Abstract:Abstract Context and objective Approximately 5-10% of diffuse large B-cell lymphoma (DLBCL) patients carry MYC gene translocations (MYC-translocation+) with a poor prognosis after standard chemotherapy. MYC-translocation+ DLBCL patients carrying BCL2 translocations (MYC+/BCL2+ double-hit lymphoma) have a worse survival. The efficacy of adjuvant radiotherapy in this setting is unknown. The purpose of this study is to evaluate the efficacy of radiotherapy as a part of the therapeutic regimen for patients with MYC-translocation+ DLBCL. Patients and methods From the International DLBCL R-CHOP consortium program, we selected 581 patients with de novo DLBCL treated with standard R-CHOP immunochemotherapy (diagnosed and treated from 2000 to 2010). We excluded patients with transformed DLBCL, primary mediastinal, cutaneous, testicular or central nervous system large B-cell lymphomas, patients with HIV infection, and patients not treated with R-CHOP. The median follow-up was 54.9 months. Fluorescence in situ hybridization assessing MYC was performed for all the patients (n=581) and results were correlated with available clinical data to identify clinicopathologic features associated with MYC translocation, and to evaluate the prognostic significance of MYC translocations regarding overall survival (OS, from the time of diagnosis to death from any cause) and progression-free survival (PFS, from the time of diagnosis to relapse or death from any cause). In the MYC-translocation+ DLBCL group, 38 patients received chemotherapy alone and 21 patients received chemotherapy with adjuvant radiation therapy. The clinicopathologic features and survival of MYC-translocation+ DLBCL patients treated with (n=21) and without radiotherapy (n=38) after immunochemotherapy were compared to in order to evaluate the radiotherapy efficacy and other confounding factors. Results MYC translocations were detected in 59 DLBCL patients (10.2%). Patients with MYC-translocation+ DLBCL more often had bulky tumors, involvement of multiple extranodal sites, and poorer OS (hazard ratio [HR]: 2.0, 95% confidence interval [CI]: 1.20 - 3.35, P= .0083) and PFS (HR: 1.96, 95% CI: 1.22 - 3.13, P= .0005) independent of the International Prognostic Index score. Poor survival was primarily attributable to patients with MYC+/BCL2+ double-hit DLBCL who were predominantly of germinal center B-cell subtype. Among MYC-translocation+ DLBCL patients, a better survival was achieved in patients who received radiotherapy (for OS, HR: .32, 95% CI: .15 - .71, P= .0049; for PFS, HR: .35, 95% CI: .17 - .73, P= .0043). Conversely, radiotherapy abolished the adverse impact of MYC translocations. In addition, radiotherapy was associated with better survival in the subset of patients with MYC+/BCL2+ double-hit lymphoma (P = .017 for OS, and P = .05 for PFS). However, owing to the common use of radiotherapy as consolidation therapy, the favorable prognoses in the group of patients who received radiotherapy could also be attributed to limited-stage disease and more frequent complete remission (CR) after first-line treatment and therefore these factors confound interpretation of the data. To address these issues, we evaluated radiotherapy efficacy in separate patient groups: patients who achieved CR, non-CR (PR/SD/PD) patients, and patient with stage I/II, or stage III/IV disease. The efficacy of radiotherapy appeared more apparent in patients with advanced disease who did not achieve CR after first-line chemotherapy. Multivariate analysis after adjustment for stage and IPI score validated that radiotherapy significantly improved OS (HR: .28, 95% CI: .10 - .81, P= .018) and PFS (HR: .32, 95% CI: .13 - .80, P= .015) of MYC-translocation+ DLBCL patients. Conclusions The presence of MYC translocations in DLBCL is an important biomarker that facilitates prognostic prediction and treatment stratification independent of the IPI score. For chemoresistant MYC-translocation+ DLBCL patients, radiotherapy seems to be an effective adjuvant regimen likely due to the higher frequency of extranodal involvement and bulky disease in MYC-translocation+ DLBCL patients. Our results provide a rationale for larger scale studies to assess the potential role of radiotherapy in the management of MYC-translocation+ DLBCL patients, particularly patients with MYC+/BCL2+double-hit DLBCL. An Escalated Dose of Bortezomib Plus Second Line Chemotherapy for the Treatment of Patients with Relapsed or Refractory Diffuse Large B-Cell Lymphoma,

Abstract:Abstract Abstract 3714 Background: The non-GCB subtype of diffuse large B-cell lymphoma(DLBCL) has inferior response to rituximab based chemothrapy. Recent reports show a constitutional expression of NF-κB pathway in non-GCB subtype of DLBCL. Bortezomib, as proteasome inhibitor, has been shown a promising new agent for the treatment of DLBCL. As how the efficacy, doses and protocol of Bortezomib need to be clarified. Objective: This trial is a pilot multicenter clinical study to evaluate the efficacy and safety of an escalated dose of bortezomib based chemotherapy for the treatment of patients with relapsed or refractory non-GCB subtype of DLBCL. Patients and Materials: A total 24 pts with DLBCL were enrolled in 4 different medical centers. According to Hans' Tissue Microarray (TMA) Classification, 23 pts were diagnosed as non-GCB subtype, and 1 patient have not done the test. 16 pts had relapsed or refractory disease, while 8 pts had fail response to the first line treatment. All the patients had received Rituximab based chemotherapy previously. There were 16 male and 8 female. The patient age ranged from 19 to 73, of which the median age was 55. Methods: All patients were given bortezomib 2.0mg/m2, day 1, I.V. (intravenous injection 12 hours before chemotherapy), 5 pts were given additional dose of 0.7 mg/m2, I.V., at day 8. The combination protocols include: Bortezomib(V)+ICE(G) 13 pts; Bortezomib(V)+HyperCVAD 7 pts; Bortezomib(V)+EPOCH 3 pts; Bortezomib(V)+DHAP 1 pts. The patients were given 1 to 5.courses differently. Results: The follow-up time were 2 to18 months, with a median time of 8 months. Of 24 pts, 21 evaluable pts received more than 2 courses of therapy: 7 pts achieved complete remission (CR 33.3%), 9 pts achieved partial remission (PR 42.8%), 5 pts had no response (NR 23.8%), The overall response rate (ORR) was 76.1%. Of 16 responsive pts, the PFS were 2 to 18 months; the average PFS was 7.6 months and median PFS was 7.5 months. Up to now, 13 pts were still alive, and 7 pts were dead, and 1 pts lost follow-up. Of 7 death, 5 were caused by disease progression with 4 pts of central nervous infiltrates, 2 were caused by severe infections. The one year overall survival rate (OS) was 65%. Of all the 24 pts, 10 pts had 3 to 4 grade of myelosuppression; 1 case with severe pulmonary infection;1 case with septicemia; 1 case with skin and soft tissue infection; 1 case with fungus infection; 3 cases with herpes zoster infection; 2 cases with skin rashes; 2 cases with hypotension, 1 case with hepatic dysfunction; 1 case with neuralgia. Conclusion: Our study showed that the escalated dose of bortezomib based chemotherapy had promising response for the treatment of relapsed or refractory non-GCB originated DLBCL. Bortezomib at a single dose of 2.0mg/m2 was safe and tolerable. No acute toxicity or vital adverse events were observed. The relapse of disease in central nervous system as well as infections were relatively common and might need further study. Disclosures: No relevant conflicts of interest to declare.

Abstract:Background: Radiotherapy (RT) is a modality of salvage therapy in relapsed/refractory (R/R) diffuse large B-cell lymphoma (DLBCL), but its efficacy is currently not well defined. This paper reports a retrospective review of patients who received salvage RT for R/R DLBCL in our hospital. Methods: We selected 32 patients who had relapsed and had progressive disease after chemotherapy or partial remission (PR) after chemotherapy. The patients had a median age of 47 years (range, 13-85 years) and were treated between January 1, 2009, and June 30, 2016. The histological type was DLBCL in all cases. Progression-free survival (PFS) and overall survival (OS) were estimated with the Kaplan-Meier method; predictors for adverse factors were evaluated using a Cox proportional hazards regression model. Results: Median age-adjusted International Prognostic Index (IPI) score was 3 (range, 0-4), and 20 (62.5%) patients had large tumors. Patients were irradiated with a median dose of 42.7 Gy (range, 30-54 Gy): 4 (12.5%) by conventional RT, 2 (6.3%) by conformal 3D technique and 26 (81.3%) by intensity-modulated RT (IMRT). Most toxicities were mild (CTCAE grade 1 or 2), including neutropenia, diarrhea, dermatitis, mucositis and dysphagia. With a median follow-up of 25.4 months (range, 0.4-98.9 months) after irradiation, the 5-year PFS and OS were 61.8% and 83.2%, respectively. In multivariate analysis, adverse factors associated with PFS in our cohort were multiple lesions. Conclusions: Due to its low toxicity and ease of use, RT should remain a salvage therapy option for patients with R/R DLBCL.
A multi-institutional analysis of diffuse large B-cell lymphoma (DLBCL) treated with consolidative radiotherapy and the impact of cell-of-origin on outcomes

Abstract:Background Patients with diffuse large B-cell lymphoma (DLBCL) with bulky disease and/or those who fail to achieve complete response benefit from the addition of radiotherapy (RT). We aim to review the outcome, as well as determine the impact of cell-of-origin, on patients undergoing consolidative RT. Patients and methods Patients with DLBCL treated with radical intent consolidative RT were included. Clinical, pathological and treatment characteristics were extracted from electronic medical records. Survival outcomes and factors that predict for disease-free survival (DFS) were analysed. Results Seventy-four patients were included in this analysis. The median follow up was 3 years (0.7-16 years). Fifty-eight percent of patients had stage I-II disease, and 61% received at least 6 cycles of chemotherapy. Cell-of-origin was discernible in 60% of patients, and approximately half were classified as Germinal centre origin. The 5-year overall survival (OS) of this group was excellent at 92% (median survival not reached). The 5-year DFS was 73% (95% CI 57-83%). Seven percent (n = 5) of patients experienced local recurrence at a median time of 6 months. Failure to achieve complete response post RT and/or initial bulky disease are significant predictors of inferior DFS. There was no association between cell-of-origin and DFS or OS. Conclusions The outcome of patients who received radiotherapy as consolidation is excellent. Patients who fail to achieve complete response after radiotherapy had poorer outcomes. Despite using radiotherapy, presence of bulky disease remains a significant predictor of disease recurrence. We did not find any association of poorer outcomes, with regards to cell-of-origin, in the use of consolidative RT.

Abstract:The c-abl proto-oncogene encodes a cytoplasmic tyrosine kinase which is homologous to the src gene product in its kinase domain and in the upstream kinase regulatory domains SH2 (src homology region 2) and SH3 (src homology region 3). The murine v-abl oncogene product has lost the SH3 domain as a consequence of N-terminal fusion of gag sequences. Deletion of the SH3 domain is sufficient to render the murine c-abl proto-oncogene product transforming when myristylated N-terminal membrane localization sequences are also present. In contrast, the human BCR/ABL oncogene of the Philadelphia chromosome translocation has an intact SH3 domain and its product is not myristylated at the N terminus. To analyze the contribution of BCR-encoded sequences to BCR/ABL-mediated transformation, the effects of a series of deletions and substitutions were assessed in fibroblast and hematopoietic-cell transformation assays. BCR first-exon sequences specifically potentiate transformation and tyrosine kinase activation when they are fused to the second exon of otherwise intact c-ABL. This suggests that BCR-encoded sequences specifically interfere with negative regulation of the ABL-encoded tyrosine kinase, which would represent a novel mechanism for the activation of nonreceptor tyrosine kinase-encoding proto-oncogenes.

Abstract:Abstract Background As the standard practice of our institution, 60Gy is prescribed to PGTV and 54Gy to PTV in the radical radiotherapy of non-small cell lung cancer (NSCLC). We estimate whether this practice could achieve similar tumor control, and protect lymphocyte at the same time. Methods Failure patterns of 46 stage III NSCLC patients received radical radio-chemotherapy were analyzed. Target delineation criterion were as follows: GTV include primary tumor and involved lymph nodes, PGTV expanded from GTV with 5-8mm to compensate for genomic uncertainty. CTV include high-risk area, PTV was extended from CTV with set-up error. 60Gy was prescribed for PGTV, 54Gy for PTV over 30 fractions. The relationship between lymphopenia during radiotherapy and dose-volume parameter was evaluated using Spearman’s correlation analysis. Results With median follow-up of 21.06 months, 22 local-regional recurrence were evidenced, 13 patients have in-field recurrence, one patient failure marginally, and the other 8 patients have out-of field recurrence. Lymphocyte is the most sensitive cell to radiation, lymphopenia during radiotherapy was associated with both PTV(r = 0.489, p = 0.003) and PGTV(r = 0.313, p = 0.076), with larger volume predicted severe lymphopenia. In addition, we observed Lung V(5), V(10) and most of heart or aorta DVH parameters (from V10-V50) are important predictors for lymphocyte nadir. Conclusions Delivered 54Gy to subclinical lesions does not compromise marginal recurrence risk, at the same time lower severe radiation-induced lymphopenia risk. This finding supports further exploration of dosage reduction to CTV in locally advanced NSCLC.
A Dosimetric Analysis of Single Fraction Lattice Radiotherapy as a Boost for the Palliative Treatment of Bulky Head and Neck Tumors

Abstract:Purpose/Objective(s) This study investigates the dosimetry achieved in VMAT/IMRT-planned lattice radiotherapy (LRT) for bulky, palliative head and neck tumors. LRT allows for the delivery of higher radiation doses without dramatically increasing toxicity by using high-dose spheres within the tumor while directing high dose away from OAR's. Our objective was to analyze LRT using 3 dose levels as a boost for palliative treatment of bulky head and neck tumors. The clinical goal of LRT boost would be to increase the effectiveness of treatment while minimizing the burden of care for patients requiring a quick palliation of symptoms associated with bulky tumors. Materials/Methods Six bulky palliative head and neck cases were used for this study. A 3D lattice of 0.75 cm spheres spaced 0.85 cm apart was applied to the planning scan and restricted to the area inside the lattice inward contour (GTV was contracted by 0.5 mm). Each case was planned with three different dose prescriptions (15 Gy, 20 Gy and 25 Gy in a single fraction) to the lattice of spheres. In addition, a standard IMRT plan was also created to deliver a uniform 20 Gy in 5 fractions to a defined PTV. The margins (CTV + PTV expansions) used were between 0.5 to 1 cm. A total of 7 plans were created for each case, which included the three single fraction doses, the five fractions of standard treatment, and a combination of the single and five fraction plans. Results The average planned tumor volume was 209.77 mL (range 137.80 – 411.01). The number of spheres used in the LRT plan ranged from 13 to 41. The median maximum dose (Dmax) and dose statistics achieved are listed in Table 1. The median ipsilateral carotid dose was high for the 25 Gy (single fraction) plan, so our results suggest that a neoadjuvant LRT boost with of a single fraction of 20 Gy would likely be safe. Conclusion Lattice radiotherapy is a viable treatment option for bulky head and neck cases treated with palliative intent. We were able to achieve acceptable dosimetric plans in both the 15 and 20 Gy boosts (single fraction) followed by 20 Gy in 5 fractions, a combination that would be expected to provide more favorable tumor control and symptom relief than traditional palliative regimens of 20-30 Gy. A clinical application with prospective dose escalation trial is under consideration.

Abstract:Background: The most common aggressive lymphoma in adults is diffuse large B-cell lymphoma (DLBCL). Consolidative radiotherapy (RT) is often administered to DLBCL patients but guidelines remain unclear, which could lead to unnecessary RT. We aimed to evaluate the value of end-of-treatment PET-CT scans, interpreted using the Deauville score (DV), to guide the utilization of consolidative RT, which may help spare low-risk DLBCL patients from unnecessary RT. Methods: We included all DLBCL patients diagnosed between 2010 and 2022 at the National Cancer Centre Singapore with DV measured at the end of the first-line chemoimmunotherapy. The outcome measure was time-to-progression (TTP). The predictive value of DV for RT was assessed based on the interaction effect between the receipt of RT and DV in Cox regression models. Results: The data of 349 patients were analyzed. The median follow-up time was 38.1 months (interquartile range 34.0-42.3 months). RT was associated with a significant improvement in TTP amongst the DV4-5 patients (HR 0.33; 95%CI 0.13-0.88; p = 0.027) but not the DV1-3 patients (HR 0.85; 95%CI 0.40-1.81; p = 0.671) (interaction's p = 0.133). Multivariable analysis reported that RT was again significantly associated with improved TTP among the DV4-5 patients (adjusted HR 0.29; 95%CI 0.10-0.80; p = 0.017) but not the DV1-3 group (HR 0.86; 95%CI 0.40-1.86; p = 0.707) (interaction's p = 0.087). Conclusion: Our results suggests that DLBCL patients with end-of-treatment PET-CT DV1-3 may not need consolidative RT. Longer follow-up and prospective randomized trials are still necessary to investigate long-term outcomes.
Intensity Modulated Radiation Therapy for Gastric Diffuse Large B-Cell Lymphoma in the Rituximab Era: Dosimetric Analysis, Clinical Outcome, and Long-term Quality of Life

Abstract:Clinical and dosimetric benefits of intensity modulated radiation therapy (IMRT) for localized primary gastric diffuse large B-cell lymphoma (PG-DLBCL) have not been fully addressed, especially in the rituximab era. This retrospective study evaluated dosimetric parameters, treatment outcomes, toxicities, and quality of life (QOL) of IMRT for patients with localized PG-DLBCL. Forty-six patients with localized PG-DLBCL underwent IMRT after RCHOP or RCHOP-like chemotherapy (71.7% of patients had rituximab). Fifteen patients (38.5%) were classified as the germinal center B cell-like (GCB) subtype and 24 patients (61.6%) as the non-GCB subtype. Dosimetric parameters of the planning target volume (PTV) and organs at risk were assessed. Overall survival (OS), progression-free survival (PFS), and locoregional control (LRC) were analyzed using the Kaplan-Meier method. At the last follow-up, patients alive without disease were invited to fill out the EORTC Quality of Life Questionnaire specified for stomach cancer (QLQ-C30-STO22). The median mean dose of PTV was 41.6 Gy. Only 0.16% and 0.73% of PTV received <90% and <95% of prescribed dose, respectively. Both dose conformity and homogeneity of PTV were good, with median conformity index (CI) of 0.87 and homogeneity index (HI) of 0.12, respectively. The median mean dose of the both kidneys, left kidney, right kidney, and liver was 12.2 Gy, 12.8 Gy, 12.2 Gy, and 16.7 Gy, respectively. The 5-year OS, PFS, and LRC rates for all patients were 80.2%, 77.2%, and 93.1%, respectively. Following chemotherapy, consolidation radiation therapy in patients with complete/partial response resulted in significantly better survival than salvage radiation therapy in patients with stable/progressive disease (OS and LRC: 90% and 97.4% vs 0% and 53.3%). The GCB subtype was related to superior OS (100% vs 62.2%, P = 0.009) and PFS (100% vs 55.6%, P = 0.015). One patient with chronic hepatitis B died of acute liver failure after 4cycles of RCHOP and 20.8Gy irradiation to liver in mean dose, and another patient with cryptogenic hepatic cirrhosis experienced grade 4 late liver dysfunction after 5 cycles of RCHOP and 22.9Gy irradiation to liver in mean dose. No other serious acute or late hepatic dysfunction, nephropathy, or gastrointestinal toxicity was observed. Thirty-three out of 35 patients (94.3%) who were alive without disease completed QLQ-C30-STO22. The global functional QOL score was 82, and all functional aspects were excellent with scores above 90. Most symptom scales were negligible with scores less than 10. IMRT achieves excellent target coverage and dose conformity. IMRT after RCHOP chemotherapy yielded favorable treatment outcomes with acceptable toxicities and good long-term QOL for localized PG-DLBCL patients, but it should be carefully delivered to patients with chronic liver diseases.

Abstract:Abstract Immune escape from graft-versus-tumor (GVT) contributes to relapse after allogeneic stem cell transplantation (SCT). High-dose radiation (XRT) mobilizes a tumor-specific immune response following direct damage to the tumor and microenvironment. Radiation's abscopal effect is also immune mediated. We hypothesized that local radiation of relapsed tumor would rekindle a GVT effect and yield abscopal tumor responses. We tested the safety and systemic effects of single-fraction XRT followed by donor lymphocyte infusion (DLI) in patients with lymphoid malignancy and refractory relapse after SCT. Eligible subjects (Table) had tumor progression following donor engraftment, withdrawal of immune suppression and/or DLI, and XRT-amenable tumor. Subjects received 8 Gy XRT to 1 or more tumors; all but 1 (with chronic GVHD) received DLI 24 hours later. Clinical monitoring included marrow function, GVHD, XRT toxicity and tumor response. Correlative monitoring for systemic immunologic and GVT effects included FDG-PET imaging, serial peripheral blood sampling, and biopsy of radiated and nonirradiated tumor. Immune effects were characterized by flow cytometry and gene expression analysis. Local XRT toxicity of Grade 3 or less presented as expected in all subjects. Grade 4 neutropenia (2) was transient and GCSF-responsive; GVHD flare was not observed. Unexpected toxicity was observed in 1 subject who developed Grade 4 diffuse alveolar hemorrhage and Grade 3 cardiomyopathy that responded to therapy. Sustained response in radiated sites was observed in 6 of 8 evaluable subjects (nonevaluable: 1 MM bony lesion; 1 subject received further Rx). RECIST responses outside the radiation field included 2 PR and 5 SD. 2 subjects with SD had regression after initial tumor flare. 3 of 4 regressions were transient, with progression by 6 months; 3 subjects demonstrated progression. A single subject who received radiation alone demonstrated systemic tumor response as well as improvement in chronic GVHD (sclerotic skin). By FDG-PET, all subjects' radiated tumors showed substantially decreased uptake at D28; SUV changes were variable at D7. Changes in non-XRT tumors demonstrated far greater variability, even within single subjects. Consistent with systemic immune activation resulting from radiation-DLI therapy, within 1 week, peripheral blood T cell proliferation (%Ki-67+) increased significantly in CD4 and CD8 T cells, as well as FoxP3+ Tregs (Wilcoxon paired nonparametric analyses; p =.004, .004 and .01, respectively). A trend (p=.078) toward increased frequency of Th1 effector CD4 cells (T-bet+CD28-) was also observed. Comparison of gene expression in nonirradiated (abscopal) tumor in 4 patients collected pre and 1 month post XRT was consistent with an IFN-mediated response to cell damage. Genes were upregulated for receptors for nuclear materials released by damaged cells (TLR2, TLR4, CLEC4E), high- and low- affinity FcgR (FCGR1A and FCGR2A) used for phagocytosis, and an annexin receptor for phagocytic cells (FPR2). The inflammasome component genes NLRP3 and CASP1 were upregulated, as was IFIT1, a Type 1 IFN inducible gene. After SCT, XRT results in local tumor regression; abscopal responses were also observed, with systemic immune responses suggested by T cell proliferation in the peripheral blood and upregulation of tissue damage receptors and IFN-inducible genes in nonirradiated tumor. Systemic tumor responses were delayed, with peak regression occurring 3-4 months after XRT, consistent with an immune-mediated effect. GVHD was not observed; significant, reversible pulmonary and marrow toxicities were infrequent. Transient abscopal responses in these DLI-refractory patients suggest plausible synergy between radiation and allogeneic GVT effects. Predictors of Local Control with Palliative Radiotherapy for Multiple Myeloma

Abstract:Introduction We performed a retrospective analysis of patients with multiple myeloma (MM) receiving palliative radiotherapy (RT) and assessed factors associated with local control, with a focus on dose/fractionation and cytogenetics. Materials and Methods We included patients who received palliative RT for MM at our institution. Cytogenetics were collected via fluorescence in situ hybridization (FISH). Follow-up imaging was used to assess local control. Results A total of 239 patients with 362 treated lesions were included. Eighty-six (36.0%) patients had high-risk cytogenetics. Most lesions received 20 Gy in 5 fractions (131, 36.2%), 8 Gray (Gy) in 1 fraction (93, 25.7%), or 30 Gy in 10 fractions (48, 13.3%). At a median follow-up of 4.3 years, 4-year local progression was 13.4% (95% CI: 10.3-17.5). No cytogenetic abnormalities were correlated with local progression, nor were double- and triple-hit status. There was a nonsignificant trend towards association between number of treated lesions and local progression [HR for >3 versus 1: 2.43 (95% CI: 0.88-6.74), p =.059]. Among patients with >3 treated lesions, equivalent dose in 2 Gy fractions ≥20 Gy reduced progression [HR: 0.05 (95% CI: 0.01-0.23), p =.0001]. Conclusion In this large study of patients with MM, modern palliative RT achieved excellent rates of long-term local control. Although there was no dose-response observed in the overall cohort, patients with high volume symptomatic disease may benefit from EQD2 ≥20 Gy. High-risk cytogenetics did not appear to influence radioresponsiveness, and standard radiation doses appear to be effective for all MM patients regardless of cytogenetics. Abstract:Introduction: Prognosis in patients (pts) with r/r DLBCL remains unsatisfactory. Pts at risk for treatment-failure cannot accurately be identified by current prognostic factors and are usually treated with standard salvage therapies. Therefore, identification of new prognostic factors in r/r DLBCL is essential for optimizing therapy with risk-adapted approaches. Metabolic tumor volume (MTV) is a strong independent prognostic factor in lymphoma, although, data in r/r DLBCL is limited. We explored the prognostic value of pre-salvage therapy PET/CT metrics in this population. Methods: We performed a retrospective intent-to-treat analysis of pts with r/r DLBCL treated with platinum based-chemotherapy at Sylvester Comprehensive Cancer Center (6/2010 to 3/2020). Only pts having measurable disease on FDG PET/CT prior to the initiation of salvage chemotherapy were included. MTV was obtained by summing the metabolic volumes of all individual lesions using the 41% of SUVmax threshold. Total lesion glycolysis (TLG) was calculated as the product of MTV and SUVmean. The individual lesion or coalesced lesion with the maximum TLG was classified as TLGmax. The global TLG (sum of each individual lesion TLG) was evaluated as well. MTV, TLGmax, TLG global and SUVmax were calculated pre and post-salvage chemotherapy by a board-certified nuclear radiologist. Hermes Hybrid 3D software was implemented for these calculations. Statistical analysis methods included nonparametric Wilcoxon rank-sum test, logistic regression, and Cox's proportional hazards regression. Results: A preliminary cohort of 79 pts was evaluated. Pts characteristics were as follows: median age 59 (range 20 to 82) years, Karnofsky performance status (KPS) ≥ 80: 71 (90%), elevated LDH: 42 (53.2%), stage III-IV: 64 (81%), and second-line age-adjusted IPI (sAAIPI) 0-1: 42 (53.2%) 2-3: 37 (46.8%). Distribution of salvage chemotherapy regimens was R-ICE 55 (70%), R-DHAP 13 (16%), R-Gem-Ox 10 (13%), and R-ESHAP: 1 (1%) pts. Treatment response was CR 30 (38%), PR 29 (36.7%) and progression of disease in 20 (25.3%) pts. After salvage chemotherapy 60 (76%) pts proceeded with cellular therapy [stem cell transplant (SCT): 41 (52%), CAR-T: 12 (15%), and SCT and CAR-T: 7 (9%) pts]. Using the 41% SUVmax threshold, pts obtaining CR after salvage chemotherapy exhibited lower baseline values vs. those obtaining PR as evidenced by MTV (median 20 vs. 88 ml, P<0.001), TLGmax (88.5 vs. 411.2, P<0.001) and TLG global (142.1 vs. 828.2, P<0.001). TLG performed better in identifying pts obtaining CR vs. those presenting with progression of disease after salvage chemotherapy (TLGmax: 88.4 vs. 491.6, P=0.043; TLG global: 142.1 vs. 946. 7; P=0.034). Baseline SUVmax was similar in CR and PR (median 13.6 vs. 15.5, P=0.467) pts. Implementing logistic regression models, MTV ≥ 54.2ml pre-salvage chemotherapy was a significant predictor of failure to achieve CR (Odds ratio [OR]= 7.89 (95%CI: 2.58-24.16); P<0.001; AUC_ROC=0.723). MTV retained its prognostic value even after adjustment with sAAIPI (OR= 7.11 (95%CI: 2.29-22.09); P<0.001; AUC_ROC: 0.748). As analyzed by intent to treat, with a median follow up of 18.8 months, the Kaplan-Meier estimates of the proportion of pts alive and progression-free (PFS) and of the overall survival (OS) at 2 years were 32.8% (95%CI: 22.6% to 43.5%) and 43.2% (95%CI: 31.8% to 54.1%), respectively. In models including one PET/CT metric variable and adjustment for sAAIPI, pre-salvage therapy MTV ≥ 88.5ml and TLGmax ≥ 43.2 were significant predictors of worse PFS (HR=1.91, P=0.026 and HR=2.20, P=0.033) and OS (HR=2.26, P=0.012 and HR =2.88, P=0.017), respectively. Figures A and B show Kaplan-Meier plots for PFS and OS by low-high MTV groups. Conclusions: Pre-salvage MTV appropriately identified pts with r/r DLBCL treated with platinum-based chemotherapy at risk for treatment failure. Furthermore, MTV and TLGmax identified a high-risk group of pts associated with shorter survival. This data can be of used to determine which pts achieve maximal benefit with a standard program of 2-3 cycles of salvage chemotherapy followed by ASCT. Updated results on salvage therapy response and survival analysis will be presented at the meeting. Prognostic value of PET/CT metrics in r/r DLBCL should be confirmed in prospective clinical studies with the ultimate goal of developing strategies for an individualized approach. Abstract:Introduction: The impact of bridging radiation therapy (bRT) for CAR T-cell therapy on absolute lymphocyte count (ALC) kinetics and treatment outcome is unknown. Methods: We retrospectively reviewed adults with relapsed/refractory aggressive large B-cell lymphoma (LBCL) who received bRT prior to CD-19 CAR-T between 11/2017-4/2023. The change in ALC (ALC Δ RT) was computed by subtracting ALC pre- and post-bRT. Percent bone marrow (%BM) irradiated was calculated by estimating skeletal BM distribution. PFS, DSS, and OS were modeled via Kaplan-Meier. Results: Fifty-one patients received bRT, of which 13 (25.5%) had bulky disease (≥7.5cm). The median bRT dose was 30Gy (range: 4-48Gy); 26 patients (51%) received ≥30Gy. Thirty-one patients (61%) received bRT comprehensively to all disease sites. The median cumulative %BM irradiated was 5.05% (range: 0-50%). At a median follow-up of 10.3 months (95% CI: 7.7-16.4), the 1-year OS, PFS, and DSS rates were 80% (95% CI: 66-99), 78% (64-87), and 82% (68-90), respectively. The incidence of ≥Grade 3 lymphopenia was 33% pre-RT and 68% post-RT, but recovered to 43% at the conditioning chemotherapy (CC) timepoint. There was no correlation between post-RT Grade ≥3 lymphopenia and the receipt of comprehensive bRT, combined modality bridging, ≥30Gy bRT, or bRT to ≥15% of BM (all p>0.2). Among patients with Grade 0-2 lymphopenia pre-RT, increased conversion to Grade ≥3 lymphopenia post-RT correlated with comprehensive or ≥30Gy bRT, but these factors did not impair ALC recovery at CC. There was no association between ALC Δ RT or post-RT ALC with 30 or 90 day response (p>0.25), DSS, PFS, or OS (p>0.3). Conclusions: Lymphocyte change during bRT is not associated with CAR-T outcomes. Persistent cytopenia risk following bRT is not associated with bRT to ≥30Gy, ≥15% of BM, or comprehensive coverage. While bRT can be delivered safely, we urge careful treatment planning when incorporating into pre-CAR-T regimens.

Intensity Modulated Radiation Therapy for Early-Stage Primary Gastric Diffuse Large B-Cell Lymphoma: Dosimetric Analysis, Clinical Outcome, and Quality of Life

Abstract:Purpose: To evaluate the dosimetric superiority, efficacy, toxicity, and quality of life (QOL) data of intensity modulated radiation therapy (IMRT) in patients with primary gastric diffuse large B-cell lymphoma (PG-DLBCL).Methods and Materials: Forty-six consecutive patients with early-stage PG-DLBCL underwent IMRT after chemotherapy. The majority of patients (61.5%) were subclassified as the non-germinal center B cell-like subtype. Dosimetric parameters of the planning target volume (PTV) and organs at risk were assessed. Survival rates were depicted with the Kaplan-Meier method and compared with the log-rank test. Quality of life was evaluated using the QLQ-C30-STO22 questionnaires at the last follow-up contact.Results: The median PTV mean dose was 41.6 Gy. Only 0.73% of the PTV received <95% of the prescribed dose, indicating excellent target coverage. The median kidney V20 and liver V30 were 14.1% and 16.1%, respectively. The 5-year overall survival (OS), progression-free survival, and locoregional control rates for all patients were 80.4%, 75.0%, and 93.2%, respectively. Stage, lactate dehydrogenase level, and immunophenotype were significant prognostic factors for OS, and only stage was a significant factor for locoregional control. Consolidation IMRT in patients with complete response after chemotherapy resulted in significantly better OS and progression-free survival than salvage IMRT in patients with non-complete response. Two of 8 patients who had chronic liver disease experienced grade 4 or grade 5 acute hepatic failure after 4 to 5 cycles of rituximab-based chemotherapy and IMRT (40 Gy). No other serious acute or late toxicity was observed. The long-term global and functional QOL scales were excellent, with negligible symptom scales.Conclusions: Intensity modulated radiation therapy yielded excellent target coverage and critical tissue sparing and achieved favorable outcomes with acceptable toxicity and good long-term QOL in early-stage PG-DLBCL. Abstract:More than a quarter of real‐world patients with DLBCL experience R/R disease following 1L treatment, and outcomes for patients with R/R disease are poor. There is a high unmet need for novel, safe, and effective treatment options for patients with R/R DLBCL. Background Although initial treatment of diffuse large B‐cell lymphoma (DLBCL) with rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone (R‐CHOP) can be effective, up to 50% of patients will develop refractory or relapsed (R/R) disease. This study aimed to provide contemporary data on characteristics, treatment patterns, and outcomes for R/R‐DLBCL. Methods Patients with incident (January 2016 to March 2021) DLBCL age ≥18 years who initiated first‐line (1L) therapy were identified from the COTA real‐world database. Baseline characteristics, treatment patterns, and real‐world outcomes, including time to next treatment (rwTTNT) and overall survival (rwOS), were assessed for the study population and by line of therapy (LOT). Results A total of 1347 eligible DLBCL patients were identified. Of these, 340 (25.2%) proceeded to receive 2L, of whom 141 (41.5%) proceeded to receive 3L, of whom 51 (36.2%) proceeded to receive 4L+. Most common treatments were R‐CHOP in 1L (63.6%), stem cell transplant (SCT) in 2L (17.9%), polatuzumab vedotin, bendamustine, and rituximab (Pola‐BR) in 3L (9.9%), and chimeric antigen receptor T‐cell therapy (CAR‐T) in 4L (11.8%). Treatment patterns were more variable in later LOTs. One‐ and 3‐year rwOS from 1L initiation were 88.5% and 78.4%, respectively. Patients who received later LOTs experienced numerically lower 1‐ and 3‐year rwOS (from 2L initiation: 62.4% and 46.4%, respectively). Conclusions In this real‐world analysis, 25.2% of patients experienced R/R‐DLBCL after 1L with poor outcomes. Given the findings of this study, there is a high unmet need for novel, safe, and effective treatment options for patients with R/R DLBCL.

Biologically Effective Dose-response Relationship for Patients with Limited-stage Small-cell Lung Cancer Treated with Combined Radiotherapy and Chemotherapy

Abstract:Although multiple studies have confirmed the value of thoracic radiation therapy (RT) in limited-stage small-cell lung cancer (LS-SCLC), the appropriate RT dose remains controversial. This study aimed to evaluate the correlation between the biologically effective RT dose and local tumor control and overall survival for patients with LS-SCLC. The medical records of 205 LS-SCLC patients treated with radical RT (≥50 Gy) and combined chemotherapy at the Fudan University Cancer Hospital between 1997 and 2006 were retrospectively reviewed. Biologically effective dose (BED) was calculated for RT dose homogenization. Patients were grouped by BED (≤57 Gy and >57 Gy, 57 Gy equivalent to 60 Gy in 30 fractions in 6 weeks). Treatment interruption during RT was used as the “marker” for treatment toxicity. Kaplan-Meier analysis was used to calculate overall survival (OS), progression-free survival (PFS), local control rate (LCR), and distant-metastasis-free survival (DMFS) for both groups of patients. Of the 205 patients, 70 had received ≤57 Gy (lower BED group) and 135 >57 Gy (higher BED group). The patient and treatment characteristics were similar in the two groups except for the daily fractions, twice-daily RT was significantly more frequent in the higher BED group (p = 0.000). RT interruptions were recorded in 46 cases (22%), with a median duration of 6 days (range, 1-18). No differences in treatment-related toxicity rates were demonstrated between the two BED groups (p = 0.339).The median follow-up time was 20.7 months (range, 3.6-102.8 months) for all analyzable patients and 50.8 months (range, 27.3-102.8 months) for patients alive. The LCR and DMFS at 2 years for the higher and lower BED groups were 83.7% and 62.5% (p = 0.024) and 55.7% and 43.0% (p = 0.196), respectively. Patients in the higher BED group had a statistically significant better 5 years OS (26.2% vs. 14.6%, p = 0.005) and PFS (19.4% vs. 12.9%, p = 0.006). There were no significant difference for OS between the once-daily and twice-daily groups (p = 0.575). Multivariable Cox regression model demonstrated that age (p = 0.004), Karnofsky performance status (p = 0.005), weight loss < 5% (p = 0.032), and BED (p = 0.043) were significant predictors of OS. This study has shown that patients treated with BED > 57 Gy have better LCR, PFS and OS compared those treated with BED ≤ 57 Gy, indicating that a positive BED-response relationship still exist even though in a scope of relatively high BED. Future studies are warranted to examine the optimal RT doses and fractionation to achieve the appropriate BED for LS-SCLC, especially with modern RT techniques.
Impact of Relative Dose Intensity of R-CCOP Regimen in Elderly Patients with Diffuse Large B-cell Lymphoma in China

Abstract:Background: The actual relative dose intensity (RDI) of the attenuated R-CCOP regimen (rituximab, cytoxan, pegylated liposomal doxorubicin [PLD], vincristine, and prednisone) has not been fully investigated in Chinese geriatric patients with diffuse large B-cell lymphoma (DLBCL). In particular, the optimum dose for PLD remains unclear. Methods: We retrospectively collected clinical data from patients with untreated DLBCL aged 65e80 years subsequently treated with the R-CCOP. The restricted cubic spline model (RCS) was used to test the non-linear relationship between the predictors and outcomes. Results: Eighty-four patients were enrolled, with a median age of 73.5 years. More than half of the patients (54.8%) received at least 6 cycles. The median dose per cycle of cytoxan and PLD were 605.5 and 19.9 mg/m(2). The 5-year progression-free survival (PFS), overall survival rate, and disease-specific survival rates were 38.7%, 44.8%, and 57.2%, respectively. The RDI of PLD (PLD-RDI, <70% vs >= 70%) was only significant in the univariate analysis (P = 0.002) but not in the multivariate analysis. The RCS model showed a decreasing trend of hazards with an increasing PLD dose per cycle after adjustment. No significant difference was observed between the low- and high-risk groups with PLD-RDI >= 70% (P = 0.548). However, patients in the high-risk group had unfavorable PFS with PLD-RDI < 70% (P = 0.006). Conclusion: The optimal dose of PLD for elderly patients with DLBCL in China remains to be determined. Evaluating the tolerance and identifying risk categories are critical for clinical decision-making in this population. Copyright (c) 2022, Formosan Medical Association. A Clinical Study of Baseline PET/CT Parameters in the Prediction of Refractory/relapsed Diffuse Large B Cell Lymphoma of Advanced Stage Patients

Abstract:1685 Introduction: Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of lymphoma. Its cure rate has improved over the last two decades in the rituximab era. However, about one-third of patients are refractory/ relapsed ultimately. Early detection of such patients and applying more intensive treatment may help improve prognosis. 18F-FDG PET / CT is widely used in initial staging and post-treatment response assessment, while its value in prognosis is still challenge. The purpose of this study was to explore whether the baseline PET/CT parameters can identify refractory/ relapsed disease in advanced stage patients. Methods: Eighty-three advanced stage untreated DLBCL patients were enrolled in this study, including 43 males and 40 females, with a median age of 60 years. There were 18 cases of Ann Arbor stage III and 65 cases of stage IV. After baseline PET/CT examination, all patients received R-CHOP or R-CHOP-like regimen treatment in our hospital and were followed up for at least 3 years. Refractory/ relapsed disease was defined as progressive or stable disease during treatment, or relapse within 1 year of diagnosis after an initial CR. The tumor VOIs were semi automatically defined on the attenuation-corrected PET images by using a threshold of SUV ≥ 2.5 on local image features extraction (LifeX) software, with segmentation correction performed manually by consensus by two experienced nuclear medicine physicians. SUVmax, metabolic tumor volume (MTV) and tumor lesion glycolysis (TLG) were then calculated. The clinical data including Ann Arbor stage, B symptom, bulky mass, bone marrow involvement, important extranodal involvement, LDH level and NCCN-IPI score were also analyzed. Univariate and multivariate logistic regression analysis was used to identify the independent risk factors for refractory / relapsing patients. PFS and OS were estimated by the Kaplan-Meyer survival analyses, and the difference was compared using a log-rank test. Univariate and multivariate Cox regression models was used to identify the independent risk factors for PFS and OS. Results: There were 44 patients in non- refractory/ relapsed group and 39 patients in refractory/ relapsed group. The MTV and TLG of refractory/ relapsed group was significantly higher than that of non- refractory/ relapsed group (MTV: 1579.9 cm3 Vs. 484.7cm3, P =0.000; TLG: 7853.8 Vs. 3015.8, P =0.000). There was no significant difference of SUVmax between the two groups (15.9 Vs. 17.5, P= 0.248). In the univariate Logistic regression analysis, Ann Arbor stage = 4 \* ROMAN IV, B symptom, bone marrow involvement, important extranodal involvement, elevated LDH level, high NCCN-IPI score, high MTV and TLG were the independent risk factors in prediction refractory/ relapsing disease. In the multivariate Logistic regression analysis, only elevated LDH level and high TLG were the independent risk factors (elevated LDH: HR= 5.233, 95% CI= 1.234-22.183, P=0.025; TLG>2098: HR= 11.914, 95% CI=3.065-46.314, P=0.000). In the univariate Cox analysis, Ann Arbor stage = 4 \* ROMAN IV, B symptom, bone marrow involvement, important extranodal involvement, elevated LDH level, high NCCN-IPI score, high MTV and high TLG were the independent risk factors for PFS. In multivariate Cox analysis, only high TLG was the independent risk factors (HR= 3.049, 95%CI: 1.250-7.438, P=0.014). As to the OS, in the univariate Cox analysis, Ann Arbor stage = 4 \* ROMAN IV, B symptom, important extranodal involvement, elevated LDH level, high NCCN-IPI score, high MTV and high TLG were the independent risk factors. In multivariate analysis, only B symptom and elevated LDH were the independent risk factors (B symptom: HR= 3.494, 95%CI: 1.489-8.197, P=0.004; elevated LDH: HR= 9.805, 95% CI:1.320-72.831, P=0.026). Conclusions: High MTV and TLG in baseline PET/CT can predict the refractory/relapse disease in patients with advanced DLBCL, and can be used as important prognostic factors.
