Abstract:Abstract The proximal tubule (PT) is known as the workhorse of the kidney, both for the range and magnitude of the functions that it performs. It is not only responsible for reabsorbing most solutes and proteins filtered by glomeruli, but also for secreting non-filtered substances including drugs and uremic toxins. The PT therefore plays a pivotal role in kidney physiology and body homeostasis. Moreover, it is the major site of damage in acute kidney injury and nephrotoxicity. In this review, we will provide an introduction to the cell biology of the PT and explore how it is adapted to the execution of a myriad of different functions and how these can differ between males and females. We will then discuss how the PT regulates phosphate, glucose and acid-base balance, and the consequences of alterations in PT function for bone and cardiovascular health. Finally, we explore why the PT is vulnerable to ischemic and toxic insults, and how acute injury in the PT can lead to maladaptive repair, chronic damage, and kidney fibrosis. In summary, we will demonstrate that knowledge of the basic cell biology of the PT is critical for understanding kidney disease phenotypes and their associated systemic complications, and for developing new therapeutic strategies to prevent these.

Growth of embryonic renal parenchyme at the interphase of a polyester artificial interstitium

Abstract:The construction of an artificial kidney module by tissue engineering or the application of cell-based therapies for the treatment of renal failure requires exact information regarding the cellbiological mechanisms of parenchyme development in combination with different kinds of biomaterials. To learn more about these processes tissue cultures are frequently used experimental tools. However, apart from experiments with early kidney anlagen there is a lack of suitable in-vitro models regarding the generation and long-term maintenance of renal tubules. In the present paper we like to demonstrate an advanced culture technique, which allows to generate tubular elements derived from renal stem cells. For the growth of tubules it is essential to fine-tune the interface between the embryonic tissue and the dead fluid space within a perfusion culture container by offering a polyester artificial interstitium. Culture was performed in IMDM supplemented with hormones and growth factors but using serum-free conditions over 14 days. Formation of tissue was then analysed by immunohistochemistry and two-dimensional (2D) electrophoresis. Culture in pure IMDM leads to a complete loss of tissue formation. In contrast, application of aldosterone (A) induces the development of numerous polarised tubules. Surprisingly, addition of epidermal growth factor (EGF), a cocktail of insulin, transferrin and selenium (ITS), retinoic acid (RA), cholecalciferol (VitD3) or bovine pituitary extract (BPT) does not further improve development of tubules, but leads to intensive cell clustering and a decrease of tubule formation. 2D Western blots of developing tissue probed with soybean agglutinin (SBA) reveal a unique pattern of newly detected proteins. It is found that growth factors do not support but abolish protein spots upregulated by aldosterone. It remains to be investigated, which cellbiological effect stimulates the embryonic cells to develop tubules in competition to cell clusters at the interphase of an artificial interstitium.
Impairment in renal medulla development underlies salt wasting in Clc-k2 channel deficiency

Abstract:The prevailing view is that the ClC-Ka chloride channel (mouse Clc-k1) functions in the thin ascending limb to control urine concentration, whereas the ClC-Kb channel (mouse Clc-k2) functions in the thick ascending limb (TAL) to control salt reabsorption. Mutations of ClC-Kb cause classic Bartter syndrome, characterized by renal salt wasting, with perinatal to adolescent onset. We studied the roles of Clc-k channels in perinatal mouse kidneys using constitutive or inducible kidney-specific gene ablation and 2D and advanced 3D imaging of optically cleared kidneys. We show that Clc-k1 and Clc-k2 were broadly expressed and colocalized in perinatal kidneys. Deletion of Clc-k1 and Clc-k2 revealed that both participated in NKCC2- and NCC-mediated NaCl reabsorption in neonatal kidneys. Embryonic deletion of Clc-k2 caused tubular injury and impaired renal medulla and TAL development. Inducible deletion of Clc-k2 beginning after medulla maturation produced mild salt wasting resulting from reduced NCC activity. Thus, both Clc-k1 and Clc-k2 contributed to salt reabsorption in TAL and distal convoluted tubule (DCT) in neonates, potentially explaining the less-severe phenotypes in classic Bartter syndrome. As opposed to the current understanding that salt wasting in adult patients with Bartter syndrome is due to Clc-k2 deficiency in adult TAL, our results suggest that it originates mainly from defects occurring in the medulla and TAL during development.

Embryonic and postnatal development of the rat renal interstitium

Abstract:Whilst antihypertensive, structural and functional roles have been proposed for the cells of the renomedullary interstitium in the adult kidney, little is known about its role in renal development. Rat kidneys were studied throughout development, prenatally at gestational ages E14-E21 and postnatally at 0-28 days, by light microscopy, transmission and scanning electron microscopy and following immunocytochemistry. Renomedullary interstitial cells were observed as early as embryonic day E14, forming a loose, orderly network around branches of the ureteric bud. Paralleling the development of the first nephron structures, renomedullary interstitial cells were arranged in a concentric circular manner around collecting ducts. Following tubular and vascular growth from the cortex into the medulla, this arrangement resulted in the characteristic 'rungs of a ladder' appearance of interstitial cells between tubules, blood vessels and the collecting ducts. Renomedullary interstitial cells were closely adherent to basement membranes of tubules, blood vessels and collecting ducts from early in development. Contacts were absent between renomedullary interstitial cells and tubular structures in the process of remodelling, such as the hair-pin bends of the loops of Henle. At these foci laminin, a basement membrane glycoprotein was specificially localised to intracellular epithelial sites, whereas in more developed areas, laminin was restricted to epithelial basement membranes. Associated with the more mature structures, laminin was also localised to intracellular granules of renomedullary interstitial cells. Thus, renomedullary interstitial cells are present prior to and appear to be actively associated with tubule repositioning in the medulla, establishing themselves as integral to the process of renal development.

Abstract:Abstract Background Hereditary renal tubular disease can cause hypercalciuria, acid-base imbalance, hypokalemia, hypomagnesemia, rickets, kidney stones, etc. If these diseases are not diagnosed or treated in time, they can cause kidney damage and electrolyte disturbances, which can be detrimental to the maturation and development of the child. Glomerular involvement in renal tubular disease patients has only been considered recently. Methods We screened 71 papers (including experimental research, clinical research, etc.) about Dent’s disease, Gitelman syndrome, and cystinosis from PubMed, and made reference. Results Glomerular disease was initially underestimated among the clinical signs of renal tubular disease or was treated merely as a consequence of the tubular damage. Renal tubular diseases affect glomerular podocytes through certain mechanisms resulting in functional damage, morphological changes, and glomerular lesions. Conclusions This article focuses on the progress of changes in glomerular podocyte function in Dent disease, Gitelman syndrome, and cystinosis for the purposes of facilitating clinically accurate diagnosis and scientific treatment and improving prognosis.

Abstract:The definitive kidney, the methanephros, is formed by two processes, nephrogenesis (the formation of glomerulus and tubules) and branching morphogenesis (the formation of collecting ducts, calyces, pelvis and ureters). The metanephric kidney takes place after the formation and involution of two embryonic kidneys, the pronephros (non functional organ) and the mesonephros (first functional kidney), that in turn evolves into the ureteric bud (UB). The metanephros appears during the fifth gestational week, and develops from the specific interaction between the epithelial ureteric bud and the undifferentiated metanephric mesenchyme (MM). This interaction is crucial for the differentiation of the mesenchyme and the induction of UB branching division (Fig. 124.1). The UB arises in response to signals elaborated by the mesenchyme and then undergoes branching morphogenesis following the invasion of the MM by the UB. This process gives a 15 branch generation. At 20–22 weeks of gestation, branching morphogenesis is completed and results in the collecting system. Mesenchymal cells that are in close contact with the invading UB undergo an epithelial transformation. The induced metanephric mesenchyme (MM) gives the nephrons, through the consecutive stages of condensation, renal vesicule, vascular cleft, and S-shaped body. The glomerular capillary tuft is formed via recruitment and proliferation of endothelial and mesangial cells precursors. The nephrons develop in successive stages from the inner to the outer area of the fetal kidney in parallel with the vascular system. In the human, the primitive glomerulus appears at approximately 9–10 weeks of gestation. The nephrogenesis is completed by 34–36th weeks of gestation [1]. About 60% of the nephrons develop during the third trimester of gestation, while nephrogenesis may continue ex-utero in preterm infants [2]. Once nephrogenesis is complete, stroma cells differentiate into fibroblasts, pericytes and lymphocytes-like cells. At birth the final nephron number varies from 800,000 and 1 million per kidney. Such variation in nephron number is due to genetic factors and to the fetal environment [3]. Several genes and molecular pathways control the formation of the renal collecting system and nephrogenesis, such as transcriptional factors (PAX-2, WT1), growth factors (IGF, EGF, TGF), oncogenes, the extracellular matrix, and vascular factors (Table 124.1) [4]. These factors act at a specific time of kidney development especially when the UB interacts with the adjacent MM. Blockade of vascular endothelial growth factor receptor (VEGF-R), inhibition of the renin angiotensin system (RAS) and knock-out for cyclooxigenase (COX)-2 gene expression are associated with impaired nephrogenesis including glomerular cysts, dysplasic tubules and tubular dysgenesis [5–7].
Sex-specific proximal tubular cell differentiation pathways identified by single-nucleus RNA sequencing

Abstract:Postnatal kidney growth is substantial and involves expansion in kidney tubules without growth of new nephrons, which are the functional units of the kidney. Proliferation and differentiation pathways underpinning nephron elongation are not well defined. To address this, we performed sequential characterization of mouse kidney transcriptomics at the single cell level. Single nuclear RNA sequencing (snRNA-seq) was performed on kidney tissue from male and female mice at 1, 2, 4 and 12 weeks of age using the 10x Chromium platform. Unbiased clustering was performed on 68,775 nuclei from 16 animals. 31 discrete cellular clusters were seen, which were identified through comparison of their gene expression profiles to canonical markers of kidney cell populations. High levels of proliferation were evident at early time points in some cell types, especially tubular cells, but not in other cell types, for example podocytes. Proliferation was especially evident in Proximal Tubular Cells (PTCs) which are the most abundant cell type in the adult kidney. Uniquely when compared to other kidney cell types, PTCs demonstrated sex-specific expression profiles at late, but not early, time points. Mapping of PTC differentiation pathways using techniques including trajectory and RNA Velocity analyses delineated increasing PTC specialization and sex-specific phenotype specification. Our single-cell transcriptomics data characterise cellular states observed during kidney growth. We have identified PTC differentiation pathways that lead to sex-specific tubular cell phenotypes. Tubular proliferative responses are of central importance in postnatal kidney growth and have also been linked to kidney recovery versus fibrosis following injury. Our unbiased and comprehensive dataset of tubular cell development can be used to identify candidate pathways for therapeutic targeting.

Blocking Connexin-43 mediated hemichannel activity protects against early tubular injury in experimental chronic kidney disease

Abstract:Background: Tubulointerstitial fibrosis represents the key underlying pathology of Chronic Kidney Disease (CKD), yet treatment options remain limited. In this study, we investigated the role of connexin43 (Cx43) hemichannel-mediated adenosine triphosphate (ATP) release in purinergic-mediated disassembly of adherens and tight junction complexes in early tubular injury. Methods: Human primary proximal tubule epithelial cells (hPTECs) and clonal tubular epithelial cells (HK2) were treated with Transforming Growth Factor Beta1 (TGF-β1) ± apyrase, or ATPγS for 48 h. For inhibitor studies, cells were co-incubated with Cx43 mimetic Peptide 5, or purinergic receptor antagonists Suramin, A438079 or A804598. Immunoblotting, single-cell force spectroscopy and trans-epithelial electrical resistance assessed protein expression, cell-cell adhesion and paracellular permeability. Carboxyfluorescein uptake and biosensing measured hemichannel activity and real-time ATP release, whilst a heterozygous Cx43+/- mouse model with unilateral ureteral obstruction (UUO) assessed the role of Cx43 in vivo. Results: Immunohistochemistry of biopsy material from patients with diabetic nephropathy confirmed increased expression of purinergic receptor P2X7. TGF-β1 increased Cx43 mediated hemichannel activity and ATP release in hPTECs and HK2 cells. The cytokine reduced maximum unbinding forces and reduced cell-cell adhesion, which translated to increased paracellular permeability. Changes were reversed when cells were co-incubated with either Peptide 5 or P2-purinoceptor inhibitors. Cx43+/- mice did not exhibit protein changes associated with early tubular injury in a UUO model of fibrosis. Conclusion: Data suggest that Cx43 mediated ATP release represents an initial trigger in early tubular injury via its actions on the adherens and tight junction complex. Since Cx43 is highly expressed in nephropathy, it represents a novel target for intervention of tubulointerstitial fibrosis in CKD. Video Abstract In proximal tubular epithelial cells (PTECs), tight junction proteins, including zona occuludens-1 (ZO-1), contribute to epithelial integrity, whilst the adherens junction protein epithelial (E)-cadherin (ECAD) maintains cell-cell coupling, facilitating connexin 43 (Cx43) gap junction-mediated intercellular communication (GJIC) and the direct transfer of small molecules and ions between cells. In disease, such as diabetic nephropathy, the pro-fibrotic cytokine transforming growth factor beta1 (TGF-β1) binds to its receptor and recruits SMAD2/3 signalling ahead of changes in gene transcription and up-regulation of Cx43-mediated hemichannels (HC). Uncoupled hemichannels permit the release of adenosine triphosphate (ATP) in to the extracellular space (↑[ATP]e), where ATP binds to the P2X7 purinoreceptor and activates the nucleotide-binding domain and leucine-rich repeat containing (NLR) protein-3 (NLRP3) inflammasome. Inflammation results in epithelial-to-mesenchymal transition (EMT), fibrosis and tubular injury. A major consequence is further loss of ECAD and reduced stickiness between cells, which can be functionally measured as a decrease in the maximum unbinding force needed to uncouple two adherent cells (Fmax). Loss of ECAD feeds forward to further lessen cell-cell coupling exacerbating the switch from GJIC to HC-mediated release of ATP. Reduction in ZO-1 impedes tight junction effectiveness and decreases trans-epithelial resistance (↓TER), resulting in increased paracellular permeability.

Abstract:Low nephron endowment at birth is a risk factor for chronic kidney disease. The prevalence of this condition is increasing due to higher survival rates of preterm infants and children with multi- organ birth defect syndromes that affect the kidney and urinary tract. We created a mouse model of congenital low nephron number due to deletion of Mta2 in nephron progenitor cells. Mta2 is a core component of the Nucleosome Remodeling and Deacetylase (NuRD) chromatin remodeling complex. These mice developed albuminuria at 4 weeks of age followed by focal segmental glomerulosclerosis (FSGS) at 8 weeks, with progressive kidney injury and fibrosis. Our studies reveal that altered mitochondrial metabolism in the post-natal period leads to accumulation of neutral lipids in glomeruli at 4 weeks of age followed by reduced mitochondrial oxygen consumption. We found that NuRD cooperated with Zbtb7a/7b to regulate a large number of metabolic genes required for fatty acid oxidation and oxidative phosphorylation. Analysis of human kidney tissue also supported a role for reduced mitochondrial lipid metabolism and ZBTB7A/7B in FSGS and CKD. We propose that an inability to meet the physiological and metabolic demands of post-natal somatic growth of the kidney promotes the transition to CKD in the setting of glomerular hypertrophy due to low nephron endowment.
Tubular atrophy in the pathogenesis of chronic kidney disease progression

Abstract:The longstanding focus in chronic kidney disease (CKD) research has been on the glomerulus, which is sensible because this is where glomerular filtration occurs, and a large proportion of progressive CKD is associated with significant glomerular pathology. However, it has been known for decades that tubular atrophy is also a hallmark of CKD and that it is superior to glomerular pathology as a predictor of glomerular filtration rate decline in CKD. Nevertheless, there are vastly fewer studies that investigate the causes of tubular atrophy, and fewer still that identify potential therapeutic targets. The purpose of this review is to discuss plausible mechanisms of tubular atrophy, including tubular epithelial cell apoptosis, cell senescence, peritubular capillary rarefaction and downstream tubule ischemia, oxidative stress, atubular glomeruli, epithelial-to-mesenchymal transition, interstitial inflammation, lipotoxicity and Na+/H+ exchanger-1 inactivation. Once a a better understanding of tubular atrophy (and interstitial fibrosis) pathophysiology has been obtained, it might then be possible to consider tandem glomerular and tubular therapeutic strategies, in a manner similar to cancer chemotherapy regimens, which employ multiple drugs to simultaneously target different mechanistic pathways.

Polyploid tubular cells: a shortcut to stress adaptation

Abstract:Tubular epithelial cells (TCs) compose the majority of kidney parenchyma and play fundamental roles in maintaining homeostasis. Like other tissues, mostly immature TC with progenitor capabilities are able to replace TC lost during injury via clonal expansion and differentiation. In contrast, differentiated TC lack this capacity. However, as the kidney is frequently exposed to toxic injuries, evolution positively selected a response program that endows differentiated TC to maintain residual kidney function during kidney injury. Recently, we and others have described polyploidization of differentiated TC, a mechanism to augment the function of remnant TC after injury by rapid hypertrophy. Polyploidy is a condition characterized by >2 complete sets of chromosomes. Polyploid cells often display an increased functional capacity and are generally more resilient to stress as evidenced by being conserved across many plants and eukaryote species from flies to mammals. Here, we discuss the occurrence of TC polyploidy in different contexts and conditions and how this integrates into existing concepts of kidney cell responses to injury. Collectively, we aim at stimulating the acquisition of novel knowledge in the kidney field as well as accelerating the translation of this basic response mechanism to the clinical sphere.

Abstract:Abstract Evolution moves in mysterious ways. Excretion of waste products by glomerular filtration made perfect sense when life evolved in the ocean. Yet, the associated loss of water and solutes became a problem when life moved onto land: a serious design change was needed and this occurred in the form of ever more powerful tubules that attached to the glomerulus. By reabsorbing typically more than 99% of the glomerular filtrate, the tubules not only minimise urinary losses, but, crucially, also maintain homeostasis: tubular reabsorption and secretion are adjusted so as to maintain an overall balance, in which urine volume and composition matches intake and environmental stressors. A whole orchestra of highly specialised tubular transport proteins is involved in this process and dysfunction of one or more of these results in the so-called kidney tubulopathies, characterised by specific patterns of clinical and biochemical abnormalities. In turn, recognition of these patterns helps establish a specific diagnosis and pinpoints the defective transport pathway. In this review, we will discuss these clinical and biochemical “fingerprints” of tubular disorders of salt-handling and how sodium handling affects volume homeostasis but also handling of other solutes.

[Exploration of renal tubular functions]

Abstract:The vital role of the kidneys is closely related to their interior homeostatic function that provides a protection to the cells regarding the consequences of environmental variations of the organism. Any primary or secondary damage caused to the tubular functions may impair this homeostasis. Two indications justify an analysis of these functions: the search for and characterization of a primary defect of one or several tubular functions and the detection of early alterations of tubular functions in glomerular renal diseases. Following a brief overview of physiological aspects, the positive and aetiologic diagnosis of a tubular alteration is described, whatever the way the disorder have been detected, aetiologic analysis of a metabolic disorder or specific screening using a dynamic test. Main criteria used to determine the renal or extrarenal origin of the metabolic disorders are reviewed. General principles are described, together with the indication and interpretation of the most current investigation procedures for proximal tubular reabsorption (calculation or measurement of bicarbonate, phosphate, or glucose Tm), distal NaCl reabsorption (response to diuretics, hypotonic saline load test), concentration/dilution (dDAVP water restriction test, water load test) or distal urinary acidification (acid load, bicarbonate load, furosemide test). Last, as a conclusion, we highlight the possibility of performing routinely genetic diagnosis of most hereditary tubular diseases (Bartter/Gitelman syndrome, nephrogenic diabetes insipidus, distal tubular acidosis, and Lowe and Dent syndromes).
Adverse effects of acute tubular injury on the glomerulus: contributing factors and mechanisms

Abstract:The intricate relationship between tubular injury and glomerular dysfunction in kidney diseases has been a subject of extensive research. While the impact of glomerular injury on downstream tubules has been well-studied, the reverse influence of tubular injury on the glomerulus remains less explored. This paper provides a comprehensive review of recent advances in the field, focusing on key pathways and players implicated in the pathogenesis of tubular injury on glomerular dysfunction. Anatomical and physiological evidence supports the possibility of crosstalk from the tubule to the glomerulus, whereby various mechanisms contribute to glomerular injury following tubular injury. These mechanisms include tubular backleak, dysfunctional tubuloglomerular feedback, capillary rarefaction, atubular glomeruli, and the secretion of factors from damaged tubular epithelial cells. Clinical evidence further supports the association between even mild or recovered acute kidney injury and an increased risk of chronic kidney disease, including glomerular diseases. We also discuss potential therapeutic interventions aimed at mitigating acute tubular injury, thereby reducing the detrimental effects on glomerular function. Abstract:In the last thirty decade, with the emergence of new trends in molecular biology and advances in high-throughput technologies, much progress has been made in basic renal physiology. Molecular genetics has allowed the identification and elucidation of the structure, function and effects of the mutations of several of the main transporters and ion channels involved in renal disorders. Some renal stone disorders, such as cystinuria and Dent's disease, have been found to be due to mutations in genes SLC3A1 (type I) (See the section "Molecular biology and genotype-phenotype correlation in tubular dysfunction") and SLC7A9 (type II and type III), (See the section "Molecular biology and genotype-phenotype correlation in tubular dysfunction") and in CLC5, respectively. Liddle syndrome, a rare cause of hypertension, is now known to be caused by a mutation in tubular transport, due to a mutation in the SCNN1B gene, encoding for a Na+ channel protein (ENaC). Nevertheless, numerous issues remain unsettled and warrant additional research. These important advances and discoveries are not without limitations and challenges as changes in individual gene expression do not always translate into changes in its protein or protein modification. This raises proteomics as the most logical next step in our understanding of biological processes, as proteins from these deregulated genes are the functional agents in the cells. Proteomics takes a global and comprehensive view of a system, involving in many cases some notion of high throughput; but in contrast to genomics, there is no single biochemical method that can be used for the analysis of all proteins. Genomics and proteomics can complement each other in clinical applications by balancing the strengths and weaknesses of each individual technology. Several proteomics approaches have been exploited to shed more light on the molecular pathophysiology of several hereditary tubular disorders, such as Fanconi and Gitelman syndromes, and have provided important insights into the defective molecular mechanisms underlying these tubulopathies. Here we summarize several of the most important discoveries arising from molecular genetic and proteomic studies on hereditary tubular dysfunctions and show how these results can complement each other to increase our comprehension of these disorders at the molecular level.
Decoupling shear stress and pressure effects in the biomechanics of autosomal dominant polycystic kidney disease using a perfused kidney-on-chip.

Abstract:Kidney tubular cells are submitted to two distinct mechanical forces generated by the urine flow: shear stress and hydrostatic pressure. In addition, the mechanical properties of the surrounding extracellular matrix modulate tubule deformation under constraints. These mechanical factors likely play a role in the pathophysiology of kidney diseases as exemplified by autosomal dominant polycystic kidney disease, in which pressure, flow and matrix stiffness have been proposed to modulate the cystic dilation of tubules with PKD1 mutations. The lack of in vitro systems recapitulating the mechanical environment of kidney tubules impedes our ability to dissect the role of these mechanical factors. Here we describe a perfused kidney-on-chip with tunable extracellular matrix mechanical properties and hydrodynamic constraints, that allows a decoupling of shear stress and flow. We used this system to dissect how these mechanical cues affect Pkd1-/- tubule dilation. Our results show two distinct mechanisms leading to tubular dilation. For PCT cells (proximal tubule), overproliferation mechanically leads to tubular dilation, regardless of the mechanical context. For mIMCD-3 cells (collecting duct), tube dilation is associated with a squamous cell morphology but not with overproliferation and is highly sensitive to extracellular matrix properties and hydrodynamic constraints. Surprisingly, flow alone suppressed Pkd1-/- mIMCD-3 tubule dilation observed in static conditions, while the addition of luminal pressure restored it. Our in vitro model emulating nephron geometrical and mechanical organization sheds light on the roles of mechanical constraints in ADPKD and demonstrates the importance of controlling intraluminal pressure in kidney tubule models.

Abstract:Introduction During the past 20 years it has become clear that polyarteritis nodosa, as described by Kussmaul & Maier (1866), is uncommon. In that 'classic' form it is a devastating disease affecting predominantly the larger muscular arteries of the splanchnic organs. However systemic vasculitis involving only the lower orders of the arterial tree is relatively common. Davson and his colleagues (1948) used the term 'microscopic polyarteritis' to describe an arteritis that was detectable only by light microscopy, in contrast to polyarteritis nodosa in which, at autopsy, the vessel involvement was often visible with the naked eye. Arteritis may occur as part of some well characterized systemic disease such as systemic lupus erythematosus (SLE), Henoch-Schoenlein purpura (HSP), or cryoglobulinaemia. These are most conveniently thought of as secondary forms of arteritis (Neild & Williams 1985). However, when these conditions affect the kidney it is their glomerular involvement that dominates the picture (Cameron et al. 1979, Meadows et al. 1972). In other conditions arteritis appears to be the primary disorder. These primary or idiopathic forms of systemic arteritis will be referred to as polyarteritis. This review is about polyarteritis affecting the kidney Rapidly progressive glomerulonephritis (RPGN) is the most serious manifestation of polyarteritis involving the kidney. RPGN is a rare syndrome characterized (1) clinically by an acute nephritic syndrome with the rapid development of renal failure, and (2) pathologically by a severe crescentic glomerulonephritis. Many patients who present with RPGN have a clinical illness suggestive of a vasculitis (Neild et al. 1983). Davson et al. (1948) were the first to propose that necrotizing glomerulonephritis with crescents might be the sole renal manifestation of polyarteritis. This concept was accepted, and recently Serra et al. (1984) confirmed in a study of renal vasculitis that there were no clinical differences between those in whom vasculitis was apparently confined to the glomerulus and those who had a histologically proven arteritis elsewhere.
The molecular and cellular anatomy of a fetal programming defect: the impact of low protein diet on the developing kidney.

Abstract:Background Low nephron number has a direct impact on the development of hypertension and chronic kidney disease later in life. While intrauterine growth restriction caused by maternal low protein diet (LPD) is thought to be a significant cause of reduced nephron endowment in impoverished communities, its influence on the cellular and molecular processes which drive nephron formation are poorly understood. Methods We conducted a comprehensive characterization of the impact of LPD on kidney development using tomographic and confocal imaging to quantify changes in branching morphogenesis and the cellular and morphological features of nephrogenic niches across development. These analyses were paired with single-cell RNA sequencing to dissect the transcriptional changes that LPD imposes during renal development to affect nephron number. Results Single cell analysis at E14.5 and P0 revealed differences in the expression of genes and pathways involved in metabolism, cell cycle, epigenetic regulators and reciprocal inductive signals in most cell types analyzed, yielding imbalances and shifts in cellular energy production and cellular trajectories. In the nephron progenitor cells, LPD impeded cellular commitment and differentiation towards pre-tubular and renal vesicle structures. Confocal microscopy revealed a reduction in the number of pre-tubular aggregates and proliferation in nephron progenitor cells. We also found changes in branching morphogenesis, with a reduction in cell proliferation in the ureteric tips as well as reduced tip and tip parent lengths by optical projection tomography which causes patterning defects. Conclusions This unique profiling demonstrates how a fetal programming defect leads to low nephron endowment which is intricately linked to changes in both branching morphogenesis and the commitment of nephron progenitor cells. The commitment of progenitor cells is pivotal for nephron formation and is significantly influenced by nutritional factors, with a low protein diet driving alterations in this program which directly results in a reduced nephron endowment.
