Abstract:Background: Neurocysticercosis (NCC) is a public health problem in most of the world. Approximately 50 million people worldwide experience this disease, and it remains one of the most important causes of neurological morbidity. Extraparenchymal NCC (basal cisterns) is associated with high rates of morbidity and mortality. Currently, minimally invasive approaches, including the endoscopic endonasal approach, are used with good functional results. Observations: A 25-year-old patient presented with headache and pain in the left hemiface. Magnetic resonance imaging (MRI) showed cystic lesions in the prepontine basal cisterns compressing and displacing the left trigeminal nerve. The cysticercal lesions were excised using a transclival endoscopic approach. There were no complications during surgery, and pathology confirmed the diagnosis of NCC. The pain subsided after surgery. No residual cysts were observed on control MRI. Lessons: Minimally invasive approaches can be used to treat tumor and infectious problems of the skull base, as in this case, with good functional results. https://thejns.org/doi/10.3171/CASE24223.
Unilateral Subfrontal Approach for Giant Tuberculum Sellae Meningioma: Single Center Experience and Review of the Literature.

Abstract:BACKGROUND:Microsurgical Transcranial approach (mTCA) is the primary choice for the resection of giant Tuberculum Sellae Meningiomas (TSM). The objective of this study is to explore surgical details of unilateral subfrontal approach.METHODS:Ten patients with giant TSM treated by unilateral subfrontal approach were included from January 2018 to June 2021. Demographic characteristics, surgical data, post-procedure complications and outcomes of patients have been descriptive analyzed, combined with systematic literature review to explore the surgical details and the prognosis of unilateral subfrontal approach.RESULTS:Ten patients include six male and four females, age range from 35 to 77 years, duration of visual impairment from 1 to 12 months, were all performed unilateral subfrontal approach. Nine patients achieved radical resection (Simpson grades I-II) through post-operative imaging confirmation, and Simpson IV resection was performed in the remaining one due to cavernous sinus invasion. The postoperative visual acuity was improved or maintained in 8 patients. Visual acuity decreased in 2 cases, including 1 case of optic nerve atrophy and the other case of optic canal not opening. Five cases with frontal sinus opened were repaired during the operation and there was no postoperative cerebrospinal fluid leakage or intracranial infection. One patient suffered from postoperative anosmia, one patient developed left limb weakness, but their symptoms have improved in the follow-up.CONCLUSION:Summarize the experience of our center and previous literature, unilateral forehead bottom craniotomy is a feasible surgical approach for giant tuberculum sellae meningioma. Intraoperative application of EC glue and pedicled fascia flap to repair the frontal sinus can prevent complications associated with frontal sinus opening. Optic canal unroofing has huge advantage in visual improvement.
A case-based review on the neuroendoscopic management of intraventricular and subarachnoid basal neurocysticercosis

Abstract:Objective: Extraparenchymal localization of neurocysticercosis (NCC) is rare in non-endemic areas. A case of mixed (intraventricular, IV, and subarachnoid basal, SAB) NCC was surgically treated using the neuroendoscope and a systematic review of the literature was performed with the aim to analyze the use of this instrument in the management of the extraparenchymal forms of the parasitic disease. Materials and methods: Medline and Embase databases were searched for studies where the neuroendoscope was used for the management of IV/SAB NCC cysts, either for the cerebrospinal fluid diversion or cyst removal. Cyst location, complete removal, cyst breakage during removal, intraoperative and postoperative complications, administration of antihelmintic therapy, outcome and follow-up period were extracted from the articles. Results: 281 patients were treated by means of the neuroendoscope. 254 patients who were described in retrospective cohort studies, came all from endemic areas, with no significant difference between sexes. Mean age at surgery was 30.7 years. Of all cysts reported in retrospective studies, 37.9% were located in the fourth ventricle. An attempt of cyst removal was described in the 84.6% of cases and an endoscopic third ventriculostomy was performed in another 76.4%. A small number of complications were reported intraoperatively (9.1%) obtaining, but a good recovery was achieved at follow-up. Only 17 ventriculoperitoneal shunts were placed after the first procedure, defining a low risk of postoperative hydrocephalus even in case of partial cyst removal. Conclusion: Neuroendoscopic removal of an extraparenchymal NCC cyst is a safe procedure that should be preferred for lateral and third ventricle localization and, in a specialized centre, even for a localization in the fourth ventricle if feasible. It is also efficient because of the possibility of performing an internal CSF diversion concomitantly to cyst removal, avoiding the complication registered with VPS. The need for cysticidal treatment after surgery should be addressed in a prospective study.

Abstract:Ventral medial pontine cavernous malformations are challenging due to the location in eloquent tissue, surrounding critical anatomy, and potential symptomatic bleeding. Conventional approaches, such as anterolateral, lateral and dorsal approach, are associated with high risk of deleterious consequences due to excessive traction and damage to the surrounding tissues. The authors present an endoscopic endonasal approach for the resection of midline ventral pontine cavernous malformations, which follows principles of optimal access to brainstem cavernous malformations as the "two-point method." No CSF leak or any other complications are obtained. The successful outcomes indicate that an individualized approach should be chosen before the surgery for brainstem cavernous malformations. With the advance of techniques, endoscopic endonasal approach could provide the most direct route to ventral pontine lesions with safety and efficiency.
Intracranial infection and acute obstructive hydrocephalus after lateral ventricle central neurocytoma resection: case report and review of the literature

Abstract:Background Central neurocytoma (CN) is a rare neurogenic intraventricular tumor that accounts for 0.25–0.5% of all intracranial tumors. Surgery is the cornerstone of therapy in most instances. Postoperative complications such as intracranial infection and hydrocephalus may be serious and even fatal. However, there are few reports on the management of complications after CN resection.Case presentation: The authors described a 41-year-old male patient, whose CN was near the interventricular foramen with a high Ki-67 index. Seven days following surgery, an intracranial infection struck the patient. On the seventeenth day, the patient experienced signs of obstructive hydrocephalus, including headache, unconsciousness, and gatism. One month after surgery, the patient recovered and was discharged with no sequela through high-grade antibiotics, lumbar cisterna drainage and external ventricular drainage.Conclusions For CN with elevated Ki-67 or growing in the lateral ventricle, particularly in the anterior region close to the interventricular foramen, obstructive hydrocephalus was more likely to be complicated after surgery. The temporary external ventricular drainage may be a substitute for VP shunt, avoiding maintenance risks and complications of VP shunt during long-term drainage. Besides, we also need to etiologically personalize the treatment approaches based on the various stages of patient for maximum clinical benefit.

Abstract:Background: Neurocysticercosis (NCC) is the commonest parasitic infection of the central nervous system. There is significant difference in incidence of NCC depending on geographic location, and incidence as high as 4% is reported in the endemic areas. It results from human affliction by the larval stage of Taenia solium. Spinal NCC is quite rare as compared with cranial NCC and accounts for 1.5%-3% of all cases. Both spine and cranium can be rarely involved in NCC, and cranial involvement usually precedes the spinal involvement. Case description: We report a case of a 51-year-old woman who had spinal involvement first in the form of spinal intradural extramedullary disease and then developed cranial involvement more than 1 year later. She developed spinal arachnoiditis. She developed communicating hydrocephalous requiring ventriculoperitoneal shunt. Later she developed isolated fourth ventricle and required excision of the fourth ventricular NCC. The unique aspects of our patient were a very aggressive course and involvement of cranium after spinal involvement. We describe her clinical course over 3 years and the management done. Conclusions: NCC can sometimes follow a very aggressive course and can involve both cranial and spinal compartments. Management of such patients is not standardized given the rarity of such cases.

Abstract:Background: Intraventricular cavernous malformations are unusual intracranial vascular malformations; their deep anatomical location complicates their surgical management. Microsurgical approaches are the gold standard approaches for the resection of ventricular lesions, however, they imply considerable neurovascular risks. Case description: A 51-year-old patient presented with acute headache, diplopia, vertigo, blurred vision, and a depressed level of consciousness. A ventricular hemorrhage was treated with a ventriculostomy and the patient was discharged without hydrocephalus. After 11 days, he developed ataxia, diplopia, and a depressed level of consciousness. The patient was diagnosed with hydrocephalus secondary to the previous third ventricle hemorrhage. An endoscopic exploration using a 30° rigid ventricular endoscope was performed; after the third ventriculostomy, an intraventricular cavernous malformation located on the floor of the third ventricle and the aqueduct of Sylvius was resected. Conclusions: Three days after the surgery, magnetic resonance imaging demonstrated a gross total resection and adequate third ventriculostomy flow. One year after the surgery, the patient was asymptomatic. Neuroendoscopy has evolved towards minimally invasiveness, and in selected cases is an equally effective surgical approach to ventricular lesions. It provides minimal cerebral cortex disruption and vascular manipulation.
Transcallosal Anterior Interforniceal Approach for Removal of Superior Midbrain Cavernous Malformations in Children: A Retrospective Series of 10 Cases in a Single Center

Abstract:BACKGROUND: Surgical treatment of brainstem cavernous malformations (CMs) remains a great challenge for neurosurgeons. Several cases published in the literature have addressed surgical approaches. However, no surgical approach has been reported to provide better exposure for CMs located in the superior midbrain. METHODS: We presented 10 cases of superior midbrain CMs in children treated in Beijing Tiantan Hospital from 2002 to 2016. The most common presenting signs and symptoms were hydrocephalus and cranial nerve deficit. We used the transcallosal anterior interforniceal approach in all patients to remove the CM lesion. RESULTS: Total resection was achieved in all patients. During the follow-up period, temporary short-term memory impairment was present in 5 patients, unilateral ptosis occurred in 1 patient, and upward gaze dysfunction occurred in 2 patients. There was complete neurologic functional improvement for cranial nerves. Exacerbation of hydrocephalus occurred in 4 patients requiring additional ventriculoperitoneal shunt postoperatively. CONCLUSIONS: The transcallosal anterior interforniceal approach can provide good exposure and direct visualization to superior midbrain CMs. Complications are shortterm and reversible.

Abstract:Two temporary occlusions of the bilateral common carotid artery for 15 min with an interval of 15 min induced severe brain tissue damage in normotensive Wistar rats. Although no lesions were seen after a single 15-min occlusion, severe ischemic lesions were found in 68% (49/72) of the rats injected to two consecutive occlusions for 15 min with an interval of 15 min, and the incidence was significantly higher than that in the rats subjected to a single 30-min occlusion (32%, 6/19) or two 15-min occlusions with an interval of 30 min (11%, 1/9). The local cerebral blood flow (lCBF) in the caudoputamen, however, was decreased by only 40-60% of pre-occlusion level during occlusions. These results suggest that intermittent incomplete disruption of CBF with short intervals could be more hazardous than a continuous interruption of the same duration.

Abstract:Racemose neurocysticercosis (RNC) is a malignant form of Taenia solium infection. It carries high mortality due to widespread intraparenchymal invasion, mass effect, and cyst rupture. Cerebellar RNC is unusual and constitutes a surgical challenge. Scarce applications of ultrasound (US) -guided resection have been reported for RNC of the posterior fossa. We report the case of a 66-year-old woman who presented with ataxia and dysmetria. Her past medical history was relevant for seizures and hydrocephalus secondary to neurocysticercosis. Because of the increasing cyst invasion and threatening mass effect in the posterior fossa, the patient underwent US-guided resection of lesions. Postoperative computed tomography (CT) demonstrated complete excision of cysts, and a 2-year follow-up magnetic resonance imaging (MRI) showed no recurrence. On neurological examination, the patient had persistent ataxia without new-onset neurological deficits. The present case study illustrates the feasibility and cost-effective approach of US-guided resection to provide enhanced operative visualization and achieve complete cyst resection.

Abstract:A brainstem tumor located at the upper pons or pontomesencephalic region is surgically challenging because of the deep-seated location and difficulty to approach, especially if the lesion is intra-axial and extends anteriorly without appearing on the pia surface. In our experience, the infracollicular entry point is feasible for such lesions and can be approached by an occipital interhemispheric transtentorial corridor through a straightforward trajectory, which ensures surgical ergonomics. Herein, we present a case of a 55-yr-old woman with a pontomesencephalic lesion removed via an occipital interhemispheric transtentorial infracollicular approach. Informed consent was obtained from the patient. Her preoperative medical course and radiological findings strongly indicated the lesion as a brainstem cavernous malformation. Although the overlying brainstem parenchyma was thin, the lesion did not appear on the pial surface. The lesion was removed via an occipital interhemispheric transtentorial infracollicular approach. During the operation, the lesion was observed to have an old hemorrhagic component and an obvious gliotic boundary, resembling the typical macropathology of a brainstem cavernous malformation. We easily dissected the lesion circumferentially off the brainstem parenchyma after thorough debulking, and a gross total resection was performed en bloc. However, postoperative pathology confirmed a diagnosis of a metastatic neuroendocrine tumor, and further systematic examination revealed cancerous lesions in the lungs. The patient experienced slight hypophrasia but recovered within 3 d postoperatively and then was discharged for further treatment. This case demonstrates the safety and efficacy of an occipital interhemispheric transtentorial infracollicular approach for brainstem tumor resection.

Abstract:Introduction and importance: Intracranial dermoid cysts are rare, constituting 0.04% to 0.6% of all intracranial tumors. They often arise from ectodermal cells trapped during neural tube formation. We report a case of spontaneous rupture of a large tentorial epithelioid cyst, which caused massive dissemination of liquid cholesterol into the subarachnoid cisterns and ventricles. Presentation of case: A 28-year-old male presented with a two-week history of headache and memory decline. CT and MRI revealed a 9x6 cm lesion in the left frontotemporal region with widespread dissemination of lipid droplets. Surgical resection was performed using a microscope combined with a neuroendoscope. Pathology confirmed a dermoid cyst. Clinical discussion: Ruptured dermoid cysts can cause significant symptoms due to the dissemination of cyst contents. Imaging is crucial for diagnosis and surgical planning. The combined microscopic and neuroendoscopic approach minimized blind spots and allowed thorough tumor exposure, facilitating complete resection with minimal residual complications. Postoperative outcomes were favorable, with imaging confirming substantial tumor removal and restored cerebrospinal fluid circulation. Conclusion: Prompt diagnosis and comprehensive surgical intervention are essential for managing ruptured intracranial dermoid cysts. Combined microscopic and neuroendoscopic techniques are effective in achieving extensive resection and reducing complications.

Treatment Outcome in Patients With Spinal Neurocysticercosis: A Systematic Review of Published Cases and Case Series

Abstract:Abstract Background Spinal neurocysticercosis is a rare central nervous system infection caused by the larval form of the Taenia solium tapeworm. Due to its rarity, most knowledge is derived from isolated case reports. Objectives This systematic review aims to evaluate existing case reports and observational studies to provide a comprehensive overview of the disease's clinical presentation, and treatment outcomes. Methods Adhering to PRISMA guidelines, a search was conducted across multiple databases including PubMed, Scopus, Embase, and Google Scholar. Case reports, case series, and observational studies were included. The review is registered with PROSPERO (CRD42024496957). Results The search yielded 163 records describing 197 patients. Symptoms ranged from one week to over three years, with the most common being paraparesis or quadriparesis (61%) and back pain. Treatment modalities varied. with a combination of surgery and cysticidal drugs being the most preferred (45.2%) treatment. Surgery was done in 77% (152/197) of cases. In 45 % of cases (89/197) cysticidal drugs were given following surgery. Medical treatment alone was given to 22% (43/197) of patients. The majority of cases (92%) irrespective of treatment modality showed clinical improvement. Post-operative complications caused three deaths. Conclusions We noted that surgery followed by cysticidal drugs was the most preferred treatment. Medical treatment alone was given to many patients. Clinical improvement was observed in most cases, regardless of the treatment option used. The use of cysticidal drugs could eliminate the need for surgery in many spinal neurocysticercosis patients.

[Microsurgical Treatment of Nonmeningeal Tumors of the Cavernous Sinus].

Abstract:OBJECTIVETo analyze the differential diagnosis of and effectiveness of surgical treatment on nonmeningeal tumors of cavernous sinus.METHODSThe clinical data, including clinical manifestations, diagnosis and differential diagnosis, operative procedure, and treatment results of 67 cases of nonmeningeal tumors of cavernous sinus, 32 males and 35 females, aged 41 +/- 16, who underwent microsurgical treatment were analyzed respectively.RESULTSThe main clinical presentation included cranial nerve paralysis (47 cases, 70.1%), neuroendocrine symptoms (11 cases, 16.4%), eye pain (9 cases, 13.6%), proptosis (7 cases, 10.6%), limb weakness (5 cases, 7.5%), headache, dizziness, lethargy, and temporal lobe epilepsy (4 cases each), and epistaxis (1 case). All patients had CT and/or MRI scanning before operation. Twenty-six patients had DSA examination and balloon occlusion test (BOT). All patients underwent orbito-zygomatico-pterional craniotomy. Twenty cases were operated on via intradural approach, 33 cases via epidural approach, and 14 cases via epidural/intradural combination approach most of which suffered from invasive pituitary adenoma and dumbbell type trigeminal neurinoma. Pathological examination revealed that hemangioma (24 cases), trigeminal schwannoma (18 cases), and invasive pituitary adenomas (11 cases) comprised the majority of tumors; the remaining tumor types were chordoma, chondroma, chondromyosarcoma, chondrofibroma, dermoid tumor, malignant nerve sheath tumor, metastatic tumor, and lymphoma. In the intradural approach group, total tumor removal was achieved in 8 cases (40%), and 7 (35%) cases and 5 (25%) cases had subtotal and partial resection respectively. In the epidural approach group, 27 cases (81.8%) had total tumor removal, while 1 case (3.0%) had subtotal and 5 cases had partial excision of the tumor. In the epiduro-transdural approach group 10 cases (71.4%) achieved total resection and 4 cases (28.6%) had subtotal resection. Postoperative follow-up lasting 6 months to 10 years showed that the pre-operative central nervous system symptoms recovered in 39 cases (58.2%). The aggressive or new symptoms mainly included paralysis of oculomotor nerve (13 cases, 19.4%), trigeminal nerve (9 cases, 13.4%), and abduct nerve (9 cases, 13.4%).CONCLUSIONDepending on pathologic type and growth pattern of the tumor, extended middle skull base epidural or epidural-transdural approach is selected. Such techniques can offer satisfactory outcome, protecting the cranial nerve functions and removing the tumor to the maximum extent.

We aim to provide the reader with a thorough analysis of the best available evidence regarding patient characteristics, optimal treatment modality, and outcomes in cases of spinal neurocysticercosis. Methods: A systematic review of the literature using PubMed, Google Scholar, and Web of Science electronic databases was made according to the PRISMA guidelines. An illustrative case of intramedullary-cervical spinal disease is also presented for illustrative purposes. Results: A total of 46 reports of 103 patients fitting the screening criteria were identified. Isolated spinal involvement was seen in 46.15% of patients. Most infections (76.92%) had an intradural extramedullary localization, with 43.27% of cases involving >1 spinal cord level. The most common presenting symptoms were motor deficits (77.88%), pain syndromes (64.42%), and sensory deficits (53.85%). Combined surgical resection and pharmacologic therapy was the most frequently used treatment modality (49.04%) and had the highest proportion of patients reporting symptomatic improvement at follow-up (78.43%). Combination therapy had a significantly higher rate of neurologic recovery compared with surgery alone (P = 0.004) or medical treatment (P = 0.035). Conclusions: Spinal involvement in neurocysticercosis should be considered in patients from or who traveled to endemic areas presenting with ring-enhancing lesions. Combined treatment with surgery followed by cysticidal and steroid medication seems to be superior to surgery or medical treatment in isolation and seems to provide the highest chances of recovery.
