger group, the high wPRS and biologically older group had a 4.30-fold increase in HR (95% CI: 2.78-6.66), at meanwhile, 1.15-fold relative excess risk was detected (95% CI: 0.30-2.75), and 22% of the overall EAC risk was attributable to the interactive effects (95% CI: 12%-31%). The 10-year absolute incidence risk indicates that biologically older individuals should begin screening procedures 4.18 years in advance, while youngers can postpone screening by 4.96 years, compared with general population. BioAgeAccel interacted positively with genetic variation and increased risk of EAC, it could serve as a novel indicator for predicting incidence. While various tools have been developed to assess biological age as an indicator of cancer risk, most do not account for complexities such as the effects of genetic variation on risk. Here, using UK Biobank data, the authors assessed the combined ability of data on genetic risk and insights from Biological Age Acceleration (BioAgeAccel), a novel indicator of biological age, to identify individuals with increased esophageal adenocarcinoma (EAC) risk. Consistent associations were observed between BioAgeAccel and elevated EAC risk. Moreover, accelerated biological aging positively interacted with EAC-related genetic variation, indicating that accelerated biological aging signatures warrant early EAC screening. image

Abstract:Background Even though aging has been demonstrated to be associated with a higher risk of atrial fibrillation (AF). It is unclear whether biological aging is associated with risk of incident AF. Objective This study aims to investigate the association between biological aging and AF. Methods A total of 371,882 participants without AF at baseline from the UK Biobank were included. The incident AF was ascertained through linkage to the UK National Health Services register. Biological age was evaluated from clinical traits using the Klemera-Doubal method Biological Age (KDM-BA) and PhenoAge algorithm, respectively. The residual discrepancies between biological age with chronological age were defined as the age accelerations (KDM-BA acceleration and PhenoAge acceleration). The Cox proportional hazards model was used to evaluate the effects of age accelerations with the risk of incident AF. Results During a mean follow-up of 13.04 years, a total of 28,076 new cases of AF were identified. Accelerated biological age was associated with an increased risk of AF, with a hazard ratio (HR) of 1.11 (95% confidence intervals [CIs] 1.10 - 1.13) per standard deviations (SD) increase in KDM-BA acceleration (10.9 years), and 1.28 (95%CI 1.27 - 1.30) in PhenoAge acceleration (5.6 years), respectively. Conclusion Accelerated biological age quantified by clinical biomarkers is associated with increased risks of AF. Biological aging may represent a potential risk factor for incident AF in midlife and older adults and a potential target for risk assessment and intervention.

Abstract:Despite an overall general decline in cancer mortality over the last two decades, endometrial cancer (EC) is increasing in both incidence and mortality globally. In addition, EC has one of the largest racial disparities in mortality, most attributed to Black women being more likely to present with the more aggressive histological subtypes that are harder to diagnose and patients presenting at later stages when they are likely to have worse outcomes. While it is known that there are genetic and environmental factors that contribute to this observed disparity, we have a limited understanding of how these factors interact to jointly determine the prevalence of EC and/or result in worst outcomes in women who self-identify as Black or African American. To gain a better understanding of factors associated with EC prevalence in racially diverse women as defined either using self-identified race/ethnicity and by genetic ancestry, we used data from the National Institutes of Health All of Us Research Program to define an EC case control cohort based on electronic health record and survey information and the availability of whole genome sequencing (WGS) data for the same program participants. We mined the genetic data to identifier carriers of pathogenic or likely pathogenic mutations in known EC predisposition syndrome genes such as Lynch syndrome and across the genome broadly. Using data from the All of Us Social Determinants of Health survey, we calculated individual-level measures that characterize the neighborhood-built environment (neighborhood physical and social disorder, walkability, crime, and residential density) and the neighborhood social environment (social cohesion and support, perceived discrimination, and stress). Linear regression models examined the associations between genetic predisposition and neighborhood social and built environments independently and jointly with endometrial cancer prevalence. Our provisional results show that EC cases are 100 times more likely to carry pathogenic or likely pathogenic germline genetic variants compared to controls and that neighborhood social and built environment factors are associated with EC incidence. Models incorporating both genetic and measures of the social and built environments show complex relationships that warrant further exploration. While we find evidence of pathogenic and likely pathogenic variants associated with EC incidence, our studies remain underpowered due to the size and the continued underrepresentation of black of African American women in study cohorts and the relatively low minor allele frequencies of likely pathogenic variants in the general population. Larger racially and ethnically diverse study cohorts that measure both genetic and non-genetic factors in the same study populations are sorely needed to better characterize the relative contribution of genetics and environmental factors in EC prevalence and ultimately to address the rising global health challenge of EC disparities. Abstract:Obesity is more strongly associated with endometrial cancer risk than any other cancer type. However, the extent to which genetic susceptibility influences the risk of endometrial cancer independent of obesity remains unknown. Thus, this study aimed to evaluate the predictive performance of polygenic risk score (PRS), body mass index (BMI) and other factors in determining endometrial cancer risk, as well as to explore joint effects on endometrial cancer. PRS was generated using 1.1 million HapMap3 variants from the largest published endometrial cancer genome-wide association study. An endometrial cancer prediction model was constructed using established endometrial cancer risk factors (age, BMI, number of live births, ever taken oral contraceptive pill, age of menarche, and predicted age of menopause). Additionally, an integrated prediction model was constructed, combining the endometrial cancer PRS with established risk factors. The ability of models to predict endometrial cancer status was assessed in unrelated participants of European ancestry from the UK Biobank cohort, encompassing 1,867 participants with endometrial cancer (860 incident cases) and 133,322 cancer-free controls. The integrated model including PRS and risk factors had a modest yet statistically significant improvement in prediction of endometrial cancer status, compared with the model using risk factors alone (AUC 0.752 versus 0.741; P = 1.28 × 10−7). When examining incident cases, participants in the middle and top tertiles of the PRS distribution had a 1.96- (95% CI 1.39-2.77; P = 1.28 × 10−4) and 3.03-fold (95% CI 2.01-4.58; P = 1.60 × 10−8) elevated risk of developing endometrial cancer, respectively, in contrast to those in the lowest tertile. As expected, elevated BMI was associated with increased endometrial cancer risk. Compared to participants with a normal BMI, overweight participants (25kg/m2 ≤ BMI < 30kg/m2) had a 1.58-fold increased hazard ratio (HR; 95% CI 1.32-1.90; P = 8.43 × 10−7), while obese participants (BMI ≥ 30kg/m2) a 3.23-fold increased HR (95% CI 2.71-3.85; P < 2.00 × 10−16). Notably, we observed multiplying effects of BMI and PRS on endometrial cancer risk, with obese participants in the top tertile of the PRS distribution having a 7.53-fold increased HR (95% CI 3.78-14.99; P = 9.41 × 10−9). In summary, these findings indicate that higher PRS was associated with endometrial cancer risk, irrespective of BMI. Moreover, the integration of PRS with established risk factors may aid in better stratifying the general population for their susceptibility to endometrial cancer. Citation Format: Tracy A. Accelerated epigenetic aging and risk of chemotoxicity in older adults with early breast cancer.

Abstract:1624 Background: Older adults with breast cancer have considerable heterogeneity in their risk of chemotoxicity; two people with the same chronological age have different risks of chemotoxicity. This heterogeneity in chemotoxicity may be due to older adults having variable rates of biological aging. Some older adults age faster biologically compared with their chronological age (i.e., accelerated epigenetic aging [AEA]), while others do not. AEA is a blood biomarker of aging, but its utility as a biomarker of chemotoxicity is unknown. Here, we evaluated the association between AEA and risk of chemotoxicity in older adults with early breast cancer. Methods: In a prospective multicenter study of 397 women age >65 with stage I-III breast cancer treated with neo/adjuvant chemo, we extracted genomic DNA from blood to estimate the participant’s rate of biological aging at baseline (prior to chemotherapy). Using two established epigenetic clocks (AgeAccelGrim and DunedinPACE), we classified individuals as either having AEA or not (dichotomized as a yes/no variable) for each clock. Our primary endpoint was grade 3+ chemotoxicity (yes/no, yes defined as any grade 3+ toxicity attributed to chemo). Using multivariable logistic regression, we examined the association between AEA and grade 3+ chemotoxicity, adjusting for sociodemographic, geriatric, and clinical covariates. Results: The median (range) pretreatment chronological age was 70 (65-85), AgeAccelGrim was -0.88 (-10.4-15.7), and DunedinPACE was 1.03 (0.69-1.45). Of the 397 women, 31 (8%) had AEA per AgeAccelGrim, 63 (16%) had AEA per DunedinPACE, and 184 (46%) experienced grade 3+ chemotoxicity. Women who had evidence of AEA as measured by AgeAccelGrim (≥6.83 years, adjusted OR = 2.73, 95% CI 1.17-6.41, p = 0.02) or DunedinPACE (≥1.17 biological years per chronological year, adjusted OR = 2.20, 95% CI 1.18-4.10, p = 0.01) had greater odds of having grade 3+ chemotoxicity compared to women without AEA (AgeAccelGrim <6.83 or DunedinPACE <1.17, respectively). Conclusions: In this cohort of older women with early breast cancer, those with pre-chemo AEA had increased odds of chemotoxicity compared to women without any evidence of AEA. Abstract:Background: Low educational attainment has been reported as a risk factor for many diseases. However, conclusion on the association between educational attainment and endometrial cancer (EC) are inconsistent in previous observational studies. This study aims to explore the potential causal association between educational attainment and EC. Methods: A Mendelian Randomization analysis was performed using publicly summary-level data sets of genome-wide association studies (GWAS). A total of 306 single-nucleotide polymorphisms (SNPs) were extracted as instrumental variables for the exposure of educational attainment from the Social Science Genetic Association Consortium GWAS summary data of 1,131,881 participants of European ancestry. SNPs of EC were obtained from the Endometrial Cancer Association Consortium, the Epidemiology of Endometrial Cancer Consortium and the UK Biobank involving 121,885 people. We conducted inverse variance weighted (IVW) to estimate the causal effect as our primary outcome. And we perform several sensitivity analyses, including MR-Egger regression, weighted median method, MR-PRESSO (Mendelian Randomization Pleiotropy Residual Sum and Outlier) global test, and leave-one-out sensitivity analysis, to evaluate the effect of pleiotropism on the causal estimates. Results: Genetic predisposition towards 4.2 years of additional educational attainment was associated with 38% lower risk of EC. (odds ratio 0.72, 95% confidence interval 0.62 to 0.83; p = 1.65*10-5). The consistent results of sensitivity analyses indicated our causal estimates were reliable. Genetic predisposition towards longer educational attainment was associated with lower risk of obesity, high waist-to-hip ratio (WHR), and diabetes. Conclusion: This study indicated that low educational attainment was a causal risk factor for EC, especially for EC with endometrioid histology. Low educational attainment might lead to EC through the mediator of obesity, high WHR, and diabetes.
Associations of combined phenotypic ageing and genetic risk with incidence of chronic respiratory diseases in the UK Biobank: a prospective cohort study

Abstract:Background: Accelerated biological ageing has been associated with an increased risk of several chronic respiratory diseases. However, the associations between phenotypic age, a new biological age indicator based on clinical chemistry biomarkers, and common chronic respiratory diseases have not been evaluated. Methods: We analysed data from 308 592 participants at baseline in the UK Biobank. The phenotypic age was calculated from chronological age and nine clinical chemistry biomarkers, including albumin, alkaline phosphatase, creatinine, glucose, C-reactive protein, lymphocyte percent, mean cell volume, red cell distribution width and white blood cell count. Furthermore, phenotypic age acceleration (PhenoAgeAccel) was calculated by regressing phenotypic age on chronological age. The associations of PhenoAgeAccel with incident common chronic respiratory diseases and cross-sectional lung function were investigated. Moreover, we constructed polygenic risk scores and evaluated whether PhenoAgeAccel modified the effect of genetic susceptibility on chronic respiratory diseases and lung function. Results: The results showed significant associations of PhenoAgeAccel with increased risk of idiopathic pulmonary fibrosis (IPF) (hazard ratio (HR) 1.52, 95% CI 1.45-1.59), COPD (HR 1.54, 95% CI 1.51-1.57) and asthma (HR 1.18, 95% CI 1.15-1.20) per 5-year increase and decreased lung function. There was an additive interaction between PhenoAgeAccel and the genetic risk for IPF and COPD. Participants with high genetic risk and who were biologically older had the highest risk of incident IPF (HR 5.24, 95% CI 3.91-7.02), COPD (HR 2.99, 95% CI 2.66-3.36) and asthma (HR 2.07, 95% CI 1.86-2.31). Mediation analysis indicated that PhenoAgeAccel could mediate 10∼20% of the associations between smoking and chronic respiratory diseases, while ∼10% of the associations between particulate matter with aerodynamic diameter <2.5 µm and the disorders were mediated by PhenoAgeAccel. Conclusion: PhenoAgeAccel was significantly associated with incident risk of common chronic respiratory diseases and decreased lung function and could serve as a novel clinical biomarker.

Abstract:Background Early-onset cancer (diagnosed under 50 years of age) is generally more aggressive. Its rising incidence is a global concern. The associations between genetic risk, lifestyle, and the risk of early-onset cancer may inform preventive strategies. Methods We studied the association between genetic risk, lifestyle and early-onset cancers among 66308 white British participants enrolled in the UK Biobank before age 50. Genetic risk was assessed using sex-specific composite total cancer polygenic risk scores (PRSs). We calculated composite PRSs as weighted sums of previously-developed cancer site-specific PRSs using weights obtained from lasso regression. We considered a breast cancer-specific PRS for analyses on breast cancer among women. A health-associated lifestyle score (HLS) was calculated based on baseline smoking, BMI (men only), physical activity, alcohol intake, and diet. Cox models were used to estimate hazard ratios (HRs) and 95% confidence intervals (CIs) of early-onset total cancer and breast cancer across genetic risk and HLS groups in women and men. Results A total of 1247 incident invasive early-onset cancer cases (820 in women, 427 in men) were documented, including 386 cases of early-onset breast cancer. In multivariable-adjusted analyses with 2-year latency, higher genetic risk (highest vs. lowest tertile of PRS) was associated with significantly increased risks of early-onset total cancer in women (composite PRS HR=1.85; 95% CI, 1.50-2.29), early onset total cancer in men (composite PRS HR=1.94; 95% CI, 1.45-2.59), and early-onset breast cancer in women (breast-cancer PRS HR=3.06; 95% CI, 2.20-4.25). The HRs (95% CI) of early-onset total cancer in women and men, and early-onset breast cancer in women associated with adopting an unfavorable lifestyle (highest vs. lowest tertile of HLS) were 1.49 (0.99, 2.25), 1.14 (0.67, 1.95), and 1.78 (0.97, 3.24), respectively, in the total study population; 1.85 (1.02, 3.36), 3.27 (0.78, 13.72), and 1.67 (0.71, 3.90), respectively, in those with high genetic risk; 1.25 (0.60, 2.57), 1.11 (0.42, 2.89), and 1.66 (0.54, 5.11), respectively, in those with intermediate genetic risk; and 1.15 (0.44, 2.98), 1.16 (0.39, 3.40), and 2.10 (0.57, 7.75), respectively, in those with low genetic risk. Compared to individuals with both low genetic risk and a favorable lifestyle, those with high genetic risk and an unfavorable lifestyle had substantially increased risks of early-onset total cancer in women (3.07; 1.64-5.78) and men (2.18; 0.78-6.11), and significantly higher early-onset breast cancer risk in women (4.11; 1.56-10.85). Conclusion Genetic and lifestyle factors were independently associated with risks of early-onset total cancer and breast cancer. Compared to those with low genetic risk, individuals with a high genetic risk may benefit more from adopting a healthy lifestyle in preventing early-onset cancer. Collaborating with human factors when designing an electronic textbook

Abstract:The development of on-line engineering textbooks presents new challenges to authors to effectively integrate text and tools in an electronic environment. By incorporating human factors principles of interface design and cognitive psychology early in the design process, a team at Sandia National Laboratories was able to make the end product more usable and shorten the prototyping and editing phases. A critical issue was simultaneous development of paper and on-line versions of the textbook. In addition, interface consistency presented difficulties with distinct goals and limitations for each media. Many of these problems were resolved swiftly with human factors input using templates, style guides and iterative usability testing of both paper and on-line versions. Writing style continuity was also problematic with numerous authors contributing to the text.

Abstract:Introduction: While normal physiologic aging causes metabolic decline, metabolic dysregulation owing to excess nutrient intake, physical inactivity, and obesity may drive accelerated aging. However, the relationship of accelerated aging with metabolic dysfunction across different levels of BMI (i.e., metabolic obesity phenotype) and whether it may mediate the relationship with obesity-related cancer (ORC) risk is not well established. Methods: To test this hypothesis, we are leveraging existing blood-based DNA methylation (DNAm) data within the Women's Health Initiative (WHI) and the Atherosclerosis Risk in Communities (ARIC) cohorts. Weight, height and blood pressure were measured by trained technicians in the respective cohorts and fasting blood glucose, triglycerides and HDL cholesterol were quatified by standard clinican assays. Metabolic obesity phenotypes are classified as metabolically healthy normal weight (MHNW), metabolically unhealthy normal weight (MUNW), metabolically healthy overweight/obese (MHO), and metabolically unhealthy overweight/obese (MUO) when defined by ATP III criteria. Age acceleration was measured as a residual from regressing pan-tissue Horvath's DNAmAge on chronological age. Results: The mean baseline age was 64 ± 6.6 years and 54 ± 5.7 year among WHI and ARIC participants, respectively and the age acceleration was measured at an mean follow-up time of 16 years from baseline for WHI and 3.5 years from baseline for ARIC cohort. The outcome of interest, ORC incidence was observed among 474 participants of which 387 were women. The median time for cancer diagnosis from baseline was 25 year and from DNAm measurement was 18.6 years. Of the 4341 (70% women) participants included in the analysis, 23.9% were MHNW, 3.2% MUNW, 39% MHO, and 33.5% MUO. A 31.5% of MHNW, 40.4% of MUNW, 28.7% of MHO, and 35.2% of MUO showed age acceleration. MUNW (OR 1.47, 95% CI: 1.03-2.11) and MUO (OR 1.18, 95% CI: 1.00-1.40) individuals at baseline had increased odds of experiencing age acceleration at a follow-up visit. The risk of all ORC combined was significantly elevated among MUO (HR 1.63, 95% CI: 1.27-2.09) compared to MHNW. Age acceleration at the follow-up visit was not significantly associated with ORC risk (HR 0.91, 95% CI: 0.75-1.11). These analyses were not adjusted for confounders. Conclusions: MUNW and MUO individuals had increased odds of age acceleration at a follow-up visit. Furthermore, MUO individuals were at an elevated risk of ORC, compared to individuals with MHNW. Abstract:Chronological age only represents the passage of time, whereas biological age reflects the physiology states and the susceptibility to morbidity and mortality. The association between biological age and lung cancer risk remains controversial. Hence, we conducted a prospective analysis in the UK Biobank study and two-sample Mendelian randomization analysis to investigate this association. Biological aging was evaluated by PhenoAgeAccel, derived from routine clinical biomarkers. Independent of chronological age, PhenoAgeAccel was positively associated with the risk of overall and histological subtypes of lung cancer. There was a joint effect of PhenoAgeAccel and genetics in lung cancer incidence. In Mendelian randomization analysis, the genetically predicted PhenoAgeAccel was associated with the increased risk of overall lung cancer, small cell, and squamous cell carcinoma. Our findings suggest PhenoAgeAccel is an independent risk factor for lung cancer, which could be incorporated with polygenic risk score to identify high-risk individuals for lung cancer.

Abstract:This study investigates the influence of aging‐related genes on endometrial cancer, a prominent gynecological malignancy with rising incidence and mortality. By analyzing gene expression differences between cancerous and normal endometrial tissues, 42 aging‐related genes were identified as differentially expressed. Utilizing the TCGA‐UCEC sample, consensus clustering divided the samples into two molecular subgroups, Aging low and Aging high, based on their expression profiles. These subgroups showed distinct prognoses and survival rates, with the Aging high group associated with DNA repair and cell cycle pathways, and the Aging low group showing suppressed metabolic pathways and increased immune cell infiltration, suggesting a potential for better immunotherapy outcomes. Mutation analysis did not find significant differences in mutation frequencies between the groups, but a high Tumor Mutation Burden (TMB) correlated with better prognosis. A risk score model was also developed, showcasing significant prognostic power. Further analysis of the SIX1 gene revealed its overexpression in cancer cells. Drug sensitivity tests indicated that the low‐risk group might respond better to chemotherapy. This research underscores the significance of aging‐related genes in endometrial cancer, offering insights into their prognostic value and therapeutic potential, which could lead to personalized treatment approaches and enhanced patient management.

Abstract:Background: In vitro studies have demonstrated the role of the BCL-2 family of genes in endometrial carcinogenesis. The role of genetic variants in BCL-2 genes and their interactions with non-genetic factors in the development of endometrial cancer has not been investigated in epidemiological studies.Patients and Methods: We examined the relationship between BCL-2 gene family variants and endometrial cancer risk among 1,028 patients and 1,922 age-matched community controls from Shanghai, China. We also investigated possible interactions between genetic variants and established risk factors (demographic, lifestyle and clinical). Individuals were genotyped for 86 tagging single nucleotide polymorphisms (SNPs) in the BCL2, BAX, BAD and BAK1 genes.Results: Significant associations with endometrial cancer risk were found for 9 SNPs in the BCL2 gene (P trend <0.05 for all). For SNPs rs17759659 and rs7243091 (minor allele for both: G), the associations were independent. The odds ratio was 1.27 (95% CI: 1.04-1.53) for women with AG genotype for the SNP rs17759659 and 1.82 (95% CI: 1.21-2.73) for women with the GG genotype for the SNP rs7243091. No interaction between these two SNPs and established non-genetic risk factors of endometrial cancer was noticed.Conclusion: Genetic polymorphisms in the BCL2 gene may be associated with the risk of endometrial cancer in Chinese women.
Accelerated biological aging, cardiovascular risks, and cardiometabolic multimorbidity: a cohort study

Abstract:Background Cardiometabolic multimorbidity (CMM) is the coexistence of two or three cardiometabolic diseases (CMDs), including diabetes, ischemic heart disease (IHD), and stroke, which are recognized as cardiometabolic physiology changes with aging. However, whether accelerated biological aging is associated with the trajectories of CMM development remains poorly understood. Objectives To evaluate the association between accelerated biological aging and the trajectories of CMM. Methods Baseline accelerated biological aging status of 341,159 adults from UK Biobank was assessed by the Klemera-Doubal method biological age (KDM-BA), PhenoAge, and frailty status. A multi-state model was employed to evaluate their associations with CMM trajectories and corresponding prediction capacities. Results During a median follow-up of 8.84 years, 26,319 participants developed at least one CMD, 2,502 of which subsequently developed CMM. Robust associations were observed between three aging measures and the risk from the first CMD (FCMD) to CMM. Adjusted HRs from FCMD to CMM of PhenoAge acceleration and KDM-BA acceleration were 1.15 (95% CI: 1.12–1.19) and 1.14 (95% CI: 1.08–1.21), respectively, and the HRs of frail individuals were 1.33 (95% CI: 1.15–1.53) compared to the non-frail. Classifying the FCMD into three individual cardiovascular diseases, we observed three aging measures separately indicated risk projections to CMM from each disease. People with accelerated aging were with 10 ~ 30% higher probability of death from FCMD or CMM than from baseline. PhenoAge acceleration could predict the CMM risk the best individually (Harrell C-statistics = 0.6254). Two cardiovascular risk scores, Framingham risk score and SCORE2, yielded similar prediction power, but with much reduced risk estimates compared to aging measures. Conclusions Accelerated biological aging provide a reservoir for a better prediction of cardiometabolic aging to optimize the subclinical prevention of CMM, and could be targeted for may provide a novel target for early intervention.

Abstract:Biological age measures outperform chronological age in predicting various aging outcomes, yet little is known regarding genetic predisposition. We performed genome-wide association scans of two age-adjusted biological age measures (PhenoAgeAcceleration and BioAgeAcceleration), estimated from clinical biochemistry markers (Levine et al., 2018; Levine, 2013) in European-descent participants from UK Biobank. The strongest signals were found in the APOE gene, tagged by the two major protein-coding SNPs, PhenoAgeAccel—rs429358 (APOE e4 determinant) (p = 1.50 × 10−72); BioAgeAccel—rs7412 (APOE e2 determinant) (p = 3.16 × 10−60). Interestingly, we observed inverse APOE e2 and e4 associations and unique pathway enrichments when comparing the two biological age measures. Genes associated with BioAgeAccel were enriched in lipid related pathways, while genes associated with PhenoAgeAccel showed enrichment for immune system, cell function, and carbohydrate homeostasis pathways, suggesting the two measures capture different aging domains. Our study reaffirms that aging patterns are heterogeneous across individuals, and the manner in which a person ages may be partly attributed to genetic predisposition.
