Abstract:Objective To evaluate the effectiveness of noninvasive positive pressure ventilation(NPPV) as a weaning strategy in patients with acute respiratory failure after failure to wean from invasive positive pressure ventilation(IPPV). Method A prospective randomized and controlled clinical trial of weaning of IPPV was carried out in patients mechanically ventilated in mode of IPPV for more than 48 hours with failure in a spontaneous breathing trial(SBT: PSV 6 cmH_2O). Patients with contraindications to NPPV were excluded. After failure the SBT, patients were randomly divided(random number) in two groups. Patients in NPPV group were extubated after being ventilated with high pressure support for 30 minutes and then placed on NPPV. Patients in IPPV group were weaned following conventional procedure. Arterial blood gases, maximal inspiratory pressure, respiratory rate,tidal volume, rapid shallow breathing index, heart rate, arterial blood pressure, and peripheral oxygen saturation were measured before and after failing the SBT. The rate of complications, including pneumonia and tracheotomy duration mechanical ventilation, days of hospital stay and outcome were observed. Findings of the two groups were vompared using the Student t test and the chi-square test. Results The percentage of complications in the NPPV group was lower(22.9% versus 72.2%, P <0.01) ,with lower incidences of pneumonia(6.1%,36.1%; P <0.01) and tracheotomy. Compared between the two groups, days of ICU stay( 14.16(3.45) d vs. 22.57( 7.71 ) d; P <0.01) and total days of mechanical ventilation(14.88±3.76 days vs. 20.68± 2.79 days, P <0.01) of NPPV group are shorter than IPPV group. Conclusions NPPV is a good alternative to the mechanically venti-lated patients who fail in initial weaning attempts. The key to successful NPPV weaning is the proper selection of weaning candidates and using NPPV as soon as possible after extubation.
Survival analysis with uncertain endpoints using an internal validation subsample

Abstract:When a true survival endpoint cannot be assessed for some subjects, an alternative endpoint that measures the true endpoint with error may be collected, which often occurs when the true endpoint is too invasive or costly to obtain. We develop nonparametric and semiparametric estimated likelihood functions that incorporate both uncertain endpoints available for all participants and true endpoints available for only a subset of participants. We propose maximum estimated likelihood estimators of the discrete survival function of time to the true endpoint and of a hazard ratio representing the effect of a binary or continuous covariate assuming a proportional hazards model. We show that the proposed estimators are consistent and asymptotically normal and develop the analytical forms of the variance estimators. Through extensive simulations, we also show that the proposed estimators have little bias compared to the naA ve estimator, which uses only uncertain endpoints, and are more efficient with moderate missingness compared to the complete-case estimator, which uses only available true endpoints. We illustrate the proposed method by estimating the risk of developing Alzheimer's disease using data from the Alzheimer's Disease Neuroimaging Initiative. Using our proposed semiparametric estimator, we develop optimal study design strategies to compare survival across treatment groups for a new trial with these data characteristics. We demonstrate how to calculate the optimal number of true events in the validation set with desired power using simulated data when assuming the baseline distribution of the true event, effect size, correlation between outcomes, and proportion of true outcomes that are missing can be estimated from pilot studies. We also propose a sample size formula that does not depend on baseline distribution of the true event and show that power calculated by the formula matches well with simulation based results. Using results from a Ginkgo Evaluation of Memory study, we calculate the number of true events in the validation set that would need to be observed for new studies comparing development of Alzheimer's disease among those with and without antihypertensive use, as well as the total number of subjects and number in the validation set to be recruited for these new trials. Predominant cardiac sympathetic modulation during wake and sleep in patients with Rett syndrome

Abstract:Background Rett syndrome (RTT) is a rare neurological disorder primarily associated with mutations in the methyl-CpG-binding protein 2 (MECP2) gene. The syndrome is characterized by cognitive, social, and physical impairments, as well as sleep disorders and epilepsy. Notably, dysfunction of the autonomic nervous system is a key feature of the syndrome. Although Heart Rate Variability (HRV) has been used to investigate autonomic nervous system dysfunction in RTT during wakefulness, there is still a significant lack of information regarding the same during sleep. Therefore, our aim was to investigate cardiovascular autonomic modulation during sleep in subjects with RTT compared to an age-matched healthy control group (HC). Method A complete overnight polysomnographic (PSG) recording was obtained from 11 patients with Rett syndrome (all females, 10±4 years old) and 11 HC (all females, 11±4 years old; p=0.48). Electrocardiogram and breathing data were extracted from PSG and divided into wake, non-REM, and REM sleep stages. Cardiac autonomic control was assessed using symbolic non-linear heart rate variability analysis. The symbolic analysis identified three patterns: 0V% (sympathetic), 2UV%, and 2LV% (vagal). Results The 0V% was higher in the RTT group than in the HC group during wake, non-REM, and REM stages (p<0.01), while the 2LV and 2UV% were lower during wake and sleep stages (p<0.01). However, the 0V% increased similarly from the wake to the REM stage in both RTT and HC groups. Conclusions Therefore, the sympatho-vagal balance shifted towards sympathetic predominance and vagal withdrawal during wake and sleep in RTT, although cardiac autonomic dynamics were preserved during sleep.

Abstract:We have evaluated the virulence of two clinically relevant species of Curvularia; Curvularia spicifera and C. hawaiiensis, using an experimental model of disseminated infection in immunocompromised mice. Several inocula were tested over a range 1 × 103–1 × 106 colony‐forming units/animal. Both species had a similar behaviour, producing a high mortality. Tissue burden and histopathology studies demonstrated that lung was the organ most affected.

Abstract:Rett Syndrome (RTT) is a complex neurodevelopmental disorder that has multi-system involvement with co-occurring epilepsy, breathing problems and autonomic dysregulation. Autonomic dysregulation can increase the risk of cardiorespiratory vulnerability in this patient group. Assessment of heart rate variability (HRV) provides an overview of autonomic health in RTT and offers insight into how the sympathetic and parasympathetic components of the nervous system function. However, to our knowledge, no study has evaluated HRV in Rett patients to assess how the dynamics of autonomic function vary with age and changes during the day and/or night. Using non-invasive wearable sensors, we measured HRV in 45 patients with RTT and examined the time and frequency domain sympathetic and parasympathetic indices. Among the HRV indices assessed, heart rate decreases with age and is lower in the night across all ages studied. The sympathetic index (SDNN) and the parasympathetic indices (RMSSD and pNN50) are not seen to change with age. Nevertheless, these indices were all higher during the day when compared to the night. Our findings appear to show that Rett patients are less adaptable to autonomic changes during the night. In the clinical setting, this might be more relevant for patients with severe psychopathology.

Abstract:Background The initiation of invasive long-term ventilation (I-LTV) for an adolescent with Rett Syndrome (RTT) involves many serious bioethical considerations. In moving towards a more inclusive model of patient participation, transparency surrounding the main influencing factors around this decision is important. Objective We aimed to identify the main drivers influencing a clinician's decision to support initiation of I-LTV for an adolescent with RTT. Method We used an anonymous online vignette-based factorial survey. The survey was distributed internationally through eight professional multi-disciplinary organisations to reach clinicians working in paediatrics. Results We analysed 504 RTT vignettes completed by 246 clinicians using mixed effect regression modelling. The main three significant influencing factors identified were: parental agreement with the decision to support initiation, the family's support network, and proximity to a tertiary care centre Additional comments from participants focused on family support, and the importance of on-going communication with the family. Conclusion As the rights of those with disabilities improve and participation of adolescents in decision-making becomes more established, effective communications with the family around goals of care and particular sensitivity and reflective practise around methods of consensus building will likely contribute to a positive decision-making process at this difficult time.

Abstract:Objective This retrospective study aimed to analyze the clinical characteristics, ventilatory strategies, and effectiveness of ventilation in pediatric patients with central apneas treated at the Sleep Medicine and Long‐Term Ventilation Unit of the Bambino Gesù Children's Hospital in Rome from 2012 to 2022. Methods Among all ventilated patients at our Center from January 2012 to December 2022, we retrospectively included children with a cAHI ≥ 1 events/h on baseline poly(somno)graphic study. Additional parameters assessed included the underlying disease, type of ventilation (non‐invasive vs. invasive), age at ventilation onset, ventilation mode, and transcutaneous capnometry parameters. To assess the effectiveness of ventilation on central apneas, we compared the cAHI at baseline and on ventilation. Results Sixty‐seven patients met the inclusion criteria for central apnea (cAHI > 1 events/h). Diagnoses included hypoxic‐ischemic encephalopathy, 15 (22.4%); Ondine syndrome, 14 (20.9%); polymalformative syndrome, 10 (14.9%); Prader‐Willi syndrome, 8 (11.9%); brain tumor, 6 (9.0%); Down syndrome, 4 (6.0%); ROHHAD syndrome, 2 (3.0%); other infrequent pathologies were, Arnold‐Chiari II, primary central apnea, epilepsy, lisosomal diseases, hydrocephalus, myopathy, obesity, Rett Syndrome. Abstract:We report the observation of a child born at 38 5/7 weeks’ gestation, who presented with marked microcephaly and suffered from persistent intractable epilepsy up to his death at the age of 5 days. Magnetic resonance imaging suggested lissencephaly. But interestingly no submicroscopical deletion in LIS-1 gene could be detected and true lissencephaly could not be confirmed histologically. Instead, primary degenerative process was considered the most probable cause of congenital pontocerebellar hypoplasia II in this case, which may be associated with a maturation delay of the cerebral hemispheres. The present case points to possible pitfalls in neuroradiological interpretation of simplified cerebral gyration and to the necessity to take into account cerebral pathologies leading to slowing or arrest of intrauterine cerebral development and delay of cerebral gyration.

Abstract:Background: Hypoxic-ischemic encephalopathy (HIE) represents one of the major causes of neonatal death and long-term neurological disability. Both hypoxic-ischemic insults and therapeutic hypothermia (TH) can affect respiratory function. Currently, there is no evidence regarding optimal respiratory management in these infants. Methods: This is a retrospective cohort study examining newborns with HIE treated with TH between January 2015 and September 2020. The study population was divided into two groups based on different respiratory assistance during TH: spontaneous breathing (Group A) or mechanical ventilation (Group B). The primary outcome of the study was the mean pCO 2 ± SD evaluation during TH in ventilated and non-ventilated asphyxiated infants. The secondary outcome was the correlation between ventilation strategy and short-term neurologic outcome according to Rutherford et al.'s MRI scoring system. Results: A total of 126 newborns were enrolled, 75 in Group A and 51 in Group B. Respiratory management was individualized, and volume guarantee (VG) ventilation was the first choice for ventilated infants. Group B infants showed more severe conditions at birth. During TH, ventilated infants showed optimal mean pCO 2 comparable with those breathing spontaneously (40.6 mmHg vs. 42.3 mmHg, respectively, p 0.091), with no significant difference in pCO2 standard deviation between (7.7 mmHg vs. 8.1 mmHg, respectively, p 0.522). Mean pH, pH standard deviation, mean pO 2 , pO 2 standard deviation, and mean respiratory rate also did not differ between groups. MRI patterns of brain injury predictive of abnormal neurodevelopmental outcomes were similar in both groups. Logistic regression analysis demonstrated that only umbilical cord arterial blood pH-affected MRI lesions were associated with poor neurodevelopmental outcomes (OR 1.505; CI 95% 1.069–2.117). Conclusions: Infants cooled after HIE should receive individualized respiratory management, not necessarily involving intubation. In those infants requiring mechanical ventilation, a volume-targeted strategy appeared to be effective in maintaining stable blood gas levels. Short-term neurological outcomes appeared comparable in ventilated and non-ventilated infants.

Abstract:In the October 2009 issue of Pediatrics , Baraton et al1 revealed that large variations in serum sodium (SNa) concentration in the very preterm neonate ( 150 mEq/L) was 13 mEq/L had hyponatremia, with an average nadir SNa concentration of 126 mEq/L. Overcorrection of hyponatremia did not seem to be a factor for this group, because the change in SNa concentration averaged 6 mEq/L per day, with the variation in SNa concentration averaging 20 mEq/L over 5 days. Abstract:Background: The use of long-term noninvasive ventilation (NIV) in children has become increasingly prevalent in recent years. Traditional follow-up methods typically involve repeated hospitalizations for overnight sleep exams. However, in-built software data and domiciliary nocturnal capnography could present a novel approach to follow-up. Aims and Objectives: This study aimed to assess the feasibility and effectiveness of a domiciliary NIV program for children in the Italian Friuli Venezia Giulia region. Methods: All pediatric patients on home NIV for more than 2 months were prospectively enrolled in the study. Results: Between June 2021 and September 2023, 30 subjects (median age: 14.9 months; 21 males) were included. Most patients had neuromuscular diseases (66.7%), with spinal muscular atrophy (SMA) being the most common diagnosis (10 patients). All enrolled subjects received at least one home visit, with 70% receiving at least two visits. A total of 97 visits were conducted, including 91 overnight capnography. Recordings had good quality in 83.5% of cases. Abnormalities were observed in 52.6% of exams, PCO2 >50 mmHg for ≥2% of recording time in 35.2%, mean PCO2 <35 mmHg in 21.6%, mean SpO2 <90% for ≥2% of recording time in 11.8%, or a combination of PCO2 and SpO2 alterations in 31.4%. Ventilatory parameters were changed in 15.6% of cases. Overall, patients and caregivers expressed satisfaction with the program. Conclusions: A home-based NIV follow-up program is feasible and well-received by families. The potential reduction in hospitalizations could reduce infectious risks, healthcare costs, and improve patients' quality of life.

Abstract:Objectives Few data on alveolar hypoventilation in Prader‐Willi syndrome (PWS) are available and the respiratory follow‐up of these patients is not standardized. The objectives of this study were to evaluate the prevalence of alveolar hypoventilation in children with PWS and identify potential risk factors. Study Design This retrospective study included children with PWS recorded by polysomnography (PSG) with transcutaneous carbon dioxide pressure (PtcCO2) or end‐tidal CO2 (ETCO2) measurements, between 2007 and 2021, in a tertiary hospital center. The primary outcome was the presence of alveolar hypoventilation defined as partial pressure of carbon dioxide (pCO2) ≥ 50 mmHg during ≥2% of total sleep time (TST) or more than five consecutive minutes. Results Among the 57 included children (38 boys, median age 4.8 years, range 0.1–15.6, 60% treated with growth hormone [GH], 37% obese), 19 (33%) had moderate‐to‐severe obstructive sleep apnea syndrome (defined as obstructive apnea‐hypopnea index ≥5/h) and 20 (35%) had hypoventilation. The median (range) pCO2 max was 49 mmHg (38–69). Among the children with hypoventilation, 25% were asymptomatic. Median age and GH treatment were significantly higher in children with hypoventilation compared to those without. There was no significant difference in terms of sex, BMI, obstructive or central apnea‐hypopnea index between both groups. Conclusion The frequency of alveolar hypoventilation in children and adolescents with PWS is of concern and may increase with age and GH treatment. A regular screening by oximetry‐capnography appears to be indicated whatever the sex, BMI, and rate of obstructive or central apneas.

Abstract:Purpose: The Prone positioning in addition to non invasive respiratory support is commonly used in patients with acute respiratory failure. The aim of this study was to assess the accuracy of an impedance-based non-invasive respiratory volume monitor (RVM) in supine and in prone position. Methods: In sedated, paralyzed and mechanically ventilated patients in volume-controlled mode with acute respiratory distress syndrome scheduled for prone positioning it was measured and compared non-invasively tidal volume and respiratory rate provided by the RVM in supine and, subsequently, in prone position, by maintaining unchanged the ventilatory setting. Results: Forty patients were enrolled. No significant difference was found between measurements in supine and in prone position either for tidal volume ( p = 0.795; p = 0.302) nor for respiratory rate ( p = 0.181; p = 0.604). Comparing supine vs. prone position, the bias and limits of agreements for respiratory rate were 0.12 bpm (-1.4 to 1.6) and 20 mL (-80 to 120) for tidal volume. Conclusions: The RVM is accurate in assessing tidal volume and respiratory rate in prone compared to supine position. Therefore, the RVM could be applied in non-intubated patients with acute respiratory failure receiving prone positioning to monitor respiratory function.

Abstract:Systematic data on endocrinopathies in Rett syndrome (RTT) patients remain limited and inconclusive. The aim of this retrospective observational two-center study was to assess the prevalence of endocrinopathies in a pediatric population of RTT patients. A total of 51 Caucasian patients (47 girls, 4 boys) with a genetically confirmed diagnosis of RTT were enrolled (mean age 9.65 ± 5.9 years). The patients were referred from the Rett Center of two Italian Hospitals for endocrinological evaluation. All the study population underwent clinical and auxological assessments and hormonal workups. MeCP2 mutations were detected in 38 cases (74.5%), CDKL5 deletions in 11 (21.6%), and FOXG1 mutations in 2 (3.9%). Overall, 40 patients were treated with anti-seizure medications. The most frequent endocrinological finding was short stature (47%), followed by menstrual cycle abnormalities (46.2%), weight disorders (45.1%), low bone mineral density (19.6%), hyperprolactinemia (13.7%) and thyroid disorders (9.8%). In the entire study population, endocrinopathies were significantly more frequent in patients with MeCP2 mutations (p = 0.0005), and epilepsy was more frequent in CDKL5 deletions (p = 0.02). In conclusion, our data highlighted that endocrinopathies are not rare in RTT, especially in patients with MeCP2 deletions. Therefore, in the context of a multidisciplinary approach, endocrinological evaluation should be recommended for RTT patients.

The Clinical Value of Noninvasive Ventilation in Chronic Hypercapnic Respiratory Failure--a 3-Year Retrospective Study

Abstract:Objective: To study the effect of noninvasive positive-pressure ventilation (NIPPV) in treatment of chronic hypercapnic respiratory failure caused by chronic obstructive pulmonary disease,obesity-hypoventilation syndrome and bronchiectasis. Methods:From 2002 to 2005,there were 116 patients with obstructive pulmonary disorders ,19 patients with obesity-hypoventilation syndrome and 46 patients with bronchiectasis were treated in shanghai No . 6 Hopital because of hupercapic respiratory failure, their clinical data were analyzed respectively. All patients were not treated by NIPPV firstly in terms of chronic hyper-capnic respiratory failure; But at the second time these inpatients were treated by NIPPV. Given pulmonary function , arterial blood gas and calculated the hospitalization dates. Results: (1)Patients had the high probability of pursuing NIPPV, the compliance rate was high (compliance rate. 91 % ). (2)The date of arterial blood gas levels were improved. The arterial PaCO2 decrease from 62±10 mmHg to 53±6 mmHg in chronic obstructive pulmonary disease( P0.01);PaCO2 from 52±10 mmHg to 42±7 mmHg in obesity-hypoventilation syndrome( P0. 001); PaCO2 from 56±6 mmHg to 44±10 mmHg in bronchiectasis(P0.005). Meanwhile the arterial PaO2 increase from 61±13 mmHg,60±12 mmHg,58±11 mmHg to 71±15 mmHg,71±11 mmHg,75±18 mmHg in the three groups respectively( all P0.001). (3)Hospitalization dates decreased in all groups after initiating NIPPV, when compared with the year before NIPPV (P0.05). Conclusion: Noninvasive ventilation has partly effect on chronic obstructive pulmonary disease, obesity-hypoventilation syndrome and bronchiectasis .

Abstract:Background: Mechanical ventilation with a tidal volume (VT) of 6 mL/kg/predicted body weight (PBW), to maintain plateau pressure (Pplat) lower than 30 cmH2O, does not completely avoid the risk of ventilator induced lung injury (VILI). The aim of this study was to evaluate safety and feasibility of a ventilation strategy consisting of very low VT combined with extracorporeal carbon dioxide removal (ECCO2R). Methods: In fifteen patients with moderate ARDS, VT was reduced from baseline to 4 mL/kg PBW while PEEP was increased to target a plateau pressure--(Pplat) between 23 and 25 cmH2O. Low-flow ECCO2R was initiated when respiratory acidosis developed (pH < 7.25, PaCO2 > 60 mmHg). Ventilation parameters (VT, respiratory rate, PEEP), respiratory compliance (CRS), driving pressure (DeltaP = VT/CRS), arterial blood gases, and ECCO2R system operational characteristics were collected during the period of ultra-protective ventilation. Patients were weaned from ECCO2R when PaO2/FiO2 was higher than 200 and could tolerate conventional ventilation settings. Complications, mortality at day 28, need for prone positioning and extracorporeal membrane oxygenation, and data on weaning from both MV and ECCO2R were also collected. Results: During the 2 h run in phase, VT reduction from baseline (6.2 mL/kg PBW) to approximately 4 mL/kg PBW caused respiratory acidosis (pH < 7.25) in all fifteen patients. At steady state, ECCO2R with an average blood flow of 435 mL/min and sweep gas flow of 10 L/min was effective at correcting pH and PaCO2 to within 10 % of baseline values. PEEP values tended to increase at VT of 4 mL/kg from 12.2 to 14.5 cmH2O, but this change was not statistically significant. Driving pressure was significantly reduced during the first two days compared to baseline (from 13.9 to 11.6 cmH2O; p < 0.05) and there were no significant differences in the values of respiratory system compliance. Rescue therapies for life threatening hypoxemia such as prone position and ECMO were necessary in four and two patients, respectively. Only two study-related adverse events were observed (intravascular hemolysis and femoral catheter kinking). Conclusions: The low-flow ECCO2R system safely facilitates a low volume, low pressure ultra-protective mechanical ventilation strategy in patients with moderate ARDS.

Abstract:Objective Dyspnoea and sleep-disordered breathing (SDB) are common in children with life-limiting conditions but studies on treatment with non-invasive ventilation (NIV) or continuous positive airway pressure (CPAP) are scarce. The aim of the study was to describe children treated with long-term NIV/CPAP within a paediatric palliative care programme in France. Methods Cross-sectional survey on children and young adults with complex medical conditions treated within the French paediatric NIV network with long-term NIV/CPAP. Characteristics of the patients were analysed and patient-related outcome measures of NIV/CPAP benefit were reported. Results The data of 50 patients (68% boys), median age 12 (0.4–21) years were analysed. Twenty-three (46%) patients had a disorder of the central nervous system and 5 (10%) a chromosomal anomaly. Thirty-two (64%) patients were treated with NIV and 18 (36%) with CPAP. NIV/CPAP was initiated on an abnormal Apnoea-Hypopnoea Index in 18 (36%) of the patients, an abnormal nocturnal gas exchange alone in 28 (56%), and after an acute respiratory failure in 11 (22%) of the patients. Mean objective NIV/CPAP adherence was 9.3±3.7 hours/night. NIV/CPAP was associated with a decrease in dyspnoea in 60% of patients, an increase in sleep duration in 60% and in sleep quality in 74%, and an improvement in parents’ sleep in 40%. Conclusions In children with life-limiting conditions, long-term NIV/CPAP may be associated with relief of dyspnoea, an improvement of SDB and an improvement in parents’ sleep.
