Abstract:Objective To investigate the value of electrical flexible neuroendoscopic management of the fourth ventricular cysticercosis.Methods 2 cases of fourth ventricular cysticercosis were transaqueductally removed with an electrical flexible neuroendoscope.In one case,the preoperative MRI showed a cystic lesion in the fourth ventricle.In another case,the preoperative MRI showed the cystic lesion in the third ventricle,but during the course of exploration of ventricles under neuroeodoscope,the lesion was found to have migrated into the fourth ventricle.Results Complete excision of cysts in the fourth ventricle was performed in both patients with no significant operative or postoperative complications.Satisfactory postoperative CSF flow around foramen magnum was detected by cine phase-contrast magnetic resonance imaging.Conclusions The whole ventricular system can be explored easily with electrical flexible neuroendoscope.Electrical flexible neuroendoscopic management of the fourth ventricular cysticercosis should be the optimal choice due to its minimal invasion,less complications,shorter length of stay and cheaper treatment costs.

Abstract:Background: Neurocysticercosis (NCC) is a public health problem in most of the world. Approximately 50 million people worldwide experience this disease, and it remains one of the most important causes of neurological morbidity. Extraparenchymal NCC (basal cisterns) is associated with high rates of morbidity and mortality. Currently, minimally invasive approaches, including the endoscopic endonasal approach, are used with good functional results. Observations: A 25-year-old patient presented with headache and pain in the left hemiface. Magnetic resonance imaging (MRI) showed cystic lesions in the prepontine basal cisterns compressing and displacing the left trigeminal nerve. The cysticercal lesions were excised using a transclival endoscopic approach. There were no complications during surgery, and pathology confirmed the diagnosis of NCC. The pain subsided after surgery. No residual cysts were observed on control MRI. Lessons: Minimally invasive approaches can be used to treat tumor and infectious problems of the skull base, as in this case, with good functional results. https://thejns.org/doi/10.3171/CASE24223.
A case-based review on the neuroendoscopic management of intraventricular and subarachnoid basal neurocysticercosis

Abstract:Objective: Extraparenchymal localization of neurocysticercosis (NCC) is rare in non-endemic areas. A case of mixed (intraventricular, IV, and subarachnoid basal, SAB) NCC was surgically treated using the neuroendoscope and a systematic review of the literature was performed with the aim to analyze the use of this instrument in the management of the extraparenchymal forms of the parasitic disease. Materials and methods: Medline and Embase databases were searched for studies where the neuroendoscope was used for the management of IV/SAB NCC cysts, either for the cerebrospinal fluid diversion or cyst removal. Cyst location, complete removal, cyst breakage during removal, intraoperative and postoperative complications, administration of antihelmintic therapy, outcome and follow-up period were extracted from the articles. Results: 281 patients were treated by means of the neuroendoscope. 254 patients who were described in retrospective cohort studies, came all from endemic areas, with no significant difference between sexes. Mean age at surgery was 30.7 years. Of all cysts reported in retrospective studies, 37.9% were located in the fourth ventricle. An attempt of cyst removal was described in the 84.6% of cases and an endoscopic third ventriculostomy was performed in another 76.4%. A small number of complications were reported intraoperatively (9.1%) obtaining, but a good recovery was achieved at follow-up. Only 17 ventriculoperitoneal shunts were placed after the first procedure, defining a low risk of postoperative hydrocephalus even in case of partial cyst removal. Conclusion: Neuroendoscopic removal of an extraparenchymal NCC cyst is a safe procedure that should be preferred for lateral and third ventricle localization and, in a specialized centre, even for a localization in the fourth ventricle if feasible. It is also efficient because of the possibility of performing an internal CSF diversion concomitantly to cyst removal, avoiding the complication registered with VPS. The need for cysticidal treatment after surgery should be addressed in a prospective study.

Abstract:Background: Therapy for large or deep cystic brain metastases is a troublesome procedure in clinical departments. Stereotactic cyst aspiration, combined with Gamma Knife radiosurgery, can be an effective treatment for cystic brain metastases. However, there is still a possibility that a reaccumulation of cystic fluid may lead to poor efficacy or even reoperation. Case presentation: We present a case of a 67-year-old man who was diagnosed with lung cancer brain metastasis. The intracranial lesion seen on imaging appeared to be cystic and located deep inside the brain with associated limb dysfunction. The patient did not respond well to chemotherapy and underwent cyst aspiration with Ommaya reservoir implantation under neuronavigation. Repeated cystic fluid reaccumulation and exacerbation of symptoms occurred during treatment. We performed repeated aspiration via the Ommaya reservoir to control the symptoms and combined it with radiotherapy. During the follow-up period of 14 months, the intracranial tumor was effectively and satisfactorily controlled. Conclusions: We highlight that Ommaya reservoir implantation during stereotactic cyst aspiration is necessary to prevent fluid reaccumulation, thereby avoiding the need for a second surgical procedure.
Suprasellar Multiple Neurocysticercal Cyst Presenting with Visual Loss

Abstract:Suprasellar cysticercosis is a rare entity. Only a few cases are reported in the literature.1 Clinically, present with visual loss and endocrinopathy and radiologically mimic craniopharyngioma, cystic pituitary adenoma, and Rathke cleft cyst.2 We present the case of a 34-year-old woman with a history of diminution of vision for 2 months. On examination, her visual acuity was 6/9 bilaterally, per the Snellen chart. The visual field showed incomplete temporal hemianopia in the right eye and a severely depressed field in the left eye. She was evaluated with magnetic resonance imaging (MRI) of the brain, which showed multiple well-defined enhancing cystic lesions in the suprasellar cistern splaying the optic chiasm. Enzyme-linked immunoelectrotransfer blot testing was not done due to the lack of availability at our center. She underwent left pterional craniotomy and excision of the cysts. Multiple cysts were noted during surgery, and all were dissected from the surrounding structures. However, there was a spillage of cystic contents during excision, and they are not associated with any adverse reactions, unlike hydatid cysts.3 She recovered well. Histopathology suggestive of cysticercal cyst, no scolexes. As per Infectious Diseases Society of America, non-operated subarachnoid neurocysticercosis requires prolonged antiparasitic agents along with steroids.4 She received 6 weeks of albendazole 15 mg/kg/day and steroids tapered over 3 weeks. At 2 years of follow-up, her visual acuity improved to 6/6 with a normal visual field. Follow-up MRI showed complete removal of all cysts with no relapse. Early surgery is safe and effective. Visual outcome depends on the timing of the diagnosis and prompt intervention.
Microsurgical Management of Fourth Ventricle Lesions Via the Median Suboccipital Keyhole Telovelar Approach

Abstract:In this 2-year retrospective analysis, 13 patients with fourth ventricle lesions who underwent microsurgical resection via the midline suboccipital keyhole telovelar approach were analyzed. This is the first study to investigate the surgical outcome and complications of using this approach to resect various types of lesions in the fourth ventricle. We aimed to clarify whether this approach has met its promise of lesion dissection. Three patients (23.1%) had intraoperative extraventricular drains. There were no immediate postoperative deaths. Gross total resection was achieved in 84.6% of the cases. The Fisher exact test showed there was no statistically significant correlation between lesion location, lesion size, brainstem invasion, and extent of resection. About two third (69.2%) of the cases were free of complications. New or worsening gait/focal motor disturbance (15.4%) was the most common neurological deficit in the immediate postoperative period. One patient (7.7%) had worse gait disturbance/motor deficit following surgical intervention. Two patients (15.4%) developed meningitis. Two patients (15.4%) required postoperative cerebrospinal fluid diversion after tumor resection, of these 2 patients, 1 (7.7%) eventually needed a permanent shunt. There were no cases of cerebellar mutism and bulbar paralysis. The median suboccipital keyhole telovelar approach provides relative wide access to resect most fourth ventricle tumors completely and with satisfactory results. In contrast, this requires the appropriate patient selection and skilled surgeons.
Diagnosis and Micro-Neurosurgery for the Fourth Cerebral Ventricle Tumors

Abstract:OBJECTIVE:To investigate the diagnostic method and analyze the result of microneurosurgical treatment for tumors of the fourth cerebral ventricle.METHODS:Tumor of the fourth ventricle was clinically diagnosed in 86 patients basing on the preliminary assessment of symptom and CT or MRI findings. Of these 86 patients treated with micro-neurosurgery, the tumors in 62 were totally removed, subtotally in 19, and partially in 5. Forty-two patients received postoperative radiotherapy.RESULTS:Three patients died postoperatively within ten days, and symptoms in 83 were improved after treatment. The average survival period was over 3 years. The pathology included 32 medulloblastomas, 23 ependymoma, 15 astrocytoma, 10 hemangiblastomas, 2 choroid plexus papillomas, and 4 epidermoid cysts.CONCLUSION:Medulloblastoma, astrocytoma and hemangiblastoma are suggested to be removed totally whenever technically possible according to the site, character and volume of the tumor. For ependymoma, if close to the brain stem, is recommended to be subtotally removed. Postoperative radiotherapy may be beneficial for malignant types.
Endoscopic management of a cavernous malformation on the floor of third ventricle and aqueduct of Sylvius: Technical case report and review of the literature

Abstract:Background: Intraventricular cavernous malformations are unusual intracranial vascular malformations; their deep anatomical location complicates their surgical management. Microsurgical approaches are the gold standard approaches for the resection of ventricular lesions, however, they imply considerable neurovascular risks. Case description: A 51-year-old patient presented with acute headache, diplopia, vertigo, blurred vision, and a depressed level of consciousness. A ventricular hemorrhage was treated with a ventriculostomy and the patient was discharged without hydrocephalus. After 11 days, he developed ataxia, diplopia, and a depressed level of consciousness. The patient was diagnosed with hydrocephalus secondary to the previous third ventricle hemorrhage. An endoscopic exploration using a 30° rigid ventricular endoscope was performed; after the third ventriculostomy, an intraventricular cavernous malformation located on the floor of the third ventricle and the aqueduct of Sylvius was resected. Conclusions: Three days after the surgery, magnetic resonance imaging demonstrated a gross total resection and adequate third ventriculostomy flow. One year after the surgery, the patient was asymptomatic. Neuroendoscopy has evolved towards minimally invasiveness, and in selected cases is an equally effective surgical approach to ventricular lesions. It provides minimal cerebral cortex disruption and vascular manipulation.

Abstract:An important step in the performance of endoscopic resection of colloid cysts of the third ventricle is the forced aspiration of cyst contents. The different consistencies these cysts may have can limit their complete resection and increase the likelihood of complications. The introduction of the ultrasonic neuroendoscopic aspirator allows cysts to be emptied more easily than with a conventional rigid aspirator, improving the feasibility of resection even in more solid cysts. The ability to regulate ultrasound and aspiration increases safety in a reduced and highly morbid space such as the third ventricle. Our objective was to determine the safety and efficiency of the ultrasonic aspirator for endoscopic resection of colloid cysts of the third ventricle. This was a retrospective descriptive study of patients with colloid cysts of the third ventricle undergoing neuroendoscopic resection using an ultrasonic aspirator between 2016–2023. Clinical, radiological, and procedural variables were studied. Mean, median and range were analyzed for quantitative variables and percentages and frequencies for qualitative variables. We present a series of 11 patients with colloid cysts of the third ventricle. The mean age was 44 years (27–69). All had biventricular hydrocephalus, with a mean cyst diameter of 15 mm (9–20). The lateral ventricle was accessed using the transforaminal approach in seven patients and the transchoroidal approach in three patients. All patients underwent septostomy. The mean endoscopy time was 40 min (29–68). Complete resection was possible in 10 patients. Median follow-up was 16 months (1–65) with 100% clinical improvement. At the end of follow-up, no patient had recurrence of the lesion. Based on our experience, the ultrasonic aspirator can be used safely and effectively for the resection of colloid cysts of the third ventricle, achieving high rates of complete resection with minimal postoperative complications.

Ventriculoscopic Surgery For Arachnoid Cysts In The Lateral Ventricle: A Comparative Study Of 21 Consecutive Cases

Abstract:Objective: To evaluate neuronavigation-guided ventriculoscopic technique in the treatment of arachnoid cysts in the lateral ventricle. Methods: Between January 2008 to November 2011, twenty-one neuronavigation-guided ventriculoscopic surgery were performed for the treatment of symptomatic arachnoid cysts in 21 patients (14 male and 7 female patients, mean age 24.1 years [ranged 1.5-61 years]) Clinical presentations varied from headache, vomiting, hemiparesis and seizure. The trajectory of ventriculoscopy was dynamically monitored and guided in real time by neuronavigation system. Cysts fenestrations were performed in fourteen cases, and cysts resection in seven cases, respectively. All patients were prospectively had a regular follow-up. Results: After operation, all patients achieved symptom resolution without surgical mortality and morbidity. Aseptic meningitis was noted in four cases with cyst resection, and all recovered quickly without advanced treatments. However, a later ependymal adhesion, occurred in one case during follow-up period. Conclusion: The combination of ventriculoscopy and neuronavigation is an accurate, effective and safe approach for the treatment of the patients with arachnoid cysts in the lateral ventricle, especially, for overcoming the topographic variation caused by intraventricular pathologies. Cystoventriculostomy is the best choice.
[Microsurgical Treatment of Nonmeningeal Tumors of the Cavernous Sinus].

Abstract:OBJECTIVETo analyze the differential diagnosis of and effectiveness of surgical treatment on nonmeningeal tumors of cavernous sinus.METHODSThe clinical data, including clinical manifestations, diagnosis and differential diagnosis, operative procedure, and treatment results of 67 cases of nonmeningeal tumors of cavernous sinus, 32 males and 35 females, aged 41 +/- 16, who underwent microsurgical treatment were analyzed respectively.RESULTSThe main clinical presentation included cranial nerve paralysis (47 cases, 70.1%), neuroendocrine symptoms (11 cases, 16.4%), eye pain (9 cases, 13.6%), proptosis (7 cases, 10.6%), limb weakness (5 cases, 7.5%), headache, dizziness, lethargy, and temporal lobe epilepsy (4 cases each), and epistaxis (1 case). All patients had CT and/or MRI scanning before operation. Twenty-six patients had DSA examination and balloon occlusion test (BOT). All patients underwent orbito-zygomatico-pterional craniotomy. Twenty cases were operated on via intradural approach, 33 cases via epidural approach, and 14 cases via epidural/intradural combination approach most of which suffered from invasive pituitary adenoma and dumbbell type trigeminal neurinoma. Pathological examination revealed that hemangioma (24 cases), trigeminal schwannoma (18 cases), and invasive pituitary adenomas (11 cases) comprised the majority of tumors; the remaining tumor types were chordoma, chondroma, chondromyosarcoma, chondrofibroma, dermoid tumor, malignant nerve sheath tumor, metastatic tumor, and lymphoma. In the intradural approach group, total tumor removal was achieved in 8 cases (40%), and 7 (35%) cases and 5 (25%) cases had subtotal and partial resection respectively. In the epidural approach group, 27 cases (81.8%) had total tumor removal, while 1 case (3.0%) had subtotal and 5 cases had partial excision of the tumor. In the epiduro-transdural approach group 10 cases (71.4%) achieved total resection and 4 cases (28.6%) had subtotal resection. Postoperative follow-up lasting 6 months to 10 years showed that the pre-operative central nervous system symptoms recovered in 39 cases (58.2%). The aggressive or new symptoms mainly included paralysis of oculomotor nerve (13 cases, 19.4%), trigeminal nerve (9 cases, 13.4%), and abduct nerve (9 cases, 13.4%).CONCLUSIONDepending on pathologic type and growth pattern of the tumor, extended middle skull base epidural or epidural-transdural approach is selected. Such techniques can offer satisfactory outcome, protecting the cranial nerve functions and removing the tumor to the maximum extent.
Vertigo: An Atypical Presentation of Neurocysticercosis Successfully Treated With Albendazole

Abstract:A 20-year-old male presented to our facility with a worsening sensation of "the room spinning around" himself for the past three weeks. In the last week, he began to experience daily spells lasting for three hours each without losing consciousness. The patient had recently migrated from Central America six weeks prior to admission. On physical examination, his vital signs were within normal limits, with no focal neurological deficits. Magnetic resonance imaging (MRI) of the brain revealed a cystic-appearing lesion in the fourth ventricle with associated mass effect on the posterior aspect of the brainstem and mild periventricular edema. Laboratory studies were unremarkable except for a positive anti-cysticercus IgG antibody, which confirmed the diagnosis of neurocysticercosis. Initially, surgery was considered, but the neurosurgeons advised medical management due to the small size of the lesion. The patient was started on albendazole 400 mg orally twice daily and dexamethasone 6 mg orally daily for 14 days. The patient responded well; his symptoms resolved by the eighth day. He was discharged home to complete his treatment and remained asymptomatic at the follow-up appointment two weeks later.
Ultrasound-guided resection of cerebellar racemose neurocysticercosis: novel insights from a unique scenario

Abstract:Racemose neurocysticercosis (RNC) is a malignant form of Taenia solium infection. It carries high mortality due to widespread intraparenchymal invasion, mass effect, and cyst rupture. Cerebellar RNC is unusual and constitutes a surgical challenge. Scarce applications of ultrasound (US) -guided resection have been reported for RNC of the posterior fossa. We report the case of a 66-year-old woman who presented with ataxia and dysmetria. Her past medical history was relevant for seizures and hydrocephalus secondary to neurocysticercosis. Because of the increasing cyst invasion and threatening mass effect in the posterior fossa, the patient underwent US-guided resection of lesions. Postoperative computed tomography (CT) demonstrated complete excision of cysts, and a 2-year follow-up magnetic resonance imaging (MRI) showed no recurrence. On neurological examination, the patient had persistent ataxia without new-onset neurological deficits. The present case study illustrates the feasibility and cost-effective approach of US-guided resection to provide enhanced operative visualization and achieve complete cyst resection.

Abstract:A 10-year-old boy was admitted to the hospital with decreased muscle strength in lower limbs. Physical examination revealed isolated spastic crural paraparesis. MRI of the thoracic spine demonstrated a cystic intramedullary lesion (figure 1). The patient underwent complete microsurgical resection of the lesion. Anatomopathologic report (figure 2) confirmed cysticercosis. Intramedullary cysticercosis is a rare parasitic infection caused by Taenia solium (0.7%–5.8% of all patients with neurocysticercosis).1 This presentation is usually underdiagnosed because, in degeneration phases, it is difficult to visualize the parasite in the MRI, mainly the scolex, and immunologic tests are negative due to intraparenchymal location.2

Abstract:Cavernous malformations surrounding the fourth ventricle are challenging lesions to access and treat surgically owing to the complexity and eloquence of adjacent neural tissue [1] Long-standing practice included tissue transgression through the overlying cerebellar cortical surface of the hemisphere or vermis [1-3]. Using natural corridors such as tonsillobiventral fissure, cerebellomedullary fissure, and tonsillouvular fissure (TUF) offers elegant access to the fourth ventricle, avoiding traversing of neural tissue [4-7]. A 32-year-old male presented with headache, nausea, vomiting, double vision, and vertigo. Neuroimaging demonstrated a 17-mm diameter cavernous malformation protruding into the left lateral recess of the fourth ventricle. The patient consented for the procedure and underwent a middline suboccipital craniotomy in a prone position. TUF approach was performed by dissecting the arachnoid to the depth of the fissure, and after identifying the tonsillomedullary segment of the posterior inferior cerebellar artery, minimal white matter transgression was used to reach cavernous malformation. Complete removal of the lesion was achieved and confirmed on postoperative imaging. The postoperative course was uneventful. TUF approach with manipulation by ipsilateral and contralateral retraction of tonsills allows the widening of the surgical corridor and better exposure of lesions of the lateral recess of the fourth ventricle [1]. TUF approach is a valuable alternative to transvermian and transcerebellar approaches that minimize the division of neural tissue [6]. To the best of our knowledge this is the first case describing the TUF approach to exophytic cavernoma presenting in the lateral recess of the fourth ventricle. Under our institutional ethical review board regulations, approval was not necessary.

Abstract:Objective: Spinal involvement in neurocysticercosis is rare and can lead to debilitating injury if not diagnosed and treated early. We aim to provide the reader with a thorough analysis of the best available evidence regarding patient characteristics, optimal treatment modality, and outcomes in cases of spinal neurocysticercosis. Methods: A systematic review of the literature using PubMed, Google Scholar, and Web of Science electronic databases was made according to the PRISMA guidelines. An illustrative case of intramedullary-cervical spinal disease is also presented for illustrative purposes. Results: A total of 46 reports of 103 patients fitting the screening criteria were identified. Isolated spinal involvement was seen in 46.15% of patients. Most infections (76.92%) had an intradural extramedullary localization, with 43.27% of cases involving >1 spinal cord level. The most common presenting symptoms were motor deficits (77.88%), pain syndromes (64.42%), and sensory deficits (53.85%). Combined surgical resection and pharmacologic therapy was the most frequently used treatment modality (49.04%) and had the highest proportion of patients reporting symptomatic improvement at follow-up (78.43%). Combination therapy had a significantly higher rate of neurologic recovery compared with surgery alone (P = 0.004) or medical treatment (P = 0.035). Conclusions: Spinal involvement in neurocysticercosis should be considered in patients from or who traveled to endemic areas presenting with ring-enhancing lesions. Combined treatment with surgery followed by cysticidal and steroid medication seems to be superior to surgery or medical treatment in isolation and seems to provide the highest chances of recovery.
Microsurgical treatment of tumors in third ventricle

Abstract:Objective To investigate the surgical approaches and techniques of microsurgical treatment for tumors in the third ventricle.Methods A total of 36 patients with third ventricular tumors were treated by microsurgery using different surgical approaches.Fifteen cases were performed by transcallosal approach(10 cases by transforaminal approach and 5 cases by interforniceal approach),8 cases by infratentorial supracerebellar approach,8 cases by trans-lamina terminalis approach and 5 cases through superior frontal gyrus.Post-operative hydrocephalus happened in 7 cases.Treatment included ventriculoperitoneal shunting(V-P shunting) in 3 cases,third ventriculostomy in 1 case and septostomy in 1 case with lateral cerebral ventricle enlargement.The other 2 cases gave up further treatment.Results Total removal was achieved in 22 cases,subtotal removal in 8 cases,and partial resection in 6 cases.Three-month follow-up showed that there were good effect in 33 patients,poor effect in 1 patient and 2 patients died.Long-term follow-up(3 months to 7 years) showed that good effects were achieved in 26 cases and 8 patients died.Eleven patients need long-term endocrine replacement therapy.Conclusion Resection of third ventricle tumors is difficult,but combination of suitable surgical approach and experienced microsurgical techniques can gain good curative effects.
Treatment Outcome in Patients With Spinal Neurocysticercosis: A Systematic Review of Published Cases and Case Series

Abstract:Abstract Background Spinal neurocysticercosis is a rare central nervous system infection caused by the larval form of the Taenia solium tapeworm. Due to its rarity, most knowledge is derived from isolated case reports. Objectives This systematic review aims to evaluate existing case reports and observational studies to provide a comprehensive overview of the disease's clinical presentation, and treatment outcomes. Methods Adhering to PRISMA guidelines, a search was conducted across multiple databases including PubMed, Scopus, Embase, and Google Scholar. Case reports, case series, and observational studies were included. The review is registered with PROSPERO (CRD42024496957). Results The search yielded 163 records describing 197 patients. Symptoms ranged from one week to over three years, with the most common being paraparesis or quadriparesis (61%) and back pain. Treatment modalities varied. with a combination of surgery and cysticidal drugs being the most preferred (45.2%) treatment. Surgery was done in 77% (152/197) of cases. In 45 % of cases (89/197) cysticidal drugs were given following surgery. Medical treatment alone was given to 22% (43/197) of patients. The majority of cases (92%) irrespective of treatment modality showed clinical improvement. Post-operative complications caused three deaths. Conclusions We noted that surgery followed by cysticidal drugs was the most preferred treatment. Medical treatment alone was given to many patients. Clinical improvement was observed in most cases, regardless of the treatment option used. The use of cysticidal drugs could eliminate the need for surgery in many spinal neurocysticercosis patients.
