Abstract:Interferon-alpha (IFN-alpha) treatment can suppress the hypersecretion syndrome associated with functional neuroendocrine tumors. Chromogranin A (CgA) is a matrix protein of neuroendocrine secretory vesicles and appears to be essential for an appropriate neuroendocrine secretory function. To test the hypothesis that IFN-alpha can directly interfere with CgA gene transcription, we performed transient transfection studies in pancreatic neuroendocrine tumor cells employing CgA-luciferase reporter gene constructs showing that IFN-alpha inhibited basal and protein kinase C-dependent CgA promoter activity. Using 5'-deletion constructs in combination with mutational analysis of the proximal CgA core promoter, a cyclic AMP response element (CRE) at -71 to -64 bp was identified as the IFN-alpha response element of the CgA gene. Furthermore, functional studies indicated that IFN-alpha exerts its effect on the CgA promoter via interference with CRE binding protein (CREB)/CREB binding protein (CBP)-dependent transactivation of the CgA-CRE.
Between-Session Reliability of Athletic Performance and Injury Mitigation Measures in Female Adolescent Athletes in the United States

Abstract:Adolescence is a fundamental period for female athletes to develop athletic performance, mitigate injury risk, and gain collegiate sport scholarships, but there is also a high incidence of sport-related injuries. Physical profiling and athlete screening can support the individualisation of training programmes; however, there is a lack of data pertaining to the reliability of athletic performance and injury surrogate measures in adolescent female athletes. The aim of this study was to quantify the between-session reliability of an athletic performance and injury mitigation testing battery in female adolescent athletes. A total of 31 post-peak height velocity (PHV) (3.00 ± 0.82 years) female athletes (age: 16.20 ± 1.20 years; standing height: 166.00 ± 6.00 cm; mass: 65.5 ± 10.70 kg) from various sports (track and field = 1; lacrosse = 2; basketball = 2; soccer = 3; softball = 11; volleyball = 12) completed two sessions of a multicomponent testing battery 48 h to 1 week apart including the assessment of 33 measures addressing lower-limb isometric strength, eccentric strength, reactive strength, linear sprint and change of direction speed, and lower limb control. Of the 33 measures, between sessions, 29 had a high to nearly perfect intraclass correlation coefficient (ICC) (0.508–0.979), and 26 measures were not statistically significantly different between sessions (p ≤ 0.05). All measures demonstrated low to acceptable coefficient variation (CV%) (0.61–14.70%). The testing battery used can be utilised for recruitment and longitudinal monitoring within sports organisations for female adolescent athletes.

Abstract:This study aimed to provide a comprehensive strength-based physiological profile of women's NCAA Division I basketball and gymnastic athletes; and to make sport-specific comparisons for various strength characteristics of the knee flexor and extensor muscles. A focus on antagonist muscle balance (hamstrings-to-quadriceps ratios, H:Q) was used to elucidate vulnerabilities in these at-risk female athletes. Fourteen NCAA Division I women's basketball and 13 gymnastics athletes performed strength testing of the knee extensors and flexors. Outcome measures included absolute and relative (body mass normalised) peak torque (PT), rate of torque development at 50, 100, 200 ms (RTD50 etc.) and H:Q ratios of all variables. The basketball athletes had greater absolute strength for all variables except for isokinetic PT at 240°s-1 and isometric RTD50 for the knee extensors. Gymnasts showed ~20% weaker body mass relative concentric PT for the knee flexors at 60 and 120°·s-1, and decreased conventional H:Q ratios at 60 and 240°·s-1 (~15%). These findings suggest that collegiate level gymnastics athletes may be prone to increased ACL injury risk due to deficient knee flexor strength and H:Q strength imbalance. Coaches may use these findings when implementing injury prevention screening and/or for individualised strength training programming centered around an athletes strength-related deficits.

Abstract:The use of isometric strength testing, particularly the isometric mid-thigh pull (IMTP) has increased dramatically over the last decade. The IMTP and isometric squat (ISqT) provide one aspect of performance monitoring with variables such as peak force and rate of force development being derived from the force-time curve. The reliability of some of these variables is conflicting in the literature, and the reporting of the reliability is not standardised across the research. The majority of research only reports intraclass correlation coefficients with very few studies reporting coefficient of variation and 90% confidence intervals. Additionally, methods used to calculate variables from the force-time curve differ across studies. An aim of muscle strength testing is to provide normative values for specific sports, allowing coaches to distinguish between performance levels or evaluate the effects of training on performance. This narrative review aims to evaluate studies that have researched the reliability and/or reported normative data for both tests. Additionally, the testing protocols and the force-time curve analysis techniques utilised are discussed, concluding with practical applications for coaches on the uses and limitations of these tests. Results demonstrate that peak force is the most reliable measure and can be used to determine maximum strength capabilities.

Abstract:Measurements of muscle-tendon unit (MTU) function can be categorised into local (e.g. tendon strain) or global (e.g. jump height) assessments. Although menstrual cycle phase may be a key consideration when implementing these assessments in female athletes, the reliability of many MTU assessments is not well defined within female populations. Therefore, the purpose of this study was to report the test-retest reliability of local and global MTU function assessments during the early follicular phase of the menstrual cycle. Seventeen naturally menstruating females (age 28.5 ± 7.3 years) completed local and global assessments of MTU function during two testing sessions separated over 24-72 hours. Local tests included Achilles’ tendon mechanical testing and isometric strength of ankle plantar flexors and knee extensors, whereas global tests included countermovement, squat, and drop jumps, and the isometric midthigh pull. Based on intraclass correlation coefficient (ICC) statistics, poor to excellent reliability was found for local measures (ICC: 0.096-0.936). Good to excellent reliability was found for all global measures (ICC: 0.788-0.985), excluding the eccentric utilisation ratio (ICC 0.738) and most rate of force development metrics (ICC: 0.635-0.912). Isometric midthigh pull peak force displayed excellent reliability (ICC: 0.966), whereas force-time metrics ranged from moderate to excellent (ICC: 0.635-0.970). Excluding rate of force development (coefficient of variation [CV]: 10.6-35.9%), global measures (CV: 1.6-12.9%) were more reproducible than local measures (CV: 3.6-64.5%). However, local metrics directly measure specific aspects of MTU function, and therefore provide valuable information despite lower reproducibility. The novel data reported here provides insight into the natural variability of MTU function within female athletes, which can be used to enhance the interpretation of other female athlete data, especially that which aims to investigate other aspects of variability, such as the menstrual cycle.

Abstract:Transthyretin cDNA was cloned from Eastern Grey Kangaroo liver and its nucleotide sequence determined. Analysis of the derived amino acid sequence of kangaroo transthyretin, together with data obtained previously for transthyretins from other vertebrate species [Duan, W., Richardson, S. J., Babon, J. J., Heyes, R. J., Southwell, B. R., Harms, P. J., Wettenhall, R. E. H., Dziegielewska, K. M., Selwood, L., Bradley, A. J., Brack, C. M. & Schreiber, G. (1995) Eur. J. Biochem. 227, 396-406], showed that the N-terminus is the region which changes most distinctly during evolution. It has been shown for human, mouse and rat transthyretins, that this region is encoded by DNA at the border of exon 1 and exon 2. Therefore, this section of transthyretin genomic DNA was amplified by PCR and directly sequenced for the Buffalo Rat, Tammar Wallaby, Eastern Grey Kangaroo, Stripe-faced Dunnart, Short-tailed Grey Opossum and White Leghorn Chicken. The splice sites at both ends of intron 1 were identified by comparison with the cDNA sequences. The obtained data suggest that the N-termini of transthyretin evolved by successive shifts of the 3' splice site of intron 1 in the 3' direction, resulting in successive shortening of the 5' end of exon 2. At the protein level, this resulted in a shorter and more hydrophilic N-terminal region of transthyretin. Successive shifts in splice sites may be an evolutionary mechanism of general importance, since they can lead to stepwise changes in the properties of proteins. This could be a molecular mechanism for positive Darwinian selection.

Abstract:Background Establishing normative strength data can assist professionals in guiding post-injury rehabilitation and be a criterion for deciding discharge for sports return. Objectives To establish reference values for isometric hip flexor muscle strength in college athletes. Methods This was a cross-sectional observational study. The athletes were submitted to an isometric muscle strength evaluation of the hip flexors through a hand-held dynamometer (Medeor Medtech Tecnologia em Saúde Industria e Comercio Ltda). The athletes remained in dorsal decubitus position on a stretcher, with the tested leg flexed 10 cm above the surface to start the test. The dynamometer was positioned on the anterior part of the leg, above the talotibial joint line. The lever arm was defined as the distance, in meters (m), between the anterior superior iliac spine and the dynamometer application point. The athletes performed three isometric contractions of 5 seconds, with a rest interval of 30 seconds between repetitions. If there was a discrepancy greater than 10% in the first three repetitions, a fourth repetition was performed. The isometric force data were expressed in kilogram-force (Kg/f) and converted to newton (N) for the calculation of torque (N.m). The normality of the data was verified using the Shapiro-Wilk test. The mean and standard deviation were calculated, followed by and by independent samples t-test. A significance level of (p > 0.05) was adopted. Results Thirteen female athletes (22 ± 2.19 years; 1.63 ± 0.06 m and 63.5 ± 16.9 kg) and 14 male athletes (23.85 ± 6.97 years; 1.75 ± 0.06 m and 72.8 ± 8.6 kg) participated in the study, totaling 27 college athletes. The athletes were classified according to the International Physical Activity Questionnaire (IPAQ) as Very Active (29.62%), Active (55.55%) and Irregularly Active (14.81%). Male athletes produced significantly more torque (109.36 ± 43.70 N.m; CI 84.12 - 134.59 N.m) when compared to female athletes (73.05 ± 14.26 N.m; CI 64.43 - 81.67 N.m). Conclusion This study provides a normative database on isometric hip flexor strength measured with a hand-held dynamometer. In general, differences in strength were present between the sexes, with men showing higher torque values compared to women. Implications The isokinetic dynamometer is the gold standard instrument for quantifying muscle strength. However, it is not accessible to all athletes. Therefore, we sought an alternative for the quantification of muscle strength in an affordable way. These data provide a description of hip flexor muscle strength in college athletes in order to assist professionals in post-injury rehabilitation, and to be a discharge criterion for sports return.

Abstract:Lower limb isometric strength is a key parameter to monitor the training process or recognise muscle weakness and injury risk. However, valid and reliable methods to evaluate it often require high-cost tools. The aim of this study was to analyse the concurrent validity and reliability of a low-cost digital dynamometer for measuring isometric strength in lower limb. Eleven physically active and healthy participants performed maximal isometric strength for: flexion and extension of ankle, flexion and extension of knee, flexion, extension, adduction, abduction, internal and external rotation of hip. Data obtained by the digital dynamometer were compared with the isokinetic dynamometer to examine its concurrent validity. Data obtained by the digital dynamometer from 2 different evaluators and 2 different sessions were compared to examine its inter-rater and intra-rater reliability. Intra-class correlation (ICC) for validity was excellent in every movement (ICC > 0.9). Intra and inter-tester reliability was excellent for all the movements assessed (ICC > 0.75). The low-cost digital dynamometer demonstrated strong concurrent validity and excellent intra and inter-tester reliability for assessing isometric strength in the main lower limb movements.

Abstract:Background: The high prevalence of injuries in female athletes necessitates a course of action that not only enhances research in this field but also incorporates improved prevention programs and regular health monitoring of highly stressed structures such as tendons and muscles. Since myometry is already used by coaches and physiotherapists, it is important to investigate whether tissue stiffness varies in different types of sports, and whether such measures are affected by an acute training session. Methods: Myometric measurements of the Achilles tendon (AT) and soleus muscle (SM) were performed in the longitudinal plane and relaxed tendon position. In total, 38 healthy professional female athletes were examined, applying a quasi-experimental study design, with subgroup analysis performed for different sports. To investigate the stiffness of the AT and SM, 24 female handball and volleyball athletes performed a standardized maximal incremental performance test on a treadmill. In this subgroup, myometric measurements were taken before and after the exercise test. Results: The measurements showed no significant difference between the mean pre- (AT: 661.46 N/m; SM 441.48 N/m) and post-exercise stiffness (AT: 644.71 N/m; SM: 439.07 N/m). Subgroup analysis for different types of sports showed significantly lower AT and SM stiffness in swimming athletes compared to handball (p = 0.002), volleyball (p = 0.000) and hammer throw athletes (p = 0.008). Conclusions: Myometry can be performed on the same day as an acute training session in healthy female professional volleyball and handball athletes. Female swimmers have significantly lower AT and SM stiffness compared to female handball, volleyball and hammer throw athletes. These results show that the stiffness differences in the AT and SM can be assessed by myometry.

Abstract:Background: As a consequence of years of soccer training and sexual maturation, there is an increase in lower body muscle mass and strength especially in the knee extensors and flexors muscles. In this context, the lack of knee joint stability, resulting from strength imbalance between knee extensor and flexors muscles, has been associated with knee injuries. The aim of this study was to compare the knee flexor and extensor muscle peak torque, average power, contralateral deficit, conventional and functional balance ratios of female soccer players from different age groups.Methods: Sixty-six female soccer players were divided into four groups: under 13 (U13), under 15 (U15), under 17 (U17) years old and professional (PRO). Flexor and extensor knee muscle strength in concentric and eccentric actions of both limbs were assessed using isokinetic dynamometer.Results: For the dominant limb, the knee concentric extensor muscles peak torques, assessed at 60 and at 240 deg/sec, and the average power of U15 group were significantly higher than U13 group. Extensor muscle average power of the PRO group was higher than U17. Dominant knee flexor average power of U15 was significantly higher than the U13 group. Peak torque at 60 deg/sec and 240 deg/sec and average power of the PRO group were higher than the U17 group. No differences were found regarding the eccentric action for flexor and extensor muscles. Conventional and functional balance ratios were similar among all age group, except for U13, which presented higher values than the U15 group for the dominant limb.Conclusions: The greatest improvement in muscular performance occur when the athlete starts practicing soccer (after U13) and when they become professional (after U17) and the balance ratios, and muscle balance ratios remain stable in all age groups, although they are below the recommended level in the literature, which may increase the risk for lower limb injury.

Inter-limb strength asymmetry in adolescent distance runners: Test-retest reliability and relationships with performance and running economy

Abstract:The purpose of this investigation was, firstly, to quantify the test-retest reliability of strength measures in adolescent distance runners; and secondly, to explore the relationships between inter-limb strength asymmetry and performance and running economy (RE) in a similar cohort of young runners. For the reliability study, twelve (n = 6 female) post-pubertal adolescent distance runners performed an isometric quarter-squat on a dual force plate and unilateral isometric hip extension and hip abduction tests on two occasions. For the correlation study, participants (n = 31) performed the strength tests plus a submaximal incremental running assessment and a maximal running test. Running economy was expressed as the average energy cost of running for all speeds below lactate turnpoint and was scaled for body mass using a previously calculated power exponent. Allometrically scaled peak force during the quarter-squat and peak torque in the hip strength tasks showed acceptable levels of reproducibility (typical error ≤6.3%). Relationships between strength asymmetry and performance and RE were low or negligible (r < 0.47, p > 0.05), except for hip abduction strength asymmetry and RE in the female participants (r = 0.85, p < 0.001, n = 16). Practitioners should consider inter-limb hip abduction strength asymmetry on an individual level, and attempting to reduce this asymmetry in females may positively impact RE.
Intra- and inter-limb strength imbalance and asymmetry in soccer: A comparison of elite senior and junior players

Abstract:Evaluation of muscle strength imbalance can be an important element in optimizing the training process of soccer players. The purpose of the study was to examine isokinetic peak torque (PT) and total work (TW) exerted by both knee extensors (quadriceps or Q) and flexors (hamstrings or H), intra-limb imbalance and the magnitude and direction of inter-limb asymmetry in top elite senior (n = 109) and junior (n = 74) soccer players. An isokinetic dynamometry was used to measure maximum peak torque of quadriceps (PT-Q) and hamstrings (PT-H) at an angular velocity of 60° ·s -1 , as well as the total work for extensors (TW-Q) and flexors (TW-H) at an angular velocity of 240° ·s -1 in the dominant (DL) and non-dominant leg (NDL) during concentric muscle contraction. Intra-limb imbalance and inter-limb asymmetries were calculated using a standard equation. Statistical analysis using t-test and Mann-Whitney U-test revealed: (a) no differences ( p > 0.05) between groups for PT-Q and PT-H, (b) greater strength levels ( p < 0.05) for TW-Q and TW-H of senior players than juniors, and (c) no differences ( p > 0.05) between groups for intra-limb imbalance and inter-limb asymmetry. Additionally, Pearson's chi-kwadrat (χ2) analysis showed no differences ( p > 0.05) between groups for intra-limb imbalance and inter-limb asymmetry in relation to the 'normative' values accepted in the literature that indicate an increase in the risk of knee injury. This study shows that isokinetic assessment can be an important tool to identify imbalances/asymmetries and to develop strategies to reduce the risk of muscle injury.

Abstract:Context : The hamstrings are the most commonly injured muscle in sports and are especially injury prone in lengthened positions. Measuring knee muscle strength in such positions could be relevant to establish injury risk. Handheld dynamometry has been shown to be a valid, reliable, and practical tool to measure isometric muscle strength clinically. The aim of this study was to assess the validity and reliability of the assessment of isometric knee muscle strength with a handheld dynamometer (HHD) at various muscle lengths, by modifying the hip and knee angles during testing. Design : Concurrent validity and test–retest reliability. Methods : Thirty young healthy participants were recruited. Hamstring and quadriceps isometric strength was measured with a HHD and with an isokinetic dynamometer, over 2 testing sessions, in a randomized order. Isometric strength was measured on the right lower limb in 6 different positions, with the hip at either 0° or 80° of flexion and the knee at either 30°, 60°, or 90° of flexion. Pearson and Spearman correlations were used to assess the validity, and intraclass correlation coefficients were calculated to establish the test–retest reliability of the HHD. Results : Good to excellent reliability and moderate to high validity were found in all the tested muscle length positions, except for the hamstrings in a seated position with the knee extended at 30°. Conclusions : The use of a HHD is supported in the clinical setting to measure knee muscle strength at varying muscle lengths in healthy adults, but not for the hamstrings in a lengthened position (hip flexed and knee extended). These results will have to be confirmed in sport-specific populations.

Reliability of isokinetic dynamometer for isometric assessment of ankle plantar flexor strength.

Abstract:Objective To evaluate isokinetic dynamometer reliability for isometric assessment of plantar flexor (PF) strength. Design Cross-sectional. Setting Testing by the same physiotherapist twice during a first session (repeatability) and once during a second session (reproducibility). Participants Twenty-two healthy subjects (44 ankles, 11 men/11 women). Main Outcome Measures Isometric PF peak torque, with and without body mass normalization, at 0° and +20° of plantar flexion. Measurement reliability was evaluated using intraclass correlation coefficient (ICC), standard error of measurement (SEM) and minimal detectable change (MDC). Results Without normalization, measurement repeatability was excellent at 0° of plantar flexion (ICC, 0.94; SEM, 6.6%; MDC, 18.4%) compared with good repeatability at +20° (ICC, 0.85; SEM, 11.1%; MDC 30.6%). Measurement repeatability following normalization was good at 0° (ICC, 0.88; SEM, 5.2%; MDC, 14.4%) and +20° (ICC, 0.79; SEM, 10.2%; MDC, 28.1%). While reproducibility was good at 0° with normalization (ICC, 0.84; SEM, 5.9%; MDC, 16.3%) or excellent without (ICC 0.92; SEM 7.5%; MDC, 20.8%), it was moderate at +20° with normalization (ICC 0.71; SEM 11.3%; MDC, 31.3%) or good without (ICC 0.78; SEM 13.0%; MDC, 36.1%). Conclusion The reliability of PF maximal isometric strength is good/excellent at 0° of plantar flexion but moderate/good at +20°.

Abstract:BACKGROUND: Test selection during the return to sport evaluation is an important aspect of ensuring athletes are safe to return to competition. OBJECTIVE: To assess the test-retest and inter-rater reliability of two new single leg hamstring bridge tests (SLHBT) and the relationships between these new tests and isokinetic strength of the quadriceps and hamstrings. METHODS: Thirty healthy college students completed the study. Two testing sessions were held three to seven days apart. Session one, participants performed two SLHBT as well as concentric isokinetic strength tests for the hamstring and quadriceps at 60, 180 and 300∘/sec. The participants performed only the two SLHBT during the second session. The first SLHBT, participants performed as many repetitions as possible of the SLHBT in 30-sec, while the second test measured how quickly the participants could perform five repetitions. Intraclass Correlation Coefficients (ICC2,1) were used to assess test-retest reliability while ICC(3,1) were used to test inter-rater reliability. RESULTS: The test-retest reliability for the SLHBTs was moderate to good, with ICC(2,1) ranging from 0.68 to 0.75, while the inter-rater reliability were excellent with ICC(3,1) all above 0.98. There were significant correlations between the SLHBT and the isokinetic tests, but all correlations were low to moderate. CONCLUSION: This study found good to excellent reliability with the new SLHBT and poor to moderate correlations were noted Therefore, SLHBT may not be an alternative to test hamstring test during the RTS evaluation due to the multiple compensatory strategies athletes may have.

Abstract:Background The aim of this study was to examine the test−retest reliability in lower limb muscle strength and rate of torque development (RTD) using isokinetic dynamometry in adults with obesity, with a body mass index (BMI) ≥ 35 kg/m2. Method Thirty‐two adults with a BMI of 43.8 ± 6.6 kg/m2 eligible for bariatric surgery were enroled in the study. Isokinetic and isometric knee extensor (KE) and flexor (KF) strength were assessed in an isokinetic dynamometer (Biodex 4) during three test sessions separated by 3−7 days. Results There were no statistical differences in peak KE and KF torque for any test modalities between sessions. Intraclass correlation (ICC) was 0.91−0.94 between sessions 1 and 2 and 0.94−0.97 between sessions 2 and 3. Standard error of measurement (SEM%) and coefficient of variation (CV) ranged across test sessions from 4.3% to 7.3%. KE RTD showed high test−retest reliability following familiarization, with ICC, CV and SEM% values ranging from 0.84 to 0.90, 13.3%−20.3% and 14.6%−24.9%, respectively. Conclusion Maximal lower limb muscle strength measured by isokinetic dynamometry showed excellent test−retest reliability manifested by small measurement errors and low CV. Reliability was slightly improved by including a familiarization session. KE RTD but not KF RTD demonstrated high test−retest reliability following familiarization. The present data indicate that isokinetic dynamometry can be used to detect even small changes in lower limb muscle strength in adults with obesity.
