Abstract:Objectives: To investigate the psychometric properties of the reflux symptom index (RSI) as short screening approach for the diagnostic of laryngopharyngeal reflux (LPR) in patients with confirmed diagnosed regarding the 24-hour multichannel intraluminal impedance-pH monitoring (MII-pH). Methods: From January 2017 to December 2018, 56 patients with LPR symptoms and 71 healthy individuals (control group) were prospectively enrolled. The LPR diagnosis was confirmed through MII-pH results. All subjects (n = 127) fulfilled RSI and the Reflux Finding Score (RFS) was performed through flexible fiberoptic endoscopy. The sensitivity and the specificity of RSI was assessed by ROC (Receiver Operating Characteristic) analysis. Results: A total of 15 LPR patients (26.8%) of the clinical group met MII-pH diagnostic criteria. Among subjects classified as positive for MII- pH diagnoses, RSI and RFS mean scores were respectively 20 (SD ± 10.5) and 7.1 (SD ± 2.5), values not significantly different compared to the negative MII-pH group. The metric analysis of the items led to the realization of a binary recoding of the score. Both versions had similar psychometric properties, α was 0.840 for RSI original version and 0.836 for RSI binary version. High and comparable area under curve (AUC) values indicate a good ability of both scales to discriminate between individuals with and without LPR pathology diagnosis. Based on balanced sensitivity and specificity, the optimal cut-off scores for LPR pathology were ≥ 5 for RSI binary version and ≥ 15 for RSI original version. Both version overestimated LPR prevalence. The original version had more sensitivity and the RSI Binary version had more specificity. Conclusions: It would be necessary to think about modifying the original RSI in order to improve its sensitivity and specificity (RSI binary version, adding or changing some items), or to introduce new scores in order to better frame the probably affected of LPR patient.

Abstract:To develop and validate the Reflux Symptom Score (RSS), a self‐administered patient‐reported outcome questionnaire for patients with laryngopharyngeal reflux (LPR).Prospective controlled study.A total of 113 patients with LPR were enrolled and treated with diet and 3 months of pantoprazole, alginate, and/or magaldrate depending on the LPR characteristics (acid, nonacid, or mixed). Eighty asymptomatic individuals completed the study. Patients and controls completed the RSS twice within a 7‐day period to assess test‐retest reliability. Internal consistency was measured using Cronbach's α for the RSS items in patients and controls. Validity was assessed by comparing the baseline RSS with the Reflux Symptom Index (RSI) and Voice Handicap Index (VHI). Seventy‐seven patients completed the RSS at baseline and after 6 and 12 weeks of treatment to assess responsiveness to change. The RSS cutoff for determining the presence and absence of LPR was examined by receiver operating characteristic analysis.Test‐retest reliability (rs = 0.921) and internal consistency reliability (α = 0.969) were high. RSS exhibited high external validity indicated by a significant correlation with the RSI (rs = 0.831). Internal validity was excellent based on the higher RSS in patients compared with controls (P = .001). RSS, RSI, and VHI scores significantly improved from pre‐ to posttreatment, indicating a high responsiveness to change. RSS >13 can be considered suggestive of LPR‐related symptoms. RSS was not influenced by the occurrence of gastroesophageal reflux disease, LPR subtypes, or patient characteristics. RSS is a self‐administered patient‐reported outcome questionnaire that demonstrates high reliability and excellent criterion‐based validity. Comparison of Reflux Symptom Index (RSI) with Reflux Finding Score (RFS) and Its Effectiveness in Diagnosis of Laryngopharyngeal Reflux Disease (LPRD)

Abstract:LPRD is a common condition in patients attending ENT OPDs. Although esophageal manometry and 24 h pH monitoring is considered the gold standard for diagnosis, it is an expensive and time consuming investigation. Newer clinical scales have been developed for diagnosing LPR such as RSI, RFS, Carlsson-Dent, ReQuest, GerdQ, etc. The objective of the study is to compare RSI with RFS and to establish its effectiveness in diagnosing LPRD among OPD patients. It's a descriptive cross-sectional study. ENT outpatients with features of LPRD were asked to fill RSI proforma (score ≥ 13 abnormal), after which they were subjected to indirect laryngoscopy to obtain the RFS (score ≥ 7 diagnostic of LPRD). RSI was compared with RFS. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy of RSI were noted. Of 165 patients, 53.9% were females. Mean RSI scores in males and females were 11.9 and 11.5 and mean RFS scores were 6.4 and 5.7 respectively. RSI had 79.1% sensitivity and 83.7% specificity in diagnosing LPRD with PPV 76.8%, NPV 85.4% and accuracy 81.8% (Chi square value 64.5, p ≤ 0.01). There was substantial agreement between RSI and RFS (Cohen's kappa: 0.625, p ≤ 0.0001). RSI is a simple tool to diagnose LPRD which doesn't mandate invasive procedures such as endoscopy or esophageal manometry. Hence it can be used effectively to diagnose LPRD in ENT outpatients and start the treatment at the earliest.
[A Pilot Study on the Diagnosis of Laryngopharyngeal Reflux Diseases].

Abstract:OBJECTIVE:To study the value of the reflux symptom index (RSI) and reflux finding score (RFS) in laryngopharyngeal reflux disease (LPRD). METHODS:Twenty-five patients with RSI scores > 13 or RFS scores > 7 and sixteen patients with RSI scores > 13 and RFS scores > 7 were suspected as LPRD. Forty one patients with the RSI scores > 13 and/or RFS scores > 7 were suspected as LPRD, 15 patients with the RSI scores 13 and RFS scores > 7 (Kappa = 0.55, u = 3.06, P < 0.01). There was a significant decreasing in RSI and RFS scoring in patients after anti-acid treatment for 3 months (t(RSI) = 8.838, P(RSI) = 0.000; t(RFS) = 5.695, P(RFS) = 0.000). CONCLUSIONS:The RSI and RFS can be used as screening tool for laryngopharyngeal reflux diseases and as a simple method for evaluating the effectiveness of treatment.

Abstract:RSA‐10 is a new instrument for documenting signs of reflux. Objective To develop and validate the Reflux Sign Assessment‐10 (RSA‐10) for documenting the physical findings of laryngopharyngeal reflux disease (LPRD). Methods Patients with LPRD at the hypopharyngeal‐esophageal multichannel intraluminal impedance‐pH monitoring and asymptomatic individuals were consecutively recruited from two European hospitals. Three experienced otolaryngologists rated RSA‐10 in patients and controls for assessing internal validity. RSA‐10 was rated within a 7‐day period to assess test‐retest reliability. Internal consistency was measured using Cronbach's α in patients and controls. Convergent validity was evaluated through a correlation analysis between RSA‐10 and Reflux Finding Score (RFS). Interrater reliability was evaluated by comparing the RSA‐10 evaluations of the three otolaryngologists through Fleiss kappa. Pre‐ to posttreatment change of RSA‐10 was evaluated to assess responsiveness to change. The RSA‐10 thresholds were examined by receiver operating characteristic analysis. Results Fifty‐five patients completed the pre‐ to posttreatment evaluations from January 2020 to December 2023. A total of 115 asymptomatic individuals completed the study. RSA‐10 reported high internal consistency reliability (α = 0.822) and test‐retest reliability (rs = 0.725). The RSA‐10 scores of patients were significantly higher than those of controls (p = 0.001), suggesting high internal validity. RSA‐10 was significantly correlated with the RFS (rs = 0.771). The interrater reliability was adequate for sub‐ and total RSA‐10 scores (k = 0.708). RSA‐10 significantly improved from baseline to 3‐month posttreatment (p = 0.001). An RSA‐10 > 13 may be suggestive of LPRD. Both RSA‐10 > 13 and Reflux Symptom Score‐12 > 11 were associated with a sensitivity of 92.7% and a specificity of 97.3%. Conclusion The RSA‐10 is a reliable and valid clinical instrument for documenting the most prevalent laryngeal and extra‐laryngeal findings associated with LPRD. Abstract:OBJECTIVE:To discuss the Chinese version of reflux symptom index (RSI) scale's reliability, validity and clinical value.METHODS:Using the Chinese version of RSI scale on the ENT outpatients. There are one hundred and seven patients, included thirty-five patients with common throat (non-suspected) and seventy-two patients with clinically suspected throat reflux. 107 patients are to complete the two scales before and after one week. Thirteen patients with common throat(non-suspected) diseases and forty-one with suspected throat reflux also received RSI testing. Among the thirty-four confirmed acid reflux disease patients, thirty patients received pharmaceutical treatments and self-performed post-RSI testing after three months.RESULTS:The RSI Scale internal reliability consistency included the test-retest reliability (0.750 - 0.971), discriminant validity and construct validity. RSI total targeting percentage of 66.7%, targeting percentage is 80.8%, discriminant validity using 2 independent samples Wilcoxon test, RSI total score compared to Z = -3.266, P = 0.001. The before and after treatment self-control chi-square test (P < 0.05), difference was statistically significant.CONCLUSIONS:RSI Simplified Chinese version has good reliability and validity, and can be used for laryngopharyngeal reflux disease (LPRD) diagnostic screening along with the efficacy for the treatment of patients with appropriate LPRD aids.

Abstract:Laryngopharyngeal reflux disease (LPRD) is a condition characterized by the regurgitation of stomach and duodenal contents into the laryngopharynx, with variable and non-specific symptoms. Therefore, developing an accurate symptom scale for different regions is essential. Notably, the symptoms of "dryness and burning sensation in the laryngopharynx or mouth" are prevalent among the Chinese population but are often omitted from conventional symptom assessment scales, such as the Reflux Symptom Index (RSI) and Reflux Symptom Score-12 (RSS-12) scales. To address this gap, our study incorporated the symptoms into the RSI and RSS-12 scales, developing the RSI-10/RSS-13 scales. Afterward, we assessed the role of the new scale's reliability (Cronbach's α and test–retest reliability), construct validity (confirmatory factor analysis and confirmatory factor analysis), and diagnostic efficiency. Our study encompassed 479 participants (average = 39.5 ± 13.4 years, 242 female) and 91 (average = 34.01 ± 13.50 years, 44 female) completed 24 h MII-pH monitoring. The Cronbach's α values of 0.80 and 0.82 for the RSI-10 and RSS-13 scales, respectively. RSI-10 and RSS-13 exhibited strong test–retest reliability (ICCs = 0.82–0.96) and diagnostic efficacy (AUC = 0.84–0.85). Furthermore, the factor analysis identified the RSS-13 and its three sub-scales (ear-nose-throat, digestive tract, respiratory tract) exhibited good to excellent structural validity (χ 2 /df = 1.95, P < 0.01; CFI = 0.95, RMSEA = 0.06, SRMR = 0.05). The AUC optimal thresholds for the RSI-10 and RSS-13 in the Chinese population were 13 and 36, respectively. Besides, the inclusion of the new item significantly improved the diagnostic efficiency of the RSI scale ( P = 0.04), suggesting that RSI-10 holds promise as a more effective screening tool for LPRD, and global validation is needed to demonstrate the impact of this new symptom on the diagnosis of LPRD.

Abstract:OBJECTIVES:Currently, there is no cost-effective tool available to diagnose laryngopharyngeal reflux (LPR) in the developing country of China. The aim of this study was to achieve a linguistic adaptation of the Chinese version of the Reflux Symptom Index (RSI-CH).STUDY DESIGN:A nonrandomized, controlled, prospective trial.METHODS:A total of 107 patients at the outpatient clinic of Peking University People's Hospital were enrolled. They were asked to fill out the RSI-CH and underwent fiber-optic laryngoscopy to complete the Reflux Finding Score (RFS). Patients underwent pH monitoring if the RSI-CH was greater than 13 or if the RFS was not less than 7. Patients were treated with Omeprazole 20 mg twice a day for 3 months if the pH monitoring was positive. The reliability (Cronbach alpha coefficient and Spearman correlation analysis), validity (sensitivity, specificity, and positive and negative predictive values), and responsivity of RSI-CH were determined.RESULTS:RSI-CH had a good reliability (Cronbach alpha coefficient was greater than .7, whereas the test-retest validity for the total score and for each item were 0.750-0.971. The scale had a good criterion validity. The consistency (66.7%), sensitivity (61.76%), and specificity (75%), and the positive and negative predictive values (80.8% and 53.6%) were considered good. The RSI-CH scores changed from 15 to 7 after treatment, and the average score of the controlled group was 6.5.CONCLUSIONS:The RSI-CH developed and validated by this study can be used as an effective diagnostic tool in identifying differentiating LPR diseases in patients whose native language is Chinese.

Abstract:To evaluate the value of the reflux diagnostic questionnaire (RDQ) in the diagnosis of gastroesophageal reflux disease (GERD).One hundred and twenty-eight patients with heartburn, substernal pain, acid reflux and regurgitation in 10 hospitals in China were assessed. Each symptom was scored according to severity and frequency (five scales). The highest score was 40. Upper gastrointestinal endoscopy and 24-h esophageal pH monitoring were carried out in patients with symptom scores of more than six, and results were compared with the RDQ score to determine the critical value for diagnosis of GERD.The RDQ score correlated positively with the severity of reflux esophagitis. Esophageal pH monitoring showed that patients with abnormal RDQ had higher DeMeester scores compared with those with normal RDQ scores (20.18 vs 16.84). Taking 12 as the critical RDQ score for GERD, achieved a maximal Youden index. The area under the receiver operating characteristic curve was 0.71, the true positive diagnostic rate was 88.07%, and the true negative diagnostic rate was 68.42% with a sensitivity of 94.12% and specificity of 50%.The RDQ might be a useful screening test for GERD.
Evaluation of the Reflux Symptom Index and the Endolaryngeal Findings Scale after Treatment in Individuals with Laryngopharyngeal Reflux

Abstract:Abstract Introduction Laryngopharyngeal reflux (LPR) is a prevalent condition in outpatients visiting the otorhinolaryngological clinic, with many controversies regarding its diagnosis and follow-up. Therefore, there is a need for clinical instruments that can diagnose individuals and monitor the results of their treatment. Objective To evaluate the applicability of the scores translated and adapted to Brazilian Portuguese: Reflux Symptom Index (RSI) and Endolaryngeal Reflux Findings Scale (ERFS), as instruments for post-treatment follow-up of LPR. Method A total of 35 individuals with atypical symptoms of gastroesophageal reflux disease and with an objective diagnosis of LPR, confirmed by high digestive endoscopy and 24-hour esophageal pH-metry dual probe test were submitted to the RSI and ERFS questionnaires, used in the pre- and post-treatment of 90 days with a Proton Pump Inhibitor. Result The evaluation of RSI and ERFS in the pre- and post-treatment showed a reduction in the results of the two scores, with a higher drop in the RSI (p < 0.001) than in the ERFS (p = 0.014). Although there was an improvement in the values in 23.9% of the individuals in the ERFS, there was no change in the category (p = 0.057), different from the RSI (p < 0.001), where there was a 67.4% improvement in values, as well as change in category (score went from positive to negative). There was a correlation between RSI and ERFS, moderate in the pretreatment and strong in the post-treatment. Conclusion The RSI and ERFS scores, when translated and adapted for Brazilian Portuguese and applied simultaneously, can be considered a good tool for post-treatment LPR follow-up.
The Evaluation of Acid Reflux Index in the Diagnosis of Pathological Gastroesophageal Reflux in Children: the Analysis of Data of Esophageal Ph Monitoring of 4109 Cases in 12 Years

Abstract:Objective The aim of the present study was to evaluate the diagnostic value of acid reflux index(RI) in the pathological gastroesophageal reflux(GER) in children and to discuss the diagnostic criteria of GER.Methods The patients performed by esophageal pH monitoring in Children's Hospital of Zhejiang University School of Medicine were all enrolled in this study from 1994 to 2006,except for the cases repeated or unfinished the measurement.According to the Boix-Ochoa criteria,in which pathological GER was confirmed if Boix-Ochoa score larger than 11.99,receiver operating characteristic(ROC) of RI was analyzed and the best diagnostic cutoff value for pathological GER was obtained.The difference and goodness of fitness of different results from other diagnostic criteria of pathological GER based on all kinds of RI were compared by statistical method such as McNemar test and kappa test.The sensitivity and specificity among different diagnostic criteria were evaluated.Results A total of 4109 cases(male 2 692 and female 1 417) performed esophageal pH monitoring were enrolled in this study.Among them,476 cases were neonate,1 553 younger than 1 year,658 younger than 3 years,837 younger than 7 years,513 younger than 12 years,and 72 younger than 16 years.There were thirteen kinds of diseases,including asthma(366 cases),bronchitis(468),pneumonia(385),persist pneumonia(143),chronic cough(184),pharyngitis(29),neonate apnea(102),neonate vomiting(283),neonate pneumonia(105),vomiting and/or upper abdominal pain(1817),hiatal hernia(140),non-cardiac chest pain(55),and malnutrition(32).ROC analysis indicated that the diagnostic cutoff value of RI was 2.9,and the sensitivity for pathological GER was 90.4%,while the specificity was 95.6%(false positive rate lower than 5.0%).The area under curve was 0.981,standard error was 0.002,and 95% confidence interval was from 0.977 to 0.984.It showed statistical significance for diagnoses of pathological GER(P=0.000).According to the new diagnostic criteria of pathological GER for both RI 4.0 criteria(RI more than 4.0%) and RI 2.9 criteria(RI more than 2.9%),the difference and goodness of fitness of results from Boix-Ochoa criteria were compared in 140 cases with hiatal hernia confirmed by surgery.The goodness of fitness from RI 2.9 criteria was significantly higher than that from RI 4.0 criteria(κ coefficient was 0.892、0.715,respectively,P all equal to 0.000).The sensitivity,accuracy,and negative predictive value for diagnostic pathological GER were significantly higher from RI 2.9 criteria than that from RI 4.0 criteria except for the specificity,while positive predictive value was almost equal between the two criteria.Conclusions RI 2.9 criteria had of significance value in the diagnosis of pathological GER.It was helpful to improve the sensitivity and accuracy of GER diagnoses in children compared with RI 4.0 criteria.
Adaptation and Validation of Reflux Symptom Index into Hindi Language (RSI-H®)

Abstract:Objective: The current study aimed to translate, culturally adapt, and validate the Reflux Symptomatic Index in the Hindi language. Secondly, we aimed to compare its outcomes between normal individuals and people suffering from laryngopharyngeal reflux. Study design: Cross-sectional study design. Material and methods: The original Reflux Symptom Index was translated into Hindi language using the forward-backward translation method. A total of 192 participants were included in the study. Among 192 participants, 57 were healthy controls, and 135 were individuals with laryngopharyngeal reflux. Internal consistency, test-retest reliability, and clinical validity were measured. Results: The majority of the population reported the presence of globus sensation. Results revealed that the Hindi version of the Reflux Symptom Index exhibited an excellent internal consistency (ie, 0.829). A significant difference was found between the mean scores of both groups, and the excellent test-retest reliability score (ie, 0.94 & 0.96) was obtained. Conclusion: We can conclude that the Hindi version of the Reflux Symptom Index exhibited a similar psychometric functional property as the Original version of the Reflux Symptom Index. So, the developed tool is a quick, reliable, and valid tool to assess laryngopharyngeal reflux.

Abstract:Objective: To develop and validate a questionnaire for supraesophageal manifestations of reflux (SER) that will facilitate its study in clinical and research settings. Study design: The Supraesophageal Reflux Questionnaire (SERQ) and previously validated Reflux Symptom Index (RSI) were subjected to multiple types of validity testing, including content validity, concurrent validity, reproducibility, and predictive validity. Results: The concurrent validity and reproducibility of both instruments was good to excellent for most items tested. The predictive validity of the SERQ was superior to the RSI when it included the covariates of history of sinusitis, use of over-the-counter antacid medications, age, gender, and body mass index. Conclusions: The SERQ will serve as both a useful clinical and research tool by offering not only SER symptom information, like the RSI, but also information about the patient's medical history and medication usage that will facilitate use of the SERQ in research protocols. Abstract:To analyze the significance of pH monitoring and reflux symptom index (RSI) in the diagnosis of laryngopharyngeal reflux disease (LPRD) and the characteristics of LPRD in symptoms and pH monitoring.RSI questionnaire survey and laryngopharyngeal pH monitoring were conducted on. 31 patients with laryngopharyngeal reflux-related symptoms, 15 males aged (50 +/- 13) years and 16 females aged (48 +/- 11) years, Gastroscopy was conducted on 13 patients to detect reflux esophagitis. The patients with the RSI scores > or = 13 were suspected as with LPRD and those with the reflux times > or = 7 or reflux area index (RAI) > or = 6.3 were diagnosed as with LPRD. McNemar test and Kappa test were performed on the RSI scores and reflux area indexes.Seventeen patients met the pH criteria. There were 8 patients suffering from voice disorders, 17 from clearing throat, 13 from excess throat mucus, 11 from dysphagia, 9 from cough after eating or with supine posture, 9 from dyspnea, 13 from cough, 12 from foreign body sensation, and 14 from digestive symptoms with the foregoing symptoms' mean scores of 1.4, 3.1, 2.1, 2.0, 1.1, 1.6, 1.8, 2.9, and 2.9 respectively. The P value with McNemar test on pH monitoring and RSI scoring was 0.289, and the P value with kappa test was 0.007. RSI was correlated with RAI(r(s) = 0.302, P = 0.049), reflux time (r(s) = 0.454, P = 0.005), and reflux time percentage (r(s) = 0.439, P = 0.007), but not correlated with the reflux frequency (r(s) = 0.252, P = 0.086). Supine reflux was seen in only 7 of the 224 reflux events in 17 patients, lasting only 96 seconds during the 84.6 minutes' supine acid exposure time.The common symptoms of LPRD are clearing throat, digestive symptoms, excess throat mucus, and foreign body sensation. Laryngopharyngeal pH monitoring and RSI scoring have the same value in diagnosing LPRD. Acid exposure in laryngopharynx is correlation to patients' self-report symptom. Almost all reflux events occur when the patients are in upright position.
Reflux Finding Score and Reflux Symptom Index Potential Indicators for Diagnosis and Management of Laryngopharyngeal Reflux Disease

Abstract:Retrograde movement of gastric contents into the pharynx is termed Laryngopharyngeal Reflux (LPR). It represents an extraesophageal manifestation associated with gastroesophageal reflux disease (GERD). The objective of the study is to investigate the clinical profile of LPR and its response to treatment. Three hundred consecutive patients who presented to the ENT outpatient department with a clinical profile of LPR were selected. The patients' symptoms were assessed using the Reflux Symptom Index (RSI), followed by an endoscopic examination of the larynx and scoring using the Reflux Finding Score (RFS). Patients were started on LPR treatment if they had an RSI score of 13 and an RFS of more than 7. The patients were then started on medication and monitored for three months. The most common symptom in our research sample (52%) was a foreign body sensation in the throat. On endoscopic examination of the larynx, the most common sign was hyperemia/erythema of laryngeal tissue, especially bilateral arytenoids. Most of our patients responded favorably to a combination of pantoprazole (40 mg) and domperidone (30 mg) for 4 weeks. This was measured by the reduction in RSI and RFS scores. LPR is a frequently encountered clinical entity, and otorhinolaryngologists should consider it when treating patients with chronic symptoms such as throat pain and voice changes. Appropriate LPR diagnosis and care can help prevent unnecessary prescriptions for antibiotics and surgical interventions in these patients.
Poor consistency between reflux symptom index and laryngopharyngeal pH monitoring in laryngopharyngeal reflux diagnosis in Chinese population

Abstract:BACKGROUND: It is unknown whether the reflux symptom index (RSI) can replace pH monitoring as a diagnostic tool for laryngopharyngeal reflux (LPR) in Chinese people. The relationships between reflux parameters and LPR symptoms also require further research.METHODS: A total of 216 Chinese patients underwent laryngopharyngeal pH monitoring and filled out an RSI questionnaire. Laryngopharyngeal pH monitoring indicated a diagnosis of LPR for patients with 7 or more episodes of reflux or a reflex area index (RAI) of 6.3 or more. The RSI questionnaire indicated a diagnosis of LPR for patients with RSI scores of 14 or higher.RESULTS: Of the 216 patients, 85 were diagnosed with LPR as assessed by the RSI, and 72 were diagnosed with LPR through laryngopharyngeal pH monitoring. The Cohen's kappa coefficient comparing LPR diagnosis consistency between RSI score and laryngopharyngeal pH monitoring was 0.133 (P=0.007). This indicated the two diagnostic methods were consistent to a low degree; the total consistency rate was only 59.7% (129/216). The sensitivity of the RSI was 48.6% (35/72), and its specificity was 82.5% (94/114). For convenience, we named the nine symptom groups in the RSI sequentially as P1-P9. P1, P2, P3, P5, P6, and P7 were all correlated with at least one reflux parameter (P<0.05), but P4, P8, and P9 were not correlated with any reflux parameters (P>0.05). A total of 72 patients were diagnosed using pH monitoring, the gold standard for LPR diagnosis. The most common symptoms of LPR were found to be P9, P3, P8, P7, and P2 in these patients. The symptoms that most seriously affected patients were P9, P8, P3, P7, and P2.CONCLUSIONS: The consistency in diagnosis of LPR between the RSI and laryngopharyngeal pH monitoring was poor, meaning the RSI is not a suitable LPR initial screening tool and cannot replace pH monitoring. Additionally, reflux symptoms P4, P8, and P9 were not correlated with any reflux parameters. The most prevalent LPR symptom was P9, followed by P3, P8, P7, and P2. The most severe symptom was also P9, followed by P8, P3, P7, and P2.

Abstract:Objective. To seek a deeper and more comprehensive understanding of two major diagnostic methods for laryngopharyngeal reflux by exploring whether and how differences exist before and after treatment between patients diagnosed by either Reflux Symptom Index (RSI) and Reflux Finding Score (RFS) or 24-hour pH monitoring.Materials and Methods. Two groups of patients who confirmed laryngopharyngeal reflux disease (LPRD) by either a combination of RSI and RFS (Questionnaire group, 35 patients) or 24-hour multichannel intraluminal impedance (MII) pH monitoring (pH Group, 23 patients) were recruited. All patients were prescribed esomeprazole 20 mg twice a day for 1 month. RSI, RFS, and acoustic parameters before and after treatment were compared between the two groups. Intrinsic correlations involving multiple parameters were investigated as well.Results. Except for excess throat mucus (P = 0.019) and subglottic edema (P = 0.042), most RSI and RFS items before treatment were not significantly different between the Questionnaire and pH Groups, and nearly all such items in both groups exhibited distinct remission after therapy (P < 0.05). Absolute value of remission in RSI after treatment was more prominent in pH Group than in the Questionnaire group (P = 0.007). Jitter (P = 0.252), shimmer (P = 0.815), and harmonics-to-noise ratio (P = 0.117) descended to normal value after treatment. Moderate to high levels of correlation were found between the patient's original status and the absolute value of remission in most items of RSI and RFS as well as voice parameters.Conclusion. The 24-hour MII pH monitoring and a combination of RSI and RFS are quite competitive with each other in selecting LPRD patients. Although treatment worked out on nearly all the symptoms, laryngeal images and voice parameters, 24-hour MII pH seems to be more promising in a greater symptom relief. The extent of relief that can take place in most of the measurements is considerably determined by their initial status.
Identifying Patients with Gastroesophageal Reflux Disease: Validation of a Practical Screening Tool

Abstract:A reliable, accurate noninvasive method for identifying patients with gastroesophageal reflux disease (GERD) in the primary care setting is needed. A population-based case-finding instrument may assist managed care organizations in identifying candidates for disease management or quality improvement programs. Our aim was to develop and validate a GERD case-finding instrument. A 12-item “GERD Screener” was developed based on literature review and expert opinion with questions about heartburn and regurgitation frequency and severity and medication use, as well demographics and exclusion criteria. Categorical and continuous scoring methods with and without medication use were developed a priori. Using a telephone interview in a medical group, we identified and enrolled 100 subjects with a history of GERD-like symptoms and 103 controls. Each subject completed the GERD Screener, a validated gastrointestinal symptom questionnaire, the Digestive Health Symptom Index (DHSI), and was evaluated independently by two gastroenterologists using a structured format. Agreement by the two physicians that symptoms were consistent with GERD and required an intervention was considered the gold standard. In all, 70 subjects were classified as having GERD and 106 as controls using the gold standard (κ = 0.73; 95% CI, 0.63–0.82). The scoring method for the screening instrument using a continuous measure of GERD symptoms (frequency and severity) and weighted medication use resulted in an area under ROC of 0.89 (95% CI, 0.84–0.94). Using a cutoff of >9 points, this measure was 83% sensitive and 83% specific. Compared to the gold standard, the DHSI GERD subscale has an area under ROC of 0.89 (95% CI, 0.84–0.94). The GERD Screener was highly correlated with the DHSI GERD subscale, r = 0.78 (95% CI 0.72–0.83; P < 0.0001). In conclusion, the GERD Screener has demonstrated construct, convergent, and predictive validity. It is shorter than existing validated instruments, practical, and easily administered, which may reduce the response and administrative burden. This may serve as a valuable case-finding instrument in primary-care and managed-care organizations wishing to implement programs to improve the quality and efficiency of care.

Abstract:At present, objective methods for diagnosing laryngopharyngeal reflux disease(LPRD) are not minimally invasive, effective, and economical. Diagnostic scales are widely used worldwide due to the advantages of inexpensive, noninvasive, and easy to operate. The reflux symptom index(RSI) and the reflux finding score(RFS) are preferred to use in clinical diagnosis. However, many controversies have appeared in the application of RSI and RFS in recent years, causing many troubles to clinical diagnosis. Therefore, this review briefly discusses the problems of RSI and RFS in clinical applications to provide reference for diagnosing LPRD accurately.
